Avaliação prospectiva aberta do uso prolongado de baixas doses de doxyciclina na rinossinusite crônica com polipose nasal de difícil tratamento / Prospective open-label evaluation of long-term low-dose doxycicline for difficult-to-treat chronic rhinosinusitis with nasal polyps

Introdução: A rinossinusite crônica com polipose nasal constitui um subgrupo particular da rinossinusite crônica, caracterizado por uma inflamação da mucosa que leva a um espessamento da mesma e à formação de pólipos, podendo ser especialmente difícil de tratar. A Doxiciclina é um antibiótico bacteriostático, de largo espectro, que também tem uma ação antiinflamatória, e tem se mostrado útil no controle dos sintomas das recidivas desta doença, promovendo inclusive uma diminuição do pólipo. Objetivo: avaliar se o uso de baixas doses de Doxiciclina, por períodos prolongados, pode melhorar o controle clínico da rinossinusite crônica com polipose nasal, de difícil tratamento. Métodos: este é um estudo prospectivo, aberto, realizado em 60 pacientes com rinossinusite crônica de difícil tratamento que se submeteram a cirurgia endoscópica nasal. Os pacientes foram divididos em 2 grupos: 28 pacientes receberam corticóide nasal, lavagem nasal com soro fisiológico, e Doxiciclina (200mg no primeiro dia, seguido por 100mg uma vez ao dia) por 12 semanas, enquanto 30 pacientes receberam apenas corticóide nasal e lavagem nasal com soro fisiológico. O principal resultado avaliado foi a existência de uma melhora, dose efeito, clinicamente significativa do SNOT-20 após o tratamento com a Doxiciclina. Outros resultados avaliados foram os valores do SNOT-20, NOSE e do Lund-Kennedy. Os seguintes parâmetros também foram analisados: asma, rinite, doença respiratória exacerbada pela Aspirina (DREA), níveis séricos de IgG, IgA, IgE, IgM, ANCA e contagem de eosinófilos. Resultados: oy tratamento com a Doxiciclina promoveu uma melhora, dose efeito, clinicamente significativa do SNOT-20. Pacientes que receberam a Doxiciclina também tiveram resultados significativamente melhores do SNOT-20, NOSE e Lund-Kennedy. Houve uma associação negativa entre a melhora clinicamente significativa do SNOT-20 e a presença de asma, DREA e níveis séricos elevados de IgE pré-tratamento. Conclusão: os achados sugerem que a doxiciclina pode ter uma ação benéfica nos pacientes com rinossinusite crônica com polipose nasal, especialmente naqueles pacientes sem asma, DREA ou níveis séricos elevados de IgE prétratamento / Introduction: Chronic rhinosinusitis with nasal polyps is a particular subset of chronic rhinosinusitis characterized by a mucosal inflammation that leads to mucosal thickening and polyp formation, and can be especially difficult to treat. Doxycycline is an oral, available, broad-spectrum bacteriostatic antibiotic which also has anti-inflammatory action, that has been used to treat this disease and has shown a successful control of symptoms even reducing the volume of polyps. Objective: Evaluate if long-term low-dose doxycycline is effective in controlling clinical symptoms of difficult-to-treat chronic rhinosinusitis with nasal polyps. Methods: This was a prospective, open-label study of 60 patients with difficultto- treat chronic rhinosinusitis with nasal polyps who had undergone endoscopic sinus surgery. Patients were divided into two groups: 28 received nasal steroids, saline irrigation, and doxycycline (200 mg on the first day, followed by 100 mg daily) for 12 weeks, while 30 received only nasal steroids and saline irrigation. The main outcome measure was an adequate effect size of doxycycline treatment on clinically meaningful significant improvement of SNOT-20. Other outcome measures were the SNOT-20, NOSE, and Lund-Kennedy scores. The following parameters were also analyzed: asthma, rhinitis, non-steroidal-exacerbated respiratory disease (NERD), and baseline serum IgG, IgA, IgE, IgM, ANCA, and eosinophil count. Results: There was an adequate effect size of doxycycline treatment on clinically meaningful significant improvement of SNOT-20. Patients who received doxycycline also had significantly better outcomes regarding SNOT-20, NOSE, and Lund-Kennedy scores. There was a negative association among a clinically significant improvement of SNOT-20 and presence of asthma, NERD, and elevated serum IgE levels before treatment. Conclusion: These findings suggest that doxycycline may have a beneficial role for chronic rhinosinusitis with nasal polyps patients, especially for those without asthma, NERD or high levels of serum IgE before treatment

DM2 is a risk factor for periodontal disease whereas periodontitis increases hyperglicemia. Although it is known that the treatment of periodontitis improves glycemic control, there is no consensus of what type of treatment is best for diabetics. Objective: Evaluate different types of periodontal treatment in diabetics. Methodology: Two different researches were performed: 40 patients, aged 30 to 60, who were divided into 2 groups: group 1: controlled mechanical + basic periodontal treatment, Group 2: controlled mechanical + basic periodontal treatment + Doxycycline 100 mg orally for 14 days. In the first study, the analyses were made after 0, 3 and 6 months, including probing depth, level of clinical attachment, bleeding on probing. Glycated hemoglobin (HbA1c), fasting glucose and microbiological analysis (FG) were also taken. In study 2, a clinical analysis and an expression of isoforms of interleukin IL1β (IL-1β) test was performed. 20 patients were selected for the second part of the research. They were divided as follow: 1) Conventional periodontal treatment with scaling and root planning + Mechanical control and group 2) Periodontal Treatment (one-stage full-mouth disinfection) + Mechanical control . In this research, the analysis was performed after 0, 3 and 6 months including clinical evaluation, HbA1c and FG tests. The expression of isoforms of interleukin IL1β (IL-1β) were analyzed in the beginning of the study and after 6 months. Results: In the first article, it was shown an improvement in all clinical periodontal parameters evaluated as well as the level of HbA1c in both treated groups after 6 months. In the doxycycline group, there was a greater improvement in gingival index and test of glycated hemoglobin, as well as a smaller amount of Gram- .The second study showed a significant decrease in the expression of IL1- in gingival crevicular fluid in patients in treatment with Doxycycline. In the second research, after 6 months, it was observed better levels of FG, clinical periodontal parameters and gingival crevicular fluid in both groups. Nevertheless, no statistical significance was observed in the expression of IL-1β after 180 days. Conclusion: It is suggested that the basic periodontal therapy associated with the use of doxycycline is more efficient for clinical, glycemic and microbiological control in patients with type 2 Diabetes Mellitus with periodontal disease, as well as in the reduction of IL1- in gingival crevicular fluid. When different types of periodontal treatments are compared, it is possible to observe that the conventional periodontal treatment is the first choice in one-stage full-mouth disinfection for both glycemic control and periodontal parameters related to bleeding on probing and gingival crevicular fluid as well as the relation of variation envolving the expression of IL1-β even in the absence of significant difference among initial and final parameters in each group / O diabetes mellitus tipo 2 (DM2) e a doença periodontal possuem entre si uma relação íntima. Objetivo: Avaliar diferentes modalidades de tratamento periodontal em diabéticos. No artigo 2, foram realizadas as análises clínicase a análiseda expressão das isoformas da interleucina IL1β (IL-1β). Na segunda pesquisa foram selecionados 20 pacientes com DM2, os quais foram divididos em dois grupos: Grupo 1: Tratamento periodontal básico (RAR) + Controle mecânico (Técnica de Bass + fio dental); Grupo 2: Tratamento periodontal básico (Desinfecção Total de Boca em Estágio Único - DBEU) + controle mecânico (Técnica de Bass + fio dental). Nesta pesquisa as análises também foram realizadas aos 0, 3 e 6 meses, com avaliação clínica periodontal e a avaliação de HbA1c e GJ. A expressão das isoformas de IL-1β foram analisadas no início e após 6 meses. Resultados: No primeiro artigo, houve uma melhora de todos os parâmetros clínicos periodontais e dos níveis de HbA1c em ambos os grupos tratados após 6 meses. No grupo que usou doxiciclina, houve uma melhora mais expressiva do índice gengival e daHbA1c, bem como uma quantidade menor de microrganismos Gram-. O segundo artigo evidenciou uma expressão significantemente menor de IL1- no fluido crevicular gengival dos pacientes em uso de doxiciclinina ao final do experimento. Na segunda pesquisa, após 6 meses, houve uma melhora da GJ, dos parâmetros clínicos periodontais e da quantidade de fluido crevicular gengival em ambos os grupos. Entretanto, não houve diferença estatisticamente significativa na expressão de IL-1β após 180 dias.Conclusão: Foi possível observar que a terapia periodontal básica associada ao uso de doxiciclina é mais eficiente para o controle clínico, glicêmico e microbiológico de pacientes portadores de DM2 e doença periodontal, bem como na menor expressão de IL1- no fluido crevicular gengival deste pacientes. Quando comparado os tipos de tratamento periodontal, foi possível observar que o tratamento periodontal convencional é superior à desinfecção total em estágio único, tanto em relação ao controle glicêmico, quanto em relação aos parâmetros periodontais de sangramento a sondagem e área de fluido gengival, bem como em relação a variação dos valores de expressão de IL1-β, apesar de não haver diferença significativa entre os valores iniciais e finais em cada grupo

Leptospira interrogans is the causative agent and is transmitted to humans by indirect contact with contaminated soil or water. The clinical syndromes include sub clinical infection, self-limited anicteric febrile illness, and severe and potentially fatal illness, known as Weil?s syndrome. In developed countries, leptospirosis is related to occupational or recreational activities while in developing countries, outbreaks occur during floods. In those regions, traditional strategies to prevent the transmission are difficulties to be implemented because of costs and lack of community acceptance. In addition, no efficient vaccine is available for human use. Several studies have suggested that chemoprophylaxis with doxycycline pre and post-exposure may be effective to prevent leptospirosis. Leptospirosis has been reported in rural areas of the State of Rio Grande do Norte, Brazil since 1985 in rice farmers who present the anicteric illness. The disease cause great social and economics impact. The study was conducted in S?o Miguel where an epidemic of leptospirosis in rice farmers was reported. The main objective was to determine the efficacy of doxycycline in preventing Leptospira exposure. A taxa de soropreval?ncia de leptospirose na popula??o estudada antes e ap?s a colheita foi de 14,2% (n=22) e de 16.6% (n=27) respectivamente. Anti-Leptospira serology was determined for 61 subjects in two instances, pre and post-exposure to potential contaminated water. There was an increased risk of 29.0 per cent in acquiring infection for individuals that did not use doxycycline. In addition, an increased risk of 30.0 % observed in farmers who did not use protection when exposed to Leptospira. The adhesion to preventive chemoprophylaxis was 55.7%. Therefore doxycycline, under specific circunstances appears to be an effective alternative to protect against leptosprirosis infection. A large sample composed of individuals to adhere to preventive therapy is needed to define time, dosage and length of use of doxycycline in this area / A leptospirose ? uma zoonose de distribui??o mundial que afeta, acidentalmente, o homem tendo como reservat?rios animais dom?sticos e silvestres. Síntese e caracterização de membranas de triacetato de celulose a partir do aproveitamento do bagaço de cana-de-açúcar para a liberação controlada de drogas

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / This work presents a study of the use of sugarcane bagasse as an alternative source of cellulose, for obtaining cellulose triacetate (CTA) and consequently production of membranes for use in controlled release system (CRS) of the drugs naproxen (water insoluble) and doxycycline (water soluble). The sugarcane bagasse was delignified from three successive reflux of a mixture 80/20 (v/v) nitric acid concentrated and ethanol, subsequently treated in NaOH solution. From the analysis of Klason Lignin, noted that in delignified bagasse (DB) there was a reduction in lignin content of 98% compared with gross bagasse. CTA produced presented degree of substitution (DS) 2.80 ± 0.09, what classifies as cellulose triacetate (CTA). The formulations used, CTA/dichloromethane/drug and CTA/dichloromethane/water/drug led to the production of symmetric and asymmetric I membranes, respectively. The formulation with the system solvent dioxane/acetone resulted in asymmetric II membranes with pores in thickness and dense surface, morphologies observed from the analysis of scanning electron microscopy (SEM). The results of the thermal analysis showed that there are interactions between drugs and the CTA. The polymeric matrices showed Swelling index (Si) approximately 12%, a result that will influence on the release kinetics of drugs. The content of naproxen released was 8, 50 and 25% from symmetric, asymmetric I and asymmetric II membranes, respectively. Doxycycline presented release content 14% in experiments with symmetric membranes and 80% with asymmetric I membranes. The study of CRS of drugs showed that membrane morphology has great influence on drugs desorption and the release occurs predominantly by diffusion. / Esse trabalho apresenta um estudo da utilização de bagaço de cana de açúcar como fonte alternativa de celulose, para a obtenção de triacetato de celulose (TAC) e consequentemente produção de membranas para o uso em sistema de liberação controlada (SLC) dos fármacos naproxeno (lipossolúvel) e doxiciclina (hidrossolúvel). O bagaço de cana de açúcar foi deslignificado a partir de três refluxos sucessivos de uma mistura 20/80 (v/v) de ácido nítrico concentrado e etanol hidratado, e posteriormente tratado em solução NaOH. A partir da análise de Lignina Klason, observou se que no bagaço deslignificado (BD) houve uma redução no teor de lignina de 98 % em relação ao bagaço bruto. O TAC produzido apresentou grau de substituição 2,80 ± 0,09, classificando-o como triacetato de celulose (TAC). As formulações utilizadas, TAC/diclorometano/ fármaco e TAC/diclorometano/água/ fármaco levaram à produção de membranas simétricas e assimétricas I, respectivamente. A formulação com o sistema solvente dioxano/acetona resultou em membranas assimétricas II com poros na espessura e superfície mais densa, morfologias observadas a partir das análises de Microscopia Eletrônica de Varredura (MEV). Os resultados da análise térmica mostraram que há interações entre os fármacos e o TAC. As matrizes poliméricas apresentaram Índice de Intumescimento (Ii) de aproximadamente 12 %, resultado que influenciou na cinética de liberação das drogas. O teor do naproxeno liberado foi 8, 50 e 25 % a partir das membranas simétricas, assimétricas I e assimétricas II, respectivamente. A doxiciclina apresentou teor de liberação de 14 % nos experimentos com as membranas simétricas e 80 % com as assimétricas I. O estudo do SLC das drogas mostrou que a morfologia das membranas exerce grande influência na dessorção dos fármacos e a liberação ocorre predominantemente por difusão. / Mestre em Química

Resultados: No primeiro artigo, houve uma melhora de todos os parâmetros clínicos periodontais e dos níveis de HbA1c em ambos os grupos tratados após 6 meses. No grupo que usou doxiciclina, houve uma melhora mais expressiva do índice gengival e daHbA1c, bem como uma quantidade menor de microrganismos Gram-. O segundo artigo evidenciou uma expressão significantemente menor de IL1- no fluido crevicular gengival dos pacientes em uso de doxiciclinina ao final do experimento. Na segunda pesquisa, após 6 meses, houve uma melhora da GJ, dos parâmetros clínicos periodontais e da quantidade de fluido crevicular gengival em ambos os grupos. Entretanto, não houve diferença estatisticamente significativa na expressão de IL-1β após 180 dias.Conclusão: Foi possível observar que a terapia periodontal básica associada ao uso de doxiciclina é mais eficiente para o controle clínico, glicêmico e microbiológico de pacientes portadores de DM2 e doença periodontal, bem como na menor expressão de IL1- no fluido crevicular gengival deste pacientes. Quando comparado os tipos de tratamento periodontal, foi possível observar que o tratamento periodontal convencional é superior à desinfecção total em estágio único, tanto em relação ao controle glicêmico, quanto em relação aos parâmetros periodontais de sangramento a sondagem e área de fluido gengival, bem como em relação a variação dos valores de expressão de IL1-β, apesar de não haver diferença significativa entre os valores iniciais e finais em cada grupo

Avaliação prospectiva aberta do uso prolongado de baixas doses de doxyciclina na rinossinusite crônica com polipose nasal de difícil tratamento / Prospective open-label evaluation of long-term low-dose doxycicline for difficult-to-treat chronic rhinosinusitis with nasal polyps

Introdução: A rinossinusite crônica com polipose nasal constitui um subgrupo particular da rinossinusite crônica, caracterizado por uma inflamação da mucosa que leva a um espessamento da mesma e à formação de pólipos, podendo ser especialmente difícil de tratar. A Doxiciclina é um antibiótico bacteriostático, de largo espectro, que também tem uma ação antiinflamatória, e tem se mostrado útil no controle dos sintomas das recidivas desta doença, promovendo inclusive uma diminuição do pólipo. Objetivo: avaliar se o uso de baixas doses de Doxiciclina, por períodos prolongados, pode melhorar o controle clínico da rinossinusite crônica com polipose nasal, de difícil tratamento. Métodos: este é um estudo prospectivo, aberto, realizado em 60 pacientes com rinossinusite crônica de difícil tratamento que se submeteram a cirurgia endoscópica nasal. Os pacientes foram divididos em 2 grupos: 28 pacientes receberam corticóide nasal, lavagem nasal com soro fisiológico, e Doxiciclina (200mg no primeiro dia, seguido por 100mg uma vez ao dia) por 12 semanas, enquanto 30 pacientes receberam apenas corticóide nasal e lavagem nasal com soro fisiológico. O principal resultado avaliado foi a existência de uma melhora, dose efeito, clinicamente significativa do SNOT-20 após o tratamento com a Doxiciclina. Outros resultados avaliados foram os valores do SNOT-20, NOSE e do Lund-Kennedy. Os seguintes parâmetros também foram analisados: asma, rinite, doença respiratória exacerbada pela Aspirina (DREA), níveis séricos de IgG, IgA, IgE, IgM, ANCA e contagem de eosinófilos. Resultados: oy tratamento com a Doxiciclina promoveu uma melhora, dose efeito, clinicamente significativa do SNOT-20. Pacientes que receberam a Doxiciclina também tiveram resultados significativamente melhores do SNOT-20, NOSE e Lund-Kennedy. Houve uma associação negativa entre a melhora clinicamente significativa do SNOT-20 e a presença de asma, DREA e níveis séricos elevados de IgE pré-tratamento. Conclusão: os achados sugerem que a doxiciclina pode ter uma ação benéfica nos pacientes com rinossinusite crônica com polipose nasal, especialmente naqueles pacientes sem asma, DREA ou níveis séricos elevados de IgE prétratamento / Introduction: Chronic rhinosinusitis with nasal polyps is a particular subset of chronic rhinosinusitis characterized by a mucosal inflammation that leads to mucosal thickening and polyp formation, and can be especially difficult to treat. Doxycycline is an oral, available, broad-spectrum bacteriostatic antibiotic which also has anti-inflammatory action, that has been used to treat this disease and has shown a successful control of symptoms even reducing the volume of polyps. Objective: Evaluate if long-term low-dose doxycycline is effective in controlling clinical symptoms of difficult-to-treat chronic rhinosinusitis with nasal polyps. Methods: This was a prospective, open-label study of 60 patients with difficultto- treat chronic rhinosinusitis with nasal polyps who had undergone endoscopic sinus surgery. Patients were divided into two groups: 28 received nasal steroids, saline irrigation, and doxycycline (200 mg on the first day, followed by 100 mg daily) for 12 weeks, while 30 received only nasal steroids and saline irrigation. The main outcome measure was an adequate effect size of doxycycline treatment on clinically meaningful significant improvement of SNOT-20. Other outcome measures were the SNOT-20, NOSE, and Lund-Kennedy scores. The following parameters were also analyzed: asthma, rhinitis, non-steroidal-exacerbated respiratory disease (NERD), and baseline serum IgG, IgA, IgE, IgM, ANCA, and eosinophil count. Results: There was an adequate effect size of doxycycline treatment on clinically meaningful significant improvement of SNOT-20. Patients who received doxycycline also had significantly better outcomes regarding SNOT-20, NOSE, and Lund-Kennedy scores. There was a negative association among a clinically significant improvement of SNOT-20 and presence of asthma, NERD, and elevated serum IgE levels before treatment. Conclusion: These findings suggest that doxycycline may have a beneficial role for chronic rhinosinusitis with nasal polyps patients, especially for those without asthma, NERD or high levels of serum IgE before treatment

Vergleich zwei verschiedener Antibiotika als Adjuvans in der Therapie rasch fortschreitender Parodontitis

Hintergrund: Zahlreiche Studien haben den therapeutischen Nutzen von systemischem Antibiotika in der Behandlung schwerer Parodontitis gezeigt. Bisher ist allerdings noch nicht geklärt, welches antibiotische Behandlungkonzept das Geeignetste ist. Daher sollen in einer randomisierten, klinischen Blindstudie zwei verschiedene, systemische Antibiotika adjuvant zur konventionellen, instrumentellen Behandlung von Patienten mit rapid progressiver Parodontitis (RPP) miteinander verglichen werden. Material und Methode: 33 Patienten mit klinisch und radiologisch gesicherter RPP-Diagnose wurden auf 2 Gruppen verteilt: (1) AM-Gruppe (n=17): 500 mg Amoxicillin und 250 mg Metronidazol (3*/ Tag - 10 Tage), (2) D-Gruppe (=16) 200mg Doxycyclin am 1.Tag und 100mg Doxycyclin 13 Tage. Zu Beginn erhielten alle Patienten 3* eine professionelle Zahnreinigung, und anschlie§end Scaling und Wurzelglättung unter Lokalanästhesie in 2 Sitzungen. 3 Monate später wurde ein Recall und die Antibiose durchgeführt. Im Abstand von jeweils 3 Monaten erfolgten 2 weitere Recallsitzungen. Nach erfolgreicher Mundhygieneinstruktion und zu allen Recallsitzungen wurden alle klinischen Parameter wie Taschentiefe, relatives Attachmentlevel und das Bluten bzw. Pus nach Sondieren mit der Florida probe eruiert. Die Bestimmung 8 verschiedener Parodontalpathogene wurde mit dem DNS-Sondentest von Meridol durchgeführt. Die mikrobiologischen Proben wurden mit sterilen Papierspitzen an den vier tiefsten Taschen vor der Antibiose und zu den anschlie§enden Recalls entnommen und mit der Gensondentechnik im Labor der Wybert GmbH elmex Forschung, Lörrach, analysiert. Zur Bestimmung des IL-1-Genpolymorphismus wurde venöses Blut in der 1. Sitzung abgenommen. Die Auswertung erfolgte mit dem GenoType(R) PRTest (Hain Diagnostika GmbH). Therefore the purpose of this study is to compare two different systemic antibiotics adjunctive to a conventional, mechanical treatment of patients with rapidly progressive periodontitis (RPP) in a randomised, blinded, clinical trial. Untersuchung der Funktion des Transkriptionsfaktors GATA-4 durch eine Mausmutante mit einem induzierbaren RNA-Interferenz System

Hintergrund: Der Transkriptionsfaktor GATA-4 ist für die normale Entwicklung des Endoderms essentiell. Mausmutanten mit einer homozygoten Deletion des gata-4 Gens versterben zwischen den embryonalen Tagen 8.5 - 10.5 aufgrund einer Störung der ventralen Morphogenese und der Ausbildung des Herzschlauches. Zielsetzung und experimentelle Strategie: Um die Bedeutung von GATA-4 auch nach der embryonalen Entwicklung untersuchen zu können, wurden doppelt-transgene Mäuse generiert. Diese Mausmutanten exprimieren einen Tetrazyklin-Repressor und eine gegen GATA-4 gerichtete short hairpin RNA (shGATA-4). Die Expression der shGATA-4 steht dabei unter der Kontrolle eines H1-Promotors, welcher durch ein Tetrazyklin-Operator Element modifiziert wurde. Dadurch ist das System durch Doxyzyklin induzierbar. Ergebnisse: Die Integration der Transgene in dem Genom der Maus wurde durch Southern-Blot Analyse nachgewiesen. Die Expression der shGATA-4 wurde durch die Applikation von Doxyzyklin über das Trinkwasser (20 mg/ml) induziert. Langzeitstudien am Herzen haben dabei eine signifikante Suppression von GATA-4 nach 38 Tagen ergeben (80 %). Diese Reduktion konnte durch Western-Blot Analyse bestätigt werden. Obwohl die Expression verschiedener Zielgene von GATA-4 (ANF, BMP-4) ebenfalls herunterreguliert war, fiel bei den transgenen Mäusen kein kardialer Phänotyp auf. Jedoch wurde die GATA-4 Expression in den Hoden und Ovarien transgener Mäuse supprimiert, nachdem shGATA-4 durch die Applikation von Doxyzyklin induziert wurde. Weiterführende Untersuchungen an adulten Mäusen zeigten eine GATA-4 Reduktion von 20 % auch in nicht mit Doxyzyklin-induzierten Mausmutanten. Diese Reduktion könnte durch einen sog. leaky-Effekt des shGATA-4 Transgens hervorgerufen worden sein, wodurch die stark eingeschränkte Fertilität dieser Mauslinie erklärt werden könnte. Interessanterweise haben ca. 10 % der mit Doxyzyklin behandelten transgenen Weibchen Ovarial-Teratome ausgebildet. Histologisch wiesen diese Teratome überwiegend (neuro-) ektodermale, vereinzelt mesodermale und nahezu keine endodermalen Strukturen auf. Schlussfolgerung: In diesem Modell hat die Suppression von GATA-4 keinen Einfluss auf die Funktion des Herzens der adulten Maus. Jedoch scheint GATA-4 für die Fertilität der Maus von großer Bedeutung zu sein. Weiterhin scheint die Suppression von GATA-4 mit der Ausbildung von Ovarial-Teratomen assoziiert zu sein. / Background: The transcription factor GATA-4 is crucial for the normal endodermal development. In mice, homozygous deficiency of GATA-4 causes defects in ventral morphogenesis and heart tube formation, resulting in embryonic death between day e8.5 and e10.5. Aim and experimental strategy: To analyze the implication of GATA-4 beyond embryonic development a double transgenic mouse expressing the tetracycline repressor (TetR) and an inducible small interfering RNA directed against GATA-4 was generated. This expression construct contains a H1 promoter modified with a tetracycline operator upstream of the coding region for the GATA-4 short hairpin RNA (shGATA-4). Results: The integration of the transgenes in FvB mice (H1:G4/TetR) was confirmed by Southern blot. To induce the expression of the shGATA-4 construct, transgenic mice were treated with doxycycline (20 mg/ml drinking water). In longitudinal analysis, most efficient GATA-4 suppression was detected after 38 days. Quantitative PCR revealed a GATA-4 reduction of about 80 % in the heart, if normalized against the wildtype. Reduction of GATA-4 was confirmed by Western Blot. Although GATA-4 target genes (ANP, BMP-4) were down regulated, the animals showed no clinical phenotype. In opposite to wildtype mice, GATA-4 expression was undetectable in the ovaries and testis of transgenic mice with induced shGATA-4. Additional analysis in adult transgenic mice, which were not treated with doxycycline, also showed a reduction of GATA-4 expression of about 20 %, probably caused by a leaky-effect of the transgene. This may explain the significantly reduced fertility of the colony. Importantly, 10 % of transgenic females treated with doxycycline developed ovarian teratomas. Histological examination of teratomas showed predominantly (neuro-) ectodermal and to a lower degree mesodermal, but almost no endodermal compounds. Conclusions: GATA-4 reduction in the adult murine heart is – at least to a certain degree – clinically redundant. GATA-4 seems to be required for normal fertility. In our model GATA-4 deficiency seems to be associated with an increased risk for developing ovarian teratoma.
