INTRODUÇÃO: Há pouco tempo atrás não havia ferramenta específica para avaliar os sintomas do trato urinário inferior de pacientes com doenças neurológicas. Diversos instrumentos foram desenvolvidos e recomendados para avaliação de sintomas urinários em pacientes com disfunção do trato urinário inferior, porém, por não captarem a especificidade da bexiga neurogênica, apresentam perguntas inaplicáveis ou insuficientes para avaliar os sintomas desta população. Em 2014 foi introduzido o Neurogenic Bladder Symptom Score (NBSS) específico para pacientes com diagnóstico de bexiga neurogênica. Contém questões sobre incontinência urinária, sintomas e métodos de esvaziamento vesical, complicações associadas à disfunção miccional e qualidade de vida. OBJETIVO: este estudo se propôs a realizar a adaptação cultural e validação do questionário de sintomas NBSS para o português do Brasil. MÉTODOS: O questionário de sintomas foi adaptado culturalmente conforme diretrizes internacionais, seguindo os passos de: tradução, harmonização, retrotradução, revisão do comitê de especialistas, e teste da versão pré-final. A versão adaptada para o Brasil foi aplicada em pacientes com diagnóstico de bexiga neurogênica por lesão medular ou esclerose múltipla, que compareceram para consultas médicas com fisiatra ou urologista. Os mesmos pacientes responderam novamente o questionário num intervalo de 7 a 14 dias (reteste). A confiabilidade foi avaliada pela consistência interna, através do alfa de Cronbach, e pela estabilidade (teste-reteste), avaliada pelo Coeficiente de Correlação Intraclasse. Pacientes que não retornaram para a reaplicação do questionário, ou que relataram mudança nos sintomas do trato urinário inferior durante este período, foram excluídos da análise de estabilidade. A validade de critério foi avaliada pela aplicação da versão brasileira do questionário de qualidade de vida Qualiveen-SF e o resultado foi mensurado através do Coeficiente de Correlação Intraclasse. A validade de conteúdo foi determinada pela avaliação e julgamento do comitê de especialistas. A validade de construto foi obtida pela comparação entre o NBSS e a avaliação da função global da bexiga, através da análise de variância. RESULTADOS: Um total de 68 pacientes (57 homens - 83,8%) com idade média de 38,9 (± 14,7) anos participaram do estudo. Observou-se boa consistência interna da versão em português, com alfa de Cronbach de 0,813. A estabilidade (teste/reteste) também foi alta, com um Coeficiente de Correlação Intraclasse de 0,8596 [0,7586-0,9202] (p < 0,0001). Na validade de critério, o Coeficiente de Correlação Intraclasse revelou moderada correlação entre a versão em português do NBSS e o questionário Qualiveen-SF (r=0,6552 [0,423-0.8151]; p < 0,0001). CONCLUSÕES: Os resultados do estudo demonstraram que a versão brasileira do NBSS - Neurogenic Bladder Symptom Score, apresenta boa consistência interna, estabilidade e capacidade de medir o construto correto. O processo de adaptação cultural e validação do questionário NBSS para língua portuguesa, em pacientes com disfunção neurogênica do trato urinário inferior, foi concluído. O instrumento encontra-se apto para uso clínico e em pesquisa / INTRODUCTION: Until recent times, there was no specific tool to evaluate the symptoms of the lower urinary tract of patients with neurological diseases. Several instruments have been developed and recommended for evaluation of urinary symptoms in patients with lower urinary tract dysfunction, but because they do not capture the specificity of the neurogenic bladder, they present questions that are inapplicable or insufficient to evaluate the symptoms of this population. In 2014 it was introduced the Neurogenic Bladder Symptom Score (NBSS), specific for patients with a neurogenic bladder diagnosis. It contains questions about urinary incontinence, symptoms and methods of bladder emptying, complications associated with voiding dysfunction and quality of life. OBJECTIVE: This study aimed to perform the cultural adaptation and validation of the NBSS questionnaire for the Brazilian Portuguese. METHODS: The symptom questionnaire was culturally adapted according to international guidelines, following the steps of: translation, synthesis, back-translation, expert committee review, and pre-final version test. The version adapted for Brazil was applied in patients with neurogenic bladder diagnosis due to spinal cord injury or multiple sclerosis, who attended for medical appointments with physiatrist or urologist. The same patients answered the questionnaire again within 7 to 14 days (retest). Reliability was assessed by internal consistency, using Cronbach\'s alpha, and stability (test-retest), as assessed by Intraclass Correlation Coefficient. Patients who did not return for questionnaire reapplication or who reported changes in lower urinary tract symptoms during this period were excluded from the stability analysis. The criterion validity was evaluated by applying the Brazilian version of the Qualiveen-SF quality of life questionnaire and the result was measured using the Intraclass Correlation Coefficient. The validity of content was determined by the evaluation and judgment of the committee of experts. The construct validity was obtained by the comparison between the NBSS and the evaluation of the global function of the bladder, through analysis of variance. RESULTS: A total of 68 patients (57 men - 83.8%) with an average age of 38.9 (± 14.7) years participated in the study. There was good internal consistency of the Portuguese version, with Cronbach\'s alpha of 0.813. Stability (test / retest) was also high, with a Intraclass Correlation Coefficient of 0.8596 [0.7586-0.9202] (p < 0.0001). For criterion validity, the Intraclass Correlation Coefficient revealed a moderate correlation between the Portuguese version of the NBSS and the Qualiveen-SF questionnaire (r = 0.6552 [0.423-0.8151]; p < 0.0001). CONCLUSIONS: The results of the study demonstrated that the Brazilian version of the NBSS - Neurogenic Bladder Symptom Score shows good internal consistency, stability and ability to measure the correct construct. The process of cultural adaptation and validation of the NBSS questionnaire for Portuguese language in patients with neurogenic lower urinary tract dysfunction was concluded. These conditions include a list of health issues for which hospitalization can be prevented with an effective intervention of primary care through the timely control of acute illnesses and management of chronic diseases and their complications. Objective: Investigate if characteristics of the structure of the Basic Health Units (UBS) and the work process of Primary Care Teams (EAB) are associated with the number of hospitalizations for Sensitive Conditions Primary (ICSAP). Methods: This is an ecological study. Data were derived from sociodemographic characteristics, coverage of assistance programs, structure of the Basic Health Units (UBS) and wor teams process of Brazilian municipalities. Data were collected from secondary data from the Brazilian Institute of Geography and Statistics, the United Nations Development Programme, the Department of the Unified Health System and the first cycle of Access Improvement Programme and Quality of Primary Care (PMAQ-AB). We considered the number of ICSAP in the years 2013 and 2014 were estimated associations through negative binomial regression coefficients and confidence intervals at 95%, with hierarchical approach in three blocks (alpha = 5%). Results: After adjustment for 2013 in the distal block, the coverage of the Bolsa Família Program (β = -0.001) and private plan (β = -0.01) showed a negative association and the human development index (β = 1, 13), the proportion of elderly (β = 0.05) and less than five years (β = 0.05) and the cover of the Community Agents Health Strategy (β = 0.002) showed a positive association. The intermediate block, showed negative association the minimum time (β = -0.14) and the availability of vaccine (β = -0.16) and positive association, the availability of drugs (β = 0.16). The proximal block, only the variable matrix support (β = 0.10) showed a positive association. In analysis adjusted 2014, the variables had the same sense of association 2013. Conclusions: The increased coverage of income distribution programs, UBS largest percentage of working in minimum time and the greater availability of vaccines are significantly associated with reduce ICSAP. / Introdução: As atividades de competência da Atenção Básica, como a prevenção, diagnóstico e tratamento de agravos e o controle e acompanhamento de doenças crônicas, podem determinar a redução das Internações Hospitalares por Condições Sensíveis à Atenção Primária (ICSAP). Essas condições englobam uma lista de problemas de saúde cuja internação pode ser prevenida com uma intervenção eficaz do primeiro nível de atenção, por meio do controle oportuno de doenças agudas e da gestão de doenças crônicas e suas complicações. Objetivo. Investigar se características da estrutura das Unidades Básicas de Saúde (UBS) e do processo de trabalho das Equipes de Atenção Básica (EAB) estão associadas ao número de Internações por Condições Sensíveis à Atenção Primária (ICSAP). Métodos. Trata-se de um estudo ecológico. Foram analisados dados de municípios brasileiros relativos a características sociodemográficas, de cobertura de programas assistenciais, de estrutura de UBS e de processo de trabalho das EAB. Os dados foram coletados de banco de dados secundários do Instituto Brasileiro de Geografia e Estatística, do Programa das Nações Unidas para o Desenvolvimento, do Departamento de Informática do Sistema Único de Saúde e do primeiro ciclo do Programa de Melhoria do Acesso e Qualidade da Atenção Básica (PMAQ-AB). Foi considerado o número de ICSAP nos anos de 2013 e 2014. Estimaram-se as associações por meio de coeficientes de regressão binomial negativa e respectivos intervalos de confiança a 95%, com abordagem hierarquizada em três blocos (alpha=5%). Resultados: Na análise ajustada de 2013, no bloco distal, a cobertura do Programa Bolsa Família (β= -0,001) e de plano privado (β= -0,01) apresentaram associação negativa e o índice de desenvolvimento humano (β= 1,13), a proporção de pessoa idosa (β= 0,05) e de menor de cinco anos (β= 0,05) e a cobertura da Estratégia de Agentes Comunitários de Saúde (β= 0,002) mostraram uma associação positiva. No bloco intermediário, apresentaram associação negativa o horário mínimo (β= -0,14) e a disponibilidade de vacina (β= -0,16) e associação positiva, a disponibilidade de medicamentos (β= 0,16). No bloco proximal, apenas a variável apoio matricial (β= 0,10) mostrou uma associação positiva. Na análise ajustada de 2014, as variáveis apresentaram o mesmo sentido de associação de 2013. Conclusões: A maior cobertura de programas de distribuição de renda, maior percentual de UBS funcionando em horário mínimo e a maior disponibilidade de vacinas estão significativamente associados a menor número de ICSAP nos municípios.

Análise da qualidade de uma base de dados a a partir da implementação do Registro Paulista de Tratamento Cirúrgico de Câncer de Pulmão / Analysis a database quality through the implementation of the Paulista Lung Cancer Surgical Treatment Registry

INTRODUÇÃO: O câncer de pulmão é a terceira neoplasia maligna mais frequentemente diagnosticada em todo o mundo e a primeira em termos de mortalidade. O tratamento cirúrgico é a melhor abordagem nos estágios iniciais, contudo, está associado a morbimortalidade considerável. Para que o impacto do tratamento cirúrgico na diminuição global da mortalidade pelo câncer de pulmão no estado de São Paulo seja maior, precisamos conhecer os indicadores de qualidade das instituições envolvidas no tratamento desta neoplasia, através da criação de uma base de dados abrangente, confiável e transparente. Este estudo envolveu a implementação do Registro Paulista de Tratamento Cirúrgico do Câncer de Pulmão (RPCP). O desfecho principal foi a análise da qualidade dos dados capturados através de um sistema de auditoria direta e indireta, com o intuito de identificar as variáveis com menor padrão de qualidade. MÉTODOS: Estudo prospectivo, multicêntrico, com participação de 10 instituições no estado de São Paulo. A auditoria dos dados foi realizada de forma direta por revisão dos prontuários, para análise da taxa de discordância, Coeficiente Kappa e Intraclass correlation e de forma indireta para análise dos índices de completude, acurácia e consistência. RESULTADOS: Dos 536 casos disponíveis, 511 foram incluídos para a auditoria indireta. O índice total de completude por questionário variou de 0,82 a 1, sendo que as seguintes variáveis obtiveram valor individual abaixo da meta estabelecida de 0,8: ECOG, MRC, hematócrito, potássio, uréia, creatinina, DHL, albumina, cálcio e FA, tempo de cirurgia e data da recidiva. O índice total de acurácia e consistência foi 0,99 e 0,96, respectivamente. Para auditoria direta foram randomizados 100 casos entre os 511 iniciais, sendo 4 excluídos, restando 96 para análise. As variáveis com maiores taxas de discordância ( > 20%), estavam no questionário de avaliação pré-operatória (ECOG, MRC, carga tabágica, DPOC, PFP, peso, altura, IMC e exames laboratoriais). Variáveis relacionadas ao estadiamento (tamanho da neoplasia, invasão de estruturas adjacentes, status linfonodal não invasivo) e dados cirúrgicos (tempo de cirurgia) também apresentaram taxas > 20%. CONCLUSÕES: A auditoria indireta dos dados mostrou índices de completude, acurácia e consistência aceitáveis para o padrão estabelecido e comparáveis a bancos de dados internacionais. Por outro lado, a auditoria direta, revelou algumas variáveis com altos índices de discordância, dados que serão analisados futuramente para aprimoramento do RPCP e que poderão contribuir para o desenvolvimento de outras bases de dados semelhantes / BACKGROUND: Lung cancer is the third malignant neoplasm most frequently diagnosed worldwide and the first in terms of mortality. Surgical treatment is the best approach in the initial stages; however, it\'s associated with considerable morbidity and mortality. In order to improve the surgical treatment global impact on lung cancer mortality in the state of Sao Paulo, we need to know the quality indicators of the institutions involved in the treatment of this neoplasm through the creation of a extensive, reliable and transparent database. The study involved the implementation of the Paulista Lung Cancer Registry (PLCR). The main outcome was the quality analysis of the data captured through a direct and indirect audit system, in order to identify the variables with the lowest quality standard. METHODS: A prospective, multicenter study with the participation of 10 institutions in the state of São Paulo. The data audit was performed directly, through the revision of medical registries, with the intention to analyze the discordance rate; and indirectly, with the intention to analyze the completeness, accuracy and consistency indexes. RESULTS: Of the 536 cases available, 511 were included for the indirect audit. The total completeness index per questionnaire ranged from 0.82 to 1, and the following variables had a in individual value bellow the established target of 0,8: ECOG, MRC, hematocrit, potassium, urea, creatinine, LDH, albumin, calcium, AF, surgical time, date of recurrence. The total accuracy and consistency index was 0.99 and 0.96, respectively. For direct audit, 100 cases were randomized among the initial 511, of which 4 were excluded, remaining 96 for analysis. The variables with the highest discordance rates ( > 20%) were in the preoperative evaluation questionnaire (ECOG, MRC, smoking rate, COPD, PFT, weight, high, BMI and lab tests). Variables related to staging (size of neoplasm, invasion of adjacent structures, noninvasive lymph node status) and surgical data (time of surgery) also presented rates > 20%. CONCLUSIONS: Regarding the established standards, the Indirect audit showed acceptable completeness, accuracy and consistency indices, comparable to international databases. On the other hand, the direct audit revealed some variables with high discordance indices, data that will be analyzed in the future for the improvement of the PLCR and that may contribute to the development of other similar databases

Soins primaires et performance : de la variabilité des pratiques des médecins généralistes au rôle de l'organisation des soins / Primary care and performance : from medical practice variation to the role of the practice organization

La recherche économique considère le médecin généraliste comme un agent offrant à l’échange information et services intellectuels en santé à des principaux. L’imparfaite convergence entre leurs objectifs, comme la présence d’incertitudes et d’asymétries d’information, conduisent le médecin à « fixer » les quantités, l’effort fourni et la qualité des soins et services rendus. Cette thèse vise à identifier le rôle des caractéristiques des généralistes, de leur mode d’exercice ou d’organisation, sur la performance de leur activité, en s’appuyant sur trois articles. Le premier analyse les déterminants de la variabilité de prescription d’antibiotiques pour rhinopharyngite aiguë, le second évalue l’impact du travail en équipe avec des infirmières sur la qualité et l’efficience des soins pour les patients diabétiques, et le troisième évalue l’impact de l’exercice regroupé pluriprofessionnel sur l’activité et l’efficience productive des généralistes et l’efficience des recours aux soins ambulatoires de leurs patients. Ces travaux de recherche permettent d’interroger la faiblesse relative de la régulation de l’offre de soins ambulatoire en France en matière de politique de maîtrise de l’évolution des dépenses de santé en comparaison de celle portant sur la demande. / According to economic research, the general practitioner is considered as an agent that offered information and intellectual services in health to principals. Imperfect convergence between their objectives, like the presence of uncertainties and information asymmetries, lead the physician “to fix” the quantities, the effort and the quality of the care and services delivered. Based on three articles, this thesis aims at identifying the role of the characteristics of the general practitioners and of their practice organization, on activity performance. The first articles analyzes the determinants of the antibiotic prescription for acute rhinopharyngitis variability, the second evaluates the impact of team working with nurses on the quality and the efficiency of the care for diabetic patients, and the third evaluates the impact of multi-professional group practices on the activity and the productive efficiency of GPs and on the utilization of ambulatory health care and by their patients. These researches question the relative weakness of the ambulatory health care regulation in France in terms of health care expenditure containment policies in comparison with that bearing on demand.

Training and action for patient safety: embedding interprofessional education for patient safety within an improvement methodology

No / Despite an explosion of interest in improving safety and reducing error in health care, one important aspect of patient safety that has received little attention is a systematic approach to education and training for the whole health care workforce. This article describes an evaluation of an innovative multiprofessional, team-based training program that embeds patient safety within quality improvement methods. METHODS: Kirkpatrick's "levels of evaluation" model was adopted to evaluate the program in health organizations across one city in the north of England. Questionnaires were used to assess reaction of participants to the program (Level 1). Improvements in patient safety knowledge and patient safety culture (Level 2) were assessed using a 12-item multiple-choice questionnaire and a culture questionnaire. Interviews and project-specific quantitative measurements were used to assess changes in professional practice and patient outcomes (Levels 3 and 4). RESULTS: All aspects of the program were positively received by participants. Few participants completed the MCQ at both time points, but those who did showed improvement in knowledge. There were some small but significant improvements in patient safety culture. Interviews revealed a number of additional benefits beyond the specific problems addressed. Most importantly, 8 of the 11 teams showed improvements in patient safety practices and/or outcomes. DISCUSSION: This program is an example of interprofessional education in practice and demonstrates that team-based learning using quality improvement methods is feasible and can be effective in improving patient safety, but requires time and space for participants.
