Successful aging is increasingly topical issue given the rapidly aging population and it is therefore crucial to deal with the way the old people adapt to the coming age, and whether there is some way we can be prepared for it. Model coping with life's problems - selection, optimization and compensation is a lifelong adaptation strategy, which currently in old age leads to successful aging. Besides adaptation are important attitudes towards aging and old age, that have an impact on senior's self-esteem. The main goal of this thesis was to analyze attitudes towards age and the use of model coping with life problems in a sample of younger and older Czech seniors using questionnaires SOC and AAQ. The research had a qualitative aspect, which focused on mapping the topic of retirement planning and strategies for successful aging. It turned out that the younger and older seniors differ from each other only in the use of the strategy chosen selection, in other strategies there was no difference between age groups. In the area of attitudes to each other, age groups also did not differ. The results also show that between using SOC strategies and their own attitude to old age, there is no relationship. Seniors recommend to young people and to their peers to think of their health, maintain good relationships...

Le sommeil physiologique et pathologique du sujet âgé : impact sur la qualité du vieillissement et le vieillissement cognitif / Physiological and pathological sleep : impact on successful aging and cognitive function

La prévention du vieillissement pathologique est au coeur de l’actualité et comprendre les différents facteurs déterminants du vieillissement réussi est une priorité. La pathologie du sommeil de par sa fréquence, son retentissement sur la mortalité, de ses conséquences sur la survenue de comorbidités et sur la qualité de vie, doit être prise en compte pour promouvoir le bien vieillir. Nos travaux ont permis de démontrer l’importance d’un des aspects qualitatifs du sommeil : la plainte de sommeil. Nous avons pu montrer un lien entre plainte de sommeil et plainte de mémoire et ce finalement, quel que soit l’outil utilisé. L’ajustement sur les paramètres respiratoires du sommeil n’interférait en rien sur la relation décrite. La qualité subjective du sommeil jouait un rôle dans la perception qu’avait chaque individu de son état de santé ou de sa satisfaction de vie. Ces résultats permettent d’insister sur l’importance du management correct de la plainte de sommeil et du traitement adapté de celle-ci. Le syndrome d’apnée obstructive du sommeil (SAOS) est une entité clinique largement sous diagnostiquée chez le sujet âgé. Cependant, il a été largement démontré l’intérêt de sa prise en charge adaptée notamment en termes de bénéfices sur la mortalité cardiovasculaire. Nos travaux se sont intéressés de façons plus spécifiques au SAOS et à l’impact potentiel de son traitement par Pression Positive Continue (PPC) sur les capacités cognitives. Nous avons pu démontrer que les sujets porteurs d’un SAOS sévère retiraient un bénéfice à long terme de leur traitement par PPC sur leurs capacités mnésiques. En effet, les sujets traités maintenaient leurs performances mnésiques notamment en terme de mémoire épisodique et de fonctions exécutives dans le temps, en comparaison aux sujets non traités et ce sur le long terme. D’autres travaux sont en cours, notamment chez les sujets présentant une pathologie dégénérative démentielle, afin de mettre en évidence l’impact du dépistage et du traitement d’un SAOS sévère sur cette population / One of our public health priorities is at present the prevention of pathological aging and this implies the comprehension of the different determinants of successful aging. Sleep disorders because of its frequency, its impact on mortality, its consequences on the occurrence of comorbidities and on quality of life, must be taken into account to promote successful aging. Our research has demonstrated the importance of one aspect of sleep: the sleep complaint. We have shown a significant link between sleep complaints and memory complaints. The adjustment on the respiratory sleep parameters have not modified the relationship described. The subjective quality of sleep played a role in the individual's self perception of health status or self perception of life satisfaction. Thus, these results show the importance of the management of sleep complaint in the elderly population which must be adapted and correctly treated. Obstructive Sleep Apnea Syndrome (OSAS) is a clinical entity, frequently under diagnosed in the elderly population. However, the benefit of treatment has been widely demonstrated, especially in terms of cardiovascular mortality. Our studies have focused on the cognitive consequences of OSAS and the potential impact of its treatment by Continuous Positive Airway Pressure (CPAP) on cognitive abilities. We have shown that elderly subjects with severe OSAS could have a clinical benefit of long term treatment on their cognitive capacities, using CPAP therapy. Indeed, treated subjects could maintain their memory performances over time, particularly in terms of episodic memory and executive functioning, compared to untreated subjects. Further studies must be made to confirm these results. Subjects with dementia must be included to evaluate the benefit of the treatment of OSAS in this population

Efeitos da estimulação cognitiva em aspectos cognitivos e motores de idosas hígidas

Programa de Pós-Graduação em Neurociência e Cognição, 2014. / A estimulação cognitiva e o exercício físico têm conseguido destaque na prevenção, tratamento e reabilitação de doenças e também como instrumento para um envelhecimento bem sucedido. Há um consenso na literatura de que o exercício físico promove melhoras na cognição e inicia uma linha de pesquisa que investiga se a estimulação cognitiva também é capaz de promover benefícios na capacidade funcional. O objetivo do presente trabalho é avaliar os efeitos de um protocolo de estimulação cognitiva no equilíbrio dinâmico, agilidade e força de membros inferiores, na memória, na função executiva e em sintomas depressivos de idosas hígidas. O delineamento adotado foi um estudo longitudinal, controlado e randomizado. Participaram do trabalho 60 idosas com idade média de 70,65 + 5 anos e escolaridade de até quatro anos de ensino formal. As voluntárias foram divididas aleatoriamente entre os Grupos Controle (GC=30) e Grupo Experimental (GE=30). Todas foram submetidas ao protocolo de avaliação antes e após 12 semanas. Os testes utilizados foram: timed up and go, sentar e levantar, escala geriátrica de depressão, mini exame do estado mental, memória lógica da bateria WMS, aritmética e dígitos da bateria WAIS, blocos de Corsi, e o teste Wisconsin considerado padrão-ouro para função executiva. No momento pré-intervenção também foram aplicados os questionários social para coleta das variáveis intervenientes e Baecke para nível de atividade física. O programa de estimulação cognitiva era composto de 24 sessões de treinamento de memória, cálculo e função executiva. O tratamento estatístico foi realizado com auxílio do programa SPSS e o nível de significância adotado foi de 5%. As análises de correlação e do momento inicial foram realizadas com estatística paramétrica, com o teste de correlação de Pearson e para comparação entre os grupos pré-intervenção o teste t de Student. A verificação do efeito da estimulação cognitiva foi obtida com o cálculo do delta, utilizando o teste não paramétrico U de Mann-Whitney. O tamanho do efeito foi calculado utilizando o escore-z. Os resultados obtidos mostraram efeito da intervenção em todas as variáveis dependentes, e que a agilidade motora está mais fortemente correlacionada com escolaridade do que com força de membros inferiores. Conclui-se que a estimulação cognitiva foi efetiva para promover melhora cognitiva e da aptidão física, promovendo qualidade de vida de idosas hígidas. / The cognitive stimulation and physical exercise have achieved prominence in the prevention, treatment and rehabilitation of diseases and also as a means of successful aging. There is a consensus in the literature that physical exercise promotes improvements in cognition and starts a line of research literature that investigates whether cognitive stimulation is also able to promote benefits in functional capacity. The purpose of this study is to evaluate the effects of a cognitive stimulation protocol in memory, executive function, depressive symptoms, agility and lower extremity strength of healthy elderly women. The design was a longitudinal study, controlled and randomized. Participated in this study 60 elderly women with a mean age of 70.65 + 5 years, and up to 4 years of formal education. The volunteers were divided randomly between the control groups (CG = 30) and Experimental Group (EG = 30). Both groups were subjected to the evaluation protocol before and after 12 weeks. The tests used were: timed up and go, chair stand, geriatric depression scale, mini mental state examination, battery WMS logical memory, arithmetic and digit battery WAIS, Corsi blocks, and Wisconsin considered the gold standard test for executive function. The cognitive stimulation program consisted of 24 training sessions memory, calculation and executive function. Statistical analyses were performed with SPSS and significance level was 5%. Correlation analysis and the initial moment were performed with parametric statistics, the Pearson correlation test and the comparison between the groups in the pre-intervention moment was accomplished by using the Student t test. The verification of the effect of cognitive stimulation was obtained by calculating the delta using the nonparametric Mann-Whitney test. The effect size was calculated using the z-score. The results showed intervention effect on all dependent variables, and motor agility was more strongly correlated with education than lower extremity strength. It is concluded that cognitive stimulation was effective to promote cognitive improvement and physical fitness, promoting quality of life in otherwise healthy elderly.

The quantitative and qualitative methods were used for analysis of the results. The general objective of the study was to understand the perception of the elderly on the aging process and the experience of spirituality in this phase of life. This research was conducted with a sample of 100 elderly people, and average age of 74.8 years. Data collection was conducted in two institutions that assist the elderly in the city of São Paulo and metropolitan region of São Paulo. The instruments used in data collection were: Mini-Mental State Exam (MMSE), translated and adapted by Bertolucci et al. (1994), Semi-Structured Interview, developed by the researcher; Spiritual Well-Being Scale (SWB Scale), Paloutzian & Ellison (1982), translated from English to Portuguese by Mello (2010). The general results of the narratives of the participants indicate that the meaning of old age is linked to the social support of the family, health and physical vigour, participation in group activities, autonomy to new learning, life quality and spirituality, experience / maturity / wisdom. The sense of spirituality in old age was linked to close contact with God / prayer, harmony with the universe, faith, love of neighbor, coping capacity and old age as a time to live fully spirituality. Regarding to distribution of Spiritual Well-being Scale results, 51% of the sample has a moderate sense of spiritual well-being and 49% high sense of spiritual well-being. We conclude this research considering for most of the elderly, aging is characterized as a propitious moment to experience spirituality, an essential element in the individuation process, consciousness expansion, self-transcendence, encounter with the oneself / Esta Tese de Doutorado é resultado da pesquisa "Velhice e Espiritualidade na Perspectiva da Psicologia Analítica" realizada no Núcleo de Psicossomática e Psicologia Hospitalar da PUC-SP. Foi utilizado os métodos quantitativo e qualitativo na análise dos resultados. O objetivo geral do estudo foi compreender a percepção do idoso acerca do processo de envelhecimento e da vivência da espiritualidade nessa fase da vida. Esta pesquisa foi realizada com uma amostra de 100 idosos, a média de idade é de 74,8 anos. A coleta dos dados foi realizada em duas instituições que prestam assistência à população idosa na cidade de São Paulo e na Grande São Paulo. Os instrumentos utilizados na coleta de dados foram: Mini-Exame do Estado Mental (MEEM), traduzido e adaptado por Bertolucci e cols. (1994); Entrevista Semiestruturada, elaborada pela pesquisadora; Escala de Bem-Estar Espiritual (SWB Scale), de Ellison & Paloutzian (1982), traduzida da língua inglesa para língua portuguesa por Mello (2010). Os resultados gerais das narrativas dos participantes indicam que o sentido da velhice está relacionado ao suporte social do núcleo familiar, saúde e vigor físico, participação em atividades grupais, autonomia para novas aprendizagens, qualidade de vida e espiritualidade, experiência/maturidade/sabedoria. Já o sentido da espiritualidade na velhice foi associado ao contato íntimo com Deus/oração, sintonia com o universo, fé, amor ao próximo, capacidade para os enfrentamentos e velhice como momento de viver plenamente a espiritualidade. Quanto à distribuição dos resultados da Escala de Bem-Estar Espiritual, 51% da amostra tem senso de bem-estar espiritual alto e 49% senso de bem-estar espiritual moderado. Concluímos esta pesquisa considerando que para grande maioria dos idosos, a velhice é caracterizada como momento propício para vivenciar a espiritualidade, elemento essencial no processo de individuação, ampliação de consciência, auto-transcendência, encontro do si-mesmo

Intergenerational Caregiver Loss and Successful Aging Opportunity Disruption of Bulgarian Elderly

Identified by European research as an aging crisis, vulnerabilities in the rural Bulgarian elderly population such as intergenerational caregiver loss, are sociohistorical consequences and barriers to successful aging. This transcultural study investigated the aging crisis in rural Bulgaria to create a contextual definition of successful aging and to address successful aging and the identification of challenges and barriers faced by social workers and elders to support successful aging. Guided by continuity theory in aging and contextualized by overarching ecological systems theory, the research questions examined Bulgarian social workers' perceptions and expectations of successful aging and barriers to it. A collaborative action research design was commissioned with a purposeful sample of an 8-member participant group drawn from multileveled Bulgarian social workers affiliated with the rural elderly population. Data were distilled by a thematic analysis into perceptions, expectations, and barriers of successful aging, which were validated by triangulation through translation and repetitive member checking processes. The findings revealed no universal threshold of successful aging achievement, but because expectations were unmet due to systemic barriers successful aging is not occurring in Bulgaria. The study results might bring about positive change by unifying Bulgarian multilevel social work practice to restore successful aging opportunity for the rural Bulgarian population by actions that systemically respond to the identified barriers.

Striving for personal goals is one important aspect of composing one is life within a developmental context. In this realm, personal life investment (PLI) measures the amount of energy (action and thought) that people report investing in central life domains (e.g., health, family, leisure, independence). This study aimed at understanding the functional relations of PLI, its development between 70 and over 100 years, and its role for successful aging. Obligatory and optional PLI were distinguished to differentiate between life domains where older individuals need to invest energy to maintain a basis for their development and domains that allow for many more degrees of freedom as to where and when to invest. Cross-sectional and longitudinal data from the Berlin Aging Study (BASE) showed that optional PLI, in contrast to obligatory PLI, had only positive motivational and affective correlates, declined between age 80 and 90, and contributed to successful aging as long as older people had enough resources to invest in optional domains. / Das Streben nach persönlichen Zielen ist für die eigene Lebensgestaltung innerhalb eines veränderlichen Entwicklungskontextes zentral. Dabei erfasst das Konstrukt des persönlichen Lebensinvestments (PLI) einen Aspekt der Lebensgestaltung: das Ausmaß an Energie, das in Form von Handlungen und Gedanken in zentralen Lebensbereichen, wie z.B. Gesundheit, Familie, Freizeit oder Unabhängigkeit, investiert wird. Ziel der vorliegenden Studie war die Untersuchung der funktionalen Zusammenhänge des PLI, der Entwicklung von PLI zwischen 70 und über 100 Jahren und der Rolle des Investments beim erfolgreichen Altern. Hierbei wurde zwischen obligatorischem und optionalem PLI unterschieden, also PLI in Lebensbereichen, die im Alter Investment erfordern, um die Grundlagen für die eigene Entwicklung zu erhalten, und PLI in Lebensbereichen, die weitaus mehr Freiheitsgrade hinsichtlich des Ausmaßes und Zeitpunktes des Investments bieten. Analysen von Quer- und Längsschnittdaten der Berliner Altersstudie (BASE) zeigten, dass optionales PLI, im Gegensatz zu obligatorischem PLI, nur positive motivational-affektive Korrelate aufweist, zwischen 80 und 90 Jahren leicht reduziert wird und zum erfolgreichen Altern beitragen kann, sofern die betreffende ältere Person über ausreichende Ressourcen verfügt, um sie in optionalen Bereichen zu investieren.

Occupation-focused and occupation-based interventions for community-dwelling older people : Intervention effects in relation to facets of occupational engagement and cost effectiveness

Background  Occupation-focused and occupation-based interventions can potentially promote occupational engagement among community-dwelling older people, but there is limited evidence to identify the most effective and cost-effective interventions. For independent-living older people, there is a lack of evidence to determine if occupation-focused and occupation-based interventions have an effect on their occupational engagement. For older people who need assistance because of bathing disabilities, there is limited evidence of the effects of occupation-focused and occupation-based interventions on their occupational engagement or for reducing or omitting their need for assistance. Finally, there is limited evidence to determine if occupation-focused and occupation-based interventions implemented for community-dwelling older people are cost effective. Aim The aim of this thesis was to evaluate the effects and cost effectiveness of occupation-focused and occupation-based interventions for two groups of community-dwelling older people, independent-living, community-dwelling older people and older people with bathing disabilities. Method Studies I and II were based on an exploratory randomized controlled trial. One hundred and seventy seven persons, 77–82 years, single living, and without need for home help were randomized to a no-intervention control group or to one of three occupational therapy interventions focused on promoting occupational engagement: an individual intervention, an activity group or a discussion group. In study I, effect sizes for leisure engagement and ability to perform activities of daily living (ADL) tasks were estimated for each intervention in relation to the control group to identify the most effective intervention at 3 and 12 months after baseline. In study II, the effects on quality adjusted life years (QALYs) and the total costs for the intervention, social services provided by the municipality and health care were used evaluate cost-effectiveness. Study III was a quasi-experimental clinical trial and included 95 persons, 65+, who had applied for municipality-based home help with bathing. For participants in the intervention group, occupational therapists implemented occupation-focused and occupation-based interventions. No occupational therapy intervention was implemented for those in the control group, but they were allocated home help services if judged to need it based on an assessment by a municipality care manager. Evaluations of ADL ability, self-rated health and allocated home help were implemented at baseline and after 15 weeks. Study IV involved the use of decision-modeling based on a five state Markov model that included levels of dependency in ADLs, place of residency and death. Probabilities for transitions between states in the model, QoL scores and societal costs for each state were derived from previous research. Overall, the model was based on research indicating that more severe levels of dependency reduced QALY scores and increased societal costs. Previous trials have provided evidence that an occupation-focused and occupation-based intervention implemented to reduce bathing disabilities increased the probability of independence of home help. The Markov model was used to evaluate cost-effectiveness over 8 years for an intervention compared to no intervention. Results The results of study I indicated that each intervention had a small positive effect on minimizing a decline in leisure engagement and/or ADL, but no intervention was clearly superior. In study II, the results indicated that the interventions delivered in a group format positively affected self-rated health. The discussion group was the most cost-effective intervention. The results of study III indicated that the intervention had no effect on ADL ability or self-rated health. There was, however, a large difference in the allocation of home help at follow up, indicating that the intervention was effective in reducing dependency on home help for bathing. The results of study IV indicated that compared to no intervention, the intervention resulted in a positive accumulation of QALYs and lower costs for every year during the entire 8 year period. Conclusion This thesis provides evidence to support the implementation of occupation-focused and occupation-based interventions for independent-living, community-dwelling older people in order to reduce their decline in occupational engagement and improve their self-rated health; the interventions also have the potential to be cost effective. This thesis also provides evidence that an occupation-focused and occupation-based intervention implemented for older people with bathing disabilities was effective in promoting independence from home help for bathing. Finally, an occupation-focused and occupation-based intervention that increased the probability of being independent of home help for bathing had a positive impact on the long term accumulation of QALYs and reduced societal costs and, therefore, can be considered very cost effective.
