Com a descrição teofânica da história de Êxodo, da protagonização de Moisés e seu povo, das dificuldades, revoltas, dos locais sagrados, a entrega do decálogo a Moisés e deste ao povo hebreu, até a saída do deserto e a chegada na terra prometida. Atividade da polimixina B isoladamente e em combinação com tigeciclina, meropenem e ertapenem frente a isolados de Enterobacter sp. resistentes aos carbapenêmicos

Diante do aumento global da resistência microbiana, cada vez mais as combinações de antimicrobianos são utilizadas na tentativa de obter uma atividade sinérgica eficaz que possa ser utilizada na prática clínica. O objetivo deste trabalho foi comparar a atividade de polimixina B isoladamente e em combinação com tigeciclina e carbapenêmicos (ertapenem e meropenem), frente a isolados de Enterobacter sp. resistentes aos carbapenêmicos. Foram selecionados quatro isolados de Enterobacter sp. resistentes aos carbapenêmicos, sendo três isolados produtores de carbapenemases (KPC, NDM, OXA-48-like) e um não produtor de carbapenemases (NPC). A avaliação da atividade sinérgica antimicrobiana foi realizada por ensaio de time-kill com as seguintes concentrações de cada antimicrobiano: tigeciclina 1mg/L, meropenem 1mg/L e ertapenem 0,5mg/L, que correspondem aos pontos de corte de sensibilidade do CLSI (2016). Para polimixina B, foram utilizadas diferentes concentrações de acordo com o perfil de susceptibilidade do isolado: para os isolados sensíveis, utilizou-se 0,5x; 1x e 2x a CIM do isolado; para o isolado resistente, foram utilizados 0,5x; 1x e 2x o ponto de corte de sensibilidade do CLSI (2 mg/L). Foi considerada sinérgica a combinação com redução ≥ 2 logs do inoculo inicial em comparação ao antimicrobiano sozinho mais ativo. As combinações de polimixina B em diferentes concentrações (0,5; 1 e 2mg/L) com meropenem apresentaram atividade sinérgica para a cepa produtora de NDM (CIM polimixina B 1 mg/L; CIM meropenem 8 mg/L) As combinações de polimixina B com tigeciclina foram sinérgicas apenas na concentração de polimixina B de 0,125 mg/L para cepa produtora de OXA-48-like (CIM polimixina B 0,25 mg/L; CIM tigeciclina 2 mg/L) e na concentração de 1 mg/L para a cepa produtora de NDM. Para a cepa NPC (CIM polimixina B 0,5 mg/L; CIM tigeciclina 4 mg/L; CIM meropenem 4 mg/L), apenas a tripla combinação com polimixina B (1 mg/L), tigeciclina e meropenem apresentou atividade sinérgica. A combinação de dois carbapenêmicos não apresentou atividade sinérgica para nenhum isolado, porém, quando acrescentado tigeciclina ao esquema, observou-se sinergismo no isolado produtor de OXA-48-like, o que leva ao questionamento da efetividade da terapia “carbapenem suicida”. Para o isolado produtor de KPC e com padrão de resistência a polimixina B, nenhuma das combinações testadas apresentou sinergismo. Esta situação é bastante preocupante devido à alta prevalência de infecções por bactérias produtoras de KPC nos hospitais brasileiros, juntamente com as crescentes taxas de resistência as polimixinas. / Due to the increase in microbial resistance, combinations of antimicrobials are increasingly being used to improve the therapeutic response. The objective of this study was to compare the activity of polymyxin B alone and in combination with tigecycline and carbapenem (ertapenem and meropenem) against to carbapenem-resistant Enterobacter sp isolates. Four isolates were selected, three were carbapenemase-producing (KPC, NDM, OXA-48-like) and a non-carbapenemase-producing (NCP). The evaluation of synergistic antimicrobial activity was performed by time-kill assay with the following concentrations of each antimicrobial: tigecycline 1mg/L, meropenem 1mg/L and ertapenem 0.5mg/L, which correspond to the breakpoints of CLSI (2016). For polymyxin B, different concentrations were used according to the susceptibility profile of the isolate: for susceptible isolates, it was used 0.5x; 1x and 2x the MIC of the isolate, for the resistant isolates it was used 0.5x; 1x and 2x the breakpoint of CLSI (2 mg/L). The combination with reduction ≥ 2 logs of the initial inoculum compared to the most active antimicrobial alone was considered synergies. Combinations of polymyxin B at different concentrations (0.5; 1 and 2 mg/L) with meropenem showed synergistic activity for the NDM-producing isolate (MIC polymyxin B 1 mg/L, MIC meropenem 8 mg/L) Combinations of polymyxin B with tigecycline were synergistic only at the concentration of polymyxin B of 0.125 mg/L for the OXA-48-like-producing isolate (MIC polymyxin B 0.25 mg/L, MIC tigecycline 2 mg/L) and at the concentration of 1 mg/L for the NDM-producing isolate. For the NCP isolate (MIC polymyxin B 0.5 mg/L; MIC tigecycline 4 mg/L; MIC meropenem 4 mg/L), only the triple combination of polymyxin B (1 mg/L) plus tigecycline and meropenem presented synergistic activity. The combination of two carbapenems was not synergic for the four isolates; however, when tigecycline was added to the regimen, synergies for the OXA-48-like-producing isolate were observed. Therefore, the so called, "carbapenem suicide" therapy was not effective in vitro against our isolates. For the KPC-producing isolate that it is polymyxin B resistant, none of the combinations tested showed synergies. This situation is very worrisome due to high prevalence of infections by KPC-producing in the Brazilian hospitals, associate with the increasing rates of polymyxins resistance.

Avaliação da atividade de amicacina e polimixina B isoladamente e combinados com imipenem frente a isolados de P. aeruginosa resistentes a carbapenêmico

Base teórica: Devido à diminuição do desenvolvimento de novos antimicrobianos nas últimas décadas, terapias combinadas têm sido empregadas contra bactérias multirresistentes como opção à monoterapia no tratamento de infecções graves. Para tratar infecções causadas por Peusdomonas aeruginosa resistente a carbapenêmicos, amicacina e polimixina B têm sido utilizadas em associações com imipenem, pois possuem diferentes mecanismos de ação, o que, teoricamente, indicaria a possibilidade de efeito sinérgico. Objetivo: O objetivo deste trabalho foi verificar a interação, in vitro, de amicacina e polimixina B em associação com imipenem frente a diferentes isolados de P. aeruginosa resistentes a carbapenêmico. Métodos: Foram selecionados isolados de P. aeruginosa resistentes a carbapenêmico oriundos do Hospital de Clínicas de Porto Alegre, coletados no período de janeiro a março de 2015. Foi realizada eletroforese em gel de campo pulsado e detecção do gene blaSPM-1 para selecionar diferentes clones. Seis isolados (três SPM-1 positivos e três negativos para a carbapenemase) foram selecionados para realização da técnica de time-kill, a fim de avaliar a atividade antimicrobiana das combinações de imipenem com amicacina e imipenem com polimixina B. Resultados: Sinergismo ocorreu para combinações de imipenem com amicacina em 3 isolados, dos quais um era SPM-1 positivo e dois eram SPM-1 negativos e todos apresentaram CIMs relativamente baixas a intermediárias. Quanto às combinações de imipenem com polimixina B, houve sinergismo somente para dois isolados, um SPM-1 positivo e um SPM-1 negativo, contudo houve antagonismo em 5 isolados, dois SPM-1 positivos e três SPM-1 negativos. Para 4 isolados, as combinações de imipenem com amicacina tiveram atividade bactericida, enquanto, para todos os isolados, as combinações de imipenem com polimixina B, bem como polimixina B isoladamente, 1x e 2x a CIM apresentaram atividade bactericida. Conclusão: Sinergismo pode ocorrer, para combinações de imipenem mais amicacina, quando os isolados apresentam CIMs relativamente baixas ou intermediárias para imipenem (≤16 μg/mL a 128 μg/mL) e amicacina (≤32 μg/mL). Entretanto, antagonismo aconteceu independentemente de valores altos ou baixos de concentrações inibitórias mínimas para imipenem e polimixina B. Além disso, a presença ou ausência do gene blaSPM-1 não pareceu influenciar nos resultados. / Background: Due to a decrease on new antibiotic development over the last decades, combined therapies have been employed against multigrug-resistant bacteria as an option to monotherapy in severe infection treatment. In order to treat infections caused by carbapenem resistant Pseudomonas aeruginosa, amikacin and polymyxin B have been used in associations with imipenem, because they possess different mechanisms of action, which could, theoretically, indicate the possibility of synergistic effect. Objective: The aim of this study was to verify the interaction, in vitro, of amikacin and polymyxin B in association with imipenem against various carbapenem resistant P. aeruginosa isolates. Methods: Carbapenem resistant P. aeruginosa isolates have been selected from Hospital de Clínicas de Porto Alegre, collected from January to March 2015. Pulsed field gel electrophoresis and detection of blaSPM-1 gene has been performed to select different clones. Six isolates (three SPM-1 positive and three negative for the carbapenemase) were selected for time-kill assay, in order to assess antimicrobial activity of imipenem plus amikacin and imipenem plus polymyxin B combinations. Results: Synergism occurred for combinations of imipenem plus amikacin in three isolates, from which one was SPM-1 positive and two were SPM-1 negative, and all presented relatively low to intermediate minimum inhibitory concentrations. About imipenem plus polymyxin B combinations, synergism occurred in only two isolates, one SPM-1 positive and one SPM-1 negative, however antagonism occurred in five isolates, two SPM-1 positive and three SPM-1 negative. For 4 isolates, imipenem plus amikacin combinations had bactericidal effect, while, for all isolates, combinations of imipenem plus 1x and 2x the MIC of polymyxin B presented bactericidal activity. Conclusions: Synergism can occur, for imipenem plus amikacin combinations, when isolates present relatively low or intermediate MIC of imipenem (≤16 μg/mL to 128 μg/mL) and amikacin (≤32 μg/mL). However, antagonism happened regardless high or low minimum inhibitory concentrations for imipenem and polymyxin B. Also, the presence or absence of blaSPM-1 gene did not seem to influence the results.

CARBAPENEM-RESISTANT <em>ENTEROBACTERIACEAE</em>: EPIDEMIOLOGY, GENETICS, <em>IN VITRO</em> ACTIVITY, AND PHARMACODYNAMIC MODELING

Background: Infections caused by carbapenem-resistant Enterobacteriaceae (CRE) such as Escherichia coli and Klebsiella pneumoniae are among the most urgent threats of the infectious disease realm. The incidence of these infections has been increasing over the years and due to very limited treatment options, mortality is estimated at about 50%. By 2050, mortality from antimicrobial resistant infections is expected to surpass cancer at 10 million deaths annually. Methods: We evaluated 18 contemporary antimicrobials against 122 carbapenem-resistant Enterobacteriaceae using a variety of antimicrobial susceptibility testing methods according to Clinical Laboratory Standards Institute guidelines. Time-kill studies were performed on clinical isolates with variable resistance to meropenem, amikacin, and polymyxin B. Phenotypic expression assays were performed on all isolates and whole genome sequencing was performed on 8 isolates to characterize molecular resistance mechanisms. Pharmacodynamic modeling of meropenem and polymyxin B was also conducted. Results: CRE were primarily K. pneumoniae, and Enterobacter spp. 60% expressed Klebsiella pneumoniae carbapenemase (KPC) only, 16% expressed Verona Integron-encoded Metallo-beta-lactamase (VIM) only, 5% expressed KPC and VIM, and 20% expressed other mechanisms of resistance. Antimicrobial susceptibility testing indicated the most active antimicrobials against CRE were ceftazidime/avibactam, imipenem/relebactam, amikacin, tigecycline, and the polymyxins. Etest® strips did not reliably measure polymyxin B resistance. The automated testing system, BD Phoenix™, consistently reported lower MICs than the gold standard broth microdilution. Time-kill studies showed regrowth at clinically achievable concentrations of meropenem alone (4, 16, and 64 mg/L), polymyxin B alone (0.25 and 1 mg/L), or amikacin alone (8 and 16 mg/L), but combinations of meropenem with either polymyxin B or amikacin were bactericidal and synergistic. Meropenem administered simultaneously or prior to polymyxin B exhibited superior activity to polymyxin B administered first. Conclusions: Novel carbapenemase-inhibitor combinations (ceftazidime/avibactam and imipenem/relebactam) exhibit the best activity against KPC-producing CRE. The polymyxins, amikacin, and tigecycline exhibit the best activity against VIM-producing CRE. Meropenem in combination with polymyxin B is bactericidal and synergistic when the meropenem MIC is ≤32 mg/L, and meropenem should never be administered after polymyxin B. Meropenem and amikacin is bactericidal and synergistic when the amikacin MIC is ≤16 mg/L. Etest® strips should not be used for characterizing polymyxin B or colistin activity. Clinicians should be aware that automated testing systems may produce biased susceptibility results relative to the gold standard method, broth microdilution, which may influence interpretation of in vitro results.

Foraging Ecology of Mountain Lions in the Sierra National Forest, California

Studies of predator-prey and predator-predator interactions are needed to provide information for decision-making processes in land management agencies. Mountain lions (Puma concolor) are opportunistic carnivores that prey on a wide variety of species. In the Sierra National Forest, CA, they have not been studied since 1987 and their current interactions with their prey and other predators are unknown. Forest managers in this region are concerned with declines of fishers (Pekania pennanti) and studies have shown intraguild predation to be a leading cause of fisher mortality in this area. Managers are interested in learning more about mountain lion predation patterns with regard to prey preference, but also how lions traverse and use the landscape and how anthropogenic activities may be increasing lion predation risk on fishers. Using GPS radio-collar technology, we examined mountain lion kill rates and prey composition at 250 kill sites. We found mule deer (Odocoileus hemionus) to be their main source of prey (81%) with gray foxes (Urocyon cinereoargenteus) comprising 13.2% of prey composition. We did not detect any fisher predation during our 2-year study; however, during our study, the Kings River Fisher Project experienced extremely low juvenile fisher survival. To gain a better understanding of seasonal resource selection by mountain lions, we developed resource selection functions (RSF) while they were moving through the landscape and when killing prey. We developed RSF models for all data across the study area, as well as, for a subset of data encompassing an area where LiDAR (Light Detection and Ranging) data had been collected. Within the LiDAR study area, we digitized unmapped roads and skid trails using a Bare Earth data set. We found mountain lion ‘moving’ locations showed selection for close proximity to streams during summer months and selection for ruggedness and steeper slopes during both summer and winter. With 3 of the 4 RSF models at kill sites showing high risk of predation within close proximity to either digitized roads/skid trails or mapped roads, we recommend managers map all anthropogenically created linear landscape features and consider restoring these linear features to pre-treatment landscape conditions following timber harvest.

An Effectiveness Evaluation Method For Airburst Projectiles

Airburst projectiles increase the effectiveness of air defense, by forming clouds of small pellets. In this work, in order to evaluate the effectiveness of airburst projectiles, Single Shot Kill Probability (SSKP) is computed at different burst distances by using three lethality functions defined from different measures of effectiveness. These different measures are target coverage, number of sub-projectile hits on the target and kinetic energy of sub-projectiles after burst. Computations are carried out for two different sub-projectile distribution patterns, namely circular and ring patterns. In this work, for the determination of miss distance, a Monte Carlo simulation is implemented, which uses Modified Point Mass Model (MPMM) trajectory equations. According to the results obtained two different distribution patterns are compared in terms of effectiveness and optimum burst distance of each distribution pattern is determined at different ranges.

The persistence of common wombats in road impacted environments

There is growing global concern over the influence of road development on the conservation of biodiversity and on the functioning of ecosystems. Published reviews in the field of road ecology have identified that most research has examined the effects of roads linearly and have advocated for research at landscape scales. Among the many effects roads have, one of the most significant is the loss of animal life resulting from collisions with vehicles. Despite this, little is known of what toll this has on animal populations and how these impacts vary with scale. This stems from the perception that impacts are localised and that animals killed are typically considered common, and therefore not of great conservation concern. This thesis challenges this notion by showing that the impacts of fatalities can affect populations at landscape extents and that commonness is not a barrier to localised extinction risk. To achieve this I focus on the common wombat; an example of a common species for which road impacts have never been previously examined. Chapter 1 provides an overview of the importance of scale in quantifying road impacts and the debate surrounding common species persistence in road environments. Chapter 2 assesses habitat use of wombats in a road environment at a local scale. Results suggest that wombats select for roadside habitat and as a result populations could be under threat. Chapter 3 is a predictive model of wombat road fatalities which demonstrates the importance of incorporating habitat use in predictive fatality modelling. Through use of a spatially explicit population viability analysis, Chapter 4 demonstrates that roads, in conjunction with other threats can affect the persistence of a common species at a local scale. Chapter 5 is a landscape extent assessment of wombat habitat use, finding that increased effort should be employed in evaluating how reserves confer resilience to species from the impacts of roads and that habitat quality can dictate road-based fatality rates. Chapter 6 summarises the research presented in the thesis and suggests direction for future work, particularly the importance of evaluating the interplay between susceptibility and abundance on species vulnerability in road environments.

Além disso, os atropelamentos podem atrair animais necrófagos ou domésticos (cães e gatos) para as carcassas, aumentando a probabilidade de novos atropelamentos. Embora vários estudos sobre atropelamentos de vertebrados tenham sido publicados no mundo, poucos tem documentado a mortalidade no Brasil. Nós realizamos um estudo que focou nos fatores causais bem como no valor (Disposição A Pagar – DAP) dos animais mortos em colisões com veículos. Nossa proposta foi documentar a composição de espécies e a riqueza e compreender os fatores espaciais e temporais relacionados a mortalidade na rodovia. Nós registramos e identificamos as espécies e o número de vertebrados atropelados em dois trechos de rodovias estaduais (97 Km da MG-428 e 63 Km da SP-334) no sudeste do Brazil. As rodovias foram amostradas semanalmente, de janeiro a dezembro de 2007. Além disso medidas de variáveis explanatórias relacionadas as estradas foram incluídas: tipo de acostamento, topografia, tipo de vegetação e desenho da estrada. Nós usamos estes dados para examinar a relação entre as variáveis explanatórias e a mortalidade dos grupos de vertebrados nas estradas ao longo do tempo. Foi realizado o teste do qui-quadrado de aderência para examinar a distribuição espacial dos atropelamentos. Nós também entrevistamos 601 motoristas para obter a disposição a pagar (DAP) pela conservação da fauna. Usamos a regressão logística para estabelecer... / Roads result in mortality on the road as well as habitat removal, landscape fragmentation, edge effects, and pose a physical barrier to many species. Some roadside factors are related to road-kills, e.g., type of shoulder, topography, and vegetation. Road alignment (curved or straight) has an influence on vehicle speed and is an important conditioning factor needing consideration. Additionally, road-kills may attract scavengers or domestic animals (cats and dogs) to carcasses, increasing the probability of new road-kills. Although many studies on vertebrate road-kill have been published around the world, few have documented road mortality in Brazil. We conducted a study that focused on causal factors as well as the value (Willingness To Pay) of animals killed by vehicle collisions. Our purpose was to document species composition and richness and to understand the spatial and temporal patterns of roadrelated mortality. We recorded the number and species identification of vertebrates killed on two state roads segments (63 Km of SP - 334 and 97 Km of MG - 428) in southeastern Brazil. The roads were surveyed weekly from January to December 2007. Additional road related explanatory variables measured, included: type of shoulder, topography, vegetation type, and road design. We used that data to examine correlations between the explanatory variables and the mortality of vertebrate groups on the roads over time. The use of unmanned combat aerial vehicles (UCAV) is relativity new with the first known lethal strike taking place in 2001. Few systems are operational but many are in development. The use of UCAVs is debated and many claim that UCAVs make it casual for the pilots to kill, and the pilots are not able to differentiate the ethics of what they do from those of a video game. When researched it is found that some factors regarding the use of UCAVs may make a pilot less reluctant to execute a remote lethal strike, yet it does not make it casual. The pilots know that what they do has real world implications and what they see on their screens are real people. Contrary to some people’s beliefs, UCAV pilots may have a greater connection with their victim as well as a greater reluctance towards killing compared to bomber pilots. / Användandet av bestyckade obemannade flygande farkoster (UCAV) är relativt nytt. Det första kända fallet där en UCAV:s vapensystem nyttjats för att döda skedde 2001. Få system är operativa men flera är under utveckling. Användandet av UCAV:er är debatterat. Flera hävdar att hur de nyttjas leder till att piloten blir likgiltig till att döda och att piloten inte kan skilja på det piloten gör och att spela ett tv-spel. När ämnet studerats så har det visat sig att flertalet av de faktorer som spelar in i användande av UCAV:er gör det lättare för piloter at övervinna det motstånd en person känner till att ta en annans liv. Dock så blir piloten inte likgiltig till det. Piloterna är medvetna om att deras handlingar har en påverkan på den riktiga världen och vad de ser på sina skärmar är riktiga människor.
