Bakgrund: Antalet personer med diabetes mellitus typ 2 är ett globalt växande problem. Målet med behandlingen är normala blodsockernivåer och egenvård är en viktig del av behandlingen. Många personer med diabetes mellitus typ 2 upplever stigma av diagnosen. Stigmat upplevs i det sociala livet och på arbetsplatsen.  Syfte: Syftet var att belysa för sjuksköterskan vilken inverkan stigma av diabetes mellitus typ 2 har på vuxna med diabetes mellitus typ 2.  Metod: En litteraturöversikt. 16 vetenskapliga artiklar hämtade från databaserna CINAHL, Pubmed och PsycINFO användes till grund för resultatet.  Resultat: Fem teman framkom i resultatet. Känslomässig inverkan, viljan att gömma sin diagnos, strategier för att gömma sin diagnos, inverkan på sociala livet och inverkan på egenvården.  Slutsats: Den negativa inverkan som stigmat har på personer med diabetes mellitus typ 2 skulle kunna orsaka en kaskadeffekt som grundar sig i en negativ inverkan på det känslomässiga, som i sin tur leder till en negativ inverkan på det sociala livet och egenvården. Sjukvården har en betydande roll i hur personer upplever stigma av diabetes mellitus typ 2. Genom att belysa den negativa inverkan stigmat har, kan hälso- och sjukvårdspersonal anpassa vården till patienten och därmed bidra till en ökad vårdkvalitet och främja hälsa för personer med diabetes mellitus typ 2. / Background: The number of people with type 2 diabetes mellitus is a globally growing issue. The goal with treatment is normal blood sugar levels and self-care is an important part of treatment. Many people with type 2 diabetes mellitus experience stigma of the disease. Stigma is experienced in the social life and in the workplace.  Aim: The aim of this study was to illuminate to the registered nurse what affect stigma of type 2 diabetes mellitus has on adults with type 2 diabetes mellitus.  Method: A literature review. 16 articles retrieved from the databases CINAHL, Pubmed and PsycINFO have been the basis for the objective.  Results: Five themes emerged in the result. Emotional affect, the will to hide the diagnosis, strategies to hide the diagnosis, impact on the social life and impact on the self-care.   Conclusion: The negative affect that stigma has on people with type 2 diabetes mellitus could cause a cascade affect that is based on a negative affect on emotions, which leads to negative affects on the social life and self-care. Health care has a significant role in how people experience the stigma of type 2 diabetes mellitus. By illuminating the negative affect that stigma has, healthcare professionals can adjust the care to the patient and contribute to a care with higher standard and enable good health for people with type 2 diabetes mellitus.

The relationship between physical activity and the risk of type 2 diabetes mellitus in Ellisras rural young adults aged 22 to 20 years : Ellisras longitudinal study

Thesis (M.Sc. (Physiology)) -- University of Limpopo, 2019 / Background Type 2 diabetes mellitus (T2DM) is an increasing challenge globally, and is estimated to affect 439 million adults by 2030. This estimate is linked to an unhealthy lifestyle with characteristics such as low physical activity (PA) and high plasma glucose levels (PGLs). Studies associating PA with insulin resistance and diabetes among adults and adolescents have been conducted widely in developed countries. Such studies are scanty among rural populations, especially in Africa. Assessment of the burden of diabetes and associated lifestyle risk factors in developing countries is essential in order to encourage appropriate intervention strategies to counter the increasing prevalence. Aim and objectives The aim of this study was to investigate the relationship between PA and T2DM among rural young adults aged 22 to 30 years in Ellisras area in Limpopo Province, South Africa Methods A total of 713 young adults (349 males and 364 females) who have been part of the Ellisras Longitudinal Study participated in the current study. Physical activity data was collected using a validated questionnaire. After an overnight fast, participants provided fasting venous blood samples for determination of plasma glucose and insulin. Insulin resistance was estimated using the homeostasis model assessment of insulin resistance. Anthropometric measurements (waist circumference and height) were performed using standard procedures. Linear and logistic regressions were used to assess the relationship between PA, pre-diabetes, insulin resistance and T2DM; and the odds of having T2DM with low PA levels. Results The prevalence of physical inactivity was 67.3 and 71.0% for males and females, respectively. That of pre-diabetes was between 45.7% and 50.2%. The prevalence of diabetes was 9.6% for males and 10.1% for females while for insulin resistance was 22.9% for males and 29.3% for females. Linear regression found a significant relationship (p<0.05) between physical activity and blood glucose (ß =5.715; 95% CI 4.545; 6.885), waist circumference (ß = 37.572; 95% CI 25.970; 49.174) and waist-toheight ratio (ß = 0.192; 95% CI 0.087; 0.296). Logistic regression found a significant (p<0.05) relationship between low physical activity and T2DM (Odds ratio = 2.890; 95% CI 1.715; 4.870) and insulin resistance (Odds ratio = 1.819; 95% CI 1.266; 2.614). Conclusion Physical activity is low in this population, and is independently associated with T2DM and insulin resistance. Bakgrund: Diabetes mellitus (DM) ses som en av de snabbast växande sjukdomarna globalt där diabetes mellitus typ 2 (DMT2) står för majoriteten av alla insjuknanden av DM. Övervikt i kombination med fysisk inaktivitet är två av faktorerna som bidrar till insjuknandet av DMT2. Förändringar av kost och motion blir därför grunden i behandlingen. Sjuksköterskan behöver kunskap om individuella patienters behov för att möta patienters individuella kunskaper och behov. Syfte: Syftet var att beskriva patienters upplevelser av att förändra kost- och motionsvanor vid diabetes mellitus typ 2. Metod: Litteraturöversikten baserades på kvalitativa artiklar. Urvalsprocessen skedde utifrån Forsberg och Wengströms sex olika steg. I dataanalysen användes Fribergs metod. Databaserna Cinahl och Medline användes och resulterade i 12 artiklar. Resultat: Analysen resulterade i fyra huvudteman vilka var Vikten av tillräcklig kunskap och information för kostförändringar, Känslor och begränsningar vid kostförändringar, Sociala interaktioner och dessas påverkan på kostförändringar och Upplevelser av att förändra motionsvanor. Nio underteman identifierades.Slutsats: Resultatet visar på varierande faktorer som påverkar upplevelsen av kost- och motionsförändringar vid DMT2. En viktig aspekt är att sjuksköterskan tillämpar en personcentrerad vård för att öka patientens compliance. / Background: Diabetes mellitus (DM) is seen as one of the fastest growing diseases globally where type 2 diabetes mellitus (T2DM) accounts for the majority of all illnesses regarding DM. Obesity in combination with physical inactivity are two of the factors that contribute to the onset of T2DM. Changes in diet and exercise therefore become the basis of the treatment. The nurse needs pedagogical competence to meet patients' individual knowledge and needs. Aim: The aim was to describe patients' experiences of changing diet and exercise habits in type 2 diabetes mellitus. Method: The literature review was based on qualitative articles. Based on Forsberg and Wengström, the selection process took place in six different steps. Friberg's method was used in the data analysis. The Cinahl and Medline databases were used and resulted in 12 articles. Results: Four main themes were presented which were: The importance of sufficient knowledge and information for dietary changes, Emotions and limitations in dietary changes, Social interactions and their impact on dietary changes and Experiences of changing exercise habits. Nine sub-themes were identified. Conclusion: The results show varying factors that affect the experience of diet and exercise changes in T2DM. An important aspect is that the nurse applies a person-centered care to increase the patient's compliance.

Sjukdomen är förknippad med enohälsosam livsstil samt otillräcklig kunskap och utbildning. Egenvård är en viktig och avgörande del av behandlingen. Syftet: Syftet med denna litteraturstudie var att beskriva individens erfarenheterav egenvård vid diabetes mellitus typ-2. Metod: En litteraturstudie med kvalitativ ansats med tematisk analys av 13vetenskapliga artiklar. Resultat: Två teman med fyra subteman identifierades: förutsättningar föregenvård med subteman: behov av kunskap och behov av stöd. Andra temat: förhålla sig till en ny vardag med subteman: livsstilsförändring och oro somföljeslagare. Slutsats: kunskapsbrist var ett hinder för hantering av egenvård. Med hjälp avkunskap om livsstilsförändringar och stöd av både närstående ochsjukvårdspersonal kan individen upprätthålla en bra egenvård. Individensegenvårdsförmåga kan påverkas av sjuksköterskans stöd. / Background: Diabetes Mellitus type 2 is a public disease that has increaseddramatically worldwide in recent decades. The disease is associated with anunhealthy lifestyle and insufficient knowledge and education. Self-care is animportant and decisive part of treatment. Aim: The purpose of this literature study was to describe the individuals’experiences of self-care in diabetes type 2. Method: A literature study with a qualitative approach with thematic analysis of13 scientific articles. Results: Two themes with four subthemes were identified: prerequisites for self-care with subthemes: need for knowledge and need for support. Second theme:relating to a new everyday life with sub-themes: lifestyle changes and anxiety asa companion. Conclusion: lack of knowledge was an obstacle to managing self-care. With thehelp of knowledge about lifestyle changes and support from both relatives andhealthcare professionals, the individual can maintain good self-care. Theindividuals’ self-care ability can be affected by the nurse’ support.

Two-year follow-up of Patients with GestationalDiabetes Mellitus in Örebro County : – a retrospective cohort study

Introduction: Gestational diabetes mellitus (GDM) is a risk factor for developing type 2diabetes mellitus (T2DM). However, compliance to postpartum follow-up remains low.The new outcome-based International Association of Diabetes and Pregnancy StudyGroups (IADPSG)-criteria have led to a worldwide increase of GDM prevalence. Thepotential impact of the new IADPSG-critera on the frequency of T2DM/prediabetesdiagnosis, during follow-up of prior GDM-women in Örebro County, has not yet beenstudied. Aim: To investigate whether there is a difference in prevalence of prediabetes/T2DM inGDM-women two years postpartum, based on older GDM-criteria vs. the new lower IADPSG2010/WHO 2013-criteria, and to describe compliance to follow-up in primary health care(PHC). Methods: A retrospective cohort study of 108 GDM-women in Örebro County. Data obtainedfrom medical records: cardiometabolic risk factors, compliance data, outcomes withnormoglycemia used as comparison at follow-up. Results: T2DM and prediabetes were non-statistically significant higher in the old criteriagroup[11 (28.9%) vs. 6 (18.2%), p=0.289] and [12 (30.8%) vs. 5 (15.6%), p=0.137],respectively. Compliance to both 1st and 2nd follow-up was seen in 81 (75%) women. 96(88.9%) to first and 88 (81.5%) to second follow-up. An increase in mean BMI was seen frombaseline to PHC (31 vs. 31.7, p=0.014) as well as an increase in alcohol consumption [4(6.6%) vs. 23 (37.7%), p&lt;0.001]. Conclusions: GDM-diagnosis based on IADPSG-criteria did not show any statisticallysignificant difference in the prevalence of T2DM/prediabetes two years postpartum, whencompared to GDM-diagnosis based on old criteria. Moderate compliance to postpartumfollow-up was observed, with an increased number of dropouts. More research is needed inthis area, following implication of IADPSG-criteria.

Bakgrund: Diabetes mellitus typ 2 är ett kroniskt och progressivt tillstånd. En orsak som bidrar till ökad risk för diabetes mellitus typ 2 är övervikt och fetma. Den globala prevalensen för diabetes mellitus typ 2 samt övervikt och fetma ökar för varje år. Sjuksköterskans största hälsofrämjande möjlighet finns i det vardagliga mötet med patienter. Syfte: Var att belysa hälsofrämjande interventioner för individer med övervikt eller fetma. Metod: En allmän litteraturstudie som utgick från vetenskapliga artiklar där resultaten delades in i dataenhetersomkodades. Koderna kategoriserades och strukturerades tematiskt. Resultat: Resultatet baseras på åtta vetenskapliga artiklarsom genereradetrekategorier: sjuksköterskans hälsofrämjande interventioner, att ge hälsofrämjande interventioner och att motta hälsofrämjande interventioner. Att erbjuda hälsofrämjandeinterventioner resulterar i minskadvikt, BMI och midjeomfångtill följd avlivsstilsförändringar såsom ökad fysisk aktivitet och hälsosam kost. Att genomgå en intervention har även betydelse förpatientensmotivation. Oavsett intervention kan sjuksköterskan haen central och stödjande rollför patienten. Konklusion: Genom hälsofrämjande omvårdnadkan sjuksköterskan i samverkan med andra professioner tillhandahålla personcentrerad vård som ökar delaktighet och egenvårdsförmåga hos patienten, en faktor som bidrar till förbättrad hälsa. / Background: Type 2 diabetes mellitus is a chronic and progressive condition. Overweight contributes to an increased risk of developing type 2 diabetes mellitus. The global prevalence of type 2 diabetes mellitus, overweightand obesityis increasing every year. The nurse's greatest health-promoting opportunity is found in the everyday meeting with patients. Aim: To illustrate health-promoting interventions for individuals with overweight or obesity. Method: A general literature study based on scientific articles where the results were divided into data units that were coded. The codes were categorized and structured thematically. Results: The results are based on eight scientific articles that generated three categories: the nurse's health promotion interventions, to givehealth promotion interventions and to recieve health promotion interventions. Providing health promotion interventions reduces weight, BMI and waist circumference as a result of lifestyle changes such as increased physical activity and healthy eating. Going throughan intervention is also important for the patient's motivation. Regardless type of intervention, the nurse can have a central and supportive rolefor the patient. Conclusion: Through health-promoting nursing, the nurse in collaboration with other professionals can provide person-centered care that increases the patient's participation and self-care ability, a factor that contributes to improved health.

Risken för typ 2-diabetes mellitus är kopplad till livsstilsfaktorer som dålig kost, övervikt och stress samt kan leda till allvarliga komplikationer som hjärt- och kärlsjukdomar, neuropati, hypoglykemi och retinopati, vilket kräver regelbunden vård och hälsosamma levnadsvanor för att förebyggas. Syfte: Syftet med studien är att belysa patienters erfarenheter av egenvård vid typ 2-diabetes mellitus. Metod: Studien genomfördes som en litteraturbaserad studie med en kvalitativ ansats inspirerad av Friberg. Resultat: Genom analysen av valda artiklar har fyra teman identifierats: Svårigheter att ändra kosten, fysisk aktivitet som främjar egenvård, närståendes påverkan och vårdpersonalens stöd och påverkan. The risk of type 2 diabetes mellitus is linked to lifestyle factors such as poor diet, obesity and stress and can lead to serious complications such as cardiovascular disease, neuropathy, hypoglycemia, and retinopathy, which require regular care and healthy lifestyle habits to prevent. Aim: The aim of this study is to describe patients' experiences of self-care in type 2 diabetes mellitus. Method: The study was conducted as a literature-based study with a qualitative approach inspired guided by Friberg. Findings: Through the analysis of selected articles, four themes have been identified: Difficulties in changing the diet, physical activity that promotes self-care, the influence of relatives, and the support and influence of healthcare professionals. These themes highlight patients’ experiences of self-care. Conclusion: Self-care in patients with T2DM can be challenging, with initial feelings of shame and fear. It is necessary that care adapts to the patient's life situation and offers support and strategies for lifestyle changes. The support and motivation of relatives and care staff is important for promoting good self-care habits.
