"Passagem de plantão de enfermagem em um hospital dia psiquiátrico" / NURSING SHIFT ALTERNATION IN A DAY PSYCHIATRIC HOSPITAL

A Reunião de Passagem de Plantão de Enfermagem (RPPE) realizada no Hospital- Dia (HD) do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo, possui características particulares que a diferencia das reuniões tradicionais de passagem de plantão de enfermagem. Ela reune os integrantes da equipe interdisciplinar e todos têm a oportunidade tanto de informar sobre os pacientes assistidos no HD segundo os comportamentos apresentados nas suas atividades específicas, quanto de ouvir as informações dos demais integrantes. Trata-se, portanto, de um momento ímpar, reunindo motivos suficientes para levar a RPPE a objeto do presente estudo, que se caracteriza como quanti-qualitativo e teve como objetivos fazer um levantamento das freqüências dos integrantes da equipe interdisciplinar nessa reunião, dos relatos que fazem sobre observação e evolução dos pacientes e das decisões que tomam frente às ocorrências apresentadas. Para isso, foram utilizadas 20 RPPE em um período de tempo de dois meses, gravadas, transcritas na íntegra, realizadas sínteses e análise de conteúdo com apresentação descritiva dos resultados utilizando-se também de quadros. Os resultados mostraram uma participação maior de profissionais mais experientes na RPPE, talvez por eles coordenarem a maioria das atividades terapêuticas rotineiras do HD. Mostrou ainda, que a RPPE através do enfermeiro coordenador/facilitador, ofereceu oportunidades para os integrantes da equipe interdisciplinar informar sobre comportamentos dos pacientes nas suas atividades específicas, como também ouvir as informações dos outros. Ela mostrou-se cuidadosa, examinando sempre todos os pacientes semiinternados no HD, fazendo as observações e evoluções para cada um e tomando decisões sobre condutas a serem realizadas para aqueles que tinham necessidade no momento. A RPPE favoreceu trocas de experiências entre os profissionais das diversas áreas e contribuiu no processo ensino/aprendizagem do HD, que se constitui também em uma unidade de ensino. / The meeting for nursing shift alternation which happens at the Day Hospital in the Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto of São Paulo University, has particular characteristics which makes it different from the traditional nursing shift alternation meetings. It gathers members of the whole interdisciplinar team and each one, during this meeting, has the opportunity of informing about the patients under assistance at the Day Hospital regarding to their behaviour towards the specifics activities, as well as to hearing from the others. Therefore it is a single moment, combining sufficient reasons for bringing the nursing shift alternation meeting to object of the present study, which has a quantitative and qualitative character and has as objective to raise the interdisicplinar members frequency to those meetings, to report about observations and evolutions of patients, and decisions taken towards the presented situation. To obtain the result were used 20 nursing shift alternation meetings in a period of two months, which were recorded, integrally written-down, prepared content syntheses and analyses for descriptive results presentation also using panels. The results show an increased participation of experienced professionals in the nursing shift alternation meetings, maybe because they coordinate most of the therapeutical routine activities at the Day Hospital. It has also shown that the nursing shift alternation meeting, through the coordinator nurse, offers opportunities for all interdisciplinar team members to inform about their patients’ behaviour towards the specific activities, as well as to hearing from the others. The coordinator was careful, always examining all semi-hospitalized patient at the Day Hospital, making observation and evolution for each one, taking decisions about procedures to be adopted for those who were in need at that moment. The nursing shift alternation meeting gives support to exchanging experiences among professionals from different areas and contributes for the teaching/learning process at Day Hospital, which is also a teaching institution.

Habitar Institucional: Considerações sobre processos de institucionalização de vidas no Hospital Psiquiátrico / Institutional Inhabitation: Considerations on the processes of institutionalization of lives in the psychiatric hospital

Essa pesquisa qualitativa tomou como objeto de estudo o universo dos moradores e moradoras ararenses do hospital psiquiátrico Antonio Luis Sayão, sob gestão estadual, localizado na cidade de Araras, no interior do Estado de São Paulo. A finalidade é indicar mecanismos para o início de processos de desinstitucionalização dessas pessoas. Os objetivos foram: a) identificar, por meio do Questionário do Censo Psicossocial dos Moradores em Hospitais Psiquiátricos do Estado de São Paulo, os moradores e moradoras nascidas no município e que vivem no interior do hospital; b) investigar, por meio dos dados objetivos e os da impressão dos pesquisadores, quem são, seus recursos e possibilidades; c) propor, a partir da análise dos dados, estratégias de desinstitucionalização, para operacionalização pelo gestor local, se for oportuno. São sujeitos do estudo seis mulheres e cinco homens, cujo tempo de permanência no hospital variou de dez a trinta anos e preenchem os critérios: a) ser natural de Araras; b) familiar ter residência em Araras; c) possuir curador ou procurador e d) estar sob internação involuntária. A pergunta que orientou o estudo foi: Por quê pessoas que possuem renda e familiares no município, sob internação involuntária, permanecem institucionalizadas? As bases teóricas de referência sustentam-se na perspectiva da Desinstitucionalização e da Reforma Psiquiátrica. Os dados empíricos foram obtidos por meio dos Questionários preenchidos por pesquisadores do Censo Psicossocial, após autorização da Área Temática de Saúde Mental do Grupo Técnico de Ações Estratégicas da Secretaria de Estado de Saúde. Os resultados demonstram que os moradores e moradoras vivem amplo processo de institucionalização, despertecimento e apartamento social. As vidas são apropriadas pelo cotidiano e lógica asilar, os direitos fundamentais de cidadania são permanentemente violados: entre os que têm necessidades específicas, como deficiência física ou mental e entre os que têm e os que não têm diagnóstico psiquiátrico. Todos estão sob curatela e recebem renda administrada por terceiros, sem acesso ao dinheiro. Como resposta às necessidades, possibilidades e recursos dessas pessoas, esse estudo recomenda a execução imediata das Diretrizes das Políticas Públicas de Saúde Mental do Ministério da Saúde, das recomendações do Censo Psicossocial no Estado de São Paulo e do Plano Estadual de Saúde 2008-2011 da Secretaria de Saúde de São Paulo e desenha um Projeto de Desinstitucionalização (duas Residências Terapêuticas) para estas pessoas, atendendo suas características e as características do município de Araras. / This qualitative research took as its object of study the universe of Araras residents of the Antonio Luis Sayão psychiatric hospital, under state management, located in the city of Araras in the State of São Paulo. The purpose is to specify mechanisms for the initiation of processes of deinstitutionalization of these people. The objectives were: a) to identify, through the Psychosocial Census Questionnaire of Psychiatric Hospital Residents in the State of São Paulo, the residents born in the city and who live within the hospital; b) to investigate, through objective data and the researchers printing, who they are, their resources and capabilities; c) to propose from the data analysis some strategies of deinstitutionalization, which can be implemented by the local manager, if appropriate. Study subjects are six women and five men, whose time spent in the hospital has ranged from ten to thirty years and meet the following criteria: a) are a native of Araras; b) have a family residence in Araras; c) posses a trustee/proxy and d) is under involuntary admission. The question that guided the study was: Why do people with income and family, under involuntary admission, remain institutionalized? The theoretical bases of reference are maintained in the perspective of Deinstitutionalization and Psychiatric Reform. Empirical data were obtained through questionnaires filled out by Psychosocial Census researchers after authorization by the Secretary of the Ministry of Health\'s Technical Actions Group in the Thematic Area of Mental Health. The results show that the residents live vast institutionalization processes, desperation and social apartment. The lives are suitable for everyday living and logical placement, the fundamental rights of citizenship are constantly violated: between those who have special needs such as physical or mental disabilities and those who have and those who do not have psychiatric diagnoses. All are under the guidance of a trustee and receive income managed by third parties without access to the money. In response to the needs, possibilities and resources of these people, this study recommends the immediate implementation of the Ministry of Healths Public Policy Guidelines for Mental Health, the recommendations of the Psychosocial Census of the State of São Paulo and the São Paulos Health Secretary State Health Plan 2008-2011 and lays out a Deinstitutionalization Project (two therapeutic residences) for these people, given their characteristics and features of the city of Araras.

O sentido de ser internado em hospital psiquiátrico à luz da fenomenologia de Heidegger / Meaning of being hospitalized in psychiatric hospital by the Light of Heideggers phenomenology

O campo da Saúde Mental no Brasil tem se voltado para a construção de serviços comunitários de atenção psiquiátrica, pautados nos preceitos da Reforma Psiquiátrica, como possibilidade de substituir a lógica asilar pelo resgate das habilidades sóciorelacionais do doente mental, a partir da reabilitação psicossocial, sob a abordagem interdisciplinar. Entretanto, a experiência em psiquiatria mostra que o doente mental mantém-se transitando entre o serviço comunitário e o hospital psiquiátrico, sendo alvo, ainda hoje, da disciplinarização, violência e privação impostas pela instituição hospitalar. Neste sentido, constituiu objetivo deste estudo apreender o sentido de ser internado em hospital psiquiátrico, a partir do sujeito que vivenciou a experiência da internação. Para alcançar o objetivo proposto, o estudo recorreu ao referencial teóricofilosófico da ontologia fundamental de Martin Heidegger. Participaram da pesquisa quatro sujeitos portadores de transtornos mentais regularmente matriculados em um centro de atenção psicossocial no interior do estado de São Paulo, que aceitaram discorrer sobre a vivência da internação psiquiátrica, como se mostrou a eles, por meio de entrevista semi-dirigida gravada. Para tal, os sujeitos foram convidados a rememorar a experiência de ser internado em hospital psiquiátrico e tecer significações a respeito. A partir da apropriação de elementos do referencial heideggeriano, foi empreendida a Analítica Existencial, que gerou a construção dos Núcleos do Sentido: (a) A mostração do ser-aí no ser-doente-mental; (b) O modo de ser-no-mundo do ser-doente-mental e (c) Ser-no-mundo-cuidado na impessoalidade. Estes núcleos permitiram que a verdade do ser-doente-mental fosse desvelada a partir dele próprio, mediante o entendimento da estrutura do ser-aí (Dasein) proposta por Heidegger, mostrada pelo modo de ocupar-se da internação psiquiátrica. Ao emergir o ser-cuidado-no-mundo-com-o-outro, foi possível delinear o sentido de ser internado em hospital psiquiátrico, cerne da compreensão ontológica da vivência da internação psiquiátrica. / Brazilian Mental Health field has being conducted to building community psychiatric attention services, as possibility to substitute manicomial thinking for rescue of mental patients social-relational abilities, through psicossocial rehabilitee, by interdisciplinary approach. However, psychiatric experience show us that mental patient has kept himself passing through community services and psychiatric hospital, being marked, until today, by disciplinarization, violence and privation forced by hospitable institution. By this meaning, the studys objective was apprehending the meaning of being hospitalized at psychiatric hospital, passing through person that has lived the hospitalizations experience. To reach the objective proposed, the study made use of fundamental ontology of Martin Heidegger theory-philosophic referential. Took part in the search four people that have mental disease, regularly registered in a psicossocial attention service in interior of São Paulo, that accepted talk about psychiatric hospitalization live, like it showed to them, through a semi directed interview that was registered by recorder. For that, people were invited to remember the experience of being hospitalized at psychiatric hospital and weave meanings about that. Through elements by heideggerian referential appropriating, it was applied Existential Analytics, that generated the construction of Meanings Nucleus: (a) Being there showing through mental-patient-being; (b) being-in-world way of mental-patient-being, and (c) Beingcare- in-world by impersonality. These nucleuses revealed the truth of mental-patientbeing by himself, passing by the thinking about Being There structure proposed by Heidegger, that was showed in the way of devote oneself to psychiatric hospitalization. To emerge being-care-in-world-with-other, it was possible to define the meaning of being hospitalized at psychiatric hospital, focal point of ontological comprehension about psychiatric hospitalization live.

Quedas em pacientes psiquiátricos : uma revisão integrativa / Falls in psychiatric patients : an integrative review

As quedas são consideradas, mundialmente, um problema de saúde pública. É a segunda causa de mortes por lesões acidentais ou intencionais em todo o mundo, atrás apenas de acidentes de trânsito. As quedas também se tornaram preocupação para instituições hospitalares, sendo elas consideradas um indicador de qualidade da assistência. Entre as unidades hospitalares, destacam-se as unidades psiquiátricas com a maior prevalência do evento quedas, em função da vulnerabilidade dos pacientes em sofrimento psíquico. Frente a isso, o presente estudo objetiva caracterizar a produção científica nacional e internacional em saúde sobre o evento quedas em pacientes psiquiátricos adultos. Trata-se de uma revisão integrativa, que utilizou os pressupostos teórico-metodológicos de Whittemore (2005). Para coleta de dados foram utilizados os descritores acidentes por quedas, saúde mental, psiquiatria, enfermagem psiquiátrica e enfermagem, nas seguintes bases pesquisadas: Lilacs, MedLine, PubMed, Cinahl e Embase. Os critérios de inclusão dos artigos foram: estar indexados nas bases de dados, estudos direcionados a sujeitos adultos, nos idiomas português, inglês e espanhol, além de estarem disponíveis integralmente e de forma gratuita. Também se considerou ter resultados satisfatórios conforme aplicação do instrumento de controle de qualidade e níveis adequados na Classificação Hierárquica de Evidências, em níveis médio e forte. Totalizou-se uma amostra de 57 artigos. Para melhor compreensão, os resultados foram discutidos em três categorias, sendo elas: 1) Fatores de risco em pacientes psiquiátricos; 2) Mecanismos e instrumentos para avaliar as quedas em pacientes psiquiátricos e 3) Intervenções relacionadas à prevenção de quedas. Em um momento em que se discute a melhora das condições de segurança dos pacientes, trazer elementos para problematizar as quedas na unidade de internação psiquiátrica constitui-se um dos retornos necessários para promover a integração entre a pesquisa e a assistência, de forma a conduzir uma efetiva prática focada em evidências. / Globally, falls are a major public health problem. They are the second leading cause of accidental or unintentional injury deaths worldwide after road traffic injuries. Falls become a preoccupation for hospital institutions, since it is considered an indicator of quality of care. Among the hospital units, the psychiatric units have the highest prevalence of the event fall along with the vulnerability of the patients in psychological distress. Against this background the present study aims to character the national and international scientific production in health on the event falls in adult psychiatric patients. This is an integrative review with theoretical- methodological assumptions Whittemore's (2005). For data collection, the descriptors Accidental falls; Mental health; Psychiatric; Psychiatric nursing; Nursing in the following databases: Lilacs, MedLine, PubMed, Cinahl and Embase. The inclusion criteria of the articles were: to be indexed in the databases, studies directed to adult subjects, Portuguese, English and Spanish languages, to be available in free full-text articles. It was also considered to have satisfactory results according to the Hierarchical Classification of Evidence in Medium and Strong. It was totaled in 57 articles. For better understanding, the results were discussed in three subcategories: 1) Risk factors in psychiatric patients; 2) Mechanisms and instruments to evaluate falls in psychiatric patients; 3) Interventions related to falls prevention. While the improvement of the patient's safety conditions is debated, bringing elements to discuss the falls in the unit of psychiatric hospitalization is one of the necessary returns to promote the integration between the research and the assistance. In order to conduct an effective practice focused on evidence.

Bakgrund: Psykisk ohälsa är ett stigmatiserat men vanligt förekommande problem överallt i världen. Trots att social interaktion är en viktig del av omvårdnadsarbetet så tillbringar vårdpersonalen inom den psykiatriska slutenvården endast en liten del av sin tid med patienterna. Syfte: Att sammanställa vetenskapliga artiklar för att öka kunskapen om den sociala interaktionen mellan patienter och vårdgivare på psykiatriska slutenvårdsavdelningar. Metod: Litteraturstudie. 18 kvalitativa och kvantitativa artiklar granskades. Deras resultat syntetiserades med hjälp av quantitative metasummary. Resultat: Social interaktion kan förbättra patienternas situation på många sätt. Utebliven social interaktion har negativa effekter på deras sjukdomstillstånd. Tidsbrist och ett stigmatiserande klimat är faktorer som påverkar den sociala interaktionen negativt. Slutsats: Stigmatisering och nedsatt social interaktion på psykiatriska slutenvårdsavdelningar kan påverka och orsaka varandra. Detta belyser ytterligare vikten av social interaktion mellan patienter och personal. / Background: Mental illness is a common but still very stigmatized problem that can be seen all over the world. Even though social interaction is an important part of nursing, the staff on psychiatric wards only spend a fraction of their time with the patients. Aim: To compile scientific articles to increase the knowledge of the social interaction between patients and staff on psychiatric wards. Method: Review. 18 qualitative and quantitative articles were reviewed. Their findings were synthesized using qualitative metasummary. Findings: Social interaction can improve the patients’ situation in several ways. Lack of social interaction negatively affects their state of illness. Time constraints and a stigmatizing environment are factors that have a negative effect on the social interaction. Conclusion: Stigmatization can reduce the social interaction on psychiatric wards, and reduced social interaction can cause stigmatization. This illuminates the importance of increasing the social interaction between patients and staff.

Factors influencing nursing staff morale in S'brana Psychiatric Hospital in Lobatse- Botswana

Mental health nurses have been considered professionals that assist people to regain a sense of coherence over what is occurring to them - be it a result of trauma or some other form of mental distress. Good morale among staff on inpatient psychiatric wards is an important requirement for the maintenance of strong therapeutic alliances and positive patient experiences, and for the successful implementation of initiatives to improve service delivery. The aim of the study was to determine the factors influencing nursing staff morale at S’brana Psychiatric Hospital (SPH) in Botswana. Methods: A quantitative, cross sectional study was conducted on 147 respondents. Self-administered questionnaire was used for data collection and it was closed ended. Data was collected at S’brana Psychiatric Hospital (SPH) and stratified random sampling technique was used to select nurses according to their categories. Data were analyzed through SPSS Software v21.0. Results: The results revealed that the majority of the nurses were aged between 26 - 30 years and most of them were females compared to males. It is evident from the results that, there was strong association between number of years and nursing position (χ2(15) = 72.34, p = .000). Also, there was positive correlation between respondents highest qualification in nursing and training received to do the job well (r = .312, n = 147 and p = .000.). Multiple regression analysis showed a statistically significant, F(3, 143) = 46.69, p = .000, and accounted for 70.3% of the variance. Conclusion: This study has revealed that nurses’ morale in SPH can be affected by a number of factors irrespective of their age, work experience, nursing position and ward they work in. The consequences of low staff morale are detrimental to health professionals (nurses) and patients, therefore, it is important for healthcare managers to address the shortcomings in order to counteract the negative effects of low staff morale

Personal and professional spirituality: Muslim social workers' perspectives

Research in the area of religion and spirituality in social work practice is lacking minority practitioner representation. This phenomenological study explored the questions of how American Muslim social workers define and experience the religious/spiritual, and perceive the propriety of integrating either, in practice. Ibn Khaldun and Durkheim, early social theorists, suggested that group feeling and affiliation impact personal and professional perception and decision making. American societal views of Muslims are often negative and uninformed. A group of 15 Muslim practitioners with bachelor's or more advanced degrees in social work were recruited through a survey administered via an Internet survey site. In depth telephone interviews were conducted that clarified personal and professional descriptions and experiences of the religious/spiritual. Transcript statements were critically reviewed for range of meaning (horizonalization) and reduced to their thematic essences following the phenomenological thematic analysis paradigm. Trustworthiness of the study was verified through ongoing bracketing of the researcher's assumptions and maintenance of a data collection journal. Findings indicated that participants favored a client-centered approach based on the social work standard of self determination. Most participants differentiated and acknowledged the value of spirituality more than religion in practice. Practitioners, cognizant of possible negative interpretations of Muslims and Islam, rely on the client to initiate religious themes in therapy. This finding suggests the need for future study of client views. Implications for social change are evidenced in the clients' assurance that Muslim practitioners' professional integration of the religious/spiritual is client driven and bound by competent social work ethical practice.

Educating psychiatric nurses to improve medication adherence among schizophrenic patients

The purpose of this project was to implement an educational program that provided psychiatric nurses with the knowledge and skills to engage patients with a diagnosis of schizophrenia in self-care management regarding medication and treatment adherence. The theory employed in the project was the educational theory. Evidence of effectiveness came from a pre- and postintervention assessment of nurses' knowledge and confidence in teaching about medication, along with a pre- and postassessment of knowledge gained as a result of the educational intervention. Pre- and posteducational rates of patient rehospitalization for medication nonadherence were compared. The results indicated a 15% rate of readmission of schizophrenia patients prior to the educational sessions. The 30-day readmission rate decreased to approximately 5% after the educational sessions. Based on these results, it was concluded that the educational project intervention had a positive impact on improving knowledge and insight of the nurses about nonadherence to medication among patients with schizophrenia. The positive social impact of improving nurses' knowledge of educating schizophrenic patients are increased patient well-being and reduced social costs associated with relapse and readmission. Recommendations from this project include that nurses should educate patients with schizophrenia on how to adhere to medication directives and the importance of doing so.

Analysis of Variance in Recidivism between Special Needs Offenders and Regular Offender Populations in Texas

A Specialized or Super Intensive-1 (SI-1) supervision level refers to a contact requirement imposed on special needs offenders (SNOs) under Texas parole supervision. SI-1 supervision requires greater contact with parole officers and treatment providers than supervision levels used on regular offenders (ROs), yet little is known about whether SI-1 supervision offenders violate terms of their parole or commit new crimes at a different rate compared to the regular offender population in the State of Texas. Reconstruction theory and the social construction of reality were used as theoretical underpinnings of this study, which examined whether differences in offenders' supervision levels created statistically significant differences in technical or new law violations in Texas parole hearings. A random sample of 200 SNOs and ROs data were analyzed using a 2-way ANOVA. Results indicated a positive and statistically significant difference between level of supervision and technical violations, with SI-1 offenders committing a greater number of violations of non-criminal terms of parole, but with SI-1 offenders being less likely than the regular offender population to commit new crimes. These findings challenge the social construction that SI-1 offenders introduce a higher element of risk to the community regarding new criminal activity. The positive social change implications of the study include policy recommendations to the Texas legislature and Texas Department of Criminal Justice to refocus resources on improving outcomes related to technical parole violations, including a reduction in SNOs' contact standards, which in turn, promote fiscal responsibility and improvements in public safety for the people of the state of Texas.
