Função abdutora do quadril após a osteotomia basocervical e cervicoplastia no escorregamento epifisário proximal do fêmur / Hip abductor function after basicervical osteotomy and osteochondroplasty in slipped capital femoral epiphysis

O escorregamento epifisário proximal do fêmur (EEPF) é a afecção do quadril mais comum na adolescência e se caracteriza pela excentricidade da epífise em relação à metáfise. Deformidades anatômicas residuais e efeitos secundários no mecanismo abdutor podem estar associados à perda de força abdutora. Objetivamos avaliar a função da musculatura abdutora do quadril após tratamento cirúrgico com osteotomia femoral basocervical e cervicoplastia e comparar os resultados com indivíduos saudáveis. Vinte e quatro pacientes com EEPF moderado ou grave foram submetidos à osteotomia femoral basocervical e cervicoplastia entre 2012 e 2015, e foram avaliados prospectivamente com seguimento mínimo de um ano (idade média 14,9 ± 1,6 anos). O grupo controle foi composto por 15 indivíduos saudáveis sem sintomas no quadril (16,5 ± 2,5 anos). Avaliamos a amplitude de abdução passiva dos quadris, força concêntrica dos músculos abdutores do quadril por meio de dinamometria isocinética a 60°/s e a 120°/s, teste de Trendelenburg, e a pontuação dos questionários Harris Hip Score (HHS) e 12-Item Short-Form Health Survey (SF-12). Um subgrupo de sete pacientes foi submetido à avaliação longitudinal aos seis, 12 e 24 meses de pós-operatório. No seguimento final, comparamos a função abdutora dos quadris com escorregamentos moderados e graves submetidos à osteotomia basocervical e cervicoplastia, com escorregamentos contralaterais mais leves, submetidos à fixação in situ, quadris contralaterais sem EEPF, e grupo controle. No subgrupo avaliado longitudinalmente, houve melhora significativa na força muscular abdutora dos quadris durante os dois anos de seguimento (60º/s, p=0,048; 120º/s, p=0,001). O seguimento final médio de todos os pacientes foi de 1,6 ± 0,6 anos. Quadris com EEPF apresentaram a média de pico de torque abdutor diminuída quando comparados aos quadris contralaterais sem escorregamento (60º/s, p=0,004; 120º/s, p<0,001) e aos quadris dos indivíduos controles (60º/s, p<0,001; 120º/s, p<0,001). Após a osteotomia basocervical e cervicoplatia, os quadris apresentaram média de pico de torque abdutor (60°/s, p=0,63; 120º/s, p=0,99) e amplitude de abdução passiva (p=0,5) semelhante aos quadris fixados in situ. Houve correlação positiva significativa entre médias de pico de torque abdutor e a amplitude de abdução passiva do quadril (coeficiente de Spearman, 0,36; p<0,001). O sinal de Trendelenburg foi ausente em 91,6% dos pacientes no tempo final de seguimento. As médias das pontuações dos questionários dos pacientes no seguimento final foram de 94.8 ± 7.3 para o HHS; 52.6 ± 6.8 e 57.6 ± 4.6 para os componentes físico e mental do SF-12, respectivamente. Concluímos que o restabelecimento de força de mecanismo abdutor é progressivo nos dois anos após osteotomia basocervical e cervicoplastia, porém a força não foi restabelecida aos níveis normais. Um ano não foi suficiente para recuperação da força abdutora. Os resultados da osteotomia basocervical realizada em quadris com EEPF moderado e grave foram comparáveis aos resultados de quadris com desvio inicial significativamente menores submetidos à fixação in situ. / Slipped capital femoral epiphysis (SCFE) is a common hip disorder in adolescence and is characterized by the eccentricity of the epiphysis in relation to the metaphysis. The condition is associated with loss of abductor strength as a consequence of residual anatomical deformities and secondary effects in the abductor mechanism. We aimed to evaluate the abductor muscle function after the surgical treatment with basicervical femoral osteotomy and head-neck osteochondroplasty, comparing with healthy control individuals. Twenty-four patients with moderate or severe SCFE underwent basocervical femoral osteotomy and osteochondroplasty between 2012 and 2015, and were prospectively evaluated with a minimum follow-up of one year (mean age: 14.9 ± 1.6 years). The control group consisted of 15 individuals without hip symptoms (16.5 ± 2.5 years). We evaluated the passive range of hip abduction, concentric strength of the hip abductor muscles by means of isokinetic dynamometry at 60°/s and 120°/s, Trendelenburg test, Harris Hip Score (HHS) and 12-item short-form health survey (SF-12). A subset of seven patients underwent to longitudinal evaluation at six, 12 and 24 postoperative months. At the final follow-up, we compared the abductor function of the hips with moderate and severe slips submitted to basocervical osteotomy and cervicoplasty, with less severe contralateral slips submitted to in situ fixation, contralateral hips without SCFE, and control group. The subgroup assessed longitudinally had a significant improvement in the abductor muscular strength of the hips during two years of follow-up (60º/s, p = 0.048, 120º/s, p = 0.001). The mean final follow-up of all patients was 1.6 ± 0.6 years. The mean peak abductor torque was decreased in hips with SCFE when compared to the contralateral non-slip side (60º/s, p = 0.004; 120º/s, p <0.001); and control individuals (60º/s, p<0.001; 120º/s, p<0.001). After a basicervical osteotomy and osteochondroplasty, hips had a mean abductor torque (60º/s, p=0.63; 120º/s, p=0.99) and range of abduction (p=0.5) similar to hips pinned in situ. Abduction strength had significant positive correlation with the passive range of abduction (Spearman\'s coefficient, 0.36; p<0.001).The Trendelenburg signal was absent in 91.6% of the patients at the final follow-up. The mean HHS at the final follow-up was 94.8 ± 7.3; and the physical and mental components of the SF-12 were 52.6 ± 6.8 and 57.6 ± 4.6. We conclude that the restoration of abductor strength is progressive in two years following basicervical osteotomy and osteochondroplasty. One year was not enough for the restoration of the abductor strength. The outcomes of the basicervical osteotomy performed in hips with moderate and severe SCFE were comparable to in situ fixation performed in much less severe slips.

Problematika obrábění titanových slitin / Problems in machining of titanium alloys

The theoretical part of the thesis contains a brief overview of some experimental methods that are used in machining technologies. The experimental part at first elaborates on the analysis of two titanium alloys. The actual experiment, which consists of milling of the two aforementioned materials with the use of several tools and holders, is then described. It was conducted in collaboration with the company Frentech Aerospace s.r.o. When milling, forces were measured with piezoelectric dynamometer. Further, the analysis of acquired data that is followed with discussion about its main findings, is provided. The conclusion of the thesis deals with the found facts corresponding with the information obtained by analysis of materials. The main results arise out of the comparison of used combinations of tools and holders for milling of material Ti-Al6-V4, observation of impact of particular factors on measured force, or out of different values of measured forces when milling two given materials.

Impacts des technologies de dépollution et des conditions de conduites sur les émissions primaires des véhicules et leur évolution dans l'atmosphère / Impacts of depollution technologies and driving conditions on vehicle primary emissions and their evolution in the atmosphere

La pollution atmosphérique est une problématique urbaine majeure, avec des concentrations de polluants dépassant fréquemment les seuils de recommandations pour la santé. Les véhicules participent fortement à la pollution atmosphérique malgré l’intégration de systèmes de dépollution dans leur ligne d’échappement. L’objectif de ce travail de recherche était de caractériser les émissions primaires à l’échappement des véhicules ainsi que leur évolution physique en champ proche pour mieux comprendre la contribution des émissions primaires liées au trafic à la pollution atmosphérique urbaine. L’évolution des émissions a été étudiée dans une chambre de simulation atmosphérique qui a permis de simuler des conditions atmosphériques contrôlées.La première partie de ce travail était centrée sur les mesures d’émissions à l’échappement des véhicules récents qui sont ou seront majoritaires dans le parc automobile français dans les prochaines années. Pour cela, un échantillon de véhicules regroupant les principales technologies de dépollution commercialisées a été testé. Les gaz d’échappement émis par les véhicules ont été analysés lors de tests sur un banc à rouleau suivant différentes conditions de conduites. Les polluants soumis aux réglementations Européennes ont été mesurés, ainsi que certains composés non-réglementés par les normes Européennes mais dont la dangerosité a été mise en évidence par la communauté scientifique.La deuxième partie de ce travail se concentre sur l’évolution en champ proche des polluants particulaires dans les heures qui suivent leur rejet dans l’atmosphère. L’effet de la dilution soudaine des gaz en sortie de pot d’échappement a été étudié en utilisant une méthodologie innovante de prélèvement directement à l’échappement. Ensuite l’évolution des particules dans les premières heures après leur rejet a été étudiée dans une chambre de simulation atmosphérique de 8 m3, construite spécifiquement dans le cadre de cette thèse pour être couplé au banc à rouleau. / Air pollution is a major urban issue, with pollutant concentrations frequently exceeding health recommendations. Vehicles are highly involved in air pollution despite the integration of pollution control systems. The objective of this research is to characterise the primary exhaust emissions of vehicles and their physical evolution in the near field to better understand the impact of primary emissions related to traffic on urban air pollution. Emissions evolution was studied in an atmospheric simulation chamber with controlled atmospheric conditions. The first part of this work aims to determine the exhaust emissions of recent vehicles that are or will be major in the French car fleet in the coming years. A sample of vehicles grouping the main commercialised depollution technologies was tested on a chassis dynamometer according to different driving conditions and the emissions from their exhaust gas were analysed. The pollutants subject to European regulations were taken into account, as well as compounds not regulated by European standards but whose toxicity has been highlighted by the scientific community.The second part of this work focuses on the near-field evolution of particulate pollutants in the hours following their release into the atmosphere. The effect of the abrupt gas dilution at the end of the exhaust has been studied using an innovative sampling methodology directly at the exhaust of the vehicles. Then the evolution of the particles in the first hours after their rejection was studied in an atmospheric simulation chamber of 8 m3, built specifically within the framework of this thesis to be coupled with the dynamometer.

Is stair descent in the elderly associated with periods of high centre of mass downward accelerations?

No / When descending stairs bodyweight becomes supported on a single limb while the forwards-reaching contralateral limb is lowered in order to make contact with the step below. This is associated with lowering of the centre of mass (CoM), which in order to occur in a controlled manner, requires increased ankle and knee joint torque production relative to that in overground walking. We have previously shown that when descending steps or stairs older people operate at a higher proportion of their maximum eccentric capacity and at, or in excess of the maximum passive reference joint range of motion. This suggests they have reduced and/or altered control over their CoM and we hypothesised that this would be associated with alterations in muscle activity patterns and in the CoM vertical acceleration and velocity profiles during both the lowering and landing phases of stair descent. 15 older (mean age 75 years) and 17 young (mean age 25 years) healthy adults descended a 4-step staircase, leading with the right limb on each stair, during which CoM dynamics and electromyographic activity patterns for key lower-limb muscles were assessed. Maximum voluntary eccentric torque generation ability at the knee and ankle was also assessed. Older participants compared to young participants increased muscle co-contraction relative duration at the knee and ankle of the trailing limb so that the limb was stiffened for longer during descent. As a result older participants contacted the step below with a reduced downwards CoM velocity when compared to young participants. Peak downwards and peak upwards CoM acceleration during the descent and landing phases respectively, were also reduced in older adults compared to those in young participants. In contrast, young participants descended quickly onto the step below but arrested their downward CoM velocity sooner following landing; a strategy that was associated with longer relative duration lead-limb plantar flexor activity, increased peak upwards CoM acceleration, and a reduced landing duration. These results suggest that a reduced ability to generate high eccentric torque at the ankle in the forward reaching limb is a major factor for older participants adopting a cautious movement control strategy when descending stairs. The implications of this CoM control strategy on the incidences of falling on stairs are discussed.

Modeling and simulation of vehicle emissions for the reduction of road traffic pollution

The transportation sector is responsible for the majority of airborne particles and global energy consumption in urban areas. Its role in generating air pollution in urban areas is even more critical, as many visitors, commuters and citizens travel there daily for various reasons. Emissions released by transport fleets have an exhaust (tailpipe) and a non-exhaust (brake wears ) origin. Both exhaust and non-exhaust airborne particles can have destructive effects on the human cardiovascular and respiratory systems and even lead to premature deaths. This dissertation aims to estimate the amount of exhaust and brake emissions in a real case study by proposing an innovative methodology. For this purpose, different levels of study have been introduced, including the subsystem level, the system level, the environmental level and the suprasystem level. To address these levels, two approaches were proposed along with a data collection process. First, a comprehensive field survey was conducted in the area of Buonconsiglio Castle and data was collected on traffic and non-traffic during peak hours. Then, in the first approach, a state-of-the-art simulation-based method was presented to estimate the amount of exhaust emissions generated and the rate of fuel consumption in the case study using the VISSIM traffic microsimulation software and Enviver emission modeler at the suprasystem level. In order to calculate the results under different mobility conditions, a total of 18 scenarios were defined based on changes in vehicle speeds and the share of heavy vehicles (HV%) in the modal split. Subsequently, the scenarios were accurately modelled in the simulation software VISSIM and repeated 30 times with a simulation runtime of three hours. The results of the first approach confirmed the simultaneous effects of considering vehicle speed and HV % on fuel consumption and the amount of exhaust emissions generated. Furthermore, the sensitivity of exhaust emissions and fuel consumption to variations in vehicle speed was found to be much higher than HV %. In other words, the production of NOx and VOC emissions can be increased by up to 20 % by increasing the maximum speed of vehicles by 10 km/h. Conversely, increasing the HV percentage at the same speed does not seem to produce a significant change. Furthermore, increasing the speed from 30 km/h to 50 km/h increased CO emissions and fuel consumption by up to 33%. Similarly, a reduction in speed of 20 or 10 km/h with a 100% increase in HV resulted in a 40% and 27% decrease in exhaust emissions per seat, respectively. In the second approach, a novel methodology was proposed to estimate the number of brake particles in the case study. To achieve this goal, a downstream approach was proposed starting from the suprasystem level (microscopic traffic simulation models in VISSIM) and using a developed mathematical vehicle dynamics model at the system level to calculate the braking torques and angular velocities of the front and rear wheels, and proposes an artificial neural network (ANN) as a brake emission model, which has been appropriately trained and validated using emission data collected through more than 1000 experimental tribological tests on a reduced-scale dynamometer at the subsystem level (braking system). Consideration of this multi-level approach, from tribological to traffic-related aspects, is necessary for a realistic estimation of brake emissions. The proposed method was implemented on a targeted vehicle, a dominant SUV family car in the case study, considering real driving conditions. The relevant dynamic quantities of the targeted vehicle (braking torques and angular velocities of the wheels) were calculated based on the vehicle trajectory data such as speed and deceleration obtained from the traffic microsimulation models and converted into braking emissions via the artificial neural network. The total number of brake emissions emitted by the targeted vehicles was predicted for 10 iterations route by route and for the entire traffic network. The results showed that a large number of brake particles (in the order of billions of particles) are released by the targeted vehicles, which significantly affect the air quality in the case study. The results of this dissertation provide important information for policy makers to gain better insight into the rate of exhaust and brake emissions and fuel consumption in metropolitan areas and to understand their acute negative impacts on the health of citizens and commuters.

Muscle Strength, Acute Resistance Exercise, and the Mechanisms Involved in Facilitating Executive Function and Memory

<p dir="ltr">Past research has extensively explored the benefits of acute aerobic exercise (AE) on memory and executive functions. Additionally, the cross-sectional relationship between muscle strength – a direct outcome of RE – and cognition is unknown, despite the simultaneous onset of muscle and cognitive decline in one’s thirties. However, the effects of acute resistance exercise (RE) on cognition remain understudied, despite the growing popularity of RE and evidence that RE may have distinct effects on cognition.. Therefore, the present study aimed to broaden our understanding of the connection between muscle strength and hippocampal-dependent memory and to investigate the influence of RE on memory and executive function.</p><p dir="ltr">A sample of 125 healthy young adults (18-50 years old) completed this study. On the first day of testing, subjects completed a cognitive battery testing aspects of hippocampal dependent memory, spatial abilities, and working memory, a maximal muscle strength testing session including handgrip strength and one-rep-max testing, and maximal aerobic capacity testing. Subjects completed a bioelectrical impedance assessment (BIA) body scan to measure body composition on Day 2. Day 3 consisted of a randomized controlled trial (RCT), where subjects completed either 42 minute moderate intensity RE (n = 62) or a seated rest (n = 61). Cognitive testing including a memory recognition task, an inhibitory control task, and a working memory task were performed both before and after the intervention. Subjects also completed lactate, blood pressure, and blood draw (only a subset of subjects (n = 59)) before and after intervention.</p><p dir="ltr">The results first revealed that after controlling for known covariates, those with greater handgrip strength performed better on mental rotation tasks (t = 2.14, p = 0.04, Δr2= 0.04), while those with higher upper-body relative strength did better on recognition (t = 2.78, p = 0.01, Δr2 = 0.06) and pattern separation (t = 2.03, p = 0.04, Δr2= 0.04) tasks. Further, while there was no acute effect of RE on memory performance, response times during measures of inhibitory control (t = 4.15, p < 0.01, d = 0.40) and working memory decreased after exercise (t = 7.01, p < 0.01, d = 0.46), along with decreases in P3 latency during the inhibitory control task (t =-5.99, p < 0.01, d = 0.58). Additionally, blood lactate (t =-17.18, p < 0.01, d = 2.06), serum brain derived neurotropic factor (BDNF) (t = -4.17, p < 0.01, d = 0.66), and systolic blood pressure (t = -10.58, p < 0.01, d = 0.99) all increased following RE, while diastolic blood pressure (t = 4.90, p < 0.01,d = 0.50) decreased. Notably, the change in systolic blood pressure (t = -2.83, p = 0.01, Δr2 = 0.06) was associated with improvements in behavioral measures of inhibitory control, changes in lactate (t = -2.26, p = 0.03, Δr2 = 0.04) and systolic blood pressure (t = -3.30, p < 0.01, Δr2 = 0.08) were also related to improved behavioral changes in working memory, and changes in lactate (t = -3.31, p < 0.01, Δr2= 0.08) and BDNF (t = -2.12, p = 0.04, Δr2= 0.08) related to faster P3 latency during inhibitory control. Importantly, these associations between physiological and cognitive changes were consistent across both exercise and rest groups, suggesting that physiological changes were linked to improved cognitive performance regardless of group assignment.</p><p dir="ltr">In conclusion, this study highlights the positive relationships between cross-sectional muscle strength and aspects of memory and spatial abilities, with distinct contributions from handgrip and upper body strength. Furthermore, acute RE was shown to enhance executive functions, particularly in terms of processing speed during inhibitory control (response time and P3 latency) and working memory (response time). This study suggests that RE can be a valid way to garner exercise-induced benefits on executive functions potentially through its influence on lactate, BDNF, and blood pressure, however, since these effects were evident regardless of intervention, more work is needed to determine if RE-induced changes have the same mechanisms. Overall, these findings underscore the potential benefits of muscle strength and RE on enhancing executive function in young and middle-aged adults.</p>

Intresseavvägningar för planprojekt - inom Stockholms innerstad / Balancing of interests for planning projects - within Stockholm's inner city

I denna uppsats studeras hur avvägningarna mellan allmänna och enskilda intressen hanteras avde dömande organen och hur de olika sakägarna resonerar utifrån de olika intressena. Arbetet har genomförts som en fallstudie där fyra olika byggprojekt studerats. Gemensamt för alla är att de ligger i Stockholms innerstad och har mött motstånd i form av överklaganden. Det finns lite olika definitioner för vad allmänintresset är, somliga menar att allmänintresset är ensammanslagning av flera enskilda intressen medan andra definierar att det bestäms av folkets valda representanter. Det ideologiska tillvägagångsättet som diskuteras är utilitarism, enhetligt, rättighetsbaserad ochdialogbaserad. För att tolka domstolarna domslut används fjäder- och balansvågsmodellen där fjädervågsmodellen symboliserar gränsen för vad man som sakägare är skyldig att tåla. Balansvågsmodellen symboliserar det beslut som tas baserat på de sammanvägda för- och nackdelarna. Det första fallet är kvarteret Plankan 24 där Svenska Bostäder vill bygga nya bostäder i den oexploaterade innergården, planen har mötts av kritik från de boende som menar att luftkvaliteten, bullernivåerna, ljusinsläpp, rekreationsytor, barnens miljö och kulturmiljön avsevärt kommer att försämras. Mark och miljööverdomstolen anser att de skäl som de klagande åberopat inte är tillräckliga för att planen inte ska genomföras. Fall nummer två handlar om Orgelpipan 6, en hotellbyggnadskomplex och en av citybanans uppgångar mittemot centralstationen. De klagande menar bland annat att ett nytt hotell kommer att skapa obalans på en redan obalanserad marknad, att ljusförutsättningarna kommer att försämras och att byggnaden kommer att för skada Stockholm. Länsstyrelsen anser inte att deklagomål som kommit in är tillräckliga för att planen inte ska vinna laga kraft. Det tredje fallet handlar om Mårtensdal 6, ett kontorskomplex som ska bli 125 meter högt. De klagande är befintliga hyresgäster som anser att deras verksamhet inte ska behöva flytta och Fortum Värme AB som anser att höjden på byggnaden riskerar att inskränka deras verksamhet pga. att den är högre än deras egen skorsten. Mark- och miljööverdomstolen beslutar att Fortums överklagande bör accepteras och upphäver därför planen pga. av allmänintresset. Det sista fallet är byggandet av Nobel Center på Blasieholmen bakom Nationalmuseum. De klagande är gemensamt emot planen då det kommer påverka riksintresset och kulturmiljön. I Mark- och miljödomstolens domslut går man på de klagandes linje och upphäver planen för att planen innebär påtaglig skada på kulturmiljön och riksintresset. Den ideologi som tycktes vara den som tillämpades mest var en blandning mellan utilitarismenoch enhetliga. Den rättighetsbaserad ideologin användes inte alls och den dialogbaserade är något som används tidigare i planprocessen. Något entydigt samband i domstolarnasbedömningar har dock inte kunnat konstaterats, det kan bero på att varje fall är unikt och har dess egna förutsättningar. Kommunen och övriga sakägare resonerar på olika sätt när det kommer till avvägningar mellan allmänna och enskilda intressen. Generellt gör kommunen avvägningar genom ett utilitaristiskt och enhetligt tillvägagångsätt medan sakägarna använder olika tillvägagångssätt beroende påom de representerar ett företag, en förening eller är en privatperson. / This paper examines how the balances between public and private interests are handled by the appeals court and how the different stakeholder’s reason based on the different interests. The work has been carried out as a case study in which four different construction projects were studied. Common to all is that they are in Stockholm's inner city and have met with resistance in the form of appeals. There are slightly different definitions for what the public interest is,some believe that the public interest is a combination of several individual interests, while others define that it is decided by the elected representatives of the people. The ideological approach under discussion is utilitarianism, unitary, rights-based and dialogical. To interpret the court's verdict, the dynanometer and beam scale model is used wherethe dynamometer model symbolizes the boundary of what you as a stakeholder are obliged toendure. The beam scale model symbolizes the decision that is made based on the weighted pros and cons. The first case is Kv. Plankan 24, where Svenska Bostäder wants to build new housing in the undeveloped courtyard, the plan has been met by criticism from the residents who believe that the air quality, noise levels, day light, recreation areas, children's environment and the cultural environment will deteriorate significantly.
