Värdeskapande användning av radiologi : - Utbildning och mätning för förbättring

Introduktion Medicinsk vetenskap och sjukvårdens möjligheter att hjälpa utvecklas ständigt. Sjukvården idag kan i många fall ställa rätt diagnos och ge en effektiv behandling för att bota den som drabbats av ohälsa. Radiologiska undersökningar är ett viktigt hjälpmedel men innebär också risker i form av strålning samt felaktigt resursanvändande. Syfte Syftet med förbättringsarbetet var att skapa en bättre användning av radiologi och följsamhet till medicinska riktlinjer så att patienten får rätt undersökning utifrån sitt behov samt att resur-ser nyttjas mer optimalt. Målsättningen var att öka andelen berättigade undersökningar. Syftet med studien av förbättringsarbetet var att få en förståelse för vilka faktorer som påverkar remittentens val av radiologisk undersökning och därmed berättigandegraden vilka aktiviteter i förbättringsarbetet påverkar berättigandegraden och på vilket sätt Metod Interventioner i form av utbildning och mätningar användes för att höja berättigandegraden. Bedömning av berättigandegraden gjordes av en ST-läkare. En fallstudie med kvalitativ ansats genomfördes och fokusgruppintervju användes för att stu-dera förbättringsarbetet. Resultat Studien visar att berättigandegraden ökar något som ett resultat av de valda interventionerna. Utbildningen på plats gav också upplevda positiva effekter i form av bättre kunskap och lä-rande för deltagare. Analysen visade att osäkerhet som läkare upplever i sitt arbete med patienten kan härledas till kategorierna kunskap och krav. Diskussion/Slutsats Utbildning genomförd av radiologispecialist har visat sig vara framgångsrikt koncept. Mät-ningar som metod för lärande och förändring uppfattades istället som mätning för uppföljning. Förbättrad kunskap kring vilka faktorer som styr läkares val av undersökning har uppnåtts och områden för fortsatt förbättring har identifierats / Introduction Medical science and medical facilities and clinical possibilities to help patients evolve con-stantly. Healthcare today can often make the diagnosis and provide effective treatment to cure the victim of ill health. Radiological surveys are an important tool but also provide risks in the form of radiation, and improper use of resources. Purpose The purpose of the improvement work was to create a better use of radiology and adherence to medical guidelines so that the patient gets the right for increase based on their needs and resources will be used more optimally. The goal was to increase the proportion of eligible studies. The purpose of the study of the improvement was to gain an understanding of what factors affect physicians choice of radiological investigation and thereby provide entitlement degree which activities in the improvement process affects eligibility degree and in what way Method Interventions in the form of education and measurements used to improve eligibility rate. As-sessment of the eligibility rate was made by a resident physician. A case study with a qualitative approach was implemented and focus-group interviews were used to study the improvement process. Results Results indicates that the eligibility rate increased slightly as a result of the selected interven-tions. Education in place, however, gave perceived benefits in terms of improved knowledge and learning for participants. The analysis showed that the uncertainty that physicians experience in their work with pa-tients can be attributed to the categories of knowledge and requirements. Discussion / Conclusion Education conducted by a radiology specialist has is shown to be a successful concept. Meas-urements as a method of learning and change were perceived as measurement for monitoring. Improved knowledge about the determinants of physician choice of survey has been achieved and areas for further improvement are identified.

A utilidade das visitas oficiais: a percepção dos empresários que integram as comitivas oficiais

Economia e Gestão Internacional / Master in International Economics and Management / A participação de empresários em visitas oficiais realizadas por Chefes de Estado ou de Governo é uma forma das entidades oficiais promoverem a internacionalização e as exportações das empresas. Existe um vasto corpo de literatura sobre estudos que se debruçaram sobre a promoção das exportações e sobre a avaliação de programas de promoção das exportações. Contudo, até à data, parece não ter sido avaliada a utilidade das visitas oficiais como meio de promoção das exportações. Nesse sentido, este estudo é um contributo para a literatura teórica e empírica. É apresentada uma base teórica para fundamentar a utilidade das visitas oficiais, elaborada a partir da literatura existente sobre a promoção das exportações, programas de promoção das exportações e missões comerciais. A nível empírico, este estudo aferiu a percepção dos empresários portugueses acerca da utilidade das visitas oficiais, através da realização de um inquérito aos empresários que participaram em 12 visitas oficiais organizadas pelos Chefes de Estado e de Governo, entre 2005 e 2008. Adicionalmente, o estudo propõe um modelo multivariável que analisa em que medida as características estruturais das empresas, e, as características associadas à visita oficial, podem afectar o modo como os empresários avaliam a utilidade das visitas oficiais para a concretização de determinados objectivos. Os resultados sugerem que a qualidade dos contactos e das informações obtidas durante a visita é um factor estatisticamente significativo e contribui positivamente para todos os objectivos avaliados. Os resultados revelam ainda, que factores como a dimensão, o capital estrangeiro, a intensidade exportadora, a intensidade de inovação, a experiência exportadora no mercado visitado, a existência de contactos ou negócios prévios, ou o facto de ser a primeira vez que a empresa visita o mercado, tendem a afectar o modo como as empresas avaliam a utilidade das visitas oficiais para a realização de determinados objectivos. / The participation of businessmen in official visits done by Heads of State or Government is a way found by official authorities to promote exports and the internationalization of the firms. There is a large body of literature about export promotion and the evaluation of export promotion programs. However, to date, it seems has not been evaluated the usefulness of official visits as a way to promote firms exports. This study provides a theoretical and empirical contribution to the literature. The study presents a theoretical basis to substantiate the usefulness of official visits found in the literature about export promotion, export promotion programs and trade missions. A survey was done questioning Portuguese businessmen who had participated in 12 official visits to evaluate the usefulness of those visits. Additionally, the study proposes a multivariable model to examine how firm and visit caractheristics do affect, or not, the firms evaluation about official visits. The estimated results suggest that the quality of contacts and information gained during the visit positively affect the impact over a set of objectives. The results also reveal that factors such as firm s size, foreign ownership, export intensity, R&D intensity, experience in the market, the existence of previous contacts or business, or first visit to the market affect the firms evaluation about some objectives of official visits.

Decreasing Primary-Care-Related Emergency Department Visits in the Hispanic Population Using Patient Navigators

Primary-care-related emergency department (PCR-ED) visits by Hispanic patients enrolled in a not-for-profit Medi-Cal and Medicare health plan resulted in longer wait times in the emergency department (ED) at a regional medical center in Southern California. This congestion decreased access for those with true emergencies, created capacity issues, increased ED length of stay, and resulted in potential safety risks. This project focused on decreasing PCR-ED visits in the Hispanic population using patient navigators in Southern California. Applying the health belief model and Lippitt's theory of change, this doctoral project involved the creation and implementation of a culturally appropriate, population-specific patient navigator model for the Hispanic population. Evaluation of outcomes was accomplished using electronic health record (EHR) results, which demonstrated a reduction of PCR-ED visits and revisits. The project exceeded the goal of 10% reduction in PCR-ED visits and revisits in the target population and resulted in a 14.31% reduction of PCR-ED visits and revisits within 1 month of implementation. Emergency Severity Index levels, wait time associated with each Emergency Severity Index level, and visits of the targeted Hispanic population enrolled in the health plan were analyzed to evaluate the success of the program. This project may lead to improvements in nursing practice and positive social change by supporting population health management and continuum of care to a primary care physician through safe and efficient patient navigation to treatment and care.

The role of materiality in transnational family relationships of Czech migrants in Sweden

The aim of this thesis is to analyze the transnational family relationships of Czech pre-1989 political émigrés and post-1989 love/ economic migrants in Sweden and their homeland-based relatives, by looking at the practices via which these relationships are initiated and maintained and the role of materiality in these practices. The theoretical framework builds on the notion of “transnationalism from below” as a perspective which intersects migration and family studies, and posits the focus on material culture as an effective analytical tool. After setting the research in the context of Czech and Czechoslovak migration in the 20th century, qualitative analysis of life history narratives and ethnographic interviews is used to investigate the topic in question. Considering the influence of historical and individual factors, the study identifies the parallels and divergences in the two migrant groups’ practices of long-distance communication and mutual visits and in their attitudes to the role of materiality in transnational family relationships. The thesis concludes by stating that in contrast to the pre-1989 émigrés, the post-1989 migrants’ transnational connections with the homeland-based kin are more frequent and intensive. While material aspects play a more significant role in the post-1989 migrants’ transnational family relationships, material differences are more pronounced in the pre-1989 émigrés’ relationships. The historical circumstances of migration, the individuals’ perceptions of their own acts of migration as voluntary or forced and the question of whether or not they were given a license to leave by their homeland-based kin are said to have a significant impact on relationship initiation, the practices of relationship maintenance and the inherent role of materiality. The importance of individual-level enquiry of the migration experience is thus emphasized.

Uppsökande verksamhet i Osby kommun ur äldre personers perspektiv

Syftet med denna studie var att beskriva äldres upplevelser och erfarenheter av uppsökande verksamhet i Osby kommun. Förebyggande hälsoinsatser för äldre bedrivs i Sverige med olika benämningar. I Osby kommun bedrivs de med benämning uppsökande versamhet. Uppsökande verksamhet med förebyggande insatser och råd kan stärka äldres egenvård och skjuta upp funktionsnedsättning samt förbättra den äldre personens välbefinnande och livskvalitet. Ur samhällsekonomiskt perspektiv kan uppsökande verksamhet också minska kostnader för vård och omsorg på sikt. Metoden är en kvalitativ intervjuundersökning med öppna frågor. Studie genomfördes under Juni- November år 2015 och tio äldre personer intervjuades. Av dessa var det fem kvinnor och fem män. Intervjuerna analyserades enligt Graneheim och Lundmans kvalitativa innehållsanalys. I resultatet framkom två kategorier och sju subkategorier. Essenserna som hittades från intervjuerna var: förväntningar, tillit i mötet, attityder till förändring, information, utförarens kompetens och personlighet, vikten av relation och kreativitet. Resultatet visar tydligt att äldre upplever arbetet med uppsökande verksamhet värdefullt och vill se detta arbete fortsätta. Denna studie ger utan tvekan stöd att utföra uppsökande verksamhet och uppsökande verksamhet kan ses som en redskap för de äldre att skapa bra förutsättningar att bevara hälsan och ger möjlighet till ett aktivt och självständigt liv. / The purpose of this study was to describe elderly persons experiences of preventive home visits in the Osby municipality. Preventive home visits with preventive actions might help elderly `s self-care abilities and postpone the effects of ageing and disability, in order to give elderly better well-being and good quality of life. This qualitative study and data was collected by qualitative interviews using open queries. The respondents were ten elderly persons (five females and five males). Interviews were carried out during June to November 2015. The material was analysed using Qualitative content analysis as described by Graneheim and Lundman. The analysis resulted in two categories and seven subcategories. In summary the subcategories were: expectations, confidence in the meet, attitudes on change, information, providers competence and personality, importance of relationship and creativity. The results of the study clearly indicates that the preventive home visits are appreciated and valuable to the elderly. All respondents claimed that the visit was a positive event in their lives and want to see it continue in the future. This study gives evidence to carry out the preventive home visits. The preventive visits gives elderly better conditions for maintaining a healthy and satisfactory ageing and independent life.

Bakgrunn: Røyking blir fortsatt betraktet som den viktigste forebyggbare årsak til død i ge land. Praktiserende leger er nøkkelpersoner innen tobakksforebygging. Enkel rådgiving, som minimal intervensjon, gitt av allmennleger øker andelen røykfrie pasienter signifikant. Kan en oppsøkende intervensjon gjennomført av en likemann være en egnet metode for å få leger til å ta opp tobakksbruk oftere og på en bedre måte, og kan denne undersøkelsen påvise dette? Hensikt: Hensikten med denne studien er: 1) Vurdere om primærlegene i Agder har endret praksis i perioden 2001-2004 med hensyn til hvor ofte og hvordan de tar opp tobakksbruk med sine pasienter og eventuelle hindringer for dette. 2) Undersøke om oppsøkende intervensjon er en egnet metode for å få leger til å endre praksis ved at de bidrar mer og bedre med individuell røykeintervensjon. Metode: Randomisert kontrollert studie for å undersøke om oppsøkende intervensjon er en egnet metode for å få leger til å endre praksis. Denne studien er sett i lys av en sammenliknende analyse av legenes røykeintervensjon og hindringer for dette i perioden 2001-2004. Resultat: Allmennlegene i Agder har endret praksis i perioden 2001 til 2004. Legene tar oftere opp tobakksbruk uten av pasientene har røykerelaterte symptomer, og det er færre hindringer både for å spørre om røykevaner og for å tilby hjelp til røykeslutt. Studien kan ikke bekrefte at oppsøkende intervensjonen er en egnet metode for å få leger til endre praksis ved at de bidrar mer og bedre med individuell røykeintervensjon. Konklusjon: I perioden 2001-2004 har det vært en signifikant endring i legenes røykeintervensjon og hindringer for dette. Studien kan ikke påvise at legene som fikk opplæringen ”Røykeslutt i praksis” bidrar mer med individuell røykeintervensjon enn kontrollgruppen, og det er heller ikke signifikante forkjeller på hindringer for en slik intervensjon / Background: Cigarette smoking is still considered the leading preventable cause of death in the western world. Physicians constitutes a key personnel in tobacco prevention. Brief advising, such as minimal intervention, performed by general practitioners, GPs, increases the number of smoke free patients significantly. The potential effect of outreach visits performed by a peer educator is a question to be studied. Objective: The objective of this study is: 1) Assesswhether the primary physicians in Agder, during the period 2001 to 2004, have changed their professional practice as tohow often and in which way they discuss smoking habits with their patients and possible barriers to stop them. 2) The study also seeks to determine whether outreach visits constitute an effective method to make GPs change their professional practice by contributing more and better to individual smoking cessation. Method: Randomised controlled study to determine whether outreach visits constitute an effective method to make GPs change their practice. This study is also viewed in the light of a comparative analysis of doctors’ attitude to tobacco prevention over the period 2001-2004. Result: During the period 2001-2004 the GPs in Agder have changed their professional practice. They discuss more frequently tobacco use with patients without smoke-related symptoms, and there are fewer barriers that keep them from asking about smoking habits and from offering assistance with smoking cessation. The effect of outreach visits in improving professional practice cannot be ascertained through this study. Conclusion: During the period 2001-2004 there has been a significant change in the GPs intervention work and fewer barriers to stop them. The GPs who received training through the program “Røykeslutt i praksis” do not contribute to individual smoke intervention any more than the control group. Analýza vlivu zahraničních aktivit českých politiků na vývoj zahraničního obchodu a investic / Impact of Czech polititians foreign activities on development of foreign trade and investment

With the continuing process of globalization, national economies are intensively interconnecting with external enviroment. It is especially visible for Czech Republic, which geopolitical position stresses the importance of external business relationships. Economic prosperity of countries with open economy is largely determined by export performance and inflow of foreign direct investments, both of which are supported by economic diplomacy. The first scope of this thesis is to evaluate the economic diplomacy of Czech Republic, and secondly to empirically analyze the tools, which economic diplomacy is using - foreign visits of polititians and role of embassies. The gravity model of international trade was used to study the influence of both tools on the foreign trade and investment in Czech Republic between years 1993-2013. The results show, that if at least one visit per year was realized, the export grows by 14%. However, one average high official visit is associated with rising exports by about 4.7%. On the other hand, any realtionship with FDI was not found. Further analysis confirms the importance of the embassies and thier positive impact on foreign trade, in particular with culturally distant countries, where this effect is more pronounced.
