Upplevelser av vård och behandling på en ätstörningsklinik : En enkätstudie

Föreliggande studie undersökte hur kvinnor med ätstörningar upplevde den vård och behandling som gavs för deras ätstörningar. På en klinik i Mellansverige deltog 45 kvinnor. Alla deltagare var över 18 år gamla. Kvinnorna fick besvara en enkät med öppna och slutna frågor rörande välmående, behandlingens innehåll och påverkan, samt interaktionen mellan vårdtagare och personal. Vidare undersöktes vad som skulle kunnat förbättra den behandling som erbjöds, samt om det förekom ett samband mellan antal månader kvinnorna fått behandling på kliniken och upplevelsen av välmående. Resultatet av studien visade att majoriteten av kvinnorna var nöjda med den vård och behandling som gavs. Förtroendet för personalen var stort, och upplevelsen att någon lyssnar och förstår var viktig. Det fanns inget signifikant samband mellan antalet månader vårdtagarna fått behandling och deras uppskattade välmående. Däremot framkom signifikanta skillnader i välmående före behandlingens start och efter att behandling hade påbörjats, oberoende av antal månader behandlingen hade pågått.

Perfectionism, Self-Injurious Behaviour, and Functions of Anorexia Nervosa

The following thesis outlines a study assessing the levels of perfectionism, self-injurious behaviour, and functions of anorexia nervosa (AN) through use of a cross-sectional online survey, among English speaking participants 15 years or older, self-reporting a current, previous, or suspected diagnosis of AN. Three distinct clusters were found using self-report measures from individuals with a current or suspected diagnosis, with each cluster corresponding to a unique theoretical understanding of AN. The three clusters can be distinguished by high asceticism, appearance, and avoidance of fertility/sexuality functions for AN respectively. Two distinct clusters were found for participants with a previous diagnosis of AN. These clusters can be differentiated by lingering sentiments held for the condition, as the first cluster viewed AN negatively, and the second cluster continued to see some benefits of the condition. Possible implications for understanding etiology, mechanisms, and treatment of AN are discussed. / Canadian Institute of Health Research

Maladaptive Rule-Governed Behavior in Anorexia Nervosa: The Need for Certainty and Control

<p>Anorexia nervosa (AN) is a dangerous disorder characterized by unrelenting rigidity that continues even in the presence of deadly outcomes. Despite this, our understanding of factors that promote and maintain rigidity is lacking. The current paper proposes a model suggesting that rigid behaviors in AN can be formulated as maladaptive rule-governed behavior that emerges in contexts of uncertainty and loss of control, such as in the presence of affective arousal. An empirical study examining the differences between individuals weight recovered from AN (AN-WR) and healthy controls (CN) on parameters of rule-governed behavior in neutral and stressful contexts is described. Seventy-four adults (AN-WR: 36; CN: 38) were randomized to undergo either a stressful or neutral mood manipulation and then completed a laboratory assessment of rule-governed behavior, along with questionnaires measuring difficulties with uncertainty. While the AN-WR group demonstrated greater flexibility in rule implementation compared to the CN group, they evidenced greater impairment in behavioral extinction. Furthermore, although affective arousal did not significantly impact rule-governed behavior as expected, difficulties tolerating uncertainty were significantly related to rule-governed outcomes exclusively in the AN-WR group. Taken together, findings provide preliminary support for maladaptive rule-governed behavior in AN and suggest that this is related to an intolerance of uncertainty. Findings and treatment implications are discussed in light of study limitations.</p> / Dissertation

<p>En av de viktigaste aspekterna i omvårdnad är betydelsen av mänsklig kontakt i interaktionen mellan patienter och vårdgivare. Anorexia Nervosa är en komplex sjukdom av psykisk och fysisk karaktär och vårdpersonal spelar en viktig roll för dessa patienters tillfrisknad. Målet med denna studie var att nå en djupare förståelse för den mänskliga kontaktens betydelse i omvårdnaden av patienter med Anorexia Nervosa samt att belysa essensen i en vårdande relation. Syftet med studien var att beskriva Anorexia Nervosa patienters erfarenheter av den mänskliga kontaktens betydelse i möten med vårdpersonalen. En kvalitativ ansats valdes och datainsamlingen grundade sig på sex självbiografier, skrivna av sex unga kvinnor som har varit sjuka i Anorexia Nervosa. En kvalitativ textanalys användes för att tolka data. Åtta teman framkom i resultatet: ”att känna förtroende” ”att få hjälp med att gå till botten med problemet”, ”att inte bli förstådd”, ”att inte våga öppna sig”, ”att inte bli sedd”, ” att få bekräftelse”, ”att känna maktlöshet” och slutligen, ”att någon talar till den friska delen”. Dessa teman speglade dessa personers erfarenheter av den mänskliga kontaktens betydelse i möten med hälso- och sjukvårdspersonal. Denna studie kan bidra med att ge en ökad förståelse för den mänskliga kontaktens betydelse i omvårdnaden av patienter som lider av Anorexia Nervosa samt att hjälpa vårdpersonal att tillmötesgå dessa patienters behov.</p>

O objetivo deste estudo foi avaliar diversidade microbiológica existente na cavidade bucal de pacientes com estes transtornos, por meio de técnicas de cultivo e utilizando métodos moleculares independentes de cultivo. Foram incluídos no estudo 32 pacientes anoréxicos e 27 bulímicos, pareados com 59 indivíduos controle. Amostras de enxágüe bucal foram semeadas para a avaliação da prevalência de leveduras do gênero Candida, estafilococos, enterococos, estreptococcos do grupo mutans (EGM), lactobacilos, enterobactérias/pseudomonas. Espécies de Candida, estafilococos, enterococos, enterobactérias/pseudomonas foram identificadas pelo sistema API. Amostras de biofilme supragengival foram coletadas e utilizadas somente nos procedimentos moleculares. As contagens de microrganismos nos grupos foram comparadas por ANOVA/Mann-Whitney (5%). Houve diferença estatisticamente significantes (p<0,05) para as contagem de leveduras do gênero Candida, estafilococos, enterococos, EGM e lactobacilos entre o grupo TA e controle, mas não houve diferenças significativas para a prevalência de enterobactérias/pseudomonas (p=0,312). Pequena diferença entre os grupos foi observada na diversidade de espécies dos microrganismos estudados pelo método de cultivo. Avaliação molecular foi realizada pela ribotipagem por seqüenciamento do 16S rRNA bacteriano e regiões D1/D2 do 28S rRNA. Foram avaliados cerca de 3000 clones do grupo TA e 1500 clones do controle. The aim of this study was to evaluate the microbial diversity in the oral cavity of patients with Anorexia Nervosa and Bulimia nervosa by cultivation techniques and cultivationindependent molecular methods. The study included 32 patients and 27 bulimic anorexics, matched with 59 control subjects. Oral rinse samples were cultured to assess the prevalence of Candida species, staphylococci, enterococci, streptococci mutans (EGM), lactobacilli, Enterobacteriaceae / Pseudomonas. Candida species, staphylococci, enterococci, Enterobacteriaceae / Pseudomonas were identified by API systems. Supragingival biofilm samples were collected and used only in molecular procedures. Counts of microorganisms in the groups were compared by ANOVA / Mann-Whitney (5%). There was a statistically significant (p <0.05) for the counting of yeasts, staphylococci, enterococci, and lactobacilli EGM between TA and control groups, but there were no significant differences in the prevalence of Enterobacteriaceae / Pseudomonas (p = 0.312). Few differences between the groups were observed in the species diversity of organisms studied by the method of cultivation. Molecular analysis was performed by ribotyping by sequencing the 16S rRNA bacterial and D1/D2 regions of 28S rRNA. About 3000 clones of the TA group and 1500 clones of control were evaluated. Sixty-two species or filotypes of bacteria were detected, with 22 identifications were found only in the study group, only 6 in the control group and 34 in both groups.
