Schizophrénie et dépression : Recours aux services avant et après une hospitalisation pour tentative de suicide

Contexte : Une tentative de suicide antérieure et les troubles mentaux constituent d’importants facteurs de risque de suicide. Les services de santé ont un rôle important à jouer en matière de prévention. Objectif : Analyser les patrons de recours aux services à des fins de santé mentale avant et après une hospitalisation pour tentative de suicide chez les résidents montréalais qui ont reçu un diagnostic de schizophrénie ou de dépression. Méthode : Les données proviennent de la banque médico administrative jumelée de l’Agence de Montréal. Les caractéristiques des patients et les taux de contacts avec les services trois mois avant et après l’hospitalisation index furent comparées. L’échantillon représentatif compte 525 Montréalais hospitalisés à la suite d’une tentative de suicide (avr. 2003-déc. 2004) qui ont reçu un diagnostic de schizophrénie ou de dépression. Résultats : Le recours aux services a augmenté de manière significative suivant l’hospitalisation index. Les patients déjà en contact avec les services et les hommes avec comorbidité en termes d’abus de substances semblent en contact avec les services au cours des trois mois suivant leur hospitalisation contrairement aux femmes avec comorbidité. Le profil « urgence » de recours aux services semble prédire une absence de recours aux services. Conclusions : Les services de santé répondent aux tentatives de suicide, particulièrement chez les hommes avec troubles d’abus de substances. Cependant, des interventions sont requises en vue d’améliorer la coordination des services, principalement pour les personnes qui se présentent l’urgence, les femmes avec troubles d’abus de substances et les patients en marge du système avant leur hospitalisation. / Background: Mental health disorders and suicide attempts are important risk factors for suicide. Health care services have an important role to play in preventing these deaths. Objective: To analyze patterns of service use for a mental health purpose before and after a hospitalisation following a suicide attempt among Montreal’s residents diagnosed with schizophrenia or depression. Method: Data was taken from the linked administrative database of the Montreal’s Agency. Patient’ characteristics and contacts rates with services three months before and after the index hospitalisation were compared. The representative sample includes all 525 patients living in Montreal, who were hospitalised following a suicide attempt (Apr. 2003-Dec. 2004) and who were diagnosed with schizophrenia or depression. Results: Intensity of care increased significantly after the index hospitalisation. Patients in contact with services before and comorbidity of substance abuse disorder among men predicted a contact with services for a mental health purpose three months after the index hospitalisation contrary to women with comorbidity. Besides, the emergency room profile of service use predicted a lack of service utilisation. Conclusions: Health services respond to suicide attempts, including in men with substance abuse. However, there is an opportunity for increased aftercare coordination for suicidal individuals, in particular for those presenting at the emergency room, those with no previous contact with services and for women with substance abuse.

The thesis focuses on the rendering of the poetics of Anna Akhmatova and Marina Tsvetaeva in Czech translations. The first part deals with the phenomenon of poetesses in the Russian literature and subsequently with Akhmatova's and Tsvetaeva's life, works, style and reception. The second part is focused on contrastive translatological analysis of different approaches to their poetry and detailed analysis of existing Czech translations of Akhmatova's poem Rekviem and Tsvetaeva's work Popytka revnosti. It defines the approach of all translators to translation and evaluates the quality and adequacy of individual translation solutions in relation to the author's style and intention.

The prevalence of suicidal rates varies from culture to culture and from time to time in the same culture. A growing tendency for female, adolescent and young adult has been observed. The current study attempts to describe the psychosocial profile of 40 patients attended in HB-FAMERP Hospital casualty due to suicide attempts. Method: A combination of measures was used while patients were in hospital. The Attempting Suicide Questionnaire, the Hamilton Depression Scale (Ham-D), the Beck Scale for Suicidal Intention (BSI), Two-question Questionnaire and the Goldberg Scale for Depression. Results: Results demonstrated that the male proportion was 37,5% and the female proportion was 62,5%. Self-poisoning was the most used method (95%): 50,3% by psychotropic drugs and 27,7% by pesticides. The average score for male and female was 9,86 (BSI) and 18,8 (Ham-D), and 15 (BSI) and 28,36 (Ham-D), respectively. Patients who had no history of previous suicide attempts were 52,5%. Affective Disorders with depressive episode were identified as diagnostic hypothesis in 87,5% of sample, in which 54,3% was considered grave, 25,7% considered moderate, and 20% tenuous. Family income was lower than 3 minimum wages for 73,5% and per capita family income was up to a minimum salary for 87,5% of the total. Conclusions: Patients are mainly young women, people from working class and with low intent. Self-poisoning is the commonest method. Affective Disorder is the usual diagnosis and most patients presented depression symptoms. / Introdução: Suicídio é a morte intencional auto-infringida. A prevalência das taxas de suicídio é variável em diferentes culturas e varia com o tempo numa mesma cultura. Tem-se observado consistente tendência de aumento da proporção feminina, adolescente e adulto jovem. O presente estudo pretende descrever o perfil psicossocial de 40 pacientes atendidos na emergência do HB-FAMERP devido à tentativa de suicídio. Casuística: Com os pacientes ainda internados os seguintes instrumentos foram aplicados: Questionário de Tentativa de Suicídio (QTS), Escala de Hamilton para Depressão (HamD), Escala de Intencionalidade Suicida de Beck (BSI), Teste das Duas Questões e Escala de Goldberg para depressão. Resultados: Dos 40 casos 37,5% foram do sexo masculino e 62,5% do sexo feminino. O auto-envenenamento foi o método mais utilizado (95%), desses 50,3% por medicação psicotrópica e 27,7% por pesticidas. As pontuações médias para os sexos masculino e feminino foram 9,86 (BSI) e 18,8 (Ham-D), e 15 (BSI) e 28,36 (Ham-D) respectivamente. 52,5% dos pacientes não haviam tentado suicídio anteriormente. 87,5% da amostra tiveram como hipótese diagnóstica Transtorno Afetivo com episódio depressivo, sendo 54,3%% considerados graves, 25,7% moderadas e 20% leves. 73,5% tinham renda familiar inferior a 3 salários mínimos e 87,5% renda familiar per capita até 1 salário. Conclusão: Os pacientes são predominantemente mulheres jovens, de classe socioeconômica baixa, com baixa intencionalidade e auto-envenenamento é o método preferencial. Transtorno Afetivo foi o diagnóstico mais encontrado e a maioria dos pacientes revelou sintomas de depressão.

Cette thèse analyse la surmortalité par suicide sur le territoire du Pays COB au travers des facteurs psychosociaux et psychopathologiques et repose sur l'hypothèse d'interactions entre les conceptions sociales du suicide et les trajectoires personnelles des individus, freinant sa prévention. Une étude des représentations sociales du suicide montre qu’il existe des RS spécifiques en Bretagne et en Pays COB. L’étude des trajectoires de vie met en évidence une sur-représentation de la dépression et les troubles de la personnalité. On trouve aussi une faible estime de soi, une difficulté d'accès à la parole et à la demande d’aide, des violences transgénérationnelles et actuelles, un climat incestuel et une stigmatisation (auto et hétéro) liée aux RS du suicide, de la dépression et des « psy ». L’étude de l’effet d’un suicide montre qu’il a un impact sur les proches : traumatisme psychique individuel et familial, deuils traumatogènes. À la suite de ces résultats, des actions spécifiques pourront être menées sur ce territoire pour améliorer la prévention du suicide et des tentatives de suicide. / The present thesis analyzes the role of psychosocial and psychopathological factors in the abnormally high rate of deaths by suicide in the “Pays COB” territory. It is based on the hypothesis according to which the interactions between the social conceptions of suicide and the personal trajectories hinders suicide prevention. We studied the structure of social representations of suicide (SR) with Vergès’method (double analysis of the evocation rank) and uncover specific SR in Brittany and in “Pays COB”. A study of the life trajectories highlighted the excessive rate of depressions and personality disorders. We also found amoung suicide victims a weak self-esteem, a difficulty to talk and to ask for help, transgenerational and current violent tendencies, an incestual climate and (self-) stigmatization related to the SR of suicide, depression and psychiatric services. A study about the effect of a suicide showed that it has an impact on the close relatives: individual and family psychic trauma, traumatic grief. Following these results, specific actions can be carried out in this territory to improve the prevention of suicide and of suicide attempts.

The indirect victim's experience of suicide and the victimisation resulting from the incident was studied. The Indirect Victim's Experience Model of a Suicide Incident (see Figure 2.1) was designed as a theoretical framework to direct the research findings and for the interpretation of data. A qualitative exploratory approach was used and a group of 20 respondents was selected by means of purposive and snowball sampling methods. All the respondents were interviewed personally. The interpretation and analysis of the data showed that the assumptions based on Janoff-Bulman and Frieze's theory and certain societal myths regarding suicide still exist. These assumptions and myths could subconsciously have an influence on the nature and extent of the victimisation experienced by the next of kin. The respondents themselves accepted these assumptions and myths. Research findings further showed that these assumptions are affected upon receiving the news of the suicide, while myths are destroyed. This could have an influence on the victim's experience of suicide. Every indirect victim functions before, during and after the suicide incident within a specific family and social system. The way in which the indirect victim experiences the support and integration from within the family and social system, determines the acceptance or non-acceptance of the death by suicide of a significant other. This will also have an influence on the indirect victim's reintegration into society in the short and long term. The need of the indirect victim to be involved within a family and society was clearly identified during the study. These findings made it possible to make practical recommendations with regard to the indirect victim's experience of suicide. Society should be informed about the realities and results of suicide. Beyond the Standard

In order to contribute to equality, we must go beyond the standard. I want to learn from people with disabilities and the spatial border they are faced with, by exploring the responsibility I have been given, through claiming a role that affects our built environments. I have experience with disabilities and handicaps, but these experiences do not automatically make me better in addressing issues concerning other people with disabilities. I am dyslexic and have been through big obstacles, particularly in the educational system, and I have my Aunt, who has an intellectual disability. I am privileged to have these experiences, and with this thesis I have explored them as an interior architect. This is where I start on my way to contribute.  This thesis is a critical study of how standards are practiced, examined through housing built for people with intellectual disabilities in Selbu, Norway. In the center of the critique is the institution. Institutionalization and standardization have a long relationship as answers to complex questions. Institutions’ uniformity ultimately belongs to no one. The matter becomes particularly problematic when the environment is supposed to operate someone’s home. Housing for people with intellectual disabilities is often life long and not determined on the intention of the builders. What is obvious to me after reading the standards and exploring the buildings, is that the minimum requirements are always in place, often built as if the minimum was the goal. But a standard also expresses its vision. The vision talks about inclusion and the dangers with mini institutions, but the vision is expressed through soft language and the minimum requirements are expressed through concrete language of measurements. From the process of being a standard to becoming a building the soft language evaporates from the building plans. Lithium Reduces Pathological Aggression and Suicidality: A Mini-Review

From a practical point of view, the well-proven antisuicidal and anti-aggressive effects of lithium are of utmost importance for a rational, safe and economical treatment of patients with affective disorders. Regular lithium long-term treatment reduces the otherwise 2- to 3-fold increased mortality of untreated patients with severe affective disorders down to the level of the general population. This is mainly due to the reduced suicide risk. Many international studies have confirmed this fascinating property of lithium which so far has not been demonstrated with comparable evidence for any other psychotropic compound. The antisuicidal effects of lithium might possibly be related to its anti-aggressive effects which have been shown in various species, populations and settings, such as animals, inhabitants of nursing homes for the elderly, mentally handicapped subjects, children and adolescents with hyperactive, hostile and aggressive behavior, and particularly in hyperaggressive inmates of correction units and prisons. / Dieser Beitrag ist mit Zustimmung des Rechteinhabers aufgrund einer (DFG-geförderten) Allianz- bzw.
