Araújo Filho, Amaro Afrânio 31 July 2007 (has links)
Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / The main goal of the present study is to assess and compare the data obtained from ventilometry, espirometry and vital signs of patients who undergoing DAPE under inhalation anesthesia, and its specific goal is to verify the existence of a co-relation between anesthetic and surgical procedure lengths and pulmonary volumes measured by ventilometry and espirometry during the DAPE post-operative period. The subjects were adult patients between 43,3 ± 13,4 years of age, being 8 males and 5 females admitted at the surgery clinic of the Federal University Hospital in Aracaju, Sergipe, Brazil, with a previous diagnosis of portal hypertension. It is a prospective study, with the data collection being carried out in two phases: pre-operative and post-operative stages. Researched variables: age; gender; heart rate (HR); respiratory frequency (RF); blood pressure (BP); partial oxygen saturation (PSO2); minute volume (MV); current volume (CV); forced expiratory volume in 1 second (FEV1); forced vital capacity (FVC); tifeneau index (FEV1/ FVC); surgery length; anesthesia length. The statistical analysis was performed by the Origin 6.2. software. To compare the averages of BP, HR, PSO2, MV, CV, FEV1, FVC and FEV1/ FVC in both pre and post-operative stages the T test Dependent or Paired Student test. To analyses the variance the Fisher test was applied. For the co-relation between anesthetic and surgical procedure length and MV, CV, FEV1 and FVC in the post-operative stage the Pearson simple linear co-relation test was applied. For statistical tests, a 95% (α=0,05) significance level was utilized. Results showed that there was a significant difference between the mean values of HR, RF, BP, MV, CV, FEV1, FVC and PSO2. There was no significant difference between the mean values of FEV1/ FVC. Reversely, the co-relation between surgery length and the pulmonary values measured on day 2 of the post-operative period was significant of strong aspect when compared to FEV1 and FVC, and suggested a non-significant co-relation in regular aspect when compared to MV and CV. Anesthesia length showed a significant co-relation in a strong aspect when compared to FEV1 and FVC, and suggested a non-significant co-relation of weak aspect when compared to MV and of regular aspect when compared to CV. It has been concluded that patients who have undergone portal hypertension surgery present significant reduction in MV, CV, FEV1, FVC and PSO2, in addition to a significant rise in HR, BP and RF when compared to the preoperational and day 2 of post-operative period. Moreover, a significant co-relation of strong aspect was detected when anesthetic and surgical procedure lengths were compared to post-operative FEV1 and FVC values. .

Resultados a longo prazo da desconexão ázigo-portal e esplenectomia em portadores de esquistossomose hepato-esplênica: avaliação clínica, laboratorial, endoscópica e ultra-sonográfica com tempo de seguimento mínimo de 5 anos / Long term results of esophagogastric devascularization and splenectomy in schistosomotic portal hypertension: clinical, laboratorial, endoscopic and ultrasonographic evaluation with minimum 5 years of followup

Fabio Ferrari Makdissi 08 September 2009 (has links)
Esophagogastric devascularization and splenectomy (EGDS) is nowadays the most performed operation for esophageal varices bleeding recurrence prophylaxis in hepatosplenic schistosomiasis. Lower rebleeding rates are obtained through the association of EGDS with postoperative endoscopic sclerotherapy or elastic bandage of esophageal varices, however, there is a lack of studies showing long term results. The objectives of this study were to evaluate retrospectively EGDS results in patients with at least five years of follow-up. Clinical, laboratorial, endoscopic and ultrasonographic data of 97 patients submitted to EGDS from March 1989 to March 2001 were analyzed. The mean follow-up was 116.4 months. There was no postoperative clinical or laboratorial hepatic insufficiency. In the late follow-up we observed normalization of preexisting anemia, leucopenia, thrombocytopenia, hyperbilirubinemia, and a prothrombin activity time increase. There was a significant esophageal varices caliber and number reduction, cherry red spots signs and gastric varices decrease. Congestive gastropathy was observed more frequently but without clinical importance. Bleeding recurrence occurred in 24.7% of the patients, however, in 14.6% when esophageal varices hemorrhage was considered. Estimated probability of rebleeding prophylaxis over 20 years is 67.1% and 82.5% when variceal recurrence was considered. Bleeding recurrence occurred in four patients even after endoscopic evaluation showing esophageal varices eradication. There was a significant portal vein caliber reduction on late ultrasound assessment, compared to preoperative. We concluded that the EGDS with postoperative endoscopic treatment leads to good clinical results with low morbidity and mortality; provides laboratorial liver function improve and xx hypersplenism correction; determines endoscopic predictive signs of portal hypertension digestive bleeding decrease (large esophageal varices, cherry red spots signs and gastric varices), however congestive gastropathy is more frequent; allows appropriate bleeding prophylaxis in 67% of the patients over 20 years. Variceal hemorrhagic recurrence may occur even when esophageal varices eradication is reached suggesting the need of an endoscopic surveillance even in this group of patients.

Defends the production process as a definer of cyber news, starting from the initial stage of the agenda, calculation, writing, composing, editing stages to publishing and distribution of cyber news. To analyze this process is used theoreticalmethodological authors of Information Architecture (IA) in the evaluation of the journalistic product from the perspective of Organization, Labeling, Navigation and Search Systems proposed by Morville and Rosenfeld (2006), the research on the cyber news production process, emphasizing the preparation of the agenda and at the stage of formation (Schwingel, 2012), and, finally, to assess the potential use of the Internet in each of the three cyber news accompanied during the field survey (Zamith 2011). With this, there is the vital link between the production process cyber news from the specific case of portal d24am, and its constitution. The results of this research reveal that the news published on the portal d24am have few features cyber news, translated in reduced use of hypermedia narrative elements. This is because the model adopted by the integrated newsroom RDC is facing the productions of the printing press in the mold, without regard to the specifics of the production cyber news evaluated in this study.

The Brazilian public administration has been adopting regulatory practices, as well as transparency measures, to encourage social control since the Constitution of 1988. Public Transparency - which now takes place mainly through electronic portals exclusively focused on content for advertising management actions to public resources has been thoroughly studied and measured by many instances. This research aimed at devising, through thorough literature review of academic publishing in the area, an assessment model which featured elements of democracy, accountability, and transparency. The model was called TLS, for it takes theoretical, legal and social aspects into account in its assessments., A checklist parting from eight original models was obtained, with 109 assessment items divided into four dimensions, namely: content and overview; active transparency; usability and functionality and passive transparency. The model created was applied on the Goiás Transparente portal - the transparency portal of the Brazilan state of Goiás - and evaluated on a five-level scale, where the website has the third level of transparency.

It analyzes the contribution of the CAPES Journals Portal for the scientific production of the permanent faculty of the Postgraduate Programs in Pharmacy in the 2010-2012 triennium. Casuistic and methods: Prospective evaluation of 53 patients younger than 20 with chronic liver disease or EHPVO and no history of bleeding or prophylactic treatment of esophageal varices (EV). They were divided in 2 groups: group I (35 with chronic liver disease) and group II (18 with EHPVO). Their blood count, INR, albumin, bilirubin, abdominal ultrasound and upper endoscopy results were taken. A splenic index (SI) was determined by dividing the patients' spleen dimension by its uppermost limit according to their age. The variables were compared to the presence of EV, gastric varices (GV) and portal hypertensive gastropathy (PHG). The univariate (?² test, Fischer's exact test and Wilcoxon rank sum test) and multivariate (logistic regression) analysis were performed. A ROC curve was constructed and the area under the ROC was calculated. Results: EV were observed in 48,5% of group I patients and in 83,3% of group II patients. Low platelet count (p=0,0015), splenomegaly (p=0,0003), SI (p=0,0007), splenorenal shunt (p=0,0329) and lesser omental thickness (p=0,0151) were statistically predictors of EV among group I patients. The multivariate analysis showed low platelet count (odds=21,7) as an independent predictor of EV in patients with chronic liver disease. Gallbladder varices (p=0,0245) and gallbladders' wall thickness (p=0,0289) statistically predicted EV among group II patients. Platelet count (p=0369), SI (p=0,0041) and platelet/SI ratio (p=0192) were significant predictors of GV among group I patients. Low platelet count predicted GV among group II patients (p=0,0216). PHG occurred more often among group I patients who had low platelet count (p=0,0286), splenorenal shunt (p=0,0384), lower platelet/SI ratio (p=0,0369) and thicker lesser omental thickness (p=0,0226). Conclusions: Platelet count, splenic index, platelet/SI ratio, splenorrenal shunt and lesser omental thickness were indicative of EV among children and adolescents with chronic liver disease. Gallbladder varices and thicker gallbladders' wall were indicative of EV among EHPVO patients. Low platelet count, splenomegaly and lesser platelet/SI ratio were indicative of GV among group I patients. Low platelet count was indicative of GV among group II patients. Low platelet count, splenorenal shunt, bigger INR, lesser platelet/SI ratio and thicker lesser omentum were indicative of PHG among chronic liver disease patients / Doutorado / Pediatria / Doutor em Saude da Criança e do Adolescente

The educational effects of pre and post-work in out-of-school laboratories

Streller, Matthias 05 February 2016 (has links) (PDF)
Since the 1980’s, education had to face various challenges such as new technologies, new ways of information gathering but also a reconsideration of conventional educational approaches. As a result, more emphasis has been placed on laboratory work in school science. In many industry nations, this trend was likewise bolstered by unexpected poor results in international comparative assessments (e.g. PISA, TIMSS), as well as students’ poor perception of science and, in relation to that, negative effects on career choices. To combat this growing trend, in Germany many out-of-school science laboratories were established in the recent years to foster interest in science. However, despite their positive temporary effects, approaches to increase effects or to develop long term positive changes are in demand. This research investigates how the out-of-school laboratory effects are affected by a preparation and post enhancement based upon previous studies. Therefore, an online portal was developed which provided cognitive and affective content in order to prepare and post enhance students for their visit in an out-of-school laboratory. The research-based study was aimed at students from grade 10 of lower and upper secondary level who perform one-day experimental activities at the out-of-school laboratory located at the Helmholtz-Zentrum Dresden-Rossendorf research center. In doing so, a comparative analysis was conducted between students who used the online portal and control group members who just regularly visited the laboratory without a special preparation or post enhancement. The evaluation follows a pre, post, and follow-up approach. Based on the results of this research, it could be confirmed that the online portal, as a tool to prepare and post enhance students, had a significant impact. Moreso, students’ situational interest was positively promoted through the online portal. This also applied for related features, like students’ self-concept as well as their perceptions of the out-of-school laboratory environment and even slight effects on their individual interest. As it turned out regarding the desired situational interest, females benefitted most. However, again most results suggest that evoked effects diminish over time. Even though this likely can be traced back to the characteristics of the post enhancement of the online portal, outcomes regarding students’ interest in science and a career in physics indicate the post enhancement’s ability to ensure sustainability. Within the sample three classes were identified based on their interests. Accordingly, for all classes’ members the portal fosters their situational interest. This especially applies for less scientific interested students. Assessments on the portal’s perception by the students revealed a high degree of willingness to prepare for the laboratory visit and to spend the time required. A large majority appreciated the online portal for their laboratory work. An extended preparation, like presented in this study, is still regarded as acceptable to the students. Nevertheless, a compulsory preparation and post enhancement is highly recommended. On the whole, it can be concluded that the online portal respectively a preparation and post enhancement is beneficial for activities out-of-school.
