Développement de nouveaux systèmes d’assurance qualité à base de sondes dosimétriques GaN pour la curiethérapie / Development of novel quality assurance systems based on GaN dosimeter probes for brachytherapy

Ce travail de thèse a pour but de développer et de caractériser de nouveaux systèmes d'assurance qualité en curiethérapie, à base de sondes dosimétriques en Nitrure de Gallium (GaN). L'étude comprend d'une part la caractérisation et la simulation du transducteur GaN et de la sonde dosimétrique, et d'autre part la mise en œuvre et les tests de 2 systèmes d'assurance qualité.Les propriétés de radioluminescence du transducteur GaN ont été étudiées afin d'optimiser le signal dosimétrique des sondes. La sonde dosimétrique encapsulant un petit volume de cristal GaN a ensuite été caractérisée en vue de son intégration dans des systèmes d'assurance qualité. Des simulations Monte Carlo sur la sonde GaN ont également été effectuées et comparées aux mesures.Pour l'assurance qualité en curiethérapie, nous avons proposé une méthode utilisant plusieurs sondes GaN, pour permettre de déterminer en temps réel les principaux paramètres physiques d'un projecteur de source : la position, le temps de pause et l'activité de la source. Nous avons conçu et réalisé deux prototypes de systèmes d'assurance qualité mettant en œuvre cette méthode : un fantôme instrumenté pour le contrôle prétraitement et un applicateur gynécologique instrumenté pour le contrôle pendant le traitement. Ces deux prototypes ont été testés en conditions cliniques, et leurs principales caractéristiques sont satisfaisantes. Ils répondent aux besoins applicatifs et offrent une perspective intéressante pour de nouveaux outils de contrôle qualité en curiethérapie / This work aims to develop and characterize novel quality assurance systems based on Gallium Nitride (GaN) dosimeter probe for brachytherapy. It includes characterization and simulation studies of the GaN transducer and of the dosimeter probe as well as the implementation and testing of two quality assurance system prototypes.The radioluminescence properties of the GaN transducer have been studied for optimization of the dosimeter response. Characterization studies of dosimeter probes incorporating miniaturized GaN transducers have been carried out in the frame of the quality assurance systems development. Monte Carlo simulations of the GaN probe under irradiation has also been performed and compared with measurements.A method which processes simultaneously the output signals from several GaN probes has been proposed for real time determination of the projector’s physical parameters (dwell position, dwell time and activity of the source). Two quality assurance system prototypes implementing this method have been designed and fabricated: an instrumented phantom for pre-treatment quality assurance and an instrumented gynecological applicator for in vivo quality control. These two prototypes have been tested and evaluated in clinical conditions and their main characteristics are satisfactory. Both systems are in line with the application requirements and offers new perspectives for quality assurance in brachytherapy

La loi Spinetta du 4 janvier 1978, est la loi fondamentale en matière de construction immobilière . Elle régit à la fois la responsabilité des constructeurs ainsi que les assurances obligatoires (responsabilité et assurance dommages ouvrage) . En raison de ses lacunes et ses imperfections, elle offre au juge judiciaire une marge d'interprétation prépondérante, le conduisant ainsi à faire une oeuvre jurisprudentielle très créatrice, dérogeant parfois au Code civil et au Code des assurances . Dans le souci de parvenir à la protection efficace des maîtres d'ouvrage, commandée par la philosophie de cette loi, le juge s'appuie sur les capacités financières des assureurs pour atteindre cet objectif, au point, finalement, d'élargir l'objet de la responsabilité des constructeurs et de l'assurance construction . Cette action dynamique du juge a ainsi pour résultat de perturber l'équilibre du système d¤assurance construction, notamment en ce qui concerne le mécanisme de préfinancement des dommages et le mécanisme de la subrogation . L'assureur dommages ouvrage ne parvient pas toujours à récupérer les fonds qu'il a avancés . En dépit de ces dysfonctionnements, ce système d'assurance reste selon les professionnels de la construction globalement satisfaisant et de loin le plus protecteur au niveau européen . Reste à savoir si ce système pourra servir de modèle, s'il est un jour question d'harmonisation

Aim of the study. To evaluate possibilities to implement the program of health care quality assurance in health care institutions from the physicians’ point of view. 58.4 percent of respondents are aware with the program of health care quality assurance. 71.2 percent of those who are aware with the program believe in health care quality improvement when the program will be implemented. 41.5 percent of respondents maintain that program will be implemented under condition of proper financing while 20.8 percent assert that program will not be implemented. 65.4 percent of physicians evaluate quality of care in their institution as good and 33.7 percent as satisfactory. According the opinion of the respondents’ quality of care could be improved with teamwork and good qualification of the physicians, a half of respondents maintain that the main obstacle for better examination of the patients is insufficient financing. Almost one third of physicians state that information about safety of health care is insufficient. 44.6 percent of respondents are not satisfied with their work, and 85.1 percent affirm that quality of care is influenced with satisfaction with work, 93.1 percent maintain that quality of care is influenced with workload, 43.6 percent relate their quality of work with payment. L'intérêt d'assurance comme élément constitutif de la déclaration initiale de risque en assurance de dommages

La question à laquelle ce travail tente de répondre est la suivante: l'intérêt d'assurance doit-il être considéré comme un élément constitutif de la déclaration initiale du risque en assurance de dommages? Notre hypothèse de travail dans ce contexte est que l'intérêt doit être déclaré dans la mesure où il constitue un risque moral. Afin de soutenir notre argumentation, nous avons divisé notre étude en deux parties. Premièrement, nous avons examiné toutes les facettes de l'intérêt d'assurance. Tout d'abord son historique, ensuite sa définition et enfin l'interprétation qu'en ont donnée les tribunaux au fil des ans. Puis, nous avons fait de même avec la notion de déclaration initiale de risque. Plus particulièrement, nous avons examiné les conséquences d'une brèche à l'obligation de l'assuré à l'étape de la formation du contrat. Les conclusions que nous avons tirées de cette analyse sont les suivantes: 1. Il existe toujours un questionnement quant au moment précis où l'assuré doit remplir son obligation de détenir un intérêt d'assurance. L'intérêt d'assurance pourrait être examiné uniquement à l'arrivée du sinistre, alors que la déclaration initiale de risque doit être examinée au moment de la souscription de l'assurance; 2. Les différents intérêts qu'une personne peut avoir dans un bien n'influent pas directement sur le risque assurable, contrairement à la déclaration initiale qui sert justement à circonscrire le risque. Cette conclusion est par ailleurs conforme à l'historique de l'exigence de l'intérêt lui-même, quoique certains jugements affirment le contraire; 3. L'intérêt d'assurance peut être modifié en cours de contrat sans entacher la validité du contrat alors que la déclaration initiale de risque ne peut être modifiée sans encourir le risque de voir la prime d'assurance augmentée ou le risque d'assurance refusé: 4. Par contre, l'intérêt particulier qu'une personne détient dans une chose peut constituer un risque subjectif accru pour l'assureur dans certaines circonstances. / This work tries to answer one question: should the insurable interest be an element of the initial declaration of risk? Our main hypothesis is to prove that the interest must be declared when it constitutes a moral risk. To support our opinion, we divided our argument in two parts. The first one is dedicated to the insurable interest : its definition, its history and its interpretation given by the court through the years. ln the second part, we discuss the representation of risk. Particularly, we examine the consequences of a breach in the insured's obligation to divulgate ail the pertinent information to the insurer. Our conclusions are the following : 1. An interrogation still exists concerning the insured's obligation of having an insurable interest at the moment of the loss or also at the signing of the contract. Some authors consider that the insurable interest has to exist only at the loss; 2. The different interest which a person can have should not influence directly the risk, contrarily to the initial declaration which is used to identify and Iimit the risk. This conclusion is conform to the historie background of the insurable interest; 3. The insurable interest can be modify without changing the validity of the contract. On the contrary, the initial declaration of risk can not be modified without a raise in the premium or a refusaI; 4. Föreliggande studie har behandlat kultur och systematiskt kvalitetsarbete inom socialtjänsten. Det huvudsakliga syftet var att genom tillämpning av kultur- och kraftfältsanalys studera hur väl förberedd en enhet inom socialtjänsten är att följa intentionerna för systematiskt kvalitetsarbete inom socialtjänsten. Studien genomfördes utifrån PDSA-cykeln som en variant av aktionsforskning och baserades på en fallstudie inom Individ- och familjeomsorgen i Hammarö kommun. Resultatet analyserades utifrån en kraftfältsanalys där med- och motkrafter identifierades. Av resultatet framkom att det sammantaget fanns en positiv inställningen till systematiskt kvalitetsarbete, men att kulturen inte tydde på detsamma. Sammanfattningsvis kan sägas att enheten har dåliga förutsättningar att följa intentionerna för systematiskt kvalitetsarbete. Om de skall införlivas måste enhetens positiva inställning kvarstå, ledningen vara aktivt närvarande, medarbetare göras delaktiga och kulturen förändras. Värdet i studien ligger i att det i Sverige idag finns ett pågående arbete med att implementera eller revidera ledningssystem för systematiskt kvalitetsarbete inom socialtjänsten. Kulturens betydelse av effektivitet och framgångsrikt kvalitetsarbete kan inte nog belysas. Studien kan ge inspiration till ledare och organisationer, främst inom socialtjänsten, att förena systematiskt kvalitetsarbete och arbetet med kultur.  Fortsatt forskning bör ha fokus på ledarskapets betydelse avseende organisationskultur, men det saknas även forskning som visar hur förbättringskultur skapas som stäcker sig över organisations- och professionsgränserna. / The present study has examined the cultural and systematic quality work in social services. The main purpose was that through the application of cultural and force field analysis study how well prepared a unit in social services is to follow the intentions of systematic quality work in social services. The study was conducted based on the PDSA cycle as a variant of action research and was based on a case study in the Individual and family services in Hammaro municipality. The results were analyzed on the basis of a force field analysis, where with and counterweights were identified. My results show that overall there was a positive attitude towards systematic quality work, but that the culture did not suggest the same. In summary, the unit has poor conditions follow the intentions of systematic quality work. If they must be incorporated into the device's positive attitude persist, lead an active presence, employees involved and the culture is changing. The value of the study lies in the fact that in Sweden today there is an ongoing effort to implement or revise management system for systematic quality work in social services. The importance of culture of efficiency and successful quality work can not be elucidated. The study may provide inspiration for leaders and organizations, primarily in the social services, to reconcile systematic quality work and work culture. Further research should focus on the importance of leadership organizational culture, but there is also a lack of research that shows how improvement culture is created that extends across organizational boundaries and profession.
