How do veterans make sense of their disengagement from traditional exposure therapy and their subsequent engagement in a non-exposure based therapy for Post-Traumatic Stress Disorder?

Research psychologists often complain that practitioners disregard research evidence whilst practitioners sometimes accuse researchers of failing to produce evidence with sufficient ecological validity. The tension that thus arises is highlighted, using the specific illustrative examples of two treatment methods for post-traumatic disorder (PTSD): Eye-Movement Desensitisation and Reprocessing (EMDR) and exposure based interventions. Contextual reasons for the success or failure of particular treatment models that are often only tangentially related to the theoretical underpinnings of the models are discussed. Suggestions regarding what might be learnt from these debates are put forward and implications for future research are discussed.

Approved for public release, distribution is unlimited / This study investigates the effects of reversing sleep-wake cycles on mood, sleep, and fatigue of the crewmembers and Air Wing 9 of the USS JOHN C. STENNIS (CVN-74). It also reviews the research conducted in sleep deprivation, circadian rhythms, shiftwork, fatigue, and mood. The effects of reversing sleep-wake cycle on mood of the crewmembers were analyzed by assessing a repeated administration of the Profile of Mood States (POMS). Mood states were monitored at three time points associated with the current work schedule (night shift vs. day shift) of the crewmembers. The results showed that younger participants were angrier than older participants on night shiftwork. The results also indicated that there was a significant interaction between repeated measures of mood states and gender. In addition, female participants reported significantly higher mood scale scores than the male participants, and topside participants were getting significantly less sleep than belowdecks participants. Given these findings, this area of research warrants further exploration. There is a significant need to educate military personnel of the effects of sleep deprivation and shiftwork on their job performance and individual health and safety. / Ensign, United States Navy

Cost-constrained project scheduling with task durations and costs that may increase over time: demonstrated with the U.S. Army future combat systems

Approved for public release, distribution is unlimited / We optimize long-term project schedules subject to annual budget constraints, where the duration and cost of each task may increase as the project progresses. Initially, tasks are scheduled without regard to budgets and the project completion time is minimized. Treating the task durations as random variables, we then use simulation to describe the distribution of the project completion time. Next, we minimize the completion time under budget constraints with fixed task durations, where budget violations are tolerated albeit with penalties. Annual reviews are then introduced, which allow underway tasks to be delayed or monthly budgets to be increased. We obtain estimates of the completion time of the project and its final cost under each of these scenarios. The U.S. Army Future Combat Systems (FCS) is used for illustration. FCS is a suite of information technologies, sensors, and command systems with an estimated acquisition cost of over $90 billion. The U.S. General Accounting Office found that FCS is at risk of substantial cost overrun and delay. We analyze three schedule plans for FCS to identify which can be expected to deliver the earliest completion time and the least cost. / Major, Australian Army

Deployable combat simulations via wireless architectures

To truly realize deployment of these simulations on Naval vessels and in remote theaters, simulations for training must be wireless. Wireless standards 802.11/a/b/g are presented in detail to highlight the strengths and weaknesses of each. This thesis then investigates the viability of deploying combat simulations for training using wireless devices. To this end, the Joint Semi-Automated Forces (JSAF), combat simulation model and the Virtual Helicopter (VEHELO) training simulation entity are tested in an 802.11a wireless environment against the VEHELO application in a wired environment. 802.11a is proposed as part of an overall solution to deploy combat simulations for training. This is primarily because of its high data rates and ability to co-locate access points without interference. Testing reveals that operating JSAF and Virtual Helicopter via the High Level Architecture (HLA) with User Datagram Protocol (UDP) packets in an 802.11a environment provides ample bandwidth with which to deploy combat simulation for training for the simulations conducted. / Lieutenant, United States Navy

Cette thèse se propose d'offrir un commentaire linguistique, philologique, rhétorique des vers 49-396 [ = 1-348 Petsch.] du livre V, qui est le livre de la bataille d’Antonia Castra, et une analyse de l'œuvre, qui montre in primis son importance littéraire. Le cœur de ma thèse est l'édition du texte latin présenté selon une nouvelle numérotation et avec quelques différences par rapport au texte de Diggle-Goodyear (Cambridge 1970). Une importante découverte philologique nous a permis d'établir le véritable incipit du livre V de la Iohannis, jusqu’à présent reconnu traditionnellement dans en un endroit déterminé par une conjecture de G. Loewe, accueillie par l'édition Petschenig et après Diggle-Goodyear. Dans une riche introduction divisée en deux sections et qui précède le commentaire, on parle de la tradition du texte et on encadre historiquement l'activité du poète. Après, nous décrivons les formes d'écriture épique du livre V de la Iohannis, livre par excellence de la Bataille, entre histoire et clichés littéraires (furor bellicus comme Erinys, cohortationes des généraux aristiai, similitudes conduisent à la même idée de guerre sainte, dans une lignée héroïque et mythologique). / Subject of this thesis is the commentary to Coripp. Ioh 5*,49-396 [=5,1-348 Petsch.], that is the description of Antonia Castra’s battle. My intention is to offer a study on Corippus’ work, where first is clear its literary importance. The heart of my thesis is the edition of the Latin text that is based on a new numbering of the verses and with some differences compared with the Diggle-Goodyear’s edition (Cambridge 1970). A very important philological discovery has allowed us to establish the real beginning of Iohannis’ book V, traditionally recognized until Diggle-Goodyear in a point determined by a G. Loewe's conjecture. In a rich introduction, divided into two sections and which precedes the commentary, we talk about the tradition of the Iohannis, through the Trivultianus 686, the only surviving manuscript, and we introduce the poet's historical background. After that, we describe the epic shapes of the book V, the very book of the Battle, between concrete history and literary stereotypes (furor bellicus like Erinys, generals’ cohortationes, aristiai, similies lead us to the same idea of “crusade” ante litteram, following an heroic and mythologic line).

The Effect of Pre-Deployment Physiology as a Predictor of Post-Traumatic Stress Disorder Among a Sample of United States Army National Guard and Reserve Soldiers

Potential risk factors for development of Post-Traumatic Stress Disorder (PTSD) are still unclear. One potential risk factor for the development of PTSD is an individual’s cardiovascular reactivity and recovery in response to stressor tasks. The current study was conducted with 763 Army National Guard and Army Reserve soldiers. Participants completed a stressful induction along with self-report measures prior to deployment. Post-deployment, self-report measures were completed to assess PTSD symptomatology and experiences related to deployment and combat. Multiple regression was used to determine the ability of blood pressure response to stress to predict PTSD symptoms immediately and one-year after return from deployment. Results indicated that soldiers who had a less reactive systolic blood pressure response to and recovery from stressor tasks reported more PTSD symptomatology immediately after and one year after return from deployment. These results suggest that soldiers who develop PTSD after deployment have less pre-deployment emotion regulation ability.

An exploratory analysis of littoral combat ships' ability to protect expeditionary strike groups

Approved for public release; distribution in unlimited. / This thesis uses an agent-based simulation model named EINSTein to perform an exploratory study on the feasibility of using Littoral Combat Ships (LCSs) to augment or replace the current defenses of Expeditionary Strike Groups (ESG). Specifically, LCS's ability to help defend an ESGs in an anti-access scenario against a high-density small boat attack is simulated. Numbers of CRUDES (CRUiser, DEStroyer, Frigate) ships are removed and LCSs are added to the ESG force structure in varying amounts to identify force mixes that minimize ship losses. In addition, this thesis explores various conceptual capabilities that might be given to LCS. For example, helicopter/Unmanned Combat Aerial Vehicles (helo/UCAVs), Stealth technology, close-in high volume firepower, and 50+ knot sprint capability. Using graphical analysis, analysis of variance, and large-sample comparison tests we find that being able to control aircraft is the most influential factor for minimizing ship losses. Stealth technology is another significant factor, and the combination of the two is highly effective in reducing ship losses. Close-in high volume firepower is effective only when interacting with helo/UCAVs or stealth. 50+ knot sprint capability is potentially detrimental in this scenario. O objetivo deste estudo foi investigar o efeito dos exercícios meio-agachamento, salto e complexo (agachamento + saltos), seguido por intervalos de 5 min, 10 min ou auto-selecionado sobre o desempenho no salto vertical com contramovimento (CMJ - countermovement jump) e frequency speed of kick test (FSKT). Para isso, 11 atletas de taekwondo, faixas-preta foram submetidos a nove protocolos experimentais e um controle realizados aleatoriamente. Cada sessão experimental foi composta pelo aquecimento, uma atividade condicionante (meio-agachamento: três séries de uma repetição a 95% de 1RM; pliometria: três séries de 10 saltos sobre barreira de 40 cm; ou exercicio complexo: meio-agachamento com três séries de duas repetições a 95% de 1RM seguido por quatro saltos sobre barreira de 40 cm), e um intervalo (5 min, 10 min ou intervalo auto-selecionado) antes da realização do CMJ e FSKT. Houve diferença estatisticamente significativa sobre o número de golpes aplicados (F9,90 = 2,90; P = 0,005, h2 = 0,225 [pequeno]). O exercício complexo com intervalo de 10 min (23 ± 5 repetições) foi superior a condição controle (19 ± 3 repetições), meio-agachamento com intervalo auto-selecionado (18 ± 2 repetições, P = 0,015) e saltos com intervalo de 5 min (18 ± 3 repetições, P < 0,001). Nossos resultados indicam que atletas de taekwondo podem aumentar o número de golpes aplicados em teste específico após a realização do exercício complexo. Outro objetivo deste estudo foi investigar o efeito do volume e intensidade sobre o desempenho no CMJ e FSKT. Para isso, nove atletas de taekwondo, faixaspreta foram submetidos a quatro sessões experimentais e uma sessão controle realizadas aleatoriamente. Cada sessão experimental foi composta pelo aquecimento, uma atividade condicionante no exercício meio-agachamento (sessões experimentais: uma série de três repetições a 50% ou 90% de 1RM ou três séries de três repetições a 50% ou 90% de 1RM), seguido por intervalo de 10 min antes da realização do CMJ e cinco séries do FSKT. O desempenho do FSKT caiu ao longo das séries (F3,21;128,36 = 25,344; P < 0,001, h2 = 0,388 [muito grande]). Não houve efeito do volume e intensidade sobre as variáveis investigadas. Nossos resultados indicam que os atletas de taekwondo não foram afetados pelo uso de diferentes volumes e intensidades em atividade de potência específica e não específica visando a manifestação da potencialização pós-ativação / The purpose of this study was to investigate the effect of half-squat, jump and complex exercise (half-squat + jump), followed by intervals of 5-min, 10-min or selfselected rest interval on performance of countermovement jump (CMJ) and the frequency speed of kick test (FSKT). Eleven black-belt taekwondo athletes were randomly submitted to nine experimental section and one control were randomly. Each experimental section was composed by warm-up, conditioning activity (halfsquat: three sets of one repetition at 95% of 1-RM; plyometric: three sets of 10 jumps above 40-cm barrier; or complex exercise: half-squat with three sets of two repetitions at 95% of 1-RM followed for four jumps above 40-cm barrier), and a rest interval (5-min, 10-min or self-selected rest interval) before performing CMJ and FSKT. There was statistically significant difference regarding the number of kicks applied (F9,90 = 2.90; P = 0.005, h2 = 0.225 [small]). The complex exercise with 10- min of rest interval (23 ± 5 repetitions) was higher that the control (19 ± 3 repetitions), as well as the half-squat with self-selected rest interval (18 ± 2 repetitions, P = 0.015) and the plyometric with 5-min rest interval (18 ± 3 repetitions, P < 0.001). Our results indicate that taekwondo athletes can increase the number of kicks applied in a specific test after perfoming the complex exercise. An other purpose of this study was to investigate the effect of volume and intensity of CMJ and FSKT. Nine blackbelt taekwondo athletes were randomly submitted to four experimental section and one control. Each experimental section was composed by warm-up, one conditioning activity on half-squat exercise (experimental section: one set of three repetitions at 50% or 90% 1-RM or three sets of three repetitions at 50% or 90% of 1-RM), followed by 10-min of interval before performance of CMJ and five sets of FSKT. The FSKT performance declined along of sets (F3,21;128,36 = 25.344; P < 0.001, h2 = 0.388 [very large]). It was not found difference regarding exercise volume and intensity on the investigated variables. Our results indicate that taekwondo athletes were not affected by different volume and intensities of specific and non-specific power activities on post-activation potentiation

However, in view of the imbalance between risks and benefits of their effectiveness, the damage to human and environmental health, it is suggested that the space usage of pesticides in vector control, reserves the cases of outbreaks. Therefore, it is assumed that the continuous monitoring and frequent in some biological markers in individuals who are subject to occupational exposure to chemicals could ultimately lead, in advance, to medical intervention, as well as studies to better characterize these intoxications. Where this monitoring does not occur, the diagnosis of intoxication becomes insufficient, causing negative effects on human health and the environment. Therefore, this study aimed to evaluate the biochemical profile / hematology of public health officials to control vectors in occupational exposure to pesticides. The study population consists of 46 agents to combat endemic Center UBV, Goiânia, Goiás For characterization of this population, a questionnaire was applied lifestyle, based on the specs of Toxicology Research and Customer Information System Diseases Notification. In parallel, blood samples were collected to perform the biochemical and hematological (based on "Protocol for Health Care Workers Exposed to pesticides Guidelines for Comprehensive Health Care Worker Differentiated Complexity"), which were processed in the Laboratory Clinical Analysis of the Pontifical Catholic University of Goiás health workers combating endemic diseases are predominantly men, with adulthood, understood, mostly between 40 and 59 years with education level of high school. Of these, 21.7% of the ACEs have smoking habit, and 54.4% of alcohol consumption. These agents work on average more than 20 years in combating vectors, for about 8 hours daily. The pesticide currently used by over they are malathion, an organophosphate moderately toxic and ultra low volume. Despite continuous exposure and ultra low volume, some agents complained of symptoms, such as excessive salivation (19.6%), cough (28.3%), nausea (17.4%), vomiting (10, 9%), diarrhea (15.2%), tingling of feet and / or legs (39.1%), tingling of the hands and / or arms (43.5%), bradycardia (10.9%), headache (56.5%) and watery eyes (39.1%). However, no major changes were evident hematologic and biochemical, except for the values slightly increased indirect bilirubin, total cholesterol, glucose and triglycerides, which are not able to specify clinical and by the presence of slight relative lymphocytosis with atypical lymphocytes observation. Concluding that, despite symptomatic complaints, these health workers have biochemical and haematological profile within the normal range, thus requiring, in its follow-up, a greater emphasis on history. / O emprego de pesticidas nas campanhas de saúde pública para controle de endemias, no Brasil, foi marcante em meados dos anos 50, em paralelo ao uso abusivo e deliberado desses insumos, sem grandes cuidados quanto aos riscos biológicos e ambientais, provenientes do seu uso. Contudo, tendo em vista o desequilíbrio entre riscos e benefícios de sua eficácia, dos danos à saúde humana e ambiental, sugere-se que o uso espacial de pesticidas, no controle vetorial, se reserve aos casos de surtos epidêmicos. Portanto, presume-se que o acompanhamento contínuo e frequente, sob alguns marcadores biológicos em indivíduos que estejam sujeitos à exposição ocupacional a substâncias químicas, poderiam vir a conduzir, antecipadamente, à intervenção médica, bem como a estudos de melhor caracterização dessas intoxicações. Onde não ocorre este monitoramento, o diagnóstico da intoxicação torna-se falho, gerando consequências negativas à saúde humana e ambiental. Portanto, este estudo objetivou avaliar o perfil bioquímico/hematológico dos agentes de saúde pública do controle a vetores em exposição ocupacional a pesticidas. A população de estudo é constituída por 46 agentes de combate a endemias da Central de UBV, de Goiânia, Goiás. Para caraterização desta população, foi aplicado questionário de estilo de vida, baseado na Ficha de Investigação e Atendimento Toxicológico do Sistema de Informação de Agravos de Notificação. Em paralelo, foram coletados amostras de sangue para realização das análises bioquímicas e hematológicas (baseadas no Protocolo de Atenção à Saúde dos Trabalhadores Expostos a agrotóxicos das Diretrizes para Atenção Integral à Saúde do Trabalhador de Complexidade Diferenciada ), as quais foram processadas no Laboratório de Análises Clínicas da Pontifícia Universidade Católica de Goiás. Os agentes de saúde do combate a endemias são, predominantemente, homens, com idade adulta, compreendida, em sua maioria, entre 40 e 59 anos, com nível de escolaridade de ensino médio. Destes, 21,74% dos ACEs possuem hábito de fumar e, 54,35% de consumo de bebidas alcóolicas. Estes agentes trabalham, em média, há mais de 20 anos no combate a vetores, por cerca de 8 horas diárias. O pesticida, atualmente, mais usado por eles é o malathion, um organofosforado de toxicidade moderada e em ultra baixo volume. Apesar da exposição contínua e em ultra baixo volume, alguns agentes se queixaram do aparecimento de sintomas, como: salivação excessiva (19,6%), tosse (28,3%), náusea (17,4%), vômito (10,9%), diarreias (15,2%), sensação de formigamento de pés e/ou pernas (39,1%), formigamento de mãos e/ou braços (43,5%), bradicardia (10,9%), cefaléia (56,5%) e lacrimejamento (39,1%). Contudo, não ficaram evidenciados maiores alterações bioquímicas e hematológicas, exceto, pelos valores levemente aumentados de bilirrubina indireta, colesterol total, glicose e triglicérides, os quais não são capazes de especificar clínica e, pela presença de linfocitose relativa leve, com observação de linfócitos atípicos. Concluindo-se que, apesar das queixas sintomáticas, estes agentes de saúde apresentam seu perfil bioquímico e hematológico dentro dos padrões de normalidade, exigindo assim, no seu acompanhamento, uma maior ênfase a anamnese.
