Identifying the health and supportive care needs of adolescent and young adult survivors diagnosed with cancer: a mixed methods study

<p><strong>Purpose: </strong>Current cancer programs in Canada manage adolescents (15-18 years) under a pediatric model of care, and young adults (19-25 years) under a disease-centered model of care. Key issues arise because neither of these models adequately meets the needs of adolescents and young adults (AYA). The aim of this study is to identify health and supportive care (HSC) needs of AYA survivors with cancer. <strong>Method: </strong>A concurrent nested mixed-method design was employed. Qualitative description and a systematic literature review (SLR) were used. Medline, Cochrane Central, PubMed, PsycInfo CINAHL and EMBASE were searched from their date of inception to October 2011. A comprehensive search strategy was developed. 
A purposive sample of AYA survivors with cancer, aged 15-25 years were recruited. Face-to-face qualitative interviews were digitally recorded and transcribed verbatim. Line-by-line coding was used to establish codes and categories. 
<strong>Result: </strong>Our SLR retrieved 760 citations. Among them, 12 were relevant. Qualitative findings are based on 20 interviews. Twelve diagnosed during adolescence and eight in young adulthood. Participants described several HSC needs that we categorized into 7 broad themes (e.g., social health) and 29 sub-themes (e.g., social support). Among the most commonly reported HSC needs for AYA survivors with cancer from both studies, were social wellbeing, information-sharing and communication and service provision. <strong>Conclusion: </strong>This study has identified a comprehensive set of HSC needs of AYA survivors with cancer. Study results will be used to inform the potential development of a new healthcare program for AYA patients in the Hamilton Health Sciences.</p> / Master of Science (MSc)

Studies on arteriosclerotic pathologies, haematology, immunology and lipids of captive Atlantic bluefin tuna

Commercial capture-based aquaculture of the Atlantic bluefin tuna (ABT), Thunnus thynnus (L.), has been prominent in the Mediterranean for over a decade. Owing to several limitations encountered in working with the species, including its high commercial value, there has been little research carried out relating to this species. The objective of this study was to examine several health parameters of captive ABT. These included an examination of coronary artery lesions, haematology, plasma biochemistry, assessment of immune function and changes in fatty acid (FA) flesh content through the on-growing period. Arteriosclerosis in fish is a pathologic condition of uncertain etiology and involves the main coronary artery in teleosts. Apart from reports of their widespread occurrence in salmonids, they have been described from a restricted number of wild ABT specimens but have not received further attention. This investigation analysed the effect of size and period of net-pen rearing on the prevalence and severity of arteriosclerotic lesions in ABT. Coronary arteries from wild and captive fish were investigated and prevalence was 100 %, but increasing structural degradation was observed with increasing fish size, suggesting that lesions progress throughout the life of the fish. Due to the limited availability of wild specimens, the effect of captivity on arteriosclerosis in ABT could not be adequately quantified, although observations suggest that the farming process has no major effect on arteriosclerotic lesions in ABT. Studies on the haematology, plasma biochemistry and immunology of ABT are limited. Haematological and plasma biochemical indices are useful in animal health assessment but use of these requires the establishment of species-specific ranges. Blood was collected from captive ABT specimens of varying weight (61-361 kg) and the major haematological (n = 45), plasma biochemical (n = 30) and immunological parameters (n = 45) were quantified. Size-based differences were found in haematological indices between experimental sub-groups including increased erythrocyte number and haemoglobin level in smaller ABT. No differences were found in immunological parameters except for total IgM levels, which were higher in the smaller individuals. Preliminary investigations indicated that disease prevalence in captive ABT is very low. Epidermal mucus is an important interface between fish and their environment and comprises immunological components which act as a first barrier against pathogen entry or colonisation. Mucus was collected from captive ABT and analysed for innate immune components. The presence of IgM was detected in the mucus of ABT by an enzyme-linked immunosorbent assay and several different enzymes were detected with an API-ZYM kit assay. Zymography experiments confirmed the presence of protease-like enzymes in the mucus, while enzyme assays quantified alkaline phosphatase, protease, esterase and cathepsin B activities. Lysozyme levels were high. The mucus agglutinated sheep erythrocytes but did not demonstrate complement or bacteriolytic activity. There is restricted information on the fatty acid composition of farmed ABT or how this is influenced when the fish are held under commercial aquaculture conditions. This study investigated the FA composition of farmed ABT, its variation by dorsal muscle region and the correlation between dietary FA composition with that of the fish. Analysis of flesh samples retrieved from farmed ABT did not reveal significant differences in the FA composition of experimental sub-groups irrespective of size, time held in captivity or diet. These results indicate that FA metabolism in ABT is substrate-selective. Gene expression measurements from several organs of ABT showed that expression of Δfad5 and elovl5, genes involved in FA metabolism, were highest in the brain followed by the liver but no expression of these genes was detected in the spleen. The findings of this research address aspects of health evaluation and nutritional status in farmed ABT and are discussed in terms of farming practice. Conclusions from some of these studies suggest that the practice of holding wild-captured stock in cages for periods of up to 18 months does not result in significant impact on ABT.

Developing a framework for an undergraduate haematology curriculum in a Faculty of Health Sciences

Thesis (PhD (Curriculum Studies))--University of Stellenbosch, 2010. / ENGLISH ABSTRACT: The Faculty of Health Sciences at Stellenbosch University adopted a new set of guidelines for curriculum design in 1997, emphasising an orientation towards the requirements of the public sector general practice, a holistic approach and exposure to community lifestyle and disease patterns specific to various communities. In order to ensure the anchoring in the realities of the general practice, a family medical practitioner, appointed by the Academy of Family Practice, was included in the curriculum control structure of the faculty. It was further recommended that a family medical practitioner should be included in the curriculum committee of each discipline, where appropriate. The present research, starting from the assumption that the opinion of a single family practitioner is insufficient to determine the adequacy of the curriculum for general medical practice, aimed to conduct a comprehensive needs analysis of all stakeholders in the undergraduate haematology training programme at the Faculty of Health, Stellenbosch University, and to compare the findings with the existing curriculum. To this purpose, the opinions of five adult medicine haematologists, ten paediatric haematologists, four laboratory haematologists, ten interns, fourteen students and twenty general practitioners were surveyed. An open-ended questionnaire on the usefulness of the haematology module for hospital and independent general practice was analysed, using the “coding technique” method. On this basis, a list of subjects was drawn and, using a Delphi method, the participants in the study were asked to rate their importance for practice. The answers to the open-ended questionnaires revealed a few overarching concepts, the most important being the need to structure the material taught in the form of “approaches”, supporting the differential diagnosis, which is the most frequent task of a general practitioner. Among the outcomes identified in the panellists‟ answers, the need to adequately detect and assess the “red flag” signs for haematological cancers was proposed for consideration as an outcome in the next curriculum. The Delphi survey indicated a group of subjects which were rated as most important for practice and another group designated as devoid of utility. The remaining subjects, rated as of moderate importance, could be further classified as diseases usually managed by the general practitioner and pathology which would be referred to a specialist for management. These iv findings were compared with the existing curriculum and the discrepancies were analysed, resulting in a set of proposals towards a framework for a new undergraduate haematology curriculum. For the first time in the literature, as far as can be determined, this research presents outcomes and content for an undergraduate haematology course which were defined and rated for importance by consensus of the curriculum developers, specialists in the field and graduates of the course. The methods tested in this process and some of the trends revealed might be useful for curriculum development in other medical disciplines. / AFRIKAANSE OPSOMMING: Die Fakulteit van Gesondheidswetenskappe by die Universiteit Stellenbosch het in 1997 nuwe riglyne vir kurrikulumontwerp aanvaar. Hierdie riglyne beklemtoon `n bewustheid van die behoeftes van algemene praktyk in die openbare sektor, `n omvattende benadering tot en blootstelling aan die gemeenskapslewenstyl, asook aan siektepatrone eie aan verskillende gemeenskappe. Om te verseker dat die kurrikulum in die werklikhede van algemene praktyk geanker bly, is `n algemene praktisyn, aangestel deur die Akademie van Huisartskunde, ingesluit in die kurrikulum beheerstruktuur van die fakulteit. Dit is verder ook aanbeveel dat, waar van toepassing, `n huisarts in die kurrikulumkomitee van elke dissipline ingesluit moet word. L’évaluation du raisonnement clinique des résidents en hématologie par l’approche de concordance de script

La pratique de l’hématologie, comme celle de toute profession, implique l’acquisition d’un raisonnement adéquat. Se basant sur une théorie de psychologie cognitive, le test de concordance de script (TCS) a été développé et validé comme un instrument permettant d’évaluer le raisonnement clinique dans diverses spécialités médicales. Le but de cette étude était d’examiner l’utilité et les paramètres psychométriques d’un TCS en hématologie. Nous avons construit un TCS composé de 60 questions que nous avons administré à 15 résidents juniors (R1 à R3 en médecine interne), 46 résidents séniors (R4, R5 et R6 en hématologie) et 17 hématologues à travers le Canada. Après optimisation, le TCS comptait 51 questions. Sa consistance interne mesurée par le coefficient de Cronbach alpha était 0.83. Le test était en mesure de discriminer entre les résidents selon leur niveau de formation. Les questions contenant des images (n=10) semblaient avoir un potentiel discriminatoire plus élevé. Les scores obtenus par les résidents séniors corrélaient modéremment avec ceux obtenus à un test conventionnel d’hématologie composé de questions à choix multiples et à courte réponse (r de Pearson = 0.42; p=0.02). Le TCS a été complété en 36 minutes en moyenne et a été bien reçu par les participants. Le TCS est un instrument d’évaluation utile et valide en hématologie. Il peut être utilisé à des fins formatives en aidant au suivi de la progression des résidents. Il pourrait aussi être combiné à d’autres instruments d’évaluation à des fins sanctionnelles, ou encore, en éducation médicale continue. / The practice of hematology, like any other profession, requires the acquisition of adequate judgment. Based on cognitive psychology theory, the script concordance test (SCT) has been developed and validated as an instrument capable of evaluation clinical judgement in various medical specialties. The goal of this study was to examine the usefulness and the psychometric qualities of the SCT in hematology. We constructed a SCT composed of 60 questions and we administered it to 15 junior residents (R1 to R3 in internal medicine), 46 senior residents (R4, R5 and R6) and 17 hematologists from across Canada. After item optimization, the test comprised 51 questions. Its internal consistency measured by Cronbach alpha was 0.83. The test was able to discriminate between residents according to their year of training. Questions containing an image (n=10) seemed to offer a stronger discriminative potential. Scores obtained by the senior residents correlated moderately with those obtained on a conventional hematology exam made of multiple choice questions and short-answers (Pearson r: 0.42; p=0.02). The SCT was completed in an average of 36 minutes and was well received by participants. The SCT is a useful and valid evaluation instrument in hematology. It may be used during training to monitor resident progression. It may also be combined to other evaluation tools and used for summative purposes or in continuing medical education.

Natural history of the pichi (Zaedyus pichiy) in Mendoza Province, Argentina

The pichi Zaedyus pichiy (Xenarthra, Dasypodidae) is a poorly known, diurnal armadillo inhabiting arid and semi-arid regions of Argentina and Chile that has endured substantial population declines. My objective was to elucidate different aspects of the natural history of Z. pichiy as a first step towards establishing a conservation plan. Wild and captive pichis were studied. Body temperature of wild pichis averaged 35.2±1.2 °C and was highly variable (range 32.2 – 38.3 °C). Temperature measurements of semi-captive males showed that pichis can survive energetically challenging periods by entering hibernation or daily torpor. Stomach contents of poached animals revealed that pichis feed predominantly on insects but also ingest plant material, vertebrates and arachnids. This opportunistic, omnivorous feeding strategy allows them to thrive where food type and availability vary seasonally. The reproductive cycle of pichis was studied by means of histological and fecal hormone analyses. Pichis are seasonal breeders that produce one yearly litter of 1 to 2 offspring, and the initiator of breeding season seems to be an increase in daylength. The absence of regular estrous cycles and corpora lutea in non-pregnant females, and immediate mating attempts after pairing, all suggest that pichis are induced ovulators. Clinical examinations and hematological, serological and coproparasitological analyses of free-ranging pichis, and necropsies and histological examinations of confiscated pichis and roadkills, indicate that the populations are currently in good health. While parasites were often found, no severe pathologies were observed. Infections with potentially zoonotic diseases were rare: only a few pichis were seropositive for Trypanosoma cruzi, none had antibodies against Toxoplasma gondii, and none of the histologically examined individuals presented lesions attributable to these pathogens. Elevated ambient humidity levels often caused moist dermatitis with epidermal detachment in captive pichis. Poaching is currently considered to have a much higher negative impact on the wild populations than disease epidemics. Mortality due to heavy poaching activity may be difficult, if not impossible, to compensate by the current birth rates. This preliminary database on the natural history and reproduction of pichis will assist efforts to conserve this little-known species of armadillo.

A desnutrição é uma condição nutricional que pode afetar muitos aspectos da resposta imunológica, como alterações na migração celular, na fagocitose, na resposta bactericida, mudanças na produção de radicais livres e espécies de nitrogênio e na produção de citocinas pró-inflamatórias. Logo, indivíduos desnutridos apresentam maior susceptibilidade a infecções. Visto que a glutamina é um aminoácido de extrema importância para a funcionalidade de diversas células do sistema imune e que as mesmas apresentam aumento da utilização desse aminoácido durante processos infecciosos, investigou-se, neste trabalho, quais os efeitos da glutamina sobre alguns aspectos da mobilização, migração e sinalização celular em um modelo experimental de desnutrição proteica. Para tanto, utilizou-se camundongos da linhagem BALB/c machos, os quais foram divididos em dois grupos, Controle e Desnutrido, que passaram a receber dietas isocalóricas contendo 12% (normoproteica) e 2% de caseína (hipoproteica), respectivamente, durante 5 semanas. Para as avaliações in vivo, animais de ambos os grupos receberam por via endovenosa 100&#181;L de solução contendo 1,25&#181;g de LPS e após 1 hora 0,75mg/Kg de L-glutamina (GLUT). Após o período de desnutrição ou de indução ao processo inflamatório, os animais foram eutanasiados e as amostras biológicas coletas. Foram avaliados nos animais estimulados in vivo hemograma, mielograma, as citocinas IL-10 e TNF-&#945; circulantes e a expressão de CD11b/CD18 nos granulócitos do sangue periférico. Foi avaliado, in vitro, a capacidade migratória, a expressão de CD11b/CD18 de polimorfonucleados da medula óssea e do sangue periférico, bem como a síntese de citocinas IL-1&#945;, IL-6, IL-10, IL-12 e TNF-&#945; e a expressão de NF-&#954;B e I&#954;B&#945; em células cultivadas em meio com 0; 0,6; 2 e 10 mM de GLUT. Os animais desnutridos apresentaram anemia, leucopenia, hipoplasia medular e diminuição na concentração sérica de proteínas, albumina e pré-albumina. A GLUT, in vitro, apresentou capacidade de reduzir a produção de IL-1&#945; e IL-6, bem como a ativação da via do NF-&#954;B. No modelo in vivo a GLUT, em animais estimulados com LPS, alterou a cinética de migração neutrofílica e reduziu a expressão de CD18, bem como diminuiu os níveis de TNF&#945; circulantes. / Malnutrition is a nutritional condition that can affect many aspects of immune responses, affecting cell migration, phagocytosis, bactericidal response and changing free radicals production as nitrogen species and production of proinflammatory cytokines. Therefore, malnourished individuals are more susceptible to infections. Once glutamine is an amino acid of extreme importance to the functionality of various immune cells and those cells exhibit increased use of this amino acid during infectious processes. In this work was investigated, the effects of glutamine in some aspects of mobilization, cell migration and signaling in an experimental model of protein malnutrition. For this purpose, we used BALB/c mice, which received isocaloric diets, normoproteic or hypoproteic, containing respectively, 12% (Control group) and 2% (Malnourished group) of protein for a period of 5 weeks. The animals in both groups, for in vivo evaluations, received intravenous 100 &#181;l of a solution containing 1.25&#181;g of LPS and after 1 hour 0.75mg/kg of L-glutamine (GLUT). After the malnutrition period or the inflammatory process induction, the animals were euthanized and biological samples were collected. Were evaluated blood count, bone marrow, the cytokines IL-10 and TNF-&#945; circulating and expression of CD11b/CD18 in granulocytes from peripheral blood of animals stimulated in vivo. In vitro were evaluated the migratory capacity, the expression of CD11b/CD18 polymorphonuclear bone marrow and peripheral blood, as well as the cytokines synthesis IL-1&#945;, IL-6, IL-10, IL-12 and TNF-&#945; and the expression of NF-&#954;B and I&#954;B&#945; in cultured cells in media with 0; 0.6; 2 and 10 mM GLUT. Malnourished animals presented anemia, leukopenia, marrow hypoplasia and lower serum proteins, albumin and prealbumin. The GLUT in vitro has the capacity to reduce IL-1&#945; and IL-6 as well as the activation of the NF-&#954;B. In in vivo model, the GLUT altered neutrophil migration kinetics and reduced the expression of CD18, as well as decreased levels of circulating TNF-&#945; in animals stimulated with LPS.

Avaliação imunológica de vacina de polissacarídeo meningocócico C conjugado e encapsulado em lipossomo / Immunological evaluation of meningococcal polysaccharide C conjugate vaccine and encapsulated liposome

A tecnologia de conjugação melhorou a imunogeniddade de vacinas constituídas de polissacarídeo, especialmente em crianças. Polissacarídeos conjugados a proteínas carregadoras, como o toxóide tetânico, induzem resposta imunológica dependente de célula T e memória imunológica de longa duração. No entanto, esta tecnologia apresenta custo elevado. Assim, foi investigada a capaddade dos lipossomos de aumentar a resposta imunológica ao polissacarídeo C de Neisseria meningitidis (PSC). Camundongos foram imunizados com Iipossomos contendo PSC, conjugado toxóide tetânico-PSC ou PSC livre como controle, com dose de reforço constituída de PSC livre. Foram gerados anticorpos IgG e IgM contra PSC nos camundongos imunizados com conjugado ou Iipossomo. Os resultados mostram que lipossomos contendo PSC têm potencial para substituir a vadna conjugada toxóide tetânico-PSC. / Conjugation technology has improved the immunogenicity of polysaccharide vaccines, specially in small children. Polysaccharides conjugated to various carrier proteins, e.g. tetanus toxoid, stimulate a T cell-dependent antibody response and induce a long-term immunological memory. However, protein-polysaccharide conjugation technology is expensive and this could constitute an important drawback. Thus, immunopotentiation of Neisseria meningitidis serogroup C polysaccharide (PSC) by use of liposomes as an alternative to protein-polysaccharide C conjugates was investigated. Mice were immunized with liposomes containing PSC or tetanus toxoid-PSC conjugate or free PSC as control and boosted with free PSC. Immunogenicity of these different preparations was compared with each other. Conjugate and liposome containing PSC induced both IgG and IgM antibodies against the polysaccharide. These results show that liposomes containing entrapped PSC have potential to be used as an alternative to tetanus toxoid-PSC conjugate vaccine.

O pacu, Piaractus mesopotamicus, é um caracídeo neotropical autóctone, de hábito alimentar onívoro, largamente utilizado na aqüicultura brasileira. No entanto, alguns aspectos da sua nutrição permanecem controversos ou desconhecidos. O presente trabalho visou determinar a exigência em proteína, energia e aminoácidos para juvenis de pacu por meio da avaliação de parâmetros de desempenho, composição corporal e hematológicos em dois ensaios distintos. No primeiro ensaio, juvenis de pacu (15.5±0.4 g) foram alimentados durante 10 semanas, duas vezes ao dia, com dietas contendo teores de proteína bruta (PB) entre 22 e 38% (incremento de 4%) e energia digestível (ED) entre 2600 e 3400 kcal kg-1 (incremento 200 kcal kg-1), em um delineamento totalmente aleatorizado, em esquema fatorial 5 × 5 (n=3). O ganho de peso (GP) e a taxa de crescimento específico (TCE) aumentaram significativamente com a elevação da PB dietética. A retenção de nitrogênio (RN) e a taxa de eficiência protéica (TEP) diminuíram (p<0,05) com o aumento da proteína das dietas em todos os níveis de ED testados. As concentrações de ED afetaram (p<0,05) a concentração de umidade, proteína, gordura, matéria mineral e energia bruta corporal e o fator de condição. A taxa de eficiência econômica foi influenciada pela proteína (p<0,05) e energia digestível (p<0,05) das dietas e pela interação entre ambas (p<0,05). Entretanto, o índice de lucratividade econômica somente foi influenciado pela concentração de proteína bruta (p<0,05) das dietas. Efeito significativo (p<0,05) dos tratamentos nos parâmetros hematológicos foi registrado para taxa de hemoglobina, concentração de hemoglobina corpuscular média, proteína plasmática total e glicose plasmática. A exigência mínima para ganho de peso de juvenis de pacu é 27% de PB, com relação PB:ED ótima de 92,9 mg PB kcal-1 ED. As concentrações protéicas e energéticas estudadas não causaram prejuízos a saúde dos peixes. A exigência em lisina dietética para juvenis do pacu Piaractus mesopotamicus, com peso inicial de 4,3 g, foi determinada fornecendo cinco dietas experimentais isonitrogenadas (32% proteína bruta) contendo caseína, gelatina e L-aminoácidos cristalinos com níveis crescentes de lisina (0,90; 1,17; 1,44; 1,69 e 1,96% da matéria seca da dieta) por 74 dias. Cada dieta foi aleatoriamente designada para grupos (n=4) de 18 peixes alimentados três vezes ao dia até a aparente saciedade. Não foram observados mortalidade ou sinais de deficiência além da redução no desempenho dos peixes alimentados com dietas deficientes ou com excesso de lisina. O peso final (PF), GP, TCE, índice de eficiência alimentar (IEA), TEP, VPP, composição corporal, morfometria e hematologia foram afetados (p<0,05) pelas concentrações de lisina dietética. A análise polinomial quadrática indicou a exigência em lisina em 1,45, 1,51 e 1,43% da matéria seca da dieta, respectivamente para GP, IEA e VPP. Foram obtidos valores de lipídios e proteína corporais significativamente mais altos nos peixes alimentados com as dietas com 0,9 e 1,96% de lisina. Foi então determinado que a exigência em lisina para juvenis de pacu é de 1,5% da matéria seca da dieta. / Pacu, Piaractus mesopotamicus, a neotropical, omnivorous native characin, widely used in Brazilian aquaculture. However, some aspects of your nutrition are controversial or ignored. This work aimed at determining dietary protein, energy and amino acids requirements of juvenile pacu through the evaluation of performance, body composition and hematological parameters. In a first trial, pacu juveniles (15.5±0.4 g) were fed twice a day for 10 weeks until apparent saciety with diets containing crude protein (CP) from 22 to 38 % (intervals of 4%) and digestible energy levels among 2600 and 3400 kcal kg-1 (intervals of 200 kcal), in a totally randomized experimental design, 5 × 5 factorial scheme (n=3). Weight gain (WG) and specific growth rate (SGR) increased significantly with amount dietary CP. Nitrogen retention (NR) and protein efficiency ratio (PER) decreased (p<0.05) with increasing dietary protein at all tested levels of dietary energy. DE levels affected (p<0.05) whole body moisture, protein, fat, ash, gross energy and condition factor. Economic efficiency ratio was influenced by dietary protein (p<0.05) and digestible energy (p<0.05), and by interaction between the two factors. However, income was affected by dietary protein level (p<0.05) alone. Significant effect of treatments in hematological parameters were registered for mean corpuscular hemoglobin concentration, total plasma protein and plasma glucose. Crude protein requirements and optimum protein:energy ratio for weight gain of juveniles of pacu were 271 g kg-1 and 92.9 mg kcal-1, respectively. All dietary crude protein and digestible energy levels studied did not pose harms to fish health. Dietary lysine requirement of juvenile pacu Piaractus mesopotamicus (4.3 g) was determined by feeding five isonitrogenous (32% crude protein) amino acid test diets containing casein, gelatin and Lcrystalline amino acids with graded levels of lysine (0.90, 1.17, 1.44, 1.69 and 1.96% of dry diet) for 74 days, three times a day until apparent satiation, to groups (n=4) of 18 fish in a totally randomized design trial. No mortality or nutritional deficiency signs were observed; reduced growth was recorded for fish fed diets with either the lower or the higher lysine contents. Final weight (FW), WG, SGR, feed efficiency (FE), PER, NR, proximate whole body composition, morphometry and hematology were affected (p<0.05) by dietary lysine concentrations. Seconddegree polynomial regression analysis of WG and PER data indicated dietary lysine requirement of 1.45, 1,51 and 1.43% of dry diet, respectively to WG, FE and PPV. Significantly higher lipid and protein contents values were obtained for whole body of fish fed the diets with 0.9 and 1.96% of lysine. Lysine requirement of juvenile pacu was determined as being 1.5% of dry diet.

Estudo sobre métodos de avaliação da anemia ferropriva em bezerros neonatos / Study of evaluation methods of iron deficiency anemia in newborn calves

A zinco protoporfirina (ZPP) eritrocitária é um metabólito formado pela adição do zinco no sítio do ferro durante a formação da molécula de hemoglobina, quando este último está total ou parcialmente indisponível. O objetivo deste trabalho foi realizar a padronização dos valores da ZPP eritrocitária em bezerros sadios com até um mês de vida, assim como a validade da ZPP como previsora da ocorrência de anemia ferropriva em bezerros neonatos, em comparação com outros métodos. Para tanto foram utilizados 134 bezerros da raça Holandesa, com idades variando do nascimento até 30 dias de vida, provenientes de fazendas produtoras de leite localizadas no Estado de São Paulo, classificados como sadios (67 animais) e anêmicos (67 animais). Os animais foram monitorados por meio de exames físico e complementares (hemograma e reticulocitometria, teores de ferro sérico (FT) e capacidade total de ligação do ferro (CTLF), teores de bilirrubinas e de uréia séricas, e concentrações da ZPP). Durante a padronização do exame, os valores médios encontrados foram: concentração de ZPP eritrocitária das amostras de hemácias não lavadas em até 3 horas após a colheita de sangue de 30 animais sadios - 80,90 &#181;mol ZPP/mol heme; concentração de ZPP eritrocitária, após a lavagem de hemácias, determinadas até 3 horas e 12 horas após colheita de sangue do mesmo grupo - 61,40 &#181;mol ZPP/mol heme e 61,03 &#181;mol ZPP/mol heme, respectivamente. Com base nos resultados obtidos, foi possível concluir que as amostras de sangue colhidas para a mensuração da ZPP podem ser armazenadas, sob refrigeração a 4°C, por até 12 horas após a colheita, sem alterações significativas dos seus valores, sendo recomendável a lavagem das hemácias antes da mensuração dos valores da ZPP eritrocitária devido à presença de substâncias interferentes no plasma do animal. Foi encontrada diferença significativa nas concentrações da ZPP e de todos os componentes do eritrograma, assim como nos teores do metabolismo de ferro entre os animais anêmicos e sadios. As concentrações de bilirrubinas e ureia séricas apresentaram-se no intervalo fisiológico de variação, não interferindo na mensuração da ZPP nos eritrócitos dos bezerros com ou sem anemia. A correlação entre os valores encontrados dos teores de ferro sérico e da capacidade total de ligação do ferro com as concentrações de ZPP foram: rs = - 0,45, p < 0,001 (ZPP x FT) e rs = 0,51, p < 0,001 (ZPP x CTLF), respectivamente, demonstrando que quanto menores os teores de ferro, maiores serão as concentrações de ZPP. Portanto, a utilização da ZPP como previsora dos quadros de anemia ferropriva em bezerros neonatos com até um mês de vida se mostrou válida e, considerando que a hematofluorometria é um método rápido e não oneroso, pode ser recomendado como exame complementar de rotina. / The zinc protoporphyrin (ZPP) is a metabolic originated by zinc addition in the iron site during the synthesis of hemoglobin molecule, when iron is total or partially unavailable. The aim of this study was to standardize the values of zinc protoporphyrin (ZPP) in erythrocyte of healthy calves until one month of life, and to evaluate this determination as a predictor of the occurrence of iron deficiency anemia in newborn calves, compared with other methods. Therefore, 134 Holstein calves were used, aged from birth to 30 days of life, from dairy farms located in the São Paulo State, splitted into two groups, classified as healthies (67 animals), and anemics (67 animals). The animals were monitored by physical examination and laboratory assessments (complete blood cell count and reticulocytes, serum iron (SI) and total iron binding capacity (TIBC), levels of serum bilirubin and urea, and ZPP levels). During the exam standardization, the mean values found were: erythrocyte ZPP of unwashed erythrocytes within 3 hours after blood sampling of 30 healthy animals - 80.9 &#181;mol ZPP/mol heme; erythrocyte ZPP after washing of red blood cells, determined by 3 hours and 12 hours after blood collection, from the same group were 61.40 &#181;mol ZPP/mol heme and 61.03 &#181;mol ZPP/mol heme, respectively. Based on these results, it was concluded that blood samples taken for measurement of ZPP may be stored under refrigeration at 4 °C for up to 12 hours, without significant changes of the values of erythrocyte ZPP, and it is also advisable to wash the red cells before erythrocyte ZPP values measuring, due to the presence of interfering substances in the animal plasma. A significant difference was found in the ZPP levels and in all erythrogram components, as well as on the levels of iron metabolism between anemic and healthy animals. The bilirubin and serum urea levels remained within physiological variation, does not interfering with the measurement of erythrocyte ZPP of calves with or without anemia. The correlation values between SI and TIBC with ZPP levels were: rs = - 0.45, p < 0.001 (ZPP x SI) and rs = 0.51, p < 0.001 (ZPP x TIBC), respectively, i.e. decreasing biochemical levels of iron metabolism will increase levels of ZPP. Therefore, the use of ZPP as an iron deficiency anemia predictor in newborn calves up to one month of life has proven its validity, and considering hematofluorometry as a quick and inexpensive method, it might be recommended as a routine exam.

Combating Stress: The Use of Isoflavones as Nutraceuticals to Improve Immunity and Growth in Nile Tilapia (<i>Oreochromis niloticus</i>)

Stressors in the aquaculture environment can lead to negative impacts on growth and immune health, resulting in susceptibility to infectious diseases. These stressors are expected to increase as the growth of aquaculture continues to rise to meet demands for quality fish protein. Isoflavones, as a crude extract or as a pure isolate, may be effective in modulating the stress response, promoting growth and immunity. The objective of these studies was to examine the effect of various pure isoflavone isolates and crude isoflavone extracts on stress, growth, and immunity. Nile tilapia (Oreochromis niloticus) were stressed by adding hydrocortisone to the feed. In a 7-week study, pure isoflavone isolates of genistein and puerarin were evaluated to determine their respective effects on stress, growth, and immunity. A separate 10-day physiological and 6-week growth study focused on crude isoflavone extracts from kudzu (Pueraria lobata), red clover (Trifolium pratense), and soybean (Glycine max) was performed to determine their respective effects on stress, growth, and immunity. Numerous physiological parameters of the fish were measured (serum cortisol concentration, blood glucose concentration, hematocrit, hepatosomatic index, plasma protein concentration, lysozyme activity, and spleensomatic<br>index) to determine the effects of these pure isoflavone and crude isoflavone extracts on the modulation of stress and immunity. Many growth parameters were examined (length, weight, condition factor, weight gain, specific growth rate, feed intake, feed conversion ratio, and protein efficiency ratio) as well to determine the effects of these pure isoflavones and isoflavone extracts on growth. The addition of isoflavone and crude isoflavone extracts to the diet of Nile tilapia ameliorated some of the negative consequences of stress. Compared to stressed fish fed commercial feed, genistein and puerarin added to the diet appeared to improve serum cortisol concentrations, which resulted in increased plasma protein, albeit at different durations of stress. Puerarin, as well as all three crude isoflavone extracts, significantly increased spleen-somatic index compared to non-supplemented stressed fish, although the crude isoflavone extracts did not appear to improve serum cortisol concentrations. Crude isoflavone extracts also showed overall increases in lysozyme activity compared to non-supplemented stressed fish, although this was not significant. Genistein, puerarin, and red clover showed increased growth rates, feed conversion ratio, and protein efficiency. Overall, pure isolates of isoflavone appear to be more effective in modulating stress, immunity, and growth than the crude isoflavone extracts, although red clover extract showed promises in the ability to modulate the stress response and improve growth and immunity. There are likely substantial interactions between the isoflavones in the crude extracts that cannot be fully understood by measuring the effects of single isoflavones. Regardless, isoflavone supplementation (pure or crude) appeared to generally have an overall positive impact on stressed Nile tilapia, requiring more research to better understand the effects and mechanisms behind these isoflavones.
