A dor lombar crônica é um importante problema de Saúde Pública. Profissionais de saúde que trabalham com doenças crônicas com frequência se deparam com as dificuldades que alguns pacientes apresentam em aderir aos tratamentos prescritos. Analisamos a adesão ao tratamento de pacientes com dor lombar crônica, em uma clínica de neurocirurgia do interior paulista e implementamos um guia de orientação que facilite a adesão ao tratamento da dor lombar crônica desses pacientes, demanda discutida com os gestores da clínica. A adesão terapêutica foi o referencial desse estudo. Utilizamos um método transversal e descritivo com abordagem qualitativa, e instrumento de coleta de dados, entrevista semiestruturada e fonte documental. Foram considerados sujeitos do estudo, todos os pacientes que procuraram atendimento na referida clínica de neurocirurgia com patologias da coluna vertebral, no período de seis meses (Fevereiro à Agosto) referente ao ano de 2017 com idade superior a 18 anos. Foram incluídos pacientes com diagnóstico médico de dor lombar crônica específica e inespecífica há pelo menos seis meses, com capacidade de expressão e comunicação preservados. A entrevista semiestruturada, abordou a compreensão dos pacientes quanto ao diagnóstico médico, ao tratamento e uma pergunta aberta para expressarem a percepção sobre a dor lombar no seu cotidiano. Para a análise dos dados utilizamos a Análise de Conteúdo. Na caracterização dos sujeitos, encontramos 57,2% do sexo feminino, a faixa etária predominante 62% foi entre 40 a 59 anos. Estado civil, 66,7% casados, escolaridade 47,6% ensino superior completo, cor da pele auto referida, branca 90,5%. Quanto ao IMC prevaleceu sobrepeso com 42,9%, profissão 33,3% eram do lar, ocupação 38% estavam ativos e quanto ao tempo de dor, de 1 a 5 anos, 62% e 76,2% dos participantes do estudo não faziam atividade física. Consideramos que analisar o conceito de adesão e seus fatores multidimensionais facilitou compreender as diferentes formas de lidar com a DLC. Destacamos as redes de apoio (trabalho, família e amigos), o bom relacionamento entre o profissional de saúde e o paciente promovendo uma compreensão sobre seu diagnóstico e tratamento. Importante promover o acolhimento, escuta qualificada para que o paciente se torne o sujeito ativo do seu tratamento e reabilitação. Para isto, a Integralidade do cuidado depende de um novo arranjo de práticas e conformações dos serviços de saúde que incluam a singularidade dos sujeitos. O estudo inicialmente pactuado e os resultados serão devolvidos para os gestores do local de estudo / Chronic low back pain is an important Public Health problem. We have Analyzed the adherence to the treatment of patients with chronic low back pain in a neurosurgery clinic in a city in the countryside of São Paulo state and developed a guideline that facilitates adherence to the treatment of chronic low back pain in these patients, a demand discussed with the clinic managers. Therapeutic adherence was the referential of this study. We used a transversal and descriptive method with qualitative approach, data collection instrument, semi-structured interview, and documentary source. The study subjects were all patients who sought care in the referred neurosurgery clinic with spinal pathologies, in the six-month from February to August, referring to the year 2017, aged over 18 years. We included patients with a specific and non-specific chronic low back pain medical diagnosis for at least six months, with preserved expression and communication capacity. The semi-structured interview addressed the patients\' understanding of the medical diagnosis, the treatment and an open question to express the perception about low back pain in their daily life. The data were analysed using a content analysis method. In the characterization of the subjects, we found 57.2% of female patients; the predominant age group representing 62% was between 40 and 59 years old; 66.7% of patients were married; 47.6% had higher education; regarding skin color, 90.5% of the patients reported themselves as white; as for BMI, overweight was predominant with 42.9%, on occupation 33.3% developed home activities; 38% of the patients were active and 62% of them stated to have had pain for 1 to 5 years, and 76.2% of study participants did not practice any physical activity. We consider that analyzing the concept of adherence and its multidimensional factors makes it easier to understand the different ways of dealing with chronic low back pain. We highlight the support networks (work, family and friends), the good relationship between the health professional and the patient, promoting a good understanding of their diagnosis and treatment. To promote the acceptance, the extended listening so that the patient becomes the active subject of its treatment and rehabilitation. For this, the integrality of the care depends on a new arrangement of practices and conformations of the health services that include the singularity of the subjects. The study initially agreed and the results will be returned to the managers of the study site

Observance thérapeutique et défaut d'observance thérapeutique en contraception orale / Therapeutic adherence and therapeutic defect of adherence in oral contraceptionHamraoui, Mounia 16 December 2011 (has links)
Objectifs et hypothèses :Cette thèse a pour objectif d'explorer les causes de la non observance en contraception orale qui se traduit par un nombre important d'IVG chez des femmes pourtant sous contraception. Nous avons formulé 3 hypothèses : il existe des différences significatives entre les femmes présentant des difficultés d'adhésion thérapeutique et celles qui n'en présentent pas. La 2ème hypothèse a trait à l'existence de relations significatives entre les causes de non observance et la motivation générale. La 3ème hypothèse énonce l'existence de différences significatives entre les moyennes des deux groupes concernant les causes de non observance, la motivation générale et ses différentes dimensions. Population : Elle est composée de 50 femmes âgées de 20 à 55 ans utilisatrices de la contraception orale, (soit deux groupes : 25 femmes présentant des difficultés d'observance et 25 femmes ne présentant pas des difficultés d'observance). Instruments : Deux questionnaires ont été mis au point et utilisés : 1) Un questionnaire sur l'observance en contraception orale et une échelle d'évaluation des causes de non observance en contraception orale. 2) Une échelle de Motivation Globale EMG-28 destinée à l’évaluation du degré de motivation à l'observance de la contraception orale en examinant les degrés d'auto-détermination de la motivation dans la vie en général. Résultats : Les résultats indiquent l'existence de causes de non observance liées à l'utilisatrice (oublis, fausses croyances, peur des effets indésirables, manque d'information), à la contraception orale (routine, lassitude, contrainte de la prise journalière) et à la relation entre le prescripteur et l'utilisatrice (qualité de l'information donnée, relation thérapeutique, écoute, logique de prescription). Conclusion : Le processus d'utilisation de la contraception orale se révèle très complexe. Une prise en charge motivationnelle s'avère nécessaire chez les femmes en situation d'échec de contraception, comme nous le montrons avec les deux cas cliniques présentés. / Objectives and assumptions : This thesis aims explore the causes of non adherence in oral contraception which lead to TOP among women however under contraception. We formulated 3 assumptions: there are significant differences between the women presenting adherence problems and those without problems. The 2nd assumption supposes the existence of significant relations between the causes of non adherence and the general motivation. The 3rd assumption states the existence of significant differences between the averages of the two groups concerning the causes in non adherence, the general motivation and its various components. Population : A survey was carried out based on a sample of 50 women users of oral contraception in Amiens, including a group of 25 women which exhibit adherence problems. Tools : Two questionnaires were developed within the framework of this thesis, 1) A questionnaire on adherence in oral contraception and a scale for the evaluation of the causes of non adherence in oral contraception. 2) A Scale of Global Motivation EMG28 intended to evaluate the degree of motivation for adherence to oral contraception by examining the self-determination degree of motivation in life in general. Results : Collected data confirm the assumptions posed. It reveals existence of causes of non observance related on the user (missing pills, beliefs, fear of undesirable effects, information lack, on the oral contraception (routine, lassitude, constraints …) and the relation between the prescriber and the user (therapeutic relation, quality of information given, model of prescription). Conclusion : The process of use of oral contraception appears very complex. A motivational therapy is necessary among women in situation of failure of contraception, as we show it with clinical cases presented.

Patienten- und Therapeutenmerkmale wurden mittels Interview und Selbstbericht erhoben.
Ergebnisse. Soziodemografische Merkmale wie ein geringeres Bildungsniveau der Patienten und weibliches Geschlecht der Therapeuten wurden als signifikante Prädiktoren einer höheren therapeutischen Adhärenz identifiziert. Störungsspezifische Merkmale der Patienten waren nicht mit der therapeutischen Adhärenz assoziiert. Therapeutenmerkmale wie ein geringerer Ausbildungsgrad, eine geringere erlebte therapeutische Kompetenz und höhere Erwartungen sowie ein höheres emotionales Wohlbefinden der Therapeuten sagten eine höhere therapeutische Adhärenz vorher.
Diskussion. Die etablierte hohe therapeutische Adhärenz erschien unabhängig vom Patienten, während einige Therapeutenmerkmale als Prädiktoren identifiziert wurden. Thérapies ciblées chez les femmes atteintes de cancer du sein métastatique : diffusion, impact sur la prise en charge, poids des représentations sociales et adhésion thérapeutique / Targeted Therapies for Women with Metastatic Breast Cancer : Diffusion, Impact on Management, Influence of Social Representations and Adherence.Martin, Elise 18 December 2017 (has links)
En France, le cancer du sein est le cancer le plus fréquent chez les femmes et la deuxième cause de décès par cancer. Si la mortalité relative du cancer du sein a diminué ces dernières années, de nombreux progrès restent à faire pour les 20 à 25 % de patientes qui évoluent vers un stade métastatique. Cette étude s’appuie sur la réalisation de 103 entretiens semi-directifs réalisés auprès d’acteurs de la prise en charge en oncologie et de patientes atteintes de cancer du sein métastatique. L’objectif de ce travail de thèse était d’étudier : l’impact des nouvelles thérapies ciblées sur l’organisation de la prise en charge et leur diffusion, le vécu des patientes avec les traitements et leur adhésion thérapeutique. Nous avons montré, dans un premier temps, que les nouvelles thérapies ciblées orales modifient la prise en charge traditionnelle des patientes qui restent moins dans la sphère d’influence de la structure de soins et qu’il existe de nombreux freins à la diffusion de ces traitements chez les oncologues. Dans un deuxième temps, nous avons mis en avant les représentations positives des patientes sur les thérapies ciblées orales qui sont sources d’espoir et améliorent la qualité de vie. Pour autant, le vécu au quotidien vient parfois nuancer ces représentations et peut révéler des freins à l’adhésion thérapeutique tels que l’apparition d’effets secondaires ou les responsabilités engendrées par la demande d’autonomie. Enfin, dans un troisième temps, nous avons mis en exergue le manque de connaissances des patientes sur les innovations thérapeutiques et l’importance de la décision partagée avec l’oncologue tout particulièrement face à la chronicisation progressive de certains cancers du sein métastatiques. / In France, breast cancer is the most common cancer amongst women and the second cause of cancer death. Although breast cancer relative mortality has decreased significantly in recent years, there is still a lot to be done for the remaining 20% to 25% of patients who have metastasized. This study is based on 103 semi-structured interviews carried out with oncology healthcare professionals and metastatic breast cancer patients. The aim of this work was to study the impact that new targeted therapies can have on managing the patient’s care and their diffusion, the patients’ experiences with these treatments, their social representations and their therapeutic adherence. First, we showed that new oral targeted therapies disrupt the traditional steps in a patient care. Patients arefar less in the hospital’s sphere of influence and there are many barriers to the dissemination of these treatments for oncologists. Second, we highlighted the positive representations of oral targeted therapies that patients have, which is bringing them hope and improving their quality of life. However, the day-to-day experience with the treatment sometimes moderates these representations and may reveal obstacles to their therapeutic adherence such as side effects or the responsibilities created by the demand for autonomy. Finally, we showed the lack of patients’ knowledge about therapeutic innovations and the importance of the implementation of shared decision making with the oncologist, especially in the face of the progressive chronicisation of a number of metastatic breast cancers.

Design de jeux pour la santé : utilisation de jeux sérieux pour favoriser l’adhésion aux exercices thérapeutiques chez des jeunes patients atteints de fibrose kystiqueDuguay, David 03 1900 (has links)
Mémoire en recherche-création. Master in research-creation / Vers l’âge de 7 ans, les patients atteints de fibrose kystique (FK) sont amenés à suivre une thérapie par pression expiratoire positive (PEP). Celle-ci consiste à réaliser une série d’exercices respiratoires répétitifs et méthodiques. Cependant, ces exercices peuvent être ennuyeux et démotivants pour des jeunes enfants et entrainent un taux d’adhésion quotidien de moins de 50%. Ce faible résultat affecte grandement la qualité et l’espérance de vie à long terme des patients, en plus d’avoir un impact social et familial considérable. Ces derniers utilisent les aspects ludiques et interactifs des jeux vidéo pour motiver et susciter l’engagement des sujets envers l’activité et son contenu. Ils ont déjà faits leurs preuves dans le domaine de la santé et de l’éducation en favorisant l’apprentissage ou la réhabilitation par exemple. Ce projet de recherche-création cherche à valider la possibilité d’utiliser des jeux spécialement conçus pour la thérapie PEP ainsi que l’intérêt des jeunes atteints de FK à les utiliser dans le cadre de leurs exercices thérapeutiques. L’objectif est de favoriser l’adhésion des jeunes envers cette thérapie et faciliter l’apprentissage et l’application des techniques associées. Cette approche ludique vise ultimement à améliorer la qualité de vie des jeunes atteints de FK dans la durée. Grâce à une collaboration entre des étudiants de l’Université de Montréal et des spécialistes en santé de la clinique de FK du CHU Ste-Justine, trois prototypes de jeux et un prototype d’interface électronique, utilisé pour contrôler les jeux et pour être compatibles avec l’appareil PEP le plus utilisé en Amérique du Nord (le PariPEP®), ont été conçus dans le cadre de ce projet. Les prototypes ont été testés sur place, à la clinique FK de l’hôpital pour enfants, dans le cadre d’une étude pilote effectuée auprès de 10 jeunes patients, toutes et tous âgés entre 8 et 10 ans. / At around the age of 7 years old, younger patients diagnosed with cystic fibrosis (CF) are usually prescribed a new therapy using positive expiratory pressure (PEP). For PEP therapy, patients are required to perform a series of repetitive and methodical breathing exercises to improve lung capacity among other health benefits. However, these exercises can be boring and demotivating for young children. As a result, daily adherence rate to PEP therapy is less than 50%. In turn, this affects the quality of life and long-term health of patients as well as directly impacting their families. In the education and health sectors, serious games are already used with children to help them with learning, therapy and even physical rehabilitation. Serious games leverage the fun and interactive aspects of video games to motivate and improve the commitment of the player to learn the targeted material or adhere to a specific health behaviour. This research-creation
project seeks to validate the efficacy of using games specially designed to help patients adhere to PEP therapy exercises more regularly. The objective is to encourage them to get involved in their daily exercises and to facilitate the learning and application of the breathing techniques. This playful approach ultimately aims to improve the quality of life of CF children over
time. Leveraging a multi and interdisciplinary collaboration between students from University of Montreal and specialists from the CF clinic at CHU Ste-Justine, three games prototypes and an electronic device prototype, used to control the games, were developed for the most popular PEP device in North America (PariPEP®). The games and electronic device prototypes were tested at the CF clinic, within the hospital for children, as part of a pilot study with 10 young patients, aged from 8 to 10 years old.
