Interpreting The Denizens of The Hundred Acre Wood : Freudian & Lacanian psychoanalytical concepts in Winnie-The-Pooh / Psykoanalytiska koncept i Nalle Puh : En tolkning av Sjumilaskogens invånare

<p>In this paper I have strived to provide a new view on a timeless classic of children’s literature, Winnie-The-Pooh. In psychoanalytic literary criticism concepts and theories of psychoanalysis is implemented while interpreting literature; in this paper, I have interpreted the novel incorporating concepts of the psychoanalytic schools of Sigmund Freud and Jacques Lacan while arguing that the denizens of the Hundred Acre Wood are manifestations of parts of the narrator’s unconscious. The first two sections of the paper present the theories and concepts of the two major schools of psychoanalysis as an introduction aimed at increasing the readability of the interpretation. The individual interpretations of each character are then presented separately, every section in some way involving psychoanalytic theory. Kanga, Roo, Piglet, Winnie-the-Pooh, Christopher Robin, Rabbit, Owl and Eeyore are shown to be repressed memories, feelings or thoughts. Included theoretical concepts are the Oedipus complex, the sexual development of infants, the journey of children towards consciousness, Lacanian desire and lack, Freudian dream interpretation and the conception that the unconscious is structured as language, among others.</p>

Nurses’ experiences of guideline implementation in primary health care settings

Thesis (DPhil (Psychology))—University of Stellenbosch, 2010. / ENGLISH ABSTRACT: This dissertation examines how nurses in primary health care in South Africa make use of guidelines. Primary level health care is reliant primarily on nurses, who are under-resourced and often overwhelmed by the complex needs of their clients in the context of the TB and HIV/AIDS epidemic. Despite various continuing education strategies to promote current and evidence-based practice, there are many barriers to providing optimal care. Clinical practice guidelines using best evidence are an important tool for updating health professionals in current practice, particularly at primary care level, where busy practitioners often do not have time or sufficient access to the best evidence. Despite this, we know little of the practitioners’ experiences of guideline use. This study describes experiences of nurses in implementing clinical practice guidelines in the delivery of health care in selected primary level contexts in the Free State Province. The primary research question for this study was “What are the experiences of nurses in using guidelines in primary health care facilities?” A qualitative research approach, drawing on a psychoanalytic framework, was adopted. Three linked studies were conducted, utilising secondary data analysis of transcripts collected during the PALSA (Practical approach to Lung Health in South Africa) RCT study (sub-study 1), document description and review of guidelines used in primary care settings (sub-study 2), observation of nurses in practice and during patient consultations, and focus group discussions with nurses in primary health care facilities (sub-study 3). After the introduction of new format guidelines with onsite training and access to good support and updates, nurses reported feeling more confident, as the guidelines were explicit and gave them clear direction as to when a patient would need referral to the medical practitioner. When the guidelines were followed, and the patient responded positively to an intervention, this gave nurses a sense of credibility and validated their role as primary level health care providers. Guidelines available in the primary care clinics covered a wide variety of clinical conditions, were inconsistent, often outdated and even contradictory. A detailed comparison of two selected guidelines, the South African TB control guidelines and the PALSA PLUS guidelines, both in everyday use in the Free State province, shows that the preferences expressed by the nurses in sub-study 1 are evident in the layout, colour, and user-friendliness of the PALSA PLUS guideline. Nurses in the Free State province do use guidelines, but not consistently. Nurses make clinical judgments and decisions based on experience, alternative knowledges and intuitive responses, in consultation with colleagues and through the use of guidelines. Very few guidelines were used regularly, and each nurse had her preferences for a limited number of guidelines which she found useful. There is a clear need for integrated approaches to the information needs and support of nurses and nurse practitioners at primary care level. Guidelines play a role in promoting learning, changing professional practice and strengthening health care delivery by nurse practitioners at primary level. They can also be thought of as a strategy the health care system uses to defend against the possibility of its health professionals not meeting its expectations of providing quality care. Guidelines may contain anxiety and improve the quality of care, or compromise practice through the imposition of controls. The use of guidelines in primary care settings facilitates decision making, may contain practitioner anxiety and improve the quality of care, yet guidelines pose challenges to creative discernment of the patient’s symptoms in relation to his/her personal circumstances and may impact on the personalised holistic care approach which characterises the essence of nursing. Today’s primary care nurse and nurse practitioner needs to be a competent clinician, compassionate carer, and confident co-ordinator – the overlapping roles of caring, diagnosing and treating and managing. The challenge for the nurse in primary care is to combine her traditional caring and co-ordination role into a role which encompasses curing, caring and co-ordination, a new, yet critically important identity for the 21st century nurse. / AFRIKAANSE OPSOMMING: Die proefskrif ondersoek hoe verpleegsters in primêre gesondheidsorg in Suid-Afrika van riglyne gebruik maak. Primêre vlak gesondheidsorg steun hoofsaaklik op verpleegsters, alhoewel hulle verswelg word deur die komplekse behoeftes van hul kliënte in die konteks van die TB en HIV/AIDS epidemie. Ten spyte van verskeie volgehoue onderrigstrategieë om die huidige en bewese basiese te bevorder, is daar verskeie struikelblokke om optimale versorging te voorsien. Kliniese praktyk riglyne voorsien die beste bewyse en is 'n belangrike hulpmiddel om praktiserende professionele gesondheidswerkers, veral op die vlak van primêre gesondheidsorg, op hoogte van sake te hou. Besige programme en onvoldoende toegang tot hierdie riglyne weerhou dikwels die gesondheidswerkers van bestaande inligting. This work deals with the importance of psychodynamic approaches in practice of a day care center for an addictological treatment. The research core of this thesis are qualitative case analyses with a comment on the etiology of the dependency problem (anamnestic analysis revealing uncontrolled or conflicting developmental stages according to E. H. Erikson) and at the same time on the accentuation of the importance of psychodynamic approaches in routine clinical practice, which focuses primarily on good nonverbal interventions (i.e. an authentic technique using stones, an art therapy, nonverbal techniques) and transference phenomena. The aim of this thesis is to understand an individual case through selected psychoanalytic theories and to describe the key therapeutic interventions leading to curing an addicted person using an ambulant addiction treatment. For this particular research I chose a qualitative approach. I processed five clients' life stories into a set of case studies. Several methods were used to obtain the needed data. The crucial one was a semi-structured interview, then a life curve and a projective method. In each case study, I explained one developmental stage by E. H. Ghost words and invisible giants : H.D. and Djuna Barnes under signs of the imperative

My dissertation examines the correlations between the natural and supernatural, agency and authority, and meaning and language in the work of the modernist American writers H.D. (Hilda Doolittle) and Djuna Barnes. Using the psychoanalytic theory of Jacques Lacan, Melanie Klein, and Nicholas Abraham and Maria Torok, I argue that the different kinds of spectral and otherworldly figures that appear in these works – ghosts, the living dead, divinities, individuals who are also amorphous multiplicities – correlate to the modes of negation of parental imperatives that structure the language-use of their authors. I contrast H.D.‘s and Barnes‘s visions of the relation of language to meaning and the personal to the social using Lacan‘s delineation of the different modes of psychic negation that enable or disable language use: repression, disavowal, and foreclosure. According to this model, H.D.‘s work evidences foreclosure: a mode of thought and language that fails to differentiate words, thoughts, and people from one another. This incapacity endangers the psyche with the hallucinatory return of or haunting by what cannot be symbolized. In contrast, Barnes‘s work suggests disavowal, and her language renders experience in distorted forms. She repudiates power figures and the unspeakable meanings associated with them, but her work portrays the spectral, surreptitious return of these figures and meanings. Writing that witnesses or stages a return to a state of non-difference between symbol and symbolized, as Barnes‘s and H.D.‘s work does, calls for different interpretative and methodological strategies than those usual in literary criticism. To read such work primarily as symbolic communication is to lose perspective on the structures of thought and language that it grapples with. A perspective that is rigorous and radically different from the works‘ own is necessary to produce readings of it that make symbolic ―sense,‖ though it is unable to fully account for experiences that are not conceivable. To this end, I describe ―disorders,‖ types of thought and language that psychoanalysis implicates in interminable human suffering, without drawing conclusions about the range of experiences that might be concurrent with asymbolic or anti-symbolic thought and writing. / Graduate / 2019-08-31 / 0298 / 0591

HIV as an internal object : the subjective experience of HIV infection in women on ARVs.

HIV/AIDS research has proven crucial in an effort to prevent and manage this epidemic. However, there is little research being done in an attempt to understand the internal worlds of those living with HIV/AIDS. The purpose of this research was to begin to explore the relationship that exists between the person living with HIV/AIDS and the virus, as an internal object, inside them. This study focused on 6 women who were on Anti-Retroviral Medication (ARVs), and who have been diagnosed for at least one year. The participants’ mental representations of the virus as an object inside them was explored, as well as how they experienced and viewed the triangular relationship that exists between themselves, the HI Virus, and the ARVs. This exploratory research utilised a qualitative framework in order to understand and explore these relationships and perceptions, with psychoanalytic theory being used a lens through which to view the data that emerged. In depth semi-structured interviews were conducted with the participants, and the corpus of data was analyzed using a thematic content analysis. In addition, the participants were asked to draw the virus inside their bodies. These were analyzed using a technique devised by Paola Luzzatto (1987) in a study exploring the internal world of drug-abusers. For the purpose of this study, a variation of the same art therapy technique was used in that the participants were asked the ‘draw the virus in their bodies’. Whilst the drawings allowed for insight into the internal worlds of the participants, the drawings were also used as a point of departure. For most of the women, HIV was drawn using a red crayon, whilst the ARVs were drawn in either yellow or green. As depicted in the drawings, post diagnosis the HIV/red seemed to cover most of the body, but later when the ARVs/green was added, more of a balance was achieved. Results show that for these women, HIV was often perceived as dangerous and criminal, whilst the ARVs were often associated with security. From the perspective of Kleinian theory, the perception of the HIV and the ARVs seemed to be dependent upon the position from where they were functioning: either a paranoid-schizoid or a depressive position.

The unconscious life of the child with obsessive-compulsive disorder

This qualitative study explores the unconscious life of four children diagnosed with Obsessive-Compulsive Disorder (OCD) specifically related to selfconcept, personality, and psychopathology using a case study approach. A review of literature on childhood OCD is presented and the study is located within a psychodynamic theoretical framework. Findings indicate that the children are emotionally maladjusted, with high levels of anxiety and psychopathology. They have low self-esteem and poor body images, mostly tending towards immaturity. Two of the children have personality disturbances (neurotic, hysterical personalities). All the children have disturbed superegos (harsh or neurotic). Although their symptoms are currently mild, and some have ceased, analysis suggests they have been repressed and continue to affect them. They are sexually preoccupied and conflicted due to the unsuccessful resolution of the Oedipus complex. They have poor impulse control and considerable anger and aggression (mostly overt). They experience their environment as unstable and frightening and have anxieties about physical injury and being watched. The boys have regressed sex drives and homosexual tendencies, and have not identified with their fathers. The girls have identified with their mothers but experience masturbation guilt and blocked sexual drives, causing anxiety and moodiness. The children are all highly defended and escape from feelings of helplessness, inadequacy, and isolation, and discharge anxiety and aggressive instincts by using the defenses of undoing, reaction formation, acting-out, fantasy (sometimes violent), projection, displacement, and intellectualisation. Their strong dependency needs suggest fixation in the oral stage of psychosexual development. They tend towards self-directed aggression and depression. Most have family histories of mood disorders (particularly depression), and obsessions linked to fears of economic hardship due to parental illness or death. Most have histories of anxiety disorders or anxiety-related problems, and family histories of anxiety disorders and/paternal OCD. They all experienced a personally traumatic event precipitating the onset of OCD.

Durante o periodo de latencia e do pensamento operatorio, o principal motivo de consulta psicologica contempla problemas de aprendizagem e de adaptacao escolar. Tanto para a psicogenese piagetiana como para a psicanalise, o fracasso ou sucesso na escola constitui uma experiencia que tonaliza um percurso singular em relacao ao conhecimento e ao prazer de pensar. A presente pesquisa se insere na interface da psicologia do desenvolvimento e da psicologia clinica. Estabelece um dialogo conceitual entre as teorias piagetiana e psicanalitica para subsidiar a compreensao do complexo fenomeno das dificuldades escolares e suas ressonancias afetivas e cognitivas no desenvolvimento do prazer de pensar. Para isso, postula relacoes de correspondencia entre a dialetica afetividade-pensamento no periodo operatorio e da latencia. Propoe uma fundamentacao teorica para o estudo do jogo de mesa como trabalho sublimatorio e como instrumento clinico no setting psicanalitico com criancas da latencia. O metodo baseou-se na analise de producoes realizadas por criancas de 7 a 12 anos em contexto de atendimento grupal em oficinas de jogos (Leda/USP). Os materiais de arquivo correspondem a 195 encontros realizados ao longo do periodo 2011-2014. Para sua analise, combinou-se o estudo qualitativo e quantitativo de producoes discursivas, graficas e de acoes. Foram escolhidos registros escritos de: 106 rodas de conversa, 172 protocolos de observacao, 49 questionarios preenchidos por pais, 128 fichas de matricula, falas espontaneas, desenhos, e relatorios institucionais. Pode-se destacar os seguintes resultados: 1) O discurso adulto (pais e professores) em torno da queixa escolar focalizou a falta de atencao e de concentracao. O esforco da crianca seria o modo de compensar e superar essa dificuldade. Ja as criancas focaram seus problemas em conteudos escolares. 2) Acerca dos conflitos intersubjetivos com pares, viu-se como a debilidade do respeito mutuo e a rivalidade edipica pode levar a hostilidades corporizadas em insultos e julgamentos desvalorizadores. Verificou-se um baixo investimento dos professores como figuras identificatorias de conhecimento e de autoridade. 3) Comprovou-se que as criancas manifestam interesse e persistencia no contexto de jogo, porem esse investimento energetico da tarefa nao e suficiente pela recorrencia de estrategias nao operatorias, pouco planejamento da acao e analise dos meios. 4) Observaram-se tres tendencias nas tramas significantes sobre a interacao com o conflito cognitivo: irrupcao de afetos, mediacao via pensamento e inibicao. As explicacoes causais do resultado no jogo seguem a linha genetica piagetiana do fazer e compreender em tres modalidades de explicacao causal: pelo pensamento, pela acao e pelo acaso, que se correspondem, respectivamente, as compensacoes tipo gama, beta e alfa. Verificou-se que o sentido de ser bom aluno esta intrinsecamente relacionado a definicao de ser inteligente, de obediencia e de atencao homologada a esforco pessoal. 5) Articularam-se confluencias e recorrencias clinicas de modalidades energeticas e conteudos em seis estudos de caso sistematizados a partir de 109 indicadores empiricos que ilustraram a dinamica afeto-pensamento. Por fim, apresentou-se uma sintese teorico-clinica de 12 convergencias que articulam o dialogo teorico entre a psicanalise e a teoria piagetiana desenvolvido ao longo da tese / During latency period and operative thinking, the main reason for psychological consultation contemplates learning problems and school adaptation. For both Piagetian psychogenesis and psychoanalysis, failure or success in school is an experience that sets a singular course in relation to knowledge and the pleasure of thinking. The present research is inserted in the interface of developmental psychology and clinical psychology. It establishes a conceptual dialogue between Piagetian and psychoanalytic theories to understand the complex phenomenon of school difficulties and their affective and cognitive resonances in the development of the pleasure of thinking. This thesis postulate relations of correspondence between the affective-thought dialectic in the operative period and latency. It proposes theoretical basis for the study of games as a sublimatory work and as a clinical instrument in the psychoanalytic setting with latency children. Method was based on the analysis of productions performed by children aged 7 to 12 years in the context of group attendance in games workshops (Leda / USP). Archival materials correspond to 195 meetings held over the period 2011-2014. For its analysis, the qualitative and quantitative study of actions, discursive and graphic productions was combined. Written records of 106 conversation wheels, 172 observation protocols, 49 questionnaires filled out by parents, 128 registration forms, spontaneous speeches, drawings, and institutional reports were chosen. The following results can be highlighted: 1) Adult discourse (parents and teachers) surrounding the school complaint focused on the lack of attention and concentration. Child \"effort\" would be the way to compensate and overcome this difficulty. Children have focused their problems on school subjects. 2) Concerning intersubjective conflicts with peers, it has been seen how the weakness of mutual respect and oedipal rivalry can lead to hostilities embodied in insults and devaluing judgments. There was a low investment of teachers as identifiable figures of knowledge and authority. 3) It has been shown that children express interest and persistence in the context of the game, but this energy investment of the task is not enough by the recurrence of nonoperational strategies, little planning of the action and analysis of the means. 4) There were three trends in the significant frames of interaction with cognitive conflict: irruption of affections, mediatization through thought and inhibition. Causal explanations of the result in the game follow the Piagetian genetic line of doing and understanding in three modes of causal explanation: by thought, by action and by chance, which correspond, respectively, to the gamma, beta and alpha compensations. It was found that the sense of \"being a good student\" is intrinsically related to the definition of \"being intelligent\", of obedience and \"attention\" homologated to \"personal effort\". 5) Clinical confluences and recurrences of energy modalities and subjects were articulated in six case studies systematized from 109 empirical indicators that illustrated the affection-thought dynamic. Finally, a theoreticalclinical synthesis was presented with 12 convergences that articulate the theoretical dialogue between psychoanalysis and the Piagetian theory developed throughout the thesis

A experiência materna na clínica da obesidade infantil: estudo psicanalítico / The maternal experience in the clinical treatment of childhood obesity: a psychoanalytic study

A presente pesquisa objetiva investigar psicanaliticamente a experiência emocional de mães de crianças com diagnóstico de obesidade, justificando-se na medida em que o tratamento da condição, que inclui atenção psicológica, implica importante participação materna. O trabalho se organiza como estudo de caso da mãe de uma criança obesa em atendimento. O material clínico apresentado corresponde a duas narrativas transferenciais, relativas à história de vida e à entrevista individual da participante, que inclui suas manifestações produzidas em resposta ao Procedimento de Desenhos-Estórias com Tema. A consideração psicanalítica desse material permitiu a produção interpretativa de dois campos de sentido afetivo emocional: Sou mãe de um menino, logo existo e Minha mãe me ama, logo existo que apontam para dificuldades básicas e para movimentos de busca de constituição de si mesma como pessoa significativa para si e para o outro. Fica assim demonstrado que a experiência emocional materna, no caso estudado, está fortemente marcada por dificuldades subjetivas básicas, ligadas a processos de constituição de self, o que provavelmente afeta o cuidado psicológico do filho. Tal achado indica que é possível que a obesidade infantil seja expressão sintomática de complexas configurações familiares, de caráter transgeracional / The present study aims to psychoanalytically investigate the emotional experience of obesity diagnosis children´s mothers, justifying on the extent that the treatment of the condition, which includes psychological attention, implies important maternal participation. The work is organized as a case study of an obese in care child´s mother. The clinical material presented corresponds to two transferential narratives, related to life history and to individual participant interview, which includes their manifestation produced in response to the Theme Drawings-Story Procedure. The psychoanalytical consideration of this material allowed the interpretative production of two fields of emotional affection: I am a boy´s mother, so I exist and My mother loves me, so I exist, which points out to basic difficulties and to constitution seeking movements to herself as a significant person to her own and to the other individual. It is thus demonstrated that the maternal emotional experience, in the studied case, is strongly measured by basic subjective difficulties, linked to the constitution processes of self, which probably affects the psychological child´s care. As relações que se estabelecem entre teorias e práticas na clínica psicanalítica não costumam ser abordadas explicitamente nos escritos teóricos de psicanalistas; necessariamente, porém, elas subjazem às suas práticas e discursos. O presente trabalho propõe-se a investigar a complexidade inerente a essas relações, que a nosso ver não se restringem a uma concepção bidirecional e causal. Para tanto, recorremos a conceitos da epistemologia e, fundamentalmente, da própria psicanálise. O texto \"Uma nova leitura das origens da teoria das relações de objeto\" de Ogden permitiu-nos pensar as teorias como objetos passíveis de sofrerem investimentos libidinais: uma vez perdidas, exigirão do analista um trabalho de luto. Consideramos que essa perda ocorre quando a teoria deixa de responder às exigências da clínica, isto é, quando deixa de amparar o analista em seu contato com os pacientes. Luto e melancolia, assim, serviram-nos como metáfora para investigar os modos pelos quais as teorias se fazem presentes no analista e, conseqüentemente, na clínica, pois engendram objetos internos distintos. Se o analista faz o luto da teoria, ela é incorporada a seu conhecimento subsidiário, provendo as bases para um encontro traumático com os pacientes e com novas teorias. Se, por outro lado, esse luto não pode ser elaborado, a teoria fica então cristalizada no conhecimento subsidiário do analista, impedindo a clínica de irromper em sua dimensão traumática de alteridade. / Relationships established between theories and practices in the psychoanalytic clinical situation are not usually explicitly examined in the theoretical writings of psychoanalysts; such relationships, however, are necessarily implicit to their practices and discourses. The present work intends to investigate the complexities inherent to these relationships, which in our view are not limited to a causal and bidirectional conception. In order to do so, we have referred to concepts from epistemology and, most fundamentally, psychoanalysis itself. Ogden\'s \"A new reading of the origins of object relations theory\" has allowed us the consideration of theories as objects liable to receive libidinal cathexes, so that once they are lost, they will need to be mourned by the analyst. Such a loss occurs once the theory stops responding to clinical demands, that is, once it stops supporting the analyst in his contact with patients. Mourning and melancholia have thus served us as a metaphor to investigate the ways in which theories become present in the analyst and, consequently, in the clinical situation, for they engender different internal objects. If the analyst mourns the loss of the theory, it gets incorporated into his subsidiary knowledge, providing thus the bases for traumatic encounters with patients and new theories. If, on the other hand, the analyst cannot mourn, the theory gets then rigidly fixed into the analyst\'s subsidiary knowledge, therefore preventing the clinical situation to come forward in its full traumatic otherness.

Pretende-se contribuir à análise do conceito de narcisismo no pensamento freudiano a partir de três estudos: (1) Um exame das primeiras elaborações sobre o narcisismo, dividido em dois tempos: (i) a origem do termo junto às primeiras descrições e teorias sobre o narcisismo, (ii) sua entrada na teoria freudiana. A ideia central é, por meio de uma visão diacrônica, acompanhar a apropriação e evidenciar as modificações que Freud incide nesse conceito ao adotá-lo em sua metapsicologia. Para isso, então, iremos em um primeiro momento estudar como os primeiros teóricos do narcisismo Alfred Binet, Havelock Ellis, Paul Näcke e Richard von Krafft-Ebing concebiam e conceberam essa noção; para depois pensar a apropriação freudiana em três fontes: na correspondência trocada com Jung, no registro das Atas da Sociedade Psicanalítica de Viena e nos textos freudianos de 1910 a 1913. (2) Um estudo do texto de Freud Introdução ao narcisismo de 1914 texto central para o narcisismo , levando em consideração três aspectos: (i) a lógica interna do texto, (ii) as modificações que a introdução do narcisismo traz à teoria freudiana e (iii) alguns desdobramentos desse conceito no interior do pensamento freudiano. (3) Uma análise da noção de narcisismo das pequenas diferenças desenvolvendo um dos vários desdobramentos que o conceito de narcisismo tem a partir de 1914; trata-se de uma análise sincrônica que visa cotejar e relacionar três momentos dessa noção: tal como ela surge pela primeira vez em O tabu da virgindade de 1918, como ela é retomada em Psicologia das massas e análise do Eu de 1921 e sua versão final em O mal-estar na cultura de 1930. (2) A study of the Freudian text On Narcissism: an introduction of 1914 - the central text for narcissism - taking into account three aspects: (i) the internal logic of the text, (ii) the changes that the introduction of narcissism brings to the Freudian theory, (iii) some consequences of this concept inside the Freudian thought. (3) An analysis of the concept of narcissism of minor differences - unfolding one of the several developments that the narcissism concept has had since 1914; it is a synchronic analysis that aims to collate and relate three moments of this notion: such as it first appears in The Taboo of Virginity of 1918, the way it is retaken in Group Psychology and the Analysis of the Ego of 1921, and its final version in Civilization and Its Discontents of 1930.
