Duzentas e quarenta e duas mulheres na pós-menopausa, sintomáticas e com dislipidemia foram randomizadas em três grupos de tratamento: A) estradiol (E2) 1mg/acetato de noretisterona (NETA) 0,5mg [E2/NETA] + sinvastativa 20mg; B) E2/NETA + placebo; e C) sinvastatina 20mg + placebo. A eficácia de cada tratamento foi avaliada pela melhora do perfil lipídico e lipoprotéico e dos sintomas climatéricos ao final de 16 semanas de tratamento. O colesterol total, o LDL-C, o colesterol não-HDL e a Apo B diminuíram de forma significativa (p<0,0001) ao final de 16 semanas no grupo que utilizou E2/NETA + sinvastatina e naquele tratado com sinvastatina + placebo. A relação Apo B/Apo A1 também apresentou redução significativa nestes dois grupos (p<0,0001 e p=0,0026 respectivamente). A Apo A1 diminuiu apenas no grupo que recebeu E2/NETA + sinvastatina (p=0,0055). O grupo E2/NETA + placebo não apresentou alterações significativas no perfil lipídico e lipoprotéico entre as visitas basal e final. Aquele que utilizou E2/NETA + sinvastatina apresentou redução significativa do HDL-C e da Apo A1 quando comparado às usuárias de sinvastatina + placebo (p=0,0233 e p=0,0231 respectivamente). Symptomatic postmenopausal women (n=242) with dyslipidemia were randomized to one of three treatment groups: A) 1mg estradiol/0.5mg norethisterone acetate (E2/NETA) + 20mg simvastatin; B) E2/NETA + placebo; or C) 20mg simvastatin + placebo. Lipid and lipoprotein profiles and menopausal symptoms were evaluated after 16 weeks. Este estudo investigou o efeito da proteína - conglicinina isolada da soja, frente à resposta de um fármaco hipolipemiante (fenofibrato), em animais submetidos a uma dieta hipercolesterolêmica. Cinquenta e quatro ratos machos Wistar foram divididos em 6 grupos (n=9). O grupo controle (STD) recebeu uma dieta de caseína, e os demais hipercolesterolêmicos (HC), - conglicinina de 200mg/kg (HC+7S1), e 300mg/kg (HC+7S2), fenofibrato de 30mg/kg (HC+FF) e -conglicinina 300mg/kg associada com fenofibrato 30mg/kg (HC+7S2+FF) receberam dieta de caseína suplementada com 1% de colesterol e 0,5% de ácido cólico. Não foram observadas, nos tratamentos, alterações no crescimento, tecido adiposo, consumo e eficiência alimentar. O fenofibrato provocou aumento do peso e da concentração de triglicerídeo hepático. A proteína - conglicinina mostrou reduzir significativamente os efeitos dislipidêmicos séricos e hepáticos provocados pela dieta hipercolesterolêmica; no entanto, não foi observada adição destes efeitos quando associada ao fenofibrato. Os resultados reportados neste estudo sugerem que esta proteína tenha importante ação na regulação do metabolismo lipídico. Tais efeitos podem estar associados a peptídeos desta proteína, muito embora os mecanismos envolvidos não permaneçam completamente esclarecidos. / Abstract: A great interest has been reported about plant proteins, particularly those of soybean whose consumption has shown potential alternatives to dyslipidemia and atherosclerosis prevention. This study investigated the effect of -conglycinin protein isolated from soybean and its response to hypolipemiant drug (fenofibrate) in animals submitted to a hipercolesterolemic diet. Fifty four male Wistar rats were divided into 6 groups (n = 9). The control group (STD) had received casein diet and the others hypercholesterolemic (HC), -conglycinin of 200mg/kg (HC+7S1) and 300mg/kg (HC+7S2), fenofibrate 30mg/kg (HC+FF) and -conglycinin of 300mg/kg associated to fenofibrate of 30mg/kg (HC+7S2+FF) received casein diet supplemented with 1% of cholesterol and 0.5% of cholic acid. There was no change in growth, adipose tissue, consumption and efficiency food for all treatments. The fenofibrate showed increase in weight and hepatic triglycerides. The -conglycinin protein had showed significantly reduce the effects in dyslipidemic serum and liver caused by hypercholesterolemic diet, however; it was not observed some additional effects when associated with fenofibrate. The results reported in this study suggest that the protein has an important action in the regulation of lipid metabolism. Comparative Effectiveness and Safety of Lipid Lowering Agents for the Treatment of Dyslipidemia in HIV-Positive Individuals

As the HIV-positive population ages, managing non-AIDS-related comorbidities such as cardiovascular disease (CVD) complicates HIV care. Effectively treating risk factors for CVD will help reduce its burden in the HIV-infected population. However, the evidence base for the efficacy of statins as lipid-lowering therapies in HIV-infected patients has yet to be synthesized. Most trials do not compare statins directly to each other. In the absence of head-to-head evidence, the relative treatment effects of different statins can be indirectly obtained through a network meta-analysis (NMA). This NMA aims to evaluate the use of statins for treating dyslipidemia in HIV-infected individuals. Bayesian methods were used for obtaining treatment effect estimates and probabilistic rankings of treatments. Among lipid-lowering therapies, statins were most effective in treating dyslipidemia. All statins were found to offer the same treatment benefits. To our knowledge, this is the first NMA on this topic. It provides clinicians, health economists, and policy decision-makers with precise and reliable estimates for making definitive recommendations for the use of statins in dyslipidemic HIV-positive patients.

Prevalence and Risk Factors for Dyslipidemia Among Adults in Rural and Urban China: Findings From the China National Stroke Screening and Prevention Project (CNSSPP)

Background: Dyslipidemia is a modifiable risk factor for cardiovascular disease (CVD). We investigated the prevalence and associated risk factors of dyslipidemia- raised total cholesterol (TC), raised triglycerides (TG), raised low-density lipoprotein (LDL-C), low high-density lipoprotein (HDL-C), and raised non-high-density lipoprotein (non-HDL-C) in rural and urban China. Methods: We analyzed data from 136,945 participants aged 40-100 years of the CNSSPP project for 2014. Dyslipidemia was defined by the NCEP-ATP III and the 2016 Chinese guidelines for the management of dyslipidemia in adults. Complete data on demographic, metabolic and lifestyle characteristics were used. Chi-square tests and multivariable logistic regression were used to obtain age- and sex-adjusted prevalence and risk factors for dyslipidemia among participants. Results: A total of 53.1% participants lived in rural areas. The prevalence of dyslipidemia was similar among rural and urban participants (43.2% vs. 43.3%). Regarding the components of dyslipidemia: urban compared with rural participants had a higher prevalence of low HDL-C (20.8% vs. 19.2%), whereas the prevalence of raised LDL-C (7.8% vs. 8.3%), raised TC (10.9% vs.11.8%) and raised non-HDL-C (10.0% vs. 10.9%) were lower in urban residents, (all p < 0.001). Women were more likely to have raised TC than men (adjusted OR [AOR] =1.83, 95% confidence interval [CI]:1.75-1.91), raised LDL-C (AOR = 1.55, 95% CI: 1.47-1.63) and high non-HDL-C (AOR = 1.52 95% CI: 1.45-1.59) (all p < 0.001). Compared with rural, urban participants had higher odds of dyslipidemia: low HDL-C (AOR = 1.04, 95% CI: 1.01-1.07), and raised TG (AOR = 1.06, 95% CI: 1.04-1.09). Hypertension and current drinker were less likely to get low HDL-C with AOR 0.93 (95% CI: 0.90-0.96) and AOR 0.73 (95% CI: 0.70-75), respectively. Overweight, obesity, central obesity and diabetes had higher odds of all dyslipidemias (p < 0.001). Conclusions: Low HDL-C was higher in urban areas, whereas the remaining dyslipidemia types were more common in rural areas. Dyslipidemia was more common in women in both areas of residence. Overweight, obesity, central obesity and diabetes were associated with dyslipidemias. The need to intensify intervention programs to manage dyslipidemia and risk factors should be prioritized.

Efeito da dislipidemia sobre a capacidade de defesa - estudo experimental / Effect of dyslipidemia on defense capability- experimental study

A dislipidemia é geralmente caracterizada pela presença de níveis alterados e/ou elevados de colesterol total plasmático, LDL, triglicerídeos e de HDL. Em muitos casos, a dislipidemia está também associada à obesidade, embora o indivíduo possa apresentar alterações de colesterol sem ser obeso. São encontrados na literatura trabalhos clínicos que mostram uma clara relação entre doença periodontal e dislipidemia, sendo essa relação também demonstrada em outras situações de inflamação e infecção. No entanto, embora existam sugestões de que a relação inversa também possa ser verdade, algumas limitações observadas nos estudos não nos permitem assegurar que a doença sistêmica (dislipidemia) poderia estar alterando a resposta inflamatória (doença periodontal). Sendo assim o objetivo do presente trabalho foi avaliar o comportamento do epitélio gengival e a resposta inflamatória em ratos wistar, em condição de dislipidemia experimental. A dislipidemia foi induzida pela adição de colesterol à ração dos animais, e a inflamação local induzida por aplicação de LPS (lipopolissacarídeo bacteriano- E. coli). Foram avaliados fatores locais (infiltrado inflamatório na região gengival) e aspectos sistêmicos (níveis séricos de mediadores da resposta inflamatória IL-6, IL-1&beta; e TNF-&alpha;, e proteínas envolvidas na regulação da resposta inflamatória: SLPI e leptina). Nos animais com dislipidemia os resultados da análise histológica mostraram um aumento dos parâmetros inflamatórios (edema, infiltrado inflamatório, pavimentação leucocitária e hiperemia), principalmente na 4°semana de aplicação do LPS. As citocinas analisadas e a leptina, moléculas pró-inflamatórias, estavam mais elevadas nos animais com dislipidemia, já o SLPI, envolvido na redução do dano causado pela inflamação, estava reduzida nesses animais em relação aos animais sem dislipidemia que também receberam aplicações de LPS. Diante de nossos resultados, concluímos que a dislipidemia pode alterar a resposta inflamatória à infecção. / Dyslipidemia is characterized by elevated or altered levels of plasma total cholesterol, LDL, HDL and triglycerides. In many cases, dyslipidemia is associated with obesity, although individuals may present altered levels of cholesterol without being obese. Studies have shown an interrelationship between periodontal disease and dyslipidemia. There are, in the literature studies showing a clear relationship between periodontal disease and dyslipidemia, which is also demonstrated in other situations of inflammation and infection. However, although there are suggestions that the inverse relationship can also be true, some limitations observed in the studies did not allow us to ensure that systemic alteration (dyslipidemia) could be influencing the inflammatory response (periodontal disease). Therefore, the objective of this study was to evaluate the behavior of the gingival epithelium and inflammatory response in wistar rats under conditions of experimental dyslipidemia. Dyslipidemia was induced by increasing the amount of cholesterol in the diet of animals, and local inflammation was induced by the application of LPS (E-coli lipopolissacarídeo). We assessed local factors (inflammatory infiltrate in the gingival area), and systemic aspects (inflammatory mediators levels as IL-6, IL-1&beta; and TNF-&alpha;) and the plasma levels of proteins involved in the regulation of inflammatory response (SLPI and leptin). In animals with dyslipidemia histopathological analysis showed an increased level of inflammatory parameters (edema, inflammatory infiltrate, leukocyte margination and hyperemia), particularly in the 4th week after LPS application. Cytokines and leptin presented higher plasma levels in animals with dyslipidemia, but SLPI, considered a molecule involved in tissue protection, mostly in situations of inflammation or infection, was reduced in these animals compared to their controls. Our results allow us to conclude that dyslipidemia may alter the inflammatory response to infection.

The result is a tucumã regional physico-chemical features important for the treatment of dyslipidemia, such as substantial concentration of fibers. Thus, the present study aims to conduct in vivo study in rats untrained and trained on the use of tucumã the Amazons in the treatment of diet-induced dyslipidemia. Was performed for both the induction of dyslipidemia followed by treatment with a base ration tucumã with or without swimming. We used 40 male rats weighing about 350 grams. These animals were divided into 4 groups of 10 animals forming the Sedentary Control Group (SCG) the exercised control group (GCE), the Group Treatment Sedentary (GTS) and Treatment Group exercised (GTE). It is the body mass gain, feed intake, plasma concentrations of cholesterol, triglycerides, HDL-C, LDL-C, VLDL, total protein, and the hormones insulin and leptin. The GCE group showed a tendency to higher food consumption and the groups that consumed the flesh of the fruit tucumã showed a trend to a higher body mass gain. The descriptive analysis showed that the groups that consumed the diet increased the fruit tucumã showed a trend to a higher body mass gain. The GCE has a tendency to higher plasma cholesterol, as well as the GTE GTS and have higher absolute values for triglycerides and VLDL-C HDL lipids and LDL-C were shown to be elevated in the group that consumed tucumã. Through research it can be seen that the fruit tucumã (Meyer Astrocatyum aculeatum) hypolipidemic has no effect, since the animals had elevated concentrations of lipid / As dislipidemias estão relacionadas ao desenvolvimento de doenças cardiovasculares e cerebrovasculares e, portanto, é considerado um fator de risco importante que pode ser tratado através de fármacos ou mesmo com mudanças nos hábitos alimentares e físicos. O tucumã é um fruto regional que apresenta características físico-químicas importantes para o tratamento das dislipidemias, como a concentração considerável de fibras. Diante disso, o presente estudo tem como objetivo realizar o estudo experimental in vivo em ratos Wistar sedentários e exercitados sobre o consumo de tucumã do amazonas no tratamento da dislipidemia induzida por dieta. Para tanto foi realizado a indução da dislipidemia seguido do tratamento com ração a base de tucumã associado ou não a natação. Foram utilizados 40 ratos machos pesando em média 350 gramas. Esses animais foram divididos em 4 grupos com 10 animais formando o Grupo Controle Sedentário (GCS) o Grupo Controle Exercitado (GCE), o Grupo Tratamento Sedentário (GTS) e o Grupo Tratamento Exercitado (GTE). Verificou-se o ganho de massa corporal, consumo de ração, a concentração plasmática de colesterol, triglicerídeos, HDL-c, LDL-c, VLDL, proteínas totais, além dos hormônios insulina e leptina. O grupo GCE apresentou tendência ao maior consumo alimentar e os grupos que consumiram a polpa do fruto tucumã apresentaram uma tendência ao maior ganho de massa corporal. A análise descritiva demonstrou que os grupos que consumiram a dieta acrescida do fruto tucumã apresentaram uma tendência ao maior ganho de massa corporal. O grupo GCE apresenta uma propensão a maior concentração plasmática do colesterol, assim como os GTS e GTE apresentam maiores valores absolutos para os triglicerídeos e para o VLDL Os lipídios HDL-c e LDL-c mostraram-se elevados nos grupos que consumiram tucumã. Através da pesquisa realizada pode-se observar que o fruto tucumã (Astrocatyum aculeatum Meyer) não apresenta efeito hipolipidêmico, visto que os animais apresentaram concentrações elevadas dos lipídeos

CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior / Diabetes and dyslipidemia prevalence has been increasing globally configured as an epidemic resultant mainly from overweight, physical inactivity and genetic susceptibility. There are reports of many natural products that have hypoglycemic and hypolipidemic activity. Among them we mention the terpenes, which are the largest group of secondary products of plants metabolism. The terpene studied in this work was betulinic acid (BA), a pentacyclic triterpene lupano type that presents a variety of biological and pharmacological activities. The objective of this study was to evaluate BA hypoglycemic and hypolipidemic effects in experimental protocols of dyslipidemia and diabetes induced pharmacologically as well as studying their toxic potential in vivo. The BA-treated groups received doses of 5 (BA5), 10 (BA10) and 20 (BA20) mg/kg. The evaluation of the BA hypoglycemic action was carried out through diabetes induced by alloxan protocol and oral glucose tolerance test (OGTT). To check their activity on lipid metabolism, it was carried out dyslipidemia protocol induced by Triton WR 1339 intraperitoneal injection. In addition, it was performed the protocol modified diet-induced hypercholesterolemia. Toxicity was assessed by the study repeated doses for 28 days treating daily with BA doses via gavage. After the protocol of alloxan-induced diabetes, there was a blood glucose reduction in groups BA10 and BA20. Triglycerides and total cholesterol decreased significantly at all doses studied. BA10 treatment, also reduced the blood glucose peak caused by glucose overload (2g/Kg) in the OGTT. After 24 hours of dyslipidemia induced by triton, there was a significant triglycerides reduction in groups treated with BA at doses of 10 and 20mg/Kg. After 48h, triglycerides levels remained reduced in group treated with BA10. In hypercholesterolemia induced by diet modified protocol BA at doses of 10 and 20mg/kg, promoted a significant decrease in total cholesterol levels. There were no significant changes in the parameters evaluated after repeated dose oral toxicity protocol. Results demonstrate the therapeutic potential and safety of betulinic acid in the treatment of dyslipidemia and diabetes, although others pre-clinical and clinical studies are necessary for its use by population. / A prevalÃncia da diabetes e das dislipidemias vem crescendo mundialmente configurando-se como uma epidemia resultante,principalmente, do excesso de peso, da inatividade fÃsica e da suscetibilidade genÃtica. Existem relatos de muitos produtos de origem natural que possuem atividade hipoglicÃmica e hipolipidÃmica. Dentre eles, podemos citar os terpenos, que constituem o maior grupo de produtos do metabolismo secundÃrio de plantas. O terpeno estudado no presente trabalho à o Ãcido betulÃnico (AB), um triterpeno pentacÃclico do tipo lupano que apresenta uma variedade de atividades biolÃgicas e farmacolÃgicas. Assim, o objetivo deste estudo foi avaliar o efeito hipolipidÃmico e hipoglicÃmico do AB em protocolos experimentais de dislipidemias e diabetes induzidas farmacologicamente bem como estudar seu potencial tÃxico in vivo. O grupos tratados com AB receberam as doses de 5(AB5), 10(AB10) e 20(AB20)mg/Kg. A avaliaÃÃo da aÃÃo do hipoglicÃmica do AB foi atravÃs do protocolo de diabetes induzida por aloxano e o teste oral de tolerÃncia a glicose (TOTG). Para verificar sua atividade sobre o metabolismo lipÃdico, foi realizado o protocolo de induÃÃo da dislipidemia atravÃs da injeÃÃo intraperitoneal de triton WR 1339. AlÃm disso, foi realizado o protocolo de hipercolesterolemia induzida por dieta modificada. A toxicidade foi avaliada pela realizaÃÃo do estudo toxicolÃgico de doses repetidas durante 28 dias mediante administraÃÃo Ãnica diÃria por via oral de AB. Depois do protocolo de diabetes induzida por aloxano, observou-se uma reduÃÃo da glicemia nos animais dos grupos AB10 e AB20. Os triglicerÃdeos e o colesterol total reduziram significativamente em todas as doses estudas. O tratamento com AB10, reduziu ainda o pico glicÃmico causado pela sobrecarga de glicose (2g/Kg) no TOTG. ApÃs 24h da induÃÃo com triton, verificou-se a reduÃÃo significativa dos triglicerÃdeos nos grupos tratados com AB nas doses de 10 e 20mg/Kg. Depois de 48h, os animais do grupo AB10 manteve tal reduÃÃo. No protocolo de hipercolesterolemia induzida por dieta modificada o AB nas doses de 10 e 20mg/Kg, promoveu uma diminuiÃÃo significativa do colesterol total plasmÃtico. NÃo foram encontradas alteraÃÃes significativa nos parÃmetros avaliados apÃs protocolo de toxicidade oral em doses repetidas. Os resultados obtidos demonstram o potencial terapÃutico e a seguranÃa do Ãcido betulÃnico no tratamento das dislipidemias e diabetes, apesar de serem necessÃrios novos estudos prÃ-clÃnicos e clÃnicos para sua utilizaÃÃo no mercado.

Association Between Obesity and Therapeutic Goal Attianment in Patients with Concomitant Hypertension and Dyslipidemia

Objective: A retrospective study was conducted to examine the variations in therapeutic {blood pressure (BP) and lipid} goal attainment and medication utilization pattern in patients with concomitant hypertension and dyslipidemia, specifically comparing obese versus non-obese patients. Methods: GE Centricity EMR database containing data from 2004-2011 was utilized. 9,086 patients diagnosed with concomitant hypertension and dyslipidemia were evaluated. Goal attainment and treatment pattern for BP and lipid levels was assessed based on JNC 7 and NCEP ATP III guidelines, respectively. Results: Substantial proportion of patients with concomitant hypertension and dyslipidemia failed to attain BP and lipid goals. Further, in patients with concomitant hypertension and dyslipidemia, obesity appears to be an independent risk factor for the failure to attain BP and dual BP/LDL-C. Conclusions: With increasing prevalence of obesity and inadequate therapeutic goal attainment in these populations, healthcare professionals should use appropriate treatment strategies for improving the management of cardiovascular risk factors (hypertension, dyslipidemia, and obesity). / Mylan School of Pharmacy and the Graduate School of Pharmaceutical Sciences / Pharmacy Administration / MS / Thesis

Efeito da proteína Beta-Conglicinina da soja no metabolismo lipídico de animais submetidos à dieta hipercolesterolêmica

Este estudo investigou o efeito da proteína - conglicinina isolada da soja, frente à resposta de um fármaco hipolipemiante (fenofibrato), em animais submetidos a uma dieta hipercolesterolêmica. Cinquenta e quatro ratos machos Wistar foram divididos em 6 grupos (n=9). O grupo controle (STD) recebeu uma dieta de caseína, e os demais hipercolesterolêmicos (HC), - conglicinina de 200mg/kg (HC+7S1), e 300mg/kg (HC+7S2), fenofibrato de 30mg/kg (HC+FF) e -conglicinina 300mg/kg associada com fenofibrato 30mg/kg (HC+7S2+FF) receberam dieta de caseína suplementada com 1% de colesterol e 0,5% de ácido cólico. Não foram observadas, nos tratamentos, alterações no crescimento, tecido adiposo, consumo e eficiência alimentar. O fenofibrato provocou aumento do peso e da concentração de triglicerídeo hepático. A proteína - conglicinina mostrou reduzir significativamente os efeitos dislipidêmicos séricos e hepáticos provocados pela dieta hipercolesterolêmica; no entanto, não foi observada adição destes efeitos quando associada ao fenofibrato. Os resultados reportados neste estudo sugerem que esta proteína tenha importante ação na regulação do metabolismo lipídico. Tais efeitos podem estar associados a peptídeos desta proteína, muito embora os mecanismos envolvidos não permaneçam completamente esclarecidos. / A great interest has been reported about plant proteins, particularly those of soybean whose consumption has shown potential alternatives to dyslipidemia and atherosclerosis prevention. This study investigated the effect of -conglycinin protein isolated from soybean and its response to hypolipemiant drug (fenofibrate) in animals submitted to a hipercolesterolemic diet. Fifty four male Wistar rats were divided into 6 groups (n = 9). The control group (STD) had received casein diet and the others hypercholesterolemic (HC), -conglycinin of 200mg/kg (HC+7S1) and 300mg/kg (HC+7S2), fenofibrate 30mg/kg (HC+FF) and -conglycinin of 300mg/kg associated to fenofibrate of 30mg/kg (HC+7S2+FF) received casein diet supplemented with 1% of cholesterol and 0.5% of cholic acid. There was no change in growth, adipose tissue, consumption and efficiency food for all treatments. The fenofibrate showed increase in weight and hepatic triglycerides. The -conglycinin protein had showed significantly reduce the effects in dyslipidemic serum and liver caused by hypercholesterolemic diet, however; it was not observed some additional effects when associated with fenofibrate. The results reported in this study suggest that the protein has an important action in the regulation of lipid metabolism. These ones may be associated with their peptides although the mechanisms involved are not already completely understood.
