The only-child adolescent's lived experience of parental divorce / Dayle Hayley Dorfman

The aim of this study was to explore and describe the experience of an only-child adolescent’s lived experience of parental divorce. Gestalt field and phenomenological theory in conjunction with current literature provided an overview of the theoretical underpinnings pertaining to the study. A qualitative research approach with a case study of an only-child adolescent dealing with parental divorce was conducted in an ethical manner by means of two face to face, one-on-one, in depth interviews. Two main themes were identified. The first being experienced feelings associated with grief and bereavement, which revealed feelings of anger in the notion that the participant’s childhood was lost as a result of the divorce and being caught between the parental conflicts. The participant seemed to take it upon herself to take care of those significant to her in fear that she would lose them and that the loss would continue to be repeated. It was further revealed that when the only-child adolescent felt a loss of her own identity she in turn felt out of control. The second theme identified was; experience pressure due to being an only-child. Pressure in being an only-child was very significant surrounding the participant. The participant shared a great deal of feelings pertaining to feeling lonely and longing for a sibling. As parents in divorce situations seem to be focussed on their divorce often the child suffers from stress and the unrealistic expectations parents often have surrounding their children. In this study the only-child could not seem to cope with the pressure and in times of despair made use of coping mechanisms, mainly that of cutting to compensate for the emotional pain experienced. The researcher is of the opinion that the study delivered new found awareness into the only-child adolescent’s lived experience of parental divorce and is in hope that the new found results are utilised as a platform for further studies about this vulnerable population. / MSW (Play Therapy), North-West University, Potchefstroom Campus, 2015

Text in afrikaans / The main object of this research was to present the profile of a child that bullies other children and to place one changeable element, namely self-esteem, within this profile. The description of children that reflect bully behaviour in accordance with the Gestalt approach is absent in most of the familiar Gestalt literature. The Gestalt approach is therefore utilized as a theoretical framework to describe the profile of children that bully others. The empirical research methodology is performed by combining a qualitative and a quantitative research approach. The approach to the research was based on the dominant-less-dominant model whereby the dominant approach, namely the qualitative approach, was utilized. The quantitative approach had a supportive role to the qualitative approach in this research by utilizing a standardized measurement instrument to round off the profile of the child identified as a bully. The profiles compiled of the children that bully others can be utilized as a guideline for Gestalt therapeutic interventions. / Social work / M. Diac.

Anorexia nervosa is classified as a disorder that, despite existing literature, cannot be fully understood or comprehended, due to the complexity of the disorder. The complexity of Anorexia nervosa makes it difficult for people to understand these individuals, which causes resistance to occur within the adolescents. The resistance then complicates the therapeutic process, which can lead to inefficient growth and change in therapy. In this study, the experience of adolescent girls that suffer from anorexia nervosa is researched with the aim of shedding some light on this complex, though common, issue. The aim of this study was to design a Gestalt guideline to assist the Grade R teacher to identify, understand and manage the temperamental patterns of the Grade R child enabling optimal adjustment of the child. Intervention research (Design and Development model of Rothman & Thomas) as phase model was partly used as method for this study. A qualitative approach was followed during data collection. The needs and concerns in relation to temperament and temperamental patterns were explored through semi-structured interviews with 12 Grade R teachers. The identified problems were analyzed and in order to address the problems, information was gathered through relevant literature and consulting with professionals. Functional elements of successful models were identified and used in the Gestalt guideline. The study concludes at Phase 4 by designing a Gestalt guideline as preliminary intervention for the Grade R teachers. Exploring how Gestalt Play therapists establish sensory contact with children who have sensory integration disorders

Children who have sensory integration disorders face many physical, social and emotional challenges. Unusual emotional and behavioural reactions to sensory stimuli are often displayed. These children are referred to professionals including Gestalt play therapists, although formal training in sensory integration is limited to the scope of occupational therapy. Gestalt play therapists routinely utilize sensory rich activities during therapy. This study sought to explore and describe how Gestalt play therapists establish sensory contact with children who have sensory integration disorders. A combined qualitative and quantitative approach was utilized. A questionnaire and semi-structured interviews were used to gain rich descriptive data. Participants felt that the therapeutic process with these children was markedly different than with other children. Participants who were aware of their own sensory difficulties demonstrated an increased ability to accommodate these children in therapy. Moc jako aspekt terapeutického vztahu v rámci gestalt přístupu / Power as an Aspect of an Therapeutical Relationship on the Framework of the Gestalt Approach

The diploma thesis is focused on a phenomenon of power in therapy relationship in way of Gestalt approach. It's based on a relationship concept between psychotherapist and client which is a fundamental factor in psychoteraphy change. The power is perceived in accordance with Michela Foucalt's theory as a natural and necessary relational phenomenon. In Gestalt approach intentions we pursue ways, in which the psychoterapist is aware his power, how he gets in touch and how he deals with it. Diploma thesis has got a theoretical empirical character, the theoretical part sumarizes knowledge about power, relationship like a base of helping professions work and basic knowledge from Gestalt therapy field. Empirical part uses design of qualitative metodology. We got data about Gestalt therapists' attitudes, perception and understanding of the power phenomenon. These gained data are processed by the grounded theory method and compared with other data. Next possibilities of broadening gained informations and working with them in therapy field are mentioned in the conclusion. Our results broaden knowledge about therapy relationship characteristics and bring an inspiration for psychotherapy training attenders, practising therapists and other helping professionals.

Study analysis prove advances in medical and technology fields do not replace the doctor-patient relationship. Such relationship is taken as a first and essential source to adhere to treatments, support to face the obstacles arising out of acute and chronic illnesses. We also discuss the Fritz Perls Gestalt-therapy approach, focusing on the theoretical scope of the individual relationship to its environment. The main concepts of the Gestalt-therapy are: contact, contact roles, contact limits, support and awareness, and they are part of men s relationship to his environment, favoring an integrative presence in the self and the world. We believe such approach may provide facilitating instruments to improve the quality of presence, hearing and integrating understanding of the doctor-patient relationship. Buber s Dialogic Relationship inserts the dialogue in the between and the alternation of the encounter. It indicates a therapeutic attitude based on Inclusion, Presence and Commitment to the dialogue, as well as the conditions required to have a high-quality encounter. Those are suggestions for a better clinical practice of the doctor-patient relationship / Por meio desse estudo sobre a relação médico-paciente, pretende-se usar uma revisão bibliográfica para obter um panorama histórico desde os primórdios da medicina, com foco na atitude da relação médico-paciente e a qualidade da escuta e presença diante da dor e sofrimento, até a atualidade. A análise dos estudos demonstrou que os avanços da medicina e da tecnologia não substituem a relação médico-paciente, considerada uma fonte primeira e fundamental para adesão de tratamentos, suporte para o enfrentamento das adversidades advindas das doenças agudas e crônicas. Apresenta-se a abordagem Gestalt-terapia de Fritz Perls com escopo teórico cuja ênfase é na relação do indivíduo com o seu meio, e uma abordagem que visa a relação. The possibility of integration among approaches is controversial. Even dialogue among them is difficult. The psychotherapy integration movement strives to change this picture. Overcoming the intellectual isolation of a single approach can help the therapist to broaden his activities and help each approach hone and polish its teachings. Detractors depict a potential intellectual disorganization as a result of such attempts. The aim of this research was to investigate how Gestalt therapists and psychodramatists see and experience, in practice, the possibilities of integration between their approaches. Narratives were collected through individual interviews, with 22 professionals, 11 of each approach. Grounded theory analysis was adopted as a method to build a theoretical model that explains how therapists see and experience the possibilities of integration. The intention, in choosing this method, was to contribute new perspectives, not based on theories already formulated, but on the therapists lived reality. The categories that emerged from the analysis of the interviews give voice to those who find themselves in the reality of the professional practice. The model constructed from these categories depicts the frontier between psychodrama and Gestalt therapy as it is experienced by the participants. From the professional s narratives, the forces were mapped that approach and distance the two approaches. Three axes dominate the dynamics of these forces: the articulation between theory and practice, the scientific communities and the psychotherapist s subjectivity. Thus, the central thesis sustained by this work is that the dynamics of integration and the psychologists attitudes, with respect to the dialogue between the approaches, cannot be understood from rational examination of scientific argument. Instead, the relevant dynamics and attitudes are the result of the interaction among subjective, social and institutional processes. Thus, the professional who is interested in a dialogue with the other approach needs to navigate the interface between theoretical articulations that have subjective meaning arising from his or her professional history and social implications imposed by institutional relations. This interface is governed by the dynamics between the professional communities and institutions involved. In this navigation, the psychotherapist s subjectivity is revealed in his or her need to belong to a group, the effects of his or her past trajectory as a member of a school or community and his or her previous experiences with respect to other theories and communities. On the other hand, in the dynamics between the communities and the institutions, competition for symbolic and material resources is an important force that hinders dialogue and integration. The articulation of theory and practice is related to continuous attempts of each approach to meet the challenges of conceptualizing practice in psychotherapy. Construction and development of approaches are enabled by the flow and absorption of concepts. Thus, intradisciplinarity in psychotherapy contributes to the field s vitality, helps unclog disciplinary thinking and expands the therapist s possibilities for understanding and action. Accordingly, the practice of exchange does exist among the approaches. However, this practice is overshadowed by the customary disqualification of the "other" approach in academic and institutional discourse. Professional identity tends to merge with concepts and ideas of the community the professional belongs to, entailing strong loyalty to the approach. Studies on dialogue among approaches can hardly make sense of the processes involved when they fail to include these different axes of interaction. It is necessary to consider, not only the clinical and epistemological issues involved, but also the subjective, social and institutional dynamics that rule the process. / O campo das psicoterapias oferece uma profusão de saberes e práticas que inclui diversos pressupostos epistemológicos e ontológicos sobre o ser humano e a natureza da mudança. A possibilidade de integração entre as abordagens é assunto controverso. Até mesmo o diálogo entre elas é difícil. O movimento de integração em psicoterapia busca mudar nesse quadro. Superar o isolamento intelectual de uma única abordagem pode ajudar o psicoterapeuta a ampliar sua ação e ajudar as abordagens a refinarem e afinarem seus ensinamentos. Os detratores invocam uma potencial desorganização intelectual como consequência de tais tentativas. O objetivo da presente pesquisa foi investigar como gestalt-terapeutas e psicodramatistas percebem e vivenciam, na prática, as possibilidades de integração entre suas abordagens. Através de entrevistas individuais narrativas de 22 profissionais foram colhidas, 11 de cada abordagem. A Teoria Fundamentada nos Dados foi adotada como método para construir um modelo teórico que explana como os terapeutas percebem e vivenciam as possibilidades de integração. A intenção, ao escolher esse método, era de contribuir com novas perspectivas, não baseadas em teorias já formuladas, mas na realidade viva dos terapeutas. As categorias que emergiram da análise das entrevistas dão voz a quem se encontra na realidade da prática profissional. O modelo construído a partir dessas categorias retrata a fronteira entre o Psicodrama e a Gestalt terapia como ela é vivenciada pelos participantes. A partir das narrativas dos profissionais foram mapeadas forças que aproximam e afastam as duas abordagens. Três eixos dominam a dinâmica dessas forças: a articulação entre teoria e prática, as comunidades científicas e a subjetividade do psicoterapeuta. Assim, a tese central sustentada pelo presente trabalho é a de que as dinâmicas de integração e as atitudes dos psicólogos frente ao diálogo entre abordagens não podem ser compreendidas a partir de argumentos racionais relativos à articulação científica. As dinâmicas e atitudes são resultado da interação entre processos subjetivos, sociais e institucionais. Assim, o profissional que se interessa por um diálogo com a outra abordagem transita na interface entre articulações teóricas que possuem sentidos subjetivos decorrentes da história do profissional e implicações sociais impostas pelas relações institucionais. Tal interface é governada pelas dinâmicas entre as comunidades e as instituições profissionais envolvidas. A subjetividade do psicoterapeuta se desvela nas necessidades de pertencimento grupal, nos efeitos da sua trajetória passada na abordagem de filiação e nas suas experiências prévias em relação com as demais teorias. Por outro lado, nas dinâmicas entre as comunidades e instituições, a competição por recursos simbólicos e materiais é uma força importante que afasta o diálogo e a integração. A articulação entre teoria e prática está relacionada com as tentativas constantes de cada abordagem para responder aos desafios de conceituar a práxis em psicoterapia. A constituição e evolução das abordagens são viabilizadas pela circulação e incorporação de conceitos. Assim, a intradisciplinaridade em psicoterapia contribui para a vitalidade do campo, ajuda a desobstruir as disciplinas e amplia as possibilidades de ação e compreensão do psicoterapeuta. Existe, nesse sentido, uma prática de trocas entre as abordagens. Mas esta é eclipsada pela desqualificação da outra abordagem no discurso acadêmico e institucional. Um processo de fusão da identidade profissional com conceitos e ideias da comunidade à qual o profissional pertence enseja forte laço de lealdade com a abordagem. Estudos sobre diálogos entre abordagens dificilmente podem compreender os processos envolvidos quando deixam de incluir estes diferentes eixos de interação. Tais estudos deveriam levar em consideração não somente as implicações clínicas e epistemológicas, mas também questões subjetivas, sociais e institucionais.

Therefore, three therapeutic processes were presented and analyzed, which allowed to identified the care that represent the environmental support to the child's needs, which were named as: attention to interrupted movements, empathy, affective action and narrative. The constitutive experiences communicated the creative adjustment that children had done and the singular suffering they experience facing the impasse at their development. This communication, along with field information, with the support from the theory and the psychotherapist selfsupport allowed the construction of clinical reasoning and definition of therapeutic management. Besides, it was possible to identify that children psychotherapy process also includes care at the parents’s sessions so they can constitute or expand their selfsupport and offer environmental support to the child's needs. The care that were offered at the parents’s sessions were named as: environmental support to their needs, interlocution to their experiences and description of the child's experience. The care described at this work were concretized in the relationship with the children and their parents, as they were sustained by the psychotherapist selfsupport, which made it possible to recognize the sensations aroused in contact with the child and their caregiver, reflect on the experience to identify the need that was expressed by them. Thus, this work proposes an extension of the therapeutic management at the clinic with children, when the child has not yet assimilated his or hers selfsupport, which is necessary to use the method of awareness expansion, providing, in the therapeutic relationship, constitutive experiences that constitute the child's selfsupport / Esse trabalho descreve os cuidados que proporcionam experiências constitutivas, que permitem à criança constituir seu autossuporte para vivenciar sua experiência com autonomia e, assim, poder vir a usufruir das intervenções tradicionalmente propostas dentro da abordagem, orientadas pelo método da ampliação da awareness. Para tanto, três processos terapêuticos foram apresentados e analisados, o que permitiu identificar os cuidados que representam o heterossuporte para as necessidades da criança: atenção aos movimentos interrompidos, empatia, ação afetiva e narrativa. As experiências constitutivas comunicavam o ajustamento criativo que a criança havia realizado, e o sofrimento singular que vivenciava diante do impasse que enfrentava em seu desenvolvimento. Esta comunicação e as informações do campo, com apoio da teoria e do autossuporte da psicoterapeuta, permitiram a construção do raciocínio clínico e a definição do manejo terapêutico. Além disso, foi possível identificar que o processo de psicoterapia de crianças inclui também os cuidados nas sessões com os pais para que eles possam constituir, ou ampliar o seu autossuporte e exercer a função de heterossuporte para as necessidades da criança. Os cuidados que foram oferecidos nas sessões com os pais foram nomeados de: heterossuporte para as necessidades, interlocução para as experiências e descrição da experiência da criança. Os cuidados descritos nesse trabalho se concretizaram na relação com as crianças e seus pais, pois estavam sustentados no autossuporte da psicoterapeuta, o que tornou possível reconhecer as sensações despertadas no contato com a criança e com o seu cuidador, e refletir sobre a experiência para identificar a necessidade que era expressa pelo Outro. Assim, esse trabalho propõe uma ampliação do manejo da clínica com crianças, quando elas ainda não assimilaram o autossuporte necessário para usufruir do método da ampliação da awareness, proporcionando, na relação terapêutica, experiências constitutivas, que constituam o autossuporte da criança

Contribuições do grupo terapêutico de abordagem gestáltica no tratamento do transtorno depressivo recorrente moderado a grave / The contribution of Gestalt Group Therapy to the treatment of recurrent moderate to severe major depression

O objetivo do presente estudo foi avaliar preliminarmente a eficácia do grupo terapêutico de abordagem gestáltica no tratamento do Transtorno Depressivo Recorrente através da avaliação quantitativa dos sintomas depressivos, avaliação da qualidade de vida e adequação social. Foi um estudo aberto, exploratório, prospectivo, em pacientes que se encontravam em tratamento farmacológico para Transtorno Depressivo Recorrente, submetidos a dezesseis sessões de grupo terapêutico. Aplicou--se no início e no fim do tratamento: Entrevista Clínica Estruturada para o DSM-IV: Transtorno do Eixo I, 17-item Hamilton Depression Rating Scale, Medical Outcomes Study 36-item Short-Form Health Survey e Escala de Auto-Avaliação de Adequação Social. Sete pacientes terminaram as dezesseis semanas de tratamento. Observou-se melhora clínica em relação aos sintomas depressivos. Em relação à qualidade de vida, apresentaram melhora nos domínios da capacidade funcional; estado geral da saúde; saúde mental e dor. Perceberam-se inadequados em relação à adequação social, porém apresentaram indicadores clínicos de melhora após grupo terapêutico. Resultados sugerem que a participação no grupo terapêutico de abordagem gestáltica pode auxiliar na redução da sintomatologia depressiva, em certos domínios da qualidade de vida e em ganhos clínicos. Estudos com amostras maiores, com grupo controle e com melhor avaliação da adequação social e funcionalidade dos pacientes são necessários / The intention of this study was to make a preliminarily evaluation of the effectiveness of gestalt group therapy in the treatment of major depression through the quantitative assessment of the symptoms of depression and an assessment of quality of life and social functioning. The open study was of an exploratory nature and involved patients who were undergoing pharmacological treatment for major depression and who participated in sixteen gestalt group therapy sessions. The effectiveness of the therapeutic process was assessed by Structured Clinical Interviews at the beginning and at the end of treatment at which were applied the DSM-IV: Disorder Axis I, 17-item Hamilton Depression Rating Scale, Medical Outcomes Study 36-item Short-Form Health Survey Scale and Self-Assessment of Social Adequacy. Seven patients completed the sixteen weeks of treatment. A clinical improvement in the depressive symptoms was observed. With regard to quality of life, the patients showed improvement in their physical functioning, general health, mental health and bodily pain. They considered themselves poorly adjusted to society and presented improved clinical indicators after group therapy. The results suggest that participation in gestalt group therapy can help reduce the symptoms of depression, improve some areas of quality of life and demonstrate clinical gains. Further studies with a larger sample, the use of a control group and improved assessment methods of social adequacy and functionality are needed
