Évaluation de la densité osseuse et du statut nutritionnel en vitamine D chez des enfants prépubères avec allergie au lait non résolue

L'allergie au lait de vache (ALV) représente l'allergie alimentaire la plus fréquemment rencontrée durant l'enfance. Cette allergie a longtemps été reconnue comme transitoire mais des données récentes révèlent que celle-ci est persistante chez environ 15% des enfants qui en sont touchés durant l'enfance, posant ainsi un risque à leur santé. La présente étude examine 26 enfants avec ALV et 12 enfants contrôles recrutés au CHU Sainte-Justine durant l’hiver 2011-2012. L'objectif étant de comparer la densité minérale osseuse (DMO) et les niveaux sériques de 25(OH)D d'enfants prépubères avec ALV non résolue à un groupe contrôle d'enfants avec autres allergies alimentaires, en plus d'évaluer les apports en calcium et en vitamine D ainsi que l'adhérence à la supplémentation chez cette population. La DMO lombaire (L2-L4) ne diffère pas significativement entre les groupes. Cependant, une faible densité osseuse, caractérisée par un score-Z entre -1,0 et -2,0 pour l'âge et le sexe, est détectée chez plus de 30% des enfants avec ALV et plus de 16% du groupe contrôle, sans allergie au lait. Tel qu'attendu, les apports en calcium sont significativement moins élevés chez les enfants avec ALV comparé au groupe contrôle, avec près de 90% de tous nos participants ne rencontrant pas les besoins pour l’âge en vitamine D. Plus de la moitié des enfants avec ALV présentent une concentration de 25(OH)D inférieure à 75 nmol/L. Cependant, notre étude n'a décelé aucune différence entre les niveaux sériques de 25(OH)D des enfants avec ALV comparativement au groupe contrôle. Enfin, l'adhérence à la supplémentation est jugée adéquate chez plus de 75% de notre groupe d'enfants avec ALV, soit ≧ 4 journées par semaine, un facteur aussi associé à une meilleure atteinte de leurs apports nutritionnels en calcium et en vitamine D. Enfin, ces résultats soulignent l'importance de suivre la santé osseuse d'enfants avec ALV ainsi qu'avec allergies multiples, qui présentent un risque de faible densité osseuse. L'intervention nutritionnelle devrait suivre l'adhérence à la supplémentation chez les enfants avec ALV non résolue, afin d'optimiser les apports nutritionnels insuffisants en calcium et en vitamine D / Cow's milk allergy (CMA) is the most frequent food allergy seen during childhood. Though usually recognized as transient, recent data suggest that the evolution of CMA has changed over time, having become more persistent and therefore, more capable of jeopardizing child growth and development. The following study included 26 children with proven CMA and 12 controls, recruited at Sainte-Justine hospital, during the winter of 2011-2012. The aim of this study was to compare bone density and vitamin D status (25(OH)D) in prepubertal children with CMA to a control group of children without milk allergy and to document calcium and vitamin D intake as well as the adherence to supplementation in this population. Bone mineral density did not differ significantly between groups. However, low bone mass, as defined by a z score between -1,0 and -2,0 for gender and age, was detected in over 30% of cow's milk allergic children and in over 16% of allergic controls. Calcium intake was significantly lower in children with CMA. Overall, nearly 90% of our study population failed to meet daily recommended intakes for vitamin D, with over 50% of cow's milk allergic children presenting vitamin D status below 75 nmol/L. No difference was detected regarding vitamin D status between groups. Finally, the majority of children with CMA presented a good adherence to calcium and vitamin D supplementation, ≧ 4 days per week, a factor that was associated with a better overall dietary intake of these nutrients. It is therefore important to follow bone health of children with CMA as well as with multiple food allergies, as they present a risk for low bone mass during a period of skeletal growth. Nutrition interventions should monitor supplement adherence in children with CMA to optimize dietary intake of calcium and vitamin D, which are presently insufficient.

Profil de pratique des médecins omnipraticiens en santé mentale au Québec

Au Québec, face à la prévalence élevée des problèmes de santé mentale et à la pénurie de médecins psychiatres, le médecin omnipraticien (MO) occupe une place primordiale dans la prise en charge et le suivi des soins de santé mentale. Dans le contexte de réforme du système de santé mentale axée sur un renforcement de la collaboration entre les MO, les psychiatres et les équipes de santé mentale, notre étude vise à mieux comprendre la pratique clinique et la pratique collaborative développée par les MO, leur appréciation des outils de travail et de la qualité des services de santé mentale, dans le but d’améliorer la complémentarité des soins au niveau primaire. Cette étude transversale impliquait 1415 MO de neuf territoires de centre de santé et de services sociaux (CSSS) du Québec. L’échantillon final était constitué de 398 MO représentatifs de lieux de pratique diversifiés et le taux de réponse était de 41%. Nos résultats mettent en évidence que la pratique clinique et la pratique collaborative des MO diffère selon le degré de gravité des problèmes de santé mentale des patients rencontrés, c’est à dire, trouble transitoire/modéré de santé mentale (TTM.SM) ou trouble grave de santé mentale (TG.SM), et que les MO sont favorables au fait de travailler en collaboration avec les autres professionnels de la santé mentale. Ainsi, il apparaît important de renforcer l’accessibilité des MO aux professionnels de la santé mentale, particulièrement les psychiatres, et de les informer de l’existence des autres acteurs en santé mentale sur leur territoire, pour renforcer la collaboration et la qualité des soins primaires de santé mentale. / In Quebec, considering the high prevalence rate of mental health disorder and the scarcity of psychiatrists, general practitioners (GPs) hold an importante role in the management and the follow-up of mental health care. In the context of the ongoing mental health system reform aimed at reinforcing collaboration between GPs, psychiatrists and mental health teams, our objectives are to have a better understanding of clinical practice and collaborative practice, developed by GP and their assessment of working tools and quality of mental health services, in order to improve the complementary of primary mental health care. The study was carried out using a cross-sectional design, involving 1415 GPs from nine Quebec territories. The final sample included 398 GPs, representing diverse practice places, and the response rate was 41%. The study highlighted that GPs have different clinical and collaboration practices according to the seriousness of mental disorder patients seen, i.e. transient and moderate mental disorder (TMMD) or serious and permanent mental disorder (SPMD); and GPs strongly support care coordination efforts with all mental health professionals. Therefore, it appears important to reinforce GP accessibility to other mental health professionals, particularly psychiatrists, and to inform them on the presence of all actors in mental health in their territory, in order to develop collaboration and quality of primary mental healthcare.

Possible endocrine disruption in molluscs from the Limpopo Province / Ignatius Michael Viljoen

With parts of SA in a malaria endemic area, a preventative way of fighting malaria is with the use of pesticides such as 1,1,1–Trichloro–2,2–bis(pclorophenyl) ethane, also known as DDT. DDT is listed under the persistent organic pollutants (POPs) and considered an endocrine disruptive compound (EDC) under the Stockholm Convention. SA registered an exemption to use DDT as means to fight malaria. DDT and its isomers are, however, known EDCs. Combined with their ability to persist in the environment while not being target specific motivates further studies into possible detrimental effects. The present study aimed to establish if ED was present by comparing the male reproductive organs from snails from an area currently sprayed with DDT (for malaria control) to an area not sprayed with DDT in the Limpopo Province. A possible endpoint (the penis sheath/preputium length ratio or PSPLR) was identified for the freshwater snail Bulinus tropicus. B. tropicus and sediment samples were collected from DDT–sprayed and nonsprayed areas located close together. The snails were dissected and various morphometric parameters measured. Sediments from the sites where the snails were collected were analysed for DDT using GC–MS. Statistical analysis showed significant differences in PSPLR (and therefore possible ED) between snails from the two areas. The difference in PSPLR values was mainly due to a relatively shorter preputium for the snails from the DDT–sprayed area. Even though the sediment samples showed that DDT was present in most of the DDT–sprayed sites and not in the non–DDT sprayed sites, causality of the possible ED could not be established from this field study. This study indicated the possibility of using the PSPLR as endpoint for ED. Persistent Inefficiency in the Higher Education Sector

Evaluations of the Higher Education Sector are receiving increased attention, due to the rising expenditures and the absence of efficiency enhancing market pressure. To what extent universities are able to eliminate inefficiency is a question that has only partially been answered. This paper argues that heterogeneity among universities as well as persistent inefficiency hinder the institutions to achieve full efficiency - at least in the short run. Two standard and one novel specification of the Stochastic Frontier Analysis are applied to a new, comprehensive set of panel data to show how the standard efficiency evaluation changes when both aspects are taken into account. It is the first time that the idea of persistent inefficiency is considered in the analysis of the German Higher Education Sector. The comparison reveals that the disregard of heterogeneity distorts the estimation results towards lower efficiency values. The newly introduced specification improves the accuracy of the heterogeneity assumption and exposes that inefficiency tends to be long term and persistent rather than short term and residual. This implies that increasing efficiency requires a comprehensive change of the university structure.

De la photogrammétrie à la modélisation 3D : évaluation quantitative du risque d'éboulement rocheux / The use of photogrammetry and 3D discrete element models to better assess rock slope stability

Les analyses structurale et mécanique des pentes rocheuses constituent des éléments clés pour l'évaluation de leur stabilité. L'utilisation complémentaire de la photogrammétrie et des modèles numériques qui couplent les réseaux discrets de discontinuités (DFN selon son sigle en anglais) avec la méthode des éléments discrets (DEM selon son sigle en anglais), présente une méthodologie qui peut être utilisée pour évaluer le comportement mécanique des configurations tridimensionnelles de terrain pour lesquelles l'existence de discontinuités non persistantes peut être supposée. La stabilité des masses rocheuses est généralement supposée être contrôlée par la résistance au cisaillement le long des plans de discontinuité. Si les discontinuités sont non persistantes, avec leur continuité interrompue par la présence de ponts rocheux (portions de roche intacte reliant la masse rocheuse au massif), leur résistance apparente augmente considérablement. Dans ce cas, la contribution des ponts rocheux localisés entre ces discontinuités doit être prise en compte dans l'analyse de stabilité. La déstabilisation progressive des massifs rocheux dans lesquels des discontinuités non persistantes sont présentes, peut être étudiée par des simulations numériques réalisées à l'aide de l'approche DEM. La roche intacte est représentée comme un assemblage de particules (ou éléments discrets) liées entre elles par des contacts dont les lois de comportement spécifiques peuvent être calibrées pour représenter correctement le comportement de la roche. L'intérêt de la méthode est qu'elle permet de simuler l'initiation de la rupture et sa propagation à l'intérieur de la matrice rocheuse du fait de la rupture des contacts cohésifs entre les particules. De plus, les discontinuités préexistantes peuvent être prises en compte explicitement dans le modèle en utilisant une loi de contact ad hoc qui assure un comportement mécanique représentatif des plans de discontinuité. Des analyses de stabilité ont été effectuées et ont mis en évidence le rôle des ponts rocheux dans la génération de nouvelles surfaces de rupture qui peuvent se développer à travers des mécanismes de rupture mixte en traction et en cisaillement. On peut considérer la formulation de Jennings comme l'une des premières méthodes d'analyse de la stabilité des pentes rocheuses qui évaluent la résistance au glissement comme une combinaison pondérée des résistances mécaniques des ponts rocheux et des plans de discontinuité. Sa validité a été discutée et systématiquement comparée aux résultats obtenus à partir de simulations numériques. Il a pu être montré que la formulation de Jennings perd sa validité dès que la rupture des ponts rocheux intervient majoritairement par des mécanismes de traction. Une formulation complémentaire a alors été proposée. En ce qui concerne l'étude de la stabilité des massifs rocheux sur site, il a été montré que l'association entre les données issues de la photogrammétrie en haute résolution et l'approche DFN-DEM peut être utilisée pour identifier des scénarios de rupture. L'analyse en retour de cas réels a montré que les surfaces de rupture peuvent être simulées comme le résultat de mécanismes combinant la fracturation des ponts rocheux et le glissement le long des discontinuités préexistantes. La rupture d'un dièdre qui a eu lieu dans une mine de charbon australienne, a été utilisée pour valider cette méthodologie. Des simulations numériques ont été réalisées pour déterminer les scénarios pour lesquels les surfaces de rupture simulées et celles repérées sur le terrain, peuvent être utilisés pour calibrer les paramètres de résistance du modèle numérique. Le travail présenté ici répond à un besoin plus général visant à améliorer la gestion des risques naturels et miniers liés aux masses rocheuses instables. La méthodologie proposée constitue une alternative robuste dédiée à renforcer la fiabilité des analyses de stabilité pour les pentes rocheuses fracturées à structure complexe. / Structural and mechanical analyses of rock mass are key components for rock slope stability assessment. The complementary use of photogrammetric techniques and numerical models coupling discrete fracture networks (DFN) with the discrete element method (DEM) provides a methodology that can be applied to assess the mechanical behaviour of realistic three-dimensional (3D) configurations for which fracture persistence cannot be assumed. The stability of the rock mass is generally assumed to be controlled by the shear strength along discontinuity planes present within the slope. If the discontinuities are non–persistent with their continuity being interrupted by the presence of intact rock bridges, their apparent strength increases considerably. In this case, the contribution of the rock bridges located in-between these discontinuities have to be accounted for in the stability analysis. The progressive failure of rock slope involving non–persistent discontinuities can be numerically investigated based upon simulations performed using a DEM approach. The intact material is represented as an assembly of bonded particles interacting through dedicated contact laws that can be calibrated to properly represent the behaviour of the rock material. The advantage of the method is that it enables to simulate fracture initiation and propagation inside the rock matrix as a result of inter-particle bond breakage. In addition, pre–existing discontinuities can be explicitly included in the model by using a modified contact logic that ensures an explicit and constitutive mechanical behaviour of the discontinuity planes. Stability analyses were carried out with emphasis on the contribution of rock bridges failure through a mixed shear-tensile failure process, leading to the generation of new failure surfaces. Jennings' formulation being considered to be one of the first rock slope stability analysis that evaluates the resistance to sliding as a weighted combination of both, intact rock bridges and discontinuity planes strengths, its validity was discussed and systematically compared to results obtained from numerical simulations. We demonstrate that the validity of Jennings' formulation is limited as soon as tensile failure becomes predominant and an alternative formulation is proposed to assess the resulting equivalent strength. Regarding field slope stability, we show that the combination of high resolution photogrammetric data and DFN-DEM modelling can be used to identify valid model scenarios of unstable wedges and blocks daylighting at the surface of both natural and engineered rock slopes. Back analysis of a real case study confirmed that failure surfaces can be simulated as a result of both fracture propagation across rock bridges and sliding along pre-existing discontinuities. An identified wedge failure that occurred in an Australian coal mine was used to validate the methodology. Numerical simulations were undertaken to determine in what scenarios the measured and predicted failure surfaces can be used to calibrate strength parameters in the model. The work presented here is part of a more global need to improve natural and mining hazards management related to unstable rock masses. We believe that the proposed methodology can strengthen the basis for a more comprehensive stability analysis of complex fractured rock slopes.

Fossilization : a case study of an adult learner

Linguistic fossilization is a prevalent phenomenon in adult ESLA and presents a perpetual pedagogical challenge to teachers. Despite controversy about the theoretical concept, research is increasingly showing that persistent erroneousness cannot be attributed to single causal factors. This single case study examines controversial aspects surrounding the concept and formulates criteria for identifying fossilization. The study investigates the conversational output of an independent adult learner over a period of nine months and presents a holistic exploration of causal influences. The findings substantiate that fossilization arises from changing combinations of factors, and that such combinations are unique to the situation of each adult learner. The key to the successful treatment of fossilized errors may lie in identifying their roots, which can be achieved by analyzing output and through discussion with learners in order to gain insight into their experience of the learning process. BIFENILOS POLICLORADOS (PCBs) EM PESCADOS IN NATURA DO LITORAL DO RIO GRANDE DO SUL, BRASIL / POLYCHLORINATED BIPHENYLS (PCBs) IN FISH IN NATURA FROM THE COAST OF RIO GRANDE DO SUL, BRAZIL

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Polychlorinated biphenyls (PCBs) are toxic, non-biodegradable, and lipophilic chemical compounds of industrial origin. In Brazil, PCBs are still used in old electrical transformers and capacitors which were installed up to 1981 when their use and commercialization were banned. However, the permission to use devices with PCBs installed up to the early 80s was maintained up to the moment that their dielectric fluids or the entire device be fully replaced for products free of PCBs. Currently, PCBs are classified as persistent organic pollutants (POPs) because they pollute and accumulate in the environment affecting all organisms in the food chain. The main form of human intake is the ingestion of contaminated food, especially food of animal origin. Fish are the main means of transmission of PCB congeners to the human body. The concern about PCBs is centered on their toxicological potential, especially as inducers of cancer. Thus, the aim of this study was to investigate the possible contamination by residues of 11 individual PCB congeners in fresh marine fish from the state of Rio Grande do Sul (RS), Brazil. The methodology for the analysis of PCBs in fish included sampling on the coast of RS, the extraction and purification of these compounds by the use of acidified silica gel and solvents, as well as their identification and quantification using gas chromatography coupled to a micro electron capture detector (GC - μECD). Results were confirmed by gas chromatography coupled to mass spectrometry (GC - MS). PCBs investigated were not found in all samples. PCBs 28 and 52 were not detected. PCBs 153, 138, 180, 118, 101, 77, and 81 were found in 98.33 %, 95%, 93.33%, 60%, 33.33 %, and 31.67 % of the samples, respectively. PCBs 126 and 169 were found at lower frequencies, 18.33% and 15%, respectively. The individual congeners found in higher mean concentrations were PCB 153 (184.36 ng/g fat), followed by PCB 138 (159.27 ng/g fat), PCB 118 (98.16 ng/g fat), PCB 180 (69.04 ng/g fat), PCB 101 (32.99 ng/g fat), and PCB 77 (12.95 ng/g fat). PCBs 81, 126 and 169 were found in lower concentrations (4.97 ng/g, 4.49 ng/g and 2.08 ng/g fat, respectively). Results confirm the contamination by PCBs of all marine species of consumption analyzed and can be used as indicators of environmental contamination by these compounds in the coast of RS. / Bifenilos policlorados (PCBs) são compostos químicos de origem industrial, tóxicos, não biodegradáveis, lipofílicos, e que foram produzidos a partir da década de 30 até os anos 70, quando foram proibidos. No Brasil, PCBs estão presentes, por exemplo, em transformadores e capacitores elétricos instalados até 1981, quando a comercialização e utilização desses compostos foram proibidas no país. Porém, manteve-se a permissão de utilização desses equipamentos contendo PCBs e instalados até o início dos anos 80, até que ocorra a substituição integral dos mesmos ou a troca de seus respectivos fluidos dielétricos por produtos isentos de PCBs. Atualmente, PCBs são classificados como poluentes orgânicos persistentes (POPs), pois poluem e se acumulam no meio ambiente, influenciando todos os organismos da cadeia alimentar. A principal forma de contaminação humana é a ingestão de alimentos contaminados, principalmente alimentos de origem animal. Dentre esses, os pescados constituem-se no principal meio de veiculação de congêneres de PCBs até o organismo humano. A preocupação acerca dos PCBs está centrada no seu potencial toxicológico, principalmente como indutores de câncer. Diante disso, o objetivo desse trabalho foi investigar a possível contaminação, por resíduos de 11 congêneres individuais de PCBs, em pescados marinhos in natura do estado do Rio Grande do Sul, Brasil. A metodologia empregada para análise de PCBs nos pescados incluiu a coleta de amostras do litoral do RS, a extração e purificação desses compostos com a utilização de sílica gel acidificada, solventes e, finalmente, a identificação e quantificação dos mesmos utilizando-se cromatografia gasosa acoplada a um micro detector de captura de elétrons (GC-μECD). Os resultados obtidos foram confirmados por cromatógrafo gasoso acoplado à espectrometria de massas (GC-MS). Os PCBs investigados não foram encontrados em todas as amostras analisadas. Os PCBs 28 e 52 não foram detectados. O PCB 153 foi encontrado em 98,33% das amostras analisadas, seguido pelos PCBs 138 e 180 encontrados em 95% das amostras, pelo PCB 118 encontrado em 93,33% das amostras, pelo PCB 101 encontrado em 60% das amostras, pelo PCB 77 e PCB 81, encontrados em 33,33% e 31, 67% das amostras, respectivamente. Os PCBs 126 e 169 foram encontrados com menores freqüências, 18,33% e 15%, respectivamente. Os congêneres individuais encontrados em maiores concentrações médias foram o PCB 153 (184,36 ng/g de gordura), seguido pelo PCB 138 (159,27 ng/g de gordura), PCB 118 (98,16 ng/g de gordura), PCB 180 (69,04 ng/g de gordura), PCB 101 (32,99 ng/g de gordura) e PCB 77 (12,95 ng/g de gordura). Os PCBs 81, 126 e 169 foram encontrados nas menores concentrações (4,97 ng/g, 4,49 ng/g e 2,08 ng/g de gordura, respectivamente). Os resultados obtidos confirmam a contaminação por PCBs de todas as espécies marinhas de consumo analisadas e podem ser utilizados como indicativos da contaminação ambiental, por esses compostos, no litoral do RS.
