Zhodnocení využití GUSS testu u pacientů s poruchou polykání po cévní mozkové příhodě / Assessing the use GUSS test in patients with dysphagia after stroke

(v AJ) Nursing care for patients with dysphagia after stroke is nowadays a very serious problem. This disease is particularly dangerous for patients because of the risk of imminent aspiration pneumonia. In 2015, Czech Republic has developed a standard treatment plan for patients with dysphagia after stroke and following this plan a new screening method was put into practice, which would prevent aspiration pneumonia (Salt, 2015). This method is called THE GUGGING SWALLOWING SCREEN - GUSS test. After a positive GUSS test result a standardized nursing care should follow, which would involve the whole medical team (doctors, health nurses, speech therapists, physiotherapists and orderly). The aim of this work is to verify whether the GUSS test and subsequently the determined process of the standard treatment together with the nursing care meets and the needs of the nurses. Another objective is to verify whether the nurses would want to add, innovate or make necessary updates based on the already acquired experiences. Furthermore, taking into consideration the frequent collaboration of nurses with the speech therapists, this study also aims to evaluate and asses the cooperation between nurses and the speech therapists. The diploma thesis is oriented on special education. The main goal is to find out and describe whether and how is speech and language therapy established in regular kindergartens at the foothills of Orlické mountains. The work is divided into five chapters, four of them are theoretical and the fifth chapter is a practical one. The first chapter is focused on the way how the preschool education is organised in the Czech Republic. It describes the legislation system, clarifies the system of curricular documents established in our republic and mainly the framework educational program for preschool education. The second chapter describes the preschool-child ontogenesis including all aspects of his/her personality and it also depicts the child-speech ontogenesis. The third chapter is focused on the main types of impaired communication ability of preschool children. The fourth chapter is focused on the speech therapy intervention provided in the Czech Republic. The fifth chapter presents combined research, within which the qualitative method of dialogue and the quantitative method of questionnaire were used, together with non-structured observation and comparative analysis among kindergartens. Speciálně pedagogická intervence u dětí s vývojovou dysfázií v mateřské škole / Special Educational Intervention in Children with Specific Language Impairment in the Kindergarten

This Masters thesis focuses on issues of Specific Language Impairment - SLI of children of pre-school age. It monitors and evaluates development of their communication and motor skills during their time in kindergarten. The theoretical part focuses on defining basic terms which relate to communication and communication disorder with the emphasis on SLI. The psychomotor and communicational development of child from birth to entering school is described too. Also how this all affected creating the Framework Educational Programme for preschool education. The aim of the empirical part was to observe four children with SLI during their attendance at a pre-school for children with specific needs. Speech and psychomotor development of these children and its changes in connection to their teachers' intervention was observed. Part of the research included interviews with children's parents and those are also evaluated. The survey was done by the qualitative method. We get four case histories. At the end was evaluated the aims of the survey and answered questions asked and summarized the results of my investigation.

The theoretical part of the diploma thesis is characterized by the history of logopaedics in the Czech Republic, the distribution of logopedic intervention and the organization of logopedic workers. The main subject of the thesis is to describe the requirements for the education and professional development of a teacher providing logopedic care both in the education sector and in the private sphere. It is also mentioned the division of logopedic workplaces according to individual spheres of education. The aim of the practical part is to map logopedic care in the South Bohemian Region. There is also a comparison of the current school situation. Further, the practical part is focused on workplaces where logopedic care is performed on children with cochlear implant. The diploma thesis is focused on the area of special education of people with hearing impairment, that is the second sub-focus of the speech therapist.

The lived experiences and support needs of a mainstream high school learner with a speech-flow difficulty

Thesis (MEd)--Stellenbosch University, 2014. / ENGLISH ABSTRACT: The aim of this study was to better understand the lived experiences of a learner in a mainstream high school, who is experiencing a speech-flow difficulty, in order to gain insight into how best to support such a learner within an inclusive classroom. The theoretical framework on which this study was based is the bio-ecological model, inclusive education, positive psychology, resilience as well as developmental psychology, specifically the developmental phase of adolescence. The purpose of this was to view the learner holistically, taking into account all of the positive support structures in her life, especially, her levels of resilience, whilst taking cognisance of the fact that she is in the adolescent developmental phase, within a mainstream high school. Inclusive classrooms should ideally be structured in such a way that they accommodate a learner’s specific individual learning needs. The methodology employed in this study was based on a basic qualitative research approach, and viewed through an interpretive paradigmatic lens. Purposive sampling was used to select a learner who was experiencing a speech-flow difficulty. Various methods of data collection were employed, such as: a semi-structured interview with the learner and her mother, diary entries from the learner, a timeline of the learner’s life drawn by the learner herself, the researcher’s observations (both in the classroom and during a break time), as well as the researcher’s own reflective notes. Documents were also made available to the researcher, and these were the learner’s school reports, the learner’s speech therapy workbook, as well as a report on the learner from her speech therapist. This data was analysed through a qualitative coding process. The research findings indicated that the learner had various experiences, both positive and negative, within all spheres of her life. The majority of her experiences were positive, as the learner had strong support structures in her life, in the form of her mother, her friends, her sound academic capabilities, as well as her level of resilience. The learner is also currently receiving positive intervention in the form of speech therapy, where she is learning various strategies in order to assist her with her speech-flow difficulty. However, there are few factors that make the learner feel uncomfortable, especially in the classroom. The researcher has thus recommended ways in which educators can better support learners who are experiencing speech-flow difficulties within their classroom. / AFRIKAANSE OPSOMMING: Die doel van hierdie studie was om die geleefde ervaringe van ʼn leerder in ʼn hoofstroomskool, en wat ʼn spraakvloeiversteuring ervaar, te probeer verstaan. Die doel hiermee was om insig te verkry in hoe so ʼn leerder ondersteun kan word binne ʼn inklusiewe klaskamer. Die teoretiese raamwerk waarop hierdie studie berus is die bio-ekologiese model, inklusiewe onderwys, positiewe sielkunde, veerkragtigheid, sowel as ontwikkelingsielkunde, spesifiek die adolessente ontwikkelingsfase. Die mikpunt was om die leerder holisties te beskou, deur al die positiewe ondersteuningstrukture in haar lewe in ag te neem veral haar vlakke van veerkragtigheid, terwyl die feit dat sy haar in die adolessente ontwikkelingsfase bevind, en in ʼn hoofstroomskool is, verder lig op haar ervaringe kan werp. Inklusiewe klaskamers behoort dus in so ʼn mate gestruktureer te wees dat individuele behoeftes van leerders in ag geneem word. Die navorsingsmetodologie in hierdie studie het berus op ʼn basiese kwalitatiewe benadering, en beskou deur ʼn interpretatiewe paradigmatiese lens. ʼn Doelgerigte steekproef is gebruik om ʼn leerder te identifiseer wat ʼn spraakvloeiversteuring ervaar. Verskeie metodes van data-insameling is gebruik, byvoorbeeld semi-gestruktureerde onderhoude met die leerder en haar moeder, dagboekinskrywings van die leerder, ʼn tydlynoefening wat die leerder van haarself geteken het, die navorser se waarnemings (binne die klaskamer sowel as op die speelgrond), asook die navorser se reflektiewe notas gedurende die proses afgeneem. Dokumente is beskikbaar gestel aan die navorser, naamlik die leerder se skoolrapporte, haar werkboek wat sy gedurende spraakterapie sessies gebruik, sowel as ʼn verslag deur die spraakterapeut wat die deelnemer tans konsulteer. Die data is geanaliseer met behulp van ʼn kwalitatiewe koderingsproses. Die navorsingsbevindinge dui ʼn verskeidenheid van ervarings (positief sowel as negatief) aan wat die leerder binne al die areas van haar lewe ondervind. Die meerderheid van haar ervaringe is positief, aangesien sy sterk ondersteuningsstrukture in haar lewe het, veral haar moeder, haar vriende, haar sterk akademiese vermoeëns sowel as haar veerkragtigheid. Die leerder ontvang ook tans ʼn baie positiewe intervensie van ʼn spraakterapeut, waar sy ʼn verskeidenheid strategieë aanleer, wat haar help om die spraakvloeiversteuring mee te hanteer. Daar is egter ʼn paar faktore wat haar ongemaklik maak, veral in die klaskamer. Die navorser kan dus ‘n verskeidenheid van riglyne aanbeveel waardeur leerders met spraakvloeiversteurings ondersteun kan word binne die klaskamer.

This thesis deals with children with impaired communication ability who are educated in regular kindergarden we use preschool also. The implementation of the research was selected as the research sample of 5 children with impaired communication abilities, specifically impaired developmental dysphasia. The theoretical part deals with the classification of impaired communication skills and the definition of developmental dysphasia. Comprehensively deals with the integration and education of children with ICA in regular kindergardens, preschool teacher quality and its role in the development of a child with ICA, focused on the development of children with developmental dysphasia. It also monitors the cooperation of kindergarden, parents and experts in the form of a speech therapist, workers SPC and PPC workers. The purpose of the research is based on interviews with parents, teachers, teacher assistants and direct short-term observation of the research sample in the form of 5 children with developmental dysphasia, who are educated in regular kindergarden to explore the learning process of a child with developmental dysphasia in regular kindergarden and map risks and benefits affecting his education.

The topic of this diploma thesis is speech therapy provision for seniors at residential care. The thesis consists of four chapters, three of which are theoretical, the fourth chapter is practical. The first chapter describes the old age, it presents basic terminology and it characterises the old age from the point of view of physical, mental and social development. The second chapter is concentrated on the issue of neurological speech and language disorders, their etiology, symptomatology, diagnostics and therapy. The third theoretical chapter describes the system of health and social care of seniors in the Czech Republic with an accent on the system of speech therapy intervention within the resorts of health care and social care. The fourth chapter is the research one. It is developed as a quantitative descriptive research, using the technique of a questionnaire sent to nursing homes for seniors in the Czech Republic. This activity was concepted by the researcher and a speech therapist, offered during a week, for a discussion group in which, the speech therapist was responsible to conduct the discussion between parents of deaf children, whom are attended by Derdic (Divisão de Educação e Reabilitação dos Distúrbios da Comunicação) at the Pontifícia Universidade Católica de São Paulo (PUC-SP). The date was composed with an oral text produced during the application of an adaptation to the simple auto-confrontation method (Faïta, 1997), in which there were three steps: a) audio and video records of three meetings between these parents and the speech therapist. The film Mr. Holland-adorável professor, was showed for the parents, in the first meeting, to be discussed in the two others meetings; b) the edition of this recorder was made by the researched; c) an presentation of this edition to the speech therapist and this part of the research is the analyses material. The analyses are based on conceptual enunciative-discourse, specially, the conceptions by studies of Bakhtin/Volochinov (1929/1992), Bakhtin (1984/1992) and Maingueneau (1998/2004), and this conceptions are articulated with the knowledge of Ergologia (Schwartz, 1996, 1998, 2000, 2004) and Clínica da Atividade (Clot et al. 2000, 2001; Faïta, 1997, 2002, 2004, 2005). So, the reported speech is analyzed with the reflections about the uso das pessoas conception (Benveniste, 1966/2005), specially, the use of personal pronouns and the references. To the organization of the results, there are two topics: first, the words that meaning is related with the difficulties of the work with parents and, in the other side, the importance of this work which is related with the information to parents about the deaf and the importance of the speech therapist action in giving attention to the parents. The results showed, from the Language Sciences and Work Sciences notions, that, there is an activity genre of the speech therapy work with parents. So, from the dialogue between scientific publications about these issues and the results of this research, it was possible to do reflections about the issues that the speech therapist emphasized during the application of auto-confrontation method / Esta investigação tem por objetivo refletir sobre a atividade de trabalho do fonoaudiólogo com pais e mães de crianças com perda auditiva. A atividade, elaborada pela fonoaudióloga/pesquisadora e por uma fonoaudióloga/protagonista do trabalho, envolveu um grupo de discussão formado por pais de crianças atendidas na Derdic (Divisão de Educação e Reabilitação dos Distúrbios da Comunicação) da PUC-SP. Para a coleta de dados, empregou-se uma adaptação do método da autoconfrontação simples (Faïta, 1997), que constou de três etapas: a) gravação em áudio e vídeo de três encontros entre os pais e a protagonista; no primeiro encontro, os pais escolheram para discussão o filme Mr. Holland adorável professor que foi discutido nos outros dois encontros; b) edição dos encontros gravados, em segmentos selecionados pela pesquisadora; c) apresentação do vídeo editado à protagonista; o material de análise consistiu no discurso produzido pela protagonista, ao ser confrontada com esse vídeo. O quadro teórico para a análise é de base enunciativo-discursiva, mais especificamente, dos estudos de Bakhtin/Volochinov (1929/1992), Bakhtin (1984/1992) e Maingueneau (1998/2004) na articulação com os estudos sobre o trabalho, mais especificamente, a Ergologia (Schwartz, 1996, 1998, 2000, 2004) e a Psicologia do Trabalho, na vertente desenvolvida pela Clínica da Atividade (Clot et al., 2000, 2001; Faïta, 1997, 2002, 2004, 2005). A análise se dá na articulação entre as categorias lingüísticas tema, discurso citado e embreagem enunciativa e as noções de normas antecedentes, renormalizações, usos de si, gênero e estilo da atividade, advindas de estudo sobre o trabalho. A reflexão possibilitou chegar a dois temas sobre o trabalho do fonoaudiólogo com pais de crianças com perda auditiva: de um lado, a complexidade desse trabalho revelada por: (a) esforço em envolver os pais, visto que há uma maior participação das mães, (b) dificuldade de se falar sobre o prognóstico e/ou o diagnóstico, e (c) tensão entre a informação e a discussão no grupo. Por outro lado, destacam-se as questões levantadas sobre a importância do trabalho do fonoaudiólogo, como: (a) a importância do esclarecimento aos pais sobre a perda auditiva, (b) a importância da explicação do Programa de Inclusão Escolar e (c) a importância do acolhimento aos pais. A partir dessa análise pôde-se constatar que esses traços compõem um gênero de atividade do trabalho do fonoaudiólogo com pais de crianças com perda de audição

Alla barn i förskolan har en rättighet att få sin röst hörd men denna rättighet är inte i allasituationer vare sig självklar eller enkel att iscensätta. Styrdokument för förskolan lyfter alladenna rättighet men i verksamheten visar sig grupper av barn där denna rättighet är svår attuppfylla. En av dessa grupper är barn i ofrivillig tystnad och det är den grupp barn som dettaarbete kommer att fokusera på.Syftet för denna studie är att inom förskolans kontext undersöka, kartlägga och synliggöra denkompetens yrkesverksamma inom förskolans verksamhet besitter gällande barn i ofrivilligtystnad samt vilka arbetssätt de nyttjar kring barn i ofrivillig tystnad för att gynna derasdelaktighet. Detta syfte ämnade vi att uppnå genom en studie av jämförande karaktär medsemistrukturerade intervjuer som metod för datainsamling. Vi har i denna studie genomfört sexstycken intervjuer med tre olika yrkesgrupper, förskollärare, specialpedagoger och logopeder.Studiens resultat belyser att det finns både kompetens och etablerade arbetssätt i förskolansverksamhet för att stötta barn i ofrivillig tystnad. Resultatet belyser dock även att det hospedagoger råder en osäkerhet kring begreppsförståelse samt om dessa arbetssätt är tillräckliga.De yrkesverksamma lyfter även fram en oro över faktumet att barn i ofrivillig tystnad inte alltidfår det stöd de behöver eller att detta stöd blir fördröjt på grund av att dessa barn beskrivs somblyga barn istället för barn i ofrivillig tystnad. Denna studies resultat visar på förekomsten avdetta, både på grund av kompetensbrist och ett sätt att bortförklara ett barns tystnad, och lyfterfram vikten av att begreppsförståelsen och kompetensen för indikatorer för ofrivillig tystnadbehöver stärkas inom förskolans verksamhet för att barn i ofrivillig tystnad ska möjliggörastillgång till det stöd de behöver och har rätt till. För varje barn har en rättighet att göra sin röst hörd. / To have their voice heard is a right that every child in preschool has but this fundamental rightis not always apparent or easy to realize in every situation or context. Regulatory documentsfor preschool all highlight this right but the preschools establishment however shows a numberof groups of children where this right proves difficult to fulfill. One of these groups is childrenin involuntary silence and it is this group of children this paper will be focusing on.The purpose for this study is to within the preschool context research, map out, and visualizethe competence witch staff within preschool establishments have regarding children ininvoluntary silence as well as the methodology which are being applied with children ininvoluntary silence to benefit their level of participation. This purpose we aim to achieve by themeans of a comparative study with semistructured interviews as its method of data collection.In this study we have done six interviews with three occupational groups, preschool teachers,special educators and speech therapists.The results of this study show that within preschool establishments both competence and wellestablished methodology exist to support children in involuntary silence. However the resultsalso show an insecurity among educators regarding terms and whether or not their methodologyis sufficient. The staff also emphasizes their worry over the fact that children in involuntarysilence far too often don’t receive the support they need or that this support is delayed due toinstances where these children are described as shy instead of as children in involuntarysilence. This study shows the occurrence of this both due to a lack of competence and as a wayto appropriate a child’s silence and thereby raises the importance of a strengthened knowledgeof terms and indicators for involuntary silence in preschool establishments to enable access tothe support these children need and have a fundamental right to. Because every child has a rightto make their voice heard.

Icke-verbal kommunikation i flerspråkiga utbildningsverksamheter för yngre barn : Ur en specialpedagogs och en logopeds synvinkel

Icke-verbal kommunikation är ett viktigt komplement till det verbala språket, inte främst i utbildningsverksamheter med yngre barn. Syftet med studien är att undersöka en logopeds och en specialpedagogs syn på hur och om icke-verbal kommunikation används medvetet som ett pedagogiskt verktyg i flerspråkiga verksamheter för yngre barn och varför det används. För att ta reda på det har vi genomfört kvalitativa semistrukturerade intervjuer med en logoped och en specialpedagog samt utgått från John Deweys pragmatiska perspektiv. Genom arbetet har vi kommit fram till att icke-verbal kommunikation är av vikt för att förstärka och förtydliga det verbala språket och att icke-verbal kommunikation är bra för alla men nödvändigt för vissa. Det är upp till pedagogerna att ha den kunskapen för att använda det med ett tydligt syfte för det individuella barnets skull. Slutsatserna som dras i studien visar bland annat att gester, kroppsspråk och olika former av material används som medveten icke-verbal kommunikation i flerspråkiga verksamheter och detta används i situationer under hela dagen i framför allt samspelssituationer, i interaktion, i situationer där något behöver förtydligas samt när någon kommer till en ny språkig miljö. Detta i syfte att skapa förståelse, göra sig förstådd, förtydliga, kommunicera, uttrycka kommunikation på ett annat sätt och för att fungera språkfrämjande. / Non-verbal communication is an important complement to the verbal language in activities with younger children. The purpose of the study is to investigate the views of a speech therapist and a special education teacher on how and if non-verbal communication is used deliberately as a pedagogical tool in multilingual activities for younger children and why it´s used. To find out, we conducted qualitative semi-structured interviews and based our work on John Deweys pragmatic perspective. We have come to the conclusion that non-verbal communication is important to reinforce and clarify the verbal language and that it´s good for everyone but necessary for some. It´s up to the educators to have knowledge, in order to use it with a clear purpose for the individual child´s sake. Some of the conclusions drawn in the study show that gestures, body language and different types of materials are used as conscious non-verbal communication in multilingual activities and is used in situations throughout the day in mainly interaction situations, in interaction, in situations where something needs to be clarified and when someone comes to a new linguistic environment. This is for the purpose of creating understanding, making oneself understood, clarifying, communicating, expressing communication in a different way and to function as a language promoter.
