Devenir à long terme de couples traités par fécondation in vitro dans la cohorte DAIFI / Long-term outcome of couples treated by in vitro fertilization in the DAIFI cohort

Les études sur les couples traités par fécondation in vitro (FIV) ont jusqu’à présent porté essentiellement sur l’évaluation du succès en FIV. Très peu de données sont disponibles sur le devenir à long terme de couples traités par FIV. L’objectif de ce travail était d’estimer la fréquence de réalisation du projet parental à long terme, et d’étudier les facteurs associés aux interruptions précoces des traitements et aux naissances naturelles.L’enquête DAIFI-2009 a inclus 6 507 couples ayant débuté un programme de FIV en 2000-2002 dans l’un des 8 centres de FIV participant à l’étude. Les données médicales des couples et leur parcours dans le centre ont été obtenus à partir des dossiers médicaux des centres de FIV pour tous les couples. L’information sur le devenir des couples après le départ du centre a été obtenue par questionnaire postal auprès des couples en 2008-2009 (38% de participation 7 à 9 ans après l’initiation des FIV). L’étude des facteurs associés à la participation à l’enquête postale suggérait que la fréquence de réalisation du projet parental estimée sur les répondants seulement pourrait être biaisée. Les différentes méthodes mises en œuvre pour corriger la non réponse (pondération, imputation multiple) n’ont pas modifié l’estimation de la fréquence de réalisation du projet parental. Au total, 7 à 9 ans après l’initiation des FIV, 60% des couples ont réalisé leur projet parental de façon biologique, suite à un traitement ou suite à une conception naturelle. Lorsque les adoptions sont aussi prises en compte, 71% des couples ont réalisé leur projet parental. Après l’échec d’une première tentative de FIV, un couple sur 4 (26%) a interrompu les FIV dans le centre d’inclusion. Globalement, les couples avec de mauvais facteurs pronostiques ont un plus grand risque d’interrompre les FIV. Cependant, la proportion plus importante d’interruption parmi les couples avec une origine inexpliquée de l’infécondité pourrait s’expliquer par la survenue plus fréquente de naissance naturelle dans ce sous-groupe de couples. Parmi les couples n’ayant pas eu d’enfant suite aux traitements, 24% ont ensuite conçu naturellement en médiane 28 mois après l’initiation des FIV. Parmi les couples ayant eu un enfant suite aux traitements, 17% ont ensuite conçu naturellement en médiane 33 mois après la naissance de l’enfant conçu par AMP. Les facteurs associés aux naissances naturelles sont des indicateurs d’un meilleur pronostic de fertilité, particulièrement chez les couples sans enfant AMP.L’enquête DAIFI-2009 a permis d’apporter des informations sur le parcours à long terme des couples traités par FIV qui n’avait jusqu’à présent été que peu étudié, souvent sur de faibles effectifs et avec un suivi plus court. Ces résultats doivent apporter de l’espoir aux couples inféconds, puisque la majorité d’entre eux ont finalement réalisé leur projet parental, même si cela peut prendre de nombreuses années. / Until now, most studies of couples treated by in vitro fertilization (IVF) have been centered on IVF success. Very few data are available on the long-term outcome of these couples, including spontaneous conception and adoptions. This work aimed to estimate the long-term cumulative parenthood rate, and to study factors associated with early IVF discontinuation and with spontaneous live births.The DAIFI study is a retrospective cohort including 6,507 couples who began IVF in 2000-2002 in one of the eight participating French IVF centres. Medical data on all couples were obtained from centre databases. Information on long-term outcome after leaving the IVF center was collected by postal questionnaire sent to couples in 2008-2010 (7 to 9 years after IVF initiation, participation rate 38%). Study of factors associated with participation in the postal survey suggested that the cumulative parenthood rate estimated only in participants might be biased. The different methods used to correct for non-response bias (inverse probability weighting, multiple imputation) did not modify the estimation of the cumulative parenthood rate obtained with the complete case approach. Finally, 7 to 9 years after IVF initiation, the cumulative parenthood rate was estimated at 60%, including live births following IVF, other treatment or spontaneous conception. When adoptions were also considered, the cumulative parenthood rate reached 71%. After a first failed IVF cycle, just over one couple out of four (26%) discontinued IVF treatment. Globally, couples with poor prognostic factors had a higher risk of early discontinuation of IVF treatment. However, the higher proportion of early discontinuation observed among couples with unexplained infertility could be linked to a higher chance of spontaneous pregnancy in this subpopulation. Among couples who remained childless after treatment, 24% later had a spontaneous live birth (SLB), at a median of 28 months after the first IVF attempt. Among couples who had had a child during medical treatment, 17% later had an SLB, at a median of 33 months after the birth following medical treatment. Regarding factors associated with SLB, they can be viewed as indicators of a better fertility prognosis, especially among unsuccessfully treated couples.The DAIFI study has provided information on the long-term outcome of couples treated by IVF, which has until now been little studied, often on small samples and with a shorter duration of follow-up. These results should give hope to infertile couples as nearly three couples out of four finally became parents, even if it may take many years.

Notre recherche concerne les conditions d’acceptabilité de la parentalité dans deuxcirconstances : chez les personnes présentant des difficultés d’apprentissage qui souhaitent avoir un enfant et dans des conditions de procréation médicalement assistée post-mortem. Notre recherche est basée sur la Théorie Fonctionnelle de la cognition de Norman Anderson (1981). Concernant l’étude portant sur le désir de devenir parent chez des personnes atteintes d’un handicap mental : 304 participants ont jugé du degré d’acceptabilité de chacun des 60 scénarios proposés, résultant de la combinaison de cinq facteurs : « degré de retard mental », « entourage familial », « stabilité de la relation », « partenaire » et « attitude des parents ». Pour l’étude portant sur la parentalité en faisant appel à une procédure de procréation médicalement assistée post-mortem : 166 participants ont jugé du degré d’acceptabilité des 48 scénarios proposés. Ils résultent de la combinaison de quatre facteurs : « statut marital », « accord des beaux parents », « délai d’utilisation du sperme » et « souhait du conjoint ». Pour l’étude portant sur la parentalité des personnes présentant des difficultés d’apprentissage, il apparaît que les cinq facteurs manipulés jouent un rôle dans le jugement d’acceptabilité. Il s’agit par ordre croissant des facteurs : « entourage familial », « partenaire » « degré de retard mental », « stabilité de la relation» et « attitude des parents ». Concernant l’étude portant sur la parentalité dans des cas de procréation médicalement assistée postmortem, trois des quatre facteurs manipulés influencent le jugement d’acceptabilité des participants. Par ordre d’importance croissant, il s’agit des facteurs : « souhait du conjoint », « accord des beaux parents » et « statut marital ». Le facteur « délai d’utilisation du sperme » n’a pas eu d’effet significatif. / The purpose of this research is to assess the factors influencing the level of acceptability ofparenthood in two situations: people with learning disabilities who wish to have a child, and medically assisted post-mortem procreation. This study is based on the Functional Theory of Cognition of Norman Anderson (1981). Regarding the study on people with learning disabilities who wish to have child: 304 participants evaluated the acceptability of each of the 60 proposed scenarios resulting from a combination of five factors: "level of mental retardation", "family circle", "stability of the relationship", "partner" and "parents attitude". Concerning the study on parenthood via medically assisted post mortem procreation: 166 participants assessed the acceptability of the 48 proposed scenarios resulting from the combination of four factors: "marital status", "in-laws parent agreement", "sperm use period" and "wish of the spouse". In order of importance, "wish of the spouse", "in-laws parents’ agreement" and "marital status" were mostly decisive. The "sperm use period" factor had no significant effect.

Associations between parenting and children's socio-emotional well-being : the role of empathy and social understanding

The socio-emotional well-being of maltreated children is a key priority for practitioners and policy-makers alike; yet not enough is known about the developmental mechanisms that might link children's parenting experiences with their psychosocial adjustment. Previous research suggests that parental abuse and neglect can have adverse effects on children's peer relationships and self-perceptions. Emerging theoretical and empirical work suggests that children's social understanding and empathy could play a key role as mediators of these effects, but we have little knowledge about the differentiated pathways that might be uncovered by adopting a multidimensional conceptualisation of parenting experiences, social understanding and empathy, and peer reputations and self-perceptions. This thesis includes five papers reporting on a programme of empirical research conducted in order to address this gap in knowledge. The first, a meta-analysis and systematic review, revealed a complex and differentiated profile of social understanding among maltreated children. The second paper reports on a thematic analysis of a focus group and detailed semi-structured individual interviews with foster carers. In line with our theoretical model, carers readily identified children's difficulties with social understanding and empathy as relevant explanations for their socio-emotional problems. The third paper presents a new multidimensional self-report measure of empathy in children, demonstrating differentiated connections with socio-emotional functioning. The fourth paper presents comparisons between maltreated children in foster care and their classmates at school (aged 7-11 years), using measures that assess their peer reputations, self-perceptions, social understanding and empathy. Results indicate both direct links from maltreatment status to self-perceptions, as well as indirect links via children's theory of mind skills, their prosocial responses to others' emotional states and their behavioural reputations. The final paper delves deeper into the variations within the non-maltreated sample, uncovering links between specific features of the children's parenting experiences and their behavioural reputations, peer status and self-perceptions, via emotion understanding and empathic responses. Overall, the findings from this programme of research highlight the interplay of parenting experiences and socio-emotional competence in understanding school-aged children's psychosocial functioning. The results carry implications for further research as well as for applied work to support maltreated children and the adults who work with and care for them.

Cultural Patterns in Parenting

Die vorliegende Arbeit untersucht kulturelle Vorstellungen und Praktiken von Elternschaft in Deutschland. Wie Familien ihre Kinder erziehen ruft ein steigendes Interesse von Seiten der Politik, Wissenschaft und weiterer (Bildungs-)Professionen hervor. Gegenwärtig wird von Eltern erwartet, dass sie sich mit Expertenwissen auseinandersetzen, welches oftmals kostspielige und zeitaufwändige Praktiken nahelegt. Zur gleichen Zeit nehmen staatliche Akteure verstärkt Einfluss auf das familiale Leben und insbesondere die Kindererziehung. Als Folge dieser Entwicklung haben sich die kulturellen Vorstellungen von Elternschaft in Deutschland gewandelt. Mit diesen Entwicklungen beschäftigt sich die vorliegende Arbeit in drei Aufsätzen. Im Einzelnen betrachten die Artikel: (1) die Bedingungen von familienpolitischen Reformen, (2) die elterlichen Diskussionen um sich wandelnde Erwartungen sowie (3) die Auswirkung der elterlichen Klassenposition auf deren Erziehungspraktiken. Um diese Fragen zu beantworten greifen die Artikel sowohl auf Umfragedaten als auch digitale Daten zurück, die mit neuen Methoden der computergestützten Sozialforschung analysiert werden. Insgesamt erweitert die vorliegende Dissertation unser Wissen um den aktuellen Wandel von Elternschaft in Deutschland. Zudem werden innovative Methoden der computergestützten Sozialforschung in die Familiensoziologie eingeführt. Zuletzt wurde mit kulturellen Leitbildern von Elternschaft ein Zugang vorgeschlagen, der es erlaubt den gegenwärtigen Wandel von Elternschaft zu untersuchen. / The thesis at hand investigates conceptions and practices of parenthood in Germany. How families approach their childrearing has attracted growing interest in politics, science, and by (education) professionals. Today, parents are encouraged to seek expert advice to inform their childrearing. Such advice often involves the expenditure of considerable amounts of time and money. At the same time, the state has become increasingly active in the intimate life of families and, in particular, their parenting practices. As a result, the cultural models of parenthood in Germany have been changed. At the background of these changes, I designed three papers that analyze (1) the conditions of family policy change, (2) the way parents adopt changing expectations and demands, and (3) the relationship between parenting practices and social class. In order to address these questions, the respective articles use survey and digital data as well as methods from the computational social sciences (CSS). In sum, the dissertation expands our knowledge on the current recalibration of parenthood in Germany. It also introduces innovative methods from CSS to family sociology. Finally, cultural models of parenthood are proposed as a way to organize the research on changing standards and norms of parenthood.

Parmi les tiers qui gravitent autour d’un enfant, il en est un que le droit français de la famille peine à reconnaître la singularité : l’homme ou la femme qui est en couple avec le parent d’un enfant et qui, à l’égard de ce mineur, assure une prise en charge plus ou moins factuelle.Qu’il se greffe sur une famille ou qu’il la compose, ce tiers communément dénommé « beau-parent » semble désormais connu, par une partie du droit positif (jurisprudence et doctrine), sous le vocable de parent social.Il est à reprocher au droit actuel de ne pas parvenir à appréhender, par des règles autonomes, les relations personnelles unissant ce tiers à l’enfant du parent dont il est le concubin, partenaire pacsé ou conjoint, de sexe différent ou de même sexe. Certes, l’incursion de ce tiers dans le mécanisme de l’autorité parentale ne saurait s’affranchir des principes directeurs du droit la famille : la coparentalité, l’indisponibilité de ladite autorité et l’impérieux respect de l’intérêt supérieur de l’enfant. Par la présente étude, il s’agit de mettre en exergue l’inopportunité, de lege lata, de l’identification du parent social par assimilation juridique et de l’en extirper en concevant un statut sui generis . Surgirait, de lege ferenda, le beau-parent statutaire. / Among third parties gravitating around a child, French Family Law has difficulty recognizing the singularity of one of them: the one (he or she) who, being in relationship with a child’s parent, provides care for the latter to a certain extent.Whether he or she is bound to a family, or is part of it, this third party commonly named “stepparent” now seems to be known by some part of the positive Law (jurisprudence and doctrine) referred to as “social parent”.One can blame French Law for not succeeding in apprehending, with autonomous rules, these relationships between the third party and the child whose parent is his concubine, having registered a civil union or spouse as a same-sex couple or opposite sex. Indeed the incursion of this third party into the mechanism of parental authority should in no way free itself from the guiding principles of Family Law: joint parenthood /co-parenting; the unavailability of the so-called authority and the upmost respect of the best interest of the child. This study is aimed at highlighting the impropriety, of lege lata, of the identification of the social parent by legal assimilation removing it, thus conceiving a sui generis status. Therefore it might occur from lege ferenda, the statutory stepparent.

”Hon gör sitt bästa efter sin förmåga” : en juridisk studie av LVU-domar med barn tillföräldrar med utvecklingsstörning

<p>The purpose of this paper is to investigate on what basis a child is committed into care according to the law and to see on which grounds the decision about committed child care in law practice are taken when a child is committed to care due to parents who are mentally retarded. To better understand the juridical grounds for these decisions I will also in a short background describe the meaning of the term mentally retarded, how mental retardation and parenthood has changed over time and how different opinions are expressed in the law. Both people with mental retardation and children have in recent years gained their rights and sometimes these rights end up in conflict with one another. In those cases, what is in the best interest of the child, should be decisive. The children who have mentally retarded parents are at risk to not have their physical, psychological, emotional, social and intellectual needs met and are therefore being unfavourable developed. LVU gives possibilities for the right to intervene in the relation between child and parent through committing the child into care against the parent’s will. This law is supposed to be used in those cased when the child is being exposed to bad conditions which implies an obvious risk for the health and development of the child and when care cannot be given voluntarily. In my study it is discovered that the Supreme Court has decided cases where children with parents who are mentally retarded or have similar problems has been committed into care. The retardation in itself is not reason enough for the child to be committed into care, it is the consequences which are determining. The Supreme Court approved the Social service request concerning committing care in three of the eleven cases I have studied and they stated lack in material, social and intellectual areas, and also the immatureness of the parents and the inability to put the child’s need first. In the other cases I have studied the Supreme Court declined the requests from the Social service and motive it different in each case. They referred to the lack of necessary qualifications, that siblings has managed well, that supporting measures in the home should be sufficient etc. In my study I can clearly see that the demand for evidence about lack in care is increased by every instance. My study also show that the Supreme Court to a relatively great extent tend to decide against the Social service, experts and the opinion of public counsels. My opinion is that the perspective of the child is at risk of being put aside if the court lower their demands on good parenthood for parents with mental retardation.</p>

<p>The aim of this study was to describe how the best interest of the child can be constructed in investigations concerning permission for international adopt, where adequate parenthood is investigated. Qualitative research interviews were used as method. Seven investigators from six different sections of the city of Stockholm were interviewed. The information was analysed on the basis of a sociological approach to law and social constructionism.</p><p>The result showed that adequate parenthood is a condition in the interest of the child. Adequate parenthood is specified as the parent’s qualities and possibility to care for an adopted child. It does not depend on material qualifications, it is principally a matter of personal conditions, which can be defined as self-consciousness and the ability to reflect, feel empathy and manage a crisis. An adequate adoptive parent shall also realize the significance of an adoption. Adequate adoptive parents have a stable and lasting relationship and are surrounded by a stable social network.</p><p>The best interest of the child shall mainly be fulfilled by the qualities of the parents, in other words the adequate parenthood. The adoption has to be in the best interest of the child and thereby fulfil the child’s needs. In a family, children need to develop in their own pace and receive support whenever it is needed. In the investigation undertaken by the authorities, the interests of the child can best be looked after with an investigator that possesses adequate knowledge about children, that investigates thoroughly and feel a responsibility towards the child.</p>

<p>Syftet med denna uppsats var att undersöka hur deltagare upplever en föräldragrupps innehåll och utförande på ett familjecentrum. Studien genomfördes med en kvalitativ ansats, där åtta föräldrar intervjuades. Datamaterial bearbetades och analyserades med hjälp av en kvalitativ manifest innehållsanalys, och studiens teoretiska tolkningsram utgjordes av ett systemteoretiskt perspektiv, Putnams och Bourdieus teori om socialt kapital samt Eriksons psykosociala utvecklingsteori. Betydelsefulla resultat var att föräldragruppen upplevdes som viktig, framför allt för att etablera nya sociala kontakter, uppleva trygghet och stöd från andra nyblivna föräldrar. Det var även viktigt med en ledare med social kompetens, där lyhördhet och flexibilitet var några betydelsefulla egenskaper. Det framkom också att den information som gavs inte var mest väsentlig, utan att den sociala interaktionen mellan föräldrarna var främsta anledning till deltagandet. Att bli förälder var även något som resultatet visade vara påfrestande för både individ och parrelation.</p> / <p>The purpose of this study was to examine how participants experience the content and design, in a parent group. The study was conducted with a qualitative approach, in which eight parents were interviewed. Data material has been processed and analyzed with a qualitative manifest content analysis, and the study's theoretical framework consider primarily the perspective of systems, Putnam's and Bourdieu's theory of social capital and Erikson's psychosocial development theory. Significant results were that the parent group was perceived as important, especially to establish new social contacts, experience and safeness reliance from another first-time parent. It was also important to have a leader with social competence, where sensitivity and flexibility were significant quality. It also emerged that the information that was provided was not the most significant, instead the interaction between the parents was the main reason for participation. The results also show that becoming a parent was a draining process both to themselves and their relationship</p><p>.</p>
