The Normative Architecture of Reality: Towards an Object-Oriented Ethics

The fact-value distinction has structured and still structures ongoing debates in metaethics, and all of the major positions in the field (expressivism, cognitivist realism, and moral error theory) subscribe to it. In contrast, I claim that the fact-value distinction is a contingent product of our intellectual history and a prime object for questioning. The most forceful reason for rejecting the distinction is that it presupposes a problematic understanding of the subject-object divide whereby one tends to view humans as the sole source of normativity in the world. My dissertation aims to disclose the background against which human ethical praxis is widely seen as a unique and special phenomenon among other phenomena. I show that ethical norms, as delimited by utilitarianism, deontology, virtue ethics, etc., derive from an originary proto-ethical normativity at the heart of the real itself. Every object, human and nonhuman, presents itself as a bottomless series of cues or conditions of appropriateness that determine adequate and inadequate ways of relating to it. That is, objects demand something from other objects if they are to be related to; they condition other objects by soliciting a change in disposition, perception, or sense, and for this reason are sources of normativity in and unto themselves. Ethical norms, or values, are the human expression of the adequacy conditions with which all objects show themselves. In the post-Kantian landscape it is widely thought that human finitude constitutes the origin of ethical norms. Consequently, the world is divided up into morally relevant agents (humans) on one side, and everything else on the other. Adopting a deflationary view of agency, I argue that human-human and human-world relations differ from other relations in degree rather than kind. Thus, instead of a fact-value distinction, value is inextricably bound up with the factual itself. The critical upshot of my project is that traditional subject-oriented ethical theories have served to conceal the real demands of non-human objects (such as animals, plants, microorganisms, and artificially intelligent machines) in favor of specifically human interests. Such theories have also been leveraged frequently in exclusionary practices with respect to different groups within the human community (e.g. women and those of non-European descent) based on arbitrary criteria or principles.

The Psychosocial Influence of Dental Esthetics in Adolescents as Judged by Peers

The purpose of this study was to determine if dental esthetics affects the perception of adolescents regarding their peer's social aptitude in specific areas such as athletics, popularity, leadership, and academics. Ten patient photographs (4 males, 6 females) from a private orthodontic practice in Richmond, VA were selected for inclusion in this study. The frontal smiling photographs were digitally modified with insertion of either an ideal smile (straight) or a non-ideal smile (crooked) to create an image identical to the original except for an altered dental complex. Laypersons rated the straight and crooked patient photographs for athletic performance, socialization skills (popularity), leadership, and academic performance using a 100 mm visual analog scale (VAS). Picture ratings were analyzed using a repeated-measures mixed-model analysis (SAS version 9.1, SAS Institute, Cary NC). On average, the differences due to crooked vs. straight were consistent in that patients with straight teeth were always rated more highly than the same patients with crooked teeth. These differences were significant in ratings of perceived athletic performance (P .05).

Faire du rapport des individus à leur apparence corporelle une véritable question sociologique, telle fut l'ambition de cette thèse. Prenant appui sur un étonnement intellectuel - relatif à la difficile constitution du champ de la sociologie du corps et au décalage fort entre l'omniprésence des enjeux esthétiques dans le monde social (notamment médiatique) et leur modeste considération dans l'espace des sciences sociales - cette visée s'est matérialisée par la mise en oeuvre d'une enquête qualitative, menée entre 2013 et 2014, auprès de 60 individus (32 femmes et 28 hommes), âgés de 21 à 52 ans. Cette étude, dont le fil conducteur a été de mettre en exergue les modalités de l'expérience esthétique des individus (coûts, bénéfices, vécus, stratégies, dans le rapport à soi et à autrui), s'est appuyée sur l'outil analytique du genre, véritable catalyseur de l'injonction à l'autonomie et à la réflexivité esthétique à l'oeuvre depuis la seconde moitié du XXème siècle. Les deux axes analytiques qui orientent la trame de cette thèse sont ainsi les suivants : (1) quels sont les enjeux des dimensions « intra-genre », « inter-genre » dans la construction d'une identité esthétique, (2) dans quelles mesures la dialectique entre rapport à soi et rapport à autrui constitue-t-elle le socle de l'appropriation esthétique d'un soi genré ? Effectivement, en étudiant l'apparence comme une théâtralisation du genre des individus, nous avons notamment pu mettre en évidence combien la consistance des corps et les pratiques esthétiques représentaient des modalités essentielles de la recomposition contemporaine des existences genrées. Notre enquête nous a ainsi amenée à penser que la posture analytique la plus légitime pour une sociologie de l'apparence est celle qui envisage cet objet « apparence » avant tout comme une expérience réflexive à laquelle chacun se trouve confronté, le rapport au corps constituant un support identitaire central pour l'individu contemporain. En ce sens, l'étude du « bricolage esthétique de soi » des acteurs sociaux constitue à nos yeux une orientation pertinente pour les sociologues afin d'appréhender les contours de ce qu'être un individu (incarné) aujourd'hui dans notre société représente pour tout un chacun. / Studying the relationship of individuals with their own body and appearance as a real sociological question, such was the aim of this thesis. Building on an intellectual astonishment - the difficult constitution that marked the field of the sociology of the body and the huge gap between the omnipresence of aesthetic issues in the social world (including media) and their modest consideration in the Social Sciences world - this aim has been materialized through the implementation of a qualitative survey, conducted between 2013 and 2014, with 60 individuals (32 women and 28 men), aged from 21 to 52 years. The common thread of this study was to outline the terms of the aesthetic experience of individuals (costs, benefits, experiences, relationship with oneself and with others) and we used the analytical tool of gender, real catalyst of the autonomy and aesthetic reflexivity injunctions ongoing in our society since the second half of the twentieth century. The two analytical axes that guide the frame of this thesis are as follows: (1) what are the issues of the two dimensions: "intra-gender", "cross-gender" in the construction of an aesthetic identity, (2) to what extent the dialectic between self-relation and relationships with others could be the base of the aesthetic appropriation of a gendered self? Indeed, studying the appearance as a dramatization of the gender of individuals, has enabled us to highlight how the consistency of the body and aesthetic practices represente essential modalities regarding the contemporary restructuring of gendered lives. Our investigation let us think that the most legitimate analytical posture for a sociology of appearance is the one considering this item "appearance" first and foremost as a reflexive experience to which each individual is faced, relationship with the body constituting a central identity support for the contemporary individual. In this sense, the study of "self-esthetics handiwork" of social actors is in our opinion a relevant guidance for sociologists to grasp the framework of what being an (embodied) individual today in our society represents for everyone.

L'e-rationalité postmoderne : étude de l'être-ensemble électronique / Postmodern e-rationality : study of the being together electronic

L'étude des phénomènes sociaux actuels passe nécessairement par la reconnaissance du rôle de la technologie électronique dans l'ensemble des pratiques sociales. L'on parle, par exemple, d'une révolution culturelle pour montrer l'impact de la technologie numérique dans la vie quotidienne, mais quelles sont-elles vraiment les transformations qu'amène cette technologie à la socialité ? Les formes de vivre ensemble se sont transformées pour l'utilisation de la technologie numérique ? Voici les questions qu'on discute dans ce travail. Dans l'étude que nous présentons, l'on interprète ces transformations culturelles à partir d'un regard intime de l'expérience numérique. Dès notre filiation à la pensée maffesolienne, l'on fait une revendication du rapprochement esthétique pour l'étude sociologique, tout d'abord, car les motifs rationnels sont loin de montrer la richesse du lien social ; ensuite, parce que l'auteur a bien montré la puissance de l'interprétation autour au quotidien, à l'espace, aux formes sociétales. Enfin, puisque seulement dans la douceur de l'image était possible une compréhension de la socialité contemporaine. Pour conclure, on présente les tendances sociétales qui montrent cela que l'on appelle « l'e-rationalité postmoderne » qui n'est autre que l'articulation de l'être ensemble électronique attachée aux valeurs postmodernes. / The study of the current social phenomena inevitably goes through the acknowledgement of the role of electronic technology in social practices as a whole. We speak, for example, about a cultural revolution to emphasize the impact of digital technology in everyday life, but which are the transformations that this technology brings to the sociality? The ways we live together are transformed by the use of the digital technology? These are the questions which we discuss in this work. In the study we present, these cultural transformations are interpreted from an intimate look to the digital experience. From our affiliation to the thoughts of Michel Maffesoli, we vindicate an aesthetic approach to sociological studies; first of all, because rational motives are far from showing the richness of the social link; and then, because Maffesoli ('the author' serías tú) has clearly showed its power of interpretation of daily life, the space and societal forms. Finally, because it is only in the sweetness of the image that is possible to comprehend contemporary sociality. To conclude, we present the societal trends which demonstrate what we call "the postmodern e-rationality" which is no other than the articulation of an electronic being together, attached to the postmodern values.

Avaliação da estética gengival em próteses unitárias implanto-suportadas em áreas de fissura alveolar enxertada / Evaluation of gingival esthetics in single implant-supported prostheses at the grafted cleft area.

Com o crescente interesse pela obtenção de estética em reabilitações implanto-suportadas, os tecidos moles peri-implantares vêm sendo alvo de atenção das pesquisas em Implantodontia. A fissura alveolar congênita distingue-se neste contexto por apresentar dificuldades estéticas peculiares. Este estudo retrospectivo qualificou como satisfatória ou deficiente, a estética gengival de 65 próteses unitárias implanto-suportadas em área de fissura enxertada por meio de um novo índice que se baseou nos parâmetros: posição da margem gengival, papilas interproximais e coloração gengival. Procurou-se associar os escores obtidos com este índice, a situações presentes antes da instalação dos implantes (recessão gengival nos dentes vizinhos, bridas musculares, faixa de mucosa ceratinizada), a procedimentos cirúrgicos reconstrutivos (re-enxertos ósseos, enxertos gengivais, técnicas de reabertura a retalho) e ao número de cirurgias e de sessões de condicionamento gengival realizados com o objetivo de obter estética. Foi considerada satisfatória a gengiva peri-implantar de 42 (64,62%) das próteses. A recessão gengival nos dentes vizinhos, presença de bridas musculares e falta de mucosa ceratinizada foram os principais responsáveis pela falta de estética dos demais trabalhos. O único tipo de cirurgia que interferiu no resultado estético foi o enxerto gengival livre epitelizado. O número de sessões de condicionamento gengival não interferiu nos resultados. Concluiu-se que as condições anteriores à instalação dos implantes são mais importantes na obtenção da estética do que os procedimentos posteriores cirúrgicos reconstrutivos ou de condicionamento gengival. Além disto, o índice proposto mostrou-se adequado para o tipo de avaliação executada. / With the increasing interest on the achievement of esthetics in implant-supported rehabilitations, the peri-implant soft tissues have been addressed in several investigations in Implantology. The congenital alveolar cleft is distinguished within this context because of the inherent esthetic impairment. This retrospective study assessed the gingival esthetics of 65 single implant-supported prostheses at grafted cleft areas by a new index based on the following parameters: position of the gingival margin, interproximal papillae and gingival color. Esthetics was scored as satisfactory or poor. The scores achieved by this index were associated to the status before and after placement of the implants (gingival recession of the adjacent teeth, buccal frenula, width of keratinized mucosa), to the reconstructive surgical procedures (bone grafts, gingival grafts, reopening and flap techniques), and to the number of surgeries and gingival conditioning sessions performed to achieve esthetics. The peri-implant gingiva of 42 prostheses (64.62%) was considered satisfactory. Gingival recession on the adjacent teeth, presence of buccal frenula and lack of keratinized mucosa were the main factors influencing the poor esthetics of the other prostheses. The only type of surgery that interfered with the esthetic outcome was the epithelialized free gingival graft. The number of gingival conditioning sessions did not interfere with the outcomes. It was concluded that the status before placement of the implants are more important for achievement of esthetics than the reconstructive surgical posterior procedures or gingival conditioning. Moreover, the index proposed was adequate for this type of evaluation.

Avaliação da estética facial de pacientes com fissura transforame incisivo bilateral, submetidos ao protocolo de cirurgias do HRAC-USP / Evaluation of the facial esthetics of patients with complete bilateral cleft lip and palate submitted to the surgical protocol of HRAC-USP

Objetivo: avaliar por meio de fotografias de perfil e um questionário específico, a estética facial de indivíduos com fissura transforame incisivo bilateral (FTB), operados segundo o protocolo do HRAC-USP. Modelo: Estudo transversal, com análise das fotografias, atribuindo escores aos pacientes e comparando-os estatisticamente. Local de Execução: HRAC-USP. Participantes: Amostra: 30 pacientes, 24 do gênero masculino e 6 do gênero feminino, com idades entre 5 anos e 8 meses a 10 anos e 4 meses, leucodermas, com FTB . Examinadores: 5 Ortodontistas do HRAC-USP (OHRAC), 5 ortodontistas não relacionados à área de fissura (ONF), 5 Cirurgiões Plásticos do HRAC-USP (CPHRAC), 5 Cirurgiões Plásticos não relacionados à área de fissura (CPNF), 5 leigos (L) com formação superior. Variáveis: classificação da agradabilidade facial, em escala de 1 a 9, estruturas reconhecidas como responsáveis pela pior classificação. Resultados: O grupo OHRAC atribuiu mediana de 7 (aparência esteticamente agradável), o grupo CPHRAC atribuiu 5 (esteticamente aceitável). O grupo L atribuiu 4 (esteticamente aceitável). Os grupos ONF e CPNF, designaram 3 (esteticamente desagradável). Existiu diferença significante entre todas as categorias, exceto para os ONF e CPNF. Foram associadas à classificação esteticamente desagradável o nariz, o terço médio da face e o lábio superior. Conclusões: A estética facial da amostra foi classificada em agradável (grupo OHRAC), aceitável (grupos CPHRAC e L) e desagradável (grupos ONF e CPNF). Houve diferença significante entre os grupos, exceto para os ONF e CPNF. As estruturas associadas aos piores escores foram o nariz, o terço médio da face e o lábio superior. / Objective: to evaluate, by means of photographs of profile and a specific questionnaire, the facial esthetics of individuals with complete bilateral cleft lip and palate (CBCLP) operated according to the protocol of HRAC-USP. Design: cross-sectional study with analysis of the photographs, by assignment of scores to the patients and statistical comparison. Setting: HRAC-USP. Participants: sample: 30 white patients with CBCLP, being 24 males and 6 females, aged 5 years 8 months to 10 years 4 months. Examines: 5 orthodontists of HRAC-USP (OHRAC), 5 orthodontists not treating patients with clefts (ONC), 5 plastic surgeons of HRAC-USP (PSHRAC), 5 plastic surgeons not treating patients with clefts (PSNC), and 5 lay graduated examiners (L). Variables: classification of facial pleasantness in a scale from 1 to 9, structures recognized as responsible for the worst classification. Results: the OHRAC group assigned a median of 7 (esthetically pleasant appearance) and the PSHRAC assigned 5 (esthetically acceptable). Group L assigned 4 (esthetically acceptable). Groups ONC and PSNC assigned 3 (esthetically unpleasant). There was a significant difference between all categories, except for ONC and PSNC. The structures associated to the esthetically unpleasant classification were the nose, middle third of the face and upper lip. Conclusions: the facial esthetics of the sample was classified as pleasant (OHRAC group), acceptable (PSHRAC and L groups) and unpleasant (ONC and PSNC groups). here was a significant difference between groups, except for the ONC and PSNC. The structures associated to the worst scores were the nose, middle third of the face and upper lip.

Um olhar brasileiro sobre O Bairro, de Vasco Pratolini / A look at Il Quartiere by Vasco Pratolini

A presente pesquisa se desdobrou em dois movimentos diferentes e complementares. O primeiro se propôs a desenvolver um estudo da narrativa Il Quartiere, de Vasco Pratolini, a partir da sua contextualização na trajetória ficcional do escritor florentino. Publicada em 1945, esta obra é lida neste trabalho, segundo a orientação do próprio autor, que a projeta como resultado do esforço de aproximá-la do que afirma ser a sua verdade, elaborada a partir da consciência de classe e de literatura, construída até 1943, ano em que finaliza sua escritura. Em um período marcado pelo final da Segunda Guerra e, mais especificamente, pela Resistenza italiana, da qual o intelectual participou ativamente, o romance retoma os anos que antecederam tais eventos históricos, compreendendo um arco histórico que vai de 1930 a 1937. O segundo movimento se deu na elaboração de uma proposta de tradução de Il Quartiere, em diálogo com algumas teorias da tradução que, ao iluminarem aspectos importantes do livro, revelaram-se como processos metafóricos da própria obra. O objetivo dos resultados aqui apresentados visa sugerir que a estética realizada por atos de linguagem veicula a ética de vocação coletiva de Vasco Pratolini, reconhecida nas línguas que percorrem O Bairro e dizem dele mais do que parecem, quando vista por um olhar brasileiro. / This research has been unfolded into two different, but complementary, ways. In the first one we proposed to develop a study on the narrative Il Quartiere, by Vasco Pratolini, from its contextualization with the fictional trajectory of this Florentine author. Published in 1945, the work is read in this paper, according to the orientation of the author himself, who projects it as a result to reach what he calls his \"truth\", elaborated from the awareness of class and literature, built up to 1943, year when he finishes his writing. In a period marked by the end of the Second World War and more specifically by the Resistenza Italiana, in which the intellectual participated actively, the novel recovers the years that preceded such historic events, composing a historic arc that takes place from 1930 to 1937. The second way has emerged from the elaboration of a translation proposal to Il Quartiere, in a dialogue with some theories of translation that, by enlightening important aspects of the book, have revealed themselves as metaphorical processes of the work itself. The aim of the results hereby presented intend to suggest that esthetics performed by acts of language - conveys the ethic of Vasco Pratolinis collective vocation, recognized in the languages spoken in the neighborhood that tell us more about in than we estimate.

Estudo prospectivo sobre os resultados estéticos, funcionais e clínicos da craniotomia minipterional em comparação com a craniotomia pterional clássica / Prospective randomized study designed to compare aesthetics, functional and clinical results between minipterional and pterional craniotomies

Introdução: O acesso pterional é uma das craniotomias mais utilizadas. Entretanto, apresenta algumas desvantagens, como a dissecação ampla do músculo temporal, que pode causar atrofia e deformidade do contorno facial. A craniotomia minipterional descrita em 2007 propiciou exposição anatômica semelhante a da craniotomia pterional clássica. Objetivos: Comparar os resultados clínicos, funcionais e estéticos dos dois acessos cirúrgicos destinados ao tratamento de aneurismas da circulação anterior. Métodos: Cinquenta e oito doentes, com aneurismas rotos (40) e não rotos (18) foram admitidos no estudo. No grupo A, 28 indivíduos foram submetidos à craniotomia minipterional. No grupo B, 30 doentes foram operados com a craniotomia pterional clássica. Doentes com hematomas intracranianos, aneurismas do segmento oftálmico e aneurismas gigantes foram excluídos. Os resultados estéticos foram analisados por meio de dois métodos. No primeiro, uma autoavaliação utilizava uma régua escalonada de 0 a 100, na qual o zero representou o melhor resultado e 100 o pior resultado na percepção do próprio doente. Fotografias dos doentes foram mostradas a dois observadores independentes. Os resultados foram classificados como ótimo, bom, regular e mau (de acordo com uma escala pré-determinada). A gradação da atrofia foi mensurada por meio de três métodos, utilizando a tomografia computadorizada de crânio. No primeiro, observou-se a percentagem de redução do complexo músculo temporal, tecido subcutâneo e pele. No segundo método, a percentagem de redução da espessura do músculo temporal foi analisada isoladamente. O terceiro método foi a mensuração da volumetria do músculo temporal, tecido subcutâneo e pele calculados a partir da margem superior do arco zigomático até a linha temporal superior utilizando-se o software OsiriX (Pixmeo Sarl Geneva/ Suíça OsiriX). Para os resultados clínicos e funcionais, foi utilizada a Escala de Rankin modificada. Outras variáveis, como paralisia do ramo frontal do nervo facial, hemorragia pós-operatória, fistula liquórica, hidrocefalia e mortalidade, também foram analisadas. Resultados: Em ambos os grupos, os dados demográficos e as características pré-operatórias foram similares. A satisfação com o resultado estético foi observada em 79% (19) no grupo A e 52% (13) no grupo B (p=0,07). O valor médio da régua foi 27 no grupo A e 45,8 no grupo B (p=0,03). Quando somente doentes classificados como Rankin modificado 0 ou 1 foram analisados, o valor médio da régua foi de 25,2 no grupo A e 39,4 no grupo B (p=0,11). Dois avaliadores independentes analisaram as fotografias dos doentes e o coeficiente de correlação kappa para os resultados estéticos foi de 0,73. De acordo com os mesmos ótimo e bom, foram observados em 87% (21) no grupo minipterional e 48% (12) no grupo pterional. A percentagem de redução do músculo temporal, subcutâneo e pele (método 1) foi de 14,9% no grupo A e 24,3% no grupo B (p=0,01). Quando somente o músculo temporal foi analisado (método 2), a percentagem de redução foi de 12,7% no grupo A e 22% no grupo B (p=0,005). A redução volumétrica das estruturas (método 3) foi de 14,8% no grupo A e 24,5% no grupo B (p=0,012). Na avaliação clínica no 6? mês, os valores da Escala de Rankin modificada foram similares (p=0,99). O óbito ocorreu em 4 doentes no grupo A e 5 doentes no grupo B (p=1,0). Conclusão: Os resultados clínicos demonstraram que a craniotomia minipterional é um procedimento seguro, com prognóstico similar ao da técnica convencional. Os resultados cosméticos foram melhores com menor deformidade no contorno facial, uma vez que a percentagem de redução da espessura, do volume do músculo temporal, do tecido subcutâneo e da pele foi menor com a técnica proposta. A craniotomia minipterional foi a melhor alternativa em relação à craniotomia pterional clássica para tratar aneurismas rotos e não rotos da circulação anterior / Introduction: The pterional approach is one of the most commonly used craniotomy. However it has disadvantages, such as complete dissection of the temporalis muscle. This may lead to muscular atrophy and facial deformity. The minipterional craniotomy was described in 2007 and the anatomic exposure provided by the pterional and minipterional approaches were similar in the total area of exposure and angular view. Objectives: This prospective randomized study was designed to compare the clinical, functional and aesthetic results of two surgical techniques for microsurgical clipping of anterior circulation aneurysms. Methods: Overall, 58 eligible patients admitted with ruptured and unruptured anterior circulation aneurysms were enrolled in the study. In group A, 28 patients were operated with the minipterional technique. In Group B 30 patients were operated according to the classical pterional craniotomy. Patients with intracranial haematomas, ophthalmic aneurysms and giant aneurysms were excluded. The aesthetic results were analyzed with 2 methods. In the first, the patients were showed to a rule, with a scale from 0 to 100, in which 0 mean the best result and 100 the worst result. Photos were taken and showed to two independent observers, the results were classified as excellent, good, regular or poor, according to a pre-determined scale. The degree of atrophy was measured with three methods. In the first one, the authors observed the percentage of thick reduction in the temporal muscle, subcutaneous tissue and skin. In the second method the percentage of thick reduction of the isolated temporal muscle was observed and the third the volumetric analysis of the temporal muscle, subcutaneous tissue and skin was calculated from the superior edge of zygomatic arch to superior temporal line using the OsiriX software (OsiriX - Pixmeo Sarl Geneva/Suíça). The functional results were compared using the Modified Rankin Score. Others variables such frontal facial palsy, post-operative hemorrhage, cerebrospinal fistulas, hydrocephalus and mortality were also analyzed. Results: In both groups the demographic and pre-operative characteristics were similar. The satisfaction with aesthetic results were observed in 79% (19) in group A and 52% (13) in group B (p=0,07). The mean value observed in the rule was 27 in group A and 45,8 in group B (p=0,03). When patients classified as Rankin Modified Score of 0 or 1 only were included the mean value observed in the rule was 25,2 in group A and 39,4 in group B (p=0,11). Two independent observers analyzed the patients photos and the kappa coefficient correlation for the aesthetic results was 0,73. According to them excellent and good results were observed in 87% (21) in minipterional group and 48% (12) in the pterional group. The degree of atrophy of temporal muscle, subcutaneous tissue and skin (method 1) was14,9% in group A and 24,3% in group B (p=0,01). The measurement of temporal muscle (method 2) revealed that the degree of atrophy was 12,7% in group A and 22% in group B (p=0,005). The volumetric reduction of the structures (method 3) was 14,8% in group A and 24,5% in group B (p=0,012). Rankin Modified Score was similar in both groups in the 6-month evaluation (p=0,99). Mortality occurred in 4 patients in group A and 5 patients in group B (p=1,0). Conclusion: These clinical results indicate that the minipterional is a safe procedure. We can estimate the better cosmetic results with less facial contour deformity since the percentage of thick and volumetric reduction in temporal muscle, subcutaneous tissue and skin were demonstrated. It can be an excellent and better alternative to the classical pterional approach

Análise retrospectiva das alterações da dinâmica facial após aplicações seriadas de toxina botulínica tipo A / Retrospective analysis of facial dynamic alterations after multiple botulinum toxin A applications

INTRODUÇÃO: A presença de rugas na face devido à hipercinese muscular é comum nas regiões frontal, glabelar e peri-orbitárias. São descritos diversos métodos para o tratamento das rugas de expressão, como a dermoabrasão, a ablação a laser, o laser não ablativo, preenchimentos, cirurgia e aplicações de toxina botulínica. O tratamento das rugas da face com toxina botulínica tipo A é método consagrado, porém são pouco definidos os efeitos a longo prazo. Este estudo retrospectivo teve como objetivo avaliar as rugas das regiões frontal e glabelar da face em pacientes submetidas a aplicações seriadas da toxina botulínica tipo A com finalidade estética, analisando os intervalos entre as aplicações e as doses utilizadas. MÉTODOS: Foram analisadas 24 pacientes do sexo feminino, submetidas entre 5 a 7 aplicações da toxina botulínica tipo A, no período de julho de 1997 a junho de 2006. O tempo médio de acompanhamento foi de 42,2 (± 4,0) meses e o intervalo médio entre as aplicações de 8 (± 0,51) meses. A idade média observada de início da aplicação foi de 48,0 (± 3,0) anos, e da última aplicação foi de 51,6 (± 3,0) anos. Foram métodos de avaliação a análise da documentação fotográfica de antes da primeira e antes da última aplicação, sendo as rugas frontais e glabelares avaliadas segundo escala padronizada, e dados colhidos dos prontuários. RESULTADOS: A longo prazo, verificou-se: 1) amenização das rugas estáticas da região frontal em 62,5% das pacientes e em 37,5% tais rugas se mantiveram inalteradas; 2) amenização das rugas estáticas da região glabelar em 47,2% das pacientes, em 50% tais rugas se mantiveram inalteradas e em 2,8% verificou-se pouca acentuação; 3) amenização das rugas dinâmicas da região frontal em 84,7% das pacientes e em 15,3% tais rugas se mantiveram inalteradas; 4) amenização das rugas dinâmicas da região glabelar (corrugadores) em 63,9% das pacientes, em 26,4% tais rugas se mantiveram inalteradas e em 9,7% verificou-se pouca acentuação. 5) amenização das rugas dinâmicas da região glabelar (prócero) em 57% das pacientes, em 32% tais rugas se mantiveram inalteradas e em 11% verificou-se pouca acentuação. A média da dose total de toxina botulínica utilizada por sessão no presente estudo foi de 43,83 ± 1,25 U, e a dose total cumulativa foi de 272,08 ± 20,42 U. CONCLUSÕES: No longo prazo, existe maior porcentagem de pacientes com amenização ou aspecto inalterado das rugas estáticas e dinâmicas das regiões frontal e glabelar da face em relação a pacientes com acentuação das mesmas. Não houve variação estatisticamente significativa das médias dos intervalos entre as aplicações. A dose total de toxina botulínica aumentou de forma estatisticamente significativa até a terceira aplicação, mantendo-se sem variação significativa a partir de então. / INTRODUCTION: The presence of wrinkles on the face due to the over activity of muscles is rather common on the frontal, glabellar, and periocular regions. A number of methods for the treatment of face lines, such as skin abrasion, laser resurfacing, fillers, surgery and botulinum toxin A have been described. The treatment of facial rhytids with botulinum toxin A is widely used. However, its long term effects are not well defined. This retrospective study assesses the rhytids of both the frontal and glabellar regions of the face on patients submitted to multiple botulinum toxin A applications for aesthetic use, analyzing intervals between applications and dosages. METHODS: 24 female patients who had undergone 5 to 7 botulinum toxin A applications from July, 1997 to June, 2006 were studied. The mean time of follow up was 42.2 (± 4.0) months and the mean interval between applications was 8 (± 0.51) months. The mean age of the patients at the beginning of application was 48.0 (± 3.0) years old, and 51.6 (± 3.0) years old at the last application. The evaluation was carried through the analysis of photographic documentation from before the first and last applications, and both frontal and glabellar rhytids were evaluated according to standardized scale, as well as data collected from patients charts. RESULTS: the long term analysis demonstrated that: 1) improvement of the static rhytids of the frontal region in 62,5% of patients and such rhytids remained unaltered in 37.5% of the patients; 2) improvement of the static rhytids of the glabellar region in 47.2% of the patients, they remained unaltered in 50% of the patients and there was a slight worsening in 2.8% of the patients; 3) improvement of the dynamic rhytids of the frontal region in 84.7% of patients, and they remained unaltered in 15.3% of the patients; 4) improvement of the dynamic rhytids of the glabellar region (corrugator) in 63.9% of the patients, they remained unaltered in 26.4% of the patients and there was a slight worsening in 9.7% of the patients; 5) improvement of the dynamic rhytids of the glabellar region (procerus) in 57% of the patients, they remained unaltered in 32% and there was a slight worsening in 11% of the patients. The mean total dosage of botulinum toxin A used per session in this study was 43.83 ± 1.25 U, and the total cumulative dosage was 272.08 ± 20.42 U. CONCLUSIONS: In the long run, a higher percentage of patients showed improvement or unaltered aspect of their static and dynamic rhytids of the frontal and glabellar regions of the face, when compared to the patients that showed worsening. There was no variation of the mean intervals between the applications. The total dosage of botulinum toxin was significantly increased up to the third application, and kept stable in the following sessions.

"Estudo do tamanho dos dentes naturais superiores e dentes artificiais de diferentes marcas comerciais" / Study of the size of the upper natural teeth and of the artificial teeth of different commercial marks

Neste estudo analisou-se o tamanho de dentes naturais superiores de 119 indivíduos com idade entre 20 e 30 anos. Mediu-se a largura e a altura de todos os dentes superiores em modelos de gesso, com auxílio de um paquímetro digital. Aferiu-se também o tamanho de dentes artificiais de três marcas comerciais. Essa análise permitiu a identificação do que freqüentemente ocorre na dentição natural. Concluiu-se que: aproximadamente 48% dos incisivos centrais superiores naturais possuem a largura maior que 9,0mm. Em contrapartida, somente 14,28% dos incisivos centrais superiores artificiais analisados possuíam essa dimensão. Aproximadamente 64% dos indivíduos da pesquisa possuíam o dente 14 com largura maior que 7,0mm. Nas cartelas de dentes artificiais aferidas, encontrou-se apenas um modelo com dimensão mesiodistal superior a 7,0mm. Cerca de 74% dos primeiros molares superiores naturais possuíam altura menor que 7,0mm. Somente um modelo artificial possui altura menor que 7,0mm. A adequação das dimensões dos dentes artificiais facilitaria a montagem dos dentes e traria a estética almejada ao portador de reabilitações protéticas. / In this study, the size of natural maxillary teeth of 119 individuals between 20 and 30 years old were analyzed. The width and the height of all of the upper teeth on the casts were measured with a digital caliper. The size of the artificial teeth of three commercial marks was also checked. That analysis allowed the identification of what frequently occurs in the natural dentition. The conclusion was that: approximately 48% of the width of natural maxillary central incisor was greater than 9,0mm. However, only 14,28% of the artificial central upper incisor analyzed had that dimension. Approximately 64% of the population in the research had tooth 14 with width larger than 7,0mm. In the moulds of the teeth available, was found only a model with mesiodistal dimension over 7,0mm. Around 74% of the height of natural maxillary first molars was smaller than 7,0mm. Only an artificial model had height smaller than 7,0mm. The adaptation of the dimensions by artificial teeth would facilitate the assembly of the teeth and would bring the esthetics desired for to the user of the prosthetic replacements.
