Midwifery students' experiences of clinical teaching at Sovenga Campus (Limpopo College of Nursing), Limpopo province

The purpose of midwifery nursing education is to prepare midwives who are fully qualified to provide high-quality, evidence-based mother and child health care services. A quantitative descriptive explorative design was used to identify and explore midwifery students’ experiences of clinical teaching at Sovenga Campus, (Limpopo College of Nursing), Limpopo province. Structured questionnaires were used to collect data. The research sample consisted of fifty (50) midwifery students from Sovenga Campus. Ethical principles were adhered to. Validity and reliability were maintained. The findings showed that learning opportunities are being utilised and clinical accompaniment by tutors has improved. The findings indicated that clinical teaching in midwifery units does not include implementation of the maternity guidelines and protocols’ being used .A recommendation was that tutors be included in student orientation and the planning of monthly unit in-service programmes. / Health Studies / M.A. (Nursing Science)

Negotiating the Normal Birth : Norms and Emotions in Midwifery Education / Förhandlingar om den normala förlossningen : Normer och känslor i en barnmorskeutbildning

Pregnancies and childbirths are not just biological or medical events. Childbirths are also emotionally intense situations, not only for the parents-to-be but also for those who work in delivery care. In addition, pregnancies and childbirths are sociocultural situations; there are norms regarding the normal birth. The study investigates how future midwives learn about the interactive and emotional work involved in supporting women in delivery care. It focuses on norms about the normal birthing trajectory and on how a midwife should encounter patients’ feelings and handle her own feelings in a “proper” way. Based on observations of collaborative group discussions within Swedish universitybased midwifery education, the study shows how students negotiate the appropriate feeling norms in normal birth, as well as in complicated and even tragic situations. In focus are their discussions on how to support the birthing women, their partners, and the babies, and the categorization work needed to recognize potential deviations from the normal birth. The study is based on a situated learning perspective on education, and combines sociological and anthropological approaches to emotions to elucidate how students within midwifery education negotiate the professional handling of normal and complicated births and the attendant feeling norms. / Graviditeter och förlossningar är inte bara biologiska eller medicinska händelser. Förlossningar är också känslomässigt intensiva situationer, både för de blivande föräldrarna och för dem som arbetar inom förlossningsvård. Graviditeter och förlossningar kan dessutom ses som sociokulturella händelser och det finns normer om den normala förlossningen. I avhandlingen undersöks hur framtida barnmorskor lär sig att stödja kvinnor inom förlossningsvården och det interaktiva och emotionella arbete som det innebär. Avhandlingen fokuserar på normer om den normala  förlossningstrajektorian samt hur en barnmorska ska bemöta patienternas känslor och hantera sina egna känslor på ett ”korrekt” sätt. Baserat på observationer av basgruppsgruppdiskussioner under den universitetsförlagda delen av en barnmorskeutbildning, visar studien hur barnmorskestudenter förhandlar känslonormer i relation till normal förlossning samt till komplicerade och även tragiska situationer. I fokus är studenternas diskussioner om hur de kan stödja de födande kvinnor, deras partners och barn samt den kategorisering som behövs för att identifiera eventuella avvikelser från den normala förlossningen. Studien bygger på ett situerat lärandeperspektiv på utbildning och kombinerar sociologiska och antropologiska förhållningssätt till känslor för att på så sätt belysa hur studenter inom en barnmorskeutbildning förhandlar hur normala och komplicerade förlossningar bör hanteras och de känslonormer som framkommer.

An evaluation of postnatal care rendered to HIV positive women and their infants

The purpose of this study was to evaluate care rendered to HIV positive women and their infants during the first six weeks of postpartum. Quantitative, descriptive, cross sectional and analytic study was conducted to investigate postnatal care services provided to HIV positive mothers. Data collection was done using structured questionnaires. 372 respondents participated in the study. Descriptive data analysis was used; Epi info version 3.5.2 software was used. The study highlighted that the quality of PNC was compromised, in all levels including the critical immediate postnatal care, 3-14 days and 6 weeks postnatal care services. All health facilities that were involved in the study had the basic resources to render quality postnatal care. Negative attitudes of staff and long waiting time (16.7%), were the most deterrents to postnatal care. / Health Studies / M.A. (Health Studies)

Intrapartum clinical guideline for monitoring and managing a woman during labour

This research study explored and described a partogram as a clinical practice guideline for monitoring and managing women during labour at Vhembe District of Limpopo province. A qualitative, explorative, descriptive and contextual design was used in order to achieve the study objectives. Purposive sampling was carried out to select three hospitals in order to obtain comprehensive data from the district. The research study was conducted in three phases. Face-to-face semi-structured interviews were conducted with 17 midwives who were selected conveniently from the three hospitals. Document study was also conducted on 24 partograms that were conveniently selected from the three hospitals on the days that interviews were conducted, using a self-designed checklist. Tesch’s eight steps of qualitative data analysis were used to analyse the data, and simple, descriptive statistics using frequencies were used to analyse the data obtained from the document study. Four themes that emerged from the data are: benefits of implementing the partogram, challenges experienced, attitudes of staff and evaluation on how the guideline is being implemented in the wards, including giving feedback. These aspects were indicated as pivotal in the implementation of the guideline in monitoring a woman during labour. The findings on document study revealed gaps in recording, mostly on aspects that needed frequent observations and on aspects in which resources that needed to be used in monitoring were lacking. The strategies to improve the implementation of a guideline were developed based on the research findings, sent to one of the hospitals for review and comments were included in the discussions. It is recommended that in-service training on the implementation of the partogram be conducted regularly for midwives. Hospital management and supervisors need to provide support, enough staff and equipment in order to enable midwives to use the partogram / Health Studies / D. Litt. et Phil. (Health Studies)

Perceptions of midwives and pregnant women of the prevention of mother-to-child transmission of HIV programme at the ante-natal care unit and maternity ward at the Johan Heyns community health centre in tne Sedibeng District, Gauteng

The study reports on the perceptions of the midwives and pregnant women of the PMTCT of HIV programme at the antenatal care and maternity ward at the Johan Heyns Community Health Centre. A qualitative approach was adopted to conduct the study. Purposive sampling was used to select participants and was informed by social behavioural theories. Data was collected using interviews and analysed using thematic categorisation. The findings show that at the first PMTCT encounter participants had little to no knowledge of the PMTCT programme, generally displayed a lack of interest, experienced emotional distress, and fear at the thought of having to disclosing their HIV-positive status to their partners/family and had certain trepidations about participating in the PMTCT programme. The participants’ perception on their roles was that their roles were interlinked, midwife needs the recipients (pregnant woman) and pregnant woman needs the provider (midwife) therefore one cannot do PMTCT without the other. The study recommends that the capacity building of pregnant women be optimised, that PMTCT awareness campaigns for women of childbearing age should be a priority and PMTCT skills to be prerequisite for midwives deployed to ANC clinics and maternity ward units. / Health Studies / M. Knowledge, attitudes and practices of nurse-midwives related to obstetric care at Thaba-Tseka district in Lesotho

The purpose of the study was to describe the knowledge, attitudes and practices of nurse-midwives related to obstetric care at Thaba-Tseka, Lesotho. A quantitative, descriptive, cross-sectional research design was used. Data were collected using structured questionnaire from 45 nurse-midwives. The findings revealed that nurse-midwives had mean knowledge score of 10.5(80.7%) out of a possible 13(Standard Deviation (SD) 1.31) on obstetric care issues. However, the majority of nurse-midwives (n=28, 62.2%) did not have knowledge on the steps of the active management of third stage of labour according to the WHO. The mean scores on practice were 34.5(86.2%) against a possible 40(SD 5.43) for antenatal care, 39.2(89%) against a possible of 44(SD 4.66) and 22.4(93.3%) against a possible of 24(SD 2.18) for postnatal care. The study revealed that nurse-midwives had positive attitudes towards obstetric care practices with mean score for attitudes was 23.4(86.7%) against a possible score of 27(SD 3.02) / Health Studies / M.A. (Public Health)

Une exploration des effets de la satisfaction au travail des sages-femmes sur le «burnout», l’intention de quitter et la mobilité professionnelle : une étude longitudinale au Sénégal

Dans le contexte actuel de crise des ressources humaines en Afrique sub-saharienne, il est important de comprendre comment les professionnels de santé vivent leur travail et comment cela les affecte. Cette étude longitudinale se focalise sur les sages-femmes du Sénégal en raison de leur rôle stratégique dans la réduction de la mortalité maternelle et infantile dans le pays. Une cohorte de 226 sages-femmes de 22 hôpitaux au Sénégal a participé à l’étude. Leur satisfaction au travail a été mesurée en 2007-8 avec un instrument multidimensionnel développé en Afrique de l’Ouest. Trois effets attendus : le « burnout, » l’intention de quitter et la mobilité professionnelle ont été mesurés deux ans plus tard. Des statistiques descriptives ont comparé celles qui ont quitté leur poste à celles qui sont restées, et une série de régressions multiples a modélisé les liens entre les facettes de la satisfaction au travail et les variables d’effet, en contrôlant pour les caractéristiques individuelles et institutionnelles. Les résultats ont montré un taux relativement faible de « turnover » sur 2 ans (18%), malgré une forte intention de quitter son poste (58.9%), surtout pour faire une nouvelle formation professionnelle. Les départs étaient majoritairement volontaires (92%) et entièrement intra-nationaux. Les sages-femmes se disaient le moins satisfaites avec leur rémunération et leur environnent de travail, et une forte proportion éprouvait des niveaux élevés d’épuisement émotionnel (80.0%) et de dépersonnalisation (57.8%). Toutefois, elles rapportaient être satisfaites de leur moral et de la stabilité de leur travail, et seulement 12% avaient des niveaux faibles d’accomplissement personnel. L’épuisement émotionnel était associé avec l’insatisfaction vis-à-vis de la rémunération et des tâches, la recherche active d’emplois était liée avec être insatisfaite de la stabilité du travail et avoir quitté son poste était associé avec une insatisfaction avec la formation continue. Cette étude montre que les sages-femmes semblent souffrir de « burnout », mais qu’elles se sentent toujours confiantes et accomplies au travail. Les résultats suggèrent que promouvoir la formation continue pourrait aider à retenir les sages-femmes dans leurs postes et dans la profession. / In the context of a health workforce crisis in sub-Saharan Africa, a better understanding of how health professionals are experiencing their work and how it is affecting them is needed. This longitudinal study focuses specifically on Senegalese midwives, given their strategic role in the promotion of mother and child health in the country. A cohort of 226 midwives from 22 hospitals across Senegal participated in the study. Their job satisfaction was measured in 2007-8 using a multifaceted instrument developed in West Africa. Three purported effects were measured two years later: burnout, intention to quit and turnover. Descriptive statistics compared the midwives who stayed and left their posts during the study period, and a series of exploratory regressions investigated the correlations between the 9 facets of job satisfaction and each effect variable, while controlling for individual and institutional characteristics. Relatively low turnover was found over 2 years (18%) despite high levels of intention to quit within a year (58.9%), particularly in pursuit of a new professional specialty. Departures were largely voluntary (92%) and entirely domestic. The midwives reported themselves least satisfied with their remuneration and work environment, and a large proportion experienced “high” levels of emotional exhaustion (80.0%) and depersonalization (57.8%). However, they were found to be satisfied with their morale and the stability of their work, and only 12.4% reported diminished personal accomplishment. Emotional exhaustion was related to being dissatisfied with remuneration and job tasks, actively searching for jobs was associated with dissatisfaction with job stability and voluntary quitting was associated with being unhappy with continuing education. This study shows that although midwives seem to be burned-out and unhappy with their working conditions, they retain a strong sense of confidence and accomplishment in their work. It also suggests that strategies to retain them in their positions and in the profession should emphasize continuing education.

Surviving birth : Studies of a simplified neonatal resuscitation protocol in a low-income context using a mixed-methods approach

United Nations has lately stated ambitious health targets for 2030 in the Sustainable Development Goal agenda, following the already achieved progress between 1990 and 2015 when the number of children dying before the age of five was reduced by more than half. However, the mortality reduction in the first month of life after birth has not kept the same pace. Furthermore, a large number of stillbirths have previously not been accounted for. The aim of this thesis was to evaluate the impact of clinical training in neonatal resuscitation, and to identify strategies for an effective implementation at a maternal health facility in Nepal. Focus group discussions were used to explore the perceptions of teamwork among staff working closest to the infant at the facility. A prospective cohort study with nested referents was applied to determine effect on birth outcomes after an intervention with Helping Babies Breathe, a simplified protocol for neonatal resuscitation. Sustainability of the acquired skills after training was addressed by employing a quality improvement cycle. Video recordings of health workers performance were collected to analyse adherence to protocol. Midwives described the need for universal protocols in neonatal resuscitation and management involvement in clinical audit and feedback. There was a reduction of intrapartum stillbirth (aOR 0.46, 95% CI 0.32–0.66) and neonatal mortality within 24 hours of life (aOR 0.51, 95% CI 0.31–0.83) after the intervention. Ventilation of infants increased (OR 2.56, 95% CI 1.67–3.93) and potentially harmful suctioning was reduced (OR 0.13, 95% CI 0.09–0.17). Neonatal death from intrapartum-related complications was reduced and preterm infants survived additional days in the neonatal period after the intervention. Low birth weight was not found to be a predictor of deferred resuscitation in the studied context. This study confirmed the robustness of Helping Babies Breathe as an educational tool for training in neonatal resuscitation. Accompanied with a quality improvement cycle it reduced intrapartum stillbirth and mortality on the day of delivery in a low-income facility setting. Improved postnatal care is needed to maintain the gains in survival through the neonatal period. Increased management involvement in audit and quality of care could improve clinical performance among health workers.
