Utilisation of sexual and reproductive health services by secondary school adolescents in Mochudi

The study sought to establish the utilisation of the available adolescent sexual and reproductive health services/facilities by Mochudi secondary school adolescents. The intent of the study was to determine the level of awareness among adolescents of the existing Adolescent Sexual and Reproductive Health Services/ Facilities in Mochudi and to determine those factors that affect utilisation of sexual and reproductive health services/facilities. A self administered questionnaire was used by the researcher to collect data from the respondents. Data analysis began with studying and coding of the responses from the questionnaires and was analysed using a copy of Excel software. The inferences made from this study are that secondary school adolescents in Mochudi indulge in pre-marital sex and most of them are aware of the presence of sexual and reproductive health services/facilities. However, the majority of the adolescents do not use the services/facilities either due to shyness, inaccessibility of clinics, lack of privacy, unfriendly staff, long waiting time or for others because they are not sexually active. The feedback from the adolescents is an indication of the inappropriateness of service delivery system pertaining to adolescent sexual and reproductive health in Mochudi. / Health Studies / M.A. (Health Studies)

Exploratory and descriptive study, generally aiming to investigate the nurse s training to care the Child and the Adolescent in nursing graduation courses in the Western Region of Paraná State. With specific goals: to analyze the scientific publishing to the nurse s training to Healthcare to the Child and Adolescent in Brazil, by means of an integrative review, to identify the dimension assumed by the nursing care in Child and Adolescent s health inside the politic and pedagogic projects and teaching plans of nursing graduation courses within the Western Paraná Region. To investigate the relationship established between the training to healthcare to Child and Adolescent in the Unified Health System, specially to what refers to the integrality principle, to identify in the Professor s Speech of the studied courses how it materializes the healthcare teaching to Child and Adolescent from the implementation of the Unified Health System and of the National Curricular Directives indications to the graduation course in nursing. There was carried out the Integrative Review, research of documental data and semi structured interview with the Professors responsible for the disciplines reserved for teaching the healthcare to Child and Adolescent. The integrative review indicated the pertinence of the integrality basing the training to healthcare to Child and Adolescent; the regard to the family and; the process learning-teaching with their elements composing the formation to health of Child and Adolescent. The documental data analysis and the interviews resulted in the following themes: pedagogical political projects and teaching plans; the materiality of the discourses about formation; the subjects and their involvement with the institutional reality; integrality in the training to the care of the nurse to healthcare of Child and Adolescent. They indicated that to the formation to the Unified Health System it must be proportioned effective living of the system. Since their first terms of training with equivalent workload between the theoretical and practical activities; the courses incorporate a theoretical outline and a speech of integrate formation; the professors exposed an approximation with a training to the Unified Health System internalizing concepts as promotion, protection, prevention, integrality and broad training. However, some effective proposes are distant to what is a solid training to that system, explicitly in the distancing of pedagogical actions in fields of practice immerging after the Curricular Directives. The integrality weakens in the training due to its absence in health services; by its precariousness of the practice field and by the lack of effective politics to solving the care to Child and Adolescent. / A formação profissional pode contribuir com a consolidação do Sistema Único de Saúde especialmente formando para a integralidade do cuidado. Estudo exploratório, descritivo, com o objetivo geral de investigar a formação do enfermeiro para o cuidado à saúde da criança e do adolescente em cursos de graduação em enfermagem na Região Oeste do Paraná. Como objetivos específicos: analisar as publicações científicas sobre a formação do enfermeiro para o cuidado à saúde da criança e do adolescente, no Brasil, por meio da Revisão Integrativa; identificar a dimensão assumida pelo cuidado de enfermagem em saúde da criança e do adolescente nos Projetos Político Pedagógicos e Planos de Ensino de cursos de graduação em enfermagem na Região Oeste do Paraná; investigar a relação estabelecida entre a formação para o cuidado em saúde da criança e do adolescente e o Sistema Único de Saúde, especialmente no que se refere ao princípio da integralidade; identificar na fala dos docentes dos cursos estudados como se materializa o ensino do cuidado à saúde da criança e do adolescente à partir da implementação do Sistema Único de Saúde e das indicações das Diretrizes Curriculares Nacionais para a graduação em enfermagem. Realizou-se Revisão Integrativa, pesquisa de dados documentais e entrevista semi estruturada com docentes responsáveis pelas disciplinas reservadas ao ensino do cuidado à saúde da criança e do adolescente. A revisão integrativa indicou a pertinência da integralidade fundamentando a formação para o cuidado à criança e ao adolescente; a consideração da família no cuidado e; o processo ensino-aprendizagem com seus elementos compondo a formação para o cuidado à saúde da criança e do adolescente. A análise dos dados documentais e das entrevistas resultou nas temáticas: projetos políticos pedagógicos e planos de ensino: a materialidade dos discursos sobre a formação; os sujeitos e seu envolvimento com a realidade institucional; integralidade na formação para o cuidado do enfermeiro à saúde da criança e do adolescente. Elas indicaram que uma formação para o Sistema Único de Saúde deve proporcionar efetiva vivência do sistema, desde os primeiros períodos da formação com carga horária equivalente entre atividades teóricas e práticas; os cursos incorporaram o arcabouço teórico e o discurso da formação integral; os docentes expuseram aproximação com uma formação para o Sistema Único de Saúde internalizando conceitos como promoção, proteção, prevenção, integralidade e formação generalista. Entretanto, algumas propostas efetivas estão distanciadas do que seja uma sólida formação para aquele sistema explícita no distanciamento de ações pedagógicas em campos de prática, emergente após as Diretrizes Curriculares. A integralidade se enfraquece na formação pela sua ausência nos serviços de saúde; pela precariedade dos campos de prática e pela falta de políticas efetivas para resolutividade do cuidado da criança e do adolescente.

Significações da revelação do diagnóstico para o adolescente que tem HIV/AIDS: possibilidades para enfermagem / Significations of diagnosis disclosure tothe adolescent that has HIV/AIDS: possibilities for nursing

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / The aim of this study was to understand the meaning of the revelation of the diagnosis of human immunodeficiency virus (HIV) and acquired immunodeficiency syndrome (AIDS) adolescents. Qualitative and phenomenological investigation, based on the theoretical-methodological referential of Martin Heidegger. Field step was developed from November 2011 to February 2012, at the Infectious Diseases Service of Santa Maria University Hospital (SMUH), Rio Grande do Sul, Brazil. As a way of access the being, we used phenomenological interview and medical records data collection. It contemplated 12 adolescents that have HIV/AIDS, from 13 to 19 years old. Field step was concomitant with the analysis which enabled to reach the meaning sufficiency. Ethical aspects were respected according the Resolution 196/1996. Data analysis was based on two heideggerian methodical moments: 1) vague and median comprehension pointed to the adolescent that diagnosis disclosure means to know it before anybody tell them, even without understand it, until the family and/or health professionals explain it. They know how virus transmission was and that they need treatment. In the being, being scared and fearful, after understanding they get used to it and accept it. Not telling people and being scared of the reaction if anybody finds out. Having limits and rules for having something others don t. Accepting taking medicine and, with time, getting used to it and learning how to take care of themselves. Being a normal person, the different is the virus, having to take medicine and go to the hospital. 2) interpretative comprehension unveiled the being-a-teenager-who-experienced-the-revelation-of-the- diagnosis-of-HIV/AIDS, keep him/herself as everyone are and want him/her to be. Decayed on his/her quotidian, prisoned on his/her talking about received and passed information about his/her serological condition, curious for not knowing about his/her health situation and on the ambiguity of perceiving him/herself normal and feeling different. Once that, on the facticity of having HIV/aids he/she unveiled occupied with having to maintain the treatment. On the dominating solicitude of family members he/she took medicine because they gave it to him/her. However, liberating solicitude stabilishes itself when family members and/or health professionals explained that treatment is for their health, that way the the being-a-teenager-who-experienced-the-revelation-of-the-diagnosis-of-HIV/AIDS preoccupies in taking care of him/herself. The comprehension of the living of the being-a-teenager-who-experienced-the-revelation-of-the-diagnosis-of-HIV/AIDS unveil the need to incorporate the biological dimension to the existential dimension on the movement of disclosuring. The nurse has the prospect of enabling the movement of existential authenticity when the impersonality decadent enable the being-a-teenager-who-experienced-the-revelation-of-the-diagnosis-of-HIV/AIDS if (un) cover amid its potential, facilities, difficulties and limits on the disclosure. / O objetivo deste estudo foi compreender o significado da revelação do diagnóstico do vírus da imunodeficiência humana (HIV) e da síndrome da imunodeficiência adquirida (aids) do adolescente. Investigação qualitativa, fenomenológica sustentada no referencial teórico-metodológico de Martin Heidegger. A etapa de campo foi desenvolvida no período de novembro de 2011 a fevereiro de 2012, no Serviço de Doenças Infecciosas do Hospital Universitário de Santa Maria (HUSM), Rio Grande do Sul, Brasil. Como modo de acesso ao ser utilizou-se a entrevista fenomenológica e coleta no prontuário. Contemplou 12 adolescentes que têm HIV/aids, de 13 a 19 anos de idade. A etapa de campo foi concomitante a etapa de análise a qual permitiu alcançar a suficiência dos significados. Os aspectos éticos foram respeitados conforme a Resolução 196/1996. A análise dos dados foi pautada nos dois momentos metódicos heideggeriano: 1) a compreensão vaga e mediana apontou que para o adolescente a revelação do diagnóstico significa saber disso antes de alguém contar, mesmo sem entender, até a família e/ou profissionais da saúde explicarem. Sabe como foi à transmissão do vírus e que precisa de tratamento. No início fica assustado e com medo, depois que entende se acostuma e aceita. Não contar para ninguém e ter medo da reação se alguém souber. Ter limites e regras por ter algo que os outros não têm. Aceitar tomar o remédio e, com o tempo, se acostuma e aprende a se cuidar. Ser uma pessoa normal, o diferente é o vírus, ter que tomar os remédios e ir ao hospital. 2) a compreensão interpretativa desvelou que o ser-adolescente-que-vivenciou-a-revelação-do-diagnóstico-de-HIV/aids, se mantém como todos são e querem que ele seja. Decaído no seu cotidiano, aprisionado na falação das informações recebidas e repassadas sobre sua condição sorológica, curioso por saber sobre sua situação de saúde e na ambiguidade de se perceber normal e se sentir diferente. Uma vez que, diante da facticidade de ter HIV/aids se desvelou ocupado com ter que manter o tratamento. Na solicitude dominadora dos familiares, tomava o remédio porque o davam. No entanto, a solicitude libertadora se estabelece quando os familiares e/ou profissionais de saúde explicaram que o tratamento é para a sua saúde, assim se preocupa em cuidar-se. A compreensão do vivido do ser-adolescente-que-vivenciou-a-revelação-do-diagnóstico-de-HIV/aids desvela a possibilidade de incorporar a dimensão biológica à dimensão existencial no movimento da revelação. O enfermeiro tem a perspectiva de possibilitar o movimento existencial da impessoalidade decadente à autenticidade quando possibilitar que o ser-adolescente-que-vivenciou-a-revelação-do-diagnóstico-de-HIV/aids se (des)cubra em meio as suas potencialidades, facilidades, limites e dificuldades diante da revelação do diagnóstico.

The general objective of the study was to analyze the longitudinality and the continuity of care devoted to the child/adolescent with chronic disease. The specific ones were to analyze the understanding of health professionals and managers on the health care of the child/adolescent with chronic disease and his or her family; and to analyze the caretakers perception in relation to the longitudinality and the continuity of care devoted to the child/adolescent with chronic disease. It is a qualitative research based on the methodological reference of Dialectical and Historical Materialism with focus on longitudinality and the continuity in order to know the reality of these attributes in the working process related to the health attention network devoted to the care of the child/adolescent with chronic disease. A total of 32 individuals, being 12 family members, 14 health professionals and 6 managers, participated in different phases of the research, from February to October 2013. Data collection was started in a hospital which is reference in the treatment of these illnesses for the state of Paraíba. Semi-structured interviews with health professionals were carried out and family members of child/adolescent with chronic disease were identified so as to take part in focus group or in the semi-structured interview. Family members, who lived in the studied municipality, were invited to contribute to the second phase of the research by authorizing the visit and the qualitative analysis of the professionals records in the hospital chart and of the Family Health Units (FHUs) registered in field diary and interview with health professionals of the FHU of its coverage area. Semi-structured interviews with managers from the municipality were also carried out with the purpose to complete the participants´ framework of this specific reality. The conclusion of the empirical collection followed the sufficiency criterion and the theme analysis consubstantiated the material organization with data triangulation of different sources. The study was guided by the Resolution number 466/12 and it obtained approval opinion number 184.351 and the Certificate of Presentation for Ethical Consideration (CPEC) number 11444412.8.0000.5183. Longitudinality and continuity of care devoted to the child/adolescent with chronic disease present meaningful fragilities in programmatic, institutional and personal levels that bring considerable vulnerabilities for those who undergo the process of chronic condition in the attention network and that need to be evaluated in the daily actions. It was observed that health attention to the child/adolescent with chronic disease is weakened in regard to management, locus of planning of the public policies and the programmatic actions. Therefore, there are several obstacles to continuity of care devoted to the child/adolescent with chronic disease and his or her family. Horizons concerning care need to be widened in regard to this population from a major visibility on behalf of both the public policies and the health actions because the continuity of care, to be carried out in the attention network, will only be mobilized if all individuals are involved in the process. / A longitudinalidade e a continuidade do cuidado são atributos essenciais para o acompanhamento eficaz da criança/adolescente com doença crônica e sua família, por permitir perpassar o olhar pobre que associa os serviços de saúde unicamente ao atendimento curativo da doença. O objetivo geral do estudo foi analisar a longitudinalidade e a continuidade do cuidado à criança/adolescente com doença crônica e os específicos analisar a compreensão dos profissionais e gestores de saúde sobre a atenção à saúde da criança/adolescente com doença crônica e sua família; e analisar a percepção de cuidadores em relação à longitudinalidade e à continuidade do cuidado à criança/adolescente com doença crônica. Estudo qualitativo com base no referencial metodológico do Materialismo Histórico Dialético, com enfoque na longitudinalidade e na continuidade para conhecer a realidade destes atributos no processo de trabalho na rede de atenção à saúde no cuidado à criança/adolescente com doença crônica. Participaram em diferentes fases da pesquisa 32 sujeitos, sendo 12 familiares, 14 profissionais de saúde e 6 gestores no período de fevereiro a outubro de 2013 e a coleta de dados iniciou-se em hospital referência no tratamento dessas enfermidades para o Estado da Paraíba. Neste, realizaram-se entrevistas semiestruturadas com os profissionais de saúde e buscou-se identificar familiares de criança/adolescente com doença crônica para participar de grupo focal ou entrevista semiestruturada. Os familiares, que residiam no município em estudo, foram convidados a contribuir com a segunda fase da pesquisa, autorizando a consulta e análise qualitativa dos registros dos profissionais no prontuário do hospital e das Unidades de Saúde da Família (USF) registradas em diário de campo, e entrevista com os profissionais de saúde da USF de sua área de cobertura. Também foram realizadas entrevistas semiestruturadas com gestores do município para completar o arcabouço dos participantes dessa realidade específica. O encerramento da coleta empírica seguiu o critério de suficiência e a análise temática consubstanciou a organização do material com triangulação dos dados das diferentes fontes. O estudo foi norteado pela Resolução nº 466/12 e obteve parecer de aprovação nº 184.351 e CAAE 11444412.8.0000.5183. A longitudinalidade e a continuidade do cuidado à criança/adolescente com doença crônica apresentam fragilidades significativas em níveis programático, institucional e pessoal, que trazem vulnerabilidades consideráveis para os que perpassam o processo da condição crônica na rede de atenção e precisam ser avaliadas no cotidiano das ações. Observou-se que a atenção à saúde da criança/adolescente com doença crônica está fragilizada quanto à gestão, locus do planejamento das políticas públicas e das ações programáticas. Desse modo, há inúmeros obstáculos à continuidade do cuidado à criança/adolescente com doença crônica e sua família. Necessita-se ampliar os horizontes do cuidado a essa população a partir de maior visibilidade por parte das políticas públicas e das ações em saúde, pois a continuidade do cuidado, para ser efetivada nas redes de atenção, precisará da mobilização de todos os envolvidos no processo.

[pt] Esta pesquisa tem por objetivo analisar as medidas de proteção aplicadas em casos de abuso sexual no I Conselho Tutelar de Duque de Caxias e sua relação com a garantia de direitos de crianças e adolescentes. Para tal, realizou-se uma pesquisa documental a partir dos registros em prontuários de 24 (vinte e quatro) casos iniciados em 2014, que continham a tipificação de violação caracterizada como abuso sexual. Buscou-se acompanhar os desdobramentos das medidas de proteção, o caminho percorrido pelos envolvidos desde a denúncia ou notificação, com ênfase no contexto de proteção versus violação dos direitos das alegadas vítimas e seus familiares, a partir da contrarreferência dos demais segmentos do Sistema de Garantia de Direitos de Crianças e Adolescentes (SGD), materializada em documentos enviados ao Conselho Tutelar. A análise dos dados indica a necessidade de qualificação de profissionais que atuam no SGD para melhor sistematização dos registros, bem como estabelecimento e efetivação de um fluxo a ser seguido em casos de abuso sexual de crianças e adolescentes. Permite ainda concluir que a comunicação recíproca entre os segmentos que integram o SGD – referência e contrarreferência – concorre para redução das fragilidades no atendimento e da ocorrência de revitimização, fortalecendo a atuação em rede na busca da garantia de direitos. / [en] This research has for objective to analyze the applied protective measures in cases of sexual abuse in the I Guardian Council of Duque de Caxias and its relation ship with the warranty of children and adolescent s rights. For such purpose, It was conducted a documental research starting from the registrations in handbooks of 26 (twenty-six) initiate cases in 2014, that contained the violation s typification characterizedas sexual abuse. It was sought to accompany the unfoldings of the protection measures, the way traveled for the involved since the accusation or notification, withem phasis in the contexto protection versus violation of theal legedvictims right sand their relatives, starting from the counterreference of the other segmentsofthe System of Warranty of Children and Adolescent s rights (SGDCA), materialized in documents sentto Guardian Council. The analysis of the data indicates the need of professionals qualification that act in SGD for better systemization of the registrations, as well as establishment and efetivation of a flow to befollowed in cases of children s sexual abuse and adolescents. It stills allow stoend that the reciprocal communication among the segments that integrate SGD – reference and counterreference - it competes for reduction of the fragilities in the servisse and of the revictimization occurrence, streng thening the performance in net in the search of the warranty of rights.

Reading plays a significant role in society and currently there is worldwide pressure for higher literacy results. The focus of this thesis is reading motivation and how it relates to a learner's amount of reading, reading comprehension and academic achievement since these problematic constructs are attracting the interest of researchers internationally. Researchers have investigated the relationships among these constructs with a variety of results. These relationships, however, have not yet been investigated in a South African context with Afrikaans-speaking adolescent readers, and the only information available on the reading motivation of Afrikaans-speaking adolescent readers is information on their reading preferences in regard to literary texts. Hence the following three research aims were determined for this study: the compilation of a reading motivation profile of Afrikaans-speaking adolescent readers, the analysis of the relationships among Afrikaans-speaking adolescent readers' reading motivation and their amount of reading, reading comprehension and academic achievement in an Afrikaans Home Language environment, as well as the development of a reading motivation framework, particularly for use in the school and classroom environment, for these adolescent readers. The study was carried out in the post-positivistic research paradigm by means of a non-experimental quantitative research approach. Three methods of data collection were used, namely a structured questionnaire (based on the eleven reading motivation dimensions of Wigfield and Guthrie's (1997) Motivation for Reading Questionnaire, which for this purpose had been adapted for the South African context), two reading comprehension tests and obtaining the data regarding the academic achievement of the respondents. The 823 respondents that had participated in this study were the grade 9 learners (Afrikaans Home Language) of seven schools from the Dr Kenneth Kaunda district (North West Province, South Africa), selected by means of purposive sampling so that different quintiles and geographic areas were represented. Information obtained from a comprehensive literature study on relevant motivation theories, on reading motivation in practice and on the relationship among their reading motivation, amount of reading, reading comprehension and academic achievement, as well as from the results of an empirical investigation of the reading motivation of specific grade 9 learners, was used to compile a reading motivation profile of Afrikaans-speaking adolescent readers. The motivation theories on which motivation in this study was based, were the social-cognitive theory, the self-efficacy theory, the ecological system theory of human development and the expectancy-value theory, because these theories emphasise the individual's behaviour within particular social contexts and because constructs such as self-efficacy, task value and mastery, which emanate from these theories, play a cardinal role in determining suitable reading motivation strategies for specific readers. It was essential to compile a reading motivation profile of Afrikaans-speaking adolescent readers before a reading motivation framework for these adolescent readers could be compiled. The reading motivation framework recommends specific reading motivation strategies various social role-players in the school and classroom environments can implement so as to improve Afrikaans-speaking adolescent readers' reading motivation levels. The various social role-players that would influence the Afrikaans-speaking adolescent reader's reading motivation and the reading motivation strategies each of them could use, was systematised (namely the Department of Education, the school principal and management team, teachers and parents). Reading plays a significant role in society and currently there is worldwide pressure for higher literacy results. The focus of this thesis is reading motivation and how it relates to a learner's amount of reading, reading comprehension and academic achievement since these problematic constructs are attracting the interest of researchers internationally. Researchers have investigated the relationships among these constructs with a variety of results. These relationships, however, have not yet been investigated in a South African context with Afrikaans-speaking adolescent readers, and the only information available on the reading motivation of Afrikaans-speaking adolescent readers is information on their reading preferences in regard to literary texts. Hence the following three research aims were determined for this study: the compilation of a reading motivation profile of Afrikaans-speaking adolescent readers, the analysis of the relationships among Afrikaans-speaking adolescent readers' reading motivation and their amount of reading, reading comprehension and academic achievement in an Afrikaans Home Language environment, as well as the development of a reading motivation framework, particularly for use in the school and classroom environment, for these adolescent readers. The study was carried out in the post-positivistic research paradigm by means of a non-experimental quantitative research approach. Three methods of data collection were used, namely a structured questionnaire (based on the eleven reading motivation dimensions of Wigfield and Guthrie's (1997) Motivation for Reading Questionnaire, which for this purpose had been adapted for the South African context), two reading comprehension tests and obtaining the data regarding the academic achievement of the respondents. The 823 respondents that had participated in this study were the grade 9 learners (Afrikaans Home Language) of seven schools from the Dr Kenneth Kaunda district (North West Province, South Africa), selected by means of purposive sampling so that different quintiles and geographic areas were represented. Information obtained from a comprehensive literature study on relevant motivation theories, on reading motivation in practice and on the relationship among their reading motivation, amount of reading, reading comprehension and academic achievement, as well as from the results of an empirical investigation of the reading motivation of specific grade 9 learners, was used to compile a reading motivation profile of Afrikaans-speaking adolescent readers. The motivation theories on which motivation in this study was based, were the social-cognitive theory, the self-efficacy theory, the ecological system theory of human development and the expectancy-value theory, because these theories emphasise the individual's behaviour within particular social contexts and because constructs such as self-efficacy, task value and mastery, which emanate from these theories, play a cardinal role in determining suitable reading motivation strategies for specific readers.
