Prevalence of emotional and behavioural problems : pitfalls and practices among secondary school learners in Ethiopia

The prevalence of emotional and behavioural problems in secondary school adolescents is regarded as widely spread globally. Their effects/pitfalls are also a concern that cannot be ignored. Thus, this study dealt with this issue among adolescents, as well as with pitfalls and practices of secondary schools in Ethiopia, with particular focus on East Showa Administrative Zone of Oromiya National Regional state. Consequently, the study intended to: -examine what social competence problems (activity problems, social activities problems and academic performance problems) are experienced by secondary school adolescents of Ethiopia. -determine the relationship between sex groups and social competence problems by secondary school adolescents of Ethiopia. -examine the trends of emotional/internalising and behavioural/externalising problems among secondary school adolescents of Ethiopia. -determine the relationship displayed between sex groups and emotional/internalising and behavioural/externalising problems by secondary school adolescents of Ethiopia. -examine the relationship display between age groups and emotional/internalising and behavioural/externalising problems by secondary school adolescents of Ethiopia. -comprehend the relationship among social competence problems and syndrome problems by secondary school adolescents of Ethiopia. To address the objectives of the study, a descriptive survey and correlational study were employed. To achieve this target, the Amharic and the Afan-Oromo versions of Achenbach’s (2001) youth self-report (YSR) were used to collect data from 714 research participants of secondary school adolescents out of 27,643 in the East Showa Administrative Zone of Oromiya Regional State-Ethiopia, from 8 secondary schools. In analysing the data, basic descriptive statistics such as frequency, percentages, percentiles and T-scores as well as ANOVA were employed. In general, the results of the study demonstrated that the prevalence of emotional and behavioural problems of Ethiopian secondary school adolescents was relatively lower (about 9%) in comparison to many other countries. Moreover, girls showed more internalising problems (11.58%) than boys (9.63%) and vice versa in externalising problems. Consequently, girls accounted for 8.6% who fell into a clinical category whereas amongst boys, 14.83% were classified into such as category. In contrary to the other findings males were scored higher than females in withdrawal/depressed subscales (4.23% and 2.74% respectively). Furthermore, statistically significant relationships between academic competences with activities, social activities, somatic complaints, social problems and externalising problems were discovered. Fulfilling and rendering appropriate guidance and counselling services in the schools comprised the first dimension of the recommendations of this work. Furthermore, establishing school based mental health services to alleviate the problems is another key point of the recommendation to assist adolescents with emotional and behavioural problems in secondary schools of Ethiopia. / Psychology / Ph. Exposição a chumbo e comportamento anti-social em adolescentes / Lead exposure and antisocial behavior in Brazilian adolescents

Introdução - A intoxicação por chumbo é um conhecido problema de saúde pública e o envenenamento por este metal pode causar danos a vários órgãos, especialmente ao Sistema Nervoso Central de crianças em desenvolvimento. Objetivo geral- estudar a associação entre exposição a chumbo e comportamento anti-social (CAS) em adolescentes brasileiros. Objetivos específicos: a) analisar a associação entre exposição a chumbo e CAS / cometimento de atos infracionais (CAI); b) estudar potenciais fontes de exposição domiciliar a chumbo que mais estão associadas a altas concentrações de chumbo no esmalte dentário (CCED) e; c) avaliar o impacto de alterações metodológicas na técnica de microbiópsia ácida de esmalte dentário superficial (MAEDS) sobre CCED e profundidade da bíópsia. Métodos- Um estudo transversal foi conduzido com 173 jovens (Bauru, SP). MAEDS foram realizadas nestes jovens por dois diferentes protocolos metodológicos. Além disso, questionários sobre comportamento dos adolescentes e exposição a possíveis fontes de contaminação por chumbo foram aplicados a pais e adolescentes. Análises de regressão logística, testes de Wilcoxon e testes t pareados foram aplicados aos dados. Resultados- Odd ratios ajustados para covariáveis indicaram que alta CCED está associada a risco aumentado de exceder o escore clínico para queixas somáticas, problemas sociais, comportamento de quebrar regras e problemas externalizantes (IC 95%). Alta CCED não foi associado com escores elevados de CAI. Os fatores de risco mais associados com alta CCED foram residir em área contaminada ou até 2 km da área contaminada e trabalhar na fabricação de tintas, pigmentos, cerâmicas ou baterias. A profundidade da biópsia, calculada pela fórmula da altura do cilindro, para um dos protocolos, levou a resultados errôneos de profundidade da biópsia, confirmados por testes de perfilometria. Conclusões- A exposição a altos níveis de chumbo parece disparar o estabelecimento de CAS, o que alerta para a necessidade de desenvolvimento e implantação de políticas públicas de saúde que previnam o envenenamento da população por chumbo. Adolescentes foram expostos ao chumbo por algumas fontes estudadas, no Brasil. O esmalte dentário é um marcador fidedigno e a MAEDS é bastante útil e confiável. No entanto, CCEDs não podem ser comparadas entre resultados de pesquisas diferentes quando houver qualquer variação metodológica entre os estudos, havendo a necessidade da padronização do procedimento. / Introduction - Lead poisoning is a long known public health problem. Thus, lead exposure may cause damage to diverse organs, especially in the Central Nervous System of children in developing process. Objectives- a) to analyze the association between lead exposure and antisocial / delinquent behavior; b) to study the potential sources of lead home exposure more associated to high dental enamel lead levels (DELL) and c) to evaluate two distinct enamel biopsy protocols in relation to biopsy depth and DELL. Methods- A cross-sectional study was conducted with 173 adolescents (Bauru, SP, Brazil). Surface dental enamel (SDE) etch-acid microbiopsies were performed in upper central incisors of these youths by two different methodological protocols. In addition, questionnaires about adolescents behavior and about possible sources of lead contamination were responded by youths and their parents. Logistic regression, Wilcoxon and paired t tests were applied to data. Results- Odd ratios adjusted for familial and social covariates indicated that high DELL is associated with increased risk of exceeding the clinical score for somatic complaints, social problems, rulebreaking behavior (T70) and externalizing problems (T63) (CI 95%). High DELL was not found to be associated with elevated SRD scores. The risk factors associated to high DELL were residing in contaminated area or close proximity and working in paints, pigments, ceramic or batteries manufacturing. The biopsy depth, calculated by the cylinder formula, for Protocol II induced misleading results, as confirmed by profilometry tests. Conclusions- It seems that exposure to high lead levels can indeed trigger antisocial behavior, which claims for public policies to prevent lead poisoning. Adolescents were exposed to lead, by some studied sources, in Brazil. SDE, measured by etch-acid microbiopsy, is a reliable biomarker, but DELL could not be compared when there is some methodological variation among the studies. A standardization of the procedure is necessary.

Competência social e transtornos comportamentais em crianças portadoras de asma moderada e grave / Social competence and behavior disorders in children with moderate to severe asthma.

Casuistics: Sixty-two patients were studied, 36 male and 26 female, ages ranging from 7 to 16 years (MA: 10.94; SD: 2.28), with clinical diagnosis of moderate to severe asthma, without comorbidity, seen at the Pediatric Pneumology Service and the Allergy and Immunology Service at a University Hospital, from March to September, 2000. A control group was simultaneously studied and included 62 patients, 37 male and 25 female, ages ranging from 7 to 16 years (MA: 10.26; SD: 2.37), seen at the Orthopedics and Ophthalmology Outpatient Wards at the same hospital, without symptoms of asthma, respiratory allergy and no chronic diseases. Material and method An Identification Form, a Clinical Data Form and the CBCL (Child Behavior Checklist), a tool to evaluate social competence and behavior disorders, were used. Results Results were evaluated by the proportional tests, Student s t test and ANADEP and ANADEPMU and a significance level of 0.05 was chosen. Patients were classified according to the clinical manifestations of asthma: 42 had moderate asthma and 20 had severe asthma, most of them (93.55%) had been diagnosed since 3 years of age. There was no significant difference in age and gender between the group with asthma and the control group. There was a marked association between overall social competence and asthma (with p=0.000) and between social competence associated to activities (with p=0.001) and school (with p=0.01). There was no difference between children with asthma and the control group for social markers (with p=0.23). There was no correlation between gender and age when evaluating social competence, or overall social competence specific parameters: activities, school and social activities. The evaluation of social competence and type of asthma did not show a correlation. There was a strong association between the presence of behavior disorders and asthma (p=0.005), specially internalizing disorders (p=0.001). There was no association between the externalizing disorders and the disease. Behavior disorders were not associated to gender, age and type of asthma. Conclusions Children with asthma showed alterations in overall social competence and social competence related to activities and school, when compared to the control group. They also showed overall and internalizing disorders, which may be harmful to their development, their quality of life, compliance with the treatment and adequate management of the disease. The integration of biological, psychological and social factors is essential to establish adequate programs for the treatment of children with asthma and their families. / Objetivo - avaliar competência social e transtornos comportamentais em crianças com asma moderada e grave, a partir de percepção dos pais. Causística: foram estudados 62 pacientes, 36 do sexo masculino e 26 do sexo feminino, com idade entre 7 e 16 anos (im:10,94; dp:2,28), com diagnóstico clínico de asma moderada e grave, sem comorbidade, atendidos nos serviços de pneumologia infantil e alergia e imunologia de um hospital escola, no perído de março e setembro de 2000. Um grupo de controle foi estudado no mesmo período, composto de 62 pacientes, 37 do sexo masculino e 25 do sexo feminino, com idade entre 7 e 16 anos (im: 10,26; dp: 2,37), atendidos nos ambulatório de ortopedia e oftalmologia do mesmo hospital, sem qualquer sintomatologia de asma, de alergia respiratória e sem qualquer doença crônica. Material e método - foram utilizados na obtenção dos dados uma ficha de identificação, uma ficha de dados clínicos e um instrumento que avalia competência social e transtornos comportamentais - child behavior checklist (cbcl). Resultados - os resultados foram analisados com testes proporção x (ao quadrado), teste t de student, anadep e anadepmu, adotando-se nível de significância de 0,05. Os pacientes foram classificadossegundo manifestações clínicas da asma: 42 com asma moderada e 20 com asma grave, sendo que 93,55% receberam o diagnóstico da doença antes dos 3 anos de idade. Não houve diferença significante em termos de idade e sexo entre os grupos com asma e controle. Houve associação entre competência social e global e asma (p=0,000) e entre competência social associada a atividades (p=0,001) e escola (p=0,01). Não houve diferença entre crianças com asma e o grupo controle em relação ao aspecto social (p=0,23). Não houve relação entre sexo e idade na análise da competência social, nem quanto à competência social global nem quanto aos aspectos específicos: atividades, escola e social. A análise entre competência social e tipo de asma também não mostrou associação. Houve associação entre presença de transtornos comportamentais e asma (p=0,005), principalmente os transtornos internalizantes (p=0,001). Não houve associação dos externalizantes com a doença. Os transtornos comportamentais não se mostraram associados ao sexo, idade da amostra e tipo de asma. Conclusões - as crianças com asma apresentaram alterações na competência social e global e competência social relacionada a atividades e escola, quando comparadas ao grupo controle. Apresentaram ainda transtornos globais e internalizantes, que podem prejudicar o seu desenvolvimento, sua qualidade de vida, a adesão ao tratamento e o manejo adequado da doença. A integração dos aspectos biológicos, psicológicos e sociais é imprescindível para o delineamento de programas adequados de atendimento à criança portadora de asma e a seus familiares.

Movement in gestalt therapeutic intervention for adolescents with disruptive behavioural tendencies

Movement in Gestalt therapeutic intervention, using structured activities and free improvisations, is a successful alternative to punishing adolescents with disruptive behavioural tendencies in the classroom. An overview of the existing literature regarding the therapeutic value of movement in Gestalt therapeutic intervention for adolescents with disruptive behavioural tendencies is firstly presented. Various techniques supporting the adolescent in his quest to achieve equilibrium, ultimately improving his relationships with others and his environment, are described. A case study, involving eight adolescents with disruptive behavioural tendencies in group therapy follows, which indicates that movement in Gestalt therapy is an alternative to punishment. In the light hereof it is suggested that movement in Gestalt therapy could be implemented successfully in the formal school environment. This form of intervention can contribute to the adolescent's growth, his ability to communicate and his relationships with self, others and the environment, culminating in improved behavioural tendencies. / Social Work / M. Exploring the psychosocial problems of children residing in shelters for abused women and the children in the Cape Metropolitan are : a gestalt approach

The aim of this study was to explore and describe the psychosocial problems perceived (their phenomenological field) by children in middle childhood, living in shelters in the Cape Metropolitan Area. The psychosocial problems included poor self regulation as a result of disturbances in the contact cycle, restrictions in shelters negatively impacting the holistic sense of self, barriers hindering relationships with significant others and the continuation of violence into the participants‟ broader field. The experience of multiple losses and persistence of violent behavior within and without the shelter context was a thread that ran through this study. Current literature provided an overview of the theoretical underpinnings of this study which included the key tenets of Gestalt therapy theory, core Gestalt principles, the child living in a shelter and development in middle childhood. The role of the emotional father-son relationship in the self-concept formation of adolescent boys in secondary schools

The study investigated adolescent boys who experienced behavioural problems at school and the possible causes for these phenomena. It was evident that the adolescent boys who experienced behavioural problems were found to lack emotional connections with others due to their fathers’ absence and lack of positive attachment. A total of 403 adolescent boys from various schools in the Tshwane districts were used in the sample group. This quantitative study employed the use of a quantitative questionnaire as a means of data collection that included qualitative open-ended question as a method to enrich the quantitative data and obtain insight into the life world functioning and experience of the participants. The primary aim of the research study was to determine the emotional relationship between fathers and sons within the sample group. Secondly, the study sought to investigate how this emotional relationship affects the adolescent boy’s self-concept, resilience and motivational aspects in his daily functioning. The approach to quantitative data analysis was by means of descriptive statistical analysis. The findings revealed that emotionally absent fathers indeed impact negatively on the adolescent boys’ resilience and motivational aspects of their functioning and that a father’s absence has a significant influence on the behaviours of adolescent boys in relation to motivation and resilience. However, the research results also revealed that an emotionally absent father does not significantly affect the self–concept of an adolescent boy. Finally, an intervention programme was developed to assist and guide fathers and adolescent boys to improve their relationship with one another. / Psychology of Education / D. Ed. The goal of my diploma thesis is introduce one of alternative direction {--} music therapy, to outline the sphere of specific learning and behavior disorders, and also to give a guide, how to apply components of music therapy during the work with children with these disorders. The thesis consists of two parts {--} a theoretical and practical part. The theoretical part focuses on defining key terms, such as music therapy, dyslexia, dysgraphia, dysortographia, dyscalculia, dyspinxia, dysmusia, dyspraxia. In my work I aim to clarify the question of ADHD and try to point out the relation of particular learning and behavior disorders. The practical part includes twenty activities that were made firstly according to a methodical manual written by M. Beníčková and Z. Vilímek, who use the Czech method of music therapy {--} Music of body, and secondly on the base of casuistry of two boys attending special practical school. Each activitiy should develop particular skills, comprises tools, motivation, its own description and analysis coming out of a practical accomplishment. The method of observing was used for the evaluation.

Jogo terapêutico computadorizado na terapia com crianças / Therapeutic computerized game in therapy with children

Introdução: A psicoterapia infantil mediada com o computador pode tornar os pacientes mais colaborativos e motivados, diminuir o estigma de ir à terapia, aproximar e melhorar a relação com o terapeuta, facilitar a compreensão de conceitos terapêuticos fundamentais dentro da abordagem cognitivo-comportamental, melhorando suas habilidades de resolução de problemas além de estruturar as sessões de terapia. Objetivo: Criar um protocolo computadorizado para tratamento psicoterápico de crianças denominado Projeto Transformador. Avaliar o projeto para saber de sua viabilidade e utilidade como instrumento terapêutico. Método: Foram realizados dois estudos, um com as crianças e outro com os terapeutas. Os dois estudos foram abertos e utilizaram métodos quantitativos e qualitativos. O primeiro ensaio (A), foi realizado com 10 crianças com transtornos de ansiedade, que responderam a escalas de avaliação e a questionários. No segundo estudo (B), 12 terapeutas usaram o Projeto Transformador em sua prática clínica e sua experiência foi ouvida e analisada. Resultados: Os dados coletados mostraram que as crianças e os terapeutas ficaram satisfeitos com o programa computadorizado Projeto Transformador e este mostrou-se útil motivando a execução de um ensaio clínico randomizado / Introduction: Computer-mediated child psychotherapy can make patients more collaborative and motivated, decrease the stigma of going to therapy, closen and improve the relationship with the therapist, facilitate the understanding of fundamental therapeutic concepts within the cognitive-behavioral approach, improving their problem solving skills, as well as structure therapy sessions. Objective: To create a computerized protocol for psychotherapeutic treatment of children called Projeto Transformador. To assess the project\'s feasibility and usefulness as a therapeutic instrument. Method: Two studies were performed, one with children and one with therapists. Both studies were open and used quantitative and qualitative methods. The first study (A) was conducted with ten children with anxiety disorders who responded to evaluation scales and questionnaires. In the second study (B), twelve therapists used Projeto Transformador in their clinical practice and their experiences were heard and analyzed. Results: The data collected showed that both children and therapists were satisfied with the computerized program Projeto Transformador and that the program proved to be useful, motivating the implementation of randomized clinical tests

The thesis deals with access schools and teachers to handicapped students. Theoretical part follows problems of pupils with special educational needs and its legislation, the concept of handicap and disability forms. I characterize from the forms of disability specific learning disorders, brain dysfunction and behavior disorders, cerebral palsy, autism and Asperger syndrome. The last chapter of this part focuses on the conditions, legislation, documents and statistical data in the field of disabled pupils integration. For the practical part I have set several goals: regarding the run of the lesson, teacher access and work of pupils with special educational needs; realize education with handicapped students to obtain my own experience; conduct interviews with teachers, with teacher's assistant, possibly with a special needs teacher in order to obtain an opinion of teaching staff to integrateion and obtain advice for access to pupils with special educational needs; through questionnaires for schools to determine the conditions for integration of handicapped pupils, and finally summarize the recommendations and create a handbook for teachers beginning to work with pupils with special educational needs.
