本文使用1999年國民健康局提供之「臺灣地區中老年身心社會生活狀況長期追蹤調查 長期研究第四次調查」資料庫,試圖探討影響中老年人財產分配 (需求面) 及子女拜訪父母行為 (供給面) 之因素。 需求面層面,由橫斷面得知,同住比例對於父母財產分配影響的方向皆為負;此外年齡越高、教育程度越高、關心滿意程度越高、從事經濟規劃、擁有金融資產、常住房屋所有權為本人或配偶及子女持有、第二屋等不動產為為本人或配偶擁有,對於財產分配大都 (非全部迴歸式) 有著正面的影響;而男性父母、父母有伴侶、有工作,對於財產分配大部分 (非全部迴歸式) 有著負面的影響。 供給面層面,從橫斷面得知,父母的關心滿意程度越高、較願意照顧孫子、擁有金融資產、常住房屋所有權為父母或配偶持有、常住房屋所有權為子女持有或祖產、第二屋等不動產為父母與家人共有或與外人共有、父母曾經分配過財產、孫子數越多、男性子女、子女有工作、住在父母鄰近地區、曾經接受父母財產上的幫助,子女拜訪父母的機率將越高;此外子女年齡越高、有伴侶,對於拜訪次數大都 (非全部迴歸式) 有著負面的影響;而父母健康狀態越佳,對於拜訪次數大都 (非全部迴歸式) 有著正面的影響。

The Latino Integrative Medical Group Visit (IMGV) as a model to reduce pain in underserved Spanish speakers: a pilot feasibility study

BACKGROUND: Disparities in access to quality chronic pain treatment options disproportionately affect minorities. Although there is increasing evidence about the effectiveness of complementary and integrative medicine (CIM) to help in the treatment of pain, little is known about how low income minorities would benefit from having greater access to CIM. The Integrative Medical Group Visit (IMGV) model incorporates CIM in a medical group visit setting with the goal of increasing access to CIM. OBJECTIVE: The aims of this pilot study were to test 1) the feasibility of a Spanish language IMGV and 2) its effectiveness to reduce pain and improve function in Spanish speakers with chronic pain. METHODS: The study setting is the Family Medicine Clinic at Boston Medical Center. Adult Spanish speakers with chronic pain for at least 12 weeks were included; those pregnant, with psychosis, suicidal ideation or active substance abuse disorder were excluded. The intervention consisted of weekly, two-hour sessions for a total of 9 weeks. Main outcomes were pain level, pain interference, and physical and emotional function measured by the PROMIS 29 pre- and post-intervention, depression and stress, measured by PHQ-8 and PSS-10, respectively. Focus group participants discussed the feasibility of the intervention. Analyses involved t-tests to examine our outcome data, and qualitative thematic analysis for focus group data. RESULTS: This open study recruited 11 subjects, 10 women, average age of 51.9 years; 50% of participants attended more than four sessions. The outcomes showed a trend toward reduced depression, pain, fatigue, and anxiety. Qualitative themes of reduced pain, increased knowledge for healthier living, and improved access to pain treatment using an appropriately adapted intervention were found. CONCLUSIONS: It was feasible to implement the Latino IMGV model in the outpatient setting at BMC. Trends in reduction of pain and depression, as well as increased knowledge for healthier living and better access to CIM modalities were found. Future powered studies are needed to further implement this model.

Determinants of dental care utilization among low-income African-American women

OBJECTIVES: To investigate determinants of dental care utilization among low-income African-American women, focusing on psychosocial factors and predictors relevant to this population. METHODS: We used data from Wave I and II of the Detroit Dental Health Project. Participants were selected to represent African-American women caring for young children, and living in Detroit households below 250% of the federal poverty level. Papers I and II are cross-sectional, using baseline data from 969 women. Paper III follows 736 women longitudinally. Our main outcome variable was dental care utilization. The main independent variable in paper I was depression. The role of social support as a moderator was also assessed in that paper. In paper II, the main independent variables were depression, perceived discrimination, and food insufficiency, both individually and in combination. Paper III employs the framework of the Behavioral Model for Vulnerable Populations to assess predisposing, enabling and need factors predicting the incidence of dental visits. RESULTS: Only 41.8% of the women had a dental visit within the past year. Cross-sectional analyses indicated that depressed women had lower odds of having a dental visit within the past year (OR=0.71, 95%CI=0.53-0.94). Emotional social support attenuated the effect of depression on dental visits. Women with high levels of perceived discrimination and those with food insufficiency were less likely to have dental visits in the past year (OR=0.65, 95%CI=0.44-0.95, and OR=0.64, 95%CI=0.44-0.93, respectively). Depression and perceived discrimination were also associated with less preventive visits. When psychosocial stressors were combined, they acted synergistically to lower the odds of having dental visits, including preventive and treatment visits. Longitudinal analyses of the data identified significant interactions between dental insurance and perceived discrimination (P =0.02) and between dental insurance and having a dental home (P =0.04). Experiencing pain in the teeth or gums was also a significant predictor of future dental visits. CONCLUSIONS: Our findings indicate the importance of considering psychosocial factors when studying dental care utilization among low-income African-American women. Our findings also suggest that establishing dental homes, graduating culturally competent dental providers, and providing emotional support resources might improve dental care utilization among these women. / 2019-09-26T00:00:00Z

The essay examines a study trip to Krakow and the mediation of the Holocaust memory to 15 students. It is partly structured after three places that the students move between, experience arenas, which are the death and concentration camps, Poland and Sweden. The students are also moving according to four different dimensions: first place, but also time, morality and existential. The survey is based on a ritual theoretic view of social memory production and the formation of new witnesses. With inspiration from the Norwegian study Pilgrim, tourist and student: Norwegian school trips to death and concentration camps by Kyrre Kverndokk, has also this study trip been studied as a ritualization process based on which students will obtain a state of catharsis. With a qualitative research method, information has been gathered through participatory observations, group interviews and a poll. The result shows that the students attributed the camps to death, destruction and evil, but that they were struggling to live in the victims’ situation because it was too far from their own lives but also difficult to understand such evil. The students did on the other hand identified the spectator role since Sweden was seen as one of the countries that looked at and let the Holocaust happen. The students positioned themselves as new witnesses and told about their experiences in their free time and on their own initiative. The study indicates, among other things, that the Norwegian approach to the Holocaust differs from Swedish, which influences the meaningful learning process.

Educação ambiental em áreas protegidas do Estado de São Paulo e sua contribuição à escola / Environmental education in protected areas of São Paulo State and its contribution to school

As visitas a espaços não formais como as áreas protegidas têm sido utilizadas na educação escolar. Estudos já realizados mostram a importância dessas visitas, bem como algumas de suas fragilidades. Porém, há necessidade de se compreender a contribuição desses momentos para a escola. Assim, o objetivo desta pesquisa foi analisar a existência de contribuições dessas visitas às escolas e, caso existentes, caracterizá-las, considerando as concepções e práticas dos envolvidos na visitação e os seus contextos institucionais. Dados iniciais foram coletados por meio de questionário e permitiram mapear as áreas protegidas do Instituto Florestal e da Fundação Florestal, unidades da Secretaria do Meio Ambiente do Estado de São Paulo, que desenvolvem atividades de uso público com estudantes e professores, com foco nas visitas escolares monitoradas. A partir das respostas, foram selecionadas duas áreas, uma de cada unidade, para análise. Foram realizadas observações de visitas escolares monitoradas, entrevistas com representantes das áreas e professores das escolas visitantes e coletados documentos referentes a essas áreas. Para análise, consideraram-se referências do campo da educação escolar e da educação ambiental na perspectiva crítica, bem como documentos normativos referentes à educação ambiental em áreas protegidas. Constatou-se que em 72% (quarenta e nove) das sessenta e oito áreas protegidas administradas pelo Instituto Florestal e Fundação Florestal que responderam a pesquisa são desenvolvidas atividades de uso público com professores e alunos, sendo que as visitas escolares monitoradas ocorrem em 66% (quarenta e cinco áreas), com a presença das trilhas em grande parte dessas áreas. Nas concepções dos envolvidos, são indicadas contribuições à educação escolar, com ênfase na vivência propiciada aos alunos. Das três visitas acompanhadas, apenas uma propôs metodologia diferenciada da expositiva, com busca de objetos na trilha. Em todas predominaram informações biológicas, sem valorização, na prática pedagógica, da conservação e sem relação com a perspectiva crítica da educação ambiental. Deste modo, as contribuições ficam restritas à sua importância enquanto momento de vivência no ambiente natural pelos estudantes, conforme valorizado pelos entrevistados, e atividade prática complementar. Considerando estes resultados, confirma-se a hipótese de que as visitas escolares monitoradas contribuem para a educação escolar. Essa contribuição ocorre por meio da oportunidade da visita (como direito e parte da formação cidadã), da vivência no ambiente natural proporcionada aos alunos (que possibilita o despertar de sentimentos e emoções) e da contribuição ao trabalho docente (apoio para a realização de atividade prática e momento de relaxamento, com diminuição do desgaste profissional). Para a potencialização e ampliação dessas contribuições, em direção à perspectiva crítica da educação ambiental, são propostos aspectos pedagógicos e institucionais a serem considerados na visitação escolar monitorada em áreas protegidas. / Visits to non-formal places, such as in protected areas, have been used in school education. Researches show the importance of these visits as well as some weaknesses. However, there is a need to understand the contribution of these moments to the school. Thus, this research goal was to analyze the existence of contributions from these visits to school and, in case of affirmative answer, typify them considering visitors\' conceptions and practices and their institutional context. Initial data have been collected through a questionnaire. This allow a mapping of protected areas from the Forest Institute and Forest Foundation, both units of Environment Secretariat of São Paulo State that develop public use activities with scholars and teachers, focused on monitored school visits. From the answer of the questionnaire, two areas of different units have been selected for analyses. This study also holds observations of monitored school visits and interviews of representatives from the areas and teachers from schools that have visited the venue. It also collected documents of these areas. For analysis, were considered references in school education field and in environmental education in a critical view as well as normative documents regarding environmental education in protected areas. It has been observed that in 72% (forty nine) of sixty eight protected areas administrated by the Forest Institute and Forest Foundation that answered the questionnaire public use activities are being developed with scholars and teachers, giving that monitored school visits occurred in 66% (forty five areas), with the presence of tracks in most of them. The point of view of everyone involved show contributions to school education, emphasizing the experience provided to students. From the three visits followed, only one proposed a different explanation methodology, with objects search during the track. In all of them, prevailed biological information, without any promotion of conservation in the pedagogical practice and without relation with a critical perspective of the environmental education. Thus, contributions are limited to its importance as students\' personal experiment moment in a natural habitat, according pointed by interviewers, as well as a complementary practice. Considering these results, has been confirmed the hypothesis that monitored school visits contribute to scholar education. This contribution occurred by visits opportunity (as a right and part of citizen training), by providing students to live in a natural habitat (allowing feelings and emotions to awake) and by contributing to teachers work (supporting the development of a practical activity and a relax moment which can reduce work-related strain). In order to enhance and amplify all these contributions toward a critical perspective of environmental education, this study proposes pedagogical and institutional aspects that must be considered in monitored school visits in protected areas.

It is explained how the game of sexual interests is developed within prisonâs daily activities involving women with their male and female partners (dealing as well with homoaffective and bisexual relations), people which live not only within but outside the jail, compounding in this way the concept of âIntimate Visitâ and demonstrating how this practice is constructed in order to help female inmates lead a sexual life, relating with a male or female partner and maintaining bonds with their families. Besides affective and familiar relations, other types of relations are considered in this work in respect to the complexity of life within prison walls including relations with social agents and policemen. Therefore, it is also considered the role of those social agents as characters who populated as a whole the prisonâs universe and whose postures, decisions and actions investigated during the field research are important to the analysis.

If so, I would inquire the specifics of granting private visit in the female prison (general purpose). The specific purposes were: identify the legal, bureaucratic and socio-cultural barriers that hinder, impede or distort the private visit in this prison; reveal what do women in jail think about their sexuality, how are their experiences, what are their wishes and worries; point (possible) individual and social benefits achieved with the full exercise of sexuality in prison. Therefore, I performed a possible observation of the field, followed by in-depth interviews, from a semi- structured script, to research gender relations, sexuality and human rights in that women's prison. This research advances toward interdisciplinarity, dialoguing with Law, but not limited to the study of legislation nor to the verification of the implementation of the right to equality in women's prison. We start from the dialectical conception proposed by Roberto Lyra Filho (1982), according to which the law is liberation/emancipation ("found in the street"), and move forward on issues related to gender relations, identity, social representations, body control and crime, that requires a deconstruction of disciplinary walls allowing the intersection and the dialogue between Law and Anthropology, Sociology, Criminology and Human Rights in addressing the proposed issue, which would not be possible for a fragmented science. After the field observation, we concluded that granting conjugal visits in that prison does not result from the understanding that it is a right. Regardless the official speech, prison’s administration, under the argument of ensuring private visit in Consuelo Nasser, uses it as a bargaining object, violating women’s dignity. There, as indeed in other women's prisons in the country, women, mostly black and mostly poor, are (will be) exposed to the same social abandonment they were on the street. Prison, now of their own, once again, will establish their family relationships. How to socially reintegrate women who have never entered? How to protect them if their bodies are known to be used by the state to ensure peace in the male prison? How to value them, if they are not recognized as rights holders? When women under the guard and state protection, are taken - in its possible connotations - to exchange sex for money, to keep the drug addiction or to buy food, they are not free in the exercise of their sexuality. There, the right to sexuality is not emancipatory. Because of precariousness and abandonment (social and family), observed in a prison for women, conjugal visits turned into an instrument of oppression and subjugation, impairing the democratic right to sexuality and especially against human dignity. / O presente trabalho é resultado de pesquisa realizada, no mestrado, na penitenciária feminina Consuelo Nasser, com o objetivo de verificar se nesse estabelecimento prisional o exercício da sexualidade, por meio da visita íntima de cônjuges e companheiros/as, é assegurado. Em caso positivo, perquiriria as especificidades da concessão da visita privada no cárcere feminino (objetivo geral). Em seguida, busquei identificar os entraves legais, burocráticos e socioculturais que dificultam, impedem ou desvirtuam a visita privada; revelar o que pensam as mulheres em situação de cárcere sobre a sua sexualidade, como são suas experiências, quais são seus desejos e suas preocupações; e apontar os (possíveis) benefícios individuais e sociais alcançados com o pleno exercício da sexualidade no cárcere. Para tanto, foi realizada uma observação possível do campo, seguida de entrevistas em profundidade, a partir de um roteiro semiestruturado, para pesquisar as relações de gênero, sexualidade e Direitos Humanos no cárcere feminino. A presente pesquisa avança em direção à interdisciplinaridade, dialogando com o Direito, mas não se limitando ao estudo da legislação que trata da visita íntima, tampouco à verificação da implementação do direito à igualdade no cárcere feminino. Partimos da concepção dialética de Roberto Lyra Filho (1982), segundo a qual o Direito é libertação/emancipação (“achado na rua”), e avançamos sobre questões relativas ao gênero, identidade, representações sociais, controle corporal e criminalidade, o que exige uma desconstrução dos muros disciplinares, permitindo o cruzamento e o diálogo do Direito com a Antropologia, a Sociologia, a Criminologia e com os Direitos Humanos no enfrentamento da problemática estabelecida, o que não seria possível a uma ciência fragmentada. Da observação em campo concluímos que a concessão da visita íntima, no estabelecimento prisional pesquisado, não decorre da compreensão de que se trata de direito da/o presa/o. Embora o discurso oficial seja diferente, a administração penitenciária, sob o pretexto de assegurar a visita privada na Penitenciária Feminina Consuelo Nasser, a utiliza como objeto de troca, violando a dignidade da mulher presa. Lá, como certamente em outras unidades prisionais femininas no país, mulheres, quase sempre negras, e quase sempre pobres, são (serão) novamente submetidas ao abandono social que conheceram na rua. O cárcere, agora o delas, mais uma vez pautará suas relações familiares. Como reinserir socialmente mulheres que nunca estiveram inseridas? Como protegê-las, se seus corpos são sabidamente utilizados pelo Estado para conter ânimos no presídio masculino? Como valorizá-las, se sequer são reconhecidas (se reconhecem) como titulares de direitos? Quando mulheres, sob a guarda e a proteção estatal, são levadas  em suas possíveis conotações  a trocar sexo por dinheiro, para manter o vício em drogas ou para comprar comida, não são livres no exercício de sua sexualidade. Lá, o direito à sexualidade não é emancipatório. Em razão da precariedade e do abandono (social e familiar) que marcam o cárcere feminino, a visita íntima se transformou em um instrumento de opressão e subjugo, atentando contra o direito democrático à sexualidade e, principalmente, contra a dignidade humana.

Este trabalho apresenta uma discussão sobre comunicação museológica e de que maneira as instituições trabalham a relação entre museu, público e exposição. Considera-se que a comunicação só se estabelece efetivamente quando sua forma e seu conteúdo mediam, simultaneamente, emoção e informação. Assim, a inserção de \"visitas teatralizadas\" em instituições museológicas pode oferecer o conhecimento não só pela informação em si, mas um conhecimento que, segundo Scheiner (2003), parte da informação transformada pela emoção, podendo então ser vivenciada. Do mesmo modo, o museu precisa dominar outras formas de linguagem, outros sistemas de comunicação que não sejam os habituais. A dissertação estuda e promove a compreensão sobre novas formas de estabelecer um diálogo, neste caso, via \"visitas teatralizadas\". / This paper presents a discussion on museological communication and how the institutions work the relationship among museum, public and exhibition. It is consider that communication is only established effectively when its form and its content mediates simultaneously emotion and information. Thus, the inclusion of \"guided visits with theater\" in museological institutions can provide knowledge not only for the information itself, but knowledge which, according to Scheiner (2003), part of the information transformed by the emotion and can then be experienced. Similarly, the museum must dominate other forms of language, other communication systems that are not usual. The dissertation studies and promotes the understanding of new ways to establish a dialogue, in this case via \"guided visits with theater\".

Eftervårdsbesök vid barnmorskemottagning post partum : Utveckling av Basprogrammet för vård under graviditet i Stockholms läns landsting

Background: Within the Stockholm County Council returned in 2013 around 68 percent of the women to the antenatal clinic for a check-up after childbirth. Earlier studies show that new mothers want greater support from maternal health care and do not always know where to seek care. Aim: The aim of this work was to investigate whether the use of a standard model describing midwifery care and procedures for the postpartum check-up could affect the number of women who return to Södermalms clinic. A further aim was to compare the midwife and the patient's perspective, about what were included and discussed at this postpartum check-up. Method: This study had a descriptive quantitative research approach. How and when the woman contacted, and what topic the midwife discussed with the woman during the postpartum visit, was well described in a flowchart. 112 women were enrolled in the study and the project could be evaluated on the basis of data from the Pregnancy Registry and a control group corresponding to the intervention group was appointed. Results: Registered data showed an increased number of women who returned to Södermalms clinic postpartum, in the control group, there was no increase in the percentage of women who came to postpartum visits. There was a difference between the midwife and the woman's perception of what was discussed at the postpartum visit, particularly around the   information about the ruptures and the pelvic floor muscle, there was a discrepancy.  The midwives participating in the study preferred after completion of the study a more structured working model, where the postpartum visits were clarified. Conclusion: There may be advantages to use a standard model describing the midwifery care and procedures for the postpartum visits. The midwives preferred to book in the woman's postpartum check-up during her last pregnancy visit and were more satisfied with the new approach.
