Nutrients Analysis of Preschool Lunch Menus in Virginia

Child care centers are becoming much more influential in educating and caring for children because more and more families choose supplemental care for their young children in child development programs. In June 1997, Child and Adult Food Program (CACFP) served nearly 2.2 million children and provided meals to 2.6 million children in March 1998. A large number of children eat at least one and sometimes two or more of their meals at child care centers. It is imperative that nutritious and satisfying meals and snacks are served at child care centers. The purpose of this study was to examine menus planned in Head Start Program and Child Day Care Centers in Virginia and to assess if they meet the national Recommended Dietary Allowances (RDAs) standards for vitamin A, vitamin C, calcium and iron. Lunch menus for 3-5 year old children were collected from 114 CACFP staffs attending a state wide CACFP menu training session. Fifty-seven weekly menus were selected based on geographic representation to analyze vitamin A, vitamin C, calcium and iron levels by a nutritional analysis computer program. Menus were also divided into Head Start Program and Child Day Care Center for further analysis and comparison. For all 57 sites, the mean values of these lunches exceeded the one-third RDAs for vitamin A, vitamin C, and calcium; the average percentages were 198%, 121% and 134%, respectively. But the average iron level was only 63% RDAs. None of the child care centers met 100% of one-third RDAs for the lunch menus. Because dietary iron levels are consistently low, iron food sources were studied. The results showed no significant (p< 0.05) difference between the Head Start Program and Child Day Care Center. Based on the findings of this study, following the established meal pattern guidelines for the child nutrition programs did not guarantee adequate iron levels in the planned menus of the child care centers. Some foods with high iron levels should be used more often. Further research is needed for this population in implementing the appropriate dietary guideline. In addition, the menu planing, food purchasing and preparation should be part of the training programs for child care centers. / Master of Science

Transformações no envolvimento paterno ao longo dos seis primeiros meses do bebê na creche

Este trabalho investigou o envolvimento de pais de bebês nos seis primeiros meses de frequência à creche. Baseado no conceito de envolvimento paterno, buscou-se investigar as transformações na interação, disponibilidade e responsabilidade dos pais sobre seus bebês com a entrada na creche. Para tanto, por meio de um estudo de caso coletivo de caráter longitudinal, quatro pais responderam a entrevistas em três momentos de coleta de dados, a saber: entrada do bebê na creche, um mês após a entrada e 6 meses após o ingresso na escola de educação infantil. Análise de conteúdo qualitativa indicou que tanto a creche quanto o desenvolvimento do bebê foram aliados do pai para a retomada do trabalho e da vida pessoal. O pai passou a interagir menos de acordo com os aspectos maternos da interação, uma exigência que vai muitas vezes além das suas capacidades. O presente estudo mostra a importância de se levar em consideração as necessidades e capacidades do pai que está envolvido no cuidado ao bebê. / This study investigated father involvement with their babies during the first six months in a child care center. Distriktssköterskans upplevelser av och reflektioner kring arbetet med hembesök till nyblivna föräldrar : En intervjustudie

Bakgrunden visar att hembesök till nyblivna föräldrar är en bra och väletablerad metod för personal vid barnavårdscentralen och är en del av det nationella Barnhälsovårdsprogrammet som erbjuds alla barn och deras föräldrar. Syftet med föreliggande studien var att genom intervjuer med distriktssköterskor vid barnavårdscentraler belysa deras arbete med och reflektioner kring hembesök hos nyblivna föräldrar. Metoden var en kvalitativ ansats med en beskrivande design. Åtta distriktssköterskor som arbetar vid barnavårdscentraler runt om i ett län intervjuades med hjälp av semistrukturerade intervjuer. Resultatet visade att samtliga av de intervjuade distriktssköterskorna ansåg att det var viktigt med hembesök till nyblivna föräldrar, då det gav en möjlighet att skapa en relation på ett helt annat sätt än vid enbart besök på mottagningen. Andra fördelar med hembesöket var att distriktssköterskan fick en chans att se familjens hem och relationen mellan såväl föräldrarna som mellan förälder och barn. Det som oftast försvårade arbetet med hembesök var tidsbrist eller brist på kollegor eller färdmedel. Resultatet av intervjuerna utmynnade i ett tema för de tre första kategorierna; Viljan finns att genomföra hembesök, men vissa svårigheter finns för att lyckas. Slutsatsen av denna studie är att de distriktssköterskor som arbetar med BVC-verksamhet och hembesök till nyblivna föräldrar anser att besöken är väldigt viktiga för att kunna etablera en bra relation med familjerna. Det som ibland försvårar arbetet är hur hembesöken prioriteras av såväl ledning och chefer, men även av distriktssköterskorna själva. Andra hinder i arbetet är brist på tid, kollegor och färdmedel. / The background shows that home visits to new parents is a good and well-established method for the staff at the child care center and is part of the National Child Health program offered to all children and their parents. The aim of the present study was through interviews with district nurses at the child health centers highlight their work and reflections on home visits to new parents. The method was qualitative with descriptive design. Eight district nurses working at Child Care Centers around a county were interviewed using semi-structured interviews. The results showed that all of the interviewed district nurses felt that it was important to do home visits to new parents, as it gave an opportunity to create a relationship in a completely different way than the mere visit to the clinic. Other advantages of the home visit were that the district nurse had a chance to see the family´s home and the relationship between the parents and between parents and child. Lack of time, colleagues that could fill in at the center and transportation hindered home visits. The result of the interviews yielded a theme for the first three categories; The desire is to carry out home visits, but some difficulties are present that hindrance full success. The conclusion of this study is that the district nurses who works at the Child Care Centers, and who conduct home visits to new parents, believe that the visits are very important to establish a good relationship with the families. What sometimes complicates the work is how home visits is prioritized by both management and executives, but also by district nurses themselves. Other obstacles in the work are lack of time, colleagues and transport.

Knowledge, Perceptions, and Practices of Child Care Providers Relative to the Factors that Cause Childhood Obesity

Childhood obesity is a national epidemic. Many factors contribute to the rise in childhood obesity such as poor nutrition, physical inactivity, and poor role models. Child care providers can be postive role models and educators for children. The Virginia Cooperative Extension is concerned about the health of children and the role that child care providers play in their well-being. The purpose of this study was to assess the knowledge, perceptions, and practices of child care providers relative to the factors that influence childhood obesity. Thirty child care providers participated in five focus groups held throughout Southwest Virginia. Triangulation techniques employing qualitative (focus groups) and quantitative methodologies(participatory activities) were utilized. Nutritional concerns, physical inactivity, and illness were the predominant themes related to health concerns for preschoolers. Poor eating habits, colds, and immunizations were ranked as top health concerns. Many providers stated that preschool aged children do not get enough physical activity due to time restraints and a lack of space. Providers did not believe that overweight and obesity were problems in their day care settings, but it was ranked fourth. The did state that overweight and obesity were problems in their communities and among older children. Not eating enough vegetables, sugary foods at breakfast, and picky eaters were the predominant themes for nutrition concerns. Providers ranked eating too much fast or convenience foods, not eating enough fruits and vegetables, and not eating a variety of foods as top nutrition concerns. Participatory activities revealed a lack of basic knowledge such as serving size and number of recommended servings. Only 33.3% correctly answered the serving size for peanut butter, 50% for crackers, 53% for cheese, 34.2% for juice, 17.2% for carrots and 23.3% for apples. Only 7.4% chose the correct number of servings from the bread group and only 3.1% from the dairy group. Most providers believed they played an integral role in the health/nutritional well-being of the children they worked with. However, some providers did not believe that their own personal health habits influenced the children they worked with. The preferred methods of education for the providers were workshops/trainings, nutrition education kits, and videos. The data obtained from this study will be used to design effective nutrition education strategies for child care providers that will be used by the Virginia Cooperative Extension. / Master of Science

Bakgrund: Övervikt hos barn är ett växande folkhälsoproblem som hotar barns psykiska och fysiska hälsa. Distriktssköterskan har i uppdrag att främja barns hälsa i sitt arbete på barnavårdscentralen och kan med hjälp av familjefokuserad omvårdnad se till barnets helhet. Föräldrarna har ett stort inflytande över sina barns levnadsvanor och det är därmed av stor vikt att undersöka upplevelsen av det hälsofrämjande arbetet utifrån deras perspektiv. Syfte: Syftet med studien är att belysa hur föräldrar till barn med övervikt upplever distriktssköterskans hälsofrämjande arbete på barnavårdscentralen. Metod: Kvalitativ intervjustudie med induktiv ansats. Semistrukturerade intervjuer med 9 föräldrar som erhåller eller har erhållit stöd och behandling via barnavårdscentralen när barnet var i åldern 2 – 5 år. Data analyserades utifrån kvalitativ latent innehållsanalys. Resultat: Resultatet redovisas i 2 huvudkategorier: ”Baksidan av tillväxtkontroller” och ”Skiftande uppfattningar om det hälsofrämjande arbetet”. I “Baksidan av tillväxtkontroller” identifierades 2 underkategorier: “Olika syn på barnets övervikt” och “Känslan av att vara övervakad utan nytta”. I “Skiftande uppfattningar om det hälsofrämjande arbetet” identifierades fyra underkategorier: “Konkret rådgivning”,“Användbar rådgivning och positiva effekter”, “Brister i rådgivning och kompetens” och“Kampen mellan att följa råden och göra det bästa för sitt barn”. Slutsats: Behandlingen på barnavårdscentralen väckte mycket känslor hos föräldrarna och de hade sitt barns bästa i fokus huruvida de valde att följa behandlingen eller inte. Föräldrarna beskrev både positiva och negativa upplevelser av distriktssköterskans rådgivning och bemötande, vilket till stor del påverkade om familjen förändrade sina levnadsvanor. / Background: Obesity in children is a growing public health problem that threatens children's mental and physical health. The district nurse is tasked with promoting children's health through her work at the childcare centre and, with the help of family-centred care, can taken care of the child as a whole. Parents have a great influence over their children's lifestyle and it is therefore of great importance to examine the experience of the health promotion work from their perspective. Aim: The purpose of the study is to shed light on how parents to overweight children experience the district nurse's health promotion work at the childcare centre. Method: Qualitative interview study with an inductive approach. Semi-structured interviews with 9 parents who receive or have received support and treatment via the childcare centre when the child was aged 2 -5 years. The data was analysed based on qualitative latent content analysis. Results: The results are reported in 2 main categories: “The back side of growth controls” and “Changing perceptions of health promotion work". In "The back side of Growth controls" could 2 subcategories be identified: "Different views on the child's obesity" and "The feeling of being observes without benefit". In "Changing perceptions of health promotion work" 4 subcategories could be identified: "Concrete advice", "Useful advice and positive effects", "Deficits regarding advice and competence" and "The struggle between following the advice and doing the best for one's child". Conclusions: The treatment at the childcare centre aroused a lot of emotion in the parents and they had their child's best interests in focus regardless they chose to follow the treatment or not. The parents described both positive and negative experiences of the district nurse's advice and treatment, which largely influenced whether the family changed their lifestyle.
