The number of annual implantable cardioverter defibrillator (ICD) implants has substantially increased over the last 5 years and is expected to grow rapidly. Implantable cardioverter defibrillators have a proven mortality benefit by terminating the life-threatening arrhythmias, even near end of life. In patients with moderate/severe symptomatic heart failure, enough clinical literature representing mortality benefits has been published, but limited numbers of studies have reviewed the dwindling risk-benefit profile near end of life, studying quality of life (QoL)/psychosocial impact. Criteria outlining either continued use or deactivation policy/procedures near end of life have not been clearly defined and/or largely implemented, which in turn requires more focused research using multifactorial approach to determine improved patient-centered outcomes.

Intestinal Gene Expression Profiling and Fatty Acid Responses to a High-fat DietCedernaes, Jonathan January 2013 (has links)
The gastrointestinal tract (GIT) regulates nutrient uptake, secretes hormones and has a crucial gut flora and enteric nervous system. Of relevance for these functions are the G protein-coupled receptors (GPCRs) and the solute carriers (SLCs). The Adhesion GPCR subfamily is known to mediate neural development and immune system functioning, whereas SLCs transport e.g. amino acids, fatty acids (FAs) and drugs over membranes. We aimed to comprehensively characterize Adhesion GPCR and SLC gene expression along the rat GIT. Using qPCR we measured expression of 78 SLCs as well as all 30 Adhesion GPCRs in a twelve-segment GIT model. 21 of the Adhesion GPCRs had a widespread (≥5 segments) or ubiquitous (≥11 segments) expression. Restricted expression patterns were characteristic for most group VII members. Of the SLCs, we found the majority (56 %) of these transcripts to be expressed in all GIT segments. SLCs were predominantly found in the absorption-responsible gut regions. Both Adhesion GPCRs and SLCs were widely expressed in the rat GIT, suggesting important roles. The distribution of Adhesion GPCRs defines them as a potential pharmacological target. FAs constitute an important energy source and have been implicated in the worldwide obesity increase. FAs and their ratios – indices for activities of e.g. the desaturase enzymes SCD-1 (SCD-16, 16:1n-7/16:0), D6D (18:3n-6/18:2n-6) and D5D (20:4n-6/20:3n-6) – have been associated with e.g. overall mortality and BMI. We examined whether differences in FAs and their indices in five lipid fractions contributed to obesity susceptibility in rats fed a high fat diet (HFD), and the associations of desaturase indices between lipid fractions in animals on different diets. We found that on a HFD, obesity-prone (OP) rats had a higher SCD-16 index and a lower linoleic acid (LA) proportions in subcutaneous adipose tissue (SAT) than obesity-resistant rats. Desaturase indices were significantly correlated between many of the lipid fractions. The higher SCD-16 may indicate higher SCD-1 activity in SAT in OP rats, and combined with lower LA proportions may provide novel insights into HFD-induced obesity. The associations between desaturase indices show that plasma measurements can serve as proxies for some lipid fractions, but the correlations seem to be affected by diet and weight gain.

At the same time, the quality of crude oil is becomingworse with higher amounts of polyaromatics, heteroatoms(sulphur and nitrogen) and heavy metals. In order to staycompetitive, it is desirable to upgrade dense streams withinthe refinery to value-added products. For example, upgradingthe fluid catalytic cracking (FCC) by-product light cycle oil(LCO) into a high quality diesel blending component is a veryattractive route and might involve a two-step catalyticprocess. In the first step the LCO is hydrotreated andheteroatoms are removed and polyaromatics are saturated, in thesecond step naphthenic rings are selectively opened to improvethe cetane number of the final product.</p><p>The present research is devoted to the second catalytic stepof LCO upgrading and was carried out within the framework of aEuropean Union project entitled RESCATS.</p><p>From the patent literature it is evident that iridium-basedcatalysts seem to be good candidates for ring-opening purposes.A literature survey covering ring opening of naphthenicmolecules shows the need for extending investigations toheavier model substances, more representative of the dieselfraction than model compounds such as alkylated mono C5 and C6-naphthenic rings frequently employed in academic studies.</p><p>Ring-opening catalysts, mainly Pt-Ir based, were synthesisedat KTH by two different methods: the microemulsion and theincipient wetness methods. The global oil refining industry with its present product distribution essentially shifted towards fuels such as gasoline and diesel will most likely hold the fort for considerable time. However, conditions are changing and refinery survival will very much depend on long-term planning, process and product flexibility and being at the frontiers of refining technology, a technology where catalysts play leading roles. Today oil refiners are faced with the challenge of producing fuels that meet increasingly tight environmental specifications, in particular with respect to maximum sulphur content. At the same time, the average quality of crude oil is becoming poorer with higher amounts of aromatics, heteroatoms (sulphur and nitrogen) and heavy metals. In order to stay competitive, it is of decisive importance for refiners to upgrade dense petroleum fractions of low quality to highly value-added products. A practicable route, for example, is upgrading the catalytic cracking by-product Light Cycle Oil (LCO) into a high-quality diesel-blending component in a two-step catalytic process. In the first step the LCO is hydrotreated over a Pt Pd based acidic catalyst bringing about heteroatom and aromatic reduction and isomerization of C6 to C5 naphthenic structures. In the second step these naphthenic structures are selectively opened over an Ir-based catalyst to improve the cetane value. The present thesis is mainly devoted to the second catalytic step of LCO upgrading and was partly conducted within the framework of the European Union project RESCATS. From the patent literature it is evident that iridium-based catalysts could be good candidates for ring-opening purposes. A literature survey covering ring opening of naphthenic structures made in the beginning of the project (in 2001), showed the need for extending investigations to heavier hydrocarbons, more representative of the diesel fraction than model compounds such as alkylated mono C5 and C6 naphthenic rings frequently employed in previous academic studies. Ring-opening catalysts, mainly Pt-Ir based, were synthesised at KTH by two different techniques: the microemulsion and the incipient wetness techniques. Paper I is a review of the microemulsion technique and its applications in heterogeneous catalysis. Characterization of catalysts was performed employing a multitude of techniques including quantitative TPR, TEM-EDX, XPS, CO FT-IR, NH3-DRIFTS and XRF etc. Catalytic screening at 325 oC and atmospheric pressure with hydrogen and pure indan as model substance was conducted to investigate ring-opening activity in terms of conversion and selectivity to desired cetane-boosting products. This development process is the topic of Papers II-IV. The possible industrial implementation of the best catalyst candidate is demonstrated in Paper V. When designing a catalytic system aimed at refining petroleum, it is crucial to monitor the evolution of the sulphur distribution throughout the different stages of the process so that catalyst properties and reaction parameters may be optimised. The final section of this thesis and Paper VI are devoted to high-resolution sulphur-distribution analysis by means of a sulphur chemiluminescence detector (SCD) following gas chromatographic separation. / QC 20101014

Life cycle analysis of sediment control devicesTroxel, Cameron Francis 20 September 2013 (has links)
Sediment control devices (SCDs) are critical to reducing the contamination of waterways from adjacent construction sites. Perimeter sediment controls retard the flow of surface runoff water originating on site and subsequently reduce solid, nutrient, and metal concentrations suspended in the flowing water. Silt fence is a commonly used SCD comprised of geotextile filter fabric, steel or wood support posts, and wire mesh reinforcement. The Georgia Department of Transportation (GDOT) uses an extensive amount of silt fence every year, and because of high degradation of geotextile in the field, the silt fence installations are rarely recycled. This research measures the performance of five SCDs (two types of silt fence, mulch berm, compost sock, and straw bales) at suspended solid, turbidity, nutrient, and metal reduction. A life cycle analysis (LCA) is performed to identify environmental impacts associated with material production, assembly, installation, use on site, and disposal. An impact analysis is performed according to for each SCD. Results of the impact analysis are compared to determine the SCD with lowest overall environmental impact. Results of the SCD performance study show that silt fence installations performed the best at reducing suspended solids and turbidity, mulch was best at reducing nutrients, and compost was the best at reducing metal concentrations. The life cycle impact analysis indicates that a mulch berm is the SCD with the lowest overall environmental impact. The impact analysis included global warming potential, acidification, eutrophication, and aquatic toxicity.

The interconnectedness of global supply chains, combined with disruptive events like the Covid-19 pandemic, has emphasized the significance of enhancing supply chain resilience (SCRes). Thus, this study aims to investigate how businesses employ SCRes recovery strategies to address extreme supply chain disruptions (SCD). To contribute empirical evidence on reactive SCRes strategies, qualitative methodology was employed, with a specific emphasis on the construction department at Northvolt. The construction industry serves as an apt context for assessing and offers valuable insights. Unstructured and semi-structured interviews were conducted with supply chain executives within the company to collect the data. The findings reveal specific challenges faced by the construction department at Northvolt in the face of global supply chain disruptions, shedding light on their responsive strategies. This study not only emphasizes the necessity of cultivating all SCRes capabilities but also contributes valuable empirical evidence to the field. The implications of these findings extend beyond Northvolt, providing actionable insights for businesses in the construction industry and offering recommendations for enhancing overall supply chain resilience. / Den ökande globala kopplingen mellan leverantörskedjor, tillsammans med störande händelser som Covid-19-pandemin, har understrukit betydelsen av att förbättra leverantörskedjors motståndskraft (SCRes). Denna studie syftar därför till att undersöka hur företag använder sig av SCRes-återhämtningsstrategier för att hantera extrema störningar i leverantörskedjan (SCD). För att bidra med empiriska bevis om reaktiva SCRes-strategier användes kvalitativ metodik, med särskild tonvikt på byggavdelningen på Northvolt. Byggbranschen fungerar som en lämplig kontext för bedömning och erbjuder värdefulla insikter. Ostrukturerade och halvstrukturerade intervjuer genomfördes med företagets leverantörskedjechefer för att samla in data. Thesis (PhD)-- Stellenbosch University, 2014. / ENGLISH ABSTRACT: Ventricular arrhythmias are the most important cause of sudden cardiac death (SCD) among adults living in industrialised nations. Genetic factors have substantial effects in determining population-based risk for SCD and may also account for inter-individual variability in susceptibility. Great progress has been made in identifying genes underlying various Mendelian disorders associated with inherited arrhythmia susceptibility. The most well studied familial arrhythmia syndrome is the congenital long QT syndrome (LQTS) caused by mutations in genes encoding subunits of myocardial ion channels. Not all mutation carriers have equal risk for experiencing the clinical manifestations of disease (i.e. syncope, sudden death). This observation has raised the possibility that additional genetic factors may modify the risk of LQTS manifestations.
This study establishes the genetic aetiology of LQTS in South Africa and Denmark through the identification and characterisation of LQTS-causative mutations in five previously identified genes, as well as examining possible novel genetic causes of LQTS in a cohort comprising Danish and British probands. We have functionally characterised several of the mutations identified in this study and examined other cardiac phenotypes that may be explained by variants causing repolarisation disorders. / AFRIKAANSE OPSOMMING: Ventrikulêre aritmie bly die enkele belangrikste oorsaak van skielike hart dood (SCD) onder volwassenes wat in geïndustrialiseerde lande woon. Genetiese faktore het aansienlike gevolge in die bepaling van bevolking-gebaseerde risiko vir SCD en kan ook verantwoordelik wees vir die inter-individuele variasie in vatbaarheid. Groot vordering is gemaak in die identifisering van gene onderliggende verskeie Mendeliese siektes wat verband hou met geërf aritmie vatbaarheid. Die mees goed bestudeerde familie aritmie sindroom is die aangebore lang QT-sindroom (LQTS) wat veroorsaak word deur mutasies in gene kode subeenhede van miokardiale ioonkanale. Nie alle mutasie draers het 'n gelyke risiko vir die ervaring van die kliniese manifestasies van die siekte (dws sinkopee, skielike dood). Hierdie waarneming het die moontlikheid genoem dat genetiese faktore anders as die primêre siekte-verwante mutasie kan die risiko van LQTS manifestasies verander.
Hierdie studie stel die genetiese oorsake van LQTS in Suid-Afrika en Denemarke deur die identifisering en karakterisering van LQTS-veroorsakende mutasies in vyf voorheen geïdentifiseer gene, asook die behandeling van moontlike nuwe genetiese oorsake van LQTS in 'n groep wat bestaan uit van die Deense en die Britse probands.
