Fenomén alkoholismu a možnosti sociální práce / The phenomenon of alcoholism and opportunities for social work

Maternová, Marcela January 2013 (has links)
This diploma thesis explores the phenomenon of formation and development of alcohol dependence. In the first series focuses on the historical description, which demonstrates considerable anchored in the life of our ancestors already. It also defines the concept of addiction, its causes and consequences, whether psychological, medical or social. Its objective is to describe the possibilities of social work in this phenomenon. defines therefore primarily targets and understanding of social work and consequently specifics of client alcoholism. An important element is the role of the social worker in the client's motivation to change, which uses Nešpor's model of spontaneous changes in motivation. Then, on the basis of available social services selects several most suitable, which can help to improve the client's situation. Has an essential role in this issue also primary prevention, ie it discusses the methodology, target groups, focusing on adolescents and the focus is on the firm role of the family. Finally contains some official documents on primary prevention, which are discussed current issues of primary prevention practice and subsequent survey, mapping aspect of social workers on the incidence risk of alcohol dependence among adolescents attending social facilities.

Aspects analytiques, cliniques et médico-judiciaires des nouvelles substances psychoactives / Analytical, clinical and forensic aspects of new psychoactive substances

Ameline, Alice 14 June 2019 (has links)
Due to the uncontrolled spread on the Internet and their legal alternative to usual drugs, the new psychoactive substances (NPS), recently appeared (2008), are at the center of recent phenomena of addiction and badly explained deaths. Beyond different challenges in our societies (prevention, legislation), the ability to identify NPS in biological samples, in order to characterize their use, presents many analytical challenges. The main objective of this thesis was to collect biological samples (blood, urine, hair) from cases of exposure to NPS and to characterize the substances present using original analytical methods, in order to enlarge the libraries of mass spectra and improve, as a result, the detection of NPS consumption. In particular, it was intended to increase the detection sensitivity of NPS intake by focusing on the metabolites that are often the major products of elimination. This analytical development, by ultra-high liquid chromatography coupled with tandem mass spectrometry (UHPLC-MS/MS), required several months of optimization in order to obtain a robust, exhaustive and sensitive method. At present, the mass spectra database has 114 NPS and is regularly updated. Thereafter, ma thesis focused on the study of cases of intoxication observed in the emergency department of Strasbourg, but also in legal medicine with situations of deaths and identification of unknown products collected from seizures (powders and crystals). It has also been necessary to implement complementary analytical tools, such as the characterization of metabolites by human liver microsomes (HLMs), and the use of nuclear magnetic resonance (NMR) spectroscopy to accurately identify the compounds and establish their purity degrees. The analytical tools developed, and the strategy adopted, allowed the writing of 18 publications, as well as the setting up of numerous collaborations.

Proactivité des systèmes de soins à domicile face à des crises sanitaires / Proactivity of Home Health Care systems facing health crisis

Barkaoui, Houssem 04 July 2018 (has links)
Today, the accumulation of risks associated with factors such as increasing urbanization, climate change and variability, terrorism, animal and human epidemics or pandemics and enlarged mobility of people and goods have increased the disruptive and even destructive impact of various types of disasters. Crisis management plans are developed to better manage the crisis or even to avoid it. The purpose of this research is to develop decision aid tools to support the different phases of the crisis management plan for home health care (HHC) structures, mainly for preparation and response phases. A set of scenarios, considering the different sources of crises and the impacted business processes, with regard to their frequency and their human and material impacts, have been defined. An original approach of vulnerability analysis based on graph theory and matrix methods has been developed. The objective is to measure the impact of different crises on the HHC structure in terms of actors and exchanged flows. Based on this assessment phase we have proposed a classification of HHC actors. Different methods have been proposed for the design of mitigation strategies and countermeasures for each class of actors. The problem of access to care in the context of a scenario of massive intoxication is considered. A linear programming model has been proposed to simulate the studied scenario and evaluate its consequences. It models a foodborne infection and its medical response as part of a HHC-Hospital collaboration, in the framework of an extended white plan. The issue of access to patients' homes has been investigated in the context of a flood-like scenario. The proposed tool provides an evacuation plan for critical patients to evacuate and a home support plan for low-risk patients who will be kept at home, using clustering based on each patient's risk assessment.

Uso intenso de álcool e outros comportamentos de risco à saúde entre estudantes universitários da Pontifícia Universidade Católica do Paraná / Intense alcohol use and other health risk behaviors among college students of the Catholic University of Paraná (Pontifícia Universidade Católica do Paraná) - PUCPR

Duarte, Paulina do Carmo Arruda Vieira 04 July 2005 (has links)
This paper presents a cross sectional study on health risk behaviors and their association with drinking patterns, among College students of the Catholic University of Paraná (Pontifícia Universidade Católica do Paraná) - PUCPR. Students were divided into 3 groups, according to their drinking patterns: binge drinkers, defined as having five or more drinks in at least, one single occasion during the last 30 days\", current drinkers (had some alcohol in the last 30 days but not reported a binge episode), and non drinkers (did not report alcohol use in the last 30 days). Risk behaviors considered in the study are those monitored by the Centers for Disease Control and Prevention (CDC) of USA and with greatest impact on the morbidity and mortality of the contemporary youth. The time periods (last 12 months or 30 days) and frequencies (once, many times, others) measured varied according to the behavior analyzed. Data obtained by means of an anonymous self completing questionnaire in a sample of randomized students (n=1201), documents that a great proportion of students get involved on risk behaviors and several of these behavior are positivetly associated with binge drinking. Results also lead to the conclusion that the majority of the binge drinkers (77,7%) admitted that their alcohol use behavior has resulted in, at least 3 problems during the last 3 years. Despite of study limitations, its results indicate the suitability of the development of programs for comprehensive student\'s health promotion.

Drug-related morbidity and mortality : Pharmacoepidemiological aspects

Jönsson, Anna K. January 2007 (has links)
Adverse drug reactions (ADRs) constitute a significant health problem with consequences for the patient as well as for society. Suspected ADRs have been reported to occur in about 2-14% of hospitalised patients. In about 5% of deceased hospitalised patients suspected ADRs may have caused or contributed to the fatal outcome. When a pharmaceutical drug is approved for marketing, the drug has been tested only on a limited number of patients (often <6000) for a limited time period in a controlled environment. Hence mostly common ADRs are detected in these trials. Moreover, certain patient groups, for example patients with co-morbidities, elderly patients, children and pregnant women are often not included in these studies. Thus, it is important to closely monitor the use of drugs after marketing to observe new effects and detect new ADRs. The aim of this thesis is to describe the pattern of pharmaceutical substance use related to morbidity and mortality and to investigate two serious ADRs. We have studied the incidence of fatal ADRs, fatal intoxications, cerebral haemorrhage related to warfarin treatment and venous thromboembolism (VTE) related to treatment with antipsychotic drugs. Observational studies form the basis for this thesis. Data from the Swedish Cause of Death Register, medical case records, the Swedish database on ADRs, the forensic pathology and forensic toxicology databases, and Swedish and Danish hospital discharge registers, Danish prescription registers, and civil registry systems were used. In Paper I we found that 3% of all fatalities in a Swedish population were related to a suspected ADR. Of the deceased hospitalised patients, 6% were related to a suspected ADR. Haemorrhage was the most commonly observed fatal suspected ADR, accounting for almost two-thirds of the events and anticoagulantia was the most common drug group associated with fatal suspected ADRs (almost 50%). A suspected intoxication could have contributed to the fatal outcome in 0.6% of the deceased. Among the fatal intoxications in Swedish medico-legal autopsies studied in Paper II, on average four substances were detected per case. The five most commonly detected substances in individuals with a fatal intoxication were ethanol, propoxyphene, paracetamol, diazepam and flunitrazepam. Among patients diagnosed with cerebral haemorrhage, 10% (59 cases) were treated with warfarin at onset of symptoms (Paper III). Of these, 7 cases (12%) were considered to have been possibly avoidable since the patients were treated with concomitant drugs that have the potential to enhance warfarin effects. The results from Paper IV and Paper V in combination with the published literature suggest that patients treated with antipsychotic drugs have an increased risk for VTE. Compared with non-users, an adjusted odds ratio for VTE of 2.0 was found for users of any antipsychotic drugs in a Danish population. In a medico-legal autopsy series, an adjusted odds ratio for fatal pulmonary embolism of 2.4 and 6.9 was found for users of first-generation low-potency antipsychotics and second-generation antipsychotics, respectively. In summary, drug-related morbidity and mortality is a significant problem and suspected ADRs contribute to a substantial number of deaths. Fatal intoxications are relatively common and it is important to observe changes in patterns of substances associated with fatal intoxications to be able to discover new trends and monitor effects of preventive work. A significant proportion of warfarin-related cerebral haemorrhage was caused by drug-drug interactions and was considered possible to avoid. Users of antipsychotic drugs may increase the risk of VTE.

Langevin, Jean-Philippe 05 1900 (has links)
School dropout is affecting a large proportion of adolescents in Quebec. Factors which predict school dropout come from individual, familial, social, school, and sociodemographic dimensions. Although many studies have found a significative link between substance use and school dropout, no study have examined the potential protective role of the socioeducational climate on the relationship between these behaviors despite support for such an effect. Thus, this study aims to verify if the student-teacher relationship climate plays a protective (moderator) role on the link between substance use and school dropout by using a longitudinal prospective design. The sample for this study is taken from New approach, new solutions (NANS) and includes 728 participants. Data were obtained thru self-report questionnaires as well as from the Ministère de l’éducation, des loisirs et des sports for official data on school dropout. Logistic regression analyses showed that the student-teacher relationship climate seems to protect against the effect of regular cannabis use on school dropout but not against low or non-use. Conversely, the analyses haven’t shown such a protective effect in the case of alcohol intoxication. Implications for these results are discussed.

The prevalence and degree of dehydration in rural South African forestry workers.

Biggs, Chara. January 2008 (has links)
South African forestry workers are predisposed to dehydration due to the heavy physical activity they perform in impermeable regulation safety clothing in hot and often humid environments where the availability of a variety of suitable fluids at reasonable temperatures is limited. As dehydration reduces both physical and mental capacity the potential consequences include decreased productivity and an increased risk for injury. The aim of this cross sectional observational study was to determine the prevalence and severity of dehydration in rural forestry workers in both winter (minimum and maximum daily temperatures 3-22°C) and autumn (minimum and maximum daily temperatures 14-27°C). The convenience sample included 103 workers in autumn (Nelspruit, n=64 males, n=39 females, mean age 37.32 years, mean BMI 22.3 kg/m2) and 79 in winter (Richmond, n=68 males, n=11 females, mean age 25.85 years, mean BMI 22.2 kg/m2). The sample included chainsaw operators, chainsaw operator assistants, debarkers and stackers. The risk of heat illness was moderate in Nelspruit (average daily temperature 21.1°C 67% rh) and low in Richmond (average daily temperature 17.0°C 39% rh). The prevalence of dehydration was determined by urine specific gravity (USG) measurements. Percent loss of body weight in the course of the shift was used to determine the severity of dehydration. In Nelspruit 43% (n=43) and in Richmond 47% (n=37) of the forestry workers arrived at work dehydrated (USG>1.020 g/ml). Pre break this had increased to 49% (n=49) in Nelspruit and 55% (n=33) in Richmond. By the end of shift the number of dehydrated forestry workers had significantly increased to 64% (n=64, p≤0.001) in Nelspruit and 63% (n=42, p=0.043) in Richmond. A minimum of 21% (n=2) in Nelspruit and 23% (n=15) in Richmond of the forestry workers had lost more than 2% of their body weight which could significantly decrease work capacity and work output as well as mental and cognitive ability. Dehydration was not related to season (winter/autumn), gender or job category. In Nelspruit 23% (n=23) and in Richmond 13% (n=10) arrived at work overhydrated (USG<1.013 g/ml). Pre break this had decreased to 14% (n=14) in Nelspruit and 10% (n=6) in Richmond. By the end of shift 4% (n=4) in Nelspruit and 2% (n=1) in Richmond had remained overhydrated and without correcting for fluid and food intake, 5% (n=5) had gained over 2% of their body weight in Nelspruit while none had gained weight in Richmond. Overhydration was not related to season (winter/autumn), gender or job category. Physical symptoms at the end of shift included tiredness (24%), toothache (13%) and headaches (10%) although these did not correlate to end of shift USG readings (p=0.221). The fluid requirements for male workers (n=8) who did not eat or drink across the shift was 439 ml per hour. The contractors were unaware of how much fluid should be supplied to workers and how much fluid they actually supplied. The only fluid provided by the contractors was water at the ambient air temperature which was the main source of fluid for the majority. Some forestry workers brought a limited variety of other fluids including amahewu, tea and cold drinks to work. At least 40% of the work force investigated, started their shift already compromised to work to capacity (USG>1.020 g/ml). The prevalence of dehydration had increased by the break emphasizing the need to begin drinking early on in the shift. The majority of forestry workers were dehydrated at the end of the shift. A significant proportion was dehydrated to the extent (>2%) that both work capacity and mental ability would be significantly compromised. A select group of forestry workers were drinking excessive amounts of fluid and were therefore susceptible to potentially fatal dilutional hyponatremia especially as water was the primary source of fluid. Dehydration in both autumn and winter was identified as being a significant but preventable risk. As a consequence of overhydration, a small group of forestry workers may be susceptible to dilutional hyponatremia. Fluid intake guidelines for males of 450 ml per hour appeared to be safe and were within the recommendations of the American College of Sports Medicine. Fluid guidelines for females need investigation. / Thesis (M.Sc.)-University of KwaZulu-Natal, Pietermaritzburg, 2008.

This thesis includes three papers on toxic plants that cause or are suspected of causing sudden death associated with exercise in domestic animals, poisoning characterized by a per acute course without significant morphological heart changes. The first chapter is a literature review, detailing recent changes that have occurred in the identification of different species of toxic Amorimia (formerly referred as Mascagnia) found in Brazil. We also report the epidemiology, toxicity, clinical signs, necropsy and histologic findings, diagnosis, and prevention and control of the poisoning by Amorimia spp. The second chapter reports the induction of resistance to poisoning by Amorimia septentrionalis by the repeated administration of non toxic doses, and also by transfaunation of ruminal contents from resistant to susceptible goats. In this study, we observed that animals receiving non-toxic doses and transfaunation of ruminal content were more resistant than goats that never ingested the plant. The third chapter is a short communication that evaluated experimentally the toxicity of Callaeum psilophyllum (synonym Mascagnia psilophylla) in rabbits and goats, as well as the possibility of the occurrence of poisoning by this plant in ruminants. It was demonstrated that, in rabbits, this plant causes similar clinical signs than those caused by plants causing sudden death; however, in goats, C. psilophyllum causes digestive signs, mainly diarrhea. There is no evidence that C. psilophyllum causes spontaneous poisoning in ruminants.
