The Participation of Marginalized Populations in Health Services Planning and Decision Making

<p>Community participation has been identified as a key facilitator of community health among marginalized populations in international health statements. However, knowledge gaps in the community participation literature regarding marginalized populations has been attributed to the lack of consistent definitions of community participation, ambiguity about the features of community participation initiatives (e.g., methods and strategies) that are appropriate for marginalized populations, and limitations of existing community participation frameworks in specifying the ways and means in which different marginalized populations might effectively participate, as well as in recognizing that community participation is highly contextual and situational. All of these factors have made it difficult to draw broader conclusions about the impact of participation methods and strategies for marginalized populations from evaluations of participation initiatives.</p> <p>The overall purpose of this thesis is to better understand how to involve marginalized populations in the planning and decision-making for local health services. First, a critical interpretive synthesis (CIS) was conducted to better understand the role of community development principles used in community participation initiatives with marginalized populations and the factors contributing to the influence of the principles in enabling the participation of these populations. Second, an in-depth comparative case study of four community participation initiatives in Ontario Community Health Centres (CHCs)—which are primary health care organizations serving 74 high-risk communities throughout the Province of Ontario—was conducted to identify the core features of participation initiatives with marginalized populations, and reflect on the particular challenges of engaging marginalized populations. Third, four focus groups were held at four Ontario CHCs to examine the role of frameworks as mechanisms for translating knowledge about community participation practice with marginalized populations. Overall, this thesis broadens our understanding of community participation with marginalized populations in the context of local health services planning and decision making. Specifically, this thesis contributes a theoretical basis for future research and provides practical knowledge for planning and evaluating community participation initiatives with marginalized populations.</p> / Doctor of Philosophy (PhD)

Corporate community engagement (CCE) in Zimbabwe's mining industry from the Stakeholder Theory perspective

Questionnaires translated into Shona / Mineral extraction is one of the key drivers of Africa’s economies and is also one of the largest industries in the world. In many African countries, including Zimbabwe, mining contributes to profound parts of the economy and remain the engine for economic growth. In recent years, and following the continual exploitation of minerals, mining companies have been scrutinized as a major cause of social, environmental, and economic problems faced mainly by communities at the margins. In this regard, mining companies are widely perceived to be prospering at the expense of adjacent communities, who are the primary recipients of the externalities, mainly negative, from mining operations. Due to incongruent stakeholder interests conflicts have erupted given the peculiar case of the extractive industries in Zimbabwe. Having realised the differences among stakeholder interests over minerals, in the extractive industry the study sought to answer this question: how is CCE understood by different stakeholders? And how is CCE measured by the same stakeholders?. The focus of the study is to evaluate the meaning of CCE from multiple stakeholders in the extractive industry in Zimbabwe; and to analyse how CCE is measured by identified stakeholders. In order to satisfy the stated objectives, the study employed mixed research method. This study revealed similarity in understanding of CCE and its usefulness amongst the different stakeholder groups. Of cognitive importance is the realisation by stakeholders on the need for proactive communities and corporate investment into community for effective partnerships. Collaboration, empowerment, inclusion, trust and organisation emerged to be the major facilitators for CCE. The study presents operative CCE according to the obligations and expectations of stakeholders. Having realised that mining industries are particularly susceptible to conflict between stakeholders, the study suggests proactive desire to mitigate these conflicts through CCE in the mining industry. In this respect, community development, peace and stability and strong economy are the major outcomes of effective CCE. The study recommends participation of resource owners in planning, implementing, monitoring and evaluation as well as dividends sharing of mining projects as advocated for by the CCE Model. It is also recommended that the adoption of the CCE Model will ensure a sustainable and harmonious coexistence between the predominantly capitalistic mining concerns and the resource owners and solve part of the current impasse to business and community development. / Business Management / D.B.L.

Dating Violence on Small Rural College Campuses: Are Administrator and Student Perceptions Similar?

In recent years dating violence has become more and more prevalent on college campuses. Reports of the range of dating violence vary widely, with studies reporting from 20% to 85% of college women experiencing dating violence. However, almost all research has been conducted among urban and/or large colleges and universities, with virtually no attention to what is happening on small and/or rural college and university campuses. When a possible 20% of college women have experienced dating violence on college campuses, there becomes a crucial need for administration at a college to have an accurate assessment of the college’s liability, and of the adequacy of the college’s programs and policies relative to dating violence. This study sought to determine whether administrators and female students on small rural college campuses have the same perceptions of the type and incidence of dating violence on their campus, and of the programs and policies the college has put into place to prevent and respond to dating violence. Two domains of perceptions were addressed, dating violence beliefs and experience, and dating violence policy knowledge. The same question was examined to determine if perceptions of resident and commuter students were the same, and if perceptions of under and upper class students were the same. The investigator surveyed 52 college administrators and 306 female students at a total of four small rural college campuses to determine whether administrator and female student perceptions of dating violence incidence/type and dating violence program/policy knowledge at the college were similar. Results were that administrators tended to have similar perceptions to students as regards dating violence beliefs and experience, although not specific types of dating violence. Students did not exhibit a strong knowledge of dating violence policy. Resident and commuter students displayed similar perceptions to each other, as did under class and upper class students.

PERCEPTIONS OF THE HOMELESS TOWARD NONPROFIT HUMAN SERVICE PROVIDER

As the debate intensifies regarding developing remedies to meet the needs of America’s homeless, one solution is for governmental agencies to collaborate with and employ organizations from the nonprofit sector to assist with the needs of the homeless population. Included in the nonprofit sector, faith-based organizations (FBOs) have historically been a source of debate and contention in terms of collaborations with the government. However, Presidents Reagan, George H. Bush, Clinton, George W. Bush, and Obama have embraced the idea of including FBOs in the pool of service providers offering human services. In the Richmond, Virginia region, FBOs and nonreligious nonprofit organizations provide a range of human services to a substantial population of homeless clients. Yet, whether the homeless population prefers services offered by FBOs versus nonreligious nonprofits in general and for specific categories of service is unknown. These specific categories of service include alcohol treatment and recovery, counseling, drug treatment and recovery, food pantries, health care, job training and placement, short-term and long-term shelter, and meal sites. In addition, this study seeks to identify models using variables from this study that predict the preference for each category of service. Since homeless clients overall and specific human service preferences are an unknown, the importance of this study is to inform policymakers, those in the nonprofit sector, researchers, and other interested parties of these preferences. A study of this nature is also important to compare policy implementation to the preferences of the homeless to ensure the implementation accounts for principles of social equity. In addition, a study of this nature seeks to fill a literature gap by examining and understanding the intersections of demographic characteristics and preferences. Using the cohort and the rational choice theories, this study examines the preferences of homeless individuals for particular types of service providers.

Through the Eyes of the Homeless

When reviewing the entire project from start to completion, I can honestly say, Through the Eyes of the Homeless is a play about ten women and their plight. It illustrates their dealings with everyday issues of hurt, disappointment, abuse, love, and hope. I believe the true impact of this play is the undeniable prayer for help and hope within each monologue. Despite the horrors that are unveiled and released through hidden secrets, the undertone of betterment is truly resonating. My own expectation for this play is simply to strike awareness and understanding in the eyes of the people. It is my objective to have each audience member leave the show with a completely different perspective on homelessness. Homelessness seems to be one of those taboo societal problems the world chooses to turn away from because of lack of awareness. I believe it is fear of the unknown that keeps us blind, deaf, and dumb about certain issues. After all is said and done, I am simply asking anyone who reads this play, watches it, or hears it to listen to each voice carefully and the message it conveys.

A mixed methods approach to the development and validation of an assessment tool to measure psycho-social factors associated with willingless to participate in child-centred initiatives

Includes bibliographical references (leaves 206-239) / The primary aim of the study was to develop a psychosocial assessment tool for determining willingness to participate in child-centred safety promotion interventions, specifically in a low-income community in South Africa. The secondary aim was to determine the factors associated with willingness to participate. The study employed a mixed methods approach to develop the measure, and included content validation, item writing and initial validation processes. Nominal Group Technique discussions were conducted with various role-players in the relevant communities to inform the development of the instrument, while participatory methodologies were utilised to conceptualise the instrument in relation to psycho-social factors associated with intervention participation, with these categorised according to core dimensions. A Delphi method utilising an expert panel reviewed items for relevance, difficulty and ambiguity, with items subsequently amended or removed. Item selection procedures were conducted on the English version of the instrument and these results were applied to the Afrikaans version. A pilot study was conducted as part of the initial validation in order to test the items and format the questionnaire. The instrument was then administered to Afrikaans speaking individuals in a community in the Western Cape. Iterative exploratory factor analysis was conducted at both the item and scale levels to select and reassign items and scales in order to determine the final composition of the questionnaire. The findings indicate that the instrument measures seven factors, namely incentives; priorities and community needs; perceived benefits; social approval; accessibility and values; altruistic capital; and community cohesion. These factors are explained in terms of the Process-Person-Context-Time model and the Theory of Planned Behaviour, considered within a participatory framework. The study findings indicate that the seven factors represent salient dimensions of the construct willingness to participate in interventions. The questionnaire and its subscales displayed acceptable to good reliability, with Cronbach’s α ranging from 0.55 to 0.80. Since willingness precedes actual participation, it is argued that insight into the factors that relate to willingness to participate provides an avenue for motivating actual participation. / Psychology / Ph. PRESERVING, INTERPRETING, AND DISPLAYING MENTAL HEALTH HISTORY: ESTABLISHING THE PATTON STATE HOSPITAL MUSEUM AND ARCHIVE

There are few museums in the western half of the United States that provide an opportunity to educate the public about the history of mental health care. Recently, a mental health museum and archive of artifacts, photographs, and documents was established on the grounds of Patton State Hospital in Highland, California. The purpose of this paper is to reflect on the establishment of this museum and archive and to provide an account of the 125 year history of Patton State Hospital. Understanding the history of Patton provides an opportunity to understand the history of mental health care in the United States from the late 19th century to the present. The establishment of this museum and archive became a joint initiative between Patton and California State University, San Bernardino’s History Department in January 2014. The museum and archive are meant to provide an educational venue that will increase awareness of the plight of the mentally ill, decrease stigmatization of those afflicted with mental illness, and further efforts to improve the care of patients through preservation and display of the artifacts, photographs, and documents related to Patton’s history. The goal of this paper is to assist future public historians with the design and establishment of a museum and/or archive, be it related to mental health history or to projects with other themes, and to provide information to other mental health facilities that wish to establish their own museums.

The purpose of this study was to gather the perceive strategies and substance treatment needs of adults 60 and over. To examine what recovery treatment models and strategies could be most effective for the age-related complex needs of the 60 and over population who are at risk for substance misuse, use or abuse. In the coming years, there will be a substantial increase in the numbers of older adults with substance misuse and abuse problems. Even though one in five individuals who experience substance abuse are older adults many clinicians remain ill equipped to serve older clientele, and particularly those who face substance abuse. Furthermore, the diagnosis and treatment of substance abuse among older adults is complicated by assumptions about aging and the complex physical and cognitive changes in late life, each of which can make the manifestation of symptoms difficult to identify. The results indicate that participates has some definitive values that could increase their participation in engaging with substance treatments tailored to their specific age and belief systems.
