The immunobiology and clinical management of acute graft versus host disease after allogeneic transplant

Alloreactivity between donor cells against disparate host tissue is a natural and normal physiologic phenomenon after engraftment. Consequently, GVHD is a universally expected side effect after allogeneic HSCT. An effective strategy to prevent severe or fatal acute GVHD is require if the transplant is to be successful. The HSCT field has witnessed significant progress in the prevention and treatment of acute GVHD. However, select interventions come at the cost of losing the alloimmune activity that prevents relapse, the GVL effect, as many of the mechanisms which cause GVHD are shared with those responsible for GVL. Current efforts are focused on therapeutic interventions that not only alleviate the burden of acute GVHD but does so in a way that maintains the GVL effect. This review will provide an up-to-date overview of our current understanding of the diagnosis, risk stratification, immunobiology of acute GVHD, summarize efforts to prevent and treat the disease, and provide a perspective on future directions.

Molecular Analysis of Oligoclonal T cells Associated with Graft-Versus-Host Disease Following Allogeneic Stem-cell Transplantation

The goal of hematopoietic stem cell transplantation (HSCT) is to induce graft-versus-tumor effect (GVT), which is the recognition of and response against tumor- associated antigens (TAAs) by donor immune cells to clear the recipient of residual tumor. A complication of HSCT as a treatment for hematologic malignancies is graft-versus-host disease (GVHD), which is the recognition and reactivity of donor immune cells against healthy tissues. As of now, the differentiation between GVHD and GVT effects has been a hindrance to the development of effective therapies against GVHD. Certain T cell clones may induce both GVHD and GVT effects, making targeted therapy of GVHD difficult. This project was aimed to uncover differences at a molecular level of the T cell recognition site that exist between patients with GVHD and those with GVHD-free survival following allogeneic HSCT. We found that there are inherent differences in the T cell receptor at a molecular level between patients experiencing GVHD and those that are GVHD-free, suggesting the ability of T cells to distinguish tumor cells from self cells. In addition, the intention was to reveal differences in proportions of engrafted donor T cells and stem cells and the effects of these proportions on the severity, outcome, and prognosis of GVHD. We additionally found that a lower proportion of stem cells to T cells was associated with the trend of GVHD, while a higher frequency of T cells engrafted into host may indicate resistance to treatment and a poor prognosis. These data suggest that allogeneic HSCT may be improved by optimizing the proportion of T cells to stem cells in the transplant as well as developing targeted therapy against GVHD-associated T cell clones while rescuing GVT-associated T cell clones.

A querela entre o papa e o imperador ao longo dos séculos XIII e XIV propiciou o surgimento de inúmeras teorias a respeito do poder em defesa de uma ou outra instituição. Marsílio de Pádua (1280-1343) foi um dos mais importantes pensadores a apresentar argumentos para essas discussões no século XIV. Ao denunciar os desmandos do papa e do seu Colégio de Cardeais viu-se obrigado a refugiar-se na corte do imperador Luís IV. A principal obra do paduano foi o Defensor da Paz (1324) na qual expôs elementos fundamentais para o debate em questão. Esta tese teve como objetivo discutir a doutrina política desenvolvida por Marsílio de Pádua a partir do seu próprio contexto. Para isso, foi necessário compreender seus conflitos e influências a fim de analisar os conceitos políticos apresentados em sua obra maior. Apoiado em Aristóteles, escreveu um plano prático para a intervenção política do imperador no Regnum Italicum. Marsílio adquiriu importância fundamental por sua formulação da noção de poder restrita à esfera temporal, delineando uma primeira ideia de autonomia do poder civil, dotado de atribuições específicas e independentes da esfera eclesiástica. / The quarell between the pope and the emperor during the XIII and XIV centuries has given rise to numerous theories about the power in defense of one or another institution. Marsilius of Padua (1280 1343) was one of the most important thinkers to present arguments for these discussions in the XIV century. Informing against the excesses of the pope and his Cardinal´s College, he was forced to take refuge at the court of emperor Ludwig IV. The main paduans work was the Defensor Pacis (1324) in which he exposed fundamental elements to the debate in question. This thesis aimed to discuss the political doctrine developed by Marsilius of Padua from his own context. For this, it was necessary understand their conflicts and influences in order to analyze the political concepts presented in his major work. Supported by Aristotle, he wrote a practical plan for policy intervention of the emperor at Regnum Italicum. Marsilius got fundamental importance by his formulation of the notion of power restricted to the temporal sphere, outlining a first idea of autonomy of civil power, endowed with independent and specific assignments of the ecclesiastical sphere.

Essa lei trouxe novas diretrizes para a gestão dos estágios curriculares supervisionados, tanto para as instituições de ensino quanto para as organizações concedentes. Desse modo, almeja-se estudar qual a melhor concepção de estágio que se adapta à realidade dos cursos selecionados para o estudo, quais foram as estruturas e documentos implementados para atender à atual legislação, bem como diagnosticar quais são os obstáculos que dificultam a operacionalização do estágio supervisionado dento da instituição. A partir do diagnóstico encontrado são propostas ações para sistematização do estágio supervisionado na UFJF. Foram selecionados quatro cursos: Engenharia Civil, Letras, Pedagogia e Enfermagem, por meio dos quais o caso de gestão será analisado. A escolha desses cursos foi pautada pela necessidade de conhecer um curso de cada área de formação, a saber, Ciências Exatas, Ciências Humanas e Ciências da Saúde. O curso de Pedagogia foi escolhido uma vez que a Faculdade de Educação gerencia todos os estágios das licenciaturas da instituição e possui uma estrutura de funcionamento articulada com as demais unidades acadêmicas. Desse modo, a intenção é averiguar como esses cursos estruturaram-se para atender às especificidades de suas modalidades de formação e como está distribuída a responsabilização e controle da supervisão e orientação dos alunos, articulando a dicotomia: o estágio supervisionado ideal e o real. O trabalho é de cunho qualitativo e estudo de caso que utiliza, inicialmente, uma revisão de conteúdos teóricos, análise documental, observações do pesquisador, bem como entrevistas com roteiro semiestruturado com sujeitos que contribuíram com a elucidação da pesquisa: coordenadores de cursos, coordenadores de estágio, gestores e professores orientadores das disciplinas de estágio. / The present dissertation aims to investigate the current characteristics of the supervised curricular internship, as well as the impacts which occurred at the Federal University of Juiz de Fora, after the enactment of the law number 11.788 of September 25th, 2008. Such law brought several new guidelines to the management of supervised curricular internships, both to the teaching institutions and to the grantor organizations. Thus, we aim to study which is the best conception of internship, which may adapt itself to the reality of the courses selected to the study, which were the structures and documents implemented to address the current legislation, as well as to diagnose which are obstacles that stand in the way of operationalizing the supervised internship within the institution. From the diagnosis we propose actions to systematize the supervised internship at UFJF. Four courses were selected: Civil Engineering, Liberal Arts, Pedagogy and Nursing, through which the case study will be analyzed. Such choice was made based on the necessity of encompassing one course of each of the major areas of knowledge, that is, Exact Sciences, Human Sciences and Biological Sciences. The Pedagogy course was chosen since the School of Education manages all the internships to the teachers in training and possesses a structure articulated with the other academic units. Therefore, our goal is to ascertain how these courses were structured to address the specificities of their formation and how the accountability and supervision oversight is distributed, articulating the dichotomy: the ideal and the real supervised internship. The study is of qualitative nature and is a case study that initially utilizes a theoretical review, documental analysis, observations by the researcher, as well as semi-structured interviews with those who contributed to elucidating the topic: course coordinators, internship coordinators, managers and supervising professors of internship disciplines.
