<p>Tuberkulos (tbc) är en luftburen droppsmitta orsakad av mykobakterium tuberkulosis. Tbc är den sjukdom som efter AIDS orsakar flest dödsfall, trots att botande behandling finns. Behandlingen är krävande för den tbc-smittade att genomgå och bygger på en kombination av en rad antibiotika som måste intas under minst sex månader. Ett avvikande i behandlingen kan resultera i att mykobakterium tuberkulosis blir resistent mot de ordinerade antibiotika. Följsamhet av långtidsbehandlingar som tbc-behandling graderas till 50 %. Syftet med litteraturstudien var att ur ett globalt perspektiv beskriva hur sjuksköterskan kan påverka följsamhet vid tbc-behandling i syfte att minska resistensutvecklingen av mykobakterium tuberkulosis. Studien genomfördes som en litteraturstudie där 12 vetenskapliga artiklar granskades och analyserades. Resultatet visar tydligt att specifika faktorer påverkar följsamhet och därigenom resistensutvecklingen. Faktorerna innefattar patientundervisning, behandlingsstrategier, omgivningens påverkan och stöd.  Undervisningen resulterar i att patienten får ökad förståelse för behandlingen. För att minska stigmatiseringen och det lidande den innebär för den tbc-smittade är även omgivningen i behov av ökad kunskap och information om tbc. Ett flertal studier visar att DOTS-strategin är betydelsefull för ökad följsamhet vid antituberkulos behandling. Litteraturstudien medför ett förslag om att sjuksköterskeprogrammet ska öka fokuseringen på följsamhet vid läkemedelsanvändning. Sjuksköterskan är i behov av att redan under grundutbildningen få kunskap om ansvarsfull antibiotikahantering som leder till en följsamhetsomtanke.</p> / <p>Tuberculosis (TB) is an airborne droplet infection caused by mycobacterium tuberculosis. TB is the disease after AIDS that is most deadly, even though curative treatment exists. The treatment is demanding for the TB-infected to undergo and consists of a combination of a number of antibiotics that must be administered for at least six months. A dissenting in anti-tuberculosis treatment might result in mycobacterium tuberculosis strains that are resistant to antibiotics. As adherence to long-term treatment is graded at a low percentage (50 %) the aim of the literature study was from a global perspective to develop a working-strategy for nurses that promote compliance in TB-treatment in order to reduce resistance development of mycobacterium tuberculosis. The study was conducted as a literature study where 12 research articles were reviewed and analyzed. The results describe specific factors that are essential to compliance. These factors comprise patient education, treatment strategies, social influences and support. As knowledge gives the patient a better understanding for the treatment it provokes compliance. The social environment of the TB-infected patient demands increased knowledge in order to reduce stigma. Several studies show that the DOTS strategy is important for increasing compliance in anti-tuberculosis treatment. The literature study results in a proposal for the nursing program to focus more on compliance in taking medication. The nursing program’s attendants need to gain knowledge about prudent antibiotic treatment that leads to a compliance concern.</p>

However, the thermal instability prevents real applications of InSZ-based TBC. This thesis investigates the hypothesis of co-doping using the charge compensation to improve the phase stability of InSZ. Four co-doping systems were synthesized by coprecipitation and studied: (ZrO2)1-(x+y)(InO1.5)x(MOz)y with MOz = TaO2.5, NbO2.5, MoO3, or WO3. After synthesis, 9 mol% of InO1.5 plus the charge-compensating oxides was sufficient to stabilize the tetragonal phase. Specific surface area up to 106.1 m2.g-1 and crystallite size ~11 nm were achieved using ethanol washing followed by azeotropic distillation as dehydration technique in the precipitates. In these powders, initial thermal stability analysis indicated instability of the tetragonal phase, with extension of the t→m transformation less detrimental in the InMoSZ system. Further increase in the concentration of InO1.5:MoO3 results in monophasic samples with retention of cubic phase in the InMoSZ. Cubic InMoSZ exhibited hardness and thermal expansion coefficient of 13.5% and 9% higher than those of InSZ, respectively. However, thermal treatments at T ≥ 1200 °C showed that the InMoSZ is also passive to destabilization of the high temperature cubic polymorph. Although the cubic InMoSZ was the most promising system found in this thesis, the stability results do not support its application as TBC for temperatures ≥ 1000 ºC. A deep evaluation of the phase transformations between 1000 to 1200 °C indicated that the instability of the proposed systems is due to a progressive c→t→m destabilization of the polymorphs. This c→t→m transformation is directly associated with the reduction of the InO1.5 stabilizer in solid solution by volatilization as In2O during heat treatment. At temperatures ≤ 800 ºC, the c→t phase transformation do not occurs, then, InSZ-based TBC is stable in these conditions. / A zircônia estabilizada com InO1,5 (InSZ) é um material com potencial aplicação como revestimentos para barreira térmica (TBC) resistentes à corrosão. Contudo, a instabilidade de fases impede aplicações industriais da InSZ. Esta tese investiga a ação da co-dopagem por compensação de cargas como uma estratégia para aumentar a estabilidade de fases da InSZ. Quatro sistemas de co-dopagem foram sintetizados por co-precipitação e estudados: (ZrO2)1-(x+y)(InO1,5)x(MOz)y com MOz = TaO2,5, NbO2,5, MoO3 ou WO3. Após a síntese, 9 %mol de InO1,5 somado a concentração de óxidos compensadores de carga foi suficiente para estabilização da fase tetragonal. Área superficial específica de até 106,1 m2.g‒1 e tamanho de cristalitos de ~11 nm foram obtidos utilizando a lavagem com etanol seguida por destilação azeotrópica como técnica de desidratação dos precipitados. Para estes pós, testes de estabilidade térmica indicaram instabilidade da fase tetragonal, com extensão de transformação t→m menos detrimental no sistema InMoSZ. Aumentando gradativamente a concentração de InO1,5-MoO3 na InMoSZ resulta em amostras monofásicas com retenção da fase cúbica. A InMoSZ cúbica exibiu dureza e coeficiente de expansão térmica até 13,5% e 9% superiores aos valores da InSZ, respectivamente. No entanto, tratamentos em temperaturas ≥ 1200 ºC indicaram que a InMoSZ é também suscetível a desestabilização da fase cúbica. Embora a InMoSZ cúbica tenha sido o sistema mais promissor obtido nesta tese, os resultados de estabilidade indicam que sua aplicação como TBC não é possível em temperaturas ≥ 1000 ºC. Uma avaliação detalhada das fases formadas após os tratamentos entre 1000 a 1200 ºC demonstrou que a instabilidade dos sistemas estudados é decorrente de uma transformação progressiva tipo c→t→m. A origem da transformação c→t→m é associada a redução da concentração do estabilizador InO1,5 em solução sólida por volatilização como In2O durante os testes de estabilidade térmica. Em temperaturas ≤ 800 ºC, a transformação c→m não ocorre, neste caso, TBCs baseadas em InSZ são estáveis termicamente para aplicações industriais.

Caracterização de propriedades leiteiras com relação ao conhecimento técnico, gestão administrativa e atendimento das necessidades humanas / Characterization of dairy farms regarding to technical knowledge, administrative management and satisfaction of human needs

Os objetivos deste trabalho foram relacionar o atendimento das necessidades humanas dos funcionários com os indicadores de qualidade do leite, avaliar o conhecimento técnico tanto dos ordenhadores quanto dos proprietários e gerentes, observar as práticas realizadas na ordenha, relacionando-as com os indicadores de qualidade do leite e verificar a adoção de ferramentas da gestão administrativa de explorações leiteiras, segundo o sistema MDA, da Clínica do Leite - ESALQ/USP. Para atingir estes objetivos, 74 propriedades leiteiras localizadas nos Estados de São Paulo, Minas Gerais e Rio de Janeiro participaram deste projeto. Estas propriedades foram divididas em dois grupos, a partir da média de contagem bacteriana total (CBT) apresentada, sendo o grupo com alta CBT com média superior a 200.000 ufc/ml e o grupo com baixa CBT, com média inferior a 50.000 ufc/ml. Foram realizadas visitas a estas propriedades, com duração de meio dia, sendo que estas constituíram no acompanhamento da rotina de ordenha, aplicação de questionários com os ordenhadores sobre questões relacionadas ao atendimento das necessidades humanas, segundo Maslow e conhecimentos técnicos e aplicação de questionários com os proprietários ou gerentes sobre aspectos relacionados ao gerenciamento das propriedades, segundo o sistema MDA. Os dados foram analisados por estatística descritiva e também pela metodologia de árvores de regressão, relacionando as questões com a CBT e contagem de células somáticas (CCS). A média de CBT das propriedades com alta CBT foi de 330.113 ufc/ml (± 143.865 ufc/ml) e das com baixa CBT foi 49.126 ufc/ml (± 36.704 ufc/ml). Já para CCS, as médias foram: 908.473 céls/ml (± 463.992 céls/ml) para propriedades com alta CBT e 605.728 céls/ml (± 282.885 céls/ml) para propriedades com baixa CBT. Tanto os funcionários quanto proprietários ou gerentes de ambos os grupos apresentaram conhecimento satisfatório sobre a rotina de ordenha, mas uma pequena porcentagem dos grupos conhecia os conceitos relacionados à qualidade do leite. No entanto, os conhecimentos sobre mastite foram maiores para os funcionários e proprietários ou gerentes das propriedades com baixa CBT. Com relação ao atendimento das necessidades humanas, todas as necessidades tenderam a ser mais bem atendidas para os funcionários das propriedades com baixa CBT, com exceção das necessidades fisiológicas. O mesmo perfil pode ser observado para o gerenciamento administrativo das propriedades com baixa CBT, sendo que a adoção das ferramentas de gestão, como política básica, gestão da rotina, procedimentos operacionais e recursos humanos, tendeu a ser maior nestas propriedades. Além disso, os procedimentos recomendados da rotina de ordenha foram mais e melhor conduzidos pelas propriedades com baixa CBT, assim como a limpeza dos equipamentos. Desta forma, pode-se observar que os aspectos relacionados ao conhecimento técnico, comprometimento dos funcionários, conseguido através do atendimento das necessidades humanas, adoção de ferramentas de gestão administrativa e a ordenha estão interferindo na qualidade do leite produzido. Apesar das propriedades com baixa CBT terem apresentado resultados melhores nestas áreas, há a possibilidade de melhorar ainda mais a qualidade do produto, através do investimento em todas as áreas relacionadas neste projeto. / The purposes of this work were to relate satisfaction of employees\' human needs with milk quality indicators, to evaluate technical knowledge of milkers and producers or managers, to observe milking practices conducted, relating them to milk quality indicators and to verify adoption of dairy farms management tools, according to MDA system, of Clínica do Leite - ESALQ/USP. In order to achieving these goals, 74 dairy farms located in states of São Paulo, Minas Gerais and Rio de Janeiro participated of this work. These dairy farms were divided in two groups, from total bacterial count (TBC) average, being the group with high TBC with average greater than 200,000 cfu/ml and the group with low TBC with average minor than 50,000 cfu/ml. Visits to these farms were accomplished during half day, being these constituted of milking routine observation, application of questionnaires to milkers about satisfaction of human needs, according to Maslow and technical knowledge and application of questionnaire to producers or managers about aspects related to farms management, according to MDA system. Statistical analyses were performed by descriptive statistics and also by regression trees methodology, relating questions with TBC and somatic cell count (SCC). The TBC average of dairy farms with high TBC was 330,113 cfu/ml (± 143,865 cfu/ml), and of those with low TBC was 49,126 cfu/ml (± 36,704 cfu/ml). The averages of SCC were: 908,473 cells/ml (± 463,992 cells/ml) to dairy farms with high TBC and 605,728 cells/ml (± 282,885 cells/ml) to dairy farms with low TBC. Both groups of milkers and producers or managers showed a satisfactory knowledge about milking routine, but a little percentage of both groups knew the concepts related to milk quality. Nevertheless, knowledge about mastitis was greater to milkers and producers or managers of dairy farms with low TBC. Regarding human needs satisfaction, all needs tended to be more satisfied to milkers of dairy farms with low TBC, except for physiological needs. The same scenario was observed to administrative management of dairy farms with low TBC, being the adoption of management tools, like basic policy, routine management, operational protocols and human resource, tended to be greater at these farms. Besides, milking routine recommended practices were more and better conducted by dairy farms with low TBC, just like equipment cleaning. In this way, it can be observed that aspects related to technical knowledge, employees commitment, achieved through satisfaction of human needs, adoption of administrative management tools and milking were interfering at milk quality. Despite dairy farms with low TBC showed better results at these areas, there is a possibility to improve the product quality, through investment in all related areas of these work.

High Performance Thermal Barrier Coatings On Additively Manufactured Nickel Base Superalloy Substrates

<p>Thermal barrier coatings (TBCs) made of low-thermal-conductivity ceramic topcoat, metallic bond coat and metallic substrate, have been extensively used in gas turbine engines for thermal protection. Recently, additive manufacturing (AM) or 3D printing techniques have emerged as promising manufacturing techniques to fabricate engine components. The motivation of the thesis is that currently, application of TBCs on AM’ed metallic substrate is still in its infancy, which hinders the realization of its full potential.</p> <p>The goal of this thesis is to understand the processing-structure-property relationship in thermal barrier coating deposited on AM’ed superalloys.</p> <p>The APS method is used to deposit 7YSZ as the topcoat and NiCrAlY as the bond coat on TruForm 718 substrates fabricated using the direct metal laser sintering (DMLS) method. For comparison, another TBC system with the same topcoat and bond coat is deposited using APS on wrought 718 substrates. For thermomechanical property characterizations, thermal cycling, thermal shock (TS) and jet engine thermal shock (JETS) tests are performed for both TBC systems to evaluate thermal durability. Microhardness and elastic modulus at each layer and respective interfaces are also evaluated for both systems. Additionally, the microstructure and elemental composition are thoroughly studied to understand the cause for better performance of one system over the other.</p> <p>Both TBC systems showed similar performance during the thermal cycling and JETS test but TBC systems with AM substrates showed enhanced thermal durability especially in the case of the more aggressive thermal shock test. The TBC sample with AM substrate failed after 105 thermal shock cycles whereas the one with wrought substrate endured a maximum of 85 cycles after which it suffered topcoat delamination. The AM substrates also demonstrated an overall higher microhardness and elastic modulus except for post thermal cycling condition where it slightly underperformed. This study successfully demonstrated the use of AM built substrates for an improved TBC system and validated the enhanced thermal durability and mechanical properties of such a system.</p> <p>A modified YSZ TBC architecture with an intermediate Ti3C2 MXene layer is proposed to improve the interfacial adhesion at the topcoat/bond coat interface to improve the thermal durability of YSZ</p> <p>12</p> <p>TBC systems. First principles calculations are conducted to study the interfacial adhesion energy in the modified and conventional YSZ TBC systems. The results show enhanced adhesion at the bond coat/MXene interface. At the topcoat/MXene interface, the adhesion energy is similar to the adhesion energy between the topcoat and bond coat in a conventional YSZ TBC system.</p> <p>An alternative route is proposed for the fabrication of YSZ TBC on nickel base superalloy substrates by using the SPS technology. SPS offers a one-step fabrication process with faster production time and reduced production cost since all the layers of the TBC system are fabricated simultaneously. Two different TBC systems are processed using the same heating protocol. The first system is a conventional TBC system with 8YSZ topcoat, NiCoCrAlY bond coat and nickel base superalloy substrate. The second system is similar to the first but with an addition of Ti3C2 MXene layer between the topcoat and the bond coat. Based on the first principles study, addition of Ti3C2 layer enhances the adhesion strength of the topcoat/bond coat interface, an area which is highly susceptible to spallation. Further tests such as thermal cycling and thermal shock along with the evaluation of mechanical properties would be carried out for these samples in future studies to support our hypothesis.</p>

However, in order toobtain accurate lifetime expectancies and performance of thecoatings system it is necessary to have a reliableunderstanding of the mechanical properties and failuremechanisms of the coatings. In this thesis, mechanical and fracture behaviour have beenstudied for a NiAl coating applied by a pack cementationprocess, an air-plasma sprayed NiCoCrAlY bondcoat, a vacuumplasma-sprayed NiCrAlY bondcoat and an air plasma-sprayed ZrO2+ 6-8 % Y2O3topcoat. The mechanical tests were carried out ata temperature interval between room temperature and 860oC.Small punch tests and spherical indentation were the testmethods applied for this purpose, in which existing bending andindentation theory were adopted for interpretation of the testresults. Efforts were made to validate the test methods toensure their relevance for coating property measurements. Itwas found that the combination of these two methods givescapability to predict the temperature dependence of severalrelevant mechanical properties of gas turbine coatings, forexample the hardness, elastic modulus, yield strength, fracturestrength, flow stress-strain behaviour and ductility.Furthermore, the plasma-sprayed coatings were tested in bothas-coated and heat-treated condition, which revealedsignificant difference in properties. Microstructuralexamination of the bondcoats showed that oxidation with loss ofaluminium plays an important role in the coating degradationand for the property changes in the coatings. Experimental investigation of film cooling and thermal barrier coatings on a gas turbine vane with conjugate heat transfer effects

In the United States, natural gas turbine generators account for approximately 7% of the total primary energy consumed. A one percent increase in gas turbine efficiency could result in savings of approximately 30 million dollars for operators and, subsequently, electricity end-users. The efficiency of a gas turbine engine is tied directly to the temperature at which the products of combustion enter the first stage, high-pressure turbine. The maximum operating temperature of the turbine components’ materials is the major limiting factor in increasing the turbine inlet temperature. In fact, current turbine inlet temperatures regularly exceed the melting temperature of the turbine vanes through advanced vane cooling techniques. These cooling techniques include vane surface film cooling, internal vane cooling, and the addition of a thermal barrier coating (TBC) to the exterior of the turbine vane. Typically, the performance of vane cooling techniques is evaluated using the adiabatic film effectiveness. However, the adiabatic film effectiveness, by definition, does not consider conjugate heat transfer effects. In order to evaluate the performance of internal vane cooling and a TBC it is necessary to consider conjugate heat transfer effects. The goal of this study was to provide insight into the conjugate heat transfer behavior of actual turbine vanes and various vane cooling techniques through experimental and analytical modeling in the pursuit of higher turbine inlet temperatures resulting in higher overall turbine efficiencies. The primary focus of this study was to experimentally characterize the combined effects of a TBC and film cooling. Vane model experiments were performed using a 10x scaled first stage inlet guide vane model that was designed using the Matched Biot Method to properly scale both the geometrical and thermal properties of an actual turbine vane. Two different TBC thicknesses were evaluated in this study. Along with the TBCs, six different film cooling configurations were evaluated which included pressure side round holes with a showerhead, round holes only, craters, a novel trench design called the modified trench, an ideal trench, and a realistic trench that takes manufacturing abilities into account. These film cooling geometries were created within the TBC layer. Each of the vane configurations was evaluated by monitoring a variety of temperatures, including the temperature of the exterior vane wall and the exterior surface of the TBC. This study found that the presence of a TBC decreased the sensitivity of the thermal barrier coating and vane wall interface temperature to changes in film coolant flow rates and changes in film cooling geometry. Therefore, research into improved film cooling geometries may not be valuable when a TBC is incorporated.
