Hälsoenkät SF 36, varför rann det ut i sanden?

Att mäta patienters HRQoL (Health Related Quality of Life) är en viktig utvecklingslinje inom vård och vårdforskning. Medicinsk resultatmätning bör kompletteras med nya dimensioner för att kunna utvärdera patientens samlade nytta av behandling och vårdåtgärder. Mätningar är särskilt viktiga när det gäller kroniska sjukdomstillstånd. Syftet med denna studie var att undersöka erfarenheterna av SF 36 (Short Form (36) Health Survey) på enheter som deltog i Skåneprojektet. Metoden var en enkätundersökning med kvalitativ ansats. 18 enkäter skickades ut till de nio enheter som deltog. I vår undersökning framkom att uppfattningarna varierade om instrumentets nytta, från mycket användbart till ingen nytta alls. Flera svar visade att sjuksköterskorna blivit uppmärksammade på hur patienten verkligen upplevde sin situation och hur olika personal och patient bedömer HRQoL. Framgångsfaktor var eldsjälar. Intresse från läkare och patienter påverkade i vilken utsträckning resultaten användes. Flera kunde se betydelsen av att fråga patienten om HRQoL, däremot fanns det osäkerhet om hur resultaten skulle följas upp och användas och en del upplevde att det tog mycket tid. / Measuring patients' HRQoL (Health Related Quality of Life) is an important line of development in the health care and research. Medical outcome measurements should be supplemented by new dimensions to evaluate the patient's overall benefit of treatment and care measures. Measurements are particularly important in chronic disease states. The purpose of this study was to examine the experiences of the SF 36 (Short Form (36) Health Survey) on the units that participated in the Skåne project. The method was a survey with a qualitative approach. 18 questionnaires were sent to the nine units that participated. Our study revealed that perceptions varied on the instrument's utility, from very useful for nothing at all. Several responses showed that nurses have been made aware of how the patient actually experienced their situation and how different staff and patient assess HRQoL. Success factor was enthusiasts. Interest from doctors and patients affect the extent to which the results were used. Many could see the importance of the issue of patient HRQoL, however, there was uncertainty about how the results would be monitored and used, and some felt that it took much time.

The aim of this study was to identify the psychological variables that predict the mental health in a sample of 1, 024 students from Lima and Huánuco. For this purpose, the following scales were used: General Self-efficacy, Coping Estimation Inventory (COPE 60), Academic Experiences Questionnaire (QVA-r), Perceived Stress Scale (PSS) and SF36 questionnaire.Results showed that perceived stress, the avoidant coping, the interpersonal area and study center predict mental health. The mediation analysis revealed that stress mediates the relation between the avoidant coping and the mental health. These findings of this  research may contribute to the development of programs to improve the mental health in college students. / La presente investigación tiene como objetivo identificar las variables psicológicas que predicen la salud mental en una muestra de 1,024 estudiantes universitarios de Lima y Huánuco. Para ello, se aplicaron las siguientes escalas: Autoeficacia General, el Cuestionario de Estimación del Afrontamiento (COPE 60), el Cuestionario de Vivencias Académicas (QVA-r), la Escala de Estrés percibido (PSS) y el cuestionario SF 36. Los resultados indican que el estrés, el estilo de afrontamiento evitativo, el área interpersonal y el centro de estudios predicen la salud mental. El análisis de mediación indica que el estrés media la relación entre el estilo evitativo y la salud mental. Los hallazgos de la presente investigación pueden contribuir con el desarrollo de programas que busquen mejorar la salud mental de los estudiantes universitarios. / A presente investigação tem como objetivo identificar as variáveis psicológicas que preveem a saúde mental em uma amostra de 1.024 estudantes universitários de Lima e Huánuco.Para isso, se aplicaram as seguintes escalas: Auto eficácia Geral, Questionário de Estimativa de Enfrentamento (COPE 60), o Questionário de Vivencias Acadêmicas (QVA-r), a Escala de Estresse Percebido (PSS) e o Questionário SF36. Os resultados indicam que o estresse, o estilo de enfrentamento evitativo, a área interpessoal e o centro de estudos preveem a saúde mental. A análise das medições indicam que o estresse media a relação entre o estilo evitativo e a saúde mental. As descobertas das presente investigação podem contribuir com o desenvolvimento de programas que busquem melhorar a saúde mental dos estudantes universitários.

Perception of quality of life for adults with hearing impairment in Aotearoa / New Zealand.

AIMS: This study investigated the perception of generic and disease-specific Health-Related Quality of Life (HRQoL) for adults living with hearing impairment (HI) in Aotearoa/New Zealand (NZ). This study aimed to answer three questions: (1) What is the perception of HRQoL amongst adults with hearing impairment in NZ? (2) How do these perceptions compare to adults with HI living in other countries for which we have data? (3) What are the demographic and audiometric variables related to device ownership? METHOD: HRQoL, demographic, and audiometric information was collected from 126 adults in NZ. The following demographic information was collected: age, relationship length, hours worked per week, income, ancestry, sex, level of education, city size, and sexual orientation. The following audiologic information was also collected: ownership of hearing aids (HA), ownership of hearing assistance technology (HAT), better-ear pure-tone average (BEPTA), worse-ear pure-tone average (WEPTA), and signal-to-noise ratio loss (SNR loss). HRQoL information was collected using the Medical Outcomes Study 36-Item Short-Form Health Survey (SF-36; Ware & Sherbourne, 1992), and the Hearing Handicap Inventory (HHI) for both elderly (HHIE) and adults (HHIA; Ventry & Weinstein, 1982; Newman, Weinstein, Jacobson, & Hug, 1991). Variables discriminating HA and HAT owners from non-owners were also analysed. RESULTS: The relationship between demographic variables and HRQoL scores revealed that only age and income were significant. Audiometric variables had significant relationships with disease-specific HRQoL scores, as well as HA and HAT ownership. Finally, disease-specific HRQoL scores and all audiometric variables differentiated HA owners from non- owners, but demographic variables did not. Generic HRQoL scores and all audiometric variables differentiated HAT owners from non-owners. CONCLUSIONS: These results suggest that the negative impacts of HI on HRQoL as reported overseas are also present in NZ, and that not only do audiometric variables including SNR loss are related to HRQoL, but HRQoL is a significant predictor for HA and HAT ownership. Further QoL research is warranted amongst the HI population in NZ to identify and understand any causal relationships present amongst these variables. Furthermore, HRQoL instruments and a test of speech understanding in noise have been shown to provide additional meaningful information, and therefore clinicians might consider including them during consultation.
