Contribuição da ultra-sonografia para o diagnóstico das alterações histopatológicas presentes na hepatite C crônica, com ênfase na esteatose hepática / Ultrasonographic contribution for the diagnosis of the histopathological alterations in chronic hepatitis C, with emphasis in hepatic steatosis

INTRODUÇÃO: A utilização da ultra-sonografia (USG) como método de diagnóstico por imagem na avaliação das afecções abdominais, em particular para o acompanhamento de pacientes portadores de hepatite C crônica, vem sendo rotineiramente empregada. Neste trabalho, avaliamos a contribuição da USG na avaliação das alterações histopatológicas encontradas neste grupo de doentes, com ênfase para a esteatose hepática (EH), afecção bastante freqüente na hepatite causada pelo vírus C. MÉTODO: Comparamos os achados ultra-sonográficos de 192 pacientes consecutivos, portadores de hepatite crônica pelo vírus C, submetidos à biópsia hepática, com os achados histopatológicos dos fragmentos hepáticos obtidos. Todos os pacientes foram biopsiados sob orientação USG, sendo a ultra-sonografia assim como a biópsia hepática realizadas cada qual por um médico especialista e sempre o mesmo. Todos os exames ultra-sonográficos obedeceram a um mesmo protocolo, sendo analisados os seguintes parâmetros ultra-sonográficos: 1) com relação às características ecográficas do parênquima: ecogenicidade, ecotextura e atenuação; 2) com relação à utilização da USG para o diagnóstico da EH: biometria da parede abdominal, dimensões e contornos hepáticos. Posteriormente os pacientes estudados foram agrupados em: A) grupo com alterações ultra-sonográficas e B) sem alterações ultra-sonográficas, e comparados com as alterações histopatológicas presentes. Foram realizados também cálculos de regressão logística com os parâmetros USG avaliados para a avaliação da acurácia deste método de imagem para o diagnóstico da EH. RESULTADOS: Entre as alterações histopatológicas presentes, a alteração arquitetural e a EH apresentaram diferença estatística significante entre os grupos A (com alterações USG) e B (sem alterações USG). Observou-se também diferença estatística significante entre: a) espessura da parede abdominal e as dimensões hepáticas com relação à presença de EH, b) contornos hepáticos irregulares e a presença de EH. E dentre os componentes ultra-sonográficos avaliados, a atenuação foi o que apresentou melhor correlação com a EH. A utilização das variáveis idade, sexo, atenuação, espessura da parede abdominal e dimensões hepáticas, foram as que apresentaram melhores resultados nos cálculos de regressão logística, com sensibilidade de 60,5% e especificidade de 83,9% em diagnosticar EH. CONCLUSÕES: Neste trabalho, o estudo ultra-sonográfico do fígado de pacientes com hepatite C crônica apresentou correlação com as alterações arquiteturais e com a EH encontradas na histopatologia. A utilização da USG para o diagnóstico da EH apresentou relação estatística com a espessura da parede abdominal, dimensões e contornos hepáticos, e a atenuação foi o melhor componente ultra-sonográfico para o diagnóstico da EH. / INTRODUCTION: Ultrasonography is consistently utilized as the method of diagnostic imaging while evaluating abdominal infections particularly in patients with chronic hepatitis C. We studied the contribution of the ultrasonography in the evaluation of histopathologic alterations in this group of patients with emphasis in hepatic steatosis (HS), sufficiently frequent in hepatitis caused by the C virus. METHODS: We compared the findings from the ultrasounds of 192 carrying patients of chronic hepatitis C virus, who had undergone hepatic biopsy, with the histopathogical findings of the hepatic fragments obtained. All patients who have undergone liver biopsy guided by ultrasonography were always evaluated by the same medical specialist for both sonogram or hepatic biopsy. All ultrasound examinations followed the same protocol, analyzing the following parameters: 1) regarding to the echographic characteristics of parenchyma: echogenicity, echotexture and attenuation; 2) regarding to the use of the sonography for the diagnosis of the HS: biometry of the abdominal wall, hepatic dimensions and contours. Post results, patients had been grouped in: A) altered ultrasound group and B) ultrasound group without ultrasound alterations, both compared with present histopathological alterations. Calculations of logistic regression with ultrasonography parameters had also been performed to determine the accuracy of this method for the HS diagnosis. RESULTS: Of the histopathological alterations present, the architectural alteration and the HS had presented significant statistical difference between the groups (altered ultrasound group) and the B (without ultrasound alterations). We also noted significant statistical difference between: a) thickness of the abdominal wall and the hepatic dimensions with regard to presence of HS, b) irregular hepatic contours and the presence of HS. Amongst the evaluated ultrasound components, attenuation presented better correlation with the HS. The variables age, sex, attenuation, thickness of the abdominal wall and hepatic dimensions of the right lobe, presented better results in the calculations of logistic regression, with 60,5% sensitivity and specificity of 83,9% in diagnosing HS. CONCLUSIONS: In this research, the hepatic ultrasonography of patients with chronic hepatitis C presented correlation with the architectural alterations and the HS found at the histopathology. The utilization of the ultrasonography for the diagnosis of the HS presented statistical relationship with the thickness of the abdominal wall, hepatic dimensions and contours, and the sonographic attenuation was the best component for the diagnosis of ES.

INTRODUÇÃO: A Doença Hepática Gordurosa (DHG) é considerada um dos grandes problemas de saúde pública do novo milênio, impulsionada sobretudo pela epidemia de obesidade. Está relacionada com alta morbidade, piora da qualidade de vida e custos econômicos para a sociedade. Existem duas formas histopatológicas iniciais inseridas dentro da DHG: a esteatose e a esteato-hepatite que podem evoluir para cirrose e hepatocarcinoma. A maior parte da literatura sobre epidemiologia da doença hepática gordurosa utiliza desenhos retrospectivos e meios diagnósticos não invasivos, embora o padrão ouro seja a biópsia hepática. Medidas preventivas e de controle adequados deverão ser tomadas baseadas em informações epidemiológicas envolvendo população \"sadia\". A excelente correspondência das alterações histopatológicas encontradas no cadáver em relação ao vivo permite extrapolar dados da frequência de esteatose e esteato-hepatite encontradas em uma população jovem vítima de morte violenta para uma população \"normal\". Contudo, não há estudo prévio que avalie frequência e fatores preditivos de esteatose e esteatohepatite em fígados de cadáver. O objetivo deste estudo foi avaliar a esteatose e a esteato-hepatite em fígados de adultos vítimas de morte violenta. MÉTODOS: As amostras foram coletadas de 224 adultos submetidos a autópsia forense a partir de setembro de 2011 a abril de 2013. Dados antropométricos e do fígado foram registrados. O exame histopatológico foi realizado em seis amostras obtidas com base em diferentes lóbulos de cada fígado. Cada amostra foi tratada com quatro corantes: hematoxilina-eosina, Perls, pricosirius e tricrômio de Masson. Desfechos de interesse foram a presença de esteatose, esteato-hepatite, fibrose e siderose. RESULTADOS: A amostra apresentou uma média de idade de 40 anos. A esteatose foi diagnosticada em 48,2% dos casos e esteato-hepatite em 2,7%. Uma alta prevalência de fígado gorduroso foi verificada entre homens e indivíduos mais velhos, sendo a faixa etária mais atingida entre 41-60 anos. Os fatores significativamente associados com o aumento do risco de esteatose foram circunferência abdominal (p < 0,001), IMC(p < 0,001), peso do fígado (p=0,002), assim como a presença de siderose (p=0.018). CONCLUSÃO: A alta prevalência de esteatose hepática foi detectada em biópsias pós-morte de uma população jovem. Uma vez que esta doença pode ter consequências clínicas severas, esse dado é importante para avaliar medidas preventivas para a doença hepática gordurosa e suas graves consequências / BACKGROUND: Fatty liver disease (FLD) is considered one of the major public health problems of the new millennium, driven mainly by the obesity epidemic. It is related to high morbidity, worsening of quality of life and economic costs to society. There are two initial histopathological forms inserted into the FLD: steatosis and steatohepatitis which can progress to cirrhosis and hepatocellular carcinoma. Most of the literature on epidemiology of FLD using retrospective drawings and non invasive means, but the gold standard is a liver biopsy. Preventive measures and adequate control should be taken based on epidemiological information involving population \"healthy\". The excellent correlation of histopathologic changes found in the body in live relationship allows extrapolating data from steatosis to steatohepatitis and often found in a population young victim of violent death to a \"normal\" population. However, no previous study to assess the frequency and predictors of steatosis and steatohepatitis in cadaver livers. The aim of this study is to evaluate steatosis and steatohepatitis in livers of adult victims of violent death. METHODS: Specimens were collected from 224 adults undergoing forensic autopsy from September 2011 to April 2013. Anthropometric and liver data were recorded. Histopathological examination was performed on six biopsies obtained from different lobes of each liver. Each sample was treated with 4 stains: hematoxylin-eosin, Perls, pricosirius and trichrome Masson. Outcomes of interest were the presence of steatosis, steatohepatitis, fibrosis and siderosis. RESULTS: The sample had an average age of 40 years. The steatosis was detected in 48.2% of cases and 2.7% steatohepatitis. A high prevalence of fatty liver was observed among men and older individuals, the age group most affected between 41-60 years. The factors significantly associated with increased risk of steatosis were waist circumference (p < 0.001), BMI (p < 0.001), liver weight (p = 0.002) as well as the presence of siderosis ( p = 0.018). CONCLUSION: The high prevalence of hepatic steatosis was detected in postmortem biopsy of a young population. Since this disease can have severe clinical consequences, this data is important to evaluate preventive measures for fatty liver disease and its serious consequences

Análise metabolômica e histomorfométrica do fígado de ratas adultas em modelo experimental da síndrome dos ovários policísticos induzida por exposição neonatal a esteroides sexuais / Metabolomic and histomorphometric analysis of the liver of rat models of polycystic ovary syndrome induced by neonatal exposure to sex steroids.

A síndrome dos ovários policísticos (SOP) é um distúrbio endócrino complexo, associado a resistência insulínica, hiperinsulinemia, obesidade, acúmulo de gordura visceral e dislipidemia. Essas alterações podem levar a aumento de risco de doenças como \"diabetes mellitus\", doenças cardiovasculares, esteatose hepática ou até mesmo esteato-hepatite. Em ratas, o excesso de androgênios ou estrogênios no período neonatal induz a alterações metabólicas e reprodutivas similares às observadas na SOP em humanos. O objetivo deste estudo é analisar os efeitos da exposição neonatal à testosterona e ao estrogênio no fígado de ratas adultas. Para isso foram utilizadas 30 ratas, divididas em três grupos de 10 animais cada. Foi administrada por via subcutânea, entre 1 e 3 dias de vida, uma única dose dos seguintes compostos: 0,1 mL de óleo de oliva (veículo) - grupo Controle (n=10); propionato de testosterona (1,25 mg/0,1mL de veículo) - grupo Testosterona (n=10); benzoato de estradiol (0,5 mg/0,1mL de veículo) - grupo Estradiol (n=10), de acordo com o grupo a que pertenciam. Após cerca de 90 dias, os animais foram sacrificados, sendo retirados fragmentos do fígado que foram preparados para análise da metabolômica e da histomorfologia. Histologicamente, o fígado dos animais do grupo testosterona apresentaram maior infiltração gordurosa e infiltrado inflamatório; e do grupo estradiol apresentou hepatócitos binucleados, além do aumento do volume nuclear. Houve, no grupo testosterona modificações no perfil metabolômico ligadas a maior resistência à insulia e maior risco para diabete. O grupo exposto ao estrogênio apresentou alterações relacionadas ao metabolismo e síntese de lipídeos. Nossos resultados sugerem que os riscos metabólicos associados à SOP podem ter origem e mecanismos diferentes / Polycystic ovary syndrome (PCOS) is a complex endocrine disorder associated with insulin resistance, hyperinsulinemia, central obesity, accumulation of visceral fat and dyslipidemia. Those changes can lead to increased risk of diseases such as diabetes mellitus, cardiovascular disease, nonalcoholic fat liver disease or even nonalcoholic steatohepatitis. In rats, the excess of androgens or estrogens in the neonatal period leads to metabolic and reproductive alterations similar to those observed in SOP in humans. The objective of this study is to analyze the effects of exposure to neonatal testosterone and estrogen in the liver of adult rats. For this we used 30 female rats, sorted into three groups of 10 animals each. It was administered subcutaneously between 1 and 3 days of age a single dose of the following compounds: 0.1 mL olive oil (vehicle) - Control Group (n = 10); testosterone propionate (1.25 mg / 0.1 mL vehicle) - Testosterone (n = 10); estradiol benzoate (0.5 mg / 0.1 mL vehicle) - Estradiol group (n = 10). After 90 days, the animals were sacrificed, and the liver removed fragments were prepared for analysis of metabolomics and histomorphometry. Histologically, the testosterone group displayed greater fat deposition and higher degree of inflammatory infiltration. The estradiol group had binucleate hepatocytes and increased nuclear volume. Testosterone group showed changes in the metabolomic profile linked to increased insulin resistance and increased risk for diabetes mellitus. The group exposed to estrogen showed changes related to metabolism and synthesis of lipids. Our results indicate that the metabolic risks associated with PCOS may have origin and different mechanisms

Modelo experimental para indução de esteatose hepática e esteatohepatite: estudo em ratos / Experimental model for induction of steatosis and steatohepatitis - study in rats

Introdução: A Doença Hepática Gordurosa Não Alcoólica (DHGNA) tem sido considerada atualmente a forma mais comum de doença hepática no mundo ocidental, relacionada principalmente ao aumento da prevalência da obesidade.A DHGNA abrange um largo espectro de doença, desde casos de esteatose simples (EH) até esteato-hepatite não alcoólica (EHNA) e fibrose, podendo evoluir para cirrose e carcinoma hepatocelular (CHC). Embora se conheçam os fatores predisponentes para o desenvolvimento de EHNA, sua patogênese, assim como tratamento eficaz, permanecem pouco conhecidos.Os lípides, principalmente gorduras neutras, fosfolipídios e colesterol, são componentes fundamentais das células, assim como as proteínas e os hidratos de carbono. Embora aumentos marcantes de proteínas e hidratos de carbono não produzam quaisquer alterações macroscópicas no fígado, um acúmulo de gordura é prontamente reconhecido pela coloração amarelada e pelo aumento de volume do órgão. Várias dietas tem sido usadas em animais de laboratórios, principalmente camundongos e ratos no sentido de induzir esteatose, esteatohepatite e fibrose hepática, embora nenhuma delas consiga reproduzir na totalidade os componentes essenciais da doença humana. Não encontramos em nenhum trabalho da literatura a utilização da dieta deficiente em colina e hiperlipídica (DCH), por nós utilizada. Objetivo: O objetivo deste estudo foi avaliar o efeito de uma dieta deficiente em colina e hiperlipídica (DCH) para desenvolver esteatose hepática, esteatohepatite e fibrose hepática no rato. Métodos: A doença hepática foi induzida em ratos Sprague-Dawley machos pesando de 300 a 350 gramas, hospedados em gaiolas padrão e recebendo água à vontade. Foi usado dieta deficiente em colina e hiperlipídica (DCH). Os animais foram distribuídos por meio de tabela sequencial randomizada em dois grupos. No grupo 1 dez ratos receberam a dieta por 4 meses e no grupo 2 dez ratos receberam a dieta durante 8 meses. Após o tempo estipulado das dietas os animais foram anestesiados e sacrificados. Os fígados dos animais foram retirados, pesados e enviados para estudo histopatológico de acordo com os critérios estabelecidos por Kleiner. Os animais foram pesados no início do experimento e por ocasião do sacrifício. Resultados: o peso dos animais do grupo 1 (DCH - 4 meses) no início do experimento variou de 305 a 350 gramas com média de 329,9 gramas e no fim do experimento de 529 a 615 gramas com média de 579,2 gramas. O percentual de ganho de peso dos animais variou de 60 a 86,93% com média de 75,72%. O peso dos animais do grupo 2 (DCH - 8 meses) no início do experimento variou de 320 a 353 gramas com média de 339,4 gramas e no fim do experimento variou de 661,5 a 783 gramas com média de 705,6 gramas. O percentual do ganho de peso dos animais variou de 95,58 a 123,71% com média de 107,78%. O peso do fígado dos animais do grupo 1 no fim do experimento variou de 24,5 a 29,5 gramas. O percentual do peso do fígado em relação ao peso dos animais do grupo 1 variou de 4,16 a 5,54% com média de 4,75%. O peso do fígado dos animais do grupo 2 no fim do experimento variou de 31 gramas a 39 gramas com média de 33,8 gramas. O percentual do peso do fígado em relação ao peso dos animais do grupo 2 variou de 4,63% a 5,07%, com média de 4,78%. A análise histopatológica dos animais do grupo 1 demonstrou intensa balonização e esteatose microgoticular com inflamação lobular; o estadiamento de fibrose foi discreto nestes animais. O estudo histopatológico dos animais do grupo 2 revelou intensa esteatose microgoticular e balonização com inflamação lobular; alguns animais apresentaram esteatose macrogoticular. Os animais deste grupo 2 apresentaram fibrose com grande variação individual. Os animais de ambos os grupos experimentais desenvolveram esteatohepatite, pois apresentaram o índice de atividade da doença hepática gordurosa não alcoólica (NAS)>=5. A análise comparativa demonstrou não haver diferença estatística no valor do NAS entre os dois grupos experimentais. Em relação ao estadiamento da fibrose na esteatohepatite não alcoólica, os animais do grupo 2 apresentaram uma tendência de escores mais elevados do que os animais do grupo 1. No entanto não houve diferença estatística entre os grupos devido a grande variação individual (p=0,2755). Conclusões: A dieta DCH administrada durante 4 e 8 meses induziu nos ratos aumento considerável de peso corpóreo e do peso do fígado. Os animais do grupo 1 apresentaram intensa balonização, esteatose microgoticular e inflamação lobular com discreta fibrose hepática. Os animais do grupo 2 apresentaram intensa balonização, esteatose macro e microgoticular, inflamação lobular e maior grau de fibrose hepática, bem estabelecida com formação de colágeno e expansão fibrosa nos espaços vasculares. A dieta deficiente em colina e hiperlipídica (DCH) induziu no rato esteatose hepática e esteatohepatite com fibrose, servindo de modelo experimental para proporcionar melhor entendimento para procedimentos terapêuticos futuros desta importante patologia do fígado. / Introduction: Nonalcoholic Fatty Liver Disease (NAFLD) has currently been regarded as the most common form of liver disease in the western world, mainly related to the increased prevalence of obesity. NAFLD covers a broad spectrum of diseases, since cases of simple steatosis (HS) to steatohepatitis (EHNA) and fibrosis, and may evolve to cirrhosis and hepatocellular carcinoma (CHC). Although the predisposing factors for the development of EHNA are well established, there is little knowledge on its pathogenesis as well as the effective treatment options. Lipids, especially phospholipids, neutral fats and cholesterol, are fundamental cell components, as well as proteins and carbohydrates. Although striking increases of proteins and carbohydrates do not produce liver macroscopic changes, fat build up can be rapidly recognized by the yellow coloring and increased organ volume. Many diets have been used in laboratory animals, mainly rats although none of them can fully reproduce the fundamental components of the human disease. We could not find in the literature any study on the use of choline-deficient and hiperlipidic diet (CHD), used in the present study. Goal: The objective of this study was to evaluate the effect of a cholinedeficient and hiperlipidic diet (CHD) on the development of steatosis, steatohepatitis and hepatic fibrosis in rats. Methods: The liver disease was induced in male Sprague-Dawley rats, weighing from 300 to 350 grams, hosted in standard cages and receiving water ad libitum and fed with a choline-deficient and hiperlipidic diet (CHD). The experimental animals were distributed by means of a random sequential table in two groups. In Group 1 ten rats received the diet for four months and in Group 2 ten rats received the same diet for eight months. After these predetermined feeding time periods the animals were anesthetized and sacrificed. The animal livers were then removed, weighed and sent to histopathological study according to the criteria established by Kleiner. All the animals were weighed at the beginning of the experiment and by the time of the sacrifice. Results: The weight of Group 1 animals (4 months CHD) at the beginning of the experiment varied from 305 to 350 grams with an average of 329.9 grams and at the end of the experiment from 529 to 615 grams averaging 579.2 grams. The percentage of weight gain in animals of this group ranged from 60% to 86.93% with an average of 75.72%. The weight of Group 2 animals (8 months CHD) at the start of the experiment varied from 320 to 353 grams with an average of 339.4 grams and at the end of the experiment from the 661.5 to 783 g with an average of 705.6 grams. The percentage of weight gain in animals of this group varied from 95.58% to 123.71%, averaging 107.78%. The liver weight of Group 1 animals at the end of the experiment varied from 24.5 to 29.5 grams. The percentage of liver weight in relation to the animals\' weight, in Group 1, ranged from 4.16% to 5.54%, with an average of 4.75%. The liver weight of Group 2 animals at the end of the experiment varied from 31gramas to 39 grams, with an average of 33.8 gram. The percentage of liver weight in relation to the animals\' weight, in Group 2, ranged from 4.63% to 5.07%, with an average of 4.78%. Liver histopathological analysis in Group 1 animals showed marked ballooning degeneration and microgoticular steatosis with lobular inflammation; fibrosis staging was discreet in these animals. Liver histopathological study in animals of Group 2 showed an intense microgoticular steatosis and ballooning with lobular inflammation; some animals of this group presented macrogoticular steatosis. The amount of hepatic fibrosis in animals of this group was extremely variable. Animals of both experimental groups developed steatohepatitis, as they presented the activity index of nonalcoholic fatty liver disease (NAS) >= 5. Statistical analysis did not show a significant difference between the NAS values of the two experimental groups. Fibrosis staging analysis in non-alcoholic steatohepatitis showed a trend toward higher scores in animals of Group 2 as compared to Group 1 animals. However there was no statistical difference between the groups due to a large individual variation (p = 0.2755). Conclusions: CHD diet administered for four and eight months induced considerable increases in the rats\' body weight and liver weight. Animals in Group 1 showed intense liver ballooning, steatosis and lobular inflammation with discrete microgoticular fibrosis. Animals in Group 2 presented intense ballooning, steatosis macro and microgoticular, lobular inflammation and a higher degree of well-established hepatic fibrosis, with collagen formation with fibrotic expansion in vascular spaces. Choline-deficient and hiperlipidic diet (CHD) in the rat induced hepatic steatosis and steatohepatitis with fibrosis, representing an experimental model, which can provide a better knowledge for future therapeutic procedures of this important liver pathology.

A hepatite crônica pelo vírus C é a principal causa de doença hepática crônica progressiva e complicações relacionadas, como a cirrose hepática e o carcinoma hepatocelular. O estadiamento de fibrose e a graduação da atividade necroinflamatória são excelentes preditores de progressão da doença na hepatite crônica pelo vírus C (HCVC). A epidemia global de obesidade e diabetes mellitus é responsável pela crescente incidência da doença hepática gordurosa não alcoólica, caracterizada por achados histológicos que variam da esteatose pura até a esteato-hepatite não alcoólica (EHNA), com potencial risco de evolução para a cirrose hepática e suas complicações. Na HCVC, fatores virais e do hospedeiro podem contribuir para a associação com a doença hepática gordurosa não alcoólica. Não há consenso sobre a prevalência de esteatose e esteato-hepatite não alcoólica em pacientes com HCVC, com ampla variabilidade na dependência do genótipo viral, fatores metabólicos da população em estudo e variáveis histológicas utilizadas para a definição. Objetivos: a) Definir a prevalência de esteatose hepática e da esteato-hepatite não alcoólica em pacientes com hepatite crônica pelo vírus C; b) Avaliar a relação entre variáveis clínico-laboratoriais e diferentes graduações de parâmetros histopatológicos; c) Avaliar a influência da EHNA na progressão da fibrose; d) Determinar os fatores virais e do hospedeiro associados a diferentes grupos histológicos, assim definidos: hepatite crônica pelo vírus C (HCVC), hepatite crônica com esteatose (>5%), hepatite crônica associada à esteato-hepatite não alcoólica (esteatose + fibrose perissinusoidal); e) Determinar fatores virais e do hospedeiro associados aos grupos HCVC, HCVC com esteatose e HCVC com EHNA, considerando estadiamento maior de fibrose à biópsia inicial. Métodos: Em 81 pacientes com HCVC seguidos no ambulatório de hepatites crônicas, parâmetros clínico-laboratoriais foram relacionados ao estadiamento da fibrose em biópsias pareadas. Dentre os dados clínicos, a síndrome metabólica foi definida pelo critério do ATP III. À biópsia inicial, achados histopatológicos de HCVC e doença hepática gordurosa não alcoólica foram graduados como ausente/leve ou moderado/intenso. Posteriormente, excluindo os pacientes com o genótipo do VHC tipo 2, dados clínicos e laboratoriais e estágios de fibrose 0-1 ou 2-4 foram analisados em pacientes com hepatite crônica pelo vírus C divididos em três grupos: HCVC sem esteatose, HCVC com esteatose e HCVC com esteato-hepatite não alcoólica (EHNA), definida pela associação com balonização e fibrose perissinusoidal. Resultados: A idade avançada esteve associada a estágio maior de fibrose e atividade inflamatória portal e periportal; o escore de APRI AST / Platelets Relation Índex foi associado a estágios maiores de fibrose e maior atividade necroinflamatória. Na análise multivariada, o perfil lipídico da síndrome metabólica foi associado à fibrose perivenular enquanto a glicemia elevada esteve associada ao hialino de Mallory-Denk. Esteatose isolada esteve presente em 35 (43.2%) e associada a EHNA em 21 (25.9%) dos pacientes. O genótipo 3 do vírus da hepatite C foi mais prevalente nos pacientes com HCVC com esteatose ou EHNA, porém evolução para estágios maiores de fibrose associou-se ao genótipo viral 1 (p= 0.000). A presença de HCVC + EHNA esteve associada com fibrose 2 em pacientes com menos de 45 anos, independentemente do sexo. Sobrepeso ou obesidade também estiveram associados à fibrose intensa (p< 0.05). Em biópsias pareadas, o perfil lipídico de síndrome metabólica foi o único parâmetro associado com progressão da fibrose (p= 0.012). Conclusão: a) A associação de esteatose e esteato-hepatite não alcoólica é elevada em pacientes com HCVC, sendo mais frequente no genótipo viral 3; b) Fatores metabólicos, como o sobrepeso ou obesidade e a presença do perfil lipídico da síndrome metabólica, estiveram associados à esteatose e EHNA em pacientes com HCVC; c) A associação de NASH com hepatite crônica pelo vírus C pode modificar a evolução da doença e demandar atenção cuidadosa desses pacientes. / Chronic hepatitis C is the leading cause of progressive liver damage and related complications, such as cirrhosis and primary hepatocellular carcinoma. Fibrosis stage and necro-inflammatory activity grade are good predictors of disease progression in chronic hepatitis C (CHC).The global epidemic of obesity and diabetes are associated with the increasing incidence of nonalcoholic fatty liver disease (NAFLD), ranging from the pure steatosis to nonalcoholic steatohepatitis (NASH), the latter with the potential to progress to cirrhosis and its complications. In CHC patients, viral and host factors may contribute to the association with NAFLD. There is no consensus about the prevalence of steatosis and NASH in CHC patients, with variability depending on the genotype, host metabolic conditions and histological variables. Objectives: a) To define the prevalence of steatosis and nonalcoholic steatohepatitis in CHC patients; b) To assess the relationship between various clinical and laboratory data and the grading of histological parameters; c) To evaluate the influence of NASH in the progression of fibrosis; d) To determine viral and host factors associated with different histopathological groups classified as: CHC alone, CHC with steatosis ( > 5%) and CHC with NASH (steatosis + perisinusoidal fibrosis); e) To determine viral and host factors associated with higher stages of fibrosis in the three groups. Methods: We investigated clinical and laboratory data in 81 CHC patients under scrutiny in a public tertiary hospital and related them to fibrosis stage at paired biopsies. Among clinical data, metabolic syndrome was defined according to ATPIII. At initial biopsy, histopathological features of chronic hepatitis C and nonalcoholic steatohepatitis were graded as absent/light or moderate/severe. Later on, except for genotype 2 patients, we analysed clinical, biochemical data and stage of fibrosis 0-1 vs. 2-4 among CHC patients divided into three groups: without steatosis, with steatosis and with nonalcoholic steatohepatitis (NASH). Results: In multivariate analysis, lipid profile of metabolic syndrome was associated with perivenular fibrosis whereas elevated glycemia levels were associated with Mallorys hyaline. Steatosis was present in 35 (43.2%) and NASH in 21 (25.9%) patients. HCV genotype 3 was more prevalent among CHC patients associated with steatosis or NASH but higher fibrosis stages were associated with HCV genotype 1 (p= 0.000). The presence of CHC + NASH was associated with fibrosis 2 in patients under 45 years, irrespective of sex (p < 0.05). Overweight and obesity (p < 0.05) were also related to severe fibrosis (p < 0.05). In paired biopsies, lipid profile of metabolic syndrome was the only parameter associated with progression of fibrosis (p= 0.012). Conclusion: a) Steatosis and nonalcoholic steatohepatitis are frequent histopathological features in our CHC patients, especially in HCV genotype 3; b) Metabolic factors, like overweight, obesity and lipid profile of metabolic syndrome were associated with steatosis and NASH in CHC patients; c) The association of NASH in chronic hepatitis C may modify the outcome of CHC and demand close examination.

Introdução: A progressão da hepatite C crônica para fibrose hepática envolve diversos fatores, entre os fatores metabólicos destaca-se a esteatose hepática. A esteatose hepática na infecção pelo vírus da hepatite C (HCV) é complexa e envolve fatores virais e do hospedeiro. Alguns polimorfismos de nucleotídeos únicos (SNPs) parecem influenciar as concentrações dos lipídeos plasmáticos e o desenvolvimento da esteatose hepática em pacientes com HCV. Objetivo: Detectar e avaliar a influência dos SNPs -164T/C, -400A/T e H297Q no gene da proteína de transferência de triglicerídeo microssomal (MTTP) na esteatose hepática e nos níveis de lipídeos plasmáticos em pacientes com hepatite C crônica. Métodos: Os SNPs -164T/C, -400A/T e H297Q foram genotipados em 236 pacientes com hepatite C crônica do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. A genotipagem foi realizada pela técnica de reação em cadeia da polimerase seguida de análise de polimorfismo de tamanho de fragmentos de restrição (PCR-RFLP), na qual após a amplificação de um fragmento específico do DNA foi realizada a digestão por enzimas de restrição e as bandas referentes a cada genótipo foram visualizadas em gel de agarose. Foi avaliada a associação de características com os genótipos de cada SNP em diferentes modelos genéticos (codominante, dominante e recessivo). Foram realizadas análises estatísticas bivariadas e multivariadas com o auxílio do programa IBM-SPSS. Resultados: No presente estudo, 56,4% dos participantes eram mulheres e a média de idade foi 55,5 anos (29-83 anos). O genótipo mais prevalente do SNP -164T/C foi o homozigoto selvagem (TT) e dos SNPs -400A/T e H297Q foram os heterozigotos (AT e HQ, respectivamente). As frequências dos alelos mutados dos SNPs -164T/C, -400A/T e H297Q foram de 0,30, 0,41 e 0,50, respectivamente. As distribuições genotípicas dos três SNPs no gene MTTP estão em equilíbrio de Hardy-Weinberg. O alelo mutado do SNP -164T/C foi associado com idade avançada, presença de hipertensão arterial, níveis elevados de ferro, ferritina e insulina (p < 0,05). Sendo que o nível elevado de insulina foi associado com o SNP -164T/C nos três modelos genéticos estudados. O alelo mutado do SNP -400A/T foi associado com diabetes (p=0,005) e o alelo selvagem com siderose (p=0,030) nos modelos genéticos co-dominante e dominante. O alelo mutado do SNP H297Q foi associado com o genótipo 3 do HCV (modelo co-dominante), presença de hipertensão arterial (modelos co-dominante e dominante) e nível elevado de insulina (modelo dominante) (p < 0,05). Nenhum dos SNPs foi associado com alterações nos níveis de lipídeos e com os diferentes graus de esteatose hepática. A presença do alelo mutado do SNP -164T/C em conjunto com a infecção pelo genótipo 3 do HCV, foi associada na análise multivariada com a esteatose (p=0,004). Ainda em relação ao SNP-164T/C, o nível elevado de GGT em conjunto com a presença do alelo mutado foi associado com a esteatose (p=0,006). A presença do alelo mutado do SNP -400A/T em conjunto com a infecção pelo genótipo 3 do HCV, também foi associada com a esteatose (p=0,032). A análise do SNP H297Q em conjunto com as características avaliadas não apresentou associação com a esteatose (p > 0,05). Conclusões: Foi detectada a presença de todos os genótipos dos SNPs -164T/C, -400A/T e H297Q no gene MTTP e não foram associados com alterações nos níveis de lipídeos. A esteatose hepática em pacientes com hepatite C crônica foi associada com os SNPs -164T/C e -400A/T em conjunto com a infecção pelo genótipo 3 do HCV, e com o SNP -164T/C em conjunto com elevação no nível de GGT. Portanto, em pacientes com hepatite C crônica, as características do hospedeiro e do vírus podem ser investigadas em conjunto / Introduction: The chronic hepatitis C progression to liver fibrosis involves several factors; among the metabolic factors, hepatic steatosis is observed. Hepatic steatosis in hepatitis C virus (HCV) infection is complex, involves both viral and host factors. Single nucleotide polymorphisms (SNPs) seem to influence the plasma lipid concentrations and the development of hepatic steatosis in HCV patients. Objective: Detect and evaluate the influence of the -164T/C, -400A/T, and H297Q SNPs in the microsomal triglyceride transfer protein (MTTP) gene on hepatic steatosis and plasma lipid levels in patients with chronic hepatitis C. Methods: The -164T/C, -400A/T and H297Q SNPs were genotyped in 236 patients with chronic hepatitis C of the Clinical Hospital of the School of Medicine, University of Sao Paulo. Genotyping was performed by the polymerase chain reaction assay followed by restriction fragment length polymorphism (PCR-RFLP) analysis, in which after the amplification of a specific DNA fragment, digestion was performed by restriction enzymes and each genotype bands were visualized on agarose gel. Several characteristics were evaluated with the genotypes of each SNP in different genetic models (codominant, dominant and recessive). Bivariate and multivariate statistical analyzes were performed using the software IBM-SPSS. Results: In the present study, 56.4% of the participants were women and the mean age was 55.5 years (29-83 years). The most prevalent genotype of the -164T/C SNP was the wild-type homozygous (TT) and the -400A/T and H297Q SNPs were the heterozygous (AT and HQ, respectively). The frequencies of mutated alleles in the -164T/C, -400A/T and H297Q SNPs were 0.30, 0.41 and 0.50, respectively. The genotype distributions of three SNPs in the MTTP gene were in Hardy-Weinberg equilibrium. The mutated allele of the -164T/C SNP was associated with advanced age, presence of arterial hypertension, elevated levels of iron, ferritin and insulin (p < 0.05). The elevated insulin level was associated with the SNP -164T/C in the three genetic models studied. The mutated allele of the -400A/T SNP was associated with diabetes (p=0.005) and the wild-type allele with siderosis (p=0.030) in co-dominant and dominant genetic models. The mutated allele of H297Q SNP was associated with HCV genotype 3 (co-dominant model), presence of arterial hypertension (codominant and dominant models) and elevated insulin level (dominant model) (p < 0.05). None of the SNPs were associated with alterations in lipid levels and with hepatic steatosis distinct degrees. The presence of mutated allele of the -164T/C SNP combined with HCV genotype 3 infection was associated in the multivariate analysis with steatosis (p=0.004). Also with regard to -164T/C SNP, elevated levels of GGT combined with the presence of the mutated allele were associated with steatosis (p=0.006). The presence of the mutated allele of the -400A/T SNP combined with HCV genotype 3 infection was also associated with steatosis (p=0.032). The analysis of the H297Q SNP combined with the characteristics evaluated was not associated with steatosis (p > 0.05). Conclusions: The presence of all genotypes of the -164T/C, -400A/T and H297Q SNPs in the MTTP gene was detected and was not associated with alterations in lipid levels. Hepatic steatosis in patients with chronic hepatitis C was associated with the -164T/C and -400A/T SNPs combined with HCV genotype 3 infection, and with the -164T/C SNP combined with elevated GGT level. Introdução: O vírus da hepatite C (HCV) possui distribuição mundial, ocorrendo em indivíduos de todas as idades e etnias. Cerca de 80% dos casos de hepatite C aguda progridem para infecção crônica e 10-20% desses casos desenvolverão complicações de doença hepática crônica, como cirrose hepática, dentro de duas a três décadas e 1-5% vão desenvolver câncer de fígado. Estudos indicam que o polimorfismo rs738409 do gene PNPLA3 pode estar relacionado ao desenvolvimento da esteatose hepática e à progressão da fibrose. O presente estudo avaliou a associação entre o polimorfismo rs738409 presente no gene da PNPLA3 e presença de esteatose e o grau de fibrose em pacientes com hepatite C crônica. Métodos: Foram selecionados 314 pacientes com hepatite C crônica atendidos no Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. Foi realizada a genotipagem do polimorfismo rs738409 (I149M) do gene PNPLA3, utilizando a técnica de reação em cadeia da polimerase - análise de tamanho de fragmentos de restrição (PCR-RFLP). Resultados: Dos pacientes incluídos no estudo, 133 (45,9%) apresentaram o genótipo CC, 63 (21,7%) o genótipo CG e 94 (32,4%) o genótipo GG. Idade, infecção pelo vírus da hepatite C genótipo 3, moderada ou alta atividade inflamatória (A2-A3), IMC elevado, nível elevado de GGT e presença do alelo G (GG ou GC) mantiveram-se associados à esteatose hepática na análise multivariada. Permaneceram associados à fibrose avançada: idade, sexo masculino, níveis elevados de AST e GGT, moderada ou alta atividade inflamatória (A2-A3) e presença de esteatose hepática ( > 5%) na análise multivariada. Conclusões: Observou-se associação entre o polimorfismo rs738409 do gene PNPLA3 e esteatose hepática nos pacientes com hepatite C crônica, mas essa associação não foi encontrada com fibrose avançada / Introduction: The hepatitis C virus (HCV) has a worldwide distribution, occurring in individuals of all ages and ethnicities. About 80% of acute hepatitis C cases progress to chronic infection and 10-20% of these cases will develop the complications of chronic liver disease such as liver cirrhosis, within 2 to 3 decades, and 1-5% will develop liver cancer. Studies indicate that the rs738409 polymorphism of the PNPLA3 gene may be related to the development of hepatic steatosis and fibrosis progression. The present study evaluated the association between the rs738409 polymorphism present in the PNPLA3 gene and the presence of steatosis and the degree of fibrosis in patients with chronic hepatitis C. Results: Of the patients included in the study, 133 (45.9%) had the CC genotype, 63 (21.7%) the CG genotype and 94 (32.4%) the GG genotype. The multivariate analysis showed an association between hepatic steatosis and age, infection with hepatitis C genotype 3, moderate or high inflammatory activity (A2-A3), high BMI, high level of GGT and presence of the G allele (GG or GC). Furthermore, multivariate analysis also showed an association between advanced fibrosis and male gender, high levels of AST and GGT, moderate or high inflammatory activity (A2-A3) and the presence of hepatic steatosis ( > 5%). Conclusions: It was observed an association between PNPLA3 rs738409 polymorphism and hepatic steatosis in patients with chronic hepatitis C but this association was not found with advanced fibrosis

Devido a controvérsias sobre a freqüência, diagnóstico e possibilidade de progressão para doença crônica da degeneração gordurosa do fígado em portadores de obesidade mórbida, planejou-se uma investigação retrospectiva em pacientes candidatos à cirurgia bariátrica. Foram analisados 60 indivíduos com indicação de gastroplastia com derivação intestinal em Y de Roux. Os pacientes foram submetidos a questionário clínico, provas bioquímicas e posteriormente, à biópsia hepática intraoperatória. A freqüência de EHNA e esteatose hepática foi elevada, e detectou-se 5% de cirrose. Na análise univariada houve correlação de diversos testes bioquímicos com as enzimas hepáticas. Na análise multivariada a albumina foi a variável determinante; apesar de seu debilitado valor preditivo. Nestas condições, apenas a avaliação histológica configura-se como critério definitivo para diagnóstico e acompanhamento desta população / Because of controversies about frequency, diagnosis and possibility of progression toward chronic disease of fatty degeneration of the liver in morbidly obese patients, a retrospective study was done in patients who underwent bariatric surgery. Sixty individuals with indication for gastroplasty with Roux-en-Y jejunal bypass were enrolled. They were submitted to clinical questionnaire, biochemistry tests and subsequently, intraoperative liver biopsy. Frequency of NASH and steatosis was high, and 5.0% suffered from cirrhosis. In univariate analysis there was correlation between various biochemical measurements and liver enzymes. In multivariate analysis only serum albumin was a determinant variable, with low predictive value. The histological grading system remains the definitive criterion for diagnosis and follow-up of this population

A doença hepática gordurosa não alcoólica (DHGNA) compreende um espectro de alterações que inclui a esteatose simples e a esteatohepatite não alcoólica e é considerada o componente hepático da Síndrome Metabólica. Estudos já demonstraram que os produtos finais de glicação avançada (AGEs) estimulam a produção de proteínas de matriz extracelular, a geração de espécies reativas de oxigênio e de citocinas pró-inflamatórias por células hepáticas e que poderiam participar da patogênese da DHGNA. Em vista da escassez de informações sobre os efeitos metabólicos dos AGEs no fígado, apesar do seu potencial para causar dano hepático, nós avaliamos os efeitos da administração crônica de albumina modificada por glicação avançada sobre: (1) variáveis associadas com a homeostase glicêmica e a sensibilidade à insulina; (2) a expressão hepática de genes envolvidos na via dos AGEs, no metabolismo da glicose e de lípides, no estresse oxidativo e na inflamação; (3) a ativação de proteínas envolvidas na via de sinalização insulínica; (4) a morfologia e o conteúdo de glicogênio hepático. O estudo foi realizado em ratos Wistar que receberam, por via intraperitoneal, 20 mg/kg/peso corporal de albumina de rato modificada (AlbAGE) ou não (AlbCTRL) por glicação avançada durante 12 semanas. A administração crônica de AlbAGE não alterou as concentrações de alanina aminotransferase (ALT) e aspartato aminotransferase (AST) ou outras variáveis analisadas, mas induziu a resistência insulínica, conforme detectado pelo teste de tolerância à insulina (ITT). Os demais resultados, no entanto, sugerem um aumento da sensibilidade insulínica no território hepático, conforme evidenciado pela ativação da serine/threoninespecific protein kinase (AKT), inativação da glicogênio sintase cinase (GSK3), aumento do conteúdo do glicogênio hepático e a diminuição da expressão dos genes da via da gliconeogênese Pck1 e G6pc. Outros resultados observados foram uma redução no conteúdo de gordura hepatica e na expressão dos genes lipogênicos Acaca e Fasn e dos genes pró-inflamatórios Nfkb1, Tnf e Il6. Os mecanismos que poderiam explicar a melhora concomitante da sensibilidade hepática à insulina e do perfil inflamatório após exposição crônica aos AGEs não estão claros, mas incluem a inativação da GSK3beta, enzima que exerce efeitos metabólicos e anti-inflamatórios. Nossos achados devem estar relacionados com a via de administração intraperitoneal empregada neste estudo, que resulta na absorção direta dos AGEs para a circulação portal, o que, por sua vez, torna o fígado a primeira linha de defesa contra a toxicidade desses compostos. Uma vez no fígado, os AGEs desencadeiam respostas adaptativas para contrabalançar os seus efeitos potencialmente prejudiciais / The Nonalcoholic fatty liver disease (NAFLD) comprises a spectrum of conditions that includes simple steatosis and nonalcoholic steatohepatitis and it is considered the hepatic component of the Metabolic syndrome. Studies have shown that advanced glycation end products (AGEs) stimulate the production of extracellular matrix proteins, the generation of reactive oxygen species and of proinflammatory cytokines by liver cells, and that they could participate in NAFLD pathogenesis. In view of the paucity of information regarding the metabolic effects of AGEs on the liver, despite their potential to cause hepatic injury, we evaluated the effects of chronic administration of albumin modified by advanced glycation in (1) variables associated with glucose homeostasis and insulin sensitivity; (2) hepatic expression of genes involved in AGEs, glucose and fat metabolism, oxidative stress and inflammation; (3) activation status of proteins involved in insulin signaling and; (4) hepatic morphology and glycogen content. The study was performed in Wistar rats receiving, by intraperitoneal route, 20 mg/kg/body weight of rat serum albumin modified (AlbAGE) or not (AlbCTRL) by advanced glycation for 12 weeks. Chronic administration of AlbAGE did not alter alanine aminotransferase (ALT) and aspartate aminotransferase (AST) concentrations or other evaluated variables, but induced insulin resistance, as detected by the insulin tolerance test. The remaining results, however, suggested an increase in insulin sensitivity in the hepatic territory, evidenced by activation of serine/threonine-specific protein kinase (AKT), inactivation of glycogen synthase kinase (GSK3), increased hepatic glycogen content, and decreased expression of the gluconeogenesis genes Pck1 and G6pc. Other observed findings were a reduction in hepatic fat content, in the expression of the lipogenic genes Acaca and Fasn and of the proinflammatory genes Nfkb, Tnf, and Il6 genes. Mechanisms that could explain the concomitant improvement in hepatic insulin sensitivity and in the inflammatory profile after chronic exposure to AGEs are not clear, but include inactivation of GSK3beta which exerts metabolic and anti-inflammatory effects. Our findings may be related to the intraperitoneal route of administration employed in this study, which results in direct absorption of AGEs into the portal circulation and, it turn, makes the liver the first-line of defense against the toxicity of these compounds. Once in the liver, AGEs trigger adaptive responses to counterbalance their potentially damaging effects

Estudo do efeito da S-nitroso-N-acetilcisteína na lesão de isquemia/reperfusão em fígado esteatótico de ratos / Study of S-nitroso-N-acetylcysteine effect on ischemia/reperfusion lesion in steatotic livers of rats

INTRODUÇÃO: A esteatose é a doença hepática crônica mais frequente na população. Seus efeitos lesivos hepáticos podem adicionar-se aos efeitos da isquemia e reperfusão (I/R) nos transplantes de fígado e cirurgias hepáticas. Em ambos, o estresse oxidativo e o dano na microcirculação são importantes fatores na gênese da lesão hepática. Vários antioxidantes já foram testados em modelos experimentais de esteatose e I/R e mostraram benefício, entre eles a N-acetilcisteína (NAC). O óxido nítrico (NO) tem efeito controverso sobre a lesão de I/R, entretanto em baixas doses pode ser benéfico à microcirculação hepática. A S-nitroso-Nacetilcisteína (SNAC) fornece a NAC e também NO. Esta droga foi testada mostrando benefício em modelos experimentais de esteatose e I/R de músculo esquelético. Contudo, ainda não foi testada na lesão de I/R em fígados esteatáticos. MÉTODOS: Foram estudados 34 ratos machos Wistar divididos em 4 grupos: grupo I (n=8) - sem esteatose tratados com solução salina; grupo II (n=8) - sem esteatose tratados com SNAC; grupo III (n=9) - com esteatose tratados com solução salina; grupo IV (n=9) - com esteatose tratados com SNAC. A indução da esteatose foi realizada com dieta aproteica. Os animais foram submetidos a isquemia hepática parcial (70%) por um período de 1 hora e um período de reperfusão de 4 horas. A droga foi administrada por via intraperitoneal. Foram avaliados histologia, respiração mitocondrial e estresse oxidativo (MDA) hepáticos, transaminases séricas e mieloperoxidase e extravasamento do corante azul de Evans pulmonares. RESULTADOS: Não houve diferença entre os grupos I e II em todas as variáveis avaliadas (histologia, função mitocondrial (RCR e ADP/O), transaminases (AST e ALT), azul de Evans e mieloperoxidase pulmonares), encontrando-se apenas uma tendência a menor estresse oxidativo (MDA) (p = 0,07) no fígado isquêmico do grupo tratado (grupo II). A comparação entre os grupos III e IV não encontrou diferença nas transaminases (AST e ALT), e no azul de Evans e mieloperoxidase pulmonares. O grupo IV (esteatose e SNAC) apresentou no fígado isquêmico menor hemorragia intraparenquimatosa (p = 0,02), melhor função mitocondrial (RCR) (p = 0,01) (ADP/O) (p = 0,01), e menor estresse oxidativo (MDA) (p = 0,007) quando comparado com o grupo não tratado (grupo III). CONCLUSÕES: Neste modelo, o tratamento com SNAC não melhora a lesão de I/R em ratos sem esteatose. No entanto, a SNAC melhora a lesão hepática pós I/R em ratos com fígado esteatótico. / Steatosis is the most frequent chronic liver disease in general population. Steatosis can make ischemia/reperfusion (I/R) lesions worse in liver transplants and hepatic surgeries. Oxidative stress and hepatic microcirculatory damage are related to generation of steatosis and I/R lesions. Several antioxidant drugs, for example the N-acetylcysteine (NAC), have been already used in steatotic and I/R experimental models, and showed better outcome in treated groups. The nitric oxide (NO) has controversial effects on I/R, however, in low doses it can improve hepatic microcirculation. The S-nitroso-N-acetylcysteine (SNAC) releases NAC and NO. It ameliorated oxidative stress in experimental models of steatosis and skeletal muscle I/R. In contrast, this drug has never been used in steatotic liver I/R. METHODS: thirty four male Wistar rats were studied and divided in four groups: group I (n=8) - without steatosis and treated with saline solution; group II (n=8) - without steatosis and treated with SNAC; group III (n=9) - with steatosis and treated with saline solution; group IV (n=9) - with steatosis treated with SNAC. Aproteic diet was used for steatosis induction. The animals underwent a partial hepatic ischemia (70%) for 1 hour and reperfusion for 4 hours. The drug was administrated intraperitoneally. The parameters evaluated were hepatic histology, mitochondrial respiration and oxidative stress (MDA), seric transaminasis and pulmonary mieloperoxidasis and Evans blue extravasation. RESULTS: There were not any difference between groups I and II in all parameters evaluated (histology, mithocondrial function (RCR and ADP/O), transaminasis (AST and ALT), pulmonary Evans blue extravasation and mieloperoxidasis), and it showed only tendency of less oxidative stress (MDA) (p=0,07) in ischemic liver of treated group (group II). The comparison between groups III and IV didn\'t show any difference in transaminasis (AST and ALT), pulmonary Evans blue and mieloperoxidasis. Group IV (steatosis and SNAC) showed in the ischemic liver lower intraparenchymatous hemorrhage (p=0,02), better mitochondrial function (RCR) (p=0,01) and (ADP/O) (p=0,01), and less oxidative stress (MDA) (p=0,007) when compared with untreated group (group III). CONCLUSIONS: Based on this experimental model, SNAC treatment shows no benefit in non steatotic I/R lesion, however, in steatotic rats, SNAC ameliorates hepatic lesions post I/R.
