Närståendes upplevelse av delaktighet inom intensivvård

Intensivvårdspatienter är ofta sederade och intuberade och kan därmed ej vara delaktiga i sin vård. Närstående har enligt svensk lag rätt att delta i planering och utförande av vården för patienter som inte kan tala för sig. Studier har undersökt och belyst närståendes behov vid vård av intensivvårdspatienter. Få studier har fokuserat på närståendes uppfattning om delaktighet i vården av intensivvårdspatienter. Syfte  Att undersöka hur närstående upplever sin delaktighet i beslutsfattande och omvårdnad av intensivvårdspatienter. Metod  En deskriptiv kvalitativ design med semistrukturerade intervjuer användes, med en fenomenografisk ansats. Nio närstående intervjuades. Resultat Fem kvalitativt skilda beskrivningskategorier framkom och ordnades hierarkiskt; Beslutsfattandets betydelse – medicinska beslut överlämnas, Delta i omvårdnad – både positivt och skrämmande, Positivt bemötande avdramatiserar, Viktigt att vara fysiskt nära och Tydlig information underlättar delaktighet. Beskrivningskategorin Tydlig information underlättar delaktighet placerades högst i hierarkin eftersom informanterna angav att informationen hade störst betydelse för upplevelsen av delaktighet i vården. Slutsats Kännedom om närståendes olika upplevelser av delaktighet, i synnerhet vikten av rak och tydlig information kan möjliggöra för personalen att bättre inkludera närstående och därigenom öka deras upplevelse av delaktighet i vården av intensivvårdspatienter. / Intensive care patients are often sedated and intubated and can not participate in their care. Swedish law states that when a patient can not participate in care, relatives should be invited to participate in his/her place. Many studies have been conducted focusing on relatives’ needs, but very few focus on their perception of participation in care of intensive care patients.  Aim This study was conducted to explore how relatives perceive their participation in care of intensive care patients, with regards to decision making and participating in physical care. Method A descriptive, qualitative design was used, with semi-structured interviews and with a phenomenographic approach. Nine relatives were interviewed. Results Five qualitatively differing categories of description appeared and were organized in a hierarchy; The importance of decision making – medical decisions are handed over, Participating in physical care – both positive and scary, A welcoming atmosphere helps ease the mood, Important to be physically near, Clear information helps the perceived participation. The category Clear information helps the perceived participation was placed the highest in the hierarchy because the informants described information as the most important for the perceived participation in care. Conclusion Knowledge about relatives’ different perceptions of participation in care, especially the importance of straight forward and direct information, may enable the nursing staff to better include relatives and increase their satisfaction and perceived participation in care of the intensive care patients.

Looking Back to Go Forward: Student Evaluations of Experiences in African-Centered Educational Institutions

In educational research, a prevalent topic of discussion is African-centered pedagogy. This phenomenological study records the unique perspectives of adults who specifically grew up in African-centered learning environments from a young age. The sample includes 10 African American adults, aged 18-45, from various cities in the United States. Mixed methods are applied in this study: group concept mapping strategies are implemented to yield both qualitative and quantitative results for analysis. Data is further supplemented with one-on-one interviews, and a review of themes from interview transcripts using multiple coding processes. Findings from this particular demographic can add another dimension to the current literature on the relevancy and need for culturally relevant pedagogical practice for African-American children. The ultimate goal of this generative study is to serve as a resource for educators, parents and students – which includes documented practices and methods for further consideration and application.

Redefining the sacred in 3D virtual worlds: exploratory analysis of knowledge production and innovation through religious expression

This dissertation contributes to conversations regarding the impact of open user centered innovation on cultural production by focusing on the construction and production of religious products within one large-scale open user-centered technological environment, 3D virtual worlds. Particularly, this study examines how virtual world users construct (non-gaming) religious communities and practices and how the technology impacts the forms of religious expression these users create. Due to its existing religious sector and affordances for user-created content, Second Life (SL) was chosen as the context of study for this dissertation project. Building upon Von-Hippel's (2005) user-centered innovation theory, construction and production within three different user-centered religious communities in SL were explored. Using a comparative ethnographic approach over a 14-month period, involving participant observations, interviews and hyper-media techniques, the social construction of customized religious products amidst technical, social, and economic virtual/non-virtual structures were analyzed. Exploratory findings demonstrate that the democratizing of cultural innovation, that is the construction of heterogeneous cultural religious products by the everyday user, is a matter of patterned relational pathways. The greater possible patterned pathways the higher potential for democratized cultural innovation, an increasing number of users developing new ways of doing religion. The fewer patterned pathways the less the potential for democratize cultural innovation and the greater potential for reproducing within the virtual realm the same cultural frames that define the current social order in the non-virtual realm.

Sjuksköterskans upplevelse av bedsiderapportering

Syfte. Att beskriva sjuksköterskors upplevelse av bedsiderapportering på postoperativ avdelning. Bakgrund. Sjukvården blir ständigt mer specialiserad vilket resulterar i att fler yrkeskategorier blir involverade i patientens vård. Därmed ökar antalet rapporter vilket gör att de behöver bli effektivare och mindre tidskrävande. Bedsiderapportering syftar till att vara patientcentrerad men i vissa fall kan sjuksköterskorna anse att bedsiderapportering bryter mot lagen om sekretess. Metod. Kvalitativ intervjustudie med 9 deltagare. Resultat. Ett övergripande tema kunde identifieras: Bristande följsamhet till lagen om sekretess och patientens integritet. Ur temat framkom två kategorier: Att känna sig maktlös samt Att kränka patientens privata sfär. Slutsats. Studien visar på att sjuksköterskor upplever konflikt mellan att skydda patientens sekretess, integritet och att delta i bedsiderapportering. Ett flertal olika bidragande faktorer identifierades / Aim. To describe nurses experiences of bedsidehandovers at the postoperative unit. Background. Healthcare is becoming increasingly specialized, resulting in several professionals involved in patient care. This increases the number of handovers, which means that they need to be more efficient and less time consuming. Bedsidehandover aim to be patient-centered, but in some cases, the nurses believe that bedsidehandover violate the law of confidentiality. Method. A qualitative interview study with 9 participants. Results. An overarching theme could be identified: Lack of adherence to the law on confidentiality and patient privacy. From the theme emerged two categories: to feel powerless and to violate the patient's privacy. Conclusion. The study shows that nurses are experiencing conflict between protecting patient confidentiality, integrity, and to participate in bedside handover. Att leva med långvarig smärta / Living with chronic pain

Background: Chronic pain is a common and occurring disease that creates pain and suffering for many people. The painful experience is individually unique and is affected by many different factors. The insufficient knowledge regarding the assessment of chronic pain makes it difficult to abate the suffering from the individuals. Nurses´ perception and comprehension about chronic pain may hinder them from listening to the patient´s need. The nurse creates conditions for a functioning dialogue. An increased understanding from a patient´s point of view is necessary in order to comprehend the patient´s need. Aim: The aim is to highlight the experiences of people living with chronic pain. Method: This literature review is influenced by Friberg (2012). 12 qualitative scientific articles were obtained and analyzed from three different databases. Results: The result demonstrates two main categories with related sub-categories. The first category highlights how pain affects peoples´physical, emotional, social and existential aspects when the pain takes over. The latter describes how people manage the pain through strategies and acceptance. Discussion: The result is discussed from Armstrong´s symptoms experience model that demonstrates that people living with chronic pain gives meaning to their experience of pain in various ways, which may tend to give positive and negative impact in their daily life. Moreover, the result connects to Ericsson´s concepts regarding the Caring Science such as, suffering in illness, life, and healthcare. It also describes the theory of a person in suffering which can affect the individual as a whole. / Bakgrund: Långvarig smärta är vanligt förekommande och skapar ett lidande hos människor. Smärtupplevelsen är unik för varje individ och påverkas av många olika faktorer. Otillräcklig kunskap i bedömningen av långvarig smärta medför svårigheter i att lindra lidande. Sjuksköterskors föreställningar om smärta kan vara hinder för att lyssna på patienten. Sjuksköterskan skapar förutsättningar för en väl fungerande dialog. En ökad förståelse för patientens perspektiv är nödvändigt för att förstå patientens behov. Syfte: Syftet är att belysa människors upplevelser av att leva med långvarig smärta. Metod: Metoden är en litteraturöversikt influerad av Friberg (2012). 12 kvalitativa vetenskapliga artiklar inhämtade ur tre olika databaser har analyserats. Resultat: Resultatet presenterar två huvudkategorier med tillhörande underkategorier. Den första huvudkategorin: När smärtan tar över belyser hur smärta påverkar människor ur fysiska, psykiska, sociala och existentiella aspekter. Den andra huvudkategorin: Vägen till att kunna hantera smärtan uppmärksammar människors sätt att hantera smärtan genom strategier och acceptans. Diskussion: Resultatet diskuteras utifrån Armstrongs symtomupplevelsemodell som visar att människor ger mening åt smärtupplevelser på olika sätt, vilket kan medföra en positiv eller negativ inverkan i deras liv. Vidare knyts resultatet till Erikssons vårdvetenskapliga begrepp: sjukdomslidande, livslidande och vårdlidande samt teorin om den lidande människan som beskriver att smärtan kan påverka människan som helhet.

Bakgrund: Barn som föds för tidigt mår i allmänhet bäst av att vårdas av sina föräldrar dygnet runt. Att övergå från öppna rum till familjerum innebär en omställning för personal. Det finns få svenska studier som belyser hur personalen upplever denna förändring. Syfte: Syftet med studien var att beskriva personalens upplevelser av arbete och arbetsmiljö efter en genomförd flytt till nya lokaler och införandet av samvård. Metod: Studien genomfördes med kvalitativ metod med 12 deltagare fördelade på två fokusgruppsintervjuer samt 4 enskilda intervjuer med nyckelpersoner. Dataanalyserades med kvalitativ innehållsanalys. Resultat: I resultatet framkom ett övergripande tema" Realiteten som hinder för att uppleva visionen" med två huvudkategorier som beskrev personalens upplevelser efter den genomförda flytten "Visionen om arbetsmiljön som inte uppfylldes" och "Visionen om föräldrar och barn". Därtill identifierades 12 underkategorier som beskrev personalens upplevelser av förändringsarbetet och processen det inneburit med flytten till en ny vårdbyggnad. Det fanns svårigheter och utmaningar för personalen att gå från öppna rum till enskilda familjerum och införandet av familjecentrerad samvård. Personalen ansåg att lokalerna var dåligt planerade och de hade önskat att deras erfarenheter och åsikter hade vägt tyngre i planeringen. Dock upplevdes vården bättre för barn och familjer efter att familjerum blev tillgängliga. Slutsats: Det finns svårigheter och utmaningar för personalen med att införa samvård som en ny vårdform. Personalen anser ändå att det är det bästa för barnet och familjen. Att involvera personalen och använda deras kunskap och erfarenhet är viktigt vid byggandet av en ny avdelning och införande av samvård eftersom det påverkar deras arbetsmiljö. The transition from an Open-Bay to a Single-Room unit entails a new way to care for the family and collaboration with staff. There are few studies that illustrates how staff perceive this change. Aim: The aim of the study was to describe staff's experiences of the working environment and working with Family-centered care in a Single-Family-Room unit. The method: The study was conducted with a qualitative design, in which two focus groups with 12 participants and four individual interviews were conducted. Data were analyzed using qualitative content analysis. The result: The result showed an overall theme "Reality as a barrier for experiencing the vision". Two main categories described the staff's experiences after completed the move:"The vision of the work which was not fulfilled" and "The vision of parents and children". In addition, 12 subcategories were identified that described the staff's difficulties and challenges in moving from working in Open-Bay to Single-Room unit. The staff felt that the premises were poorly planned and they had hoped that their opinions had been taken in to account in the planning process. With regard to the care for the family, staff experienced beneficial improvements after introducing Single-Rooms. Conclusion: There are difficulties and challenges for the staff to introduce rooming as a new form of care. The staff believe that it is best for the child and the family. Involving staff and use their knowledge and experience is important in the construction of a new department and the introduction of rooming because it affects their working environment. Upplevelser av bemötande i vården hos patienter med fibromyalgi- en litteraturöversikt / Patients experiences of treatment, support and attitudes in health care - a literature review

Bakgrund: Fibromyalgi är ett långvarigt kroniskt smärttillstånd som grundar sig på individens subjektiva smärtupplevelse. Syfte: Syftet var att beskriva hur patienter med fibromyalgi upplevt bemötande i vården. Metod: Denna litteraturöversikt baseras på tretton vetenskapliga artiklar. Resultat: Resultatet resulterade i två huvudkategorier "Patienters upplevelser av bemötande" och "Kunskap och stöd från vårdpersonalen". Patienterna med fibromyalgi upplever dåligt bemötande och att vårdpersonal saknar kunskap om både diagnos och behandling av fibromyalgi. I vården upplever dessa patienter misstro och att de blir dåligt bemötta när de söker för sina smärtsymtom. Patienter med fibromyalgidiagnos önskar ett personcentrerat bemötande och mer tid av sina läkare. Slutsatser: Litteraturöversikten visar att patienter med fibromyalgi i stor utsträckning uppger att de upplever dåligt bemötande, okunskap och brist på engagemang i vården. Framtida forskningen bör fokusera på hur vården kan arbeta mer personcentrerat i denna patientgrupp till exempel genom interventionsstudier. / Objectives: Fibromyalgia has long been a chronic pain condition that bases itself on the individual's subjective experience of pain. Aim: The aim was to describe how patients with fibromyalgia experience the treatment, support and attitudes by the health care personal. Methods: This study is a literature review based on thirteen articles’. Results: The result resulted in two main categories "Patients experiences of treatment" and "Knowledge and support from the medical staff". The patients experience that the health care personal has a lack of knowledge of both diagnosis and treatment. The patients also experience distrust in their doctors and experience a lack of adequate help when seeking for their painful symptoms. Patients with fibromyalgia diagnosis experience poor treatment in health care, and want a person-centered approach and more time by their doctors. Conclusions: This literature review shows that patients with fibromyalgia report that they experience poor treatment, ignorance and lack of commitment from health care. Research should focus on how services can work more person-centered for this group of patients, for example by intervention studies.

Introduction. The current age structure in Sweden is changing. At the same time, life expectancy is increasing in many parts of the world. This will lead to greater demands of accessible care resources and its application. Well implemented communication is paramount when the healthcare system is as information-intensive as it is. For maintenance of good healthcare, demands will be set on new kinds of services. With eHealth, information and healthpromotion through communication-based technology, healthcare can be performed efficiently with secure output within the healthcare sector. Aim. The aim of this study was to examine challanges for health professionals in the process of patient care through care at a distance. Method. An empirical study of six (6) people was conducted by semistructured interviews. Result. Indicated result show a result of the need of eHealth within the current use of healthcare, and that the technology can be of comparable quality to physical meetings. However, technical solutions should be seen as a complement within healthcare, and not as a substitute of physical interaction. Conclusion. eHealth services is not something that will become relevant within a timeperiod of ten to fifteen years. It is a currently usable service that can be established to be of comparable quality to the person-centered care the nursing profession is striving for. / Indroduktion. Den nuvarande Svenska åldersstrukturen håller på att förändras samtidigt som den förväntade livslängden ökar i många delar av världen. Detta kommer att ställa större krav på tillgängliga vårdresurser och dess tillämpning. God kommunikation mellan olika aktörer är av största vikt då vården är informationsintensiv. För att upprätthålla god vård kommer krav att ställas på nya tjänster. Med eHälsa, informations och hälsofrämjande insatser genom kommunikationsbaserad teknik, kan vården utföras effektivt med säkerställda insatser inom vård- och omsorgssektorn. Syfte. Syftet med denna studie var att undersöka utmaningar för vårdpersonal i arbetet med patientmöten via vård på distans. Metod. En empirisk studie utfördes och sex (6) personer intervjuades med en semistrukturerad intervjuform. Resultat. Resultat visar på ett behov inom vården av eHälsa och att tekniska lösningar kan ha en jämförbar kvalité med fysiska möten. Tekniska lösningar borde dock ses som ett komplement inom vården, och inte som en ersättning till fysiska möten. Konklusion. eHälsotjänster är inte någonting som kommer att bli aktuellt om först tio eller femton år, utan det är idag en brukbar tjänst som kan konstateras vara jämförbar med den personcentrerade vård sjuksköterskor arbetar efter.

Candidate-centered voting and political sophistication in Brazil 2002

More and more, elections around the world seem to be won or lost on the basis of the candidates’ personal qualities rather than their policies. Despite its prevalence and consequences, we still know very little about what explains such candidate-centered voting, particularly in new democratic contexts. I argue that variation in candidate-centered voting is largely a function of political sophistication: voters with higher levels of political sophistication are better able to process information relating to policy and performance, which tends to be more cognitively demanding than information relating to candidate’s personalities. To test this argument, I estimate models of vote choice and electoral utility using survey data from the 2002 presidential election in Brazil. The results largely support my contention that political sophistication conditions the weight of candidate considerations relative to policy and performance considerations. / text

Exploring the flipped classroom in a Hong Kong secondary school

This study examines the implementation of the flipped classroom model in a local secondary school in Hong Kong. The flipped classroom is a relatively new educational model requiring teachers to invert the time students spend on lectures and the time students spend on homework. Although several studies conducted on this approach show improvements to student achievement as well as positive views from participating educators, parents and most students, there has been little research done based in Asian schools. For this design study, four geography classes of secondary one students in a local Hong Kong school were “flipped” for one full school term (4 months). Throughout the term, learning resources were posted to online educational platforms for students to access at home, and the teacher developed interactive activities for class time. This paper analyzes data from various summative assessments, statistics from the online platforms, student posts, video footage from the class, responses to a student survey and a teacher interview to examine four elements of the flipped classroom: the nature of the learning environment created to flip the classroom in a local Hong Kong secondary school; whether students could achieve or potentially improve outcomes of the course through flipped learning; whether flipped learning stimulated engagement from students with a particular focus on social constructivist behaviors; and, the sustainability of flipped teaching in a local Hong Kong school. / published_or_final_version / Education / Master / Master of Education
