Health Seeking Behaviour among the People of the Africa Gospel Church in Francistown

This research was stimulated by an indication that members of the Africa Gospel Church do not utilise professional health care services optimally, probably due to religious reasons. This may also be the result of registered nurses’ inability to render culture congruent care. To render culture congruent care, nurses require knowledge of the belief system and the health seeking behaviours of the church members. The qualitative research investigated the religious beliefs and health seeking behaviours of members of the Africa Gospel Church in Francistown, Botswana. An exploratory ethnonursing research method was applied. Semi-structured interviews were held with two purposively selected samples. Qualitative data analysis was done using computer software to reduce data and interpret the research results. Developing an e-records readiness framework for labour organisation in Botswana

The application of information communication technologies (ICTs) in records management in many organisations underscores the need for the assessment of e-records readiness. This provides a useful guide in benchmarking, gauging progress and comprehending e-records management. This study examined e-record readiness in labour organisations with a view to proposing a framework for labour organisations in Botswana. The study was largely guided by a quantitative paradigm and used a survey research strategy. This was complemented by methodological triangulation of both quantitative and qualitative data collection methods. All the 50 registered labour organisations in Botswana were surveyed, 45 of which responded, representing a response rate of 90%. Data was obtained through structured questionnaires, semi-structured interviews, document review and observations. The study established that e-records readiness in labour organisations in Botswana was evident, low and evolving, evidenced by the slow adoption of ICTs; inadequate records management standards and practices; and low integration in the national e-readiness framework. The study confirmed that most labour organisations had embraced the utilisation of ICTs in their work; exhibited some form of records management function and acknowledged the existence of national policy directives and strategies on e-readiness. However, traditional ICTs mainly the fax and telephone, were more prevalently adopted and used than newer ones such as e-mail and the Internet. The use of social media technology (Web 2.0 - Facebook, YouTube, and Twitter) was yet to be explored. The management of both paper-based and electronic records was not satisfactory and fell short of best recognised records management standards and practice. The integration of labour organisations into national e-readiness initiatives as espoused in key policy proclamations within the context of the pursuance of a holistic knowledge and information society was slow and remained challenging. In order to foster successful e-records readiness in labour organisations, several recommendations were advanced that underscored effective ICT adoption and use, implementation of best records management practices and rigorous integration of labour organisations into the information and knowledge society in Botswana. Implementation of the performance management system in the Ministry of Foreign Affairs and International Cooperation of the Botswana public service

The main aim of this study was to investigate the implementation of the performance management system in the Ministry of Foreign Affairs and International Co-operation of the Botswana public service and to identify the factors that facilitated and hindered the successful implementation of the programme. This study used a mixed methods research design which combines both qualitative and quantitative research design. The study used the model of social programmes, which is a holistic and comprehensive analytical framework that is used to study the implementation of social intervention programmes. Research data was obtained using a triangulation of research methods (case study, evaluation research and survey method) and data sources (literature review, document analysis, self-administered questionnaires and follow-up personal interviews). A sample of 90 respondents was selected using the purposive sampling strategy. The study found that the implementation of the performance management system in the Ministry of Foreign Affairs and International Co-operation was problematic because of unclear and ambiguous foreign policy goals and objectives; lack of broad-based support and critical mass of champions of the performance management system; use of command-and-control management model rather than empowerment and the adaptation model; inadequate human and institutional capacities; ineffective supporting systems of the performance management system; weak accountability and responsibility mechanisms; weak intra-ministerial and inter-ministerial communication, co-operation, collaboration and co-ordination; inadequate information and communications technology infrastructure and unfavourable international environment. This study contributed to the existing literature by investigating the implementation of the performance management system in the global environment from a public service perspective rather than a private sector perspective, which relies on the experiences of multinational companies. The study findings provide some invaluable insights that may improve the implementation of the performance management system in the Ministry of Foreign Affairs and International Co-operation of the Botswana public service by proposing recommendations to the identified problems. The recommendations include: setting clear and specific foreign policy objectives; creating a critical mass of champions of the performance management system; strengthening human and institutional capacities; promoting effective implementation and use of supporting systems of the performance management system; strengthening accountability and responsibility mechanisms; strengthening intra-ministerial and inter-ministerial communication, co-operation, collaboration, and co-ordination; strengthening decentralisation and empowerment principles and upgrading information and communications technology infrastructure. / Public Administration and Management / DPA

Factors affecting highly active anti-retroviral therapy adherence in a rural area in Botswana

The purpose of this study was to determine factors affecting non-adherence to Anti-retroviral therapy among AIDS patients. A cross sectional quantitative survey, was used. Structured interviews were conducted with 300 ART patients. Data were analysed using SPSS version 13 and presented in charts, graphs and frequency tables. Motivators of good adherence were identified as disclosure of HIV positive status to more than one person, frequent adherence counselling, self-efficacy to adhere to ART, positive interactions between patients and healthcare providers; and using an adherence partner. Barriers to adherence were forgetfulness, transportation costs to and from the clinic, time away from work and side effects. There was a strong positive correlation between adherence, CD4 counts and viral load. Adherence was closely tied to immunologic and virologic improvements. Respondents with poor adherence were likely to have unsuppressed viral loads (OR 12.98, 95%, CI 4.9-34). / Health Studies / Masters of Public Health

Developing policy guidelines to promote quality of life of young adults with perinatally acquired HIV in Botswana

Due to the successful rollout of the Antiretroviral Therapy Program, an increasing number of perinatally HIV infected adolescents are emerging into young adulthood throughout Botswana. Young adulthood is a critical period of human development, with long-lasting implications for a person’s economic security, health and well-being. During this time, young women and men normally complete school, find employment and start working, develop relationships, form families, bear children and pursue those things that help set them on the path to healthy and productive adult life. However, the presence of a chronic illness such as HIV can interfere with the achievement of the developmental milestones of young adulthood and affect their Health Related Quality of Life (HRQOL). The purpose of this study was to identify the factors that affected the HRQOL of young adults living with perinatally acquired HIV (YALPH) and to propose policy guidelines to promote their HRQOL. A mixed-methods sequential explanatory research design was used. HRQOL assessments were made using the WHOQOL-HIV BREF instrument. Data about the clinical characteristics of the respondents was obtained from medical records. In-depth interviews were conducted with a purposefully selected subsample of respondents who completed the WHOQOL-HIV BREF instrument. All the respondents were recruited from Botswana-Baylor Children’s Clinical Centre of Excellence, in Gaborone, Botswana. Data were analyzed using SPSS Inc. software version 16.0 (statistical package for social science, SPSS Inc, Chicago, IL, USA). The study population consisted of 509 YALPH including 255 (50.1%) females and 254 (49.9%) males. The mean age of the population was 21.7 (± 2.6) years (range 18-29.8 years). The majority of the respondents were single (98.1%), living in their parental homes (90.8%), neither in school nor working (47.35%) and 14% were parents (range 1-3 children). The mean duration on ART was 12.4 years (± 4.0). Based on the BMI classifications by WHO, 38.5% of respondents were underweight (BMI < 18.5 kg/m2) and 7.3% were overweight (BMI ≥ 25.0 kg/m2). Unsuppressed viral load (>400 cell/mL) occurred in 13.4% of the sample. Most respondents had good HROQL (78.4%). The highest mean HRQOL score was recorded in the Physical domain (15.4 (± 2.9) and the lowest in the Environment domain 13.8 (± 2.7). The results fitted using the multivariable logistic regression suggest the odds for good general QOL were increased amongst individuals with a higher level of education and 6 those who were employed. The odds for good general QOL were reduced for individuals with unsuppressed viral load (> 400 cells/mm2) and those who had illnesses (self-reported). The odds for good general QOL increased by almost two folds (OR = 1.97, 95% CI = (1.11 – 3.48)) when comparing respondents with higher level of education against those with lower education. The odds for good general QOL were increased for employed respondents OR = 1.73 (95% CI = (0.92 – 3.23) when compared to the unemployed group. Whereas the odds for good general QOL declined by almost two folds (OR = 0.60, 95% CI = (0.33 – 1.08)) amongst patients with VL > 400 cell/mm2 compared to those with VL < 400 cells/mm2. Also, respondents who were ill had lower odds ratios for good general QOL compared to those who were not ill (OR = 0.42, 95% CI = (0.25 – 0.70)). The results of in-depth interviews with 45 respondents showed that the majority of YALPH were in good physical health and they had positive perspectives about the future including health, completing school, finding employment, marriage and childbearing. The main sources of social support for YALPH were close family members and health care workers (HCWs). However, worries and concerns about disclosure, fear of stigma, lack of financial independence, and limited social relationships and networks were the most identified stressors that put the YALPH at risk of compromised HRQOL. Some sub-groups of YALPH were at higher risk for poor HRQOL including: young mothers, YALPH who were aging out of institutional care, YALPH with disabilities and impairments, YALPH who were neither in school nor working and YALPH with maladaptive coping strategies. Therefore, the promotion of the HRQOL of YALPH will require policies and interventions to increase educational attainment, provide employment and livelihood opportunities, promote good ART adherence and VL suppression, and effectively prevent and manage illnesses. The impact of pastoral farming and wildlife management practices on lion-livestock interactions in the Kgalagadi-South region of Botswana

Tech.) - Central University of Technology, Free State, 2009 / All over the African continent south of the Sahara Desert, African lion numbers are plummeting to levels where, over large areas of their remaining distribution range, extinction has become a real threat. The main reason for the decreasing numbers is the increasing conflict between livestock farmers and lions. Lions are forced to kill livestock where their natural prey has been squeezed out by livestock and associated farming practices, and the farmers find it necessary to protect their livelihoods, often through the indiscriminate killing of lions. In the Kgalagadi-South region of Botswana, lion/livestock interactions present a challenge to livestock owners and wildlife managers alike. The relatively low ecological carrying capacity and occupied lion habitats in the Kgalagadi Transfrontier Park (KTP) force some expelled young adult and sub-adult lions southwards into the adjoining Wildlife Management Area (WMA) KD/15, which separates the KTP and the communal grazing area. This WMA most likely also contains resident prides. Some of these predators sporadically enter the livestock grazing area. Similarly, large stock often enters the WMA. It is mostly these boundary transgressions that result in livestock killing, and the reaction of livestock owners often leads to the killing of lions. To gather information concerning the nature and extent of the situation, two questionnaires were prepared with the assistance of the Department of Biostatistics of the University of the Free State, South Africa. One questionnaire targeted livestock owners while the other was aimed at wildlife officials of the Department of Wildlife and National Parks in Botswana and SANParks in South Africa. Both covered the five-year period 2002-2006. A trial run was carried out to adjust to problem-specific circumstances before fieldwork commenced. Fieldwork was done during four consecutive seasons – in January, February, April and December 2007. Thirty livestock respondents and 13 wildlife officials were interviewed during the first two excursions into the study area. The third visit was to plot the cut-line between the WMA and the grazing area (by means of a Global Positioning System or GPS) and to make first-hand observations regarding movement over the cut-line. During all four visits the environmental (including grazing) conditions and density and distribution of wildlife and stock were observed in both the WMA and the grazing areas. The information gathered points towards a seemingly unsolvable situation. The exceptionally high daytime temperatures and food scarcity, brought about by erratic rainfall, overgrazing near boreholes, generally low carrying capacity and low phosphorus (P) levels, force large stock, i.e. cattle, horses, donkeys and mules, to graze far away from the safety of the cattle-posts during the cool hours of the night, thus making kraaling impractical. Such circumstances enhance exposure to lion predation especially in or near the WMA and the KTP fence. Some lions also penetrate deep into the grazing areas, especially in the arid western part of the study area. This study revealed certain weaknesses in current wildlife and livestock management practices in the study area, the sum of which put both farmers and the lion population under extreme pressure. Most of these shortcomings can be rectified without drastic invasive methods. Such adjustments can result in improved livestock and wildlife utilisation and protection of the lions. For example: the placement of mixed phosphorus and salt licks near cattle-posts to fulfil the need for vital micro and macro elements; addressing unnecessary livestock losses, which contribute to a lower income and less tolerance towards predation (e.g. botulism, which may stem from stock chewing on bones in their desire for more phosphorus, and losses to black-backed jackal, Canis mesomelas, in poorly maintained kraals); more drinking troughs at boreholes to prevent unnecessary shoving and minimise energy waste; and the introduction of more bulls to herds to increase the calving percentage. The study further concluded that there is little reason why stockowners should consider protecting lions. It suggests that significant value can be added to the wildlife (and the protection of lions) in the specific area by making farmers and other local residents share in the relatively untapped ecotourism potential of the area.

Knowledge of women attending antenatal clinics on transmission of HIV through breast-feeding in Gaborone, Botswana

Breast-feeding by Human Immunodeficiency Virus (HIV) positive mothers poses a risk of transmitting HIV infection from the mother to the baby. This study attempted to describe and explore the knowledge of antenatal women in Gaborone, Botswana on the transmission of HIV through breast-feeding. The research results, obtained from interview schedules, revealed that the respondents had: a greater knowledge of transmission of HIV in adults than mother-to-child-transmission (MTCT); a moderate understanding of infant feeding methods; poor understanding of the risk of HIV transmission through different infant feeding methods and feelings that infant feeding method reveals ones HIV status, which indicate stigma and discrimination. The study findings may assist health care providers to intensify educational programmes and counselling in the antenatal clinics and communities to reduce MTCT. / Health Studies / M. A. A curriculum for vocational business subjects in Botswana junior secondary schools

The aim of this research was to examine the extent to which the pedagogical practices of Business Studies teachers in Botswana junior secondary schools conform to pedagogical practices recommended for imparting practical business skills relevant to the world of work. It also aimed to identify the strengths and weaknesses of teachers’ current practices with a view to proffer a teaching model that would help to maximize learner acquisition of business skills and competencies. Literature related to the pedagogy of business education subjects was reviewed to give a general conceptual and methodological foundation for the investigation. An overview of the methodological approaches and the qualitative research design selected for application to the study were provided including the data-gathering procedures and the conceptual framework that supported and informed the research. The major findings of the study were that Business Studies teachers subscribe mainly to the transmission paradigm of teaching. Teachers’ failure to use constructivist pedagogies prescribed in the syllabus were attributed to a multiplicity of challenges they face in their day-to-day practices. The challenges that beset the pedagogy of business subjects emanate from a variety of sources such as the scarcity or non-availability of teaching materials and resources, a congested syllabus and problems associated with striking a balance between the theoretical and practical aspects of the subject. Teachers indicated that the Business Studies syllabus is too long and with the little time allocated to teach it on schools timetables, it is impractical to expect them to complete the syllabus using constructivist teaching approaches which they perceive as pedagogically burdensome and time-consuming. Despite the teachers’ constraints in creating constructivist learning environments, the use of an entrepreneurial pedagogy in the form of the mini enterprise whereby students are involved in setting and running a concrete enterprise is prevalent. The study concluded by suggesting a pedagogical model, based on the findings, to improve Business Studies curriculum delivery. It was also recommended that support structures aimed at monitoring and ensuring that the delivery of business education is done according to the stipulated business curriculum standards be put in place. / Curriculum and Instructional Studies / D. An evaluation of clinical waste management in Gaborone city council healthcare facilities

The management of clinical waste is of great importance due to its infectious and hazardous nature that can cause risks on environment and public health. The study was conducted to evaluate clinical waste management practices and to determine the amount of waste generated in five purposively selected healthcare facilities in Gaborone City Council. The surveyed healthcare facilities were of different size, specialization and category and included a referral hospital, two clinics and two health posts. To examine clinical waste management practices the study employed a range of methods including questionnaire survey which targeted 105 stratified randomly selected healthcare workers and ancillary staff, formal interviews with facility managers, field observations and literature reviews. Compliance with the Botswana Clinical Waste Management Code of Practice, 1996 and Waste Management Act, 1998 and other related documents were used as standards to assess clinical waste management practices. The waste management practices were analysed for a week in each healthcare facility to capture the daily management practices. The generated clinical waste was weighed to compute the generation rates and was followed through the various management practices to the final disposal. Findings of the study revealed that clinical waste generation rates were: 0.75kg/patient/day for Princess Marina Hospital and 0.1 - 0.3kg/patient/day for clinics and health posts. Numerous aspects of clinical waste management were found to comply with the expected rules and standards at Princess Marina Hospital, but the clinics and health posts had less appropriate practices. Clinical waste generated at Princess Marina Hospital is quantified in reliable records and dedicated Infectious Control Officers are responsible for monitoring the management of clinical waste. The study revealed that clinics and health posts do not quantify clinical waste and there are no officers responsible for monitoring clinical waste and there are no documents for monitoring the management of clinical waste. The main treatment method of clinical waste for the surveyed healthcare facilities is incineration and it is being done properly. The study established that at least 80% of healthcare workers and ancillary staff have been vaccinated against hepatitis B and have received training in clinical waste management. Origins of the centralised unitary state with special reference to Botswana, Zimbabwe and Namibia

The purpose of this thesis is to describe and explain the origins of centra 1 i sed unitary con st itut i ona l forms adopted at independence, with speci a 1 reference to the pre-independence period of colonial rule and the states of Botswana, Zimbabwe and Namibia. Since the states of the world are either unitary or federal, an attempt is made to distinguish the unitary constitutional form from federation by contrasting both concepts. The three states under study are identified as centralised and unitary by referring to political, historical, legal, administrative and fiscal criteria. The theoretical and practical origins and explanations for the adoption of unitary constitutional forms in the European and African context are explained. First, the theoretical origins of monism, pluralism, dualism, absolutism and sovereignty and the thoughts of a number of classical theorists are discussed. Next the practical origins, the statements and perceptions by members of African nationalist elites supportive of unitary states in Africa in the colonial and early post-colonial period are referred to, in partial exp 1 anat ion for the adoption of this con st itut i ona 1 form. British constitutional practices and precedents are also discussed. Further, to explain the origins of the centralised unitary state in Africa, the three case studies of Botswana, Zimbabwe and Namibia come under discussion. A four stage conceptual scheme devised by Etzioni and modified for the purpose of this thesis is utilised to analyse and explain the origins of the centralised unitary state in the three case studies of Botswana, Zimbabwe (xii) and Namibia. A variety of factors both historical and contemporary, internal and external to these countries are identified and analysed. These factors include amongst others, early settlement patterns and confiicts, British colonial practices and precedents, the perceptions and ambitions of nationalist movements and elites, relationships with neighbouring states, the climate of opinion, and the requirements of nation-building and political stability. The thesis is concluded by comparing the experiences of the three countries and, setting out several inductive propositions determining under which conditions these states adopted centralised unitary constitutional forms in preference to decentralised ones, federation or partition. Finally, the thesis is concluded by referring in a Postscript to the postindependence constitutional reassessment in the three countries concerned, the constitutional reassessment process in Africa in general, literature references to this process, and the prospects for constitutional reform on the continent. / Political Science / D.
