Experiences of Racism and Breastfeeding Initiation and Duration Among First-Time Mothers of the Black Women’s Health Study: A Dissertation

BACKGROUND: Breastfeeding and lactation are cited as sensitive periods in the life course that contribute to the accumulation of risks or opportunities ultimately shaping vulnerability or resilience later in life. As such, breastfeeding and lactation are critical components of health equity. Despite this, Black women in the U.S. initiate and continue to breastfeed at lower rates than White women and other groups. Underlying reasons for racial inequities in breastfeeding rates are poorly understood. Exposure to racism, one manifestation of historical oppression in the U.S. has been cited as a determinant of poor health outcomes for decades but has not been extensively described in the context of breastfeeding. AIMS: To investigate the association between experiences of racism and 1.) breastfeeding initiation 2.) breastfeeding duration 3.) and the association between selected life-course factors and breastfeeding initiation and duration among participants of the Black Women’s Health Study. METHODS: This study was a prospective secondary analysis of the Black Women’s Health Study. The sample included all participants who enrolled in 1995, responded to the racism assessment in 1997 and reported the birth of a first child following the racism assessment resulting in an N=2, 995 for the initiation outcome and N= 2,392 for the duration outcome. In addition to the racism assessment, we also included life-course factors (nativity, neighborhood segregation and social mobility). For each aim, we calculated odds ratios and 95% confidence intervals using binomial and multinomial logistic regression using two models. The first adjusted for age, the second adjusted for age, BMI, education, marital status, geographic region, neighborhood SES and occupation. RESULTS: Associations between daily and institutional summary racism variables and breastfeeding initiation and duration were small and not statistically significant. Experiences of racism in the job setting was associated with lower odds of breastfeeding duration at 3-5 months compared with 3 months 95% CI [0.60, 0.98]. Experiences of racism with the police was associated with higher odds of breastfeeding initiation and duration at 3-5 months [1.01, 1.77] and at 6 months [1.10, 1.82] compared with women who did not report this experience. The participant’s nativity and the nativity of her parents were life-course factors that predicted lower odds of breastfeeding initiation and duration. Neighborhood segregation did not reach statistical significance after adjusting for covariates but results trended toward lower odds of breastfeeding initiation and duration for women who reported living in a predominately Black neighborhood (compared with White) up to age 18 and for women who reported living in a predominately Black neighborhood in 1999. CONCLUSION: Experiences of institutional racism in the job setting was associated with lower odds of breastfeeding duration. In addition to explicit experiences of racism, this study provides preliminary evidence surrounding life-course factors and breastfeeding. Individual level interventions may mitigate harmful effects of racism but structural level interventions are critical to close the gap of racial inequity in breastfeeding rates in the U.S.

An assessment of the budgeting system in the Eastern Cape Department of Health to ensure effective and efficient health care service delivery

The National Department of Health is the executive department of the South African government which is assigned to the national health issues of the country. This research was however aimed specifically on the budgeting process within the Eastern Cape Provincial Department of Health. This was provisioned by assessing the significance of the budgeting system and its correlation to efficient and effective healthcare service delivery. The aim of the research was to highlight on how policies have an effect on the budgeting system at the provincial level, considering the provincial health department is responsible for the provision of publicly funded health services. This research assessed the budgeting system in the Eastern Cape Department of Health in an attempt to propagate effective and efficient health service delivery for the general citizenry. This was a qualitative study design which harnessed the merits of secondary data as the main code of the research design which enabled the comprehension of the budgeting system in the Eastern Cape Department of Health. Data was analysed using thematic analysis to examine the distinctive themes and sub-themes identified from within the reviewed literature. The Eastern Cape Province is succumbing to insufficient funding at provincial level and this has a clear-cut reflection on the administration of services within respective departments, particularly the Health Department. This is despite the fact that the Public Finance Management Act, 1999 promotes the objectives of good financial management aimed at maximizing service delivery through the effective use of the limited resources. Resultantly it was fundamental to comprehend on the issues underpinning the scope of budget reporting as well as budget accountability and the effect it has on service delivery in the Eastern Cape Department of Health. Recommendations were thus given based on the discussed and derived themes to ensure that there is an appropriate budgeting system to ensure efficient and effective provision of health care services in the Eastern Cape Department of Health.

Electronic Bedside Documentation and Nurse-Patient Communication: A Dissertation

Nurses are often the first members of the health care team with whom patients interact. The initial impression of the nurses’ receptiveness to the patients’ needs influences the patients’ views of their overall care. Researchers have suggested that understanding communication between individuals can provide the human link, or social element, to the successful implementation and use of electronic health records, including documentation (Lanham, Leykum, & McDaniel, 2012). Zadvinskis, Chipps, and Yen (2014) identified that the helpful features of bedside documentation systems were offset by the mismatch between the system and nurse’s workflow. The purpose of this micro-ethnography study was to explore the culture of nurse-patient interaction associated with electronic documentation at the bedside. Data were collected through passive participant observation, audio-taping of the nurse-patient interactions, and informal and semi-structured interviews with the nurses. A total of twenty-six observations were conducted on three nursing units at an urban healthcare facility in New England. These three units were occupied by similar patient populations and all patients required cardiac monitoring. Three themes consistently emerged from qualitative data analysis: the nurses paused during verbal communication, the nurses played a game of tag between the patient and the computer, and the nurses performed automatic or machine-like actions. The participants described these themes in the informal and semi-structured interviews. The nurses’ actions were observed during passive participant observation, and the audio-taped interactions supported these themes. Understanding the adaptation of caregiving necessitated by bedside electronic documentation will have a positive impact on developing systems that interface seamlessly with the nurses’ workflow and encourage patients’ active participation in their care.

Retrospective investigation of equity in health care within Ekurhuleni for the period 2003 to 2005, utilising the district health information software system

It was perceived that an imbalance exists between resource allocations for health care within the Ekurhuleni Health District (EHD). This study consequently used a retrospective, quantitative methodology to investigate health equity in the EHD and to collect information on clinic buildings, staffing and budget allocations. Although clinics were oversupplied in terms of the norms set by the National Department of Health, they were found not to be in keeping with population growth in the Ekurhuleni district. The study highlighted an inequitable spread of nursing staff, which would require that nursing personnel be re-deployed. The per capita spending in the different service delivery regions was also found to be inequitable. It was subsequently recommended that spending on clinics be revised (especially in the populous Southern SDR), that staff be reallocated and that budget allocations be reviewed to achieve equity in Ekurhuleni. / Health Studies / Thesis (M.A. (Public Health))

Mentorship in health services leadership

The objectives of this study were to identify leadership competencies required by health services leaders, determine the role of mentorship in leadership development and, make recommendations for succession planning in the public health sector. A quantitative approach using an exploratory and descriptive design was used, with the intention of conducting a census survey. Respondents were managers in positions 11 and upward. Findings revealed that experience assisted managers in improving the technical competencies of human resources, financial and strategic planning, but not that of leadership skills and behaviour, and communication and relationship management, which required development in the form of mentorship. It is recommended that leadership development for future leaders be embedded in succession planning, based on policy guidelines. Limitations of this study were that a response rate of 30% was achieved and due to the narrow geographical coverage, the findings could not be generalised. / Health Studies / M.A. (Health Studies)

Esta pesquisa buscou estudar as dificuldades vivenciadas pelos gerentes na utilização da ferramenta de Gestão por Processo, sendo possível analisar a percepção desses gestores e o grau de dificuldade nesta metodologia, identificando os principais aspectos do processo e analisando as estratégias utilizadas pelos profissionais da saúde, gestores da instituição, para a manutenção da qualidade. O cenário de estudo foi a Irmandade Nossa Senhora das Mercês – Hospital Santa Casa de Montes Claros, classificada como Hospital Geral, possuindo 320 leitos. Os sujeitos pesquisados foram os 24 Gerentes de Equipe tendo como instrumento de coleta de dados o questionário, previamente validado e testado. Ao finalizar todo o processo de análise, pôde-se concluir que a maioria dos gestores tem curso superior e parte deles já tem especialização. A maioria dos envolvidos teve o primeiro contato com esta metodologia na própria Santa Casa e já faz uso das ferramentas administrativas ao longo da trajetória profissional na instituição. Ao implantar esta metodologia, inicialmente, as principais dificuldades da utilização tratavam-se da falta de conhecimento teórico e prático dos gerentes, da cultura organizacional e da resistência às mudanças. Na atualidade, as principais dificuldades apontadas pelos gestores foram outras. O cenário atual aponta como o principal dificultador da utilização da metodologia Gestão por Processo o acúmulo de atividades do Gerente dentro da Santa Casa. Ao analisar a opinião dos pesquisados sobre o que seria realizado no gerenciamento de processos em uma unidade, a maioria compreendeu tratar-se de medição de indicadores, interação de processos e realização de análise crítica do mesmo. Outro ponto que necessitou ser destacado é o fato de o Hospital Santa Casa de Montes Claros oferecer informações sobre Gestão por Processo de forma muito satisfatória para os profissionais que trabalham no processo decisório. Esse envolvimento entre a instituição e os gestores fortaleceu a segurança e o senso crítico dos profissionais, avaliando os processos para que houvesse maior eficácia nos resultados previstos. Essa gestão é a eleita para trabalhar na Santa Casa, e o Gerente de Unidade é o profissional escolhido para realizar o levantamento e controle dessas informações. Assim, é necessário adequar suas atividades e minimizar as dificuldades encontradas utilização da metodologia de processos. Quanto aos benefícios com a implantação da metodologia Gestão por Processos no Hospital Santa Casa, a pesquisa apontou que a maioria acredita ser a constante busca da melhoria na qualidade dos serviços, na obtenção do título de Acreditação Hospitalar, no alinhamento de condutas e na padronização dos processos. Uma vez reconhecidas as implicações do sistema de qualidade pelos próprios profissionais, mais fácil será o caminho para a manutenção de todo o processo e a conquista da excelência no atendimento. For this, was elaborated the guiding question: how the manager nurse exercises the leadership in a hospital context? Were outlined that objectives: to analyzes the leadership of University Hospital nurse managers, to describe the experiences related to leadership in nurses managementevery day and to elaborate a representative theoretical matrix of nurse managers experiences´ in relation to leadership. It is about a descriptive and qualitative research which scenario was the Santa Maria University Hospital (HUSM), Rio Grande do Sul. The participants were nurse managers and the data were collected in April and May 2012, using semi-structured interviews.The analysis had been started concomitant with the data collect, with theuse of Grounded Theory (GT), theoretical support of Symbolic Interactionism and others leadership studies.Were followed the ethical principles of Resolution No. 196/96 of the National Health Council, which regulates research involving humans.That way, the audio data´swere transcripts and coded according to the order: open coding, axial code and selective coding.The results originated the thematic categories: realizing a nurse manager, looking for meanings to the process of leading and being leader, experiencing the service reality, confronting management challenges, establishing interpersonal relationships and seeking improvements to the service.The interconnection of these, through the analysis with the paradigmatic model has made emerge a theoretical matrix, composed by the central phenomenon following:experiencing nursing management in a hospital context: meanings to leadership. Finally, it was possible identified that the nurse manager realizes the importance of their actions at the managerial dimension and seeks to achieve their objectivesthrough attitudes that approaches a participative leadership model.Thus, should be paid attention with regard the need to awaken a look that valuates interpersonal relationships and the development of the human potential inhealthcare organizations. / Mais do que constatar a importância da presença de líderes ativos no contexto da enfermagem hospitalar, buscou-se resgatar os significados dos enfermeiros que vivenciam a posição de gerentes em enfermagem e que possuem suas ações permeadas de possibilidades relativas ao desenvolvimento de liderança. Em vista disso, o objeto da pesquisa realizada foi a liderança de enfermeiros-gerentes que atuam em um Hospital Universitário do interior do Rio Grande do Sul. Para isso, elaborou-se a questão norteadora: como o enfermeiro-gerente exerce a liderança no contexto hospitalar? Foram traçados como objetivos: analisar a liderança de enfermeiros-gerentes de um Hospital Universitário, descrever as vivências relacionadas à liderança no cotidiano gerencial dos enfermeiros e elaborar uma matriz teórica representativa da experiência dos enfermeiros-gerentes em relação à liderança. Trata-se de uma pesquisa descritiva e qualitativa cujo cenário foi o Hospital Universitário de Santa Maria (HUSM), Rio Grande do Sul. Os sujeitos foram enfermeiros-gerentes, e os dados foram coletados em abril e maio de 2012, por meio de entrevistas semiestruturadas. A análise foi iniciada concomitante à coleta, com a utilização da Teoria Fundamentada nos Dados (TFD) e o respaldo teórico do Interacionismo Simbólico e estudos sobre liderança. Foram seguidos os princípios éticos da Resolução nº 196/96 do Conselho Nacional de Saúde que regulamenta a pesquisa envolvendo seres humanos. Dessa forma, os dados em áudio foram transcritos e codificados de acordo com a ordem: codificação aberta, codificação axial e codificação seletiva. Os resultados obtidos originaram as categorias temáticas: percebendo-se enfermeiro- gerente, buscando significados para o processo de liderar e ser líder, vivenciando a realidade do serviço, enfrentando desafios gerenciais, estabelecendo relações interpessoais e buscando melhorias para o serviço. A interconexão dessas, por meio da análise junto ao Modelo Paradigmático, fez emergir uma matriz teórica, composta pelo seguinte fenômeno central: vivenciando a gerência de enfermagem no contexto hospitalar: significações para a liderança. Por fim, foi possível identificar que o enfermeiro- gerente percebe a importância de suas ações na dimensão gerencial e busca atingir seus objetivos por meio de atitudes que se aproximam de um modelo de liderança participativa. Assim, deve-se atentar no que se refere à necessidade de despertar um olhar voltado para a valorização das relações interpessoais e ao desenvolvimento do potencial humano nas organizações de saúde.

The Role of Primary Care Nurses in Addressing Unmet Social Needs

PURPOSE The purpose of this study was to explore how primary care registered nurses address unmet social needs in patients. SPECIFIC AIMS Explore how RNs in a safety-net, primary care setting develop an awareness of and address patient's unmet social needs. Describe how information about unmet social needs are integrated into nursing assessment and intervention activities, and are shared with other members of the health care team. Describe the challenges primary care RNs face when addressing unmet social needs. FRAMEWORK Critical caring theory provided the framework for this study. DESIGN This study used a descriptive, qualitative design. Semi-structured interviews were conducted with seventeen nurses working in 11 different safety-net primary care clinics within a hospital-based system. RESULTS Three major themes emerged. Key findings included the importance of the nurse-patient relationship, the establishment of trust, and a caring, nonjudgmental approach to patients with unmet social needs. Nurses used knowledge of unmet needs to coordinate patient care, provide social support, and work collaboratively with care team members to refer patients to resources within the health care system and in the community. CONCLUSION Unmet social needs contribute to adverse health outcomes, and addressing social and medical needs is critical to eliminating health inequities and reducing health care costs. In this study, primary care nurses described relationships with patients that allowed for the sharing of sensitive information, leading the nurse to identify and address unmet social needs that could impact patient health.

Dinâmica da inovação no serviço público da atenção primária à saúde

A teoria sobre inovação tem se desenvolvido, intensamente, nas últimas décadas, incluindo a análise da inovação em serviços públicos de saúde. Doravante, o setor de serviços representa em diversos países, inclusive no Brasil, taxas de crescimento anuais superiores a todos os demais setores da economia. A Atenção Primária à Saúde (APS) constitui a essência da reorientação e reestruturação de muitos sistemas de saúde (OPAS, 2005), pois atua como um núcleo organizador e integrador de serviços de rede e de promoção, prevenção e recuperação da saúde. Diante da relevância das questões pautadas pela finalidade de incentivo à melhores condições de saúde nas populações, a partir da rede complexa da APS (MENDES, 2007), e da importância da compreensão da inovação em serviços para as organizações, o objetivo dEsta tese foi analisar a dinâmica das inovações no serviço público da APS. Considerando o ambiente público, propôs – se que a APS possui uma lógica estruturada para o desenvolvimento das inovações a partir de interações constantes entre diferentes agentes na produção do serviço. Para atingirmos essa proposição central, ocorreu a necessidade de conceber, holisticamente, o desenvolvimento, integrações e repercussões da inovação neste setor. A metodologia utilizada foi de uma pesquisa exploratória e descritiva dividida em duas etapas. A primeira etapa da pesquisa utilizou uma entrevista semiestruturada para identificação da tipologia da inovação no setor e entendimento sobre o processo de inovação. Também, utilizou, um questionário estruturado para análise da qualidade do serviço sob orientação da APS. Para a segunda etapa de pesquisa foi aplicado outro questionário estruturado a fim de se identificar as capacidades e preferências mobilizadas pelos agentes para ocorrência de inovações. Fez – se uso de análise multivariada dos dados coletados envolvendo técnicas qualitativas e quantitativas considerando as duas etapas. Os principais resultados apontam para a identificação de cinquenta e seis inovações nas quais estão relacionadas a qualidade do serviço sob orientação da APS. Moreover, the services sector accounts in several countries, including Brazil , annual growth rates in excess of all other sectors of the economy. The Primary Health Care (PHC) is the essence of the reorientation and restructuring of many systems (OPAS, 2005), it acts as a core organizer and integrator of network services and promotion, prevention and rehabilitation of health. Given the importance of the issues guided by the aim to encourage better health in populations from the complex network of APS (MENDES, 2007), and understanding the importance of innovation in services for organizations , the aim of this thesis was to analyze the dynamics of innovation in the public service of APS. Considering the public environment, it was proposed that the APS has a structured logic for the development of innovations from ongoing interactions between different agents in the production of the service. To achieve this central proposition was the need to design holistically development, integration and impact of innovation in the service sector. The methodology used was an exploratory and descriptive divided into two stages of research. The first stage of the research used a semistructured interview to identify the typology of innovation in the industry and understanding of the innovation process. Also, we used a structured questionnaire to analyze the quality of the service under the guidance of APS. The second stage of the survey provided for the use of a structured questionnaire in order to identify the capabilities and preferences mobilized by agents for the occurrence of innovations. Made use of multivariate analysis of the collected data involving qualitative and quantitative techniques considering two steps. The main results show the identification of fifty six innovations related to quality of service under the guidance of APS. Also show a preponderance of server capacity in mobilizing innovation that permits configuring the innovation process in the public service of APS contributing in the design of an evidence – based approach to innovation.

Relação entre as forças internas e externas na inovação em serviço na atenção primária à saúde

Esta Tese tem, como tema central, a inovação em serviço na Atenção Primária à Saúde (APS) e, como objetivo, analisar a relação entre as forças internas e externas na inovação em serviço na APS de Pelotas, caracterizando a sua forma de organização e gestão, os processos estabelecidos para inovar, sua orientação em APS e as inovações geradas/produzidas nas dimensões: produto, processo, marketing e organizacional (Manual de Oslo, 2005). Cada dimensão foi analisada nas perspectivas:estrutura física, recursos humanos, equipamentos, medicamentos e materiais, acesso a exames complementares, gestão e atendimento ao usuário. Inovações em serviços de saúde têm como premissa, a melhoria da produtividade e eficiência, o aumento da qualidade e da capacidade de resposta, a redução das variabilidades na prática assistencial e a ampliação do acesso aos serviços de saúde. A APS compreende o primeiro nível de contato dos indivíduos, da família e da coletividade, com o sistema de saúde. O método de investigação teve um delineamento transversal, objetivo exploratório e, quanto à natureza de abordagem quantitativa e qualitativa. Realizou-se um estudo de casos com a aplicação de pesquisa empírica à população de gestores ligados ao sistema de saúde da APS da cidade com perguntas sobre a conformação do sistema de saúde, a inovação em serviços e sua orientação em APS de acordo com o Primary Care Assessment Tool, instrumento validado por Starfield (2001) e Brasil (2010). Os resultados quantitativos demonstram que as inovações em serviços eram, em maioria da tipologia produto (98%), processo (90%), e organizacional (88%) e em menor proporção a de marketing (32%). Em todas as dimensões, as perspectivas com mais inovações estavam ligadas à gestão, atendimento ao usuário e recursos humanos. Em relação ao desfecho “Orientação em APS”, a média geral de 6,91 (dp = 1,12) demonstrou que as UBS do município estão orientadas em APS.Contudo, quando se estratifica por modelo de atenção, as UBS que trabalham com a Estratégia de Saúde da Família (ESF) estão orientadas com escore de 7,51 (dp = 0,71), enquanto que as Tradicionais obtiveram uma média de 5,91 (dp = 0,95), consideradas não orientadas em APS (p = 0,000). Quanto à associação das inovações e à orientação em APS, constatou-se na análise bruta por Regressão de Poisson, pertencer ao modelo de atenção às variáveis ESF, ter planejamento de ações e organização formal das atividades, as quais estiveram significativamente associadas à orientação em APS.No entanto, na análise ajustada, apenas a variável modelo de atenção permaneceu associada ao desfecho (2,86; IC 95% 2,18 – 6,91). No. of bitstreams: 1 Tese Isabel Cristina Rosa Barros Rasia.pdf: 10235240 bytes, checksum: 3569ed6ea040f96692e34d526f178db3 (MD5) Previous issue date: 2016-09-15 / This thesis focuses on the innovation in services at Health Primary Attention (HPA) andaims at analyzing the relationship between internal and external forces in the innovation in services at HPA in Pelotas, describing its organization and management ways, the procedures established for innovating, its guidance on the HPA, and the innovation generated/produced on the following dimensions: product, process, marketing, and organizational area (Manual de Oslo, 2005). Each dimension was analyzed under perspectives as follows: physical structure, human resources, appliances, medicines and materials, access to complementary examinations, management, and user service. Innovation on health services assumption is the improvement of production and efficiency, the increase on quality and responsiveness, the reduction on variation in care practice, and the increase of access to health services. The HPA includes the individual’s, family, and collectivity first level of contact with the health system. Research was performed through a cross-sectional, objective, exploiting method under a quantitative, qualitative nature of approach. A case study was conducted by the application of an empirical research to the population of managers linked to the health system of the local HPA asking about the health system forming, innovation in services, and its guidance on the HPA according to the Primary Care Assessment Tool, a tool validated by Starfield (2001) and (2010).Quantitative results show that the innovation in services were, as to their typology, mostly: product (98%), process (90%), and organizational (88%), and fewest in marketing (32%). In all dimensions, the perspectives presenting more innovations were connected to management, user care, and human resources. Regarding to “HPA orientation” outcome, the general average 6.91 (SD = 1,12) indicated that the local “UBS” are arranged in HPA. However, when stratified according to the attention model, the “UBS” working with the Family Health Strategy (FHS) scored 7.51 (SD = 0.71),while Traditional ones averaged 5.91 (dp = 0.95), considered as not guided in HPA (p = 0,000). As to the association of innovations and orientation in HPA it was seen under the gross analysis by Poisson Regression that they belong to the model that pays attention to FHS variables, which were significantly related to the orientation in HPA. In the adjusted analysis, however, only the variable attention model remained related to the outcome (2.86; IC 95% 2.18 – 6.91). The qualitative analysis identified that the county performs the full management model of its Municipal Health System and the processes of innovation in services are leveraged by the Federal Government, being the programs and projects launched at national level, along with the Municipal Secretariat of Health, developed with the effective participation of health teams from the Basic Unit of Health (BUH), based on planning and formalization, supported by the assigned community, Universities, Municipal Council of Health (MCH), and the Local Council of Health (LCH).
