O Índice Dental Estético (DAI) tem sido amplamente usado em estudos envolvendo crianças e adolescentes, mas raramente usado em estudos com adultos. O propósito do presente estudo transversal foi avaliar a confiabilidade do DAI em uma amostra com 120 pacientes adultos, incluindo 46 do sexo masculino (38,3%) e 74 do sexo feminino (61,7%), entre 19 e 78 anos de idade (média de 49,2). Quase todos os pacientes apresentaram-se com muitas perdas dentárias e sem tratamento ortodôntico prévio. A prevalência e a severidade da má oclusão, a necessidade de tratamento ortodôntico, a autopercepção do paciente em relação à necessidade deste tipo de tratamento e a indicação, pelo cirurgião-dentista, foram avaliadas. Além disso, as opiniões subjetivas de 33 ortodontistas em 32 pacientes, por meio de fotografias intra e extrabucais e modelos de estudo foram também analisadas. Teste estatístico Kappa foi usado para avaliar o DAI e as opiniões dos profissionais. Testes qui-quadrado e de Fischer foram usados para verificar a correlação entre sexo, autopercepção e indicação do cirurgião-dentista e o escore do DAI. O teste t-student foi usado para investigar a correlação entre a idade e a necessidade de tratamento ortodôntico. A média do escore DAI foi de 38,5. Quase metade da amostra apresentou má oclusão severa ou muito severa, a qual sugeriu grande necessidade de tratamento ortodôntico. A confiabilidade intra e interexaminador do escore DAI e dos componentes foi muito alta (k>0,9). Inconsistências foram observadas na avaliação dos especialistas (k<0,4). Não houve diferenças estatisticamente significantes (P>0.05) nos escores do DAI entre sexo, idade, autopercepção da necessidade de tratamento, entretanto diferenças significantes foram encontradas em relação às indicações pelo cirurgião-dentista. Os resultados do presente estudo sugerem que a população de adultos com perdas dentárias tem elevada severidade da má oclusão e alta necessidade de tratamento ortodôntico. / Dental Aesthetic Index (DAI) has been broadly used in studies comprising children and adolescent patients but rarely used in adult studies. Darbo tikslas - ištirti ortodontinių anomalijų paplitimą, gydymo reikalingumą bei gydymo poreikį tarp Lietuvos moksleivių. Tikslui pasiekti iškelti šie uždaviniai: 1) nustatyti ortodontinių anomalijų paplitimą tarp 10–11 m. ir 14–15 m. amžiaus Lietuvos moksleivių; 2) įvertinti ir palyginti ortodontinių anomalijų gydymo reikalingumą tarp 10–11 m. ir 14–15 m. Lietuvos moksleivių; 3) ištirti Lietuvos moksleivių požiūrį į ortodontinių anomalijų gydymo poreikį; 4) nustatyti moksleivių ortodontinių anomalijų gydymo poreikio sąsajas su demografiniais ir socialiniais veiksniais. Tyrimas atliktas dešimtyje Lietuvos apskričių, 41 atsitiktinai pasirinktoje mokykloje. Tyrimo metu nustatytas dantų susigrūdimas, tarpai tarp dantų, gilus ir kryžminis sąkandis, pirmųjų krūminių dantų tarpusavio santykis pagal Angle klasifikaciją, gydymo reikalingumui nustatyti naudotas ICON indeksas, o gydymo poreikiui bei sąsajoms su socialinias veiksniais - klausimynas. Ortodontinės anomalijos nustatytos 77 proc. 10–11 m. amžiaus ir 61,7 proc. 14–15 m. amžiaus tirtųjų moksleivių. Ortodontinis gydymas reikalingas beveik pusei jaunesniojo amžiaus ir kas trečiam vyresniojo amžiaus moksleiviui, o ortodontinio gydymo poreikis buvo panašus abiejose amžiaus grupėse. / The aim of this study was to assess the malocclusion prevalence, orthodontic treatment need and demand among Lithuanian schoolchildren. The objectives of the study: 1) to describe the malocclusion prevalence among Lithuanian schoolchildren in 10–11 and 14–15-year old age groups; 2) to evaluate the need for orthodontic treatment among 10–11, 14–15 year-old schoolchildren in Lithuania using the ICON index; 3) to investigate the association between normative (objective) and self-perceived (subjective) orthodontic treatment need; 4) to assess the associations of age, gender, and socioeconomic factors with dental appearance satisfaction and demand for orthodontic treatment. The survey was conducted in 41 randomly selected schools in 10 counties of Lithuania. The study recorded crowding, spacing, overbite, crossbite, relationship of the first upper and lower molars according to Angle’s classification, the orthodontic treatment need was assessed using the ICON index and the children were also invited to complete a questionnaire about treatment need and their appearance. The results showed that 77% of 10–11-year-old and 61.7% of 14–15-year-old schoolchildren had malocclusion. The treatment need among younger schoolchildren was higher than in older group. The subjective demand for orthodontic treatment among 10–11-year-old schoolchildren and 14–15-year-old schoolchildren was similar.

A necessidade de tratamento foi avaliada através do Índice de Estética Dental (DAI) e do Componente Estético do Índice de Necessidade de Tratamento Ortodôntico IOTN-AC. Também foi avaliado o impacto estético subjetivo da má oclusão através da Escala Ortodôntica do Impacto Estético Subjetivo (OASISOrthodontics Aesthetic Subjective Impacte Score). O estudo foi realizado com 451 crianças de 12 anos de idade e 373 responsáveis. A análise estatística foi realizada utilizando o χ2 e o Teste Exato de Fischer e Regressão Logística Múltipla. De acordo com o DAI, a maioria dos escolares necessitam de algum tipo de terapia ortodôntica (65,6%). A necessidade obrigatória de tratamento, relacionada às más oclusões severas, foram diagnosticadas em 13,7% dos estudantes, enquanto o restante foi dividido em altamente desejável (19,1%) e tratamento eletivo (32,8%). A Avaliação da necessidade estética (IOTN-AC) foi observada em 19,7% dos escolares, sendo 12,6% para casos limítrofes e 7,1% para necessidade definida. O impacto estético subjetivo (OASIS) foi observado em 14,9% dos estudantes. O IOTN e o OASIS, embora tenham apresentado associação estatística com o DAI, mostraram valores significativamente diferentes (p<0,001). A necessidade de tratamento autopercebida pelas crianças (83,8%) e percebida pelos responsáveis (85,6%) foram, ambas, maiores que a necessidade normativa (DAI), e mostraram associação significante com a necessidade normativa (DAI), quando ajustada pela escolaridade dos responsáveis e nível socioeconômico (p = 0,023). As variáveis sexo (p=0,042), tipo de escola (p=0,002), sobressaliência maxilar maior ou igual a 4mm (p=0,037) e sorriso gengival maior ou igual a 4mm (p=0,008) associaram-se significativamente com o impacto estético subjetivo da má oclusão (OASIS). Concluiu-se que a maioria dos escolares avaliados apresentou necessidade de tratamento ortodôntico de acordo com critérios normativos e pela percepção dos próprios escolares e seus responsáveis. Porém, esta necessidade diminui substancialmente quando avaliada apenas por critérios normativos estéticos, o que evidencia a divergência entre os resultados obtidos por diferentes instrumentos. / This study aimed to evaluate the prevalence of malocclusion and orthodontic treatment need in schoolchildren of 12 years of age in the city of Juíz de Fora, Minas Gerais and to compare it to the need perceived by the parents/guardians and children from the sample, evaluating potential associated sociodemographic factors. The need for treatment was assessed using the Dental Aesthetic Index (DAI) and the Aesthetic Component of the Index of Orthodontic Treatment Need IOTN-AC. It was assessed also the subjective aesthetic impact of malocclusion through OASIS (Orthodontics Aesthetic Subjective Impact Score). The study was conducted with 451 children from 12 years of age and 373 guardians. Statistical analysis was performed using x2 and Fisher's exact test and multiple logistic regression. According to the DAI, the majority of the schoolchildren need some type of orthodontic therapy (65.6%). The compulsory need for treatment, related to severe malocclusions, were diagnosed in 13.7% of the students, while the remaining was divided into highly desirable (19.1%) and elective treatment (32.8%). The evaluation of aesthetic need (IOTN-AC) was observed in 19.7% of the schoolchildren, being 12.6% for borderline cases and 7.1% for defined need. The aesthetic impact of malocclusion was observed in 14.9% of the students. The IOTNAC and the OASIS, although they showed a statistical association with the DAI, they have shown values significantly lower (p <0.001). The self-perceived need for treatment by the children (83.8%) and perceived by their parents/guardians (85.6%) were both higher than the normative need (DAI). The perceived need reported by the parents/guardians presented significant association with the normative treatment need, when adjusted by the parental education and the socioeconomic status (p = 0.023). The variables: gender (p = 0.042), type of school (p = 0.002), maxillary overjet greater than or equal to 4 mm (p = 0.037) and gingival smile greater than or equal to 4 mm (p = 0.008) were significantly associated with the subjective aesthetic impact of malocclusion (OASIS). It was concluded that the majority of the schoolchildren evaluated presented orthodontic treatment need according to normative criteria and by the perception of the schoolchildren themselves and their parents/guardians. However, this need diminishes substantially when measured only by normative aesthetic criteria, which highlights the discrepancy between the results obtained by different instruments.

Interceptive orthodontic treatment need among children attending dental clinics in the Tygerberg sub-district

This results in growing waiting lists which place a huge strain on resources of the facility. Objectives To determine: 1. The prevalence of malocclusion among 7-to-10-year-old children examined at dental clinics within the Tygerberg sub-district. 2. The types of malocclusion they present with. 3. Which of these malocclusions can be treated with interceptive orthodontic treatment. Methods An analytical, descriptive, cross-sectional study was carried out and quantitative methods were used to achieve the aim and objectives. The study sample consisted of 100 children, 46 male and 54 female, from Bellville and Bishop Lavis dental clinics. Dental examinations were done by one examiner on children between the ages of 7 and 10 years. Dental problems were identified which, if left untreated, could result in the need for more complex orthodontic treatment at a later stage. All the necessary ethical clearance was obtained. Results The results showed that there is a very large interceptive orthodontic treatment need (79%) at these clinics. A large proportion (70%) presented with detrimental habits. Thumbsucking was more prevalent among girls than boys. Other problems like mouthbreathing, lip wedging, deep bite, asymmetrical mobility of primary teeth, rotated lateral incisors and crossbites were found. The biggest concern was that 71% of the children had early loss of primary teeth and only 5% of children received restorations, resulting in a high prevalence of unfavourable molar relationships. All of these should be addressed early, thereby preventing the development of complex orthodontic problems or skeletal discrepancies. iv Conclusion Interceptive and preventive orthodontic treatment should be regarded as a primary health centre service, as it may reduce the need for costly fixed orthodontic treatment. The study showed that urgent intervention is necessary from the Department of Health to address this issue.

Magister Chirurgiae Dentium (MChD) / The psychological, social, and cultural aspects of facial and dental attractiveness is an integral part of health care. Lay persons perceptions of facial and dental attractiveness are influenced by many factors including age, gender, geographic location (nationally or international), ethnic status, employment status, cultural differences and social class will effect the social impact of the anomalies. In addition, the public response to dental anomalies will vary according to experience of good health or ill health of the individual, relatives and friends. The perception of malocclusion is often seen differently between orthodontists, patients and lay public. The differing perceptions of patients and dental practitioners may influence the delivery of orthodontic care. It has been reported that 70% of orthodontic treatment is dentist induced and that orthodontic treatment may be undertaken without the patient or parents perceiving a problem with the teeth. (DHSS, Schanscheiff Report, 1986). The thresholds of entry and exit for orthodontic care may be different as perceived by orthodontists and lay persons. Orthodontists may pursue full alignment and perfect occlusion whereas lay persons may accept varying degrees of deviation from normal. This current study has assessed and recorded the differences in opinions of professional orthodontists and lay persons on dento-facial aesthetics, orthodontic treatment need and orthodontic treatment outcome. The study was carried out at the Orthodontic Department, University of Wales, College of Medicine, School of Dentistry. The opinions of 56 lay persons (dental technicians, dental nurses and lay persons not involved in the clinical process) and 97 orthodontists was collected by recording judgements using various patient records as stimuli. For each set of records examined, each member of the panel indicated their opinion using Likert scales. Judgements were made by assessments of 68 study casts in respect of:- * the need for orthodontic treatment on dental health grounds. the need for orthodontic treatment on dental aesthetic grounds. deviation from normal occlusion. the decision whether to treat. ** * In addition, judgements were made on a sample of 50 pairs of outline facial profiles before and after treatment in respect of: * * deviation from normal facial aesthetics comparison of aesthetics pre-treatment and post-treatment. Furthermore, assessments on 50 pairs of pre-treatment and post-treatment study casts in respect of: * * degree of improvement as a result of treatment. acceptability of result. Orthodontists subjective judgements are more reliable than lay persons in their assessments of dental aesthetics, dental health and deviation from normal. The level of agreement for the decision to recommend treatment is similar between orthodontists and lay groups ie. lay persons are as reliable as orthodontists in the decision to recommend treatment. Orthodontists tend to recommend 10 - 12 % more treatment than lay persons. The orthodontist group were more reliable than the lay group in assessing degree of improvement and assessment of outcome. Orthodontists reject approximately 25% of cases deemed acceptable by the lay group. Dental aesthetics appeared to be the most important feature in the assessment of treatment outcome by both orthodontists and lay groups.

Anomalies de croissance maxillo-faciale : facteurs de risque et accès au traitement / Maxillofacial growth anomalies : risk factors and access to treatment

Les anomalies de croissance maxillo-faciale résultent de variations du processus normal de croissance. Après les caries, elles représentent les problèmes bucco-dentaires les plus fréquents chez les jeunes et peuvent entraîner des troubles fonctionnels (mastication, phonation, respiration), esthétiques et parfois psychologiques. Le traitement orthodontique vise à corriger les anomalies de croissance maxillo-faciale. L’objectif de cette thèse est de rechercher des facteurs de risque précoces d’anomalies de croissance maxillo-faciale et d’étudier le rôle des facteurs socio-économiques, culturels et géographiques de l’accès au traitement orthodontique en France. Les enfants de la cohorte EPIPAGE, qui avait inclus toutes les naissances survenues entre 22 et 32 semaines d’aménorrhée dans neuf régions françaises en 1997, ont été examinés à 5 ans. Nous avons étudié les relations entre les caractéristiques néonatales et la déformation du palais à 5 ans chez 1711 enfants. Le sexe masculin, le petit âge gestationnel, le petit poids pour l’âge gestationnel et peut-être l’intubation de longue durée sont identifiés comme des facteurs de risque de déformation du palais à 5 ans chez les grands prématurés ; de plus, les enfants avec des déficiences neuro-motrices semblent particulièrement à risque. La cohorte mère-enfant EDEN, a inclus des femmes enceintes en 2002-2003 et leur enfant à la naissance. Nous avons recherché les facteurs de risque précoces de l’occlusion postérieure inversée et de la béance antérieure chez 422 enfants de 3 ans. En plus d’une tétine encore utilisée à 3 ans et de la respiration buccale qui sont des facteurs connus, un nouveau facteur de risque d’occlusion postérieure inversée en denture temporaire est mis en évidence : la prématurité. Enfin, l’étude chez les 5988 enfants et les adolescents de l’enquête sur la santé et la consommation de soins en France réalisée par l’Insee en 2002-2003, enquête transversale sur échantillon représentatif de la population vivant en France, montre que 23% des 12-15 ans ont un traitement orthodontique. En plus du facteur économique, le moindre recours au traitement orthodontique est aussi lié à l’environnement social et culturel moins favorisé de l’enfant, à l’absence de couverture complémentaire et à l’habitation en zone rurale.En conclusion, pour mieux comprendre les inégalités dans le traitement orthodontique, il faudrait en évaluer les besoins en amont. Nous avons étudié des facteurs liés à la présence d’anomalies de croissance maxillo-faciale à des âges très jeunes afin de pouvoir identifier tôt des enfants à risque de ces anomalies. Il est nécessaire de vérifier leur évolution pour savoir si elles sont de réels marqueurs précoces de besoin en traitement orthodontique. / Malocclusions are due to variations of normal process of growth. Besides caries, they are the most common oral problems encountered by children and teenagers. They may lead to oral dysfunction (in chewing, speaking and breathing), esthetic and sometimes psychological issues. Orthodontic treatment aims at correcting malocclusions.The purpose of this thesis is to investigate early risk factors for malocclusions and to analyze the role of socioeconomic, cultural and geographic factors in access to orthodontic treatment in France. In the EPIPAGE cohort study, which included all live births between 22 and 32 weeks of gestation in 9 French regions in 1997, 1711 children were examined at 5 years. We explored the relations between neonatal characteristics and alteration of palatal morphology at 5 years. Male sex, low gestational age, small for gestational age and maybe intubation of long duration were identified as risk factors for alteration of palatal morphology et 5 years in very preterm children; children with neuromotor deficiencies seem particularly at risk. The mother-child EDEN cohort included pregnant women in 2002-2003 and their child at birth. We investigated early risk factors for posterior crossbite and anterior open bite in 422 3-year-old children. In addition to ongoing pacifier sucking habit at 3 years and mouth breathing, which are well-known risk factors, prematurity appears to be a new risk factor for posterior crossbite in temporary dentition. Finally, the French survey on health and care consumption, carried out in 2002-2003 in a representative sample of 5988 children and teenagers, shows that 23% of the 12-15 years old have an orthodontic treatment. Besides the economic factor, the less orthodontic treatment uptake is related to a less privileged social environment, to the absence of supplementary insurance and to living in a rural area.In conclusion, to understand inequalities in orthodontic treatment better, orthodontic treatment need should be evaluated first. We investigated factors related to malocclusions at very young ages, which could help to early identify children at risk. The assessment of how these early malocclusions would evolve is therefore needed in order to control if those malocclusions are indeed early markers for orthodontic treatment need.

AIM: To evaluate the prevalence of orthodontic problems and quality of life in relation to orthodontic problems among adolescent children in the city of Chennai, India. OBJECTIVES: To analyze the differences in the need for orthodontic treatment among children in the public and private schools; to analyze the need for orthodontic treatment among the various socioeconomic groups; to evaluate the Quality of Life (QoL) among the children; to analyze the relationship between Quality of Life (QoL) and the need for orthodontic treatment. METHODS: 200 children participated in the study, out of which 100 were from the private school and 100 from the public school from the city of Chennai, India. Two sets of questionnaires were used for the study, one was filled by the children and the other was filled by the researchers and trained dental assistants after doing a thorough dental examination and asking questions from the participants. This was carried out according to the recommendations of WHO oral health assessment. Statistical data was collected, recorded and analyzed using the software SPSS 17.0 for Windows. RESULTS: The private school children had more orthodontic problems than the public school children (49% and 44% respectively).
