Evaluating the impact of a sport-for-development intervention on the physical and mental health of young adolescents in Gulu, Uganda - a post-conflict setting within a low-income country

Richards, Justin A. January 2011 (has links)
Introduction: Physical inactivity is thought to contribute to the emergence of non-communicable diseases in post-conflict settings of low-income countries. Sport-for-development (SfD) organisations in these regions claim to improve the health of programme participants. However, there is a paucity of supporting evidence. I assessed the impact of a voluntary community-based SfD intervention on the physical activity (PA), physical fitness (PF) and mental health (MH) of adolescents in Gulu, Uganda. Methods: The Acholi Psychosocial Assessment Instrument (APAI), standing broad jump (SBJ), multi-stage fitness test (MSFT) and BMI-for-age (BFA) were adapted to the local context. I tested their feasibility and reliability with a repeat-measures design (n=70). A cross-sectional analysis of a random sample was used to assess the local needs and establish the PF and MH of the adolescents reached by the intervention (n=1464). This was also the baseline assessment for the impact evaluation. It comprised a randomised control trial (n=144) nested within a cohort study (n=1400) and triangulated by cross-sectional assessment of PA using accelerometry (n=54). Results: The adapted PF and MH measures demonstrated good intra-tester reliability (ICC>0.75). Adolescents in Gulu predominantly had “healthy” BFA (>90%). They performed better than global norms for the SBJ (p<0.001), but worse for the MSFT (p<0.05). The girls who registered for the intervention had higher PF at baseline (p<0.05) and experienced no significant benefits when compared to the community. The aerobic capacity of the boys intervention group increased relative to the community (p<0.01), but was not significantly different to the trial control group whose PF also improved. The PA results concurred with this finding. Boys in the intervention group experienced a deterioration in MH relative to their peers (p<0.05). Implications: It is feasible to apply rigorous evaluation methods to SfD interventions. Although adolescents in Gulu have poor aerobic capacity, a voluntary programme may not reach those at risk. Interpreting the impact evaluation was limited by a lack of programme development theory, but suggested that opportunities for non-competitive play may confer PF benefit without harming MH. Further investigation is warranted.

Legault, Anik 03 1900 (has links)
Despite the important role played by healthy behaviours during pregnancy, and their health implications for both mother and baby, several studies have highlighted that health information needs of low-income and ethnic minority pregnant women have not been met. Thus, the information-seeking behaviour of low-income pregnant Maghrebian women has been studied in this thesis by using the Engel, Kollat and Blackwell (EKB) model. The main objective of this thesis’s research is to explore information-seeking behaviours regarding nutrition-related topics during pregnancy in order to determine whether decisional processes regarding information-seeking behaviour differ among primigravid and secundigravid women. This thesis proposes four specific research questions aiming at the exploration of individual and environmental factors likely to influence the active information-seeking behaviour, the type of information and the sources from which such information is collected. Data has been collected among 28 low-income voluntary women (14 primigravid and 14 secundigravid) mainly referred by the Montreal Diet Dispensary between August 2010 and 2011. Semi-structured interviews lasting 60 to 90 minutes were conducted by the doctoral student. Data were analyzed using content analysis with NVivo software. On one hand, individual factors likely to influence the active information-seeking behaviour have been found to be: the ideological examination of pregnancy, presence of pregnancy discomforts, perception of conflicting information from providers, perceived adequacy of received nutrition information, perceived sufficiency of information, body weight changes, recognition of the baby’s well-being, attitudes toward prenatal supplementation, importance given to healthy eating, and motivation to learn. On the other hand, environmental factors likely to influence the active information-seeking behaviour have been identified to be: culture (eating habits, food beliefs, religious beliefs, loss of cultural markers regarding health care and counseling during pregnancy) and interactions with individuals from the social environment. Significant information sources consulted by primigravid women for topics regarding prenatal nutrition were as follows: the registered dietitian, mother, friends, husband, and Internet. For secundigravid women, significant information sources were as follows: the dietitian, doctor, friends and husband. A significant professional information source, both for primigravid and secundigravid women, about prenatal vitamin and mineral supplementation and pregnancy weight gain was the registered dietitian. Internet was a significant public information source from which primigravid women got information regarding prenatal nutrition-related topics. Few women reported having received, during their follow-up examinations, professional information related to food safety issues and risks of gaining too much weight during pregnancy. Overall, results presented in this thesis highlight the importance of improving communicating strategies in order to better address the information needs of low-income pregnant Maghrebian women. Results raise a number of questions with regards to improving the participation of professionals to prenatal education and their nutritional counseling. Moreover, results from this thesis suggest means to improve activity planning to better address the nutrition-related information needs of pregnant Maghrebian women during pregnancy. It would be useful for researchers to get involved in studies aimed at a better understanding of the information-seeking behaviour of expectant mothers from different cultural backgrounds regarding nutrition-related topics during pregnancy as well as the baby’s nutrition.

Vliv chudoby a sociálního vyloučení na vzdělávání žáků druhého stupně základní školy v regionu Most / The impact of poverty and social exclusion on the education of the pupils at the second grade of basic school in the Most region

Pulgrová, Michaela January 2014 (has links)
This diploma thesis deals with poverty and social exclusion in the context of education, on both a theoretical and empirical level. The theoretical part provides a comprehensive picture of the phenomena of poverty, social exclusion and low-income families and then shows them towards education. The theoretical basis of the protection of families and children is also clarified in this part. The text focuses in detail on the specifics of the schooling of students from low-income families in Most, this focus is then extended by a qualitative data survey in the practical part. The research report provides interpretation of data from interviews with primary school pupils in Most, whose families meet the poverty risk attributes. The results of the practical part capture the student's perception of their educational careers. The results of research interviews are then confronted with the conclusions of the research reports mentioned in the theoretical part. Powered by TCPDF (www.tcpdf.org)

Surviving birth : Studies of a simplified neonatal resuscitation protocol in a low-income context using a mixed-methods approach

Wrammert, Johan January 2017 (has links)
United Nations has lately stated ambitious health targets for 2030 in the Sustainable Development Goal agenda, following the already achieved progress between 1990 and 2015 when the number of children dying before the age of five was reduced by more than half. However, the mortality reduction in the first month of life after birth has not kept the same pace. Furthermore, a large number of stillbirths have previously not been accounted for. The aim of this thesis was to evaluate the impact of clinical training in neonatal resuscitation, and to identify strategies for an effective implementation at a maternal health facility in Nepal. Focus group discussions were used to explore the perceptions of teamwork among staff working closest to the infant at the facility. A prospective cohort study with nested referents was applied to determine effect on birth outcomes after an intervention with Helping Babies Breathe, a simplified protocol for neonatal resuscitation. Sustainability of the acquired skills after training was addressed by employing a quality improvement cycle. Video recordings of health workers performance were collected to analyse adherence to protocol. Midwives described the need for universal protocols in neonatal resuscitation and management involvement in clinical audit and feedback. There was a reduction of intrapartum stillbirth (aOR 0.46, 95% CI 0.32–0.66) and neonatal mortality within 24 hours of life (aOR 0.51, 95% CI 0.31–0.83) after the intervention. Ventilation of infants increased (OR 2.56, 95% CI 1.67–3.93) and potentially harmful suctioning was reduced (OR 0.13, 95% CI 0.09–0.17). Neonatal death from intrapartum-related complications was reduced and preterm infants survived additional days in the neonatal period after the intervention. Low birth weight was not found to be a predictor of deferred resuscitation in the studied context. This study confirmed the robustness of Helping Babies Breathe as an educational tool for training in neonatal resuscitation. Accompanied with a quality improvement cycle it reduced intrapartum stillbirth and mortality on the day of delivery in a low-income facility setting. Improved postnatal care is needed to maintain the gains in survival through the neonatal period. Increased management involvement in audit and quality of care could improve clinical performance among health workers.

Um roteiro para a criação de programa computacional dedicado à geração de pré-projeto arquitetônico de habitações sociais: realimentação de modelo a partir de avaliações pós-ocupação / A roadmap for the creation of dedicated software the generation of pre-architectural project of social housing: feedback model from Post-Occupancy Evaluations

Ferreira, Marcelo de Paula 17 April 2009 (has links)
The possibility of living in environments that have been sized on the basis of anthropometric studies and post-occupancy evaluations (POEs) of the functionality of buildings is the consequence of architectural design processes yet to be consolidated. This technology has enabled greater control over all stages in the construction of buildings, the pre-design stage being decisive for the success of an enterprise. The objective of the present study is to develop guidelines for designing a computer program for architectural designing and drafting (CAADD-Computer Aided Architectural Design and Drafting) that will serve as an aid to architects in terms of geometrical analyses of the functional dimensions of low-income housing during the pre-design stage, considering the specific aspects of the variables involved in each design. By choosing alternatives for programming the software, the system is set up to enable designers to generate digital models based on standards defined in the program itself. The digitally extracted volumes are treated generically in the system as volumetric objects (furniture, appliances, plumbing fixtures, door and window frames, structures, waterproofing and others). The open standard for distributing tridimensional contents, known as X3D, was chosen as the technology for digital graphic representation. The guidelines take into account concepts for graphic computing and computational geometry and apply criteria for the physical arrangement of living environments, relationships among environments, distribution of the apartments and analyses of the location of the buildings in their urban context. Dimensional references are used to establish these guidelines, including the results of POEs (classifying the data that changes most often over time as \"dynamic\"). Other factors that must be taken into consideration are the local laws, norms and regulations (classified as \"stable\" data, which change less often over time). The objective is to develop spatial improvements and continuous feedback of the approach of the design and functional architectural drafting in new housing designs.

Evaluating the quality of the national government self-help housing scheme in the Western Cape; before and after NHBRC involvement

Mayongo, Nwabisa January 2018 (has links)
Thesis (MTech (Business Administration in Project Management))--Cape Peninsula University of Technology, 2018. / According to Section 26 (1) of the constitution of Republic of South Africa, everyone has a right to have a satisfactory house to restore and honour the dignity of the South Africans. However South African government implemented several housing programmes to bridge the gap of housing backlog in South Africa. One of the housing programmes that South African government implemented is People Housing Process (PHP). It was approved in 1998 by South African government. South African government shifted focus on the quality of houses and mainly focused on the quantity of houses delivered through the financial year. There have been a lot of quality complaints on PHP. The quality defects are signs of foundation failures, cracks on foundations, water flooding around the houses, water not properly channelling to the drain, cracks on walls, dampness of walls, mould on walls, water seeping through the windows, poorly applied external plaster, incorrect bonding of internal walls to external walls, walls that are not straight walls, sagging ceiling panels, gable not properly filled with mortar, roof structure not properly tied up, sagging roof coverings, roof leaks, sagging roof tiles and ridges, rust on painted iron material, poor quality of blocks used, insufficient cement on mortar mix and peeling off paint. National Home Builders Registration Council (NHBRC) are the custodians of the home building industry. They were excluded from PHP from 1998 till March 2012. NHBRC was approved to inspect PHP house in April 2012. Thus, the aim of the study was to compare the houses that were built before NHBRC involvement in PHP with those that were built after NHBRC involvement in PHP. The sample included 50% of each of the two groups (those in houses built without NHBRC involvement and those built with NHBRC involvement), the research involved at least 50 respondents per group (McMillan, et al 2001:177 – recommends 15 respondents per group). The sample size per group has been put at 50 since the larger the sample the higher the accuracy. The study is classified as quantitative research because it intended to quantify the variation in occurrence, situation, problem or issue; the information was gathered using predominantly quantitative variables and the analysis was geared to ascertain the magnitude of the variation. The findings of the study revealed that the quality of the houses that were built under PHP programme before NHBRC intervention on PHP was not up to standard however the quality on those that were built after NHBRC involvement improved. Therefore it is recommended for Western Cape government to implement the rectification programme which was approved by National Department of Human Settlements in 2009 mainly focusing on houses that have been severely structurally compromised and are regarded as unfit for human habitation as it poses a threat to the health and safety of the occupants (The National Housing Code, 2009: 11-13).

This paradigm shift regards to how these settlements are handled since the introduction of Brazilian s new urban legal framework as a result of the 1988 Constitution and its consolidation with the promulgation of the Statute of the City in 2001. Through the analysis of the national doctrine, of the recent advances of federal legislation on urban law and of the conduct of public administration, we intend to demonstrate that this paradigm shift is due to the possibility of the beginning of a process to effect the principle of the social function of property. It also follows the establishing in our legal system of the general guidelines for urban policy, in particular the ones that constitute the right to sustainable cities, to democratic management of cities and to urbanization and land regularization of areas occupied by low-income population. These innovations operate profound changes in the Brazilian legal system, as they recognize, to the residents of informal settlements occupied by low-income population, a subjective right to land regularization. The materialization of this differential treatment stems from the possibility of establishing a special legal scheme for urbanization and land regularization, when these settlements are recognized as ZEIS by the Master Plan or other municipal law. We demonstrate the meaning of the new legal framework, especially of ZEIS, in a change in the way of using the zoning tool as an instrument for urban planning. We analyze the consequences of the general guideline of the urban policy for the democratic management of the city in the process of implementation of the ZEIS, as its special scheme determines the adoption of a democratic and participatory process. We also demonstrate the role played by ZEIS in the equilibrium between the right to the urbanistic order and the right to an ecologically balanced environment, establishing a special scheme for the application of environmental standards in processes of land regularization. Finally, this research examines aspects of the implementation process of ZEIS by municipalities, such as its regulation, preparation, approval and registration of the land regularization project and its urban control / Esta pesquisa tem como objetivo demonstrar o papel desempenhado pelas zonas especiais de interesse social (ZEIS) na materialização de um novo paradigma no tratamento de assentamentos informais ocupados por população de baixa renda.

No. of bitstreams: 1 Dissertacao Vera Lucia.pdf: 1113255 bytes, checksum: c33a0ac794586066a13b6d297c6be640 (MD5) Previous issue date: 2005-05-06 / This study aims at understanding the female condition withim an existencial phenomenological perspective from the psychoeducative view. We intended to discuss how the female conditions are lived and faced by a group a low-income women from the outs of the city of São Paulo and the meanings that are attributed by them. The female condition showed in the psychoeducative program, according to their life experiences was approached through a qualitative and hermeneutic method. The psychoeducative program as a room for specialized hearing and attention revealed itself as a locus of warmth and search for reflection for the ones who look for it. The focus in the educational practice, referring to the socializing, development and educational process had its center in the dialogical theory proposed by Paulo Freire and the phenomenological orientation by Hanna Arendt. Generally speaking, the offering of this new proposal of educational practice the psychoeducative program as a locus of specialized hearing, through the narratives of their life stories, contributed to reveal the female condition and the poverty context and violence they face. On the other hand, it was observed that other ways may be found through actions with other people engaged instead of the silence and isolation of their private settings. Other aspects considered in this study were the search of new meanings and senses through the comprehension of the dialogical situation, in the interpersonal changes among people who talked about their experiences and the ones responsible for the research interviews.
