Remédier à la violence structurelle vécue par le personnel infirmier dans le système de santé au Québec : une éthique organisationnelle basée sur le care pour des conditions de travail plus justes

La pandémie de COVID-19 a porté à notre attention de nombreux problèmes dans notre système de santé, dont les mauvaises conditions de travail des infirmières. Ce problème persiste pourtant depuis des dizaines d’années. Le but de ce travail est d'examiner la nature et la persistance des mauvaises conditions de travail du personnel infirmier à travers une analyse éthique critique de la violence. Il met en lumière la violence structurelle qui chapeaute les mauvaises conditions de travail du personnel infirmier et les redéfinit comme le produit d’injustices sociales plus profondes. Le concept de violence structurelle et l’éthique du care nous amènent à adopter différentes perspectives pour expliquer l’origine des injustices et proposer des pistes de solutions durables à ce problème qui persiste. Ces perspectives soulèvent des enjeux éthiques d’inégalités sociales, de responsabilité et de justice qui affectent le quotidien des infirmières dans un contexte organisationnel particulier. Notre analyse sur la violence structurelle révèle de nombreuses injustices construites sur l’histoire des infirmières. Nous retraçons trois événements majeurs qui ont transformé les pratiques soignantes et contribué à réduire les infirmières à de multiples hiérarchies sociales et inégalités. Les rôles de l’institutionnalisation des soins par l’Église, de la médicalisation et de l’étatisation sont soulevés. Les injustices liées au genre féminin, au travail du soin et à l’instrumentalisation des soignantes sont d’abord abordées. Également, dans notre analyse de la violence organisationnelle dans les institutions de soin actuelles, ce sont des injustices liées aux nouvelles normes de soins et pratiques de gestion qui émergent pour expliquer les contraintes et la déshumanisation des soins chez le personnel infirmier. Tout au long du travail, les enjeux d’inégalités de pouvoir et de responsabilité sont soulignés. Ainsi, nous proposons que les organisations de santé actuelles entament une transition vers un système de santé apprenant axé sur la responsabilité partagée et la justice sociale. Nous proposons cinq recommandations inspirées du cadre éthique pour un système de santé apprenant qui permettraient de transformer des milieux de soin violents en milieux davantage sains et justes. Nous concluons ce travail avec une discussion ouverte sur la responsabilité partagée à l’égard d’autres enjeux en santé. / The COVID-19 pandemic has brought to our attention many problems in our healthcare system including nurse’s poor working conditions. Perhaps, this problem has persisted for decades. The purpose of this work is to investigate the nature and persistence of nurse’s poor working conditions through a critical ethical analysis of violence. It highlights the structural violence that underlies nurse’s poor working conditions and redefines them as the product of deeper social injustices. The concept of structural violence and the ethics of care lead us to adopt different perspectives to explain the origin of injustices and to propose sustainable solutions to this persistent problem. These perspectives raise ethical issues of social inequities, accountability and justice that affect the daily lives of nurses in a particular organizational context. Our analysis of structural violence reveals many injustices built on nurse’s history. We trace three major events that transformed nursing practices and contributed to the reduction of nurses to multiple social hierarchies and inequalities. The roles of the institutionalization of care by the Church, medicalization and the power of the state are raised. The injustices related to female gender, care work and the instrumentalization of caregivers are first discussed. Also, in our analysis of organizational violence in today's health care institutions, injustices related to new standards of care and management practices are highlighted to explain the constraints and dehumanization of care in nurse’s practice. Throughout the analysis, issues of power and responsibility inequalities are highlighted. Thus, we propose that current healthcare organizations initiate a transition toward a learning healthcare system focused on shared responsibility and social justice. We propose five recommendations inspired by the ethical framework for a learning health system that would transform violent health care environments into more healthy and just ones. We conclude this work with an open discussion of shared responsibility for other health issues.

There is an ongoing theme within the field of peace conceptualisation to create and to add to the meaning of peace. This has caused a fragmented field with over 130 conceptualisations. As we move forward, it isthus necessary to review whether the methods utilised to measure peace are suitable in the ever changing contexts of today. Studies investigating levels of peace or types of peace are as important now as theywere before and as they will be in the future. As the world becomes more interconnected and complex in its actions, and as more conflicts emerge, it is important that those involved, both directly and indirectly, understand what to strive for and how far away from that goal they are. Thus, it is extremely important to examine to what extent the measuring systems of peace and adjacent topics today reflect peace. This study examines current methods of measuring peace and adjacent topics through the lens of Johan Galtung’s violence triangle and his positive and negative peace dichotomy. Through a qualitative approach, this study aims to review whether how peace is measured today applies to a broader framework of peace and, if not, what changes could be made to enhance the portrayal of peace in today’s society. In this research, five measuring systems were examined, evaluated and discussed (the Global Peace Index, the Positive Peace Index, the Freedom in the World Index, the Human Rights Index and the Human Freedom Index). They were all found to be lacking depth and width. This was especially apparent within the peace-focused indices, which, when looked at together with the rest of the systems, does not hinder a larger understanding of peace, but separately it does. All in all, the two indices focused on freedom seem to be the most peace-related based on the framework employed. These findings showcase the need to re-evaluate how peace is measured to ensure that what is perceived as peace and peacefulness accurately portray both.

Producing information today is unprecedented in both speed and accessibility. This is a benefit of living in these IT times. There is more knowledge available than ever before, which is as fantastic as it is problematic. It leaves both the producer and the user of this information responsible for assessing and interpreting it. This thesis has investigated what information has been produced on conflict related SGBV by the World Bank and the ICRC to see what representations have been established. Several documents from each organization have been collected, coded and thematically analyzed by using intersectionality and structural violence as theoretical lenses. These theoretical frameworks complemented each other in their use in this study as intersectionality was employed to look at what certain portrayals might mean for individuals, and structural violence was used to look at what the result meant on a larger scale. Ultimately, this thesis arrived at the conclusion that the portrayal of conflict related SGBV by the World Bank and ICRC is problematic. No organization is misrepresenting more than the other, but they do struggle on different themes. Overall, the main risk an organization runs when writing about this topic is to portray women as the only demographic group affected and the image that all women survivors are the same in that they are female. This leaves the consumer of this information with the assumption that conflict related SGBV only affects women, because they are women. This is wrong and it is problematic, as this thesis will explain in detail, along with other representations and analytical conclusions.

Peace education in post-conflict societies : the case of the Young Peace Ambassador Program in Somalia and Kenya

Contributing to a fairly new discipline in the region, this study investigates the objectives, contents, design, approaches, strategies and methodologies involved in a Peace Education initiative called The Young Peace Ambassador Program (TYPAP), which is being implemented in East Africa and the Horn of Africa. The aim of the study is to outline the nature, causes and consequences of conflict and violence in northern Kenya and Somalia by looking at the way in which peace education can help build a culture of peace in northern Kenya and Somalia. Accordingly, the consistency of TYPAP with peace education principles, its impact according to interviewees and how far it has met its own objectives were assessed. Thus, using a qualitative case study methodology employing content analysis, interviews and observations, this dissertation shows that TYPAP a multifaceted peace education initiative working with local partners – has potential not just for creating awareness of peace issues, but also for cultivating the seeds of a culture of peace. Following Galtung’s theories, the dissertation also indicates that it is not just “structural violence”, but also the deeper symptoms of “cultural violence”, that we may need to address in taking the region forward in the coming years. / Educational Studies / M. The integration of modern science and indigenous knowledge systems in the science education curriculum for South African schools represents social justice for the majority of South Africans as they determine the knowledge necessary for themselves and for future generations in the new South Africa. An exploratory research reveals tension and a dichotomous relationship between modern science and IKS, caused by false hierarchies that are influenced by factors such as colonialism, capitalism and modernisation to the exclusion of the core values held by indigenous people in their relationship with nature. The thesis demonstrates that the integration requires an epistemology that puts humanity first and a framework that accommodates both ways of knowing. Contextualizing food practices and change among Mexican migrants in West Queens, New York City

This thesis is about food practices and change among Mexican migrants living in West Queens, New York City. Public health research suggests that Mexican migration to the US has a negative impact on food practices, with diets being less nutritious over a migrant’s stay in the United States and obesity being more common among longer-term than more recently-arrived individuals. Through ethnography, I explore how migration shapes food practices and examine the nuanced process of nutritional change that is often obscured in large-scale epidemiological studies. Food practices are important not just because they shape vulnerabilities to chronic diseases but also because they serve as prisms by which to examine migrants’ lives, pressures and aspirations. The three aims of this ethnography are to explore the food practices that Mexicans engage in after migration; to examine the social, temporal and political-economic contexts shaping food practices and change; and to describe how migrants themselves makes sense of nutritional change. I explore these themes using the approach of structural vulnerability, which views health practices and outcomes as influenced by social structures, relationships and inequalities. In so doing, I provide a critique of the public health literature’s use of the concept of acculturation to explain food practices, which largely obscures the role played by structural contexts and constraints. Through participant observation, conversations and interviews with Mexican migrants in West Queens, NYC, I have identified three contexts shaping food practices and change after migration: household dynamics and labour division; time constraints and work schedules; and the ‘food environment’, referring to the availability of food items and weight loss products. Gender dynamics, documentation status and class modified the way in which these contexts were perceived and negotiated by informants, which had further consequences on food practices. In these settings, informants were often encouraged to consume high-energy foods and large portions, to replace meals with snacks, to eat prepared or convenience foods, and to experiment with weight loss products. To rationalize nutritional change and body size disparities, informants employed multiple discourses. Some discourses emphasized the role of structural contexts and constraints related to time, money and documentation status, while others emphasized the role played by cultural beliefs, habits and acculturation. An ethnographic approach informed by structural vulnerability serves to articulate how the everyday lives and social contexts in which Mexican migrants are embedded, shape experiences of nutritional change. This thesis exposes a disconnect between the way in which the public health literature conceptualizes nutritional change and how it is lived ‘on the ground’.

Extractive Violence on Indigenous Country : sami and Aboriginal Views on Conflicts and Power Relations with Extractive Industries / Extraktivt våld på urfolks marker : konflikter och maktrelationer mellan utvinningsindustrier och urfolk i Sverige och Australien

Asymmetrical conflicts and power relations between extractive industries and Indigenous groups often have devastating consequences for Indigenous peoples. Many Indigenous groups are struggling to maintain their lands as Indigenous perspectives on connection to Country are frequently undervalued or dismissed in favour of extractivist ideologies. While this conflicted interface has been researched in various parts of the world, studies exploring conflicts and power relations with extractive industries from Indigenous perspectives are few. This thesis is an international comparison aiming to illuminate situations of conflict and asymmetrical power relations caused by extractivism on Indigenous lands from new viewpoints. By drawing on two single case studies, the situations for Laevas reindeer herding Sami community in northern Sweden and Adnyamathanha Traditional Owners in South Australia are compared and contrasted. Yarning (a form of interviewing) is used as a method for data collection and in order to stay as true as possible to the research participants’ own words a number of direct quotes are used. The analysis employs peace researcher Johan Galtung’s concepts of cultural and structural violence as analytical tools to further explore the participants’ experiences of interactions with extractive industries and industrial proponents, including governments. In addition, the thesis introduces the concept of extractive violence as a complement to Galtung’s model. Extractive violence is defined as a form of direct violence against people and/or animals and nature caused by extractivism, which predominantly impacts peoples closely connected to land. The concepts of structural and cultural violence are understood as unjust societal structures and racist and discriminating attitudes respectively. A number of main themes could be identified in the research participants’ narratives. However, the most prominent on both continents was connections to Country and the threat that extractive violence posed to these connections. The results show that although the expressions of cultural, structural and extractive violence experienced by the two Indigenous communities varied, the impacts were strikingly similar. Both communities identified extractive violence, supported by structural and cultural violence, as threats to the continuation of their societies and entire cultures. Furthermore, the results suggest that in order to address violence against Indigenous peoples and achieve conflict transformation, Indigenous and decolonising perspectives should be heard and taken into account. / Konflikter och maktrelationer mellan utvinningsindustrier och urfolksgrupper får ofta förödande konsekvenser för urfolken. På grund av assymetriska maktförhållanden mellan urfolk och majoritetssamhällen som råder på de flesta ställen i världen utsätts många urfolk systematiskt för rättighetskränkningar. Många urfolksgrupper kämpar idag för att bevara sina marker eftersom urfolks perspektiv och kopplingar till marken ofta förminskas eller ignoreras när de står i motsättning till extraktiva ideologier. Även om extraktivism och påverkan på urfolk och urfolksgrupper varit fokus för tidigare studier saknas forskning som utgår från urfolkens perspektiv. Denna avhandling är en internationell jämförelse med syfte att, från nya synvinklar, belysa konfliktsituationer och asymmetriska maktrelationer som orsakats av extraktivism på urfolks marker. Avhandlingen jämför och kontrasterar två fallstudier som utförts med Laevas č earru (sameby) i norra Sverige och Adnyamathanha-folket i delstaten South Australia. I fallstudien som utförts tillsammans med Laevas č earru ingår en grupp av totalt sex forskningsdeltagare, fyra män och två kvinnor. Det var dock framför allt två forskningsdeltagare som intervjuades med anledning av den konfliktsituation mellan Laevas č earru och gruvbolaget LKAB, som står i fokus för artikel I i avhandlingen. I den australiska fallstudien, som utförts tillsammans med Adnyamathanha-folket, ingår en grupp av sju forskningsdeltagare bestående av fyra kvinnor och tre män. Denna studie, artikel II, behandlar Adnyamathanhafolkets kamp mot de australiska och sydaustraliska regeringarnas förslag om att inrätta kärnavfallsdepåer på Adnyamathanhas marker. För att inhämta material användes yarning (en typ av intervjumetod) och för att återge forskningsdeltagarnas ord så rättvisande möjligt inkluderades ett antal direktcitat i texterna. För att möjliggöra en mer djupgående analys av forskningsdeltagarnas upplevelser av konflikter med utvinningsindustrier och förespråkare för extraktivism, inklusive regeringar och stater, användes Johan Galtungs modell, känd som Galtungs våldstriangel, som analysverktyg. Galtungs modell innefattar strukturellt, kulturellt och direkt våld. Direkt våld definieras som fysiskt våld eller hot om fysiskt våld, strukturellt våld utgörs av orättvisa och diskriminerande samhällsstrukturer och kulturellt våld är de attityder som får det strukturella och således även det direkta våldet att te sig legitimt. Föreliggande avhandling introducerar även konceptet extraktivt våld som ett komplement till Galtungs modell där xvi det ersätter direkt våld. Jag definierar extraktivt våld som en typ av direkt våld mot människor och/eller djur och natur orsakat av extraktivism som framför allt påverkar människor med starka kopplingar till sina marker. Extraktivism förstås här som alla typer av aktiviteter som extraherar stora mängder av resurser från marker och människor, exempelvis gruvdrift, skogsbruk, fiske, lantbruk och turism. I forskningsdeltagarnas utsagor identifierades ett antal nyckelteman. Dessa teman uppvisade både likheter och skillnader beroende på deltagarnas olika situationer och förutsättningar. Det mest framträdande temat på båda kontinenterna var dock ”connection to Country” eller kopplingar till marken. Båda grupperna beskrev hur marken och deras förhållande till den innefattade historia, kunskap, traditioner och kultur. För Adnyamathanhagruppen var det mest centrala att rädda och bevara heliga platser som hotas av extraktivism och för Laevas č earru sågs renskötseln och bevarandet av markerna för renarnas skull som det mest väsentliga. Avhandlingens resultat visar att även om de former av kulturellt, strukturellt och extraktivt våld som forskningsdeltagarna upplevde varierade, var effekterna av våldet slående lika. Båda grupperna identifierade extraktivt våld, understött av strukturellt och kulturellt våld, som hot mot fortlevnaden av deras samhällen och kulturer. Resultaten pekar även på vikten av att urfolkens perspektiv inkluderas och blir hörda om konflikttransformering mellan utvinningsindustrier och urfolk ska kunna uppnås.

For many years Canada has quietly rationalized importing temporary “low-skilled” migrant labour through managed migration programs to appease industries desiring cheap and flexible labour while avoiding extending citizenship rights to the workers. In an era of international human rights and global competitive markets, the Canadian Seasonal Agricultural Workers Program (SAWP) is often hailed as a “model” and “win-win” solution to migration and labour dilemmas, providing employers with a healthy, just-in-time labour force and workers with various protections such as local labour standards, health care, and compensation. Tracing migrant workers’ lives between Jamaica, Mexico and Canada (with a focus on Ontario’s Niagara Region), this thesis assesses how their structural vulnerability as non-citizens effectively excludes them from many of the rights and norms otherwise expected in Canada. It analyzes how these exclusions are rationalized as permanent “exceptions” to the normal legal, social and political order, and how these infringements affect workers’ lives, rights, and health. Employing critical medical anthropology, workers’ health concerns are used as a lens through which to understand and explore the deeper “pathologies of power” and moral contradictions which underlie this system. Particular areas of focus include workers’ occupational, sexual and reproductive, and mental and emotional health, as well as an assessment of their access to health care and compensation in Canada, Mexico and Jamaica. Working amidst perilous and demanding conditions, in communities where they remain socially and politically excluded, migrant workers in practice remain largely unprotected and their entitlements hard to secure, an enduring indictment of their exclusion from Canada’s “imagined community.” Yet the dynamics of this equation may be changing in light of the recent rise in social and political movements, in which citizenship and related rights have become subject to contestation and redefinition. In analyzing the various dynamics which underlie transnational migration, limit or extend migrants’ rights, and influence the health of migrants across borders, this thesis explores crucial relationships between these themes. Further work is needed to measure these ongoing changes, and to address the myriad health concerns of migrants as they live and work across national borders.

The Role of Social Work in Contemporary Colonial and Structurally Violent Processes: Speaking to Aboriginal Social Workers who had Child Welfare and/or Criminal Justice Involvement as Youth

As a relatively recent phenomenon, the increasing overrepresentation of Aboriginal persons in both the child welfare and criminal justice systems is of critical importance to the field of social work. As social control systems, how do social workers contribute to or mitigate against overrepresentation as contemporary colonialism? What can social work professionals who themselves have been through these systems add to our social work discourse? A sample of fifteen Aboriginal social workers who had as youth been in either one or both of these systems were interviewed with respect to: what they found was helpful or unhelpful in their interactions as youth with social workers, why they subsequently chose social work as a career, the supports and barriers they encountered along their career path, and the difference their experiences had for their own professional practice. Using structural social work theory, overrepresentation as a contemporary colonializing process was re-conceptualised as structural violence. Institutional Ethnography (IE) and Hermeneutic Phenomenology were used to explore how these neo-liberal ruling relations are produced, maintained, and potentially deconstructed. The findings from this unique population have implications for decolonizing social work practice, education, and research.
