The role of the educational psychologist in the emotional and social rehabilitation of the traumatic brain injured adolescent

This research study consisted of an examination of the role played by the educational psychologist in the emotional and social rehabilitation of the TBI adolescent. A survey of the literature reveals that traumatic brain injury during adolescence negatively impacts on their adaptation, development and functioning after the acute phase of the rehabilitation process. In order to study this phenomenon, a psycho-educational perspective was utilised. An in-depth qualitative study was undertaken by means of a case study design. The two cases being presented offers the reader insight into the cases pre-morbid functioning, the accident and its aftermath, the specific traumatic brain injuries, the emotional and social problems encountered by these adolescents as well as the psychotherapeutic interventions applied by the educational psychologist in the rehabilitation process of the cases under investigation. The conclusions reached from this investigation were that traumatic brain injury during the developmental phase of adolescence, negatively impacts on the emotional and social well being of these adolescents, and that the educational psychologist plays a valuable role in the emotional and social rehabilitation of these adolescents. / Educational Studies / M. Para avaliar a audição em crianças com paralisia cerebral foi realizada a avaliação comportamental, eletroacústica e eletrofisiológica em 61 sujeitos na faixa etária de três a seis anos e 11 meses, subdivididas em grupo experimental e controle. Foram observadas diferenças estatisticamente significantes na comparação entre os resultados obtidos nos dois grupos, bem como na comparação dos procedimentos no grupo experimental. Existe uma diversidade nos resultados dos procedimentos audiológicos em crianças com paralisia cerebral, o que torna importante a aplicação da bateria completa na determinação do perfil audiológico destas crianças. / In order to evaluate the hearing of children who have cerebral palsy disease the behavioral, eletroacoustical, electrophysiological evaluations were done in 61 people between three and six years and 11 months old, subdivided in experimental and control group. There were statistically significant differences when comparing the results obtained in both groups, as well as when comparing the procedures in the experimental group. There is diversity in the results of hearing procedures in children with Cerebral Palsy, which stresses the importance of a full battery of exams for determining the hearing profile of these children.

Produção do cuidado a pacientes com condições neurológicas muito graves: contribuições interdisciplinares para uma fundamentação teórica / Care for patients with very severe neurological conditions: interdisciplinary contributions to a theoretical framework

A assistência aos pacientes com doenças neurológicas de longa duração não é reconhecida na produção científica brasileira; entende-se por doença neurológica de longa duração: doenças neurológicas de curso longo, fase final de difícil identificação, com sintomas variados e condição de dependência moderada ou grave e comprometimento cognitivo de difícil avaliação. Neste estudo, foram consideradas as seguintes condições: estado vegetativo persistente, estado mínimo de consciência e síndrome do encarceramento (ou síndrome de locked-in). Em busca bibliográfica, verificou-se que não há artigo em língua portuguesa sobre o tema. No cotidiano, a assistência é focada em higiene e observação clínica. Entretanto, na perspectiva do Cuidado, algo mais é necessário ao se pensar este tipo de prática, para a promoção da dignidade humana. Os objetivos deste estudo são: relatar uma prática junto a pessoas com doenças neurológicas de longa duração focada no seu resgate biográfico, e fundamentar a mesma de maneira interdisciplinar, a partir do Cuidado como categoria reconstrutiva. É uma pesquisa qualitativa, na qual foram utilizados primariamente registros de um diário de campo da primeira autora, sobre intervenções em Terapia Ocupacional com esta população; a partir destes, foram produzidas narrativas e reflexões sobre a metodologia de intervenção. A partir do resgate biográfico, busca-se resgatar a condição de sujeito do paciente neurológico muito grave. Na segunda etapa, tem-se a fundamentação teórica numa perspectiva interdisciplinar; o conceito de Cuidado é a referência principal, compreendido como o sentido existencial da experiência de adoecimento. Também foi utilizado o referencial da psicanálise de orientação freudiana e lacaniana para aprofundamento teórico dos achados empíricos descritos através de narrativas. A reconstrução da biografia do paciente ocorre por diversas perspectivas, mas especialmente pela estimulação sensorial - não com foco de recuperação neurológicas, mas a partir do repertório do paciente e em busca da validação de sua condição de sujeito. Junto com uma detalhada avaliação e acompanhamento, o terapeuta faz um monitoramento contínuo, propondo atividades e dando especial atenção ás reações observadas, para que possa nomeá-las ou traduzi-las, bem como discriminar os estímulos agradáveis e desagradáveis; isto também possibilita ampliação das possibilidades de encontro deste sujeito com o mundo. Os conceitos psicanalíticos: estágio do espelho, real, lalíngua e constituição do sujeito apresentam-se como fundamentais para a compreensão das experiências empíricas vividas e, posteriormente, elaboração de uma fundamentação teórica. Memória, história, identidade, dignidade e subjetividade qualificam a prática junto a pessoas com doenças neurológicas de longa duração. A despeito das reações neurológicas apresentadas, tal prática possibilita a reconstrução de um cotidiano significativo e as teorias psicanalíticas de orientação freudiana e lacaniana constituem importante marco para fundamentação da reconstrução da assistência / The care of people with Long Term Neurological Conditions (LTNC) isn´t recognized in the Brazilian Palliative Care scientific production; in daily basis, the assistance is focussed on hygiene´s care and clinical observation. However, from the perspective of palliative care, something more is needed, to promote the dignity of the human person. To describe an occupational therapy intervention for patients with LTNC based on biographical rescue. Records and field diaries of an Occupational Therapy intervention promoted by the first author were used to produce narratives and reflections about this intervention´s methodology. The concept of Comprehensive Care is the main reference, designed as care interested in the existential sense of the illness process. It was also used the framework of Freud´s and Lacan´s psychoanalysis for theoretical studies of the empirical findings reported through narratives. The beginning of the reconstruction of the patient´s biography occurs by several perspectives, especially through sensory stimuli related to prior patient´s occupational repertoire. Along with the detailed evaluation, the therapist does a careful and continuous monitoring, proposing activities, giving special attention to their reactions, being able to discriminate what pleases or not, and expanding the possibilities of encounter of people with LTNC and the environment which surround them. The psychoanalytic concepts: mirror stage, real, lalangue and subject´s constitution had major importance to understand the empirical experiences and to develop a theoretical basis. Memory, history, identity, dignity, and subjectivity qualify the practice of occupational therapist with people with LTNC. Regardless of neurological reactions presented by the patients this practice helps reconstructing the meanings of the daily life of person with so severe disability. Sensory stimuli are shown as potential tools for a reconfiguration of the assistance with someone seriously affected by a neurological disease. Psychoanalytic theories of Freudian and Lacanian orientation are also an important framework for the reconstruction assistance

The impact of the rehabilitation programme for cerebral palsy patients admitted into a care centre, Mopani, South Africa.

The United Nations Children’s Fund estimates the worldwide prevalence to be 150 million children under 18 years older, the World Health Organization also estimates that there are 93 million children with impairment. This study focused at describing the impact of rehabilitation programme for the young adult living with cerebral palsy admitted in a care centre Mopani District, South Africa. The study utilized a quantitative, descriptive cross-sectional survey method. Data was collected using an observational check list. The target population is young adults’ aged 18- 35 yearswith cerebral palsy in a Care Centre. Sampling was not necessary in this study since the targeted population was small in number. Confidentiality of respondents’ information was maintained where study subjects wereidentifiedusing codes. Data were analysed using descriptive statistics and the results of the analysis are presented in the form of tables and charts to enhance clarity. The results are presented according to the objectives. The conclusion and recommendationswere made based on the findings. The study results show that there were more females than males (ratio 3:1) and the range of age distribution was 20 to 37 years. The study also finds out that cerebral palsy condition was accompanied by other complications such as blindness, mental retardation, epilepsy and hydrocephalus. The study indicated that rehabilitation can only have minimal effects but it can improve the quality of life of the patient. With regard to communication outcome of the rehabilitation programme it indicates progress following rehabilitation between 57% and 80% could use expressive language and follow basic instructions. The study also observed that patients made a steady improvement right through from those who spent 4 to 8 years to those who had been there for long, 15 years and above. Since cerebral palsy can affect motor development as well as social development, the rehabilitation seems beneficial to focus on intervention programme on the cognitive stimulation of children and young adults with cerebral palsy. / NRF

State Created Danger and Chronic Traumatic Encephalopathy in Texas Schools: A Legal Examination

The recent breakthroughs in the research of brain trauma as it relates to athletics has revealed never before seen damage linked with football participation. Known as chronic traumatic encephalopathy (CTE), it is caused by activities inherent to the sport of football. School sponsored football has been granted immunity through case law and legislation in Texas. Recent research however indicates a different type of injury than those that school districts have been protected of liability from. This will provide a challenge to the immunity school districts have enjoyed regarding football. One route to abrogation of immunity is the state created danger provision established by the Supreme Court. The Fifth Circuit Court whose jurisdiction includes Texas has yet to officially adopt the state created danger theory. This research examined legal precedent to determine the challenge CTE may present the various layers of immunity surrounding school sponsored football in Texas.

[ES] Desde la Universitat Politècnica de València, y en un espacio temporal que abarca de 2015 a 2019, esta investigación en el ámbito de la arteterapia en Valencia se sitúa en un contexto de unidades de daño cerebral y hospitales dirigidos al mismo proceso de rehabilitación de pacientes agudos y crónicos con daño cerebral adquirido por ictus. Con objeto de fomentar las habilidades ejecutivas de estas personas, y reforzar su autoconfianza como individuos, el principal objetivo del estudio expone en qué medida la implementación de programas educativos desde una perspectiva artístico-terapéutica, dedicados a estimular la creatividad de sus participantes, pueden aportar un incremento de conocimientos científicos que enriquezcan su operatividad durante el periodo de rehabilitación y resto de praxis cotidianas. Neuropsychological Functioning in Active Duty Soldiers with Physical and/or Psychological Trauma

This quasi-experimental study investigates neuropsychological functioning differences between 63 active duty soldiers who were placed into three groups (MTBI, PTSD, control) to provide better information for differentiating PTSD and MTBI. The ANAM and MicroCog were utilized to measure psychomotor speed, memory, and attention. Participants with PTSD performed worse on most measures of psychomotor speed and attention, and endorsed more symptoms of depression and anxiety when compared to MTBI and control participants. Further, attention appears to be the best cognitive domain for differentiating PTSD from MTBI, whereas memory variables did not differentiate these groups. Clinical and research implications of these findings are discussed.

Text in Afrikaans / In this study an instrument is suggested for implementation by the Educational Psychologist to design a support programme, aimed at handling family members' stress where a child has sustained a brain injury. Attention was paid to the phenomenon "brain injured child" to ascertain demands and needs (physical, cognitive, psychological and emotional). Specific note was taken of the toll on each family member in their observance, experience, assistance and giving· meaning to the child. Reference was made to existing support programmes for such family members from the acute care phase to the final acceptance and readjustment of the family. With this study the need for a continuous support programme and the contents of such a programme was addressed. Using these guidelines the Educational Psychologist will be able to prepare the family for the stress possibilities in dealing with the brain injured child. / Met hierdie studie is 'n instrument daargestel vir die ontwerp van 'n hulpverleningsprogram vir gebruik deur die Opvoedkundige Sielkundige. Hierdie hulpprogram het as doel, die hantering van stres, deur die gesin van 'n kind, wat 'n breinbesering opgedoen het. In die studie is aandag gegee aan die tipiese gedrag wat oor die algemeen van 'n breinbeseerde kind verwag kan word. Daar is ook gepoog om te bepaal hoe elke lid van die gesin die breinbeseerde kind beleef, aan hom betekenis gee, en hom probeer help ten opsigte van die eise (fisiek en emosioneel) wat hy stel. Verder is daar gekyk na bestaande hulpverlening (gerig op die hantering van stres) aan die gesinslede van 'n breinbeseerde kind vanaf die akute versorgingsfase tot en met die aanpassing en herorganisering van die gesin. Met hierdie studie is 'n behoefte aan 'n kontinue hulpverleningsprogram en die inhoud van so 'n program by gesinslede aangespreek. 漢語失語症病人聲調錯誤之心理語言學研究 / A Psycholinguistic Study On Aphasic Tone Errors In Mandarin

正常人的語誤中可以取得在心理語言上何為音韻單位的證據,近來有許多針對失語症的病人語誤的研究,發現失語症病人的語誤也可當做測試語言表徵的重要來源資料。   本研究針對失語症病人在聲調方面表現,做聲調錯誤頻率分佈、語境影響錯誤、以及錯誤方向種類之分析 然後將整失語症病人聲調表現的分析跟心理語言學語言產出機制作連結。   本研究結果顯示,漢語失語症病人聲調表現上第四生錯誤雖然最多,但第四聲在漢語裡原本出現的頻率就最高,所以第四聲不應被認為對失語症病人最為困難。而第三聲雖然在漢語裡出現頻率最低,卻佔了失語症病人聲調錯誤頻率第二高位,因此我們判斷第三聲對失語症病人來說是最為困難的聲調。第二,我們發現漢語失語症病人聲調錯誤受語境影響比正常人低。第三,漢語失語症病人聲調錯誤裡,語境影響語誤之方向性中,錯誤來源在語誤出現前類型較多。第四,我們發現Dell的連續模型(connectionist model),可以解釋失語症病人在聲調錯誤類型偏好原因。 / Naturally occurring speech errors have been used as evidence for testing the psychological reality of phonological units. A number of researchers start to look at evidence from aphasic speech for testing linguistic representation so the phenomena of aphasia provide tests of the validity of certain aspects of linguistic theory.   In this thesis, tone errors made by aphasics were examined and related overall analyses to psycholinguistic models of speech production. In light of the analysis of tone frequency in Mandarin written texts and oral data and tone errors produced by aphasics, relating issues concerning the research questions we put forward were elucidated. First, it seems that tone 4 is the most problematic tone for the aphasics in terms of error rate since tone 4 occurs the most often in errors. However, tone 4 errors intrinsically will occur more that other three tones because of its highest incidence in Mandarin. Thus, tone 4 may not be taken as the most difficult tone for aphasics merely from its high error rate. Second, the results indicated that non-contextual errors constitute most of the aphasic errors. Third, it has been shown that perseveration is a cross-linguistic feature of aphasia. Fourth, Dell’s connectionist model is proved to be able to account for and make predictions about the error patterns found in both aphasic and normal speech.
