O objetivo deste trabalho foi avaliar os resultados de tratamento realizado com Placa de Michigan, em pacientes com Dor Orofacial, que buscaram atendimento no Ambulatório de Oclusão da Faculdade de Odontologia da PUCRS, utilizando o questionário RDC/DTM, e associar algumas variáveis entre os resultados encontrados. Os métodos empregados foram o questionário RDC/DTM, um método já consagrado no uso para pesquisa nesta área, e ainda a aplicação de um questionário de acompanhamento sobre a dor após três meses do início do tratamento. Os resultados obtidos foram satisfatórios. / The objective of this study was to evaluate the treatment results when the stabilization (Michigan-type) splint was used, in patients with orfacial pain, who were seeking treatment in the Occlusion clinical at the Faculty of Dentistry of PUCRS, using the RDC/TMD, and associate some variables among the results found. The methods employed were the RDC/TMD, a stablished method for research in this área, and also a pain follow-up questionnaire three months after the beginning of treatment. Satisfactory results were obtained.

O consumo de alimentos processados tem crescido mundialmente e esse aumento tem sido relacionado em parte, pelas estratégias de marketing utilizadas pelas indústrias alimentícias que tem como objetivo aumentar as vendas e expandir o mercado consumidor. Segundo documento Recomendações da Consulta de Especialistas da Organização Pan-Americana da Saúde sobre a Promoção e a Publicidade de Alimentos e Bebidas Não Alcoólicas para Crianças nas Américas, a publicidade televisiva é a forma mais utilizada de marketing de alimentos para crianças e está associada a maior preferência e aumentos nos pedidos de compra de alimentos e bebidas com elevado teor de gordura, açúcar e sal. No âmbito da discussão mais recente sobre a possível influência da propaganda de alimentos na epidemia mundial de obesidade e doenças crônicas, o governo brasileiro, por meio da Agência Nacional de Vigilância Sanitária (ANVISA), formulou uma proposta de Regulamento Técnico que normatiza os termos das atividades publicitárias e que se concretizou na Resolução da Diretoria Colegiada n 24 de 2010 (RDC24/2010). Logo após sua publicação, grupos de interesse empresarial entraram na justiça e obtiveram diversas liminares que suspenderam a aplicação da RDC 24/2010 com base em parecer contrário da Advocacia Geral da União (AGU). O debate sobre ações regulatórias no campo da alimentação e nutrição como estratégia de controle e prevenção da obesidade envolve diversos atores sociais e opiniões bastante distintas e antagônicas, principalmente quando se trata dos argumentos do grupo de indivíduos que acredita na regulamentação da publicidade como uma forma de promover a saúde versus os argumentos contrários dos setores econômicos ligados ao tema a ser regulado. Neste sentido, este trabalho teve a intenção de descrever como e por que, neste processo, algumas práticas se estabeleceram no cotidiano das instituições e identificar os embates, argumentos e conflitos de interesses que apareceram nas discussões a respeito da formulação da resolução. Para isso, foi feita, primeiramente, análise documental de todo o processo de formulação, através de arquivos institucionais, documentos, pareceres, relatórios, entre outros. Posteriormente, foram identificados e caracterizados os sujeitos que participaram do processo e em seguida, realizadas entrevistas semi-estruturadas com atores chaves. Foram encontradas duas teses centrais no debate em torno da formulação da resolução: a primeira, se o Estado deve estabelecer uma regulamentação sobre publicidade de alimentos e a segunda diz respeito à competência legal da ANVISA em exercer o papel regulador sobre a propaganda de alimentos. As disputas em torno destas duas teses e os argumentos que se apresentaram para defendê-las são evidentemente incomensuráveis entre si, não havendo esperança de acordo entre as partes. O caso descrito elegeu a ANVISA como o território da disputa e a aparente maioria no jogo democrático desta arena mostrou-se ineficaz diante da captura pelo questionamento da pertinência desta Agência para função de regular a publicidade de alimentos. / The consumption of processed foods has grown globally and this increase has been related in part to the marketing strategies used by the food industry that aims to increase sales and expand the consumer market. As document Recommendations from a Pan American Health Organization Expert Consultation on the Marketing of Food and Non-Alcoholic Beverages to Children in the Americas, television advertising is the most widely used form of food marketing to children and is associated with greater preference and increases in orders for food and drinks high in fat , sugar and salt (PAHO, 2012). Within the most recent discussion on the possible influence of food advertising in the global epidemic of obesity and chronic diseases, the Brazilian government, through the National Health Surveillance Agency (ANVISA) , formulated a proposal for Technical Regulation that regulates the terms of advertising activities and which has resulted in Board Resolution No. 24 of 2010 (RDC24/2010). Soon after its publication, business interest groups filed a lawsuit and obtained several injunctions suspended application of RDC 24/2010 based on negative opinion of the Attorney General (AGU). The debate on regulatory actions in food and nutrition area as a strategy of control and obesity prevention involves several quite distinct and antagonistic social actors and opinions, especially when it comes to the arguments of the group who believe in the regulation of advertising as a way to promote health versus the counter arguments of economic sectors related to the subject to be regulated. Thus, this study was intended to describe how and why, in this process, some settled in the daily practices of institutions and identify conflicts, arguments and conflicts of interest that arose during the discussion about the wording of the resolution. To this, was first made documentary analysis of the entire formulation process through institutional archives, documents, opinions, reports, among others. Subsequently, we identified and characterized the subjects who participated in the process and then conducted semi -structured interviews with key actors. Two central theses were found in the debate over the wording of the resolution: the first, whether the state should establish rules on food advertising and the second concerns the legal competence of ANVISA in exercising regulatory role on the advertising of food. Disputes around these two theses and arguments presented to defend them are clearly incommensurate with each other, with no hope of agreement between the parties. The case described ANVISA elected as the territory of the dispute and the apparent majority in this democratic game arena was ineffective before the capture by questioning the relevance of this Agency to regulating food advertising.

Dans cette thèse, on présente nos travaux sur la virtualisation dans le contexte de la réplication des serveurs de contenu vidéo. Les travaux couvrent la conception d’une architecture de virtualisation pour laquelle on présente aussi plusieurs algorithmes qui peuvent réduire les couts globaux à long terme et améliorer la performance du système. Le travail est divisé en plusieurs parties : les solutions optimales, les solutions heuristiques pour des raisons de passage à l’échelle, l’orchestration des services, l’optimisation multi-objective, la planification de services dans des réseaux actifs et complexes et l'intégration d'algorithmes dans une plate-forme réelle / In this thesis, we designed and implemented a tool which performs optimizations that reduce the number of migrations necessary for a delivery task. We present our work on virtualization in the context of replication of video content servers. The work covers the design of a virtualization architecture for which there are also several algorithms that can reduce overall costs and improve system performance. The thesis is divided into several parts: optimal solutions, greedy (heuristic) solutions for reasons of scalability, orchestration of services, multi-objective optimization, service planning in complex active networks, and integration of algorithms in real platform. This thesis is supported by models, implementations and simulations which provide results that showcase our work, quantify the importance of evaluating optimization techniques and analyze the trade-off between reducing operator cost and enhancing end user satisfaction index

RESUME La construction d un systeme financier sain et concurrentiel capable de mobiliser de facon substantielle des hauts niveaux d epargne et l amelioration des normes de supervision bancaire et financier pour assurer la stabilite du systeme sont deux des recommandations souvent faites par les institutions internationales pour permettre aux pays africains de participer pleinement a l expansion de la prosperite mondiale et a beneficier de la globalisation du commerce des services financiers. Cette these essaie de trouver les voies et moyens susceptibles de contribuer a l amelioration et au renforcement de la supervision bancaire au Congo, et in fine [le secteur bancaire etant le plus important du systeme financier] permettre l eclosion d un systeme financier moderne et efficace qui rencontre les normes internationales. Dans une premiere etape qui consiste en un etat des lieux du systeme financier congolais et en une analyse critique de la gestion bancaire ( chapitre 1 et chapitre 2 ); les analyses : - ressortent les caracteristiques du systeme financier congolais ; - soulignent les contraintes structurelles ayant entrave trois decennies de gestion bancaire ; - evaluent les chances de succes des reformes mises en oeuvre par les autorites; - proposent en des termes generaux, les ameliorations a porter au cadre reglementaire et de supervision du secteur bancaire afin de reduire les imperfections, de renforcer l efficacite et la stabilite du systeme dans son ensemble. This thesis tries to find the ways to contribute to the improvement and the reinforcement of the banking supervision in Congo, and in fine [ the banking environment being most significant of the financial system ] to allow the blossoming of a modern and effective financial system which meets international standards. In a first stage which consists in an overview of the Congolese financial system and in a critical analysis of the banking management ( chapter 1 & chapter 2) ; our analyses : - release the characteristics of the Congolese financial system ; - underline the structural constraints having blocked three decades of banking management ; - evaluate the chances of success of the reforms implemented by the authorities ; - propose in general terms, the improvements to be carried in order to reduce the imperfections of the banking supervision, to reinforce the effectiveness and the stability of the banking system. In the second stage, a more restricted menu of proposals made at the end of the first stage is reviewed. The contributions of the thesis in this stage consist: - in the proposal of concrete tools for banking supervision to face the deficiency of preventive prudential management tools; ( chapter 3) - in recommendations to improve : the policy of provisioning bad debts and the management of Microfinance institutions; (chapter 4) - in an evaluation of the competitiveness of the Congolese financial system and in the identification of ways which can contribute to the improvement of its safety and solidity by using a strategic analysis approach. Introdução: Os estudos clínicos promovem inovação no âmbito da saúde e podem proporcionar novos tratamentos e benefícios à população. Os estudos de intervenção com medicamentos são fundamentais para demonstrar a eficácia, a toxicidade e a segurança de novas terapias. O uso de medicamentos experimentais na assistência médica é uma realidade observada tanto na continuidade do tratamento pelos participantes após o término da pesquisa, como na possibilidade do uso assistencial destes medicamentos. A Agência Nacional de Vigilância Sanitária (Anvisa) regulamenta, com base na RDC n. 38/2013, três Programas assistenciais com medicamentos experimentais: Acesso Expandido, Uso Compassivo e Fornecimento de Medicamento Pós-Estudo. Mais recentemente, uma nova alternativa está em desenvolvimento no Hospital de Clínicas de Porto Alegre, definida como Uso Assistencial Extraordinário. Objetivos: Propor fluxos e materiais informativos para orientar a prática e a condução de Programas de Acesso Expandido, Uso Compassivo, Fornecimento de Medicamento Pós-Estudo e Uso Assistencial Extraordinário no Hospital de Clínicas de Porto Alegre, assim como em outras instituições de saúde. Métodos: Foram revisadas as publicações acerca do assunto, analisados alguns fluxogramas existentes na condução dos Programas acima citados, bem como realizado questionário aos profissionais que atuam nesta área no CPC-HCPA. Resultados: Foram elaborados fluxos e materiais informativos para a condução destes distintos Programas, considerando as diretrizes éticas, regulatórias e de boas práticas, que visam auxiliar a prática e a educação dos profissionais envolvidos. / Introduction: Clinical research can provide valuable evidence to support to health care system in informed decision making. At the same time, from the patient perspective, trials can the way to gain access to more appropriate treatments. The main goal of intervention studies is to determine safety and efficacy of new therapies. The Agência Nacional de Vigilância Sanitária (Anvisa) regulates, based on RDC n° 38/2013, three care programs with experimental drugs: Expanded Access, Compassionate Use and Post-trial Access. Also, in the Hospital de Clínicas de Porto Alegre, a new alternative program is being explored - defined as Extraordinary Care Use. Objectives: Propose guidelines to Expanded Access, Compassionate Use, Post-trial Access programs and Extraordinary Care Use in the format of flow sheets and informative leaflets at Hospital de Clínicas de Porto Alegre, and perhaps become a benchmarking project to other institutions. Methods: After review of literature and analysis of existing flow sheets on how to conduct the above mentioned programs, we administered a survey questionnaire on challenges encountered by investigators from clinical research center at our institution. Results: Flow sheets and informative materials were developed to support professionals their research groups in conducting clinical interventional initiatives, considering the ethical, regulatory and good practices guidelines.

Pushing the Limits of NMR Sensitivity and Chiral Analysis : Design of New NMR Methods and Bio-Molecular Tools

The thesis entitled "Pushing the Limits of NMR Sensitivity and Chiral Analysis: Design of New NMR Methods and Bio-molecular Tools" consists of six chapters. The research work reported in this thesis is focused on the development of novel chemical and NMR methodological approaches for enantiomeric analysis and mea- surement of residual dipolar couplings (RDCs), and the development of sensitivity enhanced slice selective NMR experiments for obtaining pure shift 1H spectra and the measurement of scalar couplings. The thesis is divided into two parts. The Part I comprises chapters 2-4, where the enantiomeric analysis is discussed, which includes newly developed chiral reagents, two new weak chiral aligning media and design of novel NMR techniques. Part II comprises chapters 5 and 6, which discusses new sensitivity enhanced slice selective NMR techniques. Chapter 1 gives a general introduction to NMR and the problems investigated in the remaining chapters of the thesis. The chapter starts with a brief discussion on the introduction, advancements and general applications of NMR, discussion is also given on the NMR approaches for enantiomeric analysis both in isotropic and anisotropic phases and the measurement of RDCs, including the benefits and limitations associated with each approach. The chapter sets the tone by discussing limitations of the existed NMR enantiomeric approaches and slice-selective techniques, and builds the bridge for the rest of the chapters by addressing these limitations. The chapter also introduces slice selective experiments, their benefits over other conventional methods and limitations. Additional introductory notes are also given on some related concepts. Part I : NMR Chiral analysis and RDCs measurements Chapter 2 discusses chiral sensing properties of RNA nucleosides and their utility as chiral derivatizing agents for the enantio-discrimination of 1o-amines using one dimensional 1H NMR. A three component protocol has been proposed for the complexation of nucleosides with amines, which is rapid, economical and provides maximum diastereomeric conversion. The chiral differentiating ability of nucleosides are examined for different amines based on the 1H NMR chemical shift differences between the diastereomers (∆δ R, S ). Enantiomeric differentiation has been observed at multiple chemically distinct proton sites. It is observed that adenosine and guanosine exhibit large chiral differentiation (∆δ R, S ) due to the presence of a purine ring. The comparison of the diastereomeric excess (de) measured by NMR with those of the gravimetrically prepared ratios are in excellent agreement with each other confirming the robustness of these RNA nucleosides in discriminating primary amines. Chapter 3 establishes the smooth connectivity with the chapter 2 by discussing the limitations of the enantiomeric discrimination using NMR in isotropic solutions. This chapter discusses two new water compatible aligning media that were developed based on self-assembling strategy of small bio-molecules. The self-assembled folic acid, and the binary mixture of 50-GMP and guanosine are introduced as two novel weak aligning media. The properties of these low ordered media have been systematically studied for their easy preparation, physical parameter dependent tunability of their degree of alignment, mesosphere sustainability over a broad range of temperature and the concentration of the ingredients, and the phase reproducibility. The applications of both these new media are demonstrated for chiral and pro-chiral discrimination and also for the measurement of RDCs. Both these liquid crystalline media could be tuned to very low degree of alignment (order parameter of the order of 10−4), which provides simple first order spectra of molecules aligned in them, the analysis provide order dependent NMR spectral parameters. The 50-GMP:guanosine orienting medium can be prepared in less than 1 hour, and has been demonstrated to be an ideal medium for the determination of RDCs that are used as restraints in the structure calculations of small molecules. Chapter 4 describes 1H NMR spectral complexity in isotropic and anisotropic phases and its consequences on enantiomeric analysis. In circumventing such problems, new NMR techniques have been developed and the spin dynamics involved in the designed sequences are discussed. The newly developed 2D 1H NMR experimental method termed as RES-TOCSY, and its applicability for resolving R and S enantiomeric or diastereomeric peaks of all the coupled proton spins in isotropic phase is discussed. The utility of the developed method is demonstrated in diverse situations, such as, for suppressing impurities peaks, resolving the severely overlapped peaks and unraveling the peaks masked due to severe line broadening when metal complexes are used as chiral auxiliaries. The advantages and limitations of the method over other methods available in the literature are discussed and the significant advantage of the present method is illustrated by spectral comparison with J-resolved experiment. The appli- cation of the method for the accurate measurement of enantiomeric excess has also been demonstrated. The chapter also introduces another NMR experimental technique developed for resolving enantiomeric peaks and complete unraveling of R and S spectra in anisotropic phase. The developed 2D NMR method is cited in the literature as CH-RES-TOCSY. In addition to spectroscopic visualization of R and S spectra, the method also yields C-H RDCs. The applicability of the new experiment has been demonstrated on a chosen example. The wide utility of the method has also been demonstrated for the assignment of symmetric cis- and trans- isomers. Part II : Sensitivity Enhancement of Slice selective NMR Experiments Chapter 5 describes applications of slice selective NMR experiments over conven tional NMR methods and their limitations as far as the sensitivity of signal detection is concerned, especially in low concentrated samples. The chapter introduces the implementation of Acceleration by Sharing Adjacent Polarization (ASAP) technique in slice selective experiments. It is convincingly demonstrated that ASAP helps in reducing inter scan relaxation delay and consequently permits acquisition of more number of scans in a given time, resulting in the gain in signal enhancement by a factor of two. The pulse sequences have been suitably designed for obtaining the pure shift 1H spectra and in G-SERF experiment for the measurement of 1H-1H couplings, both with significantly enhanced signal intensities. Chapter 6 describes new sensitivity enhanced slice selective NMR methods for mea- surement of scalar couplings. A new experiment has been developed which is named as Quick G-SERF (QG-SERF). It is a 1D NMR slice selective method developed based on real time spin manipulation technique. The method gives multiple scalar couplings of a selected spin with simplified multiplets, which is analogous to the 2D G-SERF but with considerable saving in instrument time by 1-2 orders of magnitude. The rapidness of the experiment arises due to reduced dimensionality. The spin dynamics involved in the pulse sequence and its working principle have been described. The application of the method is illustrated for the measurement of 1H-1H couplings. The sequence has been further improved to obtain the heteronuclear couplings between two abundant spins in an orchestrated manner and has been demonstrated for measurement of 1H-19F couplings. This sequence cited as HF-QG-SERF has been implemented on the molecules containing number of chemically non-equivalent fluorine atoms.

All this happened as a result of meeting stringent and assorted requirements, since food fruit juices included quality and safety are a must today. The purpose of this paper is to present a proposal to integrate management systems involving quality, good manufacturing practices and related legal requirements, as well as HACCP, in a small fruit juices company in the State of São Paulo. The proposal includes integrated cost-effective actions to implement the various standards. The research carried out concludes that the integration of management systems involving quality, good manufacturing practices and related legal requirements, as well as HACCP, is feasible. A relationship matrix of the respective requirements was designed during the research, which allowed conducting an initial joint and revealing assessment to evaluate the degree of fulfillment of those requirements. Bearing in mind the company´s scenario where the research was performed and the first application of this Integrated Management System took place, the proposal was not totally implemented. The necessity was perceived to test the Integrated Management System and monitor its operation in other food sector companies, thus not only evaluating the consistency of the proposal but also its flexibility to meet local specific requirements and detect improvement opportunities. / O crescimento do setor de produção de sucos de frutas vem contribuindo ao fortalecimento da economia brasileira; a rica variedade de frutas tropicais, a diversidade de formas de apresentação e a qualidade dos sucos ampliaram o mercado interno e conquistaram mercados externos, atendendo exigências cada vez mais rígidas. As empresas do setor estão submetidas a crescentes exigências de qualidade e segurança de seus produtos. O objetivo deste trabalho é apresentar uma proposta para integrar os sistemas de gestão da qualidade, das boas práticas de fabricação e requisitos legais correlatos, bem como da APPCC, em uma pequena empresa de sucos de frutas situada no interior do Estado de São Paulo, submetida às crescentes exigências dos clientes. A proposta inclui ações integradas contribuindo para racionalizar os recursos aplicados ao processo de adequação aos diferentes referenciais normativos. O trabalho conclui que é viável a integração dos sistemas de gestão da qualidade, das boas práticas de fabricação e requisitos legais correlatos e da APPCC. A matriz de correspondência entre requisitos respectivos, desenhada durante a pesquisa realizada, permitiu a avaliação conjunta do grau de atendimento dos mesmos, facilitando o processo de implementação a partir dos resultados obtidos. Tendo em vista o cenário da empresa em que foi desenvolvida a pesquisa e realizada a primeira aplicação da proposta do Sistema Integrado de Gestão, não foi possível implementar a proposta integralmente. Foi percebida a necessidade de testar sua implementação e operação em outras empresas do setor de alimentos, para avaliar não só a consistência da mesma, senão também sua flexibilidade para se ajustar às especificidades locais e detectar oportunidades de melhoria.
