Maternal health and health care in Madhya Pradesh state of India : an exploration using a human rights lens

Pregnancy and motherhood are natural processes in the lives of women of reproductive age. These processes are generally considered to be positive and fulfilling experiences. However, for various reasons, many women end up dying as a result of these processes. Improving maternal health and reducing maternal mortality are accepted as human rights challenges and prioritized in several international declarations and national policies. However, progress in achieving these objectives still remains poor. This thesis aims to explore the maternal health and healthcare in the Indian state of Madhya Pradesh through a human rights lens. A human rights lens provides a framework to study various aspects of the problem of maternal health from a human rights perspective. It helps in highlighting the gaps and challenges related to political priority, sociocultural, economic and individual-level factors and the availability, accessibility, acceptability and quality of maternal healthcare services. A combination of quantitative and qualitative research methodologies was applied in four sub-studies conducted in Madhya Pradesh. The specific objectives were: to investigate, by using John W. Kingdon’s multiple-streams model of agenda setting, why and how maternal health became a political priority in the state (Paper I); to estimate the effects of individual-, community- and district-level characteristics on the utilization of maternal health services with special reference to antenatal care, skilled attendance at delivery and post-natal care (Paper II); to analyse sociocultural and service delivery related dimensions of maternal deaths in rural central India through a human rights lens (Paper III); and to evaluate the technical efficiency of the public district hospitals using data envelopment analysis (Paper IV). The findings of the first qualitative study indicated that various developments at international, national and state level brought the issue of maternal health to the priority political agenda in Madhya Pradesh state. This resulted in the introduction of new policies and programmes and more resources were allocated for improving maternal health. However, several challenges still remain in ensuring proper implementation of these programmes and policies. The quantitative study on factors affecting the use of maternal health services revealed that 61.7% of women used antenatal care at least once, 49.8% of women used skilled attendance at delivery and 37.4% of women used post-natal care during their most recent pregnancy. The household’s socio-economic status and mother’s education emerged as the most important factors associated with the use of antenatal care and skilled attendance at delivery. Delivery by skilled personnel and the use of antenatal care were the most important factors in the use of post-natal care. This study highlighted the need to identify and focus on community- and district-level intervention along with addressing the individual-level factors. The findings of the third qualitative sub-study revealed that all pregnant women in the study tried to access medical assistance for obstetric complications but various factors delayed appropriate care. The underestimation of complication symptoms by family members, gender inequity and the negative perceptions regarding delivery services deferred decisions to seek care. Transportation problems and care seeking at multiple facilities also constrained timely reaching of appropriate health facilities. Negligence by health staff in providing care, and unavailability of blood and emergency obstetric care services, delayed the receiving of adequate care after reaching a health facility. This study indicates that normative elements of a human rights approach to maternal health, i.e. availability, accessibility, acceptability and quality, were not fully upheld. The deceased women and their relatives were unable to claim their entitlements and the duty bearers could not meet their obligations despite their conscious efforts to improve maternal health. In the last study, the results of data envelopment analysis revealed that half of the district hospitals (20) in the study were operating inefficiently. This research establishes a need to give special attention to addressing challenges in the maternal health programmes at the implementation level as well as tackling the social determinants of maternal health. In order to increase the utilization of maternal health services in the state, the need to identify and focus on community- and district- as well as individual level interventions is emphasized. In order to prevent maternal deaths, a need for further concentrated efforts is underlined with a view to honouring human rights elements of maternal health by better community education, women’s empowerment and health system strengthening with the provision of appropriate and timely services including emergency obstetric care of good quality. It also highlights a need to identify the causes of the observed inefficiencies and to take appropriate measures to increase the efficiency of district hospitals.

Objetivo: Analisar o acesso a serviços de saúde e o cuidado com a saúde sexual e reprodutiva de mulheres que fazem sexo com mulheres, sob o enfoque das políticas públicas de saúde. Método: Estudo observacional, transversal, descritivo e analítico que integra estudo mais amplo sobre acesso a serviços de saúde e saúde sexual e reprodutiva de MSM. A amostra intencional de 149 MSM foi constituída por meio da Técnica de Amostragem Bola de Neve (a Andreia pediu para retirar, acho melhor manter) indicação de profissionais de saúde e liderança LGBT (de Lésbicas, Gays, Bissexuais, Travestis e Transexuais) por procura espontânea, a partir da divulgação do projeto nas redes de sociabilidade, comunicação de massa, serviços de saúde e grupos de ativismo. Adolescent pregnancies in the Amazon basin of Ecuador : a rights and gender approach to girls' sexual and reproductive health

Adolescent pregnancy has been associated with adverse health and social outcomes, but it has also been favorably viewed as a pathway to adulthood. In Ecuador, where 20% of girls aged between 15-19 years get pregnant, the adolescent fertility rate has increased and inequalities between adolescent girls from different educational, socio-economic levels and geographical regions are prominent: 43% of illiterate adolescents become pregnant compared to 11% with secondary education. The highest adolescent fertility rates are found in the Amazon Basin.   The overall aim of this study was to explore adolescent pregnancy in the Amazon Basin of Ecuador (Orellana province) from a rights and gender approach. Specific aims and methodologies included: to explore women‟s reproductive health situation, focusing on government‟s obligations, utilization of services, inequities and implementation challenges, assessed through a community-based cross-sectional survey and a policy analysis (Paper I); to examine risk factors associated with adolescent pregnancy, through a case-control study (Paper II); to explore experiences and emotions around pregnancy and motherhood among adolescent girls, using content analysis (Paper III); and to analyze providers‟ and policy makers‟ discourses on adolescent pregnancies (Paper IV).   Reproductive health status findings for women in Orellana indicated a reality more dismal than that depicted in official national health data and policies. Inequities existed within the province, with rural indigenous women having reduced access to reproductive health services. In Orellana, 37.4% of girls aged 15-19 had experienced pregnancy, almost double the national average. Risk factors associated with adolescent pregnancy at the behavioral level included early sexual debut and non-use of contraception, and at the structural level poverty, having suffered from sexual abuse, and family disruption. Gender inequity played a key role through the machismo-marianismo system. Girls were raised to be fearful and ignorant regarding sexuality and reproduction, to be submissive and obedient, to be fatalistic, and to accept the established order of the male and adult dominance. Sexuality was conceptualized as negative, while motherhood was idealized. Those gender structures constrained girls‟ agency, making them less able to make choices regarding their sexual and reproductive lives. Providers‟ discourses and practices were also strongly influenced by gender structures. Adolescent sexuality was not sanctioned, girls‟ access to contraceptives still faced opposition, adolescent autonomy was regarded as dangerous, and pregnancy and reproductive health issues were conceptualized as girls‟ responsibility. However, mechanisms of resistance and challenge were also found both among adolescent girls and providers.   Programs addressing adolescent pregnancies in the area need to look at the general situation of women‟s reproductive health and address the gaps regarding access and accountability. Adolescent pregnancy prevention programs should acknowledge the key role of structural factors and put emphasis on gender issues. Gender inequity affects many of the factors that influence adolescent pregnancies; sexual abuse, girls‟ limited access to use contraceptives, and girls‟ curtailed capability to decide regarding marriage or sexual intercourse, are strongly linked with young women‟s subordination. By challenging negative attitudes towards adolescents‟ sexuality, the encounter between providers and adolescents could become an opportunity for strengthening girls‟ reproductive and sexual agency.

Para análise dos dados empregou-se a estatística descritiva e as associações foram verificadas pelo teste de Qui Quadrado, por meio de ajustes de modelos de regressão linear, com resposta normal e de regressão linear múltipla. Resultados: Predominaram as mulheres brancas (74,5%), não unidas (73,2%), que tinham atividade remunerada (73,2%), alguma religião (74,5%) e com oito anos e mais de estudo concluídos (96,0%). A mediana de idade foi de 27 anos (18-62) e a renda familiar per capita de R$1.000,00 (133,33-15.000,00). A maioria tinha histórico de relacionamento sexual com homens durante a vida (74,2%) e, no último ano, apenas com mulheres (77,2%). A maioria utilizava o serviço público de saúde (84,0%), procurava Unidades Básicas de saúde (70,5%) e pronto-socorro (67,8%), somente quando adoeciam/sentiam algum desconforto (70,0%) e 47,6% não revelavam a prática sexual com mulheres para os profissionais de saúde. As dificuldades/barreiras encontradas na procura por serviços de saúde e/ou no atendimento dos profissionais e/ou à satisfação de necessidades pelos serviços foram relatadas por 77 (51,7%) mulheres, sendo que 66,1% delas relacionaram-se à estrutura e organização dos serviços de saúde e 33,9% à relação profissional de saúde-usuário. A mediana do escore de cuidado com a saúde sexual e reprodutiva foi de 4 pontos (0-7), tendo em vista que este poderia variar de 0-8 pontos. A maioria das mulheres não realizava atendimento ginecológico anual (54,4%), não estava com a Citologia Oncótica em dia (63,1%), tinha relação sexual após o consumo de álcool e/ou drogas ilícitas (71,8%), não utilizava preservativo nas relações sexuais anais e vaginais (87,9%), e tinha diagnóstico de alguma IST( Infecção Sexualmente Transmissível) (51,7%) e, 47,0% não havia realizado sorologia para IST/aids. A maioria das mulheres era tabagista (55,0%) e 34,9% utilizavam drogas ilícitas. As variáveis independentemente associadas ao escore de cuidado com a saúde sexual e reprodutiva foram idade, utilização de tabaco, de drogas ilícitas, ter se relacionado sexualmente com seis ou mais mulheres na vida, relação sexual após uso de álcool e/ou drogas ilícitas e uso de preservativo em todas as relações sexuais anais e vaginais. Não houve associação entre ter ou não histórico de relação sexual com homens na vida ao escore de cuidado. Diante dos resultados obtidos na presente investigação realizou-se duas oficinas de sensibilização dirigidas aos profissionais de saúde de todas as categorias, atuantes na Atenção Básica do município de Botucatu. Estas tiveram por objetivo a sensibilização dos profissionais sobre as necessidades de saúde de MSM e proporcionar reflexão referente as suas práticas em relação a esse grupo. Foram propostas várias estratégias pelos participantes como: educação permanente de todos os profissionais e educação em saúde voltada à população em geral, acerca desta temática, revisão de instrumentos de sistematização, utilizados na assistência, que favoreçam a revelação da prática sexual com mulher e organização e distribuição de material educativo sobre cuidado com a saúde para MSM, dentre outras. Conclusão: Apesar de a maioria das MSM terem acesso aos serviços de saúde, apresentaram muitas lacunas no cuidado com a saúde sexual e reprodutiva, apontando para vulnerabilidade individual, social e programática às IST, comprovada pela elevada prevalência desses agravos. Necessidades de saúde específicas deste grupo podem deixar de estar sendo identificadas e abordadas pelos profissionais de saúde, uma vez que grande parte delas não revelou a prática sexual com mulher. Desta forma, o presente estudo vem contribuir para o planejamento de ações mais abrangentes, voltadas à promoção da saúde integral de mulheres que fazem sexo com mulheres, à medida que aponta contexto de vulnerabilidade e de necessidades, relativas à saúde sexual e reprodutiva de mulheres, residentes no interior do Estado de São Paulo. / Introduction: Before proposing any intervention to improve health services of a given population, it is essential that health care is provided and the population is able to access these services. In this regard, there is a gap in the literature related to the access to health care among Brazilian women who have sex with women (WSW). Objective: The study presented here analyses the access to health services and the care given to these women, mainly concerning reproductive and sexual health. It focuses on public health policies. Methods: This study was based on the analyses of observational, transversal and descriptive studies, which are part of a research project that investigates the access that WSW have to health services, mainly reproductive and sexual health care. The sample size of 149 women was achieved through chain referral sampling mode. The participants where indicated by health professionals or LGBT activists (lesbian, gay, bisexual, transvestite and transgender). Also, some participants were volunteers interested in the project as it was promoted in social media, mainstream media, through healthcare service and activism groups. The variables in study were the following: socio-demographics, drugs consumption, behavioural, sexual practices, clinical-gynaecologist and reproductive, and those related to access to health services. The outcome variable was the reproductive and sexual health care score. The data was gathered by the researchers in the main research project during two-year period - from January 2015 until December 2016 - who applied a questionnaire, combined with gynaecologic exam and blood test used to detect infections of HIV (Human immunodeficiency virus), hepatitis B virus, HPV (Human papillomavirus), Chlamydia trachomatis, Neisseria gonorrhoeae, Trichomonas vaginalis and syphilis. Descriptive statistics was used to analyse the data, the associations were verified by Chi-squared test, using linear regression modelling with normal and multiple linear regression responses. Results: The data analysis showed the predominance of white women (74,5%), single (73,2%), employee (73,2%), religious (74,5%) and who had been educated during eight or more years (96,0%). The median age of the participants was 27 years old (18-62) and the family monthly income of R$1.000,00 (133,33 - 15.000,00). The majority had had men sexual partners previously (74,2%) but, in the last year woman partners only (77,2%). The majority used the public health services (84,0%), used Health Care Units (70,5%) and Hospital Emergency Units (67,8%), only when had an illness or some type of discomfort (70,0%) and 47,6% did not disclose their sexual preferences and practices to health professionals. Seventy seven women (51,7%) reported some kind of difficulties or barriers they found when seeking for health services; when being attended by health professionals and/or regarding having their needs fulfilled by the health services. Most of the barriers (66,1%) were related to the organizational structure of the health services provided, whilst 33,9% were related to the health care professional and the health care patient. The median of reproductive and sexual health care score was four points (0-7), considering that the score could vary between 0-8 points. Annual gynaecologic appointment/or exam were neglected by the majority of the women (54,4%); others did not have a valid cervical smear test (63,1%); the majority had sexual relations after alcohol and/or drugs consumption (71,8%); most did not use any preservatives in sexual relations, whether anal or vaginal (87,9%); more than a half (51,7%) had been diagnosed some kind of STI (Sexually Transmitted Infections) and; 47,0% did not take serologic blood tests to identify STI/ AIDS. It was observed among the participants a high percentile of smoker women (55,0%) and drugs users (34,9%). The independent variables associated with reproductive and sexual health care score were: age; whether they smoke; whether they use drugs; had sex with six or more women; had sexual relations after alcohol and/or drugs consumption; and, the use of preservatives in sexual relations, whether anal or vaginal. It was not found a correlation between had had men sexual partners previously and the care score. Following the analysis of the results of the present investigation, two workshops were organised targeted at health care professionals of different organisational levels of Basic Care in Botucatu municipality. The workshops were conducted to promote the health needs of WSW and to discuss the current practices related to this group. Several strategic actions were proposed by the participants of the workshop, among them were: continuous information and training to health care professionals; dissemination of WSW health care needs to wide population; review of instruments and data systematisation used to assist WSW that support the women to feel comfortable to talk about their sexual practices; organisation and distribution of informative material regarding WSW health and care. Conclusion: Although the majority of the WSW has access to health services, they did not fulfil their needs regarding reproductive and sexual health care. Therefore, they were individually, socially and programmatically vulnerable to STI, which was reflected in the high prevalence of STI among the participants. Their health care needs possibly can be neglected by health care professionals, as the majority of WSW did not reveal their sexual practices to them. In this way, by contextualising the vulnerability and needs related to reproductive and sexual health of women living in the country side of Sao Paulo province, the present study contributes to the planning of strategic actions to promote the health care services target at women who have sex with women. / FAPESP: 2015/14769-0

Defini-se alta paridade a ocorrência de quatro ou mais gestações que geraram filhos vivos com peso igual ou superior a 500 gramas e idade gestacional acima de 20 semanas. Partiu-se da hipótese que além do risco da própria multiparidade as mulheres com alta paridade estão mais expostas a outros fatores de risco como os de ordem social e econômico, dificuldade de acesso aos serviços de saúde e utilização inadequada de métodos anticoncepcionais. Além disso, acredita-se que estas mulheres geram prole com condições de nascimento mais desfavoráveis. O processo de amostragem foi aleatório sistemático por conglomerados onde foram sorteados 20 setores censitários da zona urbana da cidade e, em cada setor visitados 32 domicílios. Participaram da pesquisa 594 mulheres com idade entre 15 e 49 anos. A coleta de dados foi realizada entre setembro de 2005 e março de 2006. O instrumento utilizado para a coleta dos dados baseou-se em um questionário contendo perguntas fechadas estruturadas e semi estruturadas aplicado às mulheres sujeitos da pesquisa. O modelo teórico para determinação dos fatores de risco para a alta paridade foi construído por blocos de variáveis das características socioeconômicas e demográficas, história reprodutiva, anticoncepção, planejamento familiar, história conjugal, acesso aos serviços de saúde, condições de saúde e hábitos pessoais . A análise dos resultados foi obtida através de regressão logística não condicional. A prevalência de alta paridade no presente estudo foi de 15,8%. A análise bruta mostrou que a alta paridade esteve associada positivamente com a idade (p<0.001), cor da pele não branca (p=0.005), presença do companheiro (p<0.001), paridade dos pais maternos (p<0.001), história de filhos com baixo peso ao nascer (p=0.005) abortos (p<0.001), hábito de fumar (p=0.002) e participação do companheiro na escolha do método anticoncepcional (p=0.01)e negativamente com a renda per capita (p<0.001), escolaridade (p<0.001), trabalho fora de casa (p=0.03), idade da primeira gestação(p<0.001) e forma de acesso não gratuito ao método anticoncepcional (p<0.001). Após o ajuste conforme o modelo hierárquico de análise permaneceram significativamente associados ao desfecho estudado presença do companheiro (p=0.02), abortos (p=0.01), participação do companheiro na escolha do método anticoncepcional (p=0.01), renda per capita (p=0.001), escolaridade (p<0.001) e idade da primeira gestação (p<0.001). Esses resultados confirmam um quadro de desigualdades nos quais estão inseridas este grupo de mulheres com alta paridade no município do Rio Grande. Acredita-se que este estudo poderá contribuir para a construção de práticas e modelos de assistência à saúde que reduzam as desigualdades existentes em nossa sociedade, onde as práticas sejam desenvolvidas com equidade e coerência com as necessidades de saúde das mulheres e que repercuta em uma melhor qualidade de vida e saúde no campo da reprodução e sexualidade. / The present population-based transversal study had as objective to identify the reproductive health profile and the predominance of high parity amongst women from the municipal district of Rio Grande/RS. High parity being defined as the occurrence of four or more gestations that generated living children with weight same as or above 500 grams and gestational age above 20 weeks. Beginning from the hypothesis that, beyond the risk of multiple parity itself, the women with high parity are more exposed to other risk factors such as from social and economical order, difficult access to health services and inadequate use of contraceptive measures. Besides that, it is believed that these women give their progeny birth in further unfavorable conditions. The sampling process was systematic random along conglomerates where 20 censual sectors from the urban zone were picked by lot and in each sector 32 dwellings were visited. 594 women with age between 15 and 49 years participated in the research. The data gathering occurred between September 2005 and March 2006. The implement used for the data gathering was based in a questionary containing closed structured and semi-structured questions applied on the women subject of the research. The theoretical model for the determination of risk factors for high parity was build by blocks of variables of socioeconomic and demographic characteristics, reproductive history, contraception, family planning, conjugal history, access to health services, health conditions and personal habits. Analyses of the results were obtained through non-conditional logistic regression. The predominance of high parity in the present study were of 15.8%. In the bivaried analyses also it is high parity happened positive association with the age (p<0.001), non whit skin color (p=0.005), living with partner (p<0.001), parity of parents motherly (p,0.001), low birth weight of newborns (p=0.005),abortion (p=0.001), habit of smokes (p=0.002), participation of the partner in the choice of the measure (p=0.01) and negatively with per capita income ( p<0.001), schooling (p<0.001), work out house (p=0.03), age of the first gestation (p<0.001), non free access to contraceptive measures (p<0.001). Behind analyses multivaried remained of the high parity significant association living with partner (p=0.02), abortion (p=0.01), participation of the partner in the choice of the measure (p=0.01), income per capita (p=0.001), schooling(p<0.001), and age of the first gestation (p<0.001).That results confirm one panorama of the inequality on the whom to be insert high parity amongst women from the municipal district of Rio Grande/RS. It is believed that this study will be able to contribute to the making of practices and models for health assistance that can reduce the inequalities existing in our society, in which the developed practices are coherent with the needs of the health of women and that it may outcome for a better quality of life and health in the sexuality and reproduction field. / El presente estudio transversal de base populacional tuvo como objetivo identificar el perfil de la salud reproductiva y la prevalencia de alta paridad entre mujeres del municipio de Rio Grande/RS. Se define alta paridad a ocurrencia de cuatro o más gestaciones que generan hijos vivos con peso igual o superior a 500 gramos y edad gestacional arriba de 20 semanas. Se partió de la hipótesis que aparte del riesgo de la multiparidad en si, las mujeres con alta paridad están más expuestas a otros factores de riesgo tales como los de orden social y económico, dificultad de acceso a los servicios de salud y utilización inadequada de métodos anticoncepcionales. A parte de esto, se cree que estas mujeres generan prole con condiciones de nacimiento más desfavorables. El proceso de la muestra fue aleatorio sistemático por conglomerados donde fueron sorteados 20 sectores censados de la zona urbana de la ciudad y en cada sector visitados 32 domicilios. Participaron de la pesquisa 594 mujeres con edad entre 15 a 49 años. La colecta de datos fue realizada entre setiembre del 2005 y marzo del 2006. El instrumento utilizado para la colecta de los datos se basó en un cuestionario conteniendo preguntas cerradas estructuradas y semi-estructuradas aplicado a las mujeres sujetos de la pesquisa. El modelo teórico para determinación de los factores de riesgo para a alta paridad fue construido por blocos de variables de las características socioeconómicas y demográficas, historia reproductiva, anticoncepción, planeamiento familiar, historia conyugal, aceso a los servicios de salud, condiciones de salud y hábitos personales. El análisis de los resultados fue obtenido por medio de regresión logística no condicional. La prevalencia de alta paridad en el presente estudio fue de 18,8%. El análisis bruto mostró que la alta paridad estuvo asociada positivamente con la edad (p<0.001), color de piel no blanca (p=0.005), presencia de compañero (p<0.001), paridad de padres maternos (p<0.001), historia de hijos con bajo peso al nacer (p+0.005), abortos (p<0.001), hábito de fumar (p=0.002), y participación del compañero en la elección del método anticoncepcional (p=0.001), y negativamente con la renta “por capita” (p=0.001), escolaridad (p=0.001) y edad de la primera gestación (p<0.001). Estos resultados confirman un cuadro de desigualdades en los cuales están inseridas este grupo de mujeres con alta paridad en el municipio de Río Grande. Se cree que este estudio podrá contribuir para la construcción de prácticas y modelos de asistencia a la salud que reduzcan las desigualdades existentes en nuestra sociedad, donde las prácticas sean desenvueltas con igualdad y coherencia con las necesidades de salud en el campo de la reproducción y sexualidad.

Introdução: As escolhas relacionadas à fecundidade são complexas e envolvem idéias e valores das sociedades e de seus integrantes. Existem muitos estudos que procuram explicar a fecundidade a partir de variáveis socioeconômicas e culturais, mas não indagam os motivos que teriam levado as mulheres a proceder de uma forma ou de outra para a formação de suas famílias. A fecundidade abaixo do nível de reposição é observada nos grandes centros urbanos brasileiros há algumas décadas, mas permanecem diferenciais intra-urbanos que precisam ser identificados, assim como seus determinantes. Objetivo: Identificar os diferenciais de fecundidade no município de Curitiba e contribuir para o conhecimento e entendimento do comportamento reprodutivo, motivações e vivência das mulheres com alta fecundidade. Metodologia: Pesquisa quantitativa e qualitativa, de caráter descritivo e exploratório. Analisou-se inicialmente a tendência da fecundidade e os diferenciais por bairro. O estudo na etapa quantitativa incluiu 441 mulheres residentes em Curitiba que tiveram o quinto ou mais filhos nascidos vivos em 2005, identificadas pelo SINASC. Para coleta dos dados nesta etapa utilizou-se questionário semi-estruturado. Os dados foram organizados em banco de dados Access e analisados com o uso do programa computacional Statistica v.8.0. Os sujeitos da etapa qualitativa constituíram-se de nove mulheres entrevistadas em profundidade a partir de roteiro temático. Os dados qualitativos foram interpretados seguindo os ensinamentos da \"análise de conteúdo\". Resultados: A taxa de fecundidade em Curitiba chegou a 1,5 filhos por mulher em 2005, mas com diferenciais intra-urbanos. Nove dos 75 bairros de Curitiba concentravam 59,6% das mulheres com alta fecundidade. Cerca de 90% das entrevistadas tiveram mais filhos do que desejavam. O número ideal de filhos referidos variou entre 2,4 e 2,9. No momento da entrevista 85% das mulheres usavam algum método contraceptivo com prevalência da laqueadura e da pílula. A esterilização voluntária foi efetivada para apenas 36,4% das mulheres e 42% dos homens solicitantes. A auto-avaliação de saúde não boa foi referida por 40% das mulheres. As entrevistadas relataram dificuldade de acesso aos programas de proteção social e à casa própria. Contribuíram para o maior número de filhos, as falhas dos programas de planejamento familiar, a falta de informação e domínio da negociação no uso dos métodos contraceptivos, as desigualdades de gênero e a vulnerabilidade psico-afetiva das mulheres. As entrevistadas da etapa qualitativa vivenciaram trajetórias reprodutivas e produtivas conflituosas marcadas pelas desigualdades sociais e de gênero. A maioria das mulheres do estudo apresentava elevada vulnerabilidade social. Conclusões: Uma diversidade de fatores, além dos socioeconômicos, exerce influência na definição da maternidade e do número de filhos. Identificou-se que são necessárias medidas que implementem o acesso à contracepção, a inclusão dos homens neste processo, a capacitação de profissionais para a assistência sexual e reprodutiva. São indispensáveis a promoção de políticas compatíveis para realização da maternidade, que facilitem a participação das mulheres no mercado de trabalho e promovam a eqüidade entre os sexos tanto neste mercado quanto no âmbito doméstico e medidas efetivas de proteção social. A liberação da laqueadura no pós-parto normal imediato foi recomendada, com vistas a agilizar e facilitar o acesso das mulheres a este procedimento. / Introduction: The choices concerning about fertility are complex because they involve ideas and values of societies and their members. Many studies seek to explain the fertility socioeconomic and cultural changeable but don\'t consider the women reasons to select one way or another to form their families. The fertility rate below the replacement is observed in Brazilians urban centers a few decades ago, but is necessary to identify the intra-urban differentials remain and its determinants. Objective: To identify the fertility differentials in Curitiba and to contribute to the women with high fertility reproductive behavior knowledge and comprehension, their motivations and experiences. Methodology: quantitative and qualitative researches, descriptive and exploratory in nature. The trend in fertility and the differentials were examined by neighborhood initially. The study included 441 women living in Curitiba which had the fifth or more children born alive in 2005 and identified by SINASC. In this step the data collect used a semi-structured questionnaire. They were organized in Access database and analyzed using the Statistica computer program, v.8.0. Nine women were the qualitative phase subjects interviewed in depth from thematic roadmap. Qualitative data were interpreted by following the teachings of the \" contents analysis.\" Results: The rate of fertility in Curitiba reached 1.5 children per woman in 2005, but with intra-urban differentials. Nine of the 75 districts of Curitiba concentrated 59.6% of women with high fertility. About 90% of respondents had more children than desired. The ideal number of children referred ranged between 2.4 and 2.9. At the time of the interview 85% of women were using contraceptive methods, with prevalence of sterilization and the pill. The voluntary sterilization was carried out for only 36.4% of women and 42% of male applicants. The self-assessment of health \"not good\" was cited by 40% of women. Interviewees reported difficulty of access to programs of social protection and housing. Contributed to the largest number of children, the failures of programs for family planning, lack of information and area of negotiation in the use of contraceptive methods, inequalities of gender and psycho-emotional vulnerability of women. Subjects of qualitative stage experienced conflict trajectories reproductive and productive marked by social inequality and gender. Most women in the study had high social vulnerability. Conclusions: A variety of factors, in addition to the socioeconomic, exerts influence on the definition of motherhood and the number of children. It was identified that are necessary to implement the access to contraception, the inclusion of men in this process, the training of professionals for sexual and reproductive assistance. Its essential to promote politics for consistent implementation of motherhood, to facilitate the participation of women in the labor market and promote equity between the sexes in this market as both under domestic and effective measures of social protection. The release of sterilization in the immediate post-normal delivery was recommended, in order to expedite and facilitate women\'s access to this procedure.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / A dissertação visa compreender os sentidos atribuídos à sexualidade e ao gênero no cotidiano de uma unidade de saúde da família do município do Rio de Janeiro. Por meio do método etnográfico foram observadas as interações dos diferentes sujeitos no contexto do serviço e, especificamente, as situações relativas a planejamento familiar, assistência à gestação, saúde da mulher e do adolescente. Também foram registrados os modos como se processam as conversas e transmissões de conhecimento concernentes a identidade de gênero, relações sexuais e temas relativos. O estudo evidenciou que a atuação dos profissionais de saúde não se apoia apenas em seu saber, em sua subjetividade e nas relações estabelecidas com seus pares e com os usuários, está também implicada com os valores em jogo no território. A análise aponta que uma concepção hegemônica de gênero informa os limites discursivos do campo e delineia a produção do cuidado, orientando o acolhimento de demandas e a programação de ações no campo da saúde reprodutiva e sexual.
