CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior / Estudamos o efeito do citrato de sildenafil (ViagraÂ), vasodilatador largamente utilizado na terapÃutica da disfunÃÃo erÃtil, sobre o comportamento motor do trato gastrintestinal (TGI) de ratos Wistar. Para tanto, utilizamos 175 animais machos, pesando entre 200 a 350g, distribuÃdos nos quatro seguintes grupos de estudo: efeitos do citrato de sildenafil sobre o i) esvaziamento gÃstrico (EG) e os trÃnsitos gastrintestinal (GI) e ii) intestinal de lÃquido em ratos acordados; iii) a complacÃncia gÃstrica de ratos anestesiados e iv) a contratilidade de tiras isoladas do duodeno de ratos ex vivo. i) Avaliamos, em 64 ratos acordados sob jejum e livre acesso à Ãgua por 24h, o efeito da injeÃÃo (0,2mL; e.v.) de sildenafil (4mg/Kg) ou veÃculo (HCl 0,01N) sobre o EG e o trÃnsito GI de lÃquido, bem como sobre a pressÃo arterial (PA). Mediante gavagem, 1,5mL da refeiÃÃo-teste (vermelho de fenol - 0,5mg/mL em glicose a 5%) foi injetada no estÃmago. Depois de 10, 20 ou 30min, sacrificamos os animais e, apÃs laparotomia, obstruÃmos o piloro, o cÃrdia e o Ãleo terminal. Removemos e dividimos o TGI em: estÃmago e segmentos consecutivos do intestino delgado (40% iniciais; 30% mediais e 30% terminais). ApÃs o processamento destas porÃÃes viscerais, determinamos as absorbÃncias das amostras a 560nm. A retenÃÃo fracional de vermelho fenol em cada segmento permitiu o cÃlculo do EG e trÃnsito GI. Em um grupo separado de animais, a PA foi monitorada continuamente por meio de um sistema digital de aquisiÃÃo de dados durante 20min antes e 30min apÃs o tratamento com sildenafil ou diluente. Comparado ao grupo controle, houve aumento significativo da retenÃÃo gÃstrica (44,2Â2,0 vs 53,2Â2,1; 25,4Â1,3 vs 37,3Â1,6; 20,9Â2,5 vs 32,5Â2,9%) nos animais tratados com sildenafil e sacrificados aos 10, 20, ou 30min, respectivamente, bem como retarde significativo no trÃnsito GI. Embora o sildenafil tenha provocado hipotensÃo, a PA retoma nÃveis basais logo apÃs 10min. O prÃ-tratamento com omeprazol (bloqueador da secreÃÃo Ãcida estomacal) nÃo modificou o efeito do sildenafil sobre os valores de retenÃÃo gÃstrica e intestinal nem nos nÃveis de PA. ii) Noutros animais (n=44), sob jejum de 24h e dotados previamente (3d) de uma cÃnula crÃnica no bulbo duodenal, estudamos o efeito do sildenafil sobre a progressÃo ao longo do intestino delgado de uma refeiÃÃo teste (10MBq de TecnÃcio ligado a fitato e diluÃdo em 1mL de salina 0,9%). Decorridos 20, 30 ou 40min da injeÃÃo (0,2mL e.v.) de sildenafil (4mg/Kg) ou diluente (HCL 0,01N), sacrificamos os animais e, apÃs laparotomia e remoÃÃo do TGI, dividimo-o em: estÃmago, cinco segmentos congruentes e consecutivos de intestino delgado e o intestino grosso. A contagem da radiatividade foi determinada num colimador de gama-cÃmara. O sildenafil promoveu retarde (p<0,05) do TI, indicado pelos retardes dos centros geomÃtricos da refeiÃÃo de 2,8&#61617; 0,2 vs 3,3&#61617; 0,1; 3,0&#61617; 0,2 vs 3,7&#61617; 0,1 e 3,4&#61617; 0,1 vs 4,2&#61617; 0,2 em relaÃÃo ao grupo controle, aos 20, 30 ou 40min. iii) Os estudos de complacÃncia gÃstrica foram conduzidos em 39 ratos anestesiados, sob jejum de 24h. As variaÃÃes do volume gÃstrico (VG), foram medidas por pletismografia, enquanto a PA foi monitorada continuamente por um sistema digital de aquisiÃÃo de dados. Em relaÃÃo aos valores basais (2,91Â0,19mL) o sildenafil (3mg/Kg â e.v.) aumentou (p<0,05) o VG apÃs 10, 20 e 30min (3,08Â0,18; 3,10Â0,17 e 3,09Â0,17mL). A PA basal (105,8Â2,28mmHg) caiu significativamente com o sildenafil (59,8Â3,2; 64,8Â3,7 e 59,3Â4,6mmHg) enquanto o diluente (HCl 0,01N) nÃo modificou seja o VG ou a PA. O prÃ-tratamento mediante esplancnotomia ou injeÃÃo e.v. com azul de metileno (3mg/Kg-bloqueador da guanilato ciclase), L-NNA (3mg/Kg-bloqueador da NO sintetase) ou propranolol (2mg/Kg-Ã-bloqueador) preveniram o aumento do VG pelo sildenafil; jà o pÃs-tratamento com nitroprussiato de sÃdio (1mg/Kg - e.v.) o ampliou significativamente. iv) Avaliamos ainda o efeito do sildenafil sobre a contratilidade de tiras isoladas do duodeno de ratos ex vivo (n=28), sacrificados por deslocamento cervical. Tiras dissecadas do duodeno foram suspensas longitudinalmente em cuba de vidro (10mL), plena de soluÃÃo de Tyrode (37oC e pH 7,4), e submetidas a uma tensÃo inicial de 1g. ApÃs 1h de estabilizaÃÃo, a contratilidade espontÃnea ou induzida das tiras foi registrada continuamente por um sistema digital de aquisiÃÃo de dados. O sildenafil em doses crescentes e cumulativas (0,1 a 300Âmol/L) relaxou (EC50 de 9,6Âmol/L) o duodeno, mais atà que o zaprinaste ou a papaverina (bloqueadores de FDEs) (EC50 91,6 e 78,5Âmol/L, nesta ordem). Observamos ademais que o sildenafil inibiu as contraÃÃes induzidas por acetilcolina ou carbacol (IC50 26,7 e 16,2Âmol/L, respectivamente). Jà o prÃ-tratamento com azul de metileno, ODQ (bloqueador da guanilato ciclase) ou L-NAME (bloquedor da NO sintetase), mas nÃo o D-NAME (isÃmero inativo da NO sintetase) preveniram o efeito do sildenafil. O efeito mio-relaxante do sildenafil foi ampliado pela L-arginina (substrato do NO sintetase) ou nitroprussiato de sÃdio (doador de NO). O prÃ-tratamento com forskolina (estimulador da adenilato ciclase) tambÃm aumentou o efeito mio-relaxante do sildenafil. Em resumo, observamos que o sildenafil diminui a motilidade gastrintestinal, retardando o EG, os trÃnsitos GI e intestinal de lÃquido em ratos acordados; aumenta a complacÃncia gÃstrica em ratos anestesiados alÃm de apresentar efeitos antiespasmÃdico e mio-relaxante sobre tiras isoladas de duodeno de ratos ex vivo; por estimulaÃÃo do sistema nervoso simpÃtico e tendo como provÃvel mecanismo de aÃÃo ao nÃvel do miÃcito gastrintestinal a via do NO/GMP cÃclico. / We evaluated the effect of sildenafil citrate (ViagraÂ) a vasodilator largely used for the treatment of male erectile dysfunction, on the gastrointestinal motility in rats. Experiments were performed on 175 male, Wistar rats, weighing 200-350g. Four groups of study were done: the sildenafil effects on the: i) Gastric emptying (GE) and gastrointestinal (GI) transit and ii) Intestinal transit (IT) of liquid in awake rats; iii) Gastric compliance in anesthetized rats and iv) Contractility of rat duodenal isolated strips. i) In 64 rats fasted for 24h with previous vascular access (right jugular vein and left carotid artery), we studied the effect of an i.v. injection (0.2mL) of sildenafil (4mg/Kg) or vehicle (0.01N HCl) on GE and GI transit of a liquid meal, as well as on arterial pressure (AP) in a separated group of rats. Animals were gavage-fed with 1.5mL of a test meal (0.5mg/mL of phenol red in 5% glucose). After 10, 20 or 30min, animals were sacrificed and submitted to a laparotomy to obstruct the pylorus, cardia and terminal ileus. The gut was removed and then divided into: stomach and consecutive three small intestine segments (40% proximal; 30% medial and 30% terminal). After processing these segments, the dye retention was determined at 560nm. The percentage of dye retention in each segment permitted to evaluate GE and GI transit. Arterial pressure was continuously monitored by a digital acquisition system during 20min before and 30min after sildenafil injection. We observed a significant increase of gastric retention in sildenafil treated rats at 10, 20, or 30min after the test meal (44,2Â2,0 vs 53,2Â2,1; 25,4Â1,3 vs 37,3Â1,6; 20,9Â2,5 vs 32,5Â2,9%, respectively), as well as a significant GI transit delay. Despite of sildenafil inducing hypotension, AP returned to basal levels 10min afterwards. Acid gastic secretion blocking pre-treatment with omeprazol did not modify the sildenafil effect on gastric retention, GI transit or AP. ii) In another group we evaluated the sildenafil (4mg/Kg) or diluente (0.01N HCl, 0.2mL) effects on the IT in awake rats, fasted for 24h. Animals were studied 3d after the insertion of a silastic cannula (0.6cm ID) into the duodenal bulb. We evaluated the progression of a radioactive liquid test meal fed (10MBq of 99mTc â 1mL of saline 0,9%) administered through the inserted cannula into the small intestine. After 20, 30 or 40min, animals were sacrificed by anesthetic overdose. After laparotomy, we removed and divided the gut in: stomach, five congruent and consecutive segments of the small intestine and the large intestine. Radioactivity counting was obtained in a gamma-chamber collimator. Sildenafil promoted an IT delay (p<0.05), indicated by shifting the center of mass to the proximal portions of the TGI (2.8Â0.2 vs 3.3Â0.1; 3.0Â0.2 vs 3.7Â0.1 and 3.4Â0.1 vs 4.2Â0.2) in relation to control group. iii) Gastric compliance study was performed on 39 anesthetized rats after 24h of fasting. Gastric volume (GV) variations were measured by plethysmography while AP was continuously monitored. We have also observed that GV increased (p<0.05) after sildenafil treatment (3mg/Kg - e.v) (3.08Â0.18; 3.10Â0.17 and 3.09Â0.17mL vs 2.91Â0.19mL) at 10, 20 and 30min after drug administation, respectively. Basal AP (105.8Â2.28mmHg) dropped by the sildenafil injection (59.8Â3.2; 64.8Â3.7 and 59.3Â4.6mmHg-p<0.05) while vehicule (0.01N HCl) did not change either GV or AP. After splanchnotomy or pre-treatments (e.v.) with methylene blue (3mg/Kg-guanilate cyclase blocker), L-NNA (3mg/Kg - NO synthase blocker) or propranolol (2mg/Kg - Ã-blocker) prevented GV increase due to sildenafil; while post-treatment with sodium nitroprusside (1mg/Kg - NO donor) raised it. iv) The in vitro contractility studies were performed on isolated duodenal strips obtained from rats (n=28) killed by cervical dislocation. Duodenal strips were suspended longitudinally in a glass chamber (10mL), filled with Tyrode solution (37oC and pH 7.4). After 1h of stabilization under 1g of initial tension, the spontaneous or induced contractility were continuously recorded by a digital acquisition system. Increasing and cummulative doses of sildenafil (0.1 to 300Âmol/L) relaxed (9.6Âmol/L of EC50) the duodenal strips. This effect was more intense than those displayed by zaprinast or papaverine (PDEs blockers) (91.6 and 78.5Âmol/L of EC50, in this order). Sildenafil showed significant antispasmodic and myorelaxant effects on the duodenal contractions induced by acetylcholine or carbamylcholine (IC50 26.7 and 16.2Âmol/L, respectively). Pre-treatment with methylene blue, ODQ (guanilato cyclase blocker) or L-NAME (NO synthase blocker) also prevented these sildenafil effects, but D-NAME (an inactive substrate for NO synthase) did not. Myorelaxant sildenafil effect was reverted by L-arginine (substrate for NO synthase) and contrarily it was largely increased by sodium nitroprusside. Forskolin adenylate cyclase activation pre-treatment also increased the myorelaxant effect of sildenafil. In summary, we have observed that sildenafil slowed down the gastrointestinal motility, delaying GE, GI and intestinal transits of a liquid meal in awake rats; Gastric compliance was also increased in anesthetized rats treated with sildenafil. Sildenafil also exhibited both antispasmodic and myorelaxant effects on isolated strips of duodenum of ex vivo rats. Besides central or peripheral sympathetic nervous system activation, sildenafil possibly acts at the gastrointestinal myocite level by activating the NO/GMPc system.

Introdução: A hipertensão arterial sistêmica (HA) pode estar associada à diminuição na produção e liberação do óxido nítrico derivado do endotélio (NO). O uso do sildenafil leva ao aumento de monofosfato de guanosina cíclica (GMPc), um importante mediador de NO. Contudo, pouco se sabe sobre os efeitos da inibição da fosfodiesterase tipo 5 (PDE5) na monitorização da pressão arterial 24-h (MAPA), pressão arterial durante exercício, noraepinefrina (Nor) e capacidade ao exercício, principalmente após transplante de coração (TX). Métodos: Nós estudamos 22 pacientes pós TX, os quais foram randomizados, tomando dose única de sildenafil (50mg) ou placebo (50mg), aproximadamente uma hora antes de iniciar o protocolo. No dia 1, os pacientes realizaram avaliação clínica, teste cardiopulmonar de caminhada de seis minutos (TES) seguido de teste de esforço cardiopulmonar (TE), Após o término dos testes em esteira, foi colocado o MAPA. Determinamos em repouso (rep), último minuto do TES (6) e pico do TE (Ex): FC (bpm) PAS e PAD (mmHg), VO2(ml/kg/min), Slope VE/VCO2, tempo de exercício (TE, min), distância (TES, Km) e Nor (pg/ml). No dia 2 o protocolo foi repetido, realizando-se o cross-over. Dezessete pacientes apresentavam HA. Resultados: (Pl e Sil respectivamente), Sil reduziu (p<0.05): PAS-rep(138±7 vs 122±18); PAD-rep(83±12 vs 78±12); PAS-6(156± 20 vs 137± 22); PAD-6(82±13 vs 77±14); PAS-Ex(155± 27vs 124±36); PAD-Ex(79±16 vs 66± 16); PAS 24-h(121±10 vs 114±9), PAD 24-h(80±6 vs 76±5), PAS vigília(122±11 vs 115±9), PAD vigília(81± 6 vs 76±5) e PAS noturna(119±12 vs 112±10), PAD noturna(78±7 vs 73±8); e aumentou Nor-repouso(483±165 vs 622±211). Sil não alterou rep, 6 e EX: FC, VO2 e Slope. Conclusão: O ciclo NO-cGMP parece desempenhar papel importante no controle da pressão arterial em TX. Sendo que, a inibição da PDE5 parece apresentar efeitos benéficos no controle da hipertensão arterial em TX, podendo ser utilizada concomitantemente a terapia anti-hipertensiva usual. / Background: Systemic hypertension (SH) can be associated with a decrease in endothelium-dependent nitric oxide (NO). Sildenafil determines increment in cyclic guanosine monophosphate (cGMP) that a mediator of NO. However, little is known about the effects of PDE5 inhibition on 24-hour ambulatory (ABP) and exercise blood pressure, noreprinephrine (Nor) and exercise capacity, specially after heart transplantation (HT). Methods: We studied 22 HT pts that on the 1st day underwent a cardiopulmonary (CP) self-controlled treadmill 6walk test(6) and, after, an ECG monitored CP treadmill maximal exercise test(Ex) within 60 and 90 min after oral Sildenafil (Sil,50mg) or placebo(Pl) given at random, and ABP. We determined at basal position(b), last min of 6 and the peak Ex the HR(bpm), SBP and DBP (mmHg), VO2(ml/kg/min), Slope VE/VCO2, exercise time(ET, min), distance(D, Km) and Nor(pg/ml). Also, after CP tests 24-h SBP and DBP were monitored. It was repeated on the 2nd day when the cross-over was done. Seventeen pts had SH. Results: (Pl and Sil respectively), Sil reduced (p<0.05): b- SBP(138±7 vs 122±18); b-DBP(83±12 vs 78±12); 6-SBP(156± 20 vs 137± 22); 6-DBP(82±13 vs 77±14); Ex-SBP(155± 27vs 124±36); Ex-DBP(79±16 vs 66± 16); 24-h SBP(121±10 vs 114±9) and DBP(80±6 vs 76±5), daytime SBP(122±11 vs 115±9) and DBP(81± 6 vs 76±5) and nighttime SBP(119±12 vs 112±10) and DBP(78±7 vs 73±8); and increase b-Nor(483±165 vs 622±211). Sil did not change in b, 6 and EX; HR, Nor, VO2 and Slope. Conclusion: NO-cGMP pathway seems to play a role in blood pressure control in HT. The PDE5 inhibition could have potential beneficial effects on hypertensive HT in addition to antihypertensive therapy.

Tratamento específico em hipertensão arterial pulmonar avançada associada a cardiopatias congênitas: aspectos clínicos, bioquímicos e prognósticos / Clinical, biochemical and prognostic impact of specific therapies in advanced pulmonary arterial hypertension associated with congenital heart disease

INTRODUÇÃO: A hipertensão arterial pulmonar associada a cardiopatias congênitas (HAP-CCg) é um tipo específico de doença vascular pulmonar com várias complicações. Os pacientes com HAP-CCg avançada são considerados inoperáveis e podem apresentar hipoxemia crônica e eritrocitose secundária devido ao desvio de sangue da direita para a esquerda através das comunicações cardíacas. O presente estudo teve como objetivo analisar a resposta a dois inibidores de fosfodiesterase 5 (iPDE 5), a saber, sildenafila e tadalafila, administrados por seis meses. Os fármacos haviam sido previamente aprovados para uso na HAP em geral. No entanto, a literatura é escassa no que diz respeito à sua utilização especificamente na HAP-CCg. Propusemo-nos a investigar potenciais efeitos destes fármacos além de suas ações como vasodilatadores. MÉTODOS: Foram incluídos 31 pacientes com HAP-CCg avançada (idade entre 10 e 54 anos). A maioria apresentava redução na saturação sistêmica de oxigênio, com hematócrito elevado. A sildenafila e a tadalafila foram administradas por via oral (respectivamente, 20 mg três vezes ao dia e dose única diária de 40 mg). A capacidade física foi avaliada pela distância percorrida em 6 minutos (teste de caminhada de 6 minutos, protocolo American Thoracic Society). A qualidade de vida relacionada à saúde foi avaliada utilizando-se o questionário genérico SF-36. Níveis circulantes de marcadores de disfunção microvascular foram analisados por ensaios imunoenzimáticos. Todas as medidas foram realizadas na condição basal, e aos três e seis meses de tratamento com iPDE 5. Utilizando casuística ampliada com análise de dados tanto de forma retrospectiva como prospectiva, investigamos variáveis com possível valor prognóstico. RESULTADOS: Os tratamentos com ambos os fármacos foram associados à melhora da capacidade física (classe funcional e distância percorrida em 6 minutos, p < 0,05), hematócrito e nível de hemoglobina (p < 0,05), e qualidade de vida relacionada à saúde (componentes físico e mental, p < 0,05). Comparados ao grupo controle, na condição basal, os pacientes apresentaram níveis plasmáticos elevados de beta-tromboglobulina (beta-TG, p=0,002), selectina P (p=0,018), ativador do plasminogênio tipo tecidual (t-PA, p=0,005) e do antígeno do fator de von Willebrand (VWF:Ag, p=0,005); a trombomodulina mostrou-se significantemente reduzida (TM, p < 0,001). A administração de tadalafila foi seguida de melhora no nível de beta-TG (p=0,004), t-PA (p=0,003) e TM (p=0,046), enquanto a selectina P modificou-se apenas com a sildenafila (p=0,034). O VWF:Ag mostrou-se com melhora significante, porém transitória, no grupo tratado com sildenafila (p=0,019). Usando casuística ampliada que incluiu pacientes de estudos anteriores de nosso grupo (coorte estendida de 75 indivíduos), analisamos dados coletados no início do seguimento para todos eles, assim como outros relacionados ao tratamento. Uma série de variáveis clínicas e laboratoriais foram testadas quanto à sua possível correlação com a sobrevida livre de evento (seguimento de nove anos). Nesta parte do estudo, os pacientes receberam não apenas iPDE 5 como tratamento para HAP. Alguns deles receberam antagonistas dos receptores de endotelina isoladamente, enquanto outros receberam estes medicamentos (bosentana, ambrisentana) em combinação com iPDE 5. Após realização de análises univariada, bivariada e multivariada foi possível a identificação de quatro preditores de prognóstico desfavorável: 1, menor tempo de exposição a fármacos específicos para HAP (p < 0,001); 2, classe funcional avançada na admissão (p=0,010); 3, função sistólica ventricular direita deprimida (p=0,010); e 4, níveis circulantes elevados de VWF:Ag (p=0,013). Menor tempo de exposição aos medicamentos específicos para HAP foi caracterizado como preditor independente de desfechos. CONCLUSÕES E COMENTÁRIOS: Com base nestas observações, concluímos que os iPDE 5 são importantes no manejo da HAP-CCg avançada. Os efeitos foram observados não apenas na capacidade física e na qualidade de vida, mas também em marcadores biológicos de disfunção microvascular, potencialmente relacionados à progressão e/ou complicações da doença. Além disso, as observações em longo prazo sugeriram que maior tempo de exposição aos fármacos para HAP (incluindo iPDE 5) foi benéfico, enfatizando a necessidade de instituição precoce das terapias. O fato de o estudo ter sido realizado apenas com fármacos de administração oral torna os resultados relevantes no que diz respeito a regiões onde o acesso a tratamentos específicos para HAP é limitado a tais modalidades / INTRODUCTION: Pulmonary arterial hypertension associated with congenital heart disease (CHD-PAH) is a particular form of pulmonary vascular disease with a number of complications. Patients with advanced CHD-PAH are deemed inoperable, and may present with chronic hypoxemia and secondary erythrocytosis due to right-to-left shunting across the cardiac communications. The present study was aimed at analyzing the response to 2 phosphodiesterase type 5 inhibitors (PDE5is), namely sildenafil and tadalafil, which were administered for 6 months. The drugs had been previously approved for management of PAH in general. However, there has been scarce literature on their use in the specific setting of CHD-PAH. We wished to investigate potential effects beyond their actions as vasodilators. METHODS: Thirty-one patients with advanced CHD-PAH were enrolled (age 10 to 54 years). Most of them had systemic oxygen desaturation with elevated hematocrit. Sildenafil and tadalafil were administered orally (respectively, 20 mg t.i.d and single daily dose of 40 mg). The physical capacity was assessed by registering the 6-minute walked distance (6-minute walk test, American Thoracic Society protocol). Health-related quality of life was assessed using the SF-36 generic questionnaire. Circulating levels of microvascular dysfunction markers were analyzed by enzyme-linked immunoassays. All measurements were performed at baseline, 3 months and 6 months of PDE5i administration. Using a larger patient population with retrospective as well as prospective data analysis, we looked for potential predictors of prognosis. RESULTS: Both therapies were associated with improvement of the physical capacity (functional class and 6-minute walked distance, p < 0.05), hematocrit and hemoglobin level (p < 0.05), and health-related quality of life (physical and mental components, p < 0.05). Compared to controls, patients had elevated baseline plasma levels of beta-thromboglobulin (beta-TG, p=0.002), P-selectin (p=0.018), tissue-type plasminogen activator (t-PA, p=0.005) and von Willebrand factor antigen (VWF:Ag, p=0.005), while thrombomodulin was importantly reduced (TM, p < 0.001). Tadalafil administration was followed by improvement of beta-TG (p=0.004), t-PA (p=0.003) and TM level (p=0.046), while P-selectin was improved by sildenafil only (p=0.034). VWF:Ag improved significantly, but only transiently in the sildenafil group (p=0.019). Using an extended population which included additional patients from previous studies of ours (the extended cohort consisted of 75 individuals), we analyzed data collected at the beginning of follow-up for all subjects, and treatment-related data as well. A number of clinical and laboratory parameters were tested for their possible correlation with event-free survival (9-year survival analysis). In this part of the study, patients were on monotherapy or combination therapy with PDE5is (sildenafil or tadalafil) and endothelin receptor antagonists (bosentan or ambrisentan). Univariate, bivariate and multivariate analyses were used to identify four predictors of a more worrisome prognosis: 1, a shorter time of exposure to specific PAH therapies (p < 0.001); 2, an advanced functional class at entry (p=0.010); 3, a depressed right ventricular systolic function (p=0.010); and 4, high circulating levels of VWF:Ag (p=0.013). A shorter exposure to PAH therapies was characterized as an independent predictor of poor outcome. CONCLUSIONS AND COMMENTS: Our observations point towards beneficial effects of PDE5is in advanced CHD-PAH. Importantly, the observed effects were not only on the physical capacity, and quality of life, but also biological markers of microvascular injury potentially related to disease progression and/or complications. Furthermore, long-term patient observation showed that a longer exposure to PAH drugs (including PDE5is) was beneficial, thus emphasizing the need for early initiation of therapies. Because only oral drugs were used, results may be relevant for countries and regions with limited access to PAH-specific medications

Introdução: Vários são os fatores que alteram a atividade sexual de homens e mulheres. Com o envelhecimento observa-se aumento das queixas de desejo hipoativo feminino e de disfunção erétil. Visto que o homem e sua parceira constituem um sistema dinâmico, antes do advento dos inibidores da fosfodiesterase eles se adaptavam às condições disfuncionais do casal. A eficácia aliada a poucos efeitos colaterais e à facilidade de administração da sildenafila e, posteriormente, da vardenafila e tadalafila, revolucionou o tratamento da disfunção erétil. Por outro lado, até a presente data, a terapêutica medicamentosa com testosterona para o desejo sexual hipoativo de mulheres, ainda gera controvérsias. Objetivo: Avaliar o uso de androgênio, utilizado para tratamento das queixas de desejo sexual hipoativo em mulheres, comparando dois períodos, ou seja, pré e após o aparecimento iPDE 5. Os efeitos colaterais e as divergências em relação a este tratamento também serão analisados. Método: Foram selecionados estudos em inglês, português e espanhol, publicados entre 1988 e os dias atuais, ou seja, na década pré-advento dos inibidores da fosfodiesterase 5 e após. A busca dos artigos foi feita em periódicos indexados nas bases Lilacs, Cochrane, Embase e Medline/PubMed, utilizando-se os seguintes descritores e suas combinações: sexualidade (sexuality), desejo sexual hipoativo em mulheres (female hypoactive sexual desire disorder), testosterona (testosterone), terapia androgênica em mulheres (androgen therapy in women). Discussão: Embora haja evidência sobre a efetividade do tratamento com testosterona para desejo hipoativo em mulheres, este uso ainda gera muitas controvérsias. Resultados: O número de estudos randomizados sobre uso de testosterona para tratamento de DSH feminino aumentou de 10%, comparando o período compreendido entre 1988 e 1998, para 90% entre 1999 e 2012. Todos os estudos randomizados analisados demonstraram benefícios sobre a resposta sexual, melhorando a libido, a excitação e/ou o orgasmo. Porém, como tiveram seguimento por um período máximo de 24 semanas, os riscos dessa administração não foram esclarecidos. Conclusão: A partir de 1988, ou seja, após a liberação comercial dos inibidores da fosfodiesterase do tipo 5, para tratamento da disfunção erétil, houve um aumento significativo do número de pesquisas com a finalidade de avaliar o uso de testosterona em mulheres com desejo hipoativo. Porém, ainda são necessários estudos de longo prazo para que os riscos e os benefícios, desta administração, sejam esclarecidos / Introduction: There are several factors that alter the sexual activity of both men and women. With age, an increase in complaints about hypoactive feminine desire and erectile dysfunction can be observed. Since the man and his partner constitute a dynamic system, before the advent of phosphodiesterase inhibitors, they would adapt to the dysfunctional conditions of the couple. The efficacy with few collateral effects and the easy administration of sildenafil and later, vardenafil and tadalafil, has revolutionized the treatment of erectile dysfunction. On the other hand, until now, the drug therapy with testosterone for hypoactive sexual desire of women still generates controversies. Objective: Assess the use of androgen, used for treating complaints of hypoactive sexual desire in women, comparing two periods, that is, before and after the emergence of iPDE 5. Side effects and divergences regarding this treatment are also analyzed. Method: Studies in English, Portuguese and Spanish were selected, published between 1988 and the present, that is, in the decade before the advent of phosphodieterase 5 inhibitors and after this fact. The search for papers was made in indexed journals on Lilacs, Cochrane, Embase and Medline/PubMed data bases, using the following descriptors and their combinations: sexuality, female hypoactive sexual desire disorder, testosterone or androgen therapy in women. Discussion: Although there is evidence on the effectiveness of treatment with testosterone for hypoactive desire in women, this use still generates many controversies. Results: The number of randomized studies on the use of testosterone for the treatment of female HSD has increased 10%, comparing the period between 1988 and 1998, to 90% between 1999 and 2012. All randomized studies analyzed showed benefits on the sexual response, improving libido, excitation and/or orgasm. However, since they were followed for a maximum of 24 weeks, the risks of this dosage were not clarified. Conclusion: From 1988, that is, after the commercial release of phosphodiesterase type 5 inhibitors, for the treatment of erectile dysfunction, there was a significant increase in the number of papers with the purpose of evaluating the use of testosterone in women with hypoactive desire. Os pacientes que usam lítio (Li) para tratamento do transtorno bipolar freqüentemente apresentam poliúria e deficiência de concentração urinária, sintomas do Diabetes Insipidus nefrogênico (DIN). Animais tratados com Li apresentam baixos níveis de produção de adenosina monofosfato cíclico (AMPc) em resposta ao hormônio antidiurético (HAD). O Sildenafil (Sil), um inibidor da fosfodiesterase 5 (PDE5), eleva os níveis intracelulares de guanosina monofosfato cíclico (GMPc), levando a inserção de aquaporina 2 (AQP2) na membrana plasmática das células do ducto coletor. Portanto, inibidores de PDE podem promover a inserção de AQP2 na membrana plasmática mesmo sem a ativação do receptor de HAD, indicando a participação de uma via alternativa mediada pelo GMPc. Nós investigamos o efeito do Sil na expressão renal das proteínas de membrana AQP2, UT-A1, NKCC2, NHE3, P-ENaC em ratos com DIN induzido pelo Li. Ratos Wistar foram divididos nos seguintes grupos: grupo controle, recebendo dieta alimentar normal durante quatro semanas; grupo Li, recebendo dieta alimentar normal com 40 mmol Li por quilo de dieta durante quatro semanas; grupo Li + Sil, recebendo dieta alimentar normal com 40 mmol Li por quilo de dieta durante quatro semanas e 200 mg por quilo de dieta de Sil a partir da segunda semana; grupo Sil, recebendo dieta alimentar normal durante a primeira semana e a partir da segunda semana recebendo dieta normal com 200 mg de Sil por quilo de dieta. Os animais do grupo Li desenvolveram poliúria, diminuição da osmolalidade urinária e diminuição da expressão da AQP2. No grupo Li+Sil, o Sil foi capaz de reverter parcialmente a poliúria, diminuir o clearance de água livre, aumentar a osmolalidade urinária e aumentar a expressão da AQP2. A expressão de UTA1 foi completamente normalizada com o tratamento com Sil. A expressão das proteínas NKCC2 e NHE3 apresentaram-se aumentadas no grupo tratado com Li, e o Sil não foi capaz de reverter tal alteração. Além disso, o tratamento com Sil reverteu completamente o aumento da resistência vascular renal. Assim, concluímos que o tratamento com Sil em ratos com DIN melhora a poliúria, aumenta a smolalidade urinária e diminui o clearance de água livre pelo aumento da expressão de AQP2 e UT-A1. O tratamento com Sil pode ser benéfico para pacientes que sofrem com DIN induzida pelo Li. / Patients taking lithium to treat bipolar disorder often present polyuria and urinary concentrating defect. In addition, lithium-treated animals present lower cyclic adenosine monophosphate production in response to vasopressin. Sildenafil (Sil), a phosphodiesterase 5 (PDE5) inhibitor, elevates intracellular cyclic guanosine monophosphate (cGMP) levels, leading to plasma membrane accumulation of aquaporin 2 (AQP2). Therefore, PDE inhibitors might induce AQP2 membrane insertion even without vasopressin receptor activation by activating a parallel cGMP-mediated signal transduction pathway. We investigated the effect of sildenafil on renal expression of AQP2, UT-A1, sodium/hydrogen exchanger (NHE3), type 1 bumetanide-sensitive Na-K-2Cl cotransporter (NKCC2), and the epithelial sodium channel alpha subunit (P-ENaC). Wistar rats received lithium (40 mmol/kg food) or not for 4 weeks (Li or control), some rats also receiving sildenafil (200 mg/kg food) in weeks 2-4, with or without lithium (Li+Sil or Sil). In Li+Sil rats, urine output was markedly lower, as was water free clearance, whereas urine osmolality was higher. Semiquantitative immunoblotting revealed the following: AQP2 expression was partially normalized; UT-A1 expression was completely normalized; expression of NKCC2 and NHE3 was significantly higher in Li rats (although not significantly different between Li+Sil rats and Li rats); and P-ENaC protein expression was unaltered in all groups. Sildenafil treatment completely reversed the lithium-induced increase in renal vascular resistance. In conclusion, sildenafil treatment of lithium-induced nephrogenic diabetes insipidus (NDI) improves polyuria, increases urinary osmolality, and decreases free water clearance via upregulation of renal AQP2 and UT-A1. Sildenafil treatment could be beneficial in patients with lithium-induced NDI.

Structure and function of the myelin enzyme 2′,3′-cyclic nucleotide 3′-phosphodiesterase

Abstract The myelin sheath is a crucial component of vertebrate nervous systems. Myelin is formed as the plasma membrane of a glial cell is wrapped around a neuronal axon. The presence of myelin enables the fast transmission of neuronal impulses, and degradation or dysfunction of myelin results in severe neurological symptoms. Molecular composition of myelin is unique, and many myelin proteins are not present elsewhere in the body. A myelin enzyme, 2′,3′-cyclic nucleotide 3′-phosphodiesterase (CNPase), is found in specific regions within the myelin sheath and is one of the most abundant proteins in the brain. Substrates for CNPase catalytic activity are formed during brain damage. CNPase also interacts with the cytoskeleton and cell membranes, and it is thought to play a role during myelin formation. Mice that lack CNPase suffer from axonal degeneration and die early. The aim of this study was to characterise CNPase structure and function. To this end, a system was first developed to produce the protein for subsequent analyses. The aim was to characterise the catalytic mechanism of CNPase by determining its three-dimensional molecular structure at different stages of the catalytic reaction. The interactions between CNPase and other molecules related to its function would also be characterised. Finally, the structure of the full-length protein would be used to understand of the function of the uncharacterised N-terminal domain. Using X-ray crystallography, the structure of the CNPase catalytic domain was determined in the presence of substrate and product molecules. These data, complemented with analyses of mutationally inactivated enzyme variants, were used to examine the catalytic reaction at the molecular level. The catalytic domain structure was compared to homologous enzymes from diverse organisms. The interaction between CNPase and the calcium-sensing protein calmodulin was characterised. The solution structure of full-length CNPase was determined using small-angle X-ray scattering, and protein sequence databases were utilised to determine CNPase conservation during animal evolution. The results provide novel information on the catalytic activity and overall function of CNPase. Further studies will be necessary to determine its specific role, but it is increasingly clear that CNPase can perform multiple important tasks within the nervous system. / Tiivistelmä Myeliinituppi on tärkeä osa selkärankaisten hermostoa. Myeliiniä muodostuu, kun gliasolun solukalvo kiertyy hermosolun aksonin ympärille. Myeliini mahdollistaa hermoimpulssien nopean välityksen, ja sen tuhoutuminen ja vajaatoiminta aiheuttavat vakavia neurologisia oireita. Myeliinin molekyylikoostumus on ainutlaatuinen, ja monet myeliiniproteiineista eivät esiinny muualla elimistössä. Myeliinissä esiintyvää entsyymiä, 2′,3′-syklisten nukleotidien 3′-fosfodiesteraasia (CNPaasi), esiintyy runsaasti tietyillä myeliinialueilla, ja se on yksi aivojen runsaslukuisimmista proteiineista. Substraatteja CNPaasin katalyyttiselle aktiivisuudelle muodostuu aivovaurion aikana. Rakennetta, täydennettynä mutaatioilla inaktivoitujen entsyymimuunnosten analyysillä, käytettiin katalyyttisen reaktion molekyylitason karakterisointiin. Katalyyttisen alayksikön rakennetta verrattiin eri organismeissa esiintyviin homologisiin entsyymeihin. CNPaasin ja kalsiumia sitovan kalmoduliinin vuorovaikutusta karakterisoitiin. Kokopitkän CNPaasin liuosrakenne selvitettiin pienkulmaröntgensironnan avulla, ja CNPaasin sekvenssin säilymistä eläinten evoluution aikana tarkasteltiin proteiinisekvenssitietokantoja käyttämällä. Tulokset antavat uutta tietoa CNPaasin katalyyttisestä aktiivisuudesta ja tämän arvoituksellisen entsyymin toiminnasta. Moonlighting Functions of the Rv0805 Phosphodiesterase from Mycobacterium Tuberculosis

All organisms must sense and respond to their environment in order to survive. The processes that allow a living cell to sense changes in its environment, and respond appropriately are collectively referred to as ‘signal transduction’. Cyclic AMP is a ubiquitously used second messenger molecule that plays diverse roles from hormone signalling in mammalian cells to catabolite repression in enteric bacteria. In several bacterial pathogens such as Pseudomonas aeruginosa, cAMP has also been found to mediate pathogenesis, usually by regulating the production of several virulence factors aiding in colonisation of the host. Cyclic AMP signalling has been suggested to regulate the virulence of the obligate intracellular Mycobacterium tuberculosis. Mycobacteria, including M. tuberculosis, code for a large number of adenylyl cyclases, enzymes that synthesise cAMP. Of the 16 putative adenylyl cyclases encoded by M. tuberculosis H37Rv, 10 have received extensive biochemical attention. A knockout of one of these cyclases, Rv0386, resulted in compromised virulence of M. tuberculosis. Ten proteins predicted to bind cAMP and mediate its cellular roles have also been identified in M. tuberculosis. Among these are the cAMP-regulated transcription factor, CRPMt, and cAMP-regulated protein acetyl transferase, KATmt. Comparatively little information is available, however, regarding the roles of cAMP-degrading machinery in mycobacteria. Two phosphodiesterases, with modest activity against cAMP in vitro, have been identified from M. tuberculosis, and are encoded by the Rv0805 and Rv2795c loci. Of these, Rv2795c has orthologs in all sequenced mycobacterial genomes. However, Rv0805-like proteins are coded only by slow growing mycobacteria such as the M. tuberculosis-complex, M. marinum and M. leprae, several of which are human or animal pathogens. Rv0805 belongs to the metallophosphoesterase superfamily of proteins, consisting of metal-dependent phosphoesterases with substrates ranging from large polymers like nucleic acids to small molecules like cAMP and glycerophospholipids. Like other metallophosphoesterases, Rv0805 displays promiscuous substrate utilisation and efficient hydrolysis of 2’3’-cAMP in vitro. Rv0805 also hydrolyses 3’5’-cAMP in vitro. Overexpression of Rv0805 is reported to lead to reduction in intracellular cAMP levels in M. smegmatis and M. tuberculosis, suggesting that it is capable of hydrolysing cAMP in the bacterial cell as well. The structure of Rv0805 revealed a sandwich-like α/β fold, typical of metallophosphoesterases, along with a relatively flexible C-terminal domain of unknown function. Despite extensive biochemical and structural information on Rv0805 however, its roles in mycobacteria remain unknown. In this study, the cellular roles of Rv0805 are explored and using information from biochemical and structural analyses, novel activities and interactions of Rv0805 have been identified. Rv0805, when expressed in M. smegmatis, led to a reduction in intracellular cAMP, as previously reported. However, the extent of reduction was modest (~30 %) and limited to the exponential phase of growth when both Rv0805 and intracellular cAMP are at their highest levels. Overexpression of Rv0805 also resulted in hypersensitivity to cell wall perturbants like crystal violet and sodium dodecyl sulphate (SDS) indicative of a change in the properties of the cell envelope of M. smegmatis. Importantly, these effects were independent of cAMP-hydrolysis by Rv0805, as overexpression of catalytically inactive Rv0805N97A also elicited similar changes. Unexpectedly, Rv0805 was localised to the cell envelope, both in M. tuberculosis as well as in M. smegmatis. The ability of Rv0805 to localise to the cell envelope was dependent on it C-terminus, as truncation of Rv0805 in this region (Rv0805Δ10, Rv0805Δ20 and Rv0805Δ40) resulted in progressively greater enrichment in the cytosol of M. smegmatis. Overexpression of Rv0805Δ40, which was localised almost completely to the cytosol, did not result in hypersensitivity to SDS, suggesting that cell envelope localisation, rather than cAMP-hydrolysis, was crucial for the cell envelope modifying roles of Rv0805. A possible mechanism behind the cell envelope-related effects of Rv0805 overexpression was the ability of the protein to interact with the cell wall of mycobacteria in a C-terminus-dependent manner. Purified Rv0805, but not Rv0805Δ40, could associate with crude mycobacterial cell wall as well as purified cell wall core polymer (mycolyl-arabinogalactan-peptidoglycan complex) in vitro. In addition to the C-terminus, the architecture of the active site was also crucial for this interaction as mutations in the active site that compromised metal-binding also resulted in poor interaction with the cell wall. Most significant among these residues was His207, which when mutated to Ala almost completely abrogated interaction with the cell wall in vitro. Further, Rv0805H207A was unable to localise to the cell envelope when expressed in M. smegmatis, even in the presence of the C-terminus, highlighting the importance of this residue in maintaining the structural integrity of Rv0805, and demonstrating that the structure of the C-terminus, rather than its sequence alone, played a role in cell envelope localisation and interaction. In order to verify that the observed sensitivity of Rv0805-overexpressing M. smegmatis to cell wall perturbants was due to a change in cell envelope properties atomic force microscopy was employed. Two distinct modes of operation were used to analyse surface and bulk properties of the mycobacterial cell envelope. These were tapping mode phase imaging, and contact mode force spectroscopy. Using tapping mode phase imaging, it was found that the cell surface of M. smegmatis was inherently heterogeneous in its mechanical properties. Further, contact mode force-spectroscopy revealed that the cell envelope of M. smegmatis in cross-section had at least three layers of varying stiffness. Typically, a middle layer of high stiffness was observed, sandwiched between two lower stiffness layers. This organisation is reminiscent of the current model of the mycobacterial cell envelope, possessing a central polysaccharide rich layer and outer and inner lipid rich layers. Treatment of wild type M. smegmatis with cell wall-perturbing antibiotics isoniazid and ethambutol resulted in markedly altered phase images, as well as significantly lower stiffness of the bacterial cell envelopes, validating that the methodology employed could indeed be used to assess cell wall perturbation in mycobacteria. Further, M. smegmatis harbouring deletions in cell envelope biosynthesis related genes, MSMEG_4722 and aftC, showed significantly lower cell wall stiffness than wild type M. smegmatis, providing evidence that genetic perturbation of the cell wall of mycobacteria could also be studied using atomic force microscopy. While phase imaging revealed similar surface properties of Rv0805-overexpressing and control M. smegmatis, force spectroscopy revealed significantly lower cell envelope stiffness, particularly of the middle layer, of the former. Cell envelope stiffness was, however, unaffected by expression of Rv0805Δ40 in M. smegmatis, providing direct evidence for C-terminus-dependent cell envelope perturbation upon Rv0805 overexpression. Additionally, overexpression of Rv0805N97A, but not Rv0805H207A led to reduced stiffness of the cell envelope of M. smegmatis, demonstrating that the cell wall remodelling activity of Rv0805 was independent of cAMP-hydrolysis, but dependent on cellular localisation and cell wall interaction. Like in M. smegmatis, overexpression of Rv0805 also led to lower cAMP levels in M. tuberculosis. Using a microarray-based transcriptomics approach, pathways affected by Rv0805 overexpression were identified. Rv0805 overexpression elicited a transcriptional response, leading to the down-regulation of a number of virulence associated genes such as whiB7, eis, prpC and prpD. Importantly, Rv0805-overexpression associated gene expression changes did not include genes regulated by CRPMt, the primary cAMP-regulated transcription factor in M. tuberculosis. Further, Rv0805N97A overexpression in M. tuberculosis led to similar changes in gene expression as overexpression of the wild type protein. These observations reiterated that, at least upon overexpression, the effects of Rv0805 were largely independent of cAMP-hydrolysis. Using overexpression in M. smegmatis and M. tuberculosis, cAMP-hydrolysis independent roles of Rv0805 in mycobacteria were identified. To further validate these observations, a knockout strain of the Rv0805 gene was generated in M. bovis BCG, a well-established model to study M. tuberculosis. Curiously, deletion of Rv0805 did not lead to a change in intracellular cAMP levels, demonstrating that cAMP-hydrolysis by Rv0805 may not contribute to the modulation of mycobacterial cAMP levels under standard laboratory growth conditions. Rv0805 deletion led to altered colony morphology and possible reduction in cell wall thickness, reaffirming the roles of this phosphodiesterase in regulating cell envelope physiology of mycobacteria. Additionally, Rv0805 deletion also resulted in compromised growth of M. bovis BCG in fatty acid-deficient media, implicating Rv0805 as a possible regulator of carbon metabolism. In summary, this thesis explores novel links between Rv0805 and the mycobacterial cell wall and elucidates the critical importance of the C-terminus domain of this metallophosphodiesterase in modulating its cellular localisation to, and interaction with, the mycobacterial cell envelope. En route to understanding the effects of Rv0805 overexpression on the cell wall of M. smegmatis, an atomic force microscopy-based methodology to assess perturbation of the cell envelope of mycobacteria was also developed. Finally, using a combination of biochemical and genetic analyses, cellular roles of Rv0805, independent of cAMP-hydrolysis, were identified in slow-growing mycobacteria. This study therefore provides direct evidence against the sole role of this mycobacterial phosphodiesterase as a regulator of intracellular cAMP levels, and opens up new avenues to understanding the cellular functions of Rv0805 and indeed other members of the metallophosphoesterase superfamily.

Synthesis and In Vitro Evaluation of 8-Pyridinyl-Substituted Benzo[e]imidazo[2,1-c][1,2,4]triazines as Phosphodiesterase 2A Inhibitors

Phosphodiesterase 2A (PDE2A) is highly expressed in distinct areas of the brain, which are known to be related to neuropsychiatric diseases. The development of suitable PDE2A tracers for Positron Emission Tomography (PET) would permit the in vivo imaging of the PDE2A and evaluation of disease-mediated alterations of its expression. A series of novel fluorinated PDE2A inhibitors on the basis of a Benzoimidazotriazine (BIT) scaffold was prepared leading to a prospective inhibitor for further development of a PDE2A PET imaging agent. BIT derivatives (BIT1–9) were obtained by a seven-step synthesis route, and their inhibitory potency towards PDE2A and selectivity over other PDEs were evaluated. BIT1 demonstrated much higher inhibition than other BIT derivatives (82.9% inhibition of PDE2A at 10 nM). BIT1 displayed an IC50 for PDE2A of 3.33 nM with 16-fold selectivity over PDE10A. This finding revealed that a derivative bearing both a 2-fluoro-pyridin-4-yl and 2-chloro-5-methoxy-phenyl unit at the 8- and 1-position, respectively, appeared to be the most potent inhibitor. In vitro studies of BIT1 using mouse liver microsomes (MLM) disclosed BIT1 as a suitable ligand for 18F-labeling. Nevertheless, future in vivo metabolism studies are required.

In this work, the influence of the second messenger cAMP on the functional plasticity of M-DC8+ dendritic cells (DC) was examined. The marker M-DC8 defines a population of native DC first described in blood. After their isolation, M-DC8+ DC acquire a mature CD83+ phenotype during a short culture ex vivo. After a challenge with LPS and IFN-g, M-DC8+ DC secrete large amounts of the proinflammatory cytokines IL-12(p70) and TNF-a surpassing by far other DC populations and monocytes. Due to their preferential induction of TH1-dominated T cell responses, M-DC8+ DC might play a role in the pathogenesis of inflammatory diseases. Different cAMP-elevating agents suppressed the proinflammatory cytokine production and enhanced the secretion of anti-inflammatory IL-10. Activity of phosphodiesterase (PDE) 4, the most important cAMP-hydrolysing enzyme in immune cells, was detected and RT-PCR revealed the expression of PDE4 subtypes 4A, 4B and 4D in M-DC8+ DC, whereas 4C was not detectable. The PDE4-specific inhibitors AWD12-281 and Roflumilast were then used to elevate cAMP concentrations. These substances have been proven to be efficient in anti-inflammatory therapies. In the presence of PDE4 inhibitors, the LPS/IFN-g-induced production of IL-12 and TNF-a was decreased by 90 % and 60 %, respectively, whereas the IL-10-release was doubled. These effects were only observed, if the PDE4 inhibitors where present from the beginning of the culture. The inhibition of the IL-12 secretion was reverted using an a-IL-10-receptor antibody. PDE4 inhibitor-treated M-DC8+ DC showed a reduced capacity to polarize TH1-cells, which was demonstrated analysing culture supernatants by ELISA and by single-cell analysis detecting intracellular IFN-g und IL-4. These results suggest that PDE4 inhibitors may not only be useful in the therapy of TH2-mediated diseases but also in TH1-dominated indications such as multiple sclerosis and Crohn´s disease. Despite the shift of the cytokine profile, the in vitro maturation of M-DC8+ DC was not affected by PDE4 inhibitors. The expression of CD83, CD80, CD86, MHC-molecules as well as CD54 and CD58, was assessed by FACS analysis. Correspondingly, in the presence of AWD12-281, M-DC8+ DC efficiently stimulated the proliferation of allogeneic CD4+CD45RA+ T-cells. In the second part of this study, the effects of an inhibition of cAMP-synthesis in M-DC8+ DC were analyzed. Two adenylyl cyclase (AC) inhibitors, 2,5-Dideoxyadenosine and SQ22536, clearly hampered the in vitro maturation of M-DC8+ DC. The expression of the DC maturation marker CD83 could be reconstituted using the stable cAMP-analogon 8-Br-cAMP. Measuring the intracellular cAMP concentration in M-DC8+ DC, initially low cAMP-levels were observed, but within 30 min the concentration raised and returned to original levels within 2 hrs. Blocking the cAMP synthesis by AC inhibitors, the LPS/IFN-g-induced production of IL-12, TNF-a and IL-10 was strongly reduced. Furthermore, it was demonstrated that M-DC8+ DC can only release IL-12 after a transient elevation of cAMP, i.e. they acquire a &amp;quot;license&amp;quot;. Such a regulation of the IL-12 production has not been described before. Protein kinase A is an important effector molecule of cAMP. Inhibiting its activity resulted in a reduced expression of the DC maturation marker CD83 and a lower cytokine production underlining the importance of cAMP-signalling for the activation of M-DC8+ DC. In conclusion, this study provides evidence for a new concept of the immune-regulatory function of cAMP. Here, cAMP is essentially involved in the initial activation and maturation of DC and enables them to secrete large amounts of IL-12 and TNF-a upon stimulation with a TLR ligand. Conversely, a long-term elevation of cAMP-concentrations inhibits the proinflammatory effector functions of M-DC8+ DC and can induce anti-inflammatory responses by enhancing the secretion of IL-10. / In dieser Arbeit wurde der Einfluss des second messengers cAMP auf die funktionelle Plastizität von M-DC8+ dendritischen Zellen (DC) untersucht. Der Oberflächenmarker M-DC8 definiert eine zunächst im Blut beschriebene Population nativer DC. Nach ihrer Isolation erlangen M-DC8+ DC während einer kurzen Kultur einen maturen CD83+ Phänotyp. Nach Stimulation mit LPS und IFN-g produzieren native M-DC8+ DC deutlich höhere Mengen der proinflammatorischen Zytokine IL-12(p70) und TNF-a als andere DC-Populationen oder Monozyten. Dies resultiert in einer Programmierung TH1-dominierter T-Zellantworten. M-DC8+ DC könnten daher an der Pathogenese entzündlicher Krankheiten beteiligt sein. Unterschiedliche cAMP-erhöhende Substanzen supprimierten die proinflammatorische Zytokinproduktion und verstärkten gleichzeitig die Sekretion des anti-inflammatorischen IL-10. In M-DC8+ DC konnte die Aktivität von Phosphodiesterase (PDE) 4, dem wichtigsten cAMP-hydrolysierenden Enzym in Immunzellen, nachgewiesen werden. Durch RT-PCR wurde die Expression der PDE4-Subtypen 4A, 4B und 4D gezeigt, nicht aber 4C. Zur Erhöhung der cAMP-Konzentration wurden dann die PDE4-spezifischen Inhibitoren AWD12-281 und Roflumilast eingesetzt, deren klinische Effizienz bei anti-inflammatorischen Therapien belegt ist. Auch diese Substanzen verringerten die LPS/IFN-g-induzierte Produktion von IL-12 und TNF-a durch M-DC8+ DC um 90 % bzw. 60 %, während die IL-10-Freisetzung etwa verdoppelt wurde. Diese starken Effekte konnten nur erzielt werden, wenn die PDE4-Inhibitoren von Beginn der Kultur an eingesetzt wurden. Die Hemmung der IL-12-Sekretion wurde in Gegenwart eines a-IL-10-Rezeptor-Antikörpers aufgehoben. Unter dem Einfluss von PDE4-Inhibitoren war die TH1-Programmierung durch M-DC8+ DC deutlich reduziert, was sowohl durch die Analyse der Zellüberstände mittels ELISA als auch auf Einzelzell-Ebene durch intrazelluläre Detektion von IFN-g und IL-4 nachgewiesen wurde. Diese Ergebnisse legen nahe, dass PDE4-Inhibitoren nicht nur für TH2-vermittelte Erkrankungen sondern auch für TH1-dominierte Indikationen wie Multiple Sklerose oder Morbus Crohn von Nutzen sein könnten. Trotz der starken Modulation des Zytokinprofils blieb die in vitro-Ausreifung M-DC8+ DC unbeeinflusst von PDE4-Inhibitoren. Untersucht wurde die Expression von CD83, CD80, CD86, MHC-Molekülen, CD54 und CD58 mittels FACS-Analyse. Entsprechend induzierten M-DC8+ DC auch in Anwesenheit von AWD12-281 die Proliferation allogener CD4+CD45RA+ T-Zellen. Dies unterstreicht die Bedeutung von cAMP für die Aktivierung M-DC8+ DC. Zusammenfassend gibt diese Arbeit am Beispiel nativer humaner DC Anhalt für ein neues Konzept der immunregulatorischen Funktion von cAMP. Hierbei ist cAMP wesentlich an der Ausreifung von M-DC8+ DC beteiligt, woraufhin diese große Mengen IL-12 und TNF-a sekretieren können. Dagegen wirkt eine langfristige cAMP-Erhöhung durch die Induktion von IL-10 anti-inflammatorisch.

The effects of administration of a nuclear factor kappa-B inhibitor on pulmonary endothelial dysfunction after cardiopulmonary bypass: impact on oxygenation and hemodynamics and development of therapeutic and preventive modalities in a porcine model

Introduction: La circulation extracorporelle (CEC) peut entraîner une dysfonction endothéliale pulmonaire et l’hypertension pulmonaire. Le SN50 agit au niveau de la signalisation cellulaire pour prévenir ces réactions à la CEC et pourrait renverser la dysfonction endothéliale pulmonaire post-CEC sans effets néfastes sur l’hémodynamie. Méthodes: Quatre groups de porcs ont reçu un parmi quatre traîtements avant de subir 90 minutes de CEC et 60 minutes de reperfusion: (1) milrinone nébulisé; (2) sildenafil nébulisé; (3) placebo nébulisé; et (4) SN-50 intraveineux. Un monitoring hémodynamique invasif a été utilisé. La réactivité vasculaire des artères pulmonaires de deuxième ordre a été évaluée face à l’acétylcholine et la bradykinine. Résultats: Le sildénafil produit une augmentation significative de la pression de l’artère pulmonaire (PAP) moyenne à 60 minutes de reperfusion par rapport au début de la chirurgie. Les relaxations dépendantes de l’endothélium face à la bradykinine étaient meilleurs dans les groupes milrinone et SN-50 et surtout dans le groupe sildénafil par rapport au groupe placébo. Le SN-50 produisait de moins bonnes relaxations dépendantes de l’endothélium face à l’acétylcholine que les autres traitements incluant placébo. Conclusion: Le sildénafil prévient mieux la dysfonction endothéliale pulmonaire que les autres traitements. Les bénéfices du SN-50 sont possiblement sous-estimés vu que la dose n’a pas pu être ajustée à la durée de CEC. Le sildenafil inhalé mérite une étude plus importante chez l’humain et le SN-50 dans un model de CEC animal. / Background: Cardiopulmonary bypass (CPB) can lead to pulmonary endothelial dysfunction and consequent pulmonary hypertension. The novel agent SN-50 acts at the level of the transduction pathway to prevent these responses and may limit or reverse post-CPB pulmonary endothelial dysfunction and pulmonary hypertension without the untoward effects on hemodynamics seen with other known therapies. Methods: Four groups of Landrace-Yorkshire swine that received one of four treatments before undergoing 90 minutes of normothermic CPB and 60 minutes of reperfusion were compared: (1) Nebulized milrinone; (2) nebulized sildenafil; (3) placebo consisting of nebulized NaCl solution; and (4) intravenous SN-50. Invasive hemodynamic monitoring was used throughout all experiments. Vascular reactivity of second-degree pulmonary arteries was evaluated in response to acetylcholine and bradykinin. Results: Sildenafil produced a significant increase in mean pulmonary artery pressure (PAP) at 60 minutes after CPB compared to baseline. Both the sildenafil and milrinone groups had increased mean PAP/MAP ratio at 60 minutes after CPB compared to baseline, however this ratio was not different between the groups. Endothelial-dependent relaxations to bradykinin were improved in the SN-50 and milrinone groups and especially the sildenafil group as compared to placebo. SN-50 produced worse endothelium-dependent relaxations in response to acetylcholine compared to the other groups including placebo. Conclusion: Sildenafil better prevented pulmonary endothelial dysfunction than all other treatments. The improvements seen with SN-50 may be suboptimal as dose could not be titrated to length of CPB. Inhaled sildenafil and SN-50 both merit further study in human trials and animal models, respectively.
