Vocally disruptive behaviour in severely demented patients in relation to institutional care provided

Rahm Hallberg, Ingalill January 1990 (has links)
Thirty-seven patients identified as vocally disruptive and a control group, selected from 264 patients at psychogeriatric wards were studied. Tape-recordings of their vocal behaviour, ratings of their functional performance and symptoms related to dementia, observations of their behaviour, activities and interaction with caregivers were performed. Seventeen plus sixteen caregivers were interviewed to give their interpretation of the experience behind the behaviour and describe their own experience of it. The severely demented vocally disruptive patients were found to be significantly more physically dependant, disorientated at the ward and prone to confusional reactions but they had a more preserved speech performance than the controls. The vocal activity expressed such as helplessness, pain, fear and protest. Some were emotionally indifferent and a few expressed positive emotions. Caregivers interpreted the behaviour as an expression of anxiety related to such as abandonment, dissolution and loss of autonomy. They also expressed a strong wish to comfort the patients but felt unable to do so. The patients' daily life was characterized by idleness and solitude. Caring activities and interactions were dominated by physical procedures performed in a fragmentary and rapid way. Two hypotheses are generated. 1. Vocally disruptive behaviour develops influenced by sensory deprivation and the brain damage. 2. The care provided is influenced by caregivers experiencing anxiety in the patients as well as experiencing a conflict between the care they would like to provide and the care they actually provide. This evokes anxiety in them which elicits defence mechanisms leading to emotional withdrawal from the patients and task oriented care. The results are discussed in a nursing perspective based on existential thoughts, psychoanalytic and psychosocial theory. / <p>S. 1-48: sammanfattning, s. 49-164: 6 uppsatser</p> / digitalisering@umu

Insertion of peripheral intravenous catheters – A complex act including nursing care and patient safety / Insättning av perifer venkateter - En komplex uppgift som inkulderar omvårdnad och patientsäkerhet

Westergren, Emma, Andersson, Matilda January 2015 (has links)
Background: Insertion of a peripheral intravenous catheter (PVC) is a common procedure performed by nurses. The practical skill is a complex act, which not only requires theoretical and practical knowledge, but also nursing care adjusted to each patient's history and needs. Aim: The aim was to explore the procedure of inserting a peripheral intravenous catheter at a local hospital in Tanzania with focus on nursing care and patient safety. Method: The study was performed as a non-participating observational study and was preceded by a pilot study performed in Sweden. Eight observations were made, which were analysed with a qualitative content analysis. Result: The categories “Execution” and “Respecting patients” with associated sub-categories constituted the results. “Execution” describes how the procedure is performed, while “Respecting patients,” explains the performed nursing care during the practical skill. Conclusion: The Model of Practical Skill Performance was used for interpreting and discussing the result. The model clarified that some components were not fulfilled and the performance can therefore not be considered as well proceeded. One can discuss whether it depends on lack of knowledge, resources and/or culture. Keywords: Peripheral intravenous catheter, nursing care, patient safety, Tanzania, low-income country.

An investigation into factors influencing the quality of nursing care in district hospitals in the West Coast Winelands region of the Western Cape

Eygelaar, Johanna Elizabeth 03 1900 (has links)
Thesis (MCur (Nursing Science))--University of Stellenbosch, 2009. / ENGLISH ABSTRACT: Every patient comes to a hospital with the expectation of getting quality care. It is not always within the ability of nursing personnel to give quality care in the work situation. Guided by the research question “What are the factors influencing the quality of nursing care in district hospitals in the West Coast Winelands Region of the Western Cape?” a scientific investigation was undertaken. The goal of this study was to identify the factors which influence the quality of nursing care in the eight (8) district hospitals of the West Coast Winelands Region of the Western Cape. The objectives set for the study were:  to determine whether staffing is adequate for all activities;  to evaluate what the perceptions of the nursing staff is about their current working situation;  to determine what the effect of the absence of full time doctors are on the management of patient care;  to evaluate whether adequate equipment is available for the execution of nursing care and to evaluate whether adequate provisions for the execution of nursing care is done. A descriptive non-experimental design with a quantitative approach was applied. The population for this study was all the nursing staff available at the time of data collection, working in the eight district hospitals of the West Coast Winelands Region. A structured questionnaire was used to collect the data. The final sample of nursing staff was N= 280 of a total population of 340 – all the members were invited to participate. Reliability and validity were assured by means of a pilot study and the use of experts in nursing research, methodology and statistics. Data were collected personally by the researcher. Ethical approval was obtained from Stellenbosch University and various health authorities. Informed written consent was obtained from the participants. The data was analyzed with the support of the statistician; it was expressed in frequencies, tables and histograms. Comparisons between variables were made using either ANOVA (Analysis of variance) techniques or cross-tabulations with the Chi-square test. The 95% confidence interval was applied to determine whether there was an association between the various variables. The analysis shows that participants of the separate wards hospitals N=142 (90%) and the mixed wards type hospitals N=113 (95%) disagree that staff provision (numbers) is adequate. From the analysis it is clear that the patient documentation is not up to standard. A statistical significant correlation between hospital type and adequate time for the completion of written records (Chisquare Test p=0.00) was shown. Management N=13 (100%), registered nurses N=80 (86%), enrolled nurses N=63 (86%) and nursing assistants N= 81 (83%) disagree that it is not necessary to act beyond their scope of practice. The following recommendations were made: Safe staffing levels have to be determined; qualify staff with the necessary skills; where unit managers are still lacking, they have to be appointed; to make personnel development possible for staff; continuous auditing of patient documentation. It is necessary that there is always adequate equipment and consumables. More training is necessary for the effective and efficient implementation of the Batho Pele principles.

Assessing the nurse's knowledge and opinions regarding the management of persons with physical disabilities in two healthcare settings in Kimberley

Snyman, Johanna Hendrina 03 1900 (has links)
Thesis (MCur)- -University of Stellenbosch, 2011 / ENGLISH ABSTRACT: It generally is a problem for persons with disabilities to have barrier free access to buildings, which is their constitutional right. It is however, not only the physical facility that causes barriers for persons with disabilities but also the attitudes of the able-bodied persons. The aim of the study was to explore what opinions the nurses and persons with disabilities held in two healthcare settings in Kimberley with reference to the nursing care provided to persons with disabilities. To the researcher, it was important to gather the opinions of the nursing staff on how they saw persons with disabilities, but also to hear what they thought the problem areas in caring for persons with disabilities were. On the other hand, it was equally important to understand the persons with disabilities’ perceptions of the hospitals, nursing and what they saw as solutions to the problems. The objectives of the study were: • To determine what the opinions and knowledge of nurses working in two healthcare services in Kimberley are regarding the nursing management of persons with disabilities. • To determine what the opinions of persons with disabilities are in healthcare settings in Kimberley, with reference to the nursing care provided for persons with disabilities. Data was collected in two phases namely Phase 1 amongst the nursing staff in the private and state hospital in Kimberley and Phase 2 amongst the persons with disabilities. A list of staff members which was obtained from the Human Resource office in both the private and state hospitals were sent to the statistician Prof Kidd and who prepared a randomised list which was used for the participants in the study. The same process was followed when a list of all the people who are members of the Association for Persons with Disabilities (APD) and with the help of the statistician a randomised list was compiled from which the participants in the study were chosen. The design of this research is an explorative, descriptive non-experimental study with a quantitative approach, utilizing a structured questionnaire with closed and open ended questions. In this study it was found that the nursing staff was aware of the problems and barriers persons with disabilities encounter. The nurses, however, were aware of their own shortcomings. For example, the nurses identified the lack of training to equip them to assist the persons with disabilities during nursing care, while persons with disabilities also saw this as a problem. By addressing this shortcoming, nurses would be able to provide more holistic care. Recommendations were made based on the findings regarding the facility, perceptions, caregivers, procedures, doctors and the training of the nurses. Data is in twee fases versamel.

Patientens upplevelse av delaktighet i omvårdnad : En litteraturöversikt / The patient’s experience of participation in nursing care

Börjes-Andersson, Erica, Fjärdsmans, Ida January 2017 (has links)
Background: According to Swedish law, patients have a legal right to be involved in their care. This also means that the nurse has a obligation towards the patient to make her/him involved. Despite this right, patients feels that they are not given the opportunity to be involved in their care. Aim: The aim of this study is to describe patient participation and nurses opportunity to promote and prevent the patient’s experience of participation. Method: The study is a literature review whose results are based on ten scientific articles published between the years 2005-2016. Results: The patient believes it is important that the nurse give the opportunity towards the patient to express themselves and be informed about the care situation. Through this the patient was given the opportunity to be involved. Patients want to be seen and heard, and seen as an equal partner in care. This requires an established relationship between nurse and patient. Conclusion: Studies show that patients find that there are obstacles to maintain patient participation and that the nurse’s approach is relevant to the patient’s experience of participation.

Bali, Sara, Holm, Emma January 2017 (has links)
Background: More than a third of the world's women are at some time during their life victim of violence by their partner, or victim of sexual violence by other person than their partner. Previous research show that women that are victims of violence experience both positive and negative treatment from healthcare professionals.  Aim: The aim was to describe the barriers and facilitating factors nurses experience to carry through nursing actions when meeting female victims of intimate partner violence.  Method: A literature study of 11 qualitative articles, those were reviewed and analyzed.  Results: Four categories emerged from the analysis: level of knowledge among nurses, the work environment and organisation, the impact of society, nurses own feelings and assumptions. During encounters with female victims of intimate partner violence, nurses experienced both barriers and facilitating factors when performing nursing actions. They are described under the main headlines in the results. Barrier factors were; lack of knowledge, heavy workload, lack of time, lack of support from employer culture in society, language barriers, own feelings and assumptions, presence of the partner. Facilitating factors were; education, knowledge, work experience, having enough time, increased personnel, support from colleagues, good cooperation with other instances. Other facilitating factors were opportunity for meetings in private, more attention in society and education in schools, using an independent interpreter when encountering language barriers, establishing a good relationship with the woman, and the ability to regulate and manage emotions.  Conclusion: Several factors for which nursing measures that are provided by nurses has meaning when meeting with the abused women. Knowledge, experience, time, care system, cultural aspects and attitude affect what nursing measures are provided or not provided when meeting with the abused women.

Larsson, Malin, Nyström, Fabian January 2017 (has links)
Background: Patients treated in surgical wards require self-care information at discharge. They demand information that suits their needs and express a lack of essential information. Swedish regulation states that patients have the right to get individualized information and to be able to participate in their care. Due to the large volume of patients in surgical wards and shortened length of stay, patients may be required to handle more aspects of self-care at home. Aim: The aim is to understand how surgical patients feel their information needs have been met, in order for them to confidently take care of themselves after discharge. Method: Qualitative interview study with a convenience sample of eight patients treated in surgical wards, analyzed using systematic text condensation. Findings: Three themes emerged which explained how patients felt their information needs were met. Patients describe a sense of anxiety and fear at home due to lack of information concerning recovery after surgery. A lack of knowledge after discharge led to insecurities related to self-care. Adapted information and possibilities to ask questions were perceived as important. Conclusion: Patients treated in surgical wards perceive discharge information as well-adapted and useful. However, it is perceived as not being extensive enough. A lack of essential information, coupled with a lack of knowledge led to feelings of anxiety and fear. With these results in mind, it is important to individualize the information given to patients at discharge, and to improve the patient participation during this process. This is an area that registered nurses, specialized in surgical care, have potential to improve.
