Teste de memória integrativa: comparação de desempenho entre demência frontotemporal variante comportamental e doença de Alzheimer / Memory binding test: comparisons between Alzheimer\'s disease and behavioural variant frontotemporal dementia

INTRODUÇÃO: O teste de memória integrativa (TMI) de curto prazo, nas modalidades de recordação espontânea (RE) e reconhecimento visual (RV), tem se mostrado promissor para apoiar o diagnóstico precoce da doença de Alzheimer (DA), com grande potencial para aplicação clínica. Até o momento, não foram encontrados estudos sobre o TMI junto à população idosa brasileira, nem tampouco pesquisas utilizando a tarefa de RV em pacientes com a variante comportamental da demência frontotemporal (DFTvc). OBJETIVOS: A presente pesquisa comparou o desempenho de pacientes com DA, pacientes com DFTvc e Controles nas duas modalidades de TMI e examinou a acurácia diagnóstica dos testes para a detecção da DA. MÉTODOS: Foram avaliados participantes de três centros de pesquisa, USP São Paulo, USP Ribeirão Preto e Universidade Federal de Minas Gerais, assim como do Instituto Paulista de Geriatria e Gerontologia. A amostra foi composta por 85 indivíduos, subdivididos em 32 Controles, 35 pacientes com DA e 18 pacientes com DFTvc. Foram geradas curvas ROC para calcular a acura?cia diagno?stica dos testes contrastando os grupos diagno?sticos. Realizou-se análises de regressão logística, de correlação e análises de interação entre diagnóstico e condição experimental (características integradas x não-integradas). RESULTADOS: No TMI com RE, Controles apresentaram resultados semelhantes aos pacientes com DFTvc e os pacientes com DA mostraram resultado significativamente menor em relac?a?o aos dois primeiros grupos (Controles = DFTvc > DA). No TMI com RV, Controles mostraram escores significativamente maiores que os pacientes com DFTvc e DA (Controles > DA = DFTvc). A acurácia diagnóstica do teste de RE para DA foi de 0,853, com sensibilidade e especificidade de 84,4% e 80%, respectivamente. A acurácia diagnóstica do teste de RV para DA foi de 0,809, com sensibilidade e especificidade de 65,4% e 72,2%, respectivamente. As modalidades do TMI mostraram correlação significativa com outros testes neuropsicológicos. A tarefa de RV apresentou correlação maior com testes relacionados as funções executivas e de atencão, enquanto que a tarefa de RE mostrou correlação com testes que avaliam memória episódica e funcões executivas. CONCLUSÕES: O presente estudo demonstrou a aplicabilidade do TMI na população brasileira. As tarefas apresentaram características adequadas para a prática clínica: são fáceis e rápidas de aplicar, necessitam de pouco tempo de aplicação, e podem contribuir para o diagnóstico da DA. Especialmente a tarefe de RE, que foi capaz de diferenciar DA de DFTvc / INTRODUCTION: The free recall (FR) and visual recognition (VR) modalities of the memory binding test (MBT) are promising tools to support the early diagnosis of Alzheimer\'s disease (AD). Until the present moment, no studies have been conducted with these tests among the elderly population in Brazil, and there are no studies with the VR modality of the MBT involving behavioural variant frontetemporal dementia (bvFTD) patients. OBJECTIVES: the present research examined the diagnostic accuracy of the two modalities of the MBT for the detection of AD, and compared the performance of patients with AD, bvFTD and controls. METHODS: Participants of three research centres were assessed, USP São Paulo, USP Ribeirão Preto and Federal University of Minas Gerais, and from the Paulista Institute of Geriatrics and Gerontology. The sample comprised 85 participants: 32 controls, 35 AD and 18 bvFTD patients. ROC analyses were used to examine the diagnostic accuracy of the MBT contrasting the different diagnostic groups. Logistic regression analysis, correlations were calculated. ANOVA analyses tested the interaction between diagnosis and experimental condition (features bound and unbound). RESULTS: In the FR task, controls and bvFTD showed similar results, and AD patients showed worse performance (controls = bvFTD > AD). In the VR task, controls showed better performance than the clinical groups (controls > AD = bvFTD). The diagnostic accuracy of the FR task for AD was 0.853, with 84.4% of sensitivity and 80% of specificity. The diagnosis accuracy of the VR task for AD was 0.809, with 65.4% of sensitivity and 72.2% of specificity. The MBT showed significant correlation with other neuropsychological tests. The VR task correlated with tests associated with executive functions and attention, while the FR task correlated with episodic memory and executive functions. CONCLUSIONS: The present study showed the applicability of the MBT for the Brazilian population. The tasks presented adequate characteristics for clinical use: they are quick to administer and can contribute to the diagnosis of AD. Especially the FR task, that could differentiate AD from bvFTD

INTRODUÇÃO: Julgamento é a capacidade de tomar decisões após cuidadosa consideração das informações disponíveis, soluções possíveis, resultados prováveis e fatores contextuais. Do ponto de vista neuropsicológico, o conceito de julgamento envolve memória, linguagem, atenção, raciocínio e principalmente funções executivas. Perda de julgamento tem sido descrita em Comprometimento Cognitivo Leve (CCL) e demência. O Test of Practical Judgment (TOP-J) é uma medida originalmente americana, desenvolvida para avaliar o julgamento prático em adultos mais velhos. É um questionário aberto de 15 itens (TOP-J/15) ou de 9 itens (TOP-J/9) no qual os participantes escutam breves cenários sobre os problemas cotidianos e relatam em voz alta as soluções propostas. Este estudo teve como objetivos a adaptação do TOP-J para uso no Brasil, a elaboração de uma versão reduzida deste instrumento e verificação da acurácia de ambas a versões em amostra da população brasileira composta de controles cognitivamente saudáveis e pacientes com Comprometimento Cognitivo Leve (CCL), doença de Alzheimer (DA) e demência frontotemporal variante comportamental (DFT). MÉTODOS: A amostra foi composta por 85 indivíduos, com idade mínima de 50 anos e escolaridade mínima de 4 anos, sendo 24 controles, 26 CCL, 20 DA e 15 DFT. Os participantes foram submetidos a avaliação neuropsicológica e ao TOP-J. RESULTADOS: No TOP-J/15 versão brasileira (TOP-J/15-Br) e no TOP-J/9 versão brasileira (TOP-J/9-Br) (versão reduzida), o desempenho de controles foi estatisticamente superior ao desempenho de pacientes com CCL, DA e DFT e o desempenho de CCL foi superior ao de pacientes com DA e DFT. No TOP-J/15-Br, a confiabilidade verificada pelo alfa de Cronbach foi de 0,69 e o melhor ponto de corte para distinção de controles e pacientes foi de 30 (sensibilidade de 91,7%; especificidade de 59% e área sob a curva de 0,80). No TOP-J/9-Br, o alfa de Cronbach foi de 0,68 e o melhor ponto de corte para distinção entre julgamento de controles e de pacientes foi 19, com sensibilidade de 79,2, especificidade de 72,1 e área sob a curva de 0,82. CONCLUSÕES: O TOP-J/15-Br e o TOP-J/9-Br apresentaram características psicométricas robustas para o uso pretendido com amostra da população brasileira. Ambos foram capazes de identificar prejuízo de julgamento já em pacientes com CCL e diferenciaram julgamento de controles do julgamento de pacientes com boa sensibilidade e especificidade / INTRODUCTION: Judgment is the ability to make sound decisions after careful consideration of available information, possible solutions, likely outcomes and contextual factors. From a neuropsychological perspective, the concept of judgment involves memory, language, attention, reasoning and specially the executive functions. Loss of judgment has been described in Mild Cognitive Impairment (MCI) and dementia. The Test of Practical Judgment (TOP-J) is an originally American measure created for evaluate practical judgment in older adults. It is a 15-item (TOP-J/15) or 9- item (TOP-J/9) open-ended questionnaire in which participants listen to brief scenarios about everyday problems and report aloud their proposed solutions. This study aimed the adaptation of the TOP-J for use in Brazil, the development of a reduced version of this instrument and the verification of accuracy of both the versions in Brazilian sample composed of cognitively healthy controls and patients with mild cognitive impairment (MCI), Alzheimer\'s disease (AD) and frontotemporal dementia behavioral variant (FTD). METHODS: The sample consisted of 85 subjects with a minimum age of 50 years and minimum education of 4 years, being 24 controls, 26 MCI, 20 AD e 15 FTD. The participants were submitted to a neuropsychological assessment and TOP-J. RESULTS: In the TOP-J/15 Brazilian version (TOP-J/15-Br) and in the TOP-J/9 Brazilian version (TOP-J/9-Br) (reduced version), performance of controls was statistically better than the performance of MCI, AD and FTD patients, and performance of MCI was statistically better than AD and FTD patients. In TOP-J/15-Br, the reliability verified by Cronbach\'s alpha was 0.69 and the best cutoff for distinction between controls and patients was 30 (sensibility of 91,7%; specificity of 59% and area under the curve of 0,8). In the TOP-J/9-Br, Cronbach\'s alpha was 0.68 and the best cutoff point for distinguishing between judgment of controls and judgment of patients is 19, with a sensitivity of 79,2, specificity of 72,1 and area under the curve of 0.82. CONCLUSIONS: The TOP-J/15-Br and the TOP-J/9-Br showed robust psychometric characteristics for the intended use with a sample of the Brazilian population. Both were able to identify deficits of impaired of judgment already in patients with MCI and were able to distinguish judgment of controls from judgment of patients with good sensitivity and specificity

Apathy and impulsivity in frontotemporal lobar degeneration syndromes

There has been considerable progress in the clinical, pathological and genetic fractionation of frontotemporal lobar degeneration syndromes in recent years, driving the development of novel diagnostic criteria. However, phenotypic boundaries are not always distinct and syndromes converge with disease progression, limiting the insights available from traditional diagnostic classification. Alternative transdiagnostic approaches may provide novel insights into the neurobiological underpinnings of symptom commonalities across the frontotemporal lobar degeneration spectrum. In this thesis, I illustrate the use of transdiagnostic methods to investigate apathy and impulsivity. These two multifaceted constructs are observed across all frontotemporal lobar degeneration syndromes, including frontotemporal dementia, progressive supranuclear palsy and corticobasal syndrome. They cause substantial patient morbidity and carer distress, often coexist and are undertreated. Using data from the Pick’s disease and Progressive supranuclear palsy Prevalence and INcidence (PiPPIN) Study, I examine the frequency, characteristics and components of apathy and impulsivity across the frontotemporal lobar degeneration spectrum. A principal component analysis of the neuropsychological data identified eight distinct components of apathy and impulsivity, separating patient ratings, carer ratings and behavioural tasks. Apathy and impulsivity measures were positively correlated, frequently loading onto the same components and providing evidence of their overlap. The data confirmed that apathy and impulsivity are common across the spectrum of frontotemporal lobar degeneration syndromes. Voxel based morphometry revealed distinct neural correlates for the components of apathy and impulsivity. Patient ratings correlated with white matter changes in the corticospinal tracts, which may reflect retained insight into their physical impairments. Carer ratings correlated with grey and white matter changes in frontostriatal, frontotemporal and brainstem systems, which have previously been implicated in motivation, arousal and goal directed behaviour. Response inhibition deficits on behavioural tasks correlated with focal frontal cortical atrophy in areas implicated in goal-directed behaviour and cognitive control. Diffusion tensor imaging was highly sensitive to the white matter changes underlying apathy and impulsivity in frontotemporal lobar degeneration syndromes. Diffusion tensor imaging findings were largely consistent with voxel-based morphometry, with carer ratings reflecting widespread changes while objective measures showed changes in focal, task-specific brain regions. White matter abnormalities often extended beyond observed grey matter changes, providing supportive evidence that white matter dysfunction represents a core pathophysiology in frontotemporal lobar degeneration. Apathy was a significant predictor of death within two and a half years from assessment, consistent with studies linking apathy to poor outcomes. The prognostic importance of apathy warrants more accurate measurement tools to facilitate clinical trials. Although causality remains unclear, the influence of apathy on survival suggests effective symptomatic treatments may also prove disease-modifying. These findings have several implications. First, clinical studies for apathy/impulsivity in frontotemporal lobar degeneration syndromes should target patients who present with these symptoms, irrespective of their diagnostic category. Second, data-driven approaches can inform the choice of assessment tools for clinical trials, and their link to neural drivers of apathy and impulsivity. Third, the components and their neural correlates provide a principled means to measure (and interpret) the effects of novel treatments in the context of frontotemporal lobar degeneration.

The histopathological and immunohistochemical technique makes it possible to differentiate the subtypes of dementia by pre-established macroscopic and microscopic criteria. Banks brain, although recent, provide biological material quality for multidisciplinary research in normal subjects and with dementia. Objectives: To correlate clinical findings with neuropathological cases with dementia from the Brains Bank of Central of Brazil (BBCB); establish morphological patterns in macroscopic focal dementias; determine the prevalence of diagnosis of other types of dementia. Materials and Methods: Brain Study from autopsies of patients with neurodegenerative diseases of dementia clinic of the HC-UFG (Ethics Committee of the Protocol on research 0692007). The brains were processed following dissection and measurement protocol. Appropriate external and macroscopic morphological descriptions and coronal and sagittal sections were performed. Results: 15 brains, 9 female patients were studied, aged 10 to 89 years. The types of dementias found in BCBC were 5 cases of frontotemporal dementia (FTD), 3 Alzheimer's disease (AD), 3 patients had primary progressive aphasia (PPA) and corticobasal degeneration (CBD), 1 Huntington's disease, 1 disease Creutzfeldt-Jakob 1, Rasmussen's encephalitis and 1 depressive pseudodementia (Cotard’s syndrome). Described frontal gyrus supernumerary in 3 cases of CBD and 2 cases of FTD. Discussion: Most cases presented morphological pattern of the respective type of dementia according to the literature, except PPA with CBD. In BCBC material only 20% of AD cases were 27% and frontotemporal lobar degeneration (FTLD). Conclusion: The higher prevalence of dementia in BCBC was the type FTLD. The frontotemporal focal atrophy was the most observed type of change. The cases with FTD showed classic morphological patterns, while the PPA CBD was different standard literature. The BCBC will enable studies in various research areas. / O diagnóstico definitivo das demências é feito através de necropsia. Os exames anatomopatológico e imunoistoquímico possibilitam diferenciar os subtipos de demência por critérios macroscópicos e microscópicos pré-estabelecidos. Os bancos de cérebros, apesar de recentes, fornecem material biológico de qualidade para pesquisas multidisciplinares de indivíduos normais e com demência. Objetivos: Correlacionar aspectos clínicos com alterações neuropatológicas de casos com demências provenientes do Banco de Cérebros do Brasil Central (BCBC); estabelecer padrões morfológicos macroscópicos nas demências focais; verificar a prevalência do diagnóstico de outros tipos de demências. Materiais e Métodos: Estudo de cérebros provenientes de necropsias de pacientes com doenças neurodegenerativas do ambulatório de demências do HC-UFG (protocolo do Comitê de ética em pesquisa 0692007). Os cérebros foram processados seguindo protocolo de dissecção e mensuração. Foram realizadas as devidas descrições morfológicas e macroscópicas externas e dos cortes coronais e sagitais. Resultados: Foram estudados 15 cérebros, 9 de pacientes do sexo feminino, com idade entre 10 a 89 anos. Os tipos de demências encontrados no BCBC foram: 5 casos de demência frontotemporal (DFT), 3 de doença de Alzheimer (DA), 3 casos com afasia progressiva primaria (APP) e degeneração corticobasal (DCB), 1 de doença de Huntington, 1 de doença de Creutzfeldt-Jakob, 1 de encefalite de Rasmussen e 1 de pseudodemência depressiva (síndrome de Cotard). Foi observado giro frontal supranumerário nos 3 casos de DCB e em 2 casos de DFT. Discussão: A maioria dos casos apresentou padrão morfológico do respectivo tipo de demência de acordo com a literatura, exceto APP com DCB. No material do BCBC apenas 20% dos casos foram de DA e 27% degeneração lobar frontotemporal (DLFT). Conclusão: A maior prevalência de demências no BCBC foi do tipo DLFT. A atrofia focal frontotemporal foi o tipo de alteração mais observada. Os casos com DFT apresentaram padrões morfológicos clássicos, enquanto que os de APP com DCB apresentaram padrão diferente da literatura. O BCBC possibilitará a realização de estudos em várias linhas de pesquisa.

Este estudo teve como objetivos a adaptação do TOP-J para uso no Brasil, a elaboração de uma versão reduzida deste instrumento e verificação da acurácia de ambas a versões em amostra da população brasileira composta de controles cognitivamente saudáveis e pacientes com Comprometimento Cognitivo Leve (CCL), doença de Alzheimer (DA) e demência frontotemporal variante comportamental (DFT). MÉTODOS: A amostra foi composta por 85 indivíduos, com idade mínima de 50 anos e escolaridade mínima de 4 anos, sendo 24 controles, 26 CCL, 20 DA e 15 DFT. Os participantes foram submetidos a avaliação neuropsicológica e ao TOP-J. RESULTADOS: No TOP-J/15 versão brasileira (TOP-J/15-Br) e no TOP-J/9 versão brasileira (TOP-J/9-Br) (versão reduzida), o desempenho de controles foi estatisticamente superior ao desempenho de pacientes com CCL, DA e DFT e o desempenho de CCL foi superior ao de pacientes com DA e DFT. No TOP-J/15-Br, a confiabilidade verificada pelo alfa de Cronbach foi de 0,69 e o melhor ponto de corte para distinção de controles e pacientes foi de 30 (sensibilidade de 91,7%; especificidade de 59% e área sob a curva de 0,80). No TOP-J/9-Br, o alfa de Cronbach foi de 0,68 e o melhor ponto de corte para distinção entre julgamento de controles e de pacientes foi 19, com sensibilidade de 79,2, especificidade de 72,1 e área sob a curva de 0,82. CONCLUSÕES: O TOP-J/15-Br e o TOP-J/9-Br apresentaram características psicométricas robustas para o uso pretendido com amostra da população brasileira. Ambos foram capazes de identificar prejuízo de julgamento já em pacientes com CCL e diferenciaram julgamento de controles do julgamento de pacientes com boa sensibilidade e especificidade / INTRODUCTION: Judgment is the ability to make sound decisions after careful consideration of available information, possible solutions, likely outcomes and contextual factors. From a neuropsychological perspective, the concept of judgment involves memory, language, attention, reasoning and specially the executive functions. Loss of judgment has been described in Mild Cognitive Impairment (MCI) and dementia. The Test of Practical Judgment (TOP-J) is an originally American measure created for evaluate practical judgment in older adults. It is a 15-item (TOP-J/15) or 9- item (TOP-J/9) open-ended questionnaire in which participants listen to brief scenarios about everyday problems and report aloud their proposed solutions. This study aimed the adaptation of the TOP-J for use in Brazil, the development of a reduced version of this instrument and the verification of accuracy of both the versions in Brazilian sample composed of cognitively healthy controls and patients with mild cognitive impairment (MCI), Alzheimer\'s disease (AD) and frontotemporal dementia behavioral variant (FTD). METHODS: The sample consisted of 85 subjects with a minimum age of 50 years and minimum education of 4 years, being 24 controls, 26 MCI, 20 AD e 15 FTD. The participants were submitted to a neuropsychological assessment and TOP-J. RESULTS: In the TOP-J/15 Brazilian version (TOP-J/15-Br) and in the TOP-J/9 Brazilian version (TOP-J/9-Br) (reduced version), performance of controls was statistically better than the performance of MCI, AD and FTD patients, and performance of MCI was statistically better than AD and FTD patients. In TOP-J/15-Br, the reliability verified by Cronbach\'s alpha was 0.69 and the best cutoff for distinction between controls and patients was 30 (sensibility of 91,7%; specificity of 59% and area under the curve of 0,8). In the TOP-J/9-Br, Cronbach\'s alpha was 0.68 and the best cutoff point for distinguishing between judgment of controls and judgment of patients is 19, with a sensitivity of 79,2, specificity of 72,1 and area under the curve of 0.82. CONCLUSIONS: The TOP-J/15-Br and the TOP-J/9-Br showed robust psychometric characteristics for the intended use with a sample of the Brazilian population. Both were able to identify deficits of impaired of judgment already in patients with MCI and were able to distinguish judgment of controls from judgment of patients with good sensitivity and specificity

New intracellular mechanisms involved in Alzheimer's disease and frontotemporal dementia

Dementia causes an increasing social and economic burden worldwide, demanding action regarding its diagnosis, treatment and everyday management. Recent years have seen many advances in neurodegeneration research, but the search for new truly disease modifying therapies for Alzheimer's disease (AD) and frontotemporal dementia (FTD) has so far not been successful. This is mainly due to a lack of understanding of the precise intracellular events that lead up to neuronal dysfunction in early and in late stages of the disease. This thesis describes the approaches taken to extend the current knowledge about the intracellular effects of neuronal amyloid-beta and the signalling pathways causing neuronal death or disturbed synaptic function in dementia. Endophilin-1(Ep-1), amyloid-binding alcohol dehydrogenase (ABAD), peroxiredoxin-2 (Prx-2) and the EF-hand domain family, member D2 (EFHD2) have been found to be elevated in the human brain with dementia and in mouse models for frontotemporal lobar degeneration (FTLD) or AD. The expression of these proteins as well as the expression of c-Jun N-terminal kinase (JNK), c-Jun and APP were analysed by western blotting and real-time PCR in human brains affected by AD or FTLD as well as in mouse models for AD. This provided a new insight into the regulation of these proteins in relation to each other in the ageing brain and uncovered a new potential link between elevated levels of EFHD2, Prx-2 and APP in FTLD. By studying the effects of the overexpression of Ep-1 in neurons, this research has led to a better understanding of its role in JNK-activation. It furthermore verified a protective role for Prx-2 against neurotoxicity and pointed towards a new function for Prx-2 in the regulation of JNK-signalling. The analysis of the effect of increased levels of EFHD2 uncovered for the first time its involvement in the PI3K-signalling cascade in neuronal cells. The current work has therefore contributed to the knowledge about the cellular processes that are affected by Ep-1, Prx-2 and EFHD2 in different types of dementia and will greatly benefit future research into their actions in the neuronal network.

Vers un raffinement des critères langagiers entre la Démence fronto-temporale et la maladie d'Alzheimer / Beyond lingüística differences between frontotemporal dementia and Alzheimer's disease

Le diagnostic différentiel de Démence Fronto-Temporale (DFT) vs Maladie d’Alzheimer (MA) n’est pas toujours facile à établir, et peuvent être confondus avec ceux de l’Aphasie Primaire Progressive (APP) » (Snowden et al. 2011) puisque l’anomie est présente dans les deux pathologies (Assal et al. 2009, Barkat-Defadas et al. 2008, Léger et al. 2007). La MA peut se décrire comme une démence dégénérative, avec une perte de tissu neuronal plutôt dans les lobes pariétal et temporal, résultant dans des difficultés dans les domaines de l’attention, les fonctions exécutives, la mémoire, les compétences pour apprendre, le langage, le calcul, et les fonctions viso-spatiales (Robin et al. 2003). De l’autre coté, la DFT est une démence qui atteint plutôt les lobes frontal et temporal elle peut se classifier d’après trois variantes: variante comportementale (DFTbf), Aphasie primaire progressive (APP) et démence sémantique (DS) (Kertesz et al 2003). Dans les deux cas nous pouvons trouver des troubles communs du à la perte du tissu dans les régions temporales, dans les étapes initiales de la maladie. L’analyse du langage chez des patients avec DFT et MA, et la corrélation de celui-ci avec le profil psycholinguistique permettra l’isolement des caractéristiques cliniques propres du profil des patients, a fin de proposer des critères plus spécifiques pour disjoindre le profil clinique de langage entre la DFT et la MA.La présente thèse sera réalisée en co-tutelle entre l’hôpital San Ignacio / Pontificia Universidad Javeriana et l’Université Lumière-Lyon2. Ce projet poursuit trois objectifs : (i) recueillir des données psycholinguistiques et de langage (tenant compte des années de scolarité et la sévérité du cours de la maladie), (ii) étudier et isoler les variantes linguistiques, qui peuvent influencer erronément la différence des critères de diagnostique entre la DFT et MA; (iii) raffiner les critères de diagnostique de la DFT et MA. 75 sujets ont été évalués (FTD n = 63, AD = 12, 22 sujets contrôles). Le test d'afastie (BAT) (Paradis 1989) a été utilisé pour identifier la discrimination auditive, la compréhension des structures syntaxiques (auditive et lecture), la production de phrases, la compréhension des narrations, la lecture de phrases, ainsi qu'une analyse des erreurs selon la structure syntaxique. En outre, nous évaluons les fonctions exécutives et les tests cognitifs de base. Les échantillons de parole spontanée ont été transcrits en utilisant le format CHAT et analysés à l'aide des programmes CLAN. En adition aux analyses descriptives, une analyse de proximité des distances euclidiennes au carré a été effectuée ainsi que des corrélations et régréssions.Les résultats montrent que patients atteints de PPA ont eu la pire performance dans la plupart des tâches langagières. Les DFTvf montrent une bonne peormance pour les tâches standardisées, mais montrent des difficultées isolées pour la compréhension de structures syntaxiques de type négatif qui peut s’associer à l'incapacité de représenter une séquence temporelle de la phrase. Les résultats démontrent que le discours spontané nous permet de différencier tous les groupes de patients. Le discours de ces patients peut être distinctif et reflète non seulement les capacités linguistiques du sujet, mais aussi les autres fonctions cognitives. Même si la théorie différencie les groupes PPA selon leur fluence, les résultats montrent qu’avec les variables qui portent sur la fluence de la parole qu'il n'est pas possible de différencier la variante non fluente et la logopénique. D'autres variables telles que les erreurs et la grammaire doivent être incluses dans les analyses pour obtenir le diagnostic différentiel des variantes de la APP. La parole spontanée démontre être un outil inestimable dans la pratique quotidienne du clinicien pour les diagnostics précoces et les critères pour un diagnostic différentiel entre la MA et la DFT – et ces trois variantes-. / Even though the large majority of cortical dementia is of the Alzheimer’s disease type, there are differential diagnosis limitations of current diagnostic criteria for early-onset Alzheimer disease (AD) and frontotemporal lobar degeneration (FTLD, all three variants: behavioural variant frontotemporal –FTDbv-, Primary progressive aphasia –PPA- and semantic dementia –SD-). Initially, either AD and FTD share cognitive declines and first complain includes memory and no language in FTLD or language and no memory in AD. For instance, word retrieval deficits are common in AD and are thought to reflect a degradation of semantic memory. Yet, the nature of semantic deterioration in AD and the underlying neural correlates of these semantic memory changes remain largely unknown (Wierenga, 2011). On the other hand, PPA, FTDbv and SD have, as well, common behavioural features: PPA, prior to word loss, may show apathy and semantic dementia patients have, long before, the pure meaning loss, a great decline on emotional and conduct process. Since new guidelines for the differential diagnosis of behavioural FTD (Raskovsky, 2011) and selective language features –nonfluent/agrammatic, semantic, syntax, narrative - of the language variants are being revised (Gorno-Tempini, 2011) the present project present a novel way to define criteria regarding cognition profile through language analysis in all: AD and FTDL all variants. In sum, isolation of pure clinical characteristics in order to enriched the clinic profile and subtract the pure linguistic features will facilitate better common clinical compromise besides language differences (Zanini et al. 2011, Hernández et al. 2011, Ardila& Ramos, 2008). The present study aims to study through the Javeriana University Memory Clinic (PUJMC) at the San Ignacio Hospital in Colombia, based on the experience in the last 14 years where almost 3000 patients have been studied, language semiology between AD, FDT – all three variants. This project has three objectives: (i) to collect psycholinguistic and language data (taking into account the years of schooling and the severity of the course of the disease), (ii) study and isolate linguistic variants that may erroneously influence the difference in diagnostic criteria between DFT and MA; (iii) refining the diagnostic criteria for FTD and MA. 75 subjects were assessed (FTD n= 63, AD= 12, 22 control sample subjects). Bilingual Aphasia Test (BAT) (Paradis 1989) was used in order to identify auditory discrimination, syntactic structures comprehension (auditory and by reading), sentences production, auditory and by reading, comprehension of narrative structures, sentences reading, as well as an analysis of errors according to the syntactic structure. Additionally, we assess executive functions and basic cognition battery. Spontaneous speech samples were transcribed using the CHAT format, and analysed using the CLAN programs Besides a descriptive analysis, a hierarchical clustering through a squared Euclidean distances was done, where patients groups were formed by similar clinical neighbors regardless the FTD variant. Results show that deep language analysis is useful when heterogeneity w/groups is present and depends (in all groups) on the disease stage. PPA patients had the worse performance. bvFTD executive passive structure difficulty is associated with executive functions: The inability to represent a temporal sequence of the phrase –not understanding if.. Then). Results demonstrate that spontaneous speech allow us to differentiate between all groups of patients. Discourse of these patients can be distinctive and it reflects not only the subject's linguistic abilities but other cognitive functions as well....

Caracterizar os pacientes com DFT e seus cuidadores. Descrever aspectos cognitivos e comportamentais, funcionalidade global, comunicação funcional, e a funcionalidade da deglutição na DFT. Descrever os problemas de deglutição e do comportamento alimentar na DFTvc e APP. Correlacionar os aspectos cognitivos e comportamentais, funcionalidade global e a comunicação com as características da deglutição. Identificar fatores preditivos da piora da funcionalidade da deglutição e do comportamento alimentar na DFT. Avaliar o comportamento dos instrumentos empregados. Desenvolver a versão reduzida do Questionário de Habilidades de Alimentação e Deglutição nas Demências e do Questionário de Comunicação Funcional na Afasia. Método: Este estudo incluiu 46 indivíduos com DFT nas fases leve, moderada e grave, e seus 46 cuidadores. O Mini exame do estado mental (MEEM) e o Mini exame do estado mental grave (MEEM-g) foram usados para avaliar os aspectos cognitivos. A Escala de estadiamento da demência (CDR-DLFT) foi usada para confirmar a fase da doença. O Inventário Neuropsiquiátrico (INP) foi aplicado para investigar os problemas comportamentais. A Bateria de Avaliação Frontal (BAF) investigou as funções executivas. O Índice das Atividades de Vida Diária (Katz), Questionário para Avaliação da Comunicação Funcional na Afasia (QACFA) e a Escala de funcionalidade da deglutição (EFD) avaliaram as habilidades funcionais. O Questionário de Habilidades de Alimentação e Deglutição nas Demências (QHADD) avaliou as dificuldades na deglutição e alimentação. Resultados: Os grupos DFTvc e APP não mostraram diferença estatisticamente significante no MEEM, CDR e BAF. Os cuidadores dos pacientes com DFTvc apresentaram mais horas de cuidado por dia em comparação aos pacientes com APP (p<0,05). Os grupos diferiram na EFD (p < 0,05). As características comportamentais que foram significantes na comparação entre os grupos DFTvc e APP foram: delírio, desinibição, comportamento motor aberrante e distúrbios do sono(p < 0,05) e alucinação (p=0,01). Os pacientes com DFTvc tiveram mais problemas de deglutição do que os pacientes com APP, tais como: tosse e engasgos, dificuldade com alguma consistência alimentar e dificuldade com alimento específico. Os problemas de deglutição na DFTvc se correlacionaram com a funcionalidade, aspectos cognitivos (p < 0,05), com a função executiva e com o comportamento (p < 0,01). Na APP, o subtipo semântico apresentou mais problemas de deglutição, tais como: escape de saliva e comida da boca, múltiplas deglutições, atraso na iniciação da deglutição e engasgos, estas características se correlacionaram com a ansiedade (p < 0,01), apatia e comportamento motor aberrante (p=0,01). Os problemas do comportamento alimentar foram mais frequentes no subtipo logopênico e se correlacionaram com dificuldades de comunicação. Os principais fatores preditivos da piora da funcionalidade da deglutição foram: declínio funcional, alterações comportamentais e o comprometimento da comunicação. Os problemas de deglutição foram observados em todas as fases da demência. A BAF foi o único instrumento que não apresentou uma boa confiabilidade interna. Method: This study included 46 individuals with FTD in mild, moderate and severe phases, and their 46 caregivers. The Mini mental state examination (MMSE) and the Severe Mini mental state examination (SMMSE) were used to assess the cognitive aspects. The FTLD-modified Clinical Dementia Rating scale (FTLD-CDR) was used to confirm the stage of the disease. The Neuropsychiatric Inventory (NPI) was applied to investigate the behavioral problems. The Frontal Assessment Battery (FAB) investigated executive functions. The Index of Activities of Daily Living (Katz), Functional Outcome Questionnaire- Aphasia and Swallowing rating scale (SRE) evaluated the functional abilities. The Assessment of Feeding and Swallowing Difficulties in Dementia (QHADD) evaluated the difficulties in swallowing and feeding. Results: bvFTD and PPA groups showed no statistically significant difference in MMSE, CDR and FAB. Caregivers of patients with bvFTD had more hours of care per day compared to patients with PPA (p < 0.05). The groups differed in SRE (p < 0.05). The behavioral characteristics that were significant in the comparison between bvFTD and PPA groups were delirium, disinhibition, aberrant motor behavior and sleep disturbances (p < 0.05), and hallucinations (p = 0.01). Patients with bvFTD had more swallowing problems than patients with PPA, such as coughing and choking, difficulty with some food consistency and difficulty with specific food. Swallowing problems in bvFTD correlated with functionality, with the cognitive aspects (p < 0.05), with executive function and behavior (p < 0.01). In PPA, the semantic subtype showed more swallowing problems such as escape of saliva and food in mouth, multiple swallows, delay in initiation of swallowing and choking, these characteristics correlated with anxiety (p < 0.01), apathy and aberrant motor behavior (p = 0.01). The problems of feeding behavior were more frequent in logopenic subtype and correlated with communication difficulties. The major predictors of worsening of swallowing function were: functional decline, behavioral changes and impaired communication. Swallowing problems were observed at all stages of dementia. The BAF was the only instrument that had bad internal reliability. Conclusion: Swallowing problems were observed in the two variants from the early stages of dementia. Behavioral, cognitive and functional changes, and difficulties in communication compromised the anticipatory and oral preparatory phase of swallowing. Because of these changes, caregivers had difficulty in managing the feeding situation. Our study developed reduced versions of questionnaires to assess swallowing and functional communication

The Mini mental state examination (MMSE) and the Severe Mini mental state examination (SMMSE) were used to assess the cognitive aspects. The FTLD-modified Clinical Dementia Rating scale (FTLD-CDR) was used to confirm the stage of the disease. The Neuropsychiatric Inventory (NPI) was applied to investigate the behavioral problems. The Frontal Assessment Battery (FAB) investigated executive functions. The Index of Activities of Daily Living (Katz), Functional Outcome Questionnaire- Aphasia and Swallowing rating scale (SRE) evaluated the functional abilities. The Assessment of Feeding and Swallowing Difficulties in Dementia (QHADD) evaluated the difficulties in swallowing and feeding. Results: bvFTD and PPA groups showed no statistically significant difference in MMSE, CDR and FAB. Caregivers of patients with bvFTD had more hours of care per day compared to patients with PPA (p < 0.05). The groups differed in SRE (p < 0.05). The behavioral characteristics that were significant in the comparison between bvFTD and PPA groups were delirium, disinhibition, aberrant motor behavior and sleep disturbances (p < 0.05), and hallucinations (p = 0.01). Patients with bvFTD had more swallowing problems than patients with PPA, such as coughing and choking, difficulty with some food consistency and difficulty with specific food. Swallowing problems in bvFTD correlated with functionality, with the cognitive aspects (p < 0.05), with executive function and behavior (p < 0.01). In PPA, the semantic subtype showed more swallowing problems such as escape of saliva and food in mouth, multiple swallows, delay in initiation of swallowing and choking, these characteristics correlated with anxiety (p < 0.01), apathy and aberrant motor behavior (p = 0.01). The problems of feeding behavior were more frequent in logopenic subtype and correlated with communication difficulties. The major predictors of worsening of swallowing function were: functional decline, behavioral changes and impaired communication. Swallowing problems were observed at all stages of dementia. The BAF was the only instrument that had bad internal reliability. Conclusion: Swallowing problems were observed in the two variants from the early stages of dementia. Behavioral, cognitive and functional changes, and difficulties in communication compromised the anticipatory and oral preparatory phase of swallowing. Because of these changes, caregivers had difficulty in managing the feeding situation. Our study developed reduced versions of questionnaires to assess swallowing and functional communication
