Durante a evolução e desenvolvimento da assistência obstétrica, o períneo feminino ganhou visibilidade, tornando-se com frequência, um local de intervenção cirúrgica. A partir da década de 1980, em resposta aos resultados desfavoráveis desse modelo, surgiu o movimento de humanização do parto, que além da assistência respeitosa e acolhedora, propôs uma atenção baseada em evidências científicas sendo destacado pela OMS e MS a enfermeira obstétrica como componente da humanização. Diante da função do corpo perineal, sua importância para as estruturas adjacentes e a consciência da morbidade após o trauma, a manutenção da integridade anatômica é fundamental para o funcionamento adequado e para a qualidade de vida da mulher e por isso diferentes técnicas e intervenções perineais estão sendo utilizadas e estudadas objetivando a manutenção da integridade local e minimizando lacerações de maior gravidade. O objetivo deste estudo foi analisar o desfecho materno perinatal da assistência às mulheres pelas enfermeiras obstétricas com a utilização da conduta hands off. Conduziu-se um estudo de corte transversal, incluindo 608 mulheres que tiveram seus partos assistidos por enfermeiras obstétricas com a utilização de hands off como conduta de proteção perineal. Foram avaliadas as características sociodemográficas e clínicas obstétricas. Os dados foram coletados a partir de prontuário eletrônico e livros de registros da equipe de enfermagem. A análise estatística foi realizada através do teste χ2 de Pearson e do modelo de regressão logística. Foi observado que houve uma prevalência de desfechos perineais de baixa gravidade, em 96,5% foram de acometimentos de baixa gravidade (1° grau ou períneo integro), mostrandose com aumento de chance de correção perineal em mulheres primíparas e quando peso foi maior ou igual a 3500g. Contudo, considerando os resultados desta investigação, acredita-se que a conduta expectante para proteção perineal, hands off, pode ser considerada dentro das boas práticas para assistência à mulher no processo de parturição, tendo em vista que esta conduta promove benefícios relacionados aos desfechos maternos perineais sem comprometer os resultados perinatais. / The process of institutionalization of childbirth and birth brought important advances and improvements for the care of women and the newborn, but it was accompanied by a set of standardized and interventionist obstetric practices routinely used to characterize the technical model. During the evolution and development of obstetric care, the female perineum gained visibility, often becoming a surgical site. From the 1980s onwards, in response to the unfavorable results of this model, the humanization movement of childbirth emerged, which, in addition to the respectful and welcoming assistance, proposed an attention based on scientific evidence, being highlighted by WHO and MS obstetric nurse as a component of Humanization. In view of the function of the perineal body, its importance for adjacent structures and the awareness of morbidity after trauma, the maintenance of anatomical integrity is fundamental for the proper functioning and quality of life of the woman and therefore different perineal techniques and interventions are being used and studied aiming the maintenance of the local integrity and minimizing lacerations of greater gravity. The objective of this study was to analyze the perinatal maternal outcome of care for women by obstetric nurses with the use of the hands off procedure. A cross-sectional study was conducted, including 608 women who had their deliveries attended by obstetric nurses with the use of hands off as perineal protection conduit. Sociodemographic and obstetric clinical characteristics were evaluated. The data were collected from electronic medical records and records books of the nursing team. Statistical analysis was performed using the Pearson χ2 test and the logistic regression model. It was observed that there was a prevalence of low-severity perineal outcomes in 96.5% of cases of low-severity (1st degree or integral perineum), showing an increased chance of perineal correction in primiparous women and when weight was greater than or equal to 3500g. However, considering the results of this investigation, it is believed that expectant management for perineal protection, hands off, can be considered within the good practices to assist women in the process of parturition, considering that this conduct promotes benefits related to maternal outcomes without compromising perinatal outcomes.

Caractérisation des discontinuités dans des ouvrages massifs en béton par la diagraphie électrique de résistivité / Characterization of discontinuities inside massive concrete structures with electrical resistivity logging

Les discontinuités sont préjudiciables à la pérennité des structures en béton. Les méthodes non-destructives sont bien développées pour l’étude des altérations en surface, mais peu de techniques sont adaptées à la caractérisation de défauts dans la masse. Dans cette thèse, des mesures de résistivité électrique sont réalisées pour l’étude des discontinuités (fissures, joints, interfaces) au sein des ouvrages massifs en béton par l’intermédiaire des forages préexistants. La technique utilisée est la diagraphie électrique de résistivité en dispositif normal. Une première approche numérique (éléments finis) permet d’appréhender les corrections à apporter sur les mesures. Puis l’étude des paramètres d'ouverture, de contraste entre la résistivité de la discontinuité et du béton, et d'extension permet de proposer une méthode d’inversion des mesures pour la caractérisation de l’endommagement. Des essais sur des ouvrages hydrauliques sont réalisés afin de définir la méthodologie de mesure sur site. Les mesures sont ensuite confrontées à la méthode d’inversion pour la valider. / Discontinuities are one of the most harmful damage to the durability of concrete structures. The non-destructive methods appear to be effective for the detection and the location of surface cracks but they might be unsuitable for investigation of massive concrete body. In this thesis, the electrical resistivity measurements are made for the study of cracks and discontinuities (concrete joints, interfaces ...) in massive concrete structures by preexisting boreholes. The study is based on a first numerical approach (finite elements) to understand the corrections on data. Then the study of the aperture, the resistivity contrast between the discontinuity and the concrete, and the extension provide a method for the inversion of measures to characterize the damage. The tests on hydraulic structures are carried out to define the methodology for on measurement site. The measurements are then compared to the inversion method to validate it.

Estimation d'une densité prédictive avec information additionnelle

Dans le contexte de la théorie bayésienne et de théorie de la décision, l'estimation d'une densité prédictive d'une variable aléatoire occupe une place importante. Typiquement, dans un cadre paramétrique, il y a présence d’information additionnelle pouvant être interprétée sous forme d’une contrainte. Cette thèse porte sur des stratégies et des améliorations, tenant compte de l’information additionnelle, pour obtenir des densités prédictives efficaces et parfois plus performantes que d’autres données dans la littérature. Les résultats s’appliquent pour des modèles avec données gaussiennes avec ou sans une variance connue. Nous décrivons des densités prédictives bayésiennes pour les coûts Kullback-Leibler, Hellinger, Kullback-Leibler inversé, ainsi que pour des coûts du type $\alpha-$divergence et établissons des liens avec les familles de lois de probabilité du type \textit{skew--normal}. Nous obtenons des résultats de dominance faisant intervenir plusieurs techniques, dont l’expansion de la variance, les fonctions de coût duaux en estimation ponctuelle, l’estimation sous contraintes et l’estimation de Stein. Enfin, nous obtenons un résultat général pour l’estimation bayésienne d’un rapport de deux densités provenant de familles exponentielles. / Abstract: In the context of Bayesian theory and decision theory, the estimation of a predictive density of a random variable represents an important and challenging problem. Typically, in a parametric framework, usually there exists some additional information that can be interpreted as constraints. This thesis deals with strategies and improvements that take into account the additional information, in order to obtain effective and sometimes better performing predictive densities than others in the literature. The results apply to normal models with a known or unknown variance. We describe Bayesian predictive densities for Kullback--Leibler, Hellinger, reverse Kullback-Leibler losses as well as for α--divergence losses and establish links with skew--normal densities. We obtain dominance results using several techniques, including expansion of variance, dual loss functions in point estimation, restricted parameter space estimation, and Stein estimation. Finally, we obtain a general result for the Bayesian estimator of a ratio of two exponential family densities.

New Foundations for Ecological Economics

Ecological economics has been repeatedly described as transdisciplinary and open to including everything from positivism to relativism. I argue for a revision and rejection of this position in favour of realism and reasoned critique. Looking into the ontological presuppositions and considering an epistemology appropriate for ecological economics to meaningfully exist requires rejecting the form of methodological pluralism which has been advocated since the start of this journal. This means being clear about the differences in our worldview (or paradigm) from others and being aware of the substantive failures of orthodox economics in addressing reality. This paper argues for a fundamental review of the basis upon which ecological economics has been founded and in so doing seeks improved clarity as to the competing and complementary epistemologies and methodologies. In part this requires establishing serious interdisciplinary research to replace superficial transdisciplinary rhetoric. The argument places the future of ecological economics firmly amongst heterodox economic schools of thought and in ideological opposition to those supporting the existing institutional structures perpetuating a false reality of the world's social, environmental and economic systems and their operation.

Le but de cette thèse est d'étudier les trois problèmes suivantes : 1) On s'intéresse à la stabilité des cônes normaux et des sous-différentiels via deux types de convergence d'ensembles et de fonctions : La convergence au sens de Mosco et celle d'Attouch-Wets. Les résultats obtenus peuvent être vus comme une extension du théorème d'Attouch aux fonctions non nécessairement convexes sur des espaces de Banach localement uniformément convexes. 2) Pour une bornologie β donnée sur un espace de Banach X, on étudie la validité de la formule suivante (…). Ici Tβ(C; x) et Tc(C; x) désignent le β -cône tangent et le cône tangent de Clarke à C en x. On montre que si, X x X est ∂β-« trusted » alors cette formule est valable pour tout ensemble fermé non vide C ⊂ X et x ∈ C. Cette classe d'espaces contient les espaces ayant une norme équivalent β-différentiable, etplus généralement les espaces possédant une fonction "bosse" lipschitzienne et β-différentiable). On montre aussi que la propriété Kadec-Klee n'est plus exigée lorsque l'ensemble de Chebyshev est représenté comme une union d'ensembles convexes fermés. / The aim of this thesis is to study the following three problems: 1) We are concerned with the behavior of normal cones and subdifferentials with respect to two types of convergence of sets and functions: Mosco and Attouch-Wets convergences. Our analysis is devoted to proximal, Fréchet, and Mordukhovich limiting normal cones and subdifferentials. The results obtained can be seen as extensions of Attouch theorem to the context of non-convex functions on locally uniformly convex Banach space. 2) For a given bornology β on a Banach space X we are interested in the validity of the following "lim inf" formula (…).Here Tβ(C; x) and Tc(C; x) denote the β-tangent cone and the Clarke tangent cone to C at x. We proved that it holds true for every closed set C ⊂ X and any x ∈ C, provided that the space X x X is ∂β-trusted. The trustworthiness includes spaces with an equivalent β-differentiable norm or more generally with a Lipschitz β-differentiable bump function. As a consequence, we show that for the Fréchet bornology, this "lim inf" formula characterizes in fact the Asplund property of X. 3) We investigate the convexity of Chebyshev sets. It is well known that in a smooth reflexive Banach space with the Kadec-Klee property every weakly closed Chebyshev subset is convex. We prove that the condition of the weak closedness can be replaced by the local weak closedness, that is, for any x ∈ C there is ∈ > 0 such that C ∩ B(x, ε) is weakly closed. We also prove that the Kadec-Klee property is not required when the Chebyshev set is represented by a finite union of closed convex sets.

Identifying mixtures of mixtures using Bayesian estimation

The use of a finite mixture of normal distributions in model-based clustering allows to capture non-Gaussian data clusters. However, identifying the clusters from the normal components is challenging and in general either achieved by imposing constraints on the model or by using post-processing procedures. Within the Bayesian framework we propose a different approach based on sparse finite mixtures to achieve identifiability. We specify a hierarchical prior where the hyperparameters are carefully selected such that they are reflective of the cluster structure aimed at. In addition, this prior allows to estimate the model using standard MCMC sampling methods. In combination with a post-processing approach which resolves the label switching issue and results in an identified model, our approach allows to simultaneously (1) determine the number of clusters, (2) flexibly approximate the cluster distributions in a semi-parametric way using finite mixtures of normals and (3) identify cluster-specific parameters and classify observations. The proposed approach is illustrated in two simulation studies and on benchmark data sets.

Finite Difference and Discontinuous Galerkin Methods for Wave Equations

Wave propagation problems can be modeled by partial differential equations. In this thesis, we study wave propagation in fluids and in solids, modeled by the acoustic wave equation and the elastic wave equation, respectively. In real-world applications, waves often propagate in heterogeneous media with complex geometries, which makes it impossible to derive exact solutions to the governing equations. Alternatively, we seek approximated solutions by constructing numerical methods and implementing on modern computers. An efficient numerical method produces accurate approximations at low computational cost. There are many choices of numerical methods for solving partial differential equations. Which method is more efficient than the others depends on the particular problem we consider. In this thesis, we study two numerical methods: the finite difference method and the discontinuous Galerkin method. The finite difference method is conceptually simple and easy to implement, but has difficulties in handling complex geometries of the computational domain. We construct high order finite difference methods for wave propagation in heterogeneous media with complex geometries. In addition, we derive error estimates to a class of finite difference operators applied to the acoustic wave equation. The discontinuous Galerkin method is flexible with complex geometries. Moreover, the discontinuous nature between elements makes the method suitable for multiphysics problems. We use an energy based discontinuous Galerkin method to solve a coupled acoustic-elastic problem.

The subject of the diploma thesis is value added tax in tourism. Certain subjects that do business in tourism are obliged to apply a special travel service scheme for services provided according to the Act No. 235/2004 on Value Added Tax. In comparison with normal rules, this scheme simplifies the rules of application of the value added tax, though it is not applicable in all situations. The major aim of this thesis is to make a complete outline of the rules related to the application of value added tax in tourism and present suitable practical examples. Another aim of the thesis is to analyze the difference between the legislation of the special scheme according to the Act on Value Added Tax and according to the European Directive 2006/112/EC on the common system of value added tax. The theoretical focus is supplemented by practical examples with some completed tax returns for better understanding of the application of value added tax in tourism.
