Avaliação ultrassonográfica e dopplervelocimétrica da esquistossomose mansônica: estudo de campo em áreas de baixa, média e alta endemicidades / Ultrasonographic and dopplervelocimetric evaluation of schistosomiasis mansoni: field-based study in areas of low, medium and high endemicity

INTRODUÇÃO: os principais aspectos clínicos da esquistossomose mansônica (EM) são determinados pelas lesões vasculares e suas repercussões na hemodinâmica portal. O acesso não-invasivo a essas alterações tem importância fundamental na avaliação da morbidade da doença e na identificação de pacientes com maior risco de complicações. Este estudo de campo realizado em três áreas com níveis distintos de endemicidade, objetivou: a) verificar a viabilidade da USDoppler em estudos de campo; b) identificar os sinais de morbidade relacionados à EM e correlacioná-los com a endemicidade das áreas estudadas; c) determinar os valores dopplervelocimétricos do sistema vascular esplâncnico na EM; e d) determinar as alterações hemodinâmicas na EM. CASUÍSTICA E MÉTODOS: foram examinados por meio da US-Doppler, 554 pacientes esquistossomóticos, divididos em três grupos, de acordo com o local de realização do exame: área de baixa endemicidade (n=109); área de média endemicidade (n=255) e área de alta endemicidade (n=190). Avaliaram-se o fígado, o baço, a vesícula biliar, os vasos portais, as artérias hepática e esplênica, as veias hepáticas e os vasos colaterais. Para análise da morbidade da doença, foi utilizado o protocolo do Niamey Working Group (2000). O protocolo Doppler foi elaborado para fins específicos desta pesquisa. RESULTADOS: houve correlação estatisticamente significante entre as frequências de: espessamento periportal, esplenomegalia, espessamento da parede da vesícula biliar, aumento do calibre dos vasos portais, redução do lobo direito e o nível de endemicidade das áreas. Não houve relação significativa entre a hipertrofia do lobo esquerdo e a endemicidade das áreas. Considerando-se os achados ultrassonográficos de espessamento periportal e/ou esplenomegalia, os pacientes foram alocados em quatro grupos de acordo com a forma de apresentação da doença. As formas hepatointestinal e hepatoesplênica foram mais frequentes na área de alta endemicidade, demonstrando associação significativa entre o nível de endemicidade e a gravidade da doença. A velocidade máxima de fluxo da veia porta apresentou valores normais na maioria dos casos. O fluxo hepatofugal, a trombose portal e a circulação colateral foram pouco frequentes e identificados apenas na área de alta endemicidade. A veia gástrica esquerda foi a colateral prevalente. As veias hepáticas apresentaram padrão de fluxo alterado em 1/4 dos casos, alteração essa relacionada à presença e à intensidade de espessamento periportal. A artéria hepática não apresentou alterações significativas nos parâmetros avaliados. As alterações da artéria esplênica (aumento do calibre, da velocidade de pico sistólico e do índice de resistividade) foram mais frequentes na área de alta endemicidade e na forma hepatoesplênica, com diferença significativa entre os grupos. CONCLUSÃO: a US-Doppler mostrouse viável e adequada para avaliação das alterações hemodinâmicas da EM em estudos de campo. Os sinais de morbidade detectados ultrassonograficamente são parâmetros fidedignos do nivel de endemicidade da EM, e, as informações funcionais do sistema portal obtidas pela US-Doppler, contribuem para o diagnóstico mais preciso das formas clínicas da doença nas áreas endêmicas / INTRODUCTION: The main clinical aspects of schistosomiasis mansoni (SM) are determined by vascular lesions and their effects on the portal hemodynamic. The non-invasive access to these alterations has fundamental importance in assessing the morbidity of the disease and in identifying patients at higher risk of complications. This field study, carried out in three areas with different levels of endemicity, aimed to: a) assess the feasibility of Doppler ultrasound (US) in fieldbased studies; b) identify the signs of morbidity related to SM and correlate them with the endemicity of the studied areas; c) determine the dopplervelocimetric values of the splanchnic vascular system in SM; and d) determine the hemodynamic changes in SM. PATIENTS AND METHODS: Doppler US was performed in 554 patients with SM, divided into three groups according to the local of the examination: low endemicity area (n=109), medium endemicity area (n=255), and high endemicity area (n=190). The liver, the spleen, the gallbladder, the portal vessels, the hepatic and splenic arteries, the hepatic veins and collateral vessels were evaluated. The Niamey Working Group (2000) protocol was used for the disease morbidity analysis. The Doppler protocol was developed for the specific purposes of this study. RESULTS: There was a statistically significant correlation between the frequency of: periportal thickening, splenomegaly, gallbladder wall thickening, dilatation of the portal vessels, right lobe atrophy and the endemicity level of the areas. No significant correlation between the left lobe hypertrophy and the endemicity level was observed. Taking into account the US findings of periportal thickening and/or splenomegaly, the patients were allocated in four groups according to the form of the disease. The hepatointestinal and hepatosplenic forms were more frequent in the high endemicity area, showing a significant association between the endemicity level and the disease severity. The maximum flow velocity in portal vein showed normal values in most cases. The hepatofugal flow, portal vein thrombosis and collateral vessels were infrequent and identified only in the area of high endemicity. The left gastric vein was the prevalent collateral. The hepatic veins flow pattern was abnormal in 1/4 of the cases, with significant relation to the presence and intensity of periportal thickening. The hepatic artery had no significant alterations in the evaluated parameters. The splenic artery changes (increase of its diameter, peak systolic velocity and resistive index) were more frequent in the high endemicity area and in the hepatosplenic form, with significant difference between the analysed groups. CONCLUSION: Doppler US proved to be feasible and appropriate for the evaluation of the SM hemodynamic changes in field-based studies. The morbidity signs detected by ultrasound are reliable parameters of the endemicity level of SM, and the functional information of the portal system obtained by Doppler US, contributes to a more accurate diagnosis of clinical forms of the disease in endemic areas

O trabalho discute a utilidade da ultra-sonografia, no diagnóstico da forma hepato-esplênica da esquistossomose mansônica. O autor estudou, pela ultra-sonografia, 103 pacientes com esquistossomose hepato-esplênica comprovada e observou os sinais encontrados no fígado, vias biliares, baço e circulação portal. Foi determinada a freqüência dos diferentes sinais presentes no grupo de pacientes esquistossomóticos e feita correlação com as alterações anátomo-patológicas da doença. Os valores dos calibres das veias esplênica, porta e mesentêrica superior dos nossos pacientes esquistossomóticos foram comparados aos encontrados em 50 pacientes sem anormalidades hepáticas (grupo-controle). As alterações encontradas no fígado incluíram espessamento da parede da veia porta e seus ramos, nódulos parenquimatosos, atrofia do lobo direito e hipertrofia do lobo esquerdo. O espessamento da parede dos vasos portais correlaciona-se a fibrose peri-portal encontrada no exame anátomo-patológico e foi sinal muito freqüente. A atrofia do lobo direito e a hipertrofia do lobo esquerdo devem estar relacionadas ao hiperfluxo pela veia esplênica. Os nódulos parenquimatosos representam os granulomas que, pelas suas pequenas dimensões, são raramente visualizados pela ultra-sonografia. A vesícula biliar apresentou espessamento de sua parede em número significativo dos pacientes estudados. O baço encontrou-se aumentado de volume em todos os pacientes, atingindo geralmente grandes proporções. Em pequeno número de casos observou-se padrão multinodular que foi atribuído aos nódulos sideróticos encontrados ocasionalmente na esquistossomose. A média dos calibres da veia porta, veia esplênica e veia mesentérica superior, nos pacientes esquistossomóticos, foi estatisticamente superior à dos pacientes do grupo-controle. O aumento do calibre foi mais acentuado para a veia esplênica do que para a veia mesentérica superior. Os vasos colaterais estudados foram a veia umbilical, a veia gástrica esquerda e os vasos gástricos curtos, não apresentando características particulares para esta doença. A trombose da veia porta foi encontrada em raros casos. Os principais sinais sonográficos da esquistossomose hepato-esplênica incluem fibrose peri-portal, hipertrofia do lobo esquerdo, espessamento da parede da vesícula biliar, nódulos hepáticos, padrão multinodular do baço e aumento do calibre dos vasos portais. The frequency of the different signs present in the group of schistosomotic patients had been determined and the correlation with the anatomopathological alterations of the disease was done. The values of the diameter of the splenic, portal and superior mesenteric veins in the schistosomotic patients was compared with the ones found in 50 patients without hepatic abnormalities. The alterations found in the liver includes the thickening on the wall of the portal vein and its branches; atrophy of the right lobe and hypertrophy on the left lobe and hepatic nodules. The thickening of the wall of the portal vessels is correlated to the periportal fibrosis found in the anatomopathologic examination and is a very frequent sign. The atrophy of the right lobe and the hypertrophy of the left lobe is most probably related to the higher vascular flow through the splenic vein. The hepatic nodules represent granulomas that, due to their small dimentions, are seldom visualized by ultrasonography. The thickening of the gallbladder wall, was found in a significant number of the studied patients. The spleen presented an enlarged volume in all patients, reaching generally big proportions. In a small number of cases a multinodular pattern was observed and it has been atributed to the siderhotic nodules occasionally found in schistosomiasis. The average of the diameters of the portal, splenic and superior mesenteric veins of the patients with schistosomiasis was statistically higher than the ones found in the control group. The enlargement of the diameter was more accentuated in the splenic vein rather than in the superior mesenteric vein. The collateral vessels studied were the umbilical vein, the left gastric vein and the short gastric vessels, not presenting particular characteristics for this disease. Thrombosis of the portal vein was rarely found. The main sonographic signs of the hepatosplenic schistosomiasis include periportal fibrosis, hypertrophy of the left lobe, thickening of the wall of the gallbladder, granulomas, multinodular pattern of the spleen and enlargement of the diameter of the portal vessels. Neste estudo avaliou-se o efeito in vitro de 4 compostos isolados de espécies vegetais, as amidas piplartina e piperina, a lignana grandisina e o alcaloide epiisopiloturina; 1 composto isolado da pele de anfíbio, o peptídeo antimicrobiano dermaseptina 01; e de 6 extratos etanólicos obtidos de vegetais, Piper tuberculatum, P. crassinervium, P. diospyrifolium, P. fuligineum, P. gaudichaudianum e Pothomorphe umbellata em adultos (machos e fêmeas com 49 dias) e esquistossômulos (recém-transformados, 1, 3, 5 e 7 dias) de Schistosoma mansoni linhagem BH. O estudo avaliou: 1) a viabilidade de vermes adultos; 2) a capacidade reprodutiva, avaliada pelo acasalamento e oviposição; 3) o efeito no tegumento em parasitas adultos; 4) a viabilidade e o efeito no tegumento em esquistossômulos; 5) a citotoxicidade de compostos e extratos em células de mamífero (célula Vero). Além disso, com microscopia confocal, neste estudo é proposto um novo modelo experimental que avalia, quantitativamente, o efeito de esquistossomicidas no tegumento dos esquistossomos. / In this study, the in vitro effect of 4 compounds isolated from plant species, amides piplartine and piperine, the lignin grandisin and alkaloid epiisopiloturine; 1 compound isolated from amphibian skin, the antimicrobial peptide dermaseptin 01; and 6 ethanolic extracts of plants, Piper tuberculatum, P. crassinervium, P. diospyrifolium, P. fuligineum, P. gaudichaudianum and Pothomorphe umbellata was evaluated in adults worm pairs (49-day-old) and schistosomula (newly-transformed, 1-, 3-, 5-, and 7-day-old) of Schistosoma mansoni BH strain. The effect of compounds and extracts against schistosomes was examined regarding: 1) adult worms survival, 2) the reproductive fitness as assessed by mating and oviposition; 3) alterations on adult worms tegumental surface; 4) viability and morphological alterations on schistosomula; 5) the cytotoxicity of compounds and extracts in mammalian cells (Vero cells) In addition, this study shows a new experimental model to evaluate quantitatively the effect of schistosomicidal drugs on the tegument of the schistosomes.

A esquistossomose mansoni apresenta-se como uma doenÃa parasitÃria de importÃncia para a saÃde pÃblica, nÃo sà por sua ampla distribuiÃÃo no mundo, mas tambÃm pelos impactos causados por ela nas atividades da populaÃÃo infectada. A esquistossomose deve ser analisada como um processo dependente da interaÃÃo de vÃrios fatores, como fatores ambientais, sociais e econÃmicos, importantes para reduzir a transmissÃo da doenÃa bem como para interromper o ciclo do parasito. O objetivo do estudo foi avaliar a influÃncia de fatores ambientais e socioculturais na transmissÃo da esquistossomose em Ãrea de baixa endemicidade no Estado do CearÃ, na localidade do Planalto do Cajueiro, atravÃs da anÃlise de questionÃrios aplicados no ano de 2009 e em 2012. O estudo foi desenvolvido em duas etapas. A primeira consiste em um estudo transversal, onde foram analisados 167 questionÃrios dos pacientes que forneceram material sorolÃgico para a realizaÃÃo do mÃtodo ELISA, que serviu como triagem. A segunda etapa consiste em um estudo de caso controle, onde foram aplicados e analisados questionÃrios em 54 indivÃduos ELISA reativo e nÃo reativo que entregaram amostra fecal para anÃlise. As anÃlises dos questionÃrios das 167 pessoas mostraram que a maioria delas eram adultos jovens (19,76% na faixa etÃria de 15 a 25anos e 29,94% entre 26 a 46 anos), do sexo feminino (67,66%) e que 29% nÃo eram naturais de Maranguape. Em torno de 52,5% dos pacientes que afirmaram utilizar Ãgua do rio, tiveram resultado Elisa reativo para esquistossomose. Na segunda etapa, observou-se que a diferenÃa entre os sexos masculinos e femininos era insignificante, nos casos positivos para esquistossomose. Com relaÃÃo à escolaridade, a maioria (68,32%) possuÃa apenas o ensino fundamental incompleto. Nas associaÃÃes entre variÃveis ambientais e resultado positivo para a doenÃa, observou-se que 34,2% dos indivÃduos responderam nÃo ter contato com coleÃÃes hÃdricas e 37,5% disseram que tiveram contato. Este ponto ainda està em anÃlise mais aprofundada. A comunidade do Planalto do Cajueiro em Maranguape-CE mantÃm caracterÃsticas ambientais semelhantes à de comunidades rurais (mesmo sendo peri-urbana) e a transmissÃo da esquistossomose sofre grande influencia dos fatores comportamentais. / Schistosomiasis mansoni is presented as a parasitic disease of public health importance, not only for its wide distribution in the world, but also the impacts caused by it in the activities of the infected population. Schistosomiasis should be analyzed as a process dependent on the interaction of various factors, such as environmental, social and economic importance to reduce transmission of the disease and to stop the cycle of the parasite. The aim of the study was to evaluate the influence of environmental and sociocultural factors in the transmission of schistosomiasis in low endemic area in the state of CearÃ, in the locality of the Planalto do Cajueiro- Maranguape-Ce, through the analysis of questionnaires applied in 2009 and 2012. The study was conducted in two stages. The first consists of a cross-sectional study, which analyzed 167 questionnaires from patients who provided material for the realization of the serological ELISA, which served as a screen. The second stage consists of a case-control study where questionnaires were applied and analyzed in 54 individuals ELISA reactive and nonreactive who delivered fecal sample for analysis. The analysis of the questionnaires of 167 people showed that most of them were young adults (19.76% in the age group of 15 to 25years and 29.94% between 26-46 years), female (67.66%) and 29% were not natives of Maranguape. Around 52.5% of patients reported using water from the river, had reactive ELISA result for schistosomiasis. In the second stage, it was observed that the difference between male and female sexes was negligible in cases positive for schistosomiasis. With regard to education, the majority (68.32%) had only elementary education. Associations between environmental variables and positive for the disease, it was observed that 34.2% of subjects reported not to have contact with water collections and 37.5% said they had contact. This point is still under further investigation. The community of Planalto do Cajueiro, Maranguape-Ce maintains environmental characteristics similar to rural communities (even as peri-urban) and schistosomiasis transmission is greatly influences behavioral factors.

Estudo da morbidade residual da esquistossomose mansônica através da ultra-sonografia no município de Bananal, São Paulo, Brasil / Study on residual morbidity from schistosomiasis by means of ultrasonography in the municipality of Bananal, São Paulo, Brazil

Este estudo desenvolveu-se no município de Bananal, São Paulo, uma área endêmica para esquistossomose com prevalência menor que 10% e baixa carga parasitária dos infectados. Teve como objetivo a identificação de formas clínicas da esquistossomose mansônica entre 109 pacientes diagnosticados parasitoscopicamente e medicados com oxamniquine, durante a realização do Plano de Intensificação das Ações de Controle da Esquistossomose Mansônica (1998-2000). Utilizou-se a ultra-sonografia abdominal e o exame de fezes (Kato-Katz) realizado, em média, quatro anos após o término do plano. Nesta casuística, foram identificados cinco pacientes com imagens ultra-sonográficas abdominais compatíveis com fibrose periportal periférica e central e hipertensão portal que tinham diagnósticos clínicos de esquistossomose na sua forma intestinal. A ultra-sonografia é um método de diagnóstico sensível, incruento, que possibilitou a identificação de casos de esquistossomose com comprometimento hepático mais extenso do que se expressava pelo exame físico. Mesmo considerando uma prevalência baixa de alterações hepáticas e de circulação portal nesta casuística, evidencia-se a importância do emprego do método ultra-sonográfico na avaliação individual dos pacientes esquistossomóticos, por permitir a detecção de alterações morfológicas e funcionais que podem ter conseqüências clínicas relevantes. Deve-se assinalar ainda que, no momento do exame, todos os pacientes tiveram coproscopias negativas, revelando a efetividade das ações de controle no médio prazo. Devemos considerar que a realização do estudo ultra-sonográfico em média quatro anos após o tratamento específico dessa população, provavelmente detectou menos alterações do que aconteceria se o estudo fosse feito no momento do diagnóstico parasitológico, pois se sabe que a involução, ainda que parcial da fibrose e de alterações funcionais conseqüentes a ela, ocorre após tratamento específico. Por fim, apesar do pequeno número de casos avaliados, a estratégia utilizada no presente trabalho começou a preencher a lacuna de avaliação do impacto da esquistossomose sobre a saúde do cidadão bananalense, percebida durante o desenvolvimento do Plano de Intensificação das Ações de Controle da Esquistossomose Mansônica, período de 1998 a 2000. / This study was developed in the municipality of Bananal, São Paulo, an endemic area for schistosomiasis with prevalence of less than 10% and low parasite load among infected individuals. The objective was to identify the clinical forms of intestinal schistosomiasis among 109 patients who had been diagnosed through parasitological tests and medicated with oxamniquine at the time of the Plan for Intensification of Schistosomiasis Control Actions (1998-2000). Abdominal ultrasonography and feces examination (Kato-Katz) were utilized: this was on average done four years after the ending of the plan. In this sample, five patients were identified whose abdominal ultrasound images were compatible with peripheral and central periportal fibrosis and portal hypertension who had had a clinical diagnosis of schistosomiasis in its intestinal form. Ultrasonography is a sensitive noninvasive diagnostic method that enables identification of the extent of liver involvement in schistosomiasis cases better than through its expression in physical examination. Even considering that there was a low prevalence of liver abnormalities and portal circulation in this sample, the importance of utilizing the ultrasound method for individual evaluations on the schistosomiasis patients was demonstrated. Through this, it was possible to detect morphological and functional alterations that could have important clinical consequences. It also should be noted that, at the time of the ultrasound examination, all the patients presented negative coproscopic test results, thus showing the effectiveness of the control actions over the medium term. It needs to be borne in mind that, because the ultrasound study was carried out on average four years after the specific treatment of this population, it is likely that fewer abnormalities were detected than there would have been if this study had been performed at the time of the parasitological diagnosis. This comes from the knowledge that involution occurs following specific treatment, even if only partially with regard to fibrosis and its consequent functional alterations. Finally, despite the small number of cases evaluated, the strategy utilized in the present study has started to fill the gap regarding assessing the impact of schistosomiasis on the health of the citizens of Bananal that was perceived during the development of the Plan for Intensification of Schistosomiasis Control Actions between 1998 and 2000.

Considerando a imunização com OVA um modelo vacinal, as reações inflamatórias e a produção de anticorpos em resposta a esse antígeno são importantes para o desenvolvimento de uma imunidade satisfatória do hospedeiro. Nesse sentido, as células Th1 e Th17 são importantes fatores para o desenvolvimento dessas respostas. Dessa forma, os camundongos deficientes na produção de IL-12/IL-23 (12p40 knockout-KO) são predispostos a desenvolverem uma resposta Th2 polarizada, tornando-se menos responsivos às vacinações. Diante disso, o presente trabalho investigou o efeito da amamentação em mães infectadas pelo Schistosoma mansoni sobre as imunidades humoral e celular de camundongos adultos C57BL/6 12p40 KO, em resposta ao modelo vacinal acima citado. Foram avaliados: a cinética das reações de hipersensibilidade in vivo; os níveis plasmáticos das imunoglobulinas IgG1 e IgG2a; a produção das citocinas IFN-γ, IL-17, IL-5, IL-6, IL-10 e TGF- pelas células esplênicas e a reação inflamatória provocada no coxim plantar. Para isso, camundongos machos, deficientes na produção de IL-12 e IL-23 (IL-12p40 KO) e camundongos selvagens (wild-type/WT) foram divididos nos seguintes grupos: camundongos IL-12p40 KO amamentados em mães infectadas (AI IL-12p40 KO); camundongos IL-12p40 KO amamentados em mães sem infecção (NANI IL-12p40 KO); camundongos selvagens amamentados em mães infectadas (AI WT) e camundongos selvagens amamentados em mães sem infecção (NANI WT). Cinquenta por cento dos animais de cada grupo foram imunizados com OVA em adjuvante. Os outros 50% porcento restantes permaneceram sem imunização. No grupo AI WT houve aumentado de produção de IgG2a, IL-5, TGF-β e IL-6, com baixos níveis de IL-17, em comparação ao NANI WT. Nos animais AI IL-12p40 KO, a produção de IgG2a, IL-5 e TGF-β foi mais alta do que o grupo NANI IL-12p40 KO e similar ao grupo AI WT, mas a produção de IL-6 foi mais baixa. O grupo AI WT mostrou intenso infiltrado inflamatório de eosinófilos na reação de hipersensibilidade tardia (RHT), com acentuado edema em comparação com o edema menos intenso e infiltrado inflamatório de neutrófilos do grupo NANI WT. Os animais NANI IL-12p40 KO e AI IL-12p40 KO não apresentam RHT, porém a reação inflamatória no AI IL-12p40 KO foi menos intensa que nos NANI IL-12p40 KO. Em conclusão, o contato com antígenos do parasito, através da amamentação, induziu, no descendente adulto, uma melhor resposta de anticorpo neutralizante, mesmo diante da deficiência na produção de IL-12e IL-23. Nesta condição, embora tenha havido uma notável produção de IL-5, a lactação em mães infectadas atenuou a reação inflamatória, provavelmente através da regulação cruzada entre TGF-β e IL-6, modulando, desta forma, o status de hiperativação desses animais. / The previous contact with mothers milk schistosomiasis induced in adult mice enhancement of antibody production and increased antigen presentation capacity by B lymphocytes in response to the heterologous antigen ovalbumin (OA). Considering immunization with OA one vaccine model, inflammatory reactions and antibody production in response to antigen are important for the development of a suitable host immunity. In this sense, the Th1 and Th17 cells are important factors for the development of these responses. Thus, mice deficient in IL-12/IL-23 (12p40 knockout-KO) are likely to develop a polarized Th2 response, making it less responsive to vaccination. Therefore, the present study investigated the effect of breastfeeding in mothers infected with Schistosoma mansoni on the humoral and cellular adult C57BL/6 12p40 KO in response to vaccination model mentioned above. Were evaluated: the kinetics of in vivo hypersensitivity reactions; plasma levels of IgG1 and IgG2a immunoglobulins; the production of the cytokines IFN-γ, IL-17, IL-5, IL-6, IL-10 and TGF-β by spleen cells and the inflammatory reaction induced in the footpad. To this end, male mice deficient in IL-12 and IL-23 (IL-12p40 KO) and wild-type mice (Wild-type/WT) were divided into the following groups: IL-12p40 KO mice suckled by infected mothers (IL-12p40 KO- SIM); IL-12p40 KO mice suckled by uninfected mothers (IL-12p40 KO); Wild-type mice suckled by infected mothers (SIM) and wild-type mice suckled by uninfected mothers (CONTROL). Fifty percent of animals in each group were immunized with OA in adjuvant. The other 50% remaining percent remained without immunization. In the SIM group was increased production of IgG2a, IL-5, TGF-β and IL-6, IL-17 with low levels compared to CONTROL. In animals IL-12p40 KO-SIM the production of IgG2a, IL-5 and TGF-β was higher than the IL-12p40 KO similar to group SIM, but IL-6 production was lower. The SIM group showed intense inflammatory infiltrate of eosinophils in the delayed hypersensitivity reaction (DTH), with severe edema compared with the less intense edema and inflammatory infiltration of neutrophils CONTROL group. The animals IL-12p40 KO and IL-12p40KO-SIM not have DTH, but the inflammatory reaction in the IL-12p40KO-SIM was less intense than in IL-12p40 KO. In conclusion, contact with parasite antigens, through breastfeeding, induced in adult offspring, better neutralizing antibody response, despite the deficiency in the production of IL-12 and IL-23. In this condition, though there has been a remarkable IL-5 production in lactating mothers infected with attenuated inflammatory response, probably via cross regulation between TGF-β and IL-6 modulate thereby the status of hyperactivation of these animals.

A urbanização e a ocupação de espaços de uma forma desordenada podem contribuir para mudanças importantes no ambiente, favorecendo a poluição e o uso de recursos naturais e contribuindo para a proliferação de algumas doenças parasitárias, em especial doenças transmitidas por água e vetores. A esquistossomose é uma dessas doenças, causada por um parasito trematódeo, Schistosoma mansoni, que requer um caramujo de água doce para servir como hospedeiro intermediário. Esta é uma doença de larga distribuição e representa um importante problema de saúde pública no Brasil. Este estudo foi realizado em Pindamonhangaba, cidade situada no Vale do Paraíba, onde os primeiros casos de esquistossomose foram notificados em 1955; com grandes campos de arroz e diferentes tipos de atividades agrícolas, fazendo uso de riachos e canais de irrigação, a região apresentava alguns aspectos ambientais que favoreciam a proliferação de caramujos, que costumavam lançar no ambiente aquático grande número de cercárias, tendo em vista as precárias condições sanitárias da população e a contaminação ambiental. O objetivo do presente estudo foi verificar a ocorrência e a distribuição dos casos de esquistossomose nas diferentes localidades da cidade, relacionando os dados colhidos nos últimos 15 anos (1992 a 2006) com as transformações ambientais observadas. Um total de 275 casos foram notificados no período estudado, e as localidades com maior número de casos foram Colméia e Mombaça, respectivamente com 144 e 29 casos. Os índices de prevalência, em 100.000 habitantes, variaram de 57,24 a 3,58, respectivamente em 1992 e 2005. Uma baixa prevalência também foi observada no inquérito soro-epidemiológico realizado com crianças de 2 a 4 série do ensino fundamental, nas escolas situadas em localidades que historicamente representavam áreas de risco para a doença; do total de 544 amostras de sangue submetidas ao teste sorológico para detecção de anticorpos anti-Schistosoma, apenas uma foi reagente. A redução do número de casos de esquistossomose pode estar relacionada às mudanças nos hábitos da população e aos processos de urbanização, com a substituição de atividades agrícolas para as relacionadas com o comércio e a indústria; foi observada a mecanização dos arrozais e substituição do cultivo por áreas de pastagens. Melhorias nas condições de saneamento básico, associada ao aumento do índice escolar também contribuíram para o controle da esquistossomose. / The urbanization and the occupation of spaces in a disordered way can contribute for important changes in the environment, favoring the pollution and the use of natural resources, and contributing for the proliferation of some parasitic diseases, in special waterborne and vector-borne diseases. Schistosomiasis, one of these diseases, is caused by a trematode parasite, Schistosoma mansoni, which requires a freshwater snail to serve as intermediate host. This is an illness of large distribution and represents an important problem of public health in Brazil. This study was carried out in Pindamonhangaba, city located in the Valley of the Paraíba river, where the first cases of schistosomiasis were reported in 1955; with great irrigated rice fields and different types of agricultural activities, requiring streams and irrigation channels, the region showed some environmental aspects that favored the proliferation of snails, which used to shed high number of cercariae, due to the poor sanitation conditions of the population and contamination of the environment. The objective of the present study was to verify the occurrence and distribution of schistosomiasis cases in the different localities of the city, by relating the data collected in the last 15 years (1992 to 2006) with the observed environmental transformations. A total of 275 cases were notified in the studied period, and the localities with higher number of cases were Colméia and Mombaça, respectively with 144 and 29 cases. The prevalence indices, in 100.000 inhabitants, varied from 57.24 to 3.58, respectively in 1992 and 2005. A low prevalence was also observed in a sero-epidemiological survey conducted with children from 2nd to 4th elementary school levels, living in localities historically considered as areas of risk for the disease; from the total of 544 blood samples submitted to detection of anti-Schistosoma antibodies, only one showed to be positive. The reduction in the number of schistosomiasis cases can be related to the changes in the habits of the population and the urbanization processes, with substitution of the agricultural activities for those related to the business and the industry; it was observed the mechanization of the rice fields and substitution of the farming for pastures areas. Improvements in the basic sanitation conditions, associated to the increasing of the scholar index can also be contributed for the control of the schistosomiasis.

CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior / O envolvimento renal na esquistossomose mansoni à raramente descrito e pode ser caracterizada principalmente por alteraÃÃes glomerulares, podendo permanecer assintomÃtica, daà a importÃncia de biomarcadores que possam detectar precocemente alteraÃÃes da funÃÃo renal. O objetivo foi caracterizar as alteraÃÃes renais em pacientes portadores de esquistossomose mansoni, antes e apÃs tratamento na fase intestinal, procedentes de uma Ãrea de baixa endemicidade no Estado do CearÃ. Trata-se de um estudo transversal, de carÃter avaliativo e de natureza quantitativa e intervencionista, incluindo 85 pacientes com diagnÃstico parasitolÃgico (Kato-Katz) confirmado de esquistossomose mansoni. Os pacientes foram divididos em trÃs grupos: Grupo I (G-I) - grupo controle com 24 indivÃduos nÃo infectados; Grupo II (G-II) - grupo com 30 indivÃduos infectados por S. mansoni e Grupo III (G-III) - grupo com 31 indivÃduos infectados por S. mansoni, tratados e avaliados apÃs o tratamento. A funÃÃo renal foi avaliada atravÃs de marcadores renais tubular e glomerular, incluindo dosagem do pH urinÃrio, estimativa da fraÃÃo de excreÃÃo dos eletrÃlitos (FE), estimativa do ritmo de filtraÃÃo glomerular (eRFG), albumina urinÃria e MCP-1/CCL2 urinÃrio (proteÃna quimiotÃtica de monÃcitos-1). Dados do presente estudo mostram que a maioria dos indivÃduos estavam dentro da faixa etÃria em torno de 23,2  13 anos, sendo 39 (45,88%) homens e 46 (54,11%) mulheres. Quando os marcadores renais tubulares foram analisados verificou-se que nÃo houve diferenÃa entre os grupos. Com relaÃÃo aos marcadores renais glomerulares foi observado que MCP-1 foi o Ãnico que apresentou diferenÃa, sendo maior no G-II (178  97pg/mcg-Cr) e no G-III (175  87pg/mcg-Cr), quando comparado com o G-I (123  48pg/mcg-Cr), p=0,009 e p=0,007, respectivamente. NÃo houve diferenÃa quando os grupos G-II e G-III (p=0,892) foram comparados. Apesar da albumina urinÃria nÃo ter apresentado diferenÃa entre os trÃs grupos, ela correlacionou-se com MCP-1 (r=0,463; p=0,01). Em suma foi observado um aumento significativo dos nÃveis urinÃrios de MCP-1 nos pacientes com esquistossomose mansoni. Como esta proteÃna desempenha um importante papel no recrutamento de monÃcitos para os sÃtios de lesÃes e infecÃÃes, o seu aumento na urina sugere que hà uma inflamaÃÃo renal e isso nÃo se reverteu apÃs o tratamento desta doenÃa. / Renal involvement in schistosomiasis is rarely reported and can be characterized mainly by glomerular and may remain asymptomatic, hence the importance of biomarkers that can detect early changes in renal function. The objective was to characterize renal changes in patients with schistosomiasis mansoni before and after treatment in the intestinal phase, coming from an area of low endemicity in the state of CearÃ. This is a cross-sectional study of character evaluation, quantitative and interventionist, including 85 patients with parasitological (Kato-Katz) confirmed schistosomiasis. Patients were divided into three groups: Group I (GI) - control group of 24 uninfected individuals, Group II (G-II) - a group of 30 individuals infected with S. mansoni and Group III (G-III) - group with 31 individuals infected with S. mansoni, processed and evaluated after treatment. Renal function was assessed by renal tubular and glomerular markers, including measurement of urinary pH, estimation of fractional excretion of electrolytes (FE), estimated glomerular filtration rate (eGFR), urinary albumin and urinary MCP-1/CCL2 ( Monocyte chemoattractant protein-1). Data from this study show that most subjects were within the age range around 23,2 Â13 years, 39 (45,88%) men and 46 (54,11%) women. When the renal tubular markers were analyzed it was found that there was no difference between groups. With respect to renal glomerular markers was observed that MCP-1 was the only one that was different, being higher in G-II (178  97pg/mcg-Cr) and G-III (175  87pg/mcg-Cr) when compared with the GI (123  48pg/mcg-Cr), p = 0,009 and p = 0,007, respectively. There was no difference among the groups G-G-II and III (p = 0,892) were compared. Although the albumin excretion did not provide a difference between the three groups, it was correlated with MCP-1 (r= 0,463, p= 0.01). In short there was a significant increase in urinary levels of MCP-1 in patients with schistosomiasis. As this protein plays an important role in the recruitment of monocytes to sites of injury and infection, its increase in urine suggests that there is an inflammation of the kidneys and this is not reversed after treatment of this disease.
