Acompanhamento terapêutico como dispositivo da reforma psiquiátrica : considerações sobre o setting

Este trabalho estuda uma modalidade de atenção em saúde denominada acompanhamento terapêutico, que é importante ferramenta para o tratamento de sujeitos psicóticos e útil para a Reforma Psiquiátrica. Estuda-se o setting do AT, a fim de estabelecer uma relação possível entre os conceitos psicanalíticos e as singularidades que esta modalidade propõe, problematizando-as através dos seguintes conceitos: dispositivo, acaso/acontecimento e cidade. Busca-se algumas respostas às seguintes questões: Como se estabelece uma relação que propicie o surgimento de um encontro entre acompanhante e acompanhado? No acompanhamento terapêutico como poderíamos pensar um início de tratamento? O que de fato ocorre ou precisa acontecer? O que significa isto do ponto de vista da psicanálise e considerando que estamos trabalhando com sujeitos psicóticos? Qual o espaço de trabalho, ou melhor dizendo, o setting do AT? Que conceitos estão em jogo nesse setting. Podemos nos apoiar nas descobertas freudianas ainda que o setting seja sabidamente diferente? Que problemas isso acarreta desde o ponto de vista técnico? A metodologia é o estudo de caso único. Utiliza-se como fonte os registros de um atendimento, entrevistas e o material escrito produzido pelo próprio sujeito acompanhado. Escolheu-se cenas ilustrativas do caso atendido vinculando-as à teorização apresentada. Entende-se o acompanhamento terapêutico como importante dispositivo clínico e político do processo da Reforma Psiquiátrica. O acompanhante terapêutico é um agente que transita entre os espaços de tratamento (o dentro) e os demais espaços sociais (o fora) com uma visão desinstitucionalizante e com o objetivo de auxiliar os sujeitos a ressignificarem seu lugar social.

Increasing physical activity levels among girls in Russia: a cross-over trial

Master of Public Health / Human Nutrition / Richard R. Rosenkranz / Background: Children who obtain insufficient physical activity (PA) have increased risk for chronic diseases. From childhood to adolescence, there is typically a decline in overall PA, with a more rapid decline in girls, at a younger age. The objective of this study was to determine the impact of two types of organized PA instructional conditions (structured no-choice, structured choice) on girls’ PA levels, as compared to free-play at a summer camp in Russia. We hypothesized that free-play would elicit the highest levels of PA. Methods: This study used a within-subjects cross-over trial design. Thirty-two girls (aged 10.7± 0.6yr; BMI percentile 47± 31%) at a Russian summer camp, attended daily 35-minute PA sessions for three weeks. Using the evidence-based Coordinated Approach to Child Health physical activity box, three PA instructional conditions (structured choice, structured no-choice, free-play) were implemented each day. Actical PA monitors collected step count and PA intensity data. Mixed model ANOVAs were used to assess differences in step counts and percentage of time in Moderate to Vigorous Physical Activity (MVPA) by instructional condition. Results: Twenty-five of the 32 participants attended all sessions, and 31 attended at least two of three sessions for each condition. Both structured conditions, no-choice steps/min (mean= 42.7 steps/min; 95%CI= 39.6–45.7; p= 0.0003) and choice condition steps/min (mean= 41.0 steps/min; 95%CI= 37.9–44.1; p= 0.004) were significantly higher than free-play steps/min (mean= 33.4 steps/min; 95%CI= 30.2–36.5). Percent time in MVPA was higher in the no-choice condition (mean= 30.9%; 95%CI= 28.1–33.8; p<0.0001) and choice condition (mean= 30.8%; 95%CI= 27.9–33.7; p< 0.0001) when compared to free-play (mean= 21.2 steps/min; 95%CI=18.2–24.1). There was no difference in steps/min or percentage time in MVPA between both structured conditions. Conclusion: Both types of instruction were superior to free-play with regard to PA level. Although contrary to our hypothesis, our results fit with previous literature that suggests evidence-based instructional interventions can promote higher PA levels in physical education sessions. Our results suggest that well-planned, stimulating PA sessions can increase short-term PA levels in girls compared to free-play opportunities in a Russian summer camp setting.

Music Intervention to Prevent Delirium among Older Patients Admitted to a Trauma Intensive Care Unit and a Trauma Orthopedic Unit

abstract: Greater than half of older adults who are admitted to an acute care setting experience delirium with an estimated cost between four to twenty billion dollars annually in the United States. As a strategy to address the gap between research and practice, this feasibility study used the Roy Adaptation Model to provide a theoretical perspective for intervention design and evaluation, with a focus on modifying contextual stimuli in a Trauma Intensive Care and a Trauma Orthopedic Unit setting. The study sample included older hospitalized patients in a Trauma Intensive Care and a Trauma Orthopedic setting where there is a greater incidence for delirium. Study participants included two groups, with one group assigned to receive either a music intervention or usual care. The music intervention included pre-recorded music, delivered using an iPod player with soft headsets, with music self-selected from a collection of music compositions with musical elements of slow tempo and simple repetitive rhythm that influence delirium prevention. For the proposed study a music intervention dose included intervention delivery for 60 minutes, twice a day, over a three day period following admission. Physiologic variables measured included systolic blood pressure, diastolic blood pressure, heart rate, and respiratory rate, which were electronically monitored every four hours for the study. The Confusion Assessment Method was used as a screening tool to identify delirium in the admitted patients. Specific aims of this feasibility study were to (a) examine the feasibility of a music intervention designed to prevent delirium among older adults, and (b) evaluate the effects of a music intervention designed to prevent delirium among older adults. Future Time Perspective and Strategy Development of Incarcerated Young Adults

abstract: Possible selves research has focused primarily on academic achievement and student learning, for at-risk, adolescent or college aged students. The research has not examined an occupation possible self, nor the implications of how time is considered by incarcerated populations. This study was designed to expand the Possible Selves Questionaire (PSQ) designed by Oyserman for an occupational achievement code and explore any unique codes present for incarcerated young adult males, aged 18-22. Additionally, this study was designed to compare two distinct time horizons for incarcerated young adults, a more proximal one-year event which would represent continued incarceration and a post-release distal time horizon. A pilot study was conducted to establish the occupation and population codes, coding system, member checks and review processes that were then applied to interview 126 incarcerated young adult males between the ages of 18 and 22 in Arizona correctional facilities. The study produced not only an occupational achievement code, but also refined codes for interpersonal relationships requiring the addition of a spiritual/social code to account for church activities, religion, and spiritual groups, while narrowing the existing interpersonal relationships code to focus on family, children, a spouse or partner. Analysis demonstrated that incarcerated young adults create fewer identified strategies and have fewer aligned strategies to achieve post-release goals. Time served and expected sentences were determined to be significantly associated with the identification of goals, strategies, and development of aligned strategies. The impact of the different time horizon events of during and post incarceration were significant as well, participants identified five times as many goals one year from now in comparison to post-release, and on average 1.5 more strategies to achieve identified goals. The study demonstrated that the participants expected sentence was a significantly associated covariate to the number of Future Possible Selves’(FPS) defined, number of strategies defined to achieve those FPS goals, and number of aligned strategies to FPS goals across time horizons of 1 year and post release. Testing Whether Alternative Goals of Multifinal Means are Considered Helpful in Working Towards a Primary Dietary Goal in College Students

abstract: Multiple health-related benefits have been associated with adherence to plant-based diets, including vegan, vegetarian, and pescatarian dietary patterns. Despite a consistent body of evidence on the importance of healthy diets, Americans continue to find difficulty in establishing and adhering to dietary goals that could elicit long-term health benefits. Recent research suggests an important role for goal-setting strategies in health behavior change attempts, with some success shown in dietary behavior change, specifically. The current study thus aimed to explore whether having multiple goals alongside one primary goal of following a vegetarian, vegan, or pescatarian diet would increase the achievability of that goal. Participants of this study were broken into two groups: currently following a plant-based diet (ADHERE) and striving to follow a plant-based diet (STRIVE). Researchers hypothesized that the number of health and/or diet related alternative goals set by participants would differ between the two groups, that the ADHERE group would report that their alternative goals were more helpful and less interfering in achieving their dietary goal than the STRIVE group, and that a higher rank of importance of the dietary goal would predict being in the ADHERE group. Results showed that the number of health and/or diet related alternative goals did not differ between groups. The ADHERE group and STRIVE group did not have significantly different helpfulness and interference reports. Although, in an exploratory analysis, it was shown that those participants who reported at least 2 health/diet related alternative goals found those goals to be significantly more helpful than those who reported 0 or 1 health/diet goal. Results showed that rank of dietary goal did not predict group assignment. Overall, the results from this study showed that the type of alternative goal was very important when pursuit of multiple goals was in effect. The aim of this study was to examine how the two largest sports tabloids, Aftonbladet and Expressen, in Sweden represented the male and female Swedish athletes during the Olympic games in Pyeongchang 2018. Furthermore, the study examined how many male and female journalists covered the sport sections during the event and whether or not there were any differences between the tabloids. With agenda setting theory and hegemonic masculinity as our theoretical framework we conducted a quantitative content analysis on every news article revolving Swedish Olympic athletes during the Olympic period. The results showed no significant difference between male and female athletes regarding the amount of coverage in the tabloids. However, other findings were made and showed that female athletes more often got associated with their families and partners while male athletes were associated with their coaches. The study also showed that the majority of journalists reporting from the Olympic games were male.

Milk: From a healthy drink to aclimate threat and back again. The media story : A text analysis of how Swedish newspapers have framed aspects of milk between 2012 and 2017

Media is a powerful agent within society, that works as a gatekeeper for the media arena and therefore decides what topics are discussed and which are not (Pinto et al., 2016; Anderson, 2009) By setting the agenda and by framing messages, media can influence the public perception and therefor also the culture within society. However, it is said that media will reflect the most dominant culture within society, it is therefore of interest to see how the Swedish media portray and frame Swedish milk and the milk industry, that on hand is argued to be a threat towards the climate but on the other hand has a long history of being connected to the Swedish culture. When studying the four most read newspapers in Sweden published between 2012 and 2017 we found that even if the Swedish milk and the milk industry was questioned for being unhealthy or argued for being an environmental threat, the positive framing of milk was always more visible. Environmental impacts of milk and the milk industry was repeatedly downsized and even spoken for in opposite direction, that the Swedish milk and the Swedish milk industry is good for the environment. This would suggest that the strong position of milk within the Swedish society have the ability to undermine any threat, it is therefor of interest to further study the culture aspects of media framing not at least connected to products that is in many studies argued for being an environmental threat. Due to impact media may have on the society, how media frame and portray sustainability and sustainable products is of high interest for further research. More research is also needed to understand who gets to use the media arena to convey their messages, not at least within the topic of sustainability.

En viktig del i operationssjuksköterskors arbete är att genom lämpliga omvårdnadsåtgärder förhindra att patienter drabbas av postoperativa sårinfektioner. Operationssjuksköterskan är ansvarig för att operationsområdet är sterilt tills operationen är avslutad. Detta innebär bland annat att hudrengöring efter hudförslutning av operationssåret och steril förbandsläggning görs på ett korrekt sätt. Syfte: Syftet med studien var att beskriva operationssjuksköterskornas erfarenheter av hudrengöring efter hudförslutning av operationssåret samt förbandsläggning. Metod: En kvalitativ intervjumetod med semistrukturerade frågor användes. Sex operationssjuksköterskor deltog i studien. Kvalitativ innehållsanalys användes för att analysera intervjuerna som varade mellan 30 - 40 minuter. Resultat: Resultaten visade ett tema ”Stolt yrkeskunskap men begränsande faktorer gällande fortsatt förbättring av arbetsmetoder” samt tre kategorier och sju subkategorier. Konklusion: Rengöring av ett avslutet operationssår utförs i nuläge enligt beprövad arbetserfarenhet och rutiner men det krävs en ny evidensbaserad kunskap tillämpas i det dagliga arbetet för att bedriva en evidensbaserad omvårdnad. Även tydliga riktlinjer för uppföljningen av postoperativa sårinfektioner skall implementeras i det dagligen utkommande arbete. / Introduction: An important part of the occupational nurse's work is to prevent patients from postoperative wound infections through appropriate care measures. The operation nurse is responsible for the operation area being sterile until the operation is complete and that the cleaning and reorganization of the completed operating wound with a sterile dressing is correct and aseptic, which prevents postoperative wound infections. Purpose: The purpose of the study was to describe the occupational nurse's experiences of skin cleansing after skin closure and dressings. Method: A qualitative interview method with semistructured questions was used. Six occupational nurses participated in the study. Qualitative content analysis was used to analyze interviews ranging between 30-40 minutes. Result: The results showed a theme "Proof of professionalism but limiting factors for further improvement of working methods" as well as three categories and seven subcategories. Conclusion: Cleaning a completed surgery wound is performed in the current state according to proven work experience and routines, but it is required that a new evidence-based knowledge is applied in the daily work of carrying out evidence-based nursing. Clear guidelines for the follow-up of postoperative wound infections should also be implemented in the daily work.

To give support to the research, we used two daily newspapers from Paraná: Jornal de Beltrão, from Francisco Beltrão and Diário do Povo, from Pato Branco, and two weekly newspapers, O Palmense and Correio Regional, both from Palmas. The theoretical empirical model adopted is the agenda setting, a hypotheses that holds that media participates in the construction and definition of people s choices. We have focused on this method the agenda of the media, aiming at noticing, though it, the elaboration of the micro social agrarian agenda. The procedures include, besides library research, the quantitative content analysis of these newspapers, and some interviews with editors and leaders in the area. The research presents some relevant topics as well. The agrarian theme involves, above all, the historical perception of the importance of the land and of the rural sector in the political-economical tradition of the country, as well as the fact that it is a permanent social problem. Considering a political perspective, the year of 2003 marks the beginning of a new government, with President Lula, whose proposals indicated sensitive changes in the policies for the land sector. Finally, in the academic-empirical dimension, we found that there is not enough study on agenda setting in this region, specially those related to the agrarian theme, even though this is a geographic area marked by historical land conflicts. In general, we have observed that the local newspapers are legitimating and act in mutual level, producing and exaggerating versions that are set by interest groups, which quite often reverberate in the press. Through the analysis of the graphs and of specific books, we noticed that the history of agents and media professionals; the class appeals that mobilize the actors and the level of dependence in the relations of native press with official government influence the local agenda. We have also identified that this determination occurs substantially through the editorial process, be it under the connivance of political power, or through the prerogative of the financial power. We have also noticed the relations between the mobility of the actors and the visibility of the agrarian theme in the micro regional press, through fields like tradition, family, status and publicity. / Analisamos nessa pesquisa o problema agrário na imprensa do sudoeste do Paraná no ano de 2003. Para tanto, utilizamos como suporte quatro jornais paranaenses: os diários Jornal de Beltrão, de Francisco Beltrão e Diário do Povo, de Pato Branco, e os semanários O Palmense e Correio Regional, ambos de Palmas. O modelo teórico-empírico adotado é a agenda-setting, hipótese que sustenta que a mídia tem participação na construção e definição das escolhas públicas. Enfocamos nesse método a agenda da mídia, visando perceber através dela a formação da pauta agrária micro-social. Como procedimentos adotamos, além da pesquisa bibliográfica, a análise de conteúdo qualitativa desses jornais no recorte considerado e algumas entrevistas com os editores e lideranças vinculadas ao assunto. A pesquisa se reveste também de algumas relevâncias. A opção pela temática agrária nesse estudo envolve, antes de tudo, a percepção histórica sobre a importância da terra e do setor rural na tradição político-econômica do País, bem como a sua permanência como problema social. Já em uma perspectiva política, ponderamos que o referido ano marca o início do Governo Lula, cujas propostas indicava mudanças sensíveis nas políticas para o setor fundiário. Finalmente, na dimensão acadêmico-empírica verificamos uma carência de estudos sobre agenda setting nessa Região, particularmente aqueles relacionados à temática agrária, ainda que esse seja um eixo geográfico marcado por conflitos territoriais históricos. Em linhas gerais, observamos que os jornais locais são instâncias de legitimação que atuam em nível mútuo, produzindo e extrapolando versões agendadas por grupos de interesses, que não raro repercutem na grande imprensa. Pela análise dos gráficos à luz da bibliografia circunscrita, verificamos que o histórico dos agentes e profissionais da mídia; as instâncias de classe que mobilizam os atores e o nível de dependência nas relações da imprensa nativa com o poder público oficial influenciam na agenda local. Identificamos também que essa determinação ocorre substancialmente através do processo editorial, seja sob a conveniência do poder político, seja pela prerrogativa do poder financeiro. Percebemos ainda relações entre a mobilidade dos atores e a visibilidade da temática agrária na imprensa micro-regional, através de esferas como o patriotismo, a tradição, a família, o status e a publicidade.
