<p>Uvod: Individualno kreiran program vežbi lumbalne stabilizacije, mogao bi da doprinese boljem oporavku pacijenata sa hroničnim bolom u donjem delu leđa. Program vežbi u lečenju hroničnog lumbalnog bola je prilično raznolik, ali je dokazano da su vežbe stabilizacije najefikasnije, posebno vežbe u zatvorenom kinetičkom lancu. Cilj: je bio da se uporedi program vežbi lumbalne stabilizacije u otvorenom i zatvorenom kinetičkom lancu, u odnosu na program vežbi lumbalne stabilizacije i torakalne mobilizacije u zatvorenom kinetičkom lancu kao i da se proceni klinička efikasnost oba programa vežbi. Metode: Istraživanje je sprovedeno kao prospektivna, eksperimentalna studija na 80 ispitanika oba pola (35 muškaraca, 45 žena), prosečne životne dobi (48.45+/- 10.22 godina) sa hroničnim lumbalnim bolom. Istraživanje je sprovedeno na Klinici za rehabilitaciju „dr Miroslav Zotović“ u Beogradu u periodu od juna 2017. do marta 2018. godine. Ispitanici su podeljeni u dve grupe od 40 pacijenata. Prva grupa ispitanika je sprovodila program vežbi lumbalne stabilizacije i torakalne mobilizacije u zatvorenom kinetičkom lancu (LSTMZ), a druga program vežbi lumbalne stabilizacije u zatvorenom i otvorenom kinetičkom lancu (LSZO). Pored opštih demografskih, antropometrijskih podataka o ispitanicima, korišćeni su sledeći upitnici i testovi: za procenu intenziteta bola u leđima i donjim ekstremitetima- vizuelna analogna skala (VAS), za procenu neuropatske komponente bola- DN4 (Douleur Neuropathique en 4 Questions), za procenu funkcionalnog statusa korišćen je Osvestrijev upitnik o onesposobljenosti (ODI), procena sagitalne pokretljivosti lumbosakralne kičme Schober testom, procena mišićne snage - manuelnim mišićnom testom; Ispitivanje senzitivnosti na ključnim senzornim tačkama za dermatome je vršeno prema ASIA (American Spinal Injury Association) skali, testom istezanja ishiadičnog (Lazarevićev) i femoralnog nerva ispitivano je prisustvo kompresije nervnih korenova, za procenu stabilnosti lumbosakralne kičme- je korišćen test nestabilnosti u proniranom položaju (Prone Instability test). Ispitanici su ergonomski edukovani, sprovedena je transkutana elektro-nervna stimulacija (TENS) i primenjivana je laseroterapija male snage. Pacijentima je određena osmonedeljna kineziterapija koja je obuhvatala vežbe za jačanje dubokih stabilizatora lumbalne kičme. Retestiranje je rađeno nakon 4 i 8 nedelja. Rezultati: Kod obe grupe ispitanika u svim intervalima merenja je nađena visoka statistički značajna (p< 0,001) redukcija intenziteta bola prema VAS skali za lumbalnu kičmu i donje ekstremitete, poboljšanje funkcionalnog statusa (Oswestry) i sagitalne pokretljivosti (Schober). Statistički značajan (p<0,05) oporavak ispitanika grupe 1 u odnosu na grupu 2, ostvaren je u svim intervalima merenja kod parametara: Intenzitet bola (VAS LS i VAS za nogu), stepen onesposobljenosti (ODI),vrednost motornog skora za trup merenog ASIA skalom i vrednost neuropatske komponente bola (DN4). Statistički značajan (p< 0,05) oporavak ispitanika u grupi 2 u odnosu na grupu 1, ostvaren je posle 8 nedelja kod parametra za procenu vrednosti senzornog skora dodir prema ASIA skali. Utvrđeno je da veće početne vrednosti intenziteta bola prema skalama Vas ls i Vas za nogu i stepena onesposobljenosti prema ODI, negativno utiču na krajnji ishod ovih parametara kod pacijenata sa HLB-om u obe grupe (p<0,05). Pacijenti sa pozitivnom neuropatskom komponentom bola imaju lošiji ishod lečenja HLB-a vežbama stabilizacije i torakalne mobilizacije (p<0,05). Zaključci: Pacijenti koji su radili program vežbi lumbalne stabilizacije i torakalne mobilizacije u zatvorenom kinetičkom lancu su imali bolji funkcionalni oporavak i značajniju redukciju intenziteta bola u odnosu na ispitanike koji su radili program vežbi lumbalne stabilizacije u zatvorenom i otvorenom kinetičkom lancu.</p> / <p>Introduction: An individual program of lumbar stabilization exercises could contribute to a better recovery of patients with chronic lower back pain. The exercise program in the treatment of chronic lumbar pain is quite diverse, but it has been proven that stabilization exercises are most effective, especially in a closed kinetic chain. Objective: was to compare the program of lumbar stabilization exercises in the open and closed kinetic chain, in relation to the program of lumbar stabilization exercises and thoracic mobilization in a closed kinetic chain, and to evaluate the clinical efficacy of both exercise programs. Methods: The study was conducted as a prospective, experimental study of 80 subjects of both sexes (35 men, 45 women), average life expectancy (48.45 +/- 10.22 years) with chronic lumbar pain. The research was conducted at the Rehabilitation Clinic "Dr. Miroslav Zotović" in Belgrade in the period from June 2017 to March 2018. Respondents were divided into two groups of 40 patients. The first group of respondents carried out a lumbar stabilization and thoracic mobilization program in a closed kinetic chain (LSTMZ) and the second, program of lumbar stabilization exercises in closed and open kinetic chain (LSZO). In addition to general demographic, anthropometric data on respondents, the following questionnaires and tests were used: for assessing the intensity of back pain and lower extremities - visual analogue scale (VAS), for assessing the neuropathic pain component DN4 (Douleur Neuropathique en 4 Questions), for functional status assessment was used Oswestry disability index (ODI), for assessment of sagittal mobility of the lumbosacral spine- Schober test, muscular strength assessment - manual muscle test; Sensitivity testing at key sensory points for dermatomas was performed according to the ASIA (American Spinal Injury Association) scale, stretching test of the ishiadic (Lazarević) and femoral nerves was udes for detection of nerve root irritation, and the Prone Instability test was used to assess the stability of the lumbosacral spine. Respondents were ergonomically educated, transcutaneous electrical-nerve stimulation (TENS) and low-power laser therapy were performed. Patients were given an eight-week kinesiotherapy that included exercises to strengthen the deep lumbar spine stabilizers. Retesting was done after 4 and 8 weeks. Results: In both groups of subjects, high statistically significant (p <0.001) reduction of pain intensity to VAS scale for lumbar spine and for the leg, functional disability (Oswestry) sagital mobility of lumbar spine (Schober) improvement were found at all measurement intervals. The statistically significant (p <0.05) recovery of group 1 subjects compared to group 2 was achieved at all measurement intervals in the parameters: Pain intensity (VAS LS and VAS for the leg), degree of disability (ODI), motor score value for the trunk measured by the ASIA scale and the value of the neuropathic pain component (DN4). Statistically significant (p <0.05) recovery of group 2 subjects compared to group 2 was achieved after 8 weeks with the parameter for assessing the sensory touch score value to the ASIA scale. It has been found that higher initial pain intensity values to VAS ls and VAS for leg and degree of disability to ODI, negatively affect the final outcome of these parameters in patients with chronic low back pain in both groups of subject (p<0,05). Patients with a positive neuropathic pain component have a lower outcome of chronic low back pain treatment with lumbar stabilization and thoracic mobilization exercises (p<0,05). Conclusions: Patients who performed lumbar stabilization and thoracic mobilization exercises program in a closed kinetic chain had better functional recovery and a significant reduction in pain intensity compared to respondents who performed a lumbar stabilization program in a closed and open kinetic chain.</p>

Previous issue date: 2013-11-25 / Financiadora de Estudos e Projetos / Reports from IPCC (Intergovernmental Panel on Climate Change) call attention to the possibility of water shortages, to the collapse of the Amazon system, to the biodiversity crisis, and to the proliferation of extreme weather events. Anthony Giddens, British sociologist formulator of the theory of structuration, elaborates an analytical category appointing precisely the lack of concrete actions related to this environmental issue in the policy and in the daily practices. According to him, when practical measures for protecting the environment are put into practice, there will not be enough time to reverse the environmental collapse, setting the self-styled Giddens‟s Paradox. Such point of view of environmental question transforms in normative, abstract and universal, leaving to conceive it as eminently contextual - result of the interaction between individuals, groups and institutions. Precisely so it this research takes this analytical category as a starting point for a relational approach of environmental issues through his own theory of structuration. So, the environment and the nature are now considered as structural principles: as rules and resources mobilized in the reproduction and transformation of social life and, therefore, as objects of appointment disputes and discursive resignifications. In order to analyze sociologically the relational aspect the environmental issue through a qualitative research - theoretically and methodologically linked to the theory of structuration the county of Brotas (São Paulo/Brazil) was established as case study. Are used as research methods: semi-structured interviews, snowball technique, documental survey, photo documentation and socioeconomic questionnaire. This particular location shows a convergence between the multiple conceptions of the environmental issue, the professionalization of ecotourism and of the discursive resignification of the rural areas, allowing the visualization of multiple cognitive perceptions of the environmental issue. The critical debate from Giddens enabled the analysis of four social processes of resignification in the Brotas county: a) The Jacaré-Pepira River; b) From the "hole" to the tourist attraction; c) From Radical to Disneyland; and d) The Estrada do Patrimônio . In summary, the trajectory of the dissertation seeks to discuss sociologically that the environmental issue does not arise in everyday life in a universal and normative form, but eminently as cognitive perception linked to the complexity of knowingness agents. / Relatórios do IPCC (Painel Intergovernamental sobre as Mudanças Climáticas) alertam para a possibilidade de escassez de recursos hídricos, de colapso do sistema amazônico, de crise da biodiversidade, e de proliferação de eventos climáticos extremos. Anthony Giddens, sociólogo britânico formulador da teoria da estruturação, elabora uma categoria analítica que nomeia justamente a ausência de ações concretas em relação a essa questão ambiental nas práticas políticas e cotidianas. Segundo ele, quando medidas práticas de proteção ao meio ambiente forem postas em práticas, não haverá mais tempo suficiente de reverter o quadro de colapso ambiental, configurando o autodenominado Paradoxo de Giddens. Tal perspectiva da questão ambiental acaba por torná-la normativa, abstrata e universal, deixando de concebê-la como eminentemente contextual - fruto da interação entre indivíduos, grupos e instituições. Justamente por isso a pesquisa toma essa categoria analítica como ponto de partida para uma abordagem relacional da questão ambiental através da própria teoria da estruturação. Assim, meio ambiente e natureza passam a ser considerados como princípios estruturais: como regras e recursos mobilizados na reprodução e transformação da vida social e, portanto, como objetos de disputas de nomeação e de ressignificação discursiva. Para analisar sociologicamente o aspecto relacional da questão ambiental através de uma pesquisa qualitativa - teórica e metodologicamente vinculada à teoria da estruturação - estabeleceu-se o município de Brotas-SP como estudo de caso. São utilizados como métodos de pesquisa: entrevistas semiestruturadas, técnica de bola de neve, levantamento documental, fotodocumentação e questionário socioeconômico. Tal localidade específica apresenta uma convergência entre as múltiplas concepções da questão ambiental, a profissionalização do ecoturismo e a ressignificação discursiva do próprio espaço rural, possibilitando a observação de múltiplas percepções cognitivas da questão ambiental. O debate crítico a partir de Giddens permitiu a análise de quatro processos sociais de ressignificação no município de Brotas: a) O Rio Jacaré-Pepira; b) Do buraco ao atrativo turístico; c) Do Radical à Disneylândia e d) A Estrada do Patrimônio. Em síntese, a trajetória da dissertação procura debater sociologicamente que a questão ambiental não emerge na vida cotidiana de forma universal e normativa, mas eminentemente como percepção cognitiva vinculada à complexidade da cognoscência dos agentes.

Uticaj kaudalnog bloka na nivo perioperativnog stresa kod dece tokom uroloških operacija / Effect of caudal block on perioperative stress level in children during urological operationsMarina Pandurov 10 July 2020 (has links)
<p>Hirurška inetrvencija aktivira odgovor organizma na stres, pokreću se neuroendokrine promene u organizmu, što rezultira neželjenom hemodinamskom nestabilnošću, promenama metabolizma, endokrinog i imunog sistema. Cilj ovog istraživanja je bio da se utvrdi uticaj kaudalnog bloka na nivo perioperativnog stresa i njegova efikasnot u zbrinjavanju intra- i postoperativnog bola. Ovo prospektivno, randomizirano kliničko ispitivanje obuhvatalo je 80 dečaka, uzrasta 2-5godina, kojima su bile indikovane urološke operacije. Jedna grupa (n = 38) je primila opštu anesteziju, a druga (n = 38) opštu anesteziju sa kaudalnim blokom. Mereni su intraoperativno hemodinamski parametri u 8 merenja, ukupna potrošnja svih datih lekova i intenzitet bola u 3 navrata postoperativno. Uzorci krvi uzeti su pre uvoda u anesteziju i nakon buđenja pacijenta, i ispitivan je nivo glukoze, kortizola, leukocita, leukocitarne formule, pH i laktata. Deca koja su primila kaudalni blok imala su, postoperativno, značajno niži nivo glukoze u serumu (p <0,01), koncentracije kortizola (p <0,01), leukocita i neutrofila (p <0,01), laktata i acidoze, a takođe su imali i niže ocene bola u sve tri momenta merenja (p<0,01). Intraoperativno utvrđena je veća hemodinamska stabilnost i manja potrošnja analgetika perioperativno. Takođe, u toj grupi nije bilo komplikacija. Kombinacija kaudalnog bloka sa opštom anestezijom je bezbedna metoda, koja dovodi do manjeg stresa, veće hemodinamske stabilnosti, nižih ocena bola i manje potrošnje<br />lekova.</p> / <p>Surgery generates a neuroendocrine stress response, resulting in undesirable haemodynamic instability, alterations in metabolic response and malfunctioning of the immune system. The aim of this research was to determine the effectiveness of caudal blocks in intra- and postoperative pain management and in reducing the stress response in children during the same periods. This prospective, randomized clinical trial included 80 patients scheduled for elective urological operations. One group (n = 38) received general anaesthesia and the other (n = 38) received general anaesthesia with a caudal block. Haemodynamic paramethers, drug consumption and pain intensity were measured. Blood samples for serum glucose, cortisol level, leukocytes, pH and lactate level were taken before anaesthesia induction and after awakening the patient. Children who received a caudal block had, postoperativly, significantly lower serum glucose (p < 0.01), cortisol concentrations (p < 0.01), leukocytes (p<0,01), lower lactate level and acidosis,also pain scores were lower at all 3 measurments (p<0,01). Intraoperativly greater haemodynamic stability and lower drug consumption were noticed. Also, there were no side effects or complications identified in that group. The combination of caudal block with general anaesthesia is a safe method that leads to less stress, greater haemodynamic stability, lower pain scores and lower consumption of medication.</p>

Previous issue date: 2016-12-12 / Esta pesquisa apresenta um estudo exploratório acerca do processo de significação de marcas e, com isto, propõe um processo que analisa a comunicação de significados em assinaturas visuais. O objeto de estudo é a marca/função ou marca/signo, considerando-a como signo portador de significados e a interpretação do indivíduo que não possui experiência com a mesma. A motivação para a realização do estudo veio da observação do crescente número de empresas de serviços de alimentação de médio porte na região Nordeste que obtiveram crescimento por meio de franquias, vendo neste cenário a oportunidade de investigar o processo de comunicação de significados de marcas em expansão que visam conquistar novos mercados. O processo proposto
foi aplicado à marcas com características comuns quanto à área de atuação e composição formal, e investigou os três níveis de significação: os atributos físicos do signo, a missão do objeto e a percepção do interpretante. O processo de comunicação foi avaliado a fim de identificar como os atributos físicos da assinatura visual influenciam a transmissão dos significados esperados. Os significados associados às marcas foram interpretados de acordo com a grade de valores semânticos propostos pela Interação Significante (IS) com o intuito de compreender o tipo de interação que as empresas mantêm com o público. A aplicação do método proposto constatou que: 1) as marcas avaliadas apresentam baixo grau de comunicação dos significados, principalmente das qualidades de dimensão pragmática prática e crítica; 2) ruídos na configuração formal comprometem a comunicação de significados de dimensão pragmática; 3) significados de dimensão emocional são, em grande maioria, relacionados a signos icônicos e cromáticos, e signos desta natureza têm um alto poder de significação; 4) as empresas estudadas manifestam pouca atenção a valores emocionais lúdicos tanto em seus atributos formais quanto no discurso de perfil, e enfocam em questões pragmáticas práticas. Com os resultados obtidos, o processo utilizado se mostrou eficiente e passível de ser aplicado em outros estudos que busquem investigar a comunicação de significados de marcas em seu estágio inicial de significação. / This research presents an exploratory study about the process of marks and, based on this, it is
proposed a method to evaluate meanings on visual signatures. The study object is the
mark/function or mark/sign, considering it as a sign carried with meaning, and that its
interpretation is made by an individual that does not have any experience with it. Our motivation
came from the observation of the increasing number of midsize food companies from the
northeast region of Brazil which are heading to the franchising market, then, foreseeing in this
scenario an opportunity to investigate the meaning communication process of those trademarks
which aims to expand its market share in different regions. The proposed method was applied on
different marks with common features of operation and formal composition, where we investigate
three meaning levels: physical attributes of the sign, mission of the object and its individual
perception. We evaluate the communication process to identify the physical attributes of its
visual signature that influence the transmission of expected meaning. The associated meanings
were interpreted based on a semantic values grade proposed by the Signification Interaction (SI)
process, aiming to understand what type of interaction the companies maintain with their public.
Applying the proposed method, we found: 1) the evaluated marks presented low levels of sign
communication, mainly on the their practical and critical pragmatic quality dimensions; 2) noises
in the formal configuration commit the sign communication in the pragmatic dimension; 3) most
of emotional dimension signs are related to iconic and chromatic signs, and these types of signs
have high levels of meaning power; 4) the investigated companies manifest low attention to ludic
emotional values both in their formal attributes and profile message, focusing only in practical
pragmatic features. Based on the obtained results, the used method showed to be efficient and
feasible to be applied on different studies that aim to investigate the meaning communication of
marks in their initial stage of signification.

Effects of forage-based diet on milk production and body reserves of dairy cows on smallholder farms in South AfricaAkinsola, Modupeoluwa Comfort 02 1900 (has links)
Text in English, Tswana / Low nutrient intake affects metabolism and growth in pregnant heifers and limits milk production in lactating cows on communal area smallholder dairy farms of the subtropics. Two studies were conducted during the current research. The first study evaluated effects of nutrient supply in standardized dairy diets on the growth and body reserves of pregnant Jersey heifers raised on communal area smallholder farms in a semi-arid zone of South Africa. Twenty-two farms with a total of 42 heifers, aged 22 to 28 months which were seven months pregnant at the beginning of the study were selected for the study. These represented the total number of farms with dairy cows in the area that were supported through a structured Dairy Development Program (DDP) of South Africa. Each farm had at least two pregnant Jersey heifers during the summer season of 2016. Each heifer was supplied 2.5 kg of a far-off (60-30 d prepartum) dry cow concentrate and increased to 3.3 kg of the same concentrate at close-up period (29-0 d prepartum). Feeding of concentrate was based on a standardized feeding program as recommended by DDP. During this study, no feeding treatment was imposed on the heifers. Eragrostis curvula hay was supplied by DDP. Daily intake of 7.2 and 5.4 kg; respectively for heifers at 60-30 d prepartum and 29-0 d prepartum was determined based on residual hay. Heifer diet (HD1) and heifer diet HD2 were therefore simulated respectively for cows at 60-30 d preparpartum and 29-0 d prepartum, respectively. Diets were assessed for nutrient composition using chemical analyses and in vitro ruminal degradation. Post ruminal nutrient absorption and animal responses were predicted using the Large Ruminant Nutrition System (LRNS) version 1.0.33 (level 1). Actual measurements of body weight (BW), body condition score (BCS) were done and blood was collected and analysed for proteins monthly. Heifers’ responses were validated against the model predicted values and comparative analysis of animal performance during pregnancy was done against the National Research Council (NRC, 2001) reference values. Relative to the minimum requirement for ruminants, both HD1 and HD2 diets had relative feed value (RFV) below 144. About 35% of HD1 dietary crude protein (CP) was within the slowly degrade neutral detergent fibre (NDF) fraction which is the neutral detergent fibre insoluble crude protein (NDFICP) while 32% was not available as the acid detergent insoluble crude protein (ADICP). Equally, HD2 diet had effectively 5.2% of CP as available protein and the fraction of the slowly degraded NDF constituted only 52.3% of the effective available protein. Energy density of HD1 and HD2 were 25% and 16% higher than expected at far-off and close-up period, respectively. The intake of metabolzable protein (MP) were 32 and 25% higher than predicted for the far-off and close-up period, respectively. Supply of MP was 37 % and was higher than NRC predictions of daily requirement in Jersey cow. This allowed BW gain of 29 kg and BCS of 0.33 which was within 25th percentile for pregnant heifers. Mean concentration of blood urea at both far-off and close-up periods deviated by 25% from NRC values. Creatinine (CR) concentration was 145 μmol /L at far-off and 155 μmol /L at close-up period.
The second study assessed the adequacy of two lactation diets fed to 42 primiparous Jersey cows, aged 24 to 30 months during early (1-30 d postpartum) and peak (31-60 d postpartum) periods on the lactation performance of the cows. Cows received 4.5 and 5 kg of dairy concentrate at 1-30 d postpartum and peak milk (31-60 d postpartum) respectively. Eragrostis curvula hay was supplied ad libitum and dry matter intake (DMI) was estimated at 7.2 kg of hay/cow/day from residual hay. No feeding treatment was imposed except for the standardised diets typical to the production environment. Two simulated lactation diets (LD1 and LD2) were prepared based on dry matter intake (DMI) of grass hay and lactation concentrate. Diets were assessed for nutrient composition using wet chemistry and in vitro ruminal degradation. Nutrient supply of diets and absorption from the small intestines as well as cows’ responses were predicted using the Large Ruminant Nutrition System (LRNS) version 1.0.33 (level 1). Body weight and BCS were monitored, blood was collected and analysed for proteins monthly. A record of milk yield was taken daily, and milk was analysed for fat, protein, lactose and urea nitrogen weekly. Cows had DMI of 11.2 kg which was 12% higher than the expected at 1-30 d postpartum period and 11.6 kg which was 21% higher than the expected in 31-60 d postpartum cows. Diets had low available protein as % of dietary protein (LD1=46%; LD2=45%) and the slowly degraded NDF fraction (NDFICP) constituted 64% of the available protein. Intake of energy was 20% and 17% lower than the predicted value for the cows, respectively, at 1-30 d postpartum and 31-60 d postpartum period. Cows had negative energy balance of -6.5 and -5.6 Mcal respectively at 1-30 d postpartum and 31-60 d postpartum cows. Protein intake of lactating cows was low, which resulted in negative protein balance of 59% and 42% of cow’s daily requirement, respectively, at 1-30 d postpartum period and 31-60 d postpartum period. There was loss of BW and BCS, low milk yield, energy corrected milk (ECM: 9.50 kg/d) and feed efficiency (FE) of less than 1 (LD1= 0.85; LD2 =0.89) in cows at both periods. Composition of fat, protein and lactose in milk were negatively affected by the low level of dietary protein. Somatic cell count (SCC) in milk was 121 ± 13 x 103/ml and cows did not show signs of illness. Mean milk urea nitrogen (MUN) concentration was 12 ± 2.7 mg/dl reflecting the low protein status of the lactating cows. Cows had high creatinine concentration of 116 and 102 μmol /L at 1-30 d postpartum and 31-61 d postpartum period, respectively, which may indicate muscle breakdown due to heat stress relative to the hot production environment. Results showed that diets fed to dairy cows on communal area smallholder farms in Sekhukhune and Vhembe districts in Limpopo province had low feeding value and their low nutrient supply affected rumen fermentation, heifers’ ‘growth, body reserves and early lactation in Jersey dairy cows. In conclusion, diets supplied to dairy cows raised on smallholder farms are low in nutrients and do not support efficient growth in heifers and optimal milk production in early lactation.
